Класична куга свиња (ККС) је веома контагиозно
обољење домаћих и дивљих свиња. Економски посматрано ККС представља
највећу претњу свињарској производњи једне земље. У многим земљама,
ККС је успешно искорењена применом опсежних мера контроле присуства
вируса ККС у природним резервоарима, биосигурносне заштите,
нешкодљивог уништавања оболелих свиња, контроле промета животиња,
упоредо са програмом масовне вакцинације. Успешно заустављање
циркулације вируса ККС кроз пријемчиву популацију и прекидање ланца
инфекције уз примену вакцинације, захтева висок ниво имунизације
свиња. Ако се вакцинација спроводи систематски на територији целе
земље, долази до значајног смањивања клиничких случајева ККС и до
заустављања циркулације вируса. Искуства су показала да је
ерадикацију уз примену масовне вакцинације могуће постићи у року од
годину дана. Да би се обезбедио одговарајући ниво имунитета стада и
избегли нежељени ефекти ниског обухвата вакцинацијом, као што је
присуство „маскираних“ случајева ККС, потребно је обезбедити
константну вакцинацију свиња на терену. Упоредо са имунопрофиласком
неопходно је спроводити и мониторинг вакцинације. Мониторинг
вакцинације је заснован на плану узорковања, теренском истраживању
и препознавању ризичних фактора који условљавају низак обухват
имунизацијом. Циљ испитивања је био да се одреди одговарајући
поступак израчунавања величине репрезентативног узорка, односно
потребног броја насељених места, породичних фарми, сеоских
газдинстава и свиња, код којих треба спровести епизоотиолошко
истраживање и испитивање сероконверзије, ради одређивања обухвата
вакцинацијом против ККС. У раду је описана студија пресека,
примерена екстензивним условима држања свиња и урађен прорачун
репрезентативног узорка на нивоу популације свиња у Републици
Србији. Такође је извршено истраживање обухвата вакцинацијом против
ККС на територији три општине у Републици Србији и испитана
ефикасност имунизације.

▼ High levels of immunization coverage are required to
maintain control of infectious diseases that previously caused
significant illness and death in Canada. However, childhood
immunizations have been so successful that many Canadians have
never seen the impact of these diseases, with some parents becoming
more concerned about the safety of vaccines than about the
infectious diseases that they were designed to prevent. The purpose
of this thesis was to determine if parental concern about vaccine
safety is significantly associated with incomplete childhood
immunizations, and whether other personal or systemic factors play
a role in influencing uptake. The thesis consists of four related
papers focusing on: (1) The value and challenges of applying the
population health framework in nursing research; (2) The benefits
and drawbacks of using postal surveys for public health nursing
research; (3) Assessment of the accuracy of the immunization
database utilized in this study in terms of immunization status
classification, as compared to parent report; and, (4) The findings
of the multivariate data analysis in the study. The main
conclusions of this thesis are as follows: (1) The population
health framework, while presenting challenges for nurse
researchers, provides a valuable framework for assessing the
multi-level factors influencing health behaviours, such as
immunization uptake; (2) Awareness of the potential for survey
error is essential when using postal surveys in research, and
measures must be taken to minimize, assess, and control for
non-response bias; (3) The immunization database used in this study
had a low, but not inconsiderable, number of misclassification
errors of immunization status, while parent report had a greater
number of errors; and (4) Parental concern about vaccine safety was
indeed associated with incomplete immunizations, but other personal
and systemic factors were also associated with vaccine uptake
and/or mediated the relationship between parental concern and
uptake.

▼ A brucelose bovina, causada principalmente pela Brucella abortus (B. abortus), é uma enfermidade infectocontagiosa responsável por perdas reprodutivas e que possui implicações com a saúde pública. O estado de Mato Grosso é um dos estados brasileiros mais importantes da pecuária nacional, sendo também o estado que apresenta a mais alta prevalência de brucelose bovina em nível nacional. Este estudo teve por objetivo propor um método para cálculo de cobertura vacinal contra brucelose bovina, de modo que fosse possível avaliar o impacto da vacinação sobre a dinâmica da enfermidade na população bovina existente no estado de Mato Grosso a partir da análise de informações registradas junto ao Instituto de Defesa Agropecuária do Mato Grosso INDEA/MT referentes aos exercícios de 2006-2010. Observou-se que em torno de 30% do total de fêmeas pertencem aos grupos etários em idade vacinal e que 47,18% do rebanho estadual foi vacinado durante os meses em que ocorreu a vacinação contra febre aftosa, demonstrando uma possível associação entre os procedimentos, que pode ser justificada pela economia e a maior praticidade de se manejar o rebanho uma única vez para se realizar as duas imunizações. O cálculo das vacináveis foi realizado com base na aproximação proporcional do efetivo de fêmeas considerando para cada grupo etário o intervalo da janela vacinal de seis meses. A taxa de cobertura vacinal foi estimada com base em método que considera a evolução de coortes de fêmeas nascidas ao longo de um ano. A obtenção das taxas reais de mortalidade e natalidade possibilitou a realização de novas simulações de modo a avaliar o real impacto dessa estratégia na redução da prevalência de brucelose bovina. Os resultados obtidos com base neste cálculo fornecem a melhor estimativa possível com base nos dados atualmente disponíveis na maioria dos estados. Conclui-se que é necessária uma padronização nacional na forma de cálculo de cobertura vacinal, que o estado do Mato Grosso manteve-se com níveis acima de 80% de cobertura vacinal entre os anos de 2006 a 2010 e considerando-se as taxas de natalidade e mortalidade reais e a perspectiva de manutenção deste nível de cobertura vacinal, estima-se demoraria um pouco mais de dez anos para reduzir em 50% a proporção de fêmeas positivas da população inicial e em torno de duas décadas para reduzir a prevalência ao nível de 2%. Vale ressaltar que este modelo considerou apenas o efeito da estratégia de vacinação, desconsiderando outros pontos de ação do programa como, por exemplo, a certificação de propriedades livres e o controle da movimentação animal no Estado, fatores estes que poderiam auxiliar na aceleração do processo de redução da prevalência no estado.

▼ Rabies is a zoonotic, rapidly progressive, fatal virus which targets the central nervous system and is mainly transmitted by bites and scratches from domestic dogs (Canis lupus familiaris) acting as the main reservoir of disease. Not only dogs and humans play a role in the dynamics of rabies, it is also known as a disease that is of conservation interest. Wild carnivore populations have been affected by rabies virus over the past 20 years. The design and success of long-term rabies control programs aimed at domestic dogs in developing countries may be affected by many factors such as high density populations and high turnover rates.
The objective of this study was to collect data through a household-level census in three rural communities in the sub-district of Bushbuckridge, Mpumalanga Province of South Africa, bordering a large privately-owned conservation area. With this data we aim to assess rabies-
vaccination coverage and other factors that might influence the success of the on-going
vaccination campaign in the study areas. A descriptive analysis of household characteristics, dog demographics, and contraception demand was performed.
A total of 1086 households were interviewed representing a total of 5115 persons and 413 dogs. Dog densities were found to be 169 dogs/km2, 128 dogs/km2, and 133 dogs/km2. We found that the dog:human ratio is 1:11 and 1:15 in the three studied communities. Of all the households included in this study 227 (21%) were DOHH and 863 (79%) of them NOHH.
More than 60% of the dogs were found to be free roaming in all three communities. The dog populations were comprised principally of adults (>1 year of age) which made up 52 - 69% of the dog populations in the three communities. The sex ratio of the dog population in all three communities is skewed towards males. The average number of litters in the past twelve months ranged from 1,0-1,3 litter(s), the mean size of the litter was 5,0-5,2 pups and the mortality in the first week after birth 0-45,9%. Neutered dogs (<12%) are not a common finding in any of the three communities. Owners were willing to pay an average of $8 for the 2-year contraception injection.
The
vaccination coverage range in each of the three communities was 48.6%-57.3%, 68.7%-77.4% and 53.3%-77.8%. We did not detect any significance between confinement characteristics and the
vaccination status of dogs in the three communities. Our results show that over 85% of dogs in all three communities were vaccinated during a
vaccination campaign where house-to-house visits were carried out.
Veterinarians, medical practitioners, and health authorities have the responsibility to apply intersectoral collaboration under the motto of ‘One Health’. We need to strive for a high level of risk perception among dog owners and an increased belief in the benefits of
vaccination through public education activities. Proactive and sustainable

▼ A vacinação das crianças no primeiro ano de vida é fundamental para a prevenção de várias doenças transmissíveis e é um dos fatores associados à redução da taxa de mortalidade infantil. A identificação da cobertura vacinal e dos fatores responsáveis pelo retardo ou pela falta de imunização é fundamental para a adequada monitorização dos programas de vacinação e para se identificar e atingir as crianças que não são vacinadas corretamente. Este estudo teve como objetivo analisar a cobertura vacinal em crianças de 0 a 18 meses de idade residentes no município de São Luís - MA. O presente estudo descreve os resultados de um inquérito domiciliar para estimar a cobertura vacinal do esquema básico até o segundo ano de vida em São Luís - Ma, da coorte nascida em 2005. O estudo adotou a metodologia preconizada pela Organização Pan-Americana da Saúde para a realização de inquéritos de cobertura vacinal. Das 630 entrevistas previstas foram realizadas 561 (89%). Os resultados mostram maior proporção de meninas, havendo um predomínio de crianças brancas à medida que se elevam os níveis de escolaridade e renda da família. A distribuição da cobertura vacinal por setores censitários é heterogênea, encontrando-se as piores coberturas geralmente em estratos de menor nível socioeconômico. A BCG foi a vacina de melhor cobertura e observa-se uma diferença significativa entre os dados de produção e os dados obtidos através do inquérito, referentes a todas as vacinas. As coberturas vacinais aos 18 meses (doses válidas) encontram-se inferiores ao desejado, exceto para BCG (96% de cobertura). A Cobertura vacinal (doses válidas), esquema básico completo para o conjunto das vacinas é de apenas 51%. Os dados do inquérito tiveram um resultado muito aquém quando comparados com os dados administrativos. Os dados aqui coletados demonstram que aqueles registrados pelos órgãos oficiais, superestimam a cobertura desse segmento populacional sugerindo imunidade em massa, fator este errôneo gerando uma falsa proteção e que demonstra uma falha no sistema de avaliação do serviço, sendo então necessárias outras estratégias para monitorização das ações do Programa Nacional de Imunização.
Vaccination of children in the first year of life is essential for the prevention of various diseases and is one of the factors associated with reduced infant mortality rate. The identification of coverage and the factors responsible for delay or lack of immunization is critical for proper monitoring of vaccination programs and to identify and reach children who are not vaccinated properly. This study aimed to assess vaccination coverage in children 0-18 months of age living in São Luís - MA. ▼ Detailed and up-to-date information on
vaccination coverage is essential to
monitor the degree of realization of a
vaccination recommendation or the
success of immunization programmes and – if necessary – to enhance
communication activities. Since data on
vaccination coverage is systematically
collected only at school entry in Germany, respective data for the adult
population was lacking so far. The main objectives of this dissertation were
i) to assess
coverage for standard vaccinations for adults (seasonal
influenza, tetanus, pertussis) and for the
vaccination against pandemic
influenza (season 2009/10) and ii) to identify factors associated with low
vaccination coverage. For this purpose we used data from the ‘German Health
Update’ Survey (GEDA). GEDA is a large (~22,000 participants), computer
assisted telephone interviewing (CATI) survey on health-relevant topics. The
study population of GEDA is representative for the German-speaking adult
population in Germany. GEDA was implemented in 2009 and is conducted annually
by the Robert Koch Institute as part of Germany’s national health monitoring.
For this dissertation data from GEDA09, GEDA10 and a follow-up interviewing of
~2,500 GEDA10 participants were analysed. In a comparison of the three main
target groups for seasonal influenza
vaccination, health care workers (HCW)
had a significantly lower
vaccination coverage (seasons 2007/08-2010/11:
average
coverage 26%) than persons ≥60 years (52%) and persons with underlying
chronic diseases (42%). The EU-goal of reaching
coverage of ≥75% in elderly
and chronically ill persons has not been achieved in Germany yet. Acceptance
of pandemic influenza
vaccination in season 2009/10 was very low in the German
adult population (
coverage: 9%). In total, 73% of adults had an adequate
tetanus
vaccination status (
vaccination in the past ten years). Only 11% of
persons belonging to a risk group for pertussis (e.g. HCW, persons with close
contact to infants) had an adequate pertussis
vaccination status. Among other
things living in the Eastern federal states and being of younger age were
independently associated with both higher tetanus and pertussis
vaccination
coverage. This dissertation contributed to closing the data gap regarding
vaccination coverage of adults living in Germany. Moreover, population
subgroups with particular low
coverage were identified (e.g. HCW for seasonal
influenza
vaccination, elderly persons for tetanus
vaccination). Our results
can be used to develop tailored strategies to close
vaccination gaps among
adults in Germany.

▼ Patients with cancer are at risk for post influenza complications due to their underlying disease as well as their (immunosuppressive) treatment; therefore influenza
vaccination is recommended to prevent influenza infections. Adherence to
vaccination recommendations is however low, most likely explained by the lack of evidence regarding efficacy/effectiveness and optimal timing of
vaccination in this particular patient group. This thesis addresses the burden of influenza in adult patients with either haematological cancers or solid tumours and response to influenza virus
vaccination as well as optimal timing of
vaccination during the chemotherapy cycle in patients with malignancies treated with chemotherapy. To assess the burden of influenza, incidence of influenza and other respiratory virus infections in patients with acute myeloid leukemia (AML) was studied as well as excess mortality in cancer patients during the seasonal influenza period in the Netherlands. Forty-four percent of patients with AML presenting with fever and/or upper respiratory tract complaints tested positive for an influenza virus or other respiratory viruses, influenza viruses (A and B) accounted for most of the infections. Regarding excess mortality, a trend for higher number of cancer deaths was observed during influenza season in the Netherlands. Moreover, a trend was found for a positive association between influenza incidence and number of cancer deaths. The second part of this thesis addresses efficacy of
vaccination; antibody responses and the broadness of the antibody response to influenza
vaccination were studied in breast cancer and colorectal cancer patients. As a surrogate end-point of vaccine efficacy, the acquired antibody levels after
vaccination are considered to be above threshold of protection (seroprotection) and increase in antibody titres (seroconversion) were presented. In breast cancer patients overall antibody responses were adequate. Statistically significant higher responses were observed in patients who received early
vaccination (day 5 after chemotherapy) when compared to late
vaccination (day 16). In colorectal cancer patients overall serologic responses were adequate with no statistically difference found between early and late
vaccination. In addition to the determination of vaccine virus specific antibody titres in breast cancer patients, the broadness of the response was assessed by using a protein microarray consisting of 11 different influenza strains. A wide range in serum antibody response after
vaccination was found, overall the response was broad because of the significant increase of antibodies against strains not represented in the vaccine. Higher age and previous influenza

▼ Neste estudo, hipotetizou-se que existia alta incidência de cirurgiões dentistas com baixa cobertura vacinal para hepatite B e positividade para contato prévio ou corrente para a hepatite viral do tipo B. O presente estudo transversal avaliou a condição vacinal e perfil sorológico dos cirurgiões dentistas para hepatite viral do tipo B no município de Porto Velho/RO - Brasil. Foram analisadas pela técnica de ELISA oitenta amostras de sangue (soro) dos cirurgiões dentistas residentes no município de Porto Velho/RO, visando a detecção dos marcadores sorológicos do Vírus da hepatite B: HBsAg, anti HBc total, anti-HBc IgM e anti-HBs. As amostras anti HBc Total positivas foram testadas para o marcador anti HBc IgM. Foram analisados os achados laboratoriais com dados de formação profissional, uso de equipamento de proteção individual (EPI) e índice vacinal entre os profissionais. Os dados foram comparados utilizando-se o teste Kruskal-Wallis (p<0,05). Dos cirurgiões dentistas avaliados, 45% faziam clínica geral e 38% relataram atualizar-se profissionalmente uma vez ao ano. A maioria (p<0,05) dos participantes (59%) recebeu as três doses da vacina contra hepatite B e 11% apenas duas doses. Dentre os que receberam três doses a maior (p<0,05) frequência (47%) apresentou soroconversão decorrente da vacinação. Embora abaixo do número esperado de 100%, a maioria dos cirurgiões dentistas de Porto Velho recebeu as três doses da vacina o que em geral acarretou imunidade. Entretanto, o perfil sorológico nem sempre foi compatível com a cobertura vacinal. Nossos achados sugerem a necessidade de execução de provas laboratoriais para confirmação e/ou monitoramento do perfil sorológico decorrente da vacinação contra hepatite B.
This search supposed that it had a high incidence of dentists with low vaccine covering for hepatitis B and a positive for previous or current contact for the virus hepatitis of type B. The transversal study evaluated the vaccines condition and serum profile of the dentists for viral hepatitis of type B in Porto Velhos city in Rondonias state Brazil. eighty samples of blood (serum) of the resident surgeons had been analyzed by the ELISAs technique of the dentists in Porto Velhos city, to only find out about the serums detention markers of the Virus of hepatitis B: HBsAg, anti total HBc, anti-HBc IgM and anti-HBs. The positive samples anti Total HBc had been tested for the marker anti HBc IgM. And also it had been analyzed at the lab findings with data of professional formation, equipment used as an individual protection (EPI) and vaccine index between the professionals. ▼ Atribuem-se variações nas coberturas vacinais (CV) em <1 ano às limitações do cálculo do indicador, relativas ao registro de doses e à escolha do denominador. Para este último, a cobertura do Sinasc para nascidos vivos (NV) e estimativas populacionais fazem variar o cálculo das CV em algumas áreas. Critérios que orientem a escolha do denominador tornamse necessários às ações do PNI. Este estudo descritivo ecológico-espacial avaliou estimativas populacionais do IBGE/Datasus, número de nascimentos pela metodologia RCoortes e registros do Sinasc em 2005 para as CV. O numerador foi o número de 3as doses de DTP+Hib, indicadora das CV em <1ano. Compararam-se CV por estratos de cobertura e porte populacional dos municípios em relação à cobertura do Sinasc e estimativas populacionais. Valores atípicos e mais elevados das CV ocorreram quando se utilizou NV em municípios de pequeno porte sugerindo sub-registro no Sinasc. Nestes os dados populacionais parecem menos precisos. Os Coeficientes de Correlação de Pearson (R2) entre o número de NV e população para municípios com <100 NV/ano e com 100-999 NV foram, respectivamente, 59,8% e 87,8%; em municípios com 1000+ NV R2 foi >95%. Indica-se que o número de NV seja o denominador adequado para o cálculo das CV, considerando a cobertura crescente do Sinasc. Estimativas RCoortes aproximam-se de dados populacionais especialmente nas UF e devem ser um denominador alternativo. Recomenda-se cautela nos municípios de pequeno porte, monitorando-se as coberturas vacinais e do Sinasc. Variations in vaccine coverage (VC) in children less than one year of age is attributed to limitations in doses registry and in choosing adequate denominator. Sinasc coverage to live births (LV) and population estimators may lead to VC variability in certain areas. Criteria to guide denominators choice are necessary to PNI. An ecologic-spatial study was carried out to evaluate and compare population estimates (IBGE/Datasus), number of LV by RCoortes method and Sinasc in 2005 to calculate VC by states and municipalities in Brazil. Numerator was number of 3rd doses of DTP+Hib in <1 year old. Atypical and high VC figures were observed in municipalities of small population, suggesting Sinasc underregistration. For those, population estimates seemed less precise. Pearson correlation coefficients (R2) between number of live births and population estimates for municipalities with <100 LV/yr e com 100-999 LV were, respectively, 59.8% e 87.8%; in municipalities with 1000+ LV R2 was >95%. It is suggested that the number of LV become the adequate denominator to calculate VC, considering increase in Sinasc coverage. RCoortes estimators are close to population estimates, especially for States and may be an alternative denominator. The Albanian system of epidemiological surveillance for infectious diseases has been and continues to be mandatory: the infectious diseases included in this system should be reported by law. The statutory notification system dates from the years ’50. In spite of small improvements over the time, the essential features of the system remained as before until 1998. Before the years ’98, the surveillance system have had registered a shortage and deficiency on the information flow on infectious diseases due to the non application of active and sentinel epidemiological surveillance, related directly to quantitative and qualitative shortage of the existing form of mandatory reporting of infectious diseases.
Today in Albania, the information flow on infectious diseases has been realized and continues to be realized through the monthly reporting form (named-14/Sh). The monthly reporting form of infectious diseases (14/Sh) is compiled by the epidemiological service in the district level and sent to the Department of Epidemiology and Biostatistics at the Public Health Institute (under the direction of the Minister of Health nr.189, dt.08.08.1995, and the approval of the Statistical Institute, 134 letters, dated, 08.25.1995). The monthly data are collected by the departments of epidemiology in the 36 districts from general practitioners, hospitals and laboratories. Surveillance system in Albania has in use 4 cards of notification. Group A (14-1/Sh) includes infectious diseases of the utmost importance to public health, group B / 1 (14-2/Sh) are those for which there is an obligation to notify rapid (within 1-3 days), group B / 2 (14-3/Sh) includes tuberculosis diseases and group B / 3 (14-4Sh) sexually transmitted diseases.
Regarding the immunization system has started at the end of the 1950 with diphtheria toxoid and later with DT and DTP in the early 1970s, tuberculosis vaccine (BCG), measles and polio vaccine. After 1993, in the immunization calendar are included other vaccines such as viral hepatitis B, trivalent vaccine (measles, rubella and mumps) and at the end homophiles’ influenza type B which is introduced in 2009. Since 1993, UNICEF started to provide the whole vaccines according to the WHO recommendation for immunization. In 1995, Ministry of Health in Albania introduced also the immunization against hepatitis B for all newborns. After the year 1993 in the immunization calendar have been introduced the following vaccines:
1995 - Viral hepatitis B,
2000 – bivalent vaccine of measles & rubella (MR)
2005 – trivalent vaccine measles, mumps and rubella (MMR)
2009 – penta-vaccine DTP + Hep.B + HiB (influenza hemophilus B).

based upon the mandatory notification according to the Protection Against
Infection Act. A continuous collection of nationwide
vaccination coverage is
primarily performed at school entry examinations. Hence, knowledge on
nationwide incidences of non-notifiable diseases and
vaccination coverage of
target groups of
vaccination programmes is scarce. The aim of this thesis is
to investigate if data from associations of statutory health insurance
physicians (ASHIPs) can be used to determine
vaccination coverage and
incidences of vaccine preventable diseases representative for Germany.
Methods. The ASHIPs provide pseudonymous physicians billing data including
data on vaccinations, measles diagnoses, mumps diagnoses, pertussis diagnoses,
varicella diagnoses and herpes zoster diagnoses to the Robert Koch Institute.
Study population is the statutory health insured population (85.5% of the
German population). Results. We developed a methodological concept that
ensures quality of ASHIP data and established adequate denominators to
calculate
vaccination coverage and incidences. Analyses of varicella
vaccinations, influenza vaccinations and measles diagnoses showed that using
ASHIP data, age specific
vaccination coverage and incidences can be
determined. Furthermore, these data are suitable to investigate if
vaccinations are given at the recommended age, if persons are regularly
vaccinated and if there is underreporting within the mandatory notification
system. Conclusions. The ASHIP data provide information of the implementation
of
vaccination recommendations and therefore, close a gap in knowledge. The
established methodological concept for the use of ASHIP data forms a basis for
a continuous monitoring of nationwide age specific
vaccination coverage and
incidences of vaccine preventable diseases.

represent the controversial issue about the H1N1 flu
vaccination
and its relation to the public’s perception toward the health
threat. It includes a survey analysis on the public’s perceptions
toward the H1N1 flu vaccine and a narrative analysis of the
newspaper
coverage of it. The author focuses on how narratives of
the novel H1N1 flu vaccine are designed by reporters as narrators,
and how people think about the H1N1 flu vaccine based on some key
variables that are thought to be the factors of influencing
people’s perception and attitude toward the H1N1 flu vaccine. By
analyzing the results and examining the relation between the two
analyses, the conclusion suggests a narrative framework that helps
health communicators to better convey health information and
enhance the public’s health knowledge, and inform the public
without causing panic.

▼ Introduction: Systematic monitoring of vaccination coverage (VC) is necessary to targetpublic health interventions. In Greece, childhood VC is not monitored routinely.Most of thecurrent knowledge on immunization uptake is based on two national population-basedsurveys conducted in 1997 and in 2001, using representative samples of children.Furthermore, there have not been any published studies to identify determinants ofvaccination uptake in the country. By identifying potential parental beliefs, barriers and socialfactors associated with underimmunization, modifications may be made in strategiesdesigned to ensure that more children receive all recommended vaccines. In 2006 and 2012,we conducted three cross-sectional studies to: i) estimate the VC among children in Greece(2006), ii) identify the potential effects of parental beliefs, perceived barriers andsocioeconomic characteristics on immunization status of children (2006) and iii) monitor thetrends in parental beleifs and attitudes towards childhood vaccination (2006 and 2012).Methods: In all three surveys, we used stratified cluster sampling, among children attendingthe first year of the Greek Grammar school (about 6 years of age) and theirparents/guardians. All pupils in the selected clusters (school classes) provided theirvaccination booklet and their parents/guardians completed a questionnaire regarding beliefsand attitudes towards immunization.We calculated proportions,Risk Ratios (RR) and RiskDifferences(RD) allowing for the sampling designs.Results: In 2006 and 2012, 88% (N= 3878) and 80% (N=1004) of pupils, respectively,provided their vaccination booklet and their parents/guardians completed a questionnaire. Ofall participant children in 2006, 91%(95%CI 90–93) had received 5 doses of DTP, 77%(95%CI 75–80) 2 doses of MMR, 20% (95%CI 18–22) 3 doses of hepatitis B vaccine by theage of 12 months, 72%(95%CI 70-74) 1 dose of meningitis-C vaccine and 13% (95%CI 4-6) 1dose of varicella vaccine. Furthermore, 64% (95%CI 61–66) had received all therecommended doses and vaccines and 52%(95%CI 50–55) had an age-appropriatevaccination status. Of all 1st doses of MMR and Hepatitis B vaccines, 70% (95%CI 67-72) and67% (95%CI 64-68) were administered in private practices, respectively. Parents/guardiansindicated a high degree of perceived necessity of immunization with the 99% agreeing thatvaccination is beneficial for their children. Belonging to a minority group (RR 0.53; 95%CI0.45-0.64, RR 0.41; 95%CI 0.32-0.53), having other siblings (RR 0.73; 95%CI 0.62-0.84, RR0.71; 95%CI 0.59-0.81), and perceiving long distance to immunization site as a barrier (RR0.93; 95%CI 0.86-1.01, RR 0.87; 95%CI 0.76-0.99) were independent predictors of bothcomplete and age-appropriate vaccination status, respectively. ▼ Objetivos: Estimar a cobertura vacinal contra a influenza em idosos; identificar os motivos de não adesão da vacina e analisar os fatores associados à adesão à vacinação. Metodologia: Estudo transversal de base populacional, desenvolvido com dados do Projeto Saúde, Bem-Estar e Envelhecimento (SABE). Foram incluídas pessoas de 60 anos e mais, residentes no Município de São Paulo/SP, Brasil. A amostra final foi constituída de 1.399 idosos, de ambos os sexos, selecionados a partir de amostragem probabilística por conglomerados. A análise de dados levou em consideração os pesos de amostragem, propiciando a inferência de representatividade das conclusões. A variável dependente foi o relato de vacinação contra a influenza no ano de 2006. As variáveis independentes incluíram características demográficas, socioeconômicas, comportamentais, condições de saúde autorreferidas e uso e acesso de serviços de saúde. Como medida de efeito e associação entre variáveis, utilizou-se a razão de prevalências e intervalos de confiança de 95 por cento, conforme estimadas diretamente e com ajuste multivariado por meio da regressão de Poisson. Resultados: Registrou-se cobertura vacinal autorreferida de 73,8 por cento. O principal motivo relatado para a não adesão à vacinação foi não acreditar na vacina. Observou-se menor percentual de vacinados entre os idosos na faixa etária de 60 a 69 anos. As variáveis que se mostraram associadas à vacinação e permaneceram no modelo final foram: idade, 70-79 anos (RP = 1,13; IC 95 por cento: 1,06-1,21), 80 anos e mais (RP = 1,11; IC 95 por cento: 1,02-1,21); número de doenças crônicas, uma (RP = 1,13; IC 95 por cento: 1,01-1,27), duas ou mais (RP = 1,18; IC 95 por cento: 1,06-1,32) e atendimento à saúde no último ano (RP = 1,40; IC 95 por cento: 1,08-1,80). Associação negativa foi encontrada para os idosos que sofreram internação no último ano (RP = 0,84; IC 95 por cento: 0,75-0,96). Conclusão: Os fatores associados à vacinação contra a influenza em idosos apresentaram estrutura multidimensional, incluindo características demográficas, condições de saúde e uso e acesso de serviços de saúde. No entanto, as variáveis socioeconômicas não associaram com a adesão à medida, indicando que o acesso à vacinação não diferiu entre os estratos sociais. ▼ INTRODUCTION: There is extensive evidence that vaccination is one of the most effective public health strategies. Vaccination coverage of the child population is a crucial health indicator, which reflects the overall level of public health services provided. Systematic and periodic monitoring of childhood vaccination uptake at a national level is instrumental for drawing valuable conclusions regarding planning, monitoring of the implementation and assessing national vaccination policies.The Roma are one of the most vulnerable minority groups in Europe. In Greece, Roma are experiencing severe social inequalities, they are not sufficiently integrated in Greek society and they face serious problems in relation to housing, employment, education and health. Published research on Roma health issues is sparse, scattered and fragmentary, and is characterized by serious methodological limitations. A common denominator of all findings is estimates of poorer health and higher mortality rates compared to the majority population, as well as higher prevalence of health-related risk factors in the Roma population, and the child population in particular. The scarce reliable data on Roma vaccination coverage are mainly derived from small local studies and show very low rates of vaccination. AIM: The aim of the present study is to obtain reliable quantitative estimates of vaccination coverage of the Greek Roma child population at a national level, to identify associated risk factors, as well as to investigate Roma parental perceptions about, and barriers to, child vaccination in Greece. METHODS: We carried out a cross sectional vaccination coverage survey of Roma children at a national level using stratified cluster sampling. Thirty Roma settlements (clusters), stratified by geographical region and settlement type (settlements mostly consisting of houses, of houses and shacks/tents, or mostly of shacks/tents), were included; 7–10 children aged 2–6 years were selected from each settlement using systematic sampling. This was a modification of the 30 by 7 sampling design developed by World Health Organization. Information on children’s vaccination coverage was collected from multiple sources (child health booklet, health service records, parental reporting); parents and 2–4 key persons (Roma and health/welfare professionals) from each of the selected Roma settlements provided information regarding perceptions among Greek Roma about immunization and barriers to vaccination. In the analysis we applied weights for each stratum reflecting its relative Roma population size, which were identified through a consensus process among experts in the field of Roma issues in Greece. In addition, we organized five focus groups with the participation of young Roma mothers, grandmothers and Roma health mediators in order to enrich the interpretation of quantitative findings and to further understand parental perceptions and barriers regarding Roma childhood vaccinations. ▼ We sought to characterize the temporal trends in nasopharyngeal carriage of macrolideresistant pneumococci during a period with increased heptavalent pneumococcal conjugate vaccine (PCV7) coverage in Central Greece.Methods:Streptococcus pneumoniae isolates were recovered from 2649 nasopharyngeal samples obtained from day-care center attendees in Central Greece during 2005–2009.A phenotypic and genotypic analysis of the isolates was performed, including the identification of macrolide resistance genes mef(A), subclasses mef(A) and mef(E), as well as erm(B).Results:Of the 1105 typeable S. pneumoniae isolates, 265 (24%) were macrolide-resistant; 22% in 2005, 33.3% in 2006, 23.7% in 2007, and 20.5% in 2009 (P=0.398). Among these macrolideresistant pneumococci, 28.5% possessed erm(B), 24.3% erm(B)+mef(E), 41.8% mef(E), and 5.3% mef(A). A mef gene as the sole resistance determinant was carried by 31% of macrolide-resistant isolates belonging to PCV7 serotypes and 75.8% of the non-PCV7 serotypes.Across the 4 annual surveillances, pneumococci carrying mef(A) gradually disappeared, whereas serotype 19F isolates carrying both erm(B) and mef(E) persisted without significant yearly fluctuations. Among isolates belonging to non-PCV7 serotypes, macrolide-resistance was observed in those of serotypes 6A, 19A, 10A, 15A, 15B/C, 35F, 35A, and 24F. In 2009, ie 5 years after the introduction of PCV7 in our country, 59% of macrolide-resistant pneumococci belonged to non-PCV7 serotypes.Conclusions:Across the study period, the annual frequency of macrolide-resistant isolates did not change significantly, but in 2009 a marked shift to non-PCV7 serotypes occurred. Overall, more than half of the macrolide-resistant isolates possessed erm(B) either alone or in combination with mef(E). erm(B) dominated among isolates belonging to PCV7 serotypes, but not among those of non-PCV7 serotypes.
