In spite of the highly fluctuating oil prices seen over the past number of years, the availability of relatively inexpensive sources of liquid hydrocarbons is diminishing, and the development of economically and environmentally viable alternative technologies to supply these hydrocarbons is vital. Projecting into the short and mid-term futures, the world economy is expected to continue to depend on hydrocarbon resources for stability, and thus technologies to produce these resources are important. Synthesis gas (CO and H₂), can be produced from a number of sources and feedstocks, such as biomass, waste and natural gas, and synthesis gas can be converted into liquid fuels such as hydrocarbons and alcohols, which are currently produced in large quantities in the conventional petrochemical industry. The selective oxidation of hydrocarbons, and in particular natural gas of which CH₄ the primary constituent, is notoriously difficult to control, mainly due to propensity of the reaction to generate to products of complete combustion. N₂O is a potent greenhouse gas (CO₂(e) = 300) and its concentration in the atmosphere is continuously increasing [Lee, Ryu et al, International Journal of Greenhouse Gas Control, January 2011, Pages 167–176]. Despite its greenhouse gas notoriety, N₂O is also recognised as a more selective oxidant than O₂ and has been used to selectively oxidise CH₄ to produce valuable products including synthesis gas. The production of synthesis gas, via its reaction with N₂O could be beneficial environmentally, as it could be used to provide valuable products when used to reaction with natural gas. The main aim of this research work is to investigate the scope of selective oxidation of natural gas (mainly CH₄) with N₂O over cobalt catalysts. The catalytic work was carried out in a continuous flow fixed bed micro reactor. Catalysts were prepared according to literature methods, as well as a new novel deposition method. The techniques used for catalyst characterization were powder X-ray diffraction (XRD), X-ray photoelectron spectroscopy (XPS), In situ Fourier transform infrared spectroscopy (FTIR), nitrogen adsorption/desorption isotherms, temperature program desorption (TPD), chemisorption, transmission electron microscopy (TEM). The elemental composition of catalyst samples was determined by using inductively coupled plasma optical emission spectrometer (ICP-OES). Based upon the experimental results obtained, the oxidation of CH₄ with N₂O over cobalt exchanged ZSM-5 (Co-ZSM-5) catalyst produced synthesis gas at a relatively high yield (19 %). When O₂ (instead of N₂O) was used as an oxidant over Co-ZSM-5 catalyst, synthesis gas was not formed. Similarly, when CH₄ and N₂O reacted over a cobalt on alumina catalyst, synthesis gas was not produced. Interestingly, H₂ was not detected as a reaction product when CH₄ with N₂O reacted over a Fe-ZSM-5 catalyst and only CO_x and H₂O were formed. L'intégration du métal dans les nœuds technologiques sub-20 nm requiert une conformité supérieure à celle permise par la PVD. Les techniques de CVD, plus spécifiquement la MOCVD et l'ALD, ont été identifiées comme les meilleures solutions pour le dépôt de métal. Pour une application de métal de grille, les alliages carbo-nitrurés de titane et tantale sont considérés comme les plus prometteurs. Dans ce travail une revue détaillée des mécanismes de dépôt par MOCVD et ALD, ainsi que sur l'influence du plasma sur les matériaux déposés est réalisée. Dans un premier temps, les fenêtres de procédés possibles pour un ajustement des propriétés des métaux sont inspectées attentivement. L'accent est mis sur l'impact du plasma sur le métal et sur les mécanismes réactionnels inhérents grâce à une caractérisation poussée du plasma. Par la suite, l'intégration de ces métaux est étudiée avec une analyse précise des interactions se déroulant aux interfaces. La corrélation entre les propriétés physico-chimiques et le comportement électrique des empilements métal/diélectrique à forte permittivité est soutenue par une analyse XPS. Finalement, le dopage aluminium de dépôts de TiN et TaN MOCVD est étudié pour l'obtention de grilles n-mos et p-mos. Par comparaison des propriétés et comportements du dopage aluminium de métaux déposés par PVD et MOCVD, des mécanismes de diffusion sont proposés afin d'expliquer le rôle de l'aluminium sur les variations observées.

For the sub-20 nm technological nodes metal conformity requirements are beyond the possibilities of the currently used PVD deposition technique. CVD techniques, more specifically MOCVD and ALD, are identified as the best techniques for metal deposition. For metal-gate application, titanium and tantalum carbo-nitrides alloys are considered as the most promising. In this work, a detailed review of MOCVD and ALD deposition mechanisms and plasma influence on the deposited material is carried out. First, process windows for successful tuning of the metal properties are examined. Plasma impact on the metal and the inherent reaction mechanisms are also highlighted with the help of plasma characterisation. Then great importance is given to the integration of these metals, by careful study of the interactions taking place at the interfaces. Correlations between physico-chemical properties and electrical behavior of the metal/high-k dielectric stack are introduced thanks to XPS characterisation. Finally, aluminium doping of MOCVD TiN and TaN is considered for n-mos and p-mos gate characteristics achievement. By comparison of the properties and behaviours of Al doped metals deposited by PVD and MOCVD, diffusion mechanisms are proposed to explain the role of Al in the observed changes.

Les travaux rapportés dans ce mémoire concernent le développement de la réactivité de N-tosylhydrazones dans la création de liaisons carbone-carbone et carbone-azote ainsi que leurs applications à la synthèse des analogues de l’isocombrétastatine A-4 (isoCA-4), aux propriétés antivasculaires.Au cours de ce travail, des molécules de type 1,1-diaryléthylène furent synthétisées par un couplage pallado-catalysé entre des N-tosylhydrazones et des aryles halogénés. Ainsi, des oléfines polysubstituées avec un motif alcoxyle, analogues de l’isoCA-4, ont présenté des activités biologiques intéressantes. De même, les N-tosylhydrazones constituent des intermédiaires pour la synthèse de 1,5-énynes aromatiques. La cyclisation de ces derniers a pu être contrôlée selon la nature du catalyseur utilisé. En effet, en présence de l’or comme catalyseur, une cyclisation 6-endo-dig majoritaire est obtenue alors qu’en présence du palladium, une cyclisation 5-exo-dig exclusive est observée. Cette méthode permettrait de synthétiser des analogues contraints de l’isoCA-4. Le couplage réducteur, catalysé au cuivre, entre des N-tosylhydrazones et des amines et conduisant à des motifs arylalkylamines fut développé. Enfin, un processus cascade entre des 2’-halogéno-biaryl- N-tosylhydrazones et des amines fournit des dérivés du fluorène par la création d’une liaison CN et d’une liaison CC sur le même atome carbénique.

This manuscript reports the development of new metal-catalyzed reactions using N-tosylhydrazones as coupling partners for the creation of carbon-carbon and carbon-nitrogen bonds. These reactions are applied in the synthesis of analogues of isocombretastatine A-4 (isoCA-4), a vascular disrupting agent.First, a palladium-catalyzed cross-coupling reaction between N-tosylhydrazones and aryl halides was developed. By this means, polysubstituted olefins with an alkoxy motif were synthesized and presented good biological activities. Then, aromatic 1,5-enynes were synthesized by a multi-step approach using N-tosylhydrazones as intermediates. The regioselectivity of the cyclisation of these enynes was controlled depending on the reaction conditions. Under gold catalysis, a major 6-endo-dig cyclisation pattern leads to naphthalene derivatives while under palladium catalysis; an exclusive 5-exo-dig cyclisation furnishes benzofulvène derivatives. Conformationnally restricted isoCA-4 analogues could be synthesized by applying this method. A copper-catalyzed reductive coupling between N-tosylhydrazones and amines leading to -branched amine derivatives was also developed. Finally, a cascade reaction between 2’-halo-biaryl-N-tosylhydrazones and amines furnished fluoren scaffold by the formation of CC and CN bonds on the same carbenic atom.

▼ A Translingual Literary Practice: Literatures in Contact The aim of this thesis is to explore literary translingualism. The translingual is a term that is becoming widespread in academia, but is still in need of fuller definition and of being distinguished from other terms with which it overlaps. This thesis uses the term translingual to refer to texts which use more than one language in specific interactive ways, emphasising the dynamic, fluid and generative qualities in texts which cross cultural and linguistic borders and boundaries, rather than defining such texts in a static and additive manner. Translingualism involves the capacity for languages in texts to influence and transform each other in the context of exchanges. Thus, the methodological approach used in this thesis integrates literary studies with findings in language contact studies, since its objective is to understand the way languages in contact in texts influence each other in transformative ways, rather than merely co-existing in the same diglossic space. This necessarily involves a text-focussed interpretive practice, which I term a translingual literary practice (TLP). This approach focuses on the ways linguistic elements are exchanged between or synthesised from two or more linguistic systems. On an aesthetic level, the languages may be used in such a way as to create innovations and produce a new type of literary text that challenges homogenous language systems or dominant discourses. A review of relevant extant secondary literature and a necessarily tentative definition of the translingual, whose purpose is to make clear the differences to similar-sounding terms which are often used indiscriminately, is followed by chapters addressing a wide range of examples of translingual writing from different genres, cultures and language combinations. Without claiming to provide definitive or final answers, the study's overall goal is to move forward the understanding of the translingual, its scope ? what it is and what it is not ? and its transformative force. L’amélioration de qualité de la formation universitaire dans tous les domaines et dans le domaine de la didactique des langues étrangères en particulier est une des directions prioritaires du développement des systèmes éducatifs nationaux. Les chercheurs autour du monde supposent que l’éducation moderne doit être basée sur l’approche par compétences, qui s’est formée sur la base des approches précédentes. En même temps la pratique d’enseignement moderne ne tient presque aucun compte de l’expérience unique des éducateurs de XXème siècle qui tachaient de trouver des solutions pour les même problèmes de l’éducation qui nous font face aujourd’hui, surtout en didactique des langues étrangères. Cette expérience demande un nouveau traitement dans le contexte de l’approche par compétences et celui de la formation linguistique. Parmi les éducateurs dont les conceptions pédagogiques sont proches des conceptions modernes des objectifs de l’éducation on peut nommer C. Freinet et V. A. Sukhomlinski. Dans le contexte de l’approche par compétence c’est possible d’actualiser l’expérience de ces didacticiens, qui ont créé les conceptions pédagogiques stimulant le développement des compétences, quoique ces éducateurs n’utilisassent pas le terme «compétence». L’actualité de cette problématique pour le système éducatif de Russie a conditionné notre inteprétation d’objectif de cette recherche qui consistait en définition de l’influence des conceptions pédagogiques de V. A. Sukhomlinski et C. Freinet sur le développement de la formation universitaire orientée par l’approche par compétences en prenant comme exemple la formation des compétences culturelles dans le processus de l’éducation à l’université ou à la faculté des langues étrangères en Russie. Dans le cadre de cette étude nous avons appliqué la méthodologie de l’expérience pédagogique pour argumenter l’efficacité de la méthode basée sur les conceptions pédagogiques de C. Freinet et V. A. Sukhomlinski en formation des compétences culturelles au niveau universitaire. Cette étude s’appuie sur un ensemble de théories et de recherches en sciences de l’éducation et en pédagogie. Les résultats reçus peuvent être appliqués dans la rédaction de programmes et de plans de la formation universitaire en Russie.

Enhancement of the quality of the university education in all majors and especially in language education is a priority direction of the development of national education systems. The researchers around the globe believe that the competence-based approach is a basis for education in today’s world. This approach has formed on the basis of the other approaches of the past. Meanwhile the modern educational practice practically ignores the unique experience of the XXth century educators who aspired to resolve the same educational issues that we face nowadays, especially in the sphere of language teaching. Passagens se refere à transição, deslocamento, mudança, transmissão e continuidade. Percepção do corpo como um todo na ação do fazer; no movimento e suas ressonâncias, na conexão com a respiração e como isso se transmite na modelagem dos trabalhos. Se passagem implica aqui em relacionar diferenças, algo que se dá entre o ser e a matéria, estamos falando de uma matéria inerte, fria e amorfa que recebe, registra e devolve uma ação1 que se dá a partir do contato com as mãos e o corpo. No início trabalhei a partir de galhos de árvores como uma estrutura para a modelagem, determinada pelo crescimento do vegetal; matéria que se organizava movida por um princípio de energia vital, sobrepondo neles a argila que se agregava sobre os movimentos da sua forma. Num segundo momento, durante a secagem dos trabalhos, fui realizando outras intervenções, até o momento da queima, quando os galhos desapareceram deixando vazios e vestígios de seu contato com a argila. A composição de massas cerâmicas associada à queima como processo de transformação, onde o forno é construído adequando-se a cada trabalho são questões também importantes que vem sendo desenvolvidas. Iniciada através da pintura e da gravura, a presente pesquisa expandiu-se aos processos da cerâmica. Trabalhando com procedimentos de destruição e regeneração, a relação entre intenção e acaso se dá por sobreposições contínuas. Na montagem dos trabalhos, diferentes contextos trazem questões específicas ao processo de criação, envolvendo a elaboração de instalações e pensando as peças como partes de um corpo que se articula pela aproximação e combinação entre elementos diversos, incluindo as pinturas, madeiras encontradas, como pedaços de árvores e gravuras.

Passages refers to transition, displacement, change, transmission and continuity. Perception of the body as a whole in the action of making; the movement and its resonances in connection with breathing and how this is transmitted in modeling the pieces. If passages here means relating differences, something that occurs somewhere between the being and the matter, we are talking about a cold, amorphous and inert matter which absorbs, register and gives back an action1 from the contact with the hands and body. Initially I worked from tree branches as a structure determinate by the growing of the plant, matter which organizes itself moved by a principle of vital energy, superposing the clay on the movements of its forms. Afterwards, as the pieces would get dry, Ive realized other interventions on it, until the moment of the firing, when the branches disappeared, leaving voids and vestiges of its contact with clay. The blending of ceramic materials as a composed paste, associated with firing as a process of transformation, in which the kiln is build in adequacy to each work, are also important issues that are being developed in this research. ▼ Background: The automated dispensing cabinets (ADCs) now allow for more rapid access to medications for both providers and pharmacists. However, automation may generate its own challenges with patient care. Medication overrides from ADCs circumvent pharmacist verification and creates an opportunity for medication errors. Methods: A 60-week quasi-experimental study has been conducted at large Veterans Affairs (VA) academic medical center from January 1, 2019 to February 29, 2020 to assess the efficacy of the ISMP-endorsed interventions in reducing medication overrides. Three interventions were implemented for this study: 1:1 nursing education, medication override list, and ADC medication override privilege modification. The interrupted-time series with multiple regression analysis was conducted to assess the efficacy of each intervention. The primary endpoint was the rate of medication overrides (primarily controlled substances and antibiotics) from the unit ADC at each intervention time periods. The secondary endpoints included medication override rates for controlled substances and fentanyl intravenous piggyback (IVPB), the most common overridden item, at each study intervention time periods. The other secondary endpoint was the comprehensive medication override rates for all medications in the unit ADC after November 1, 2019. Results: Total of 1,783 medication overrides from January 1, 2019 to February 29, 2020 were included in the final analysis from 616 patients. The interrupted time series with multiple logistic regression showed that the 1:1 nursing education significantly reduced the medication overrides (t = -6.10 [95% CI: -15.34 to -7.75]; P < 0.0001) and the decreased trend was maintained afterwards. No significance was found from the medication override list (t = -0.91 [95% CI: -5.17 to 1.94]; P=0.366) and the nursing ADC access privilege restriction (t = -0.82 [95% CI: -4.75 to 1.98; P=0.414]). Secondary endpoints have seen similar results. The 1:1 nursing education significantly reduced controlled substance (t = -6.34 [95% CI: -17.79 to -9.25]; P < 0.0001) and fentanyl IVPB override rates (t = -3.08 [95% CI: -43.69 to -9.28]; P = 0.003). The medication override list did not statistically reduce the controlled substances and fentanyl IVPB; whereas, ADC medication override privilege modification made a significant impact on fentanyl IVPB (t = -2.47 [95% CI: -34.08 to -3.56]; P=0.017). All medication overrides after November 1, 2019 have also significantly decreased monthly medication override rate from 7.63% to 2.90%. Conclusion: An interdisciplinary approach to ISMP-endorsed interventions significantly reduced the overall medication overrides rates in Veterans Affairs intensive care unit. ▼ Mg2Si-based thermoelectrics are currently the most promising, environmentally benign and inexpensive materials for power generation. The objective of this thesis is to characterize Mg2Si-based thermoelectric materials using state-of-the-art synchrotron radiation techniques including infrared reflectivity/absorption spectroscopy and high resolution X-ray powder diffraction. This is complemented by density functional theory (DFT) calculations. Also reported here is the main research: the analysis of the electronic structure and transport properties of doped Mg2Si using experimental and theoretical methods. To enhance the thermoelectric performance, Mg2Si doped with a single component of Bi or Sb were studied. The investigation showed by doping the electron carrier concentrations were increased. In particular, dc conductivities of the doped samples were extracted from the analysis of infrared reflectivity spectra employing the Drude free electron model. We found the conductivity was lower when determined by infrared measurements rather than in-situ four point probe measurements of the bulk sample because of the limited penetration depth of infrared (IR) radiation and the very small spot size. In particular, we were able to extract the electrical conductivity, relaxation times and electron effective masses of the samples. DFT calculations reproduced the experimental observations and show a substantial increase in the Seebeck coefficients. The next step was to study the effect after doping with two different dopants. For this purpose, we investigated the effect of Ge substitute Si in Bi doped Mg2Si. In particular, the dc conductivities of the doped samples were extracted from the analysis of infrared reflectivity spectra. From the IR data, we extracted the relevant parameters for electrical transport. The experimental data were explained with theoretical DFT calculations in which the calculated densities of states (DOS) of the Ge- and Bi-doped Mg2Si samples were found to be very similar, and therefore to have comparable Seebeck coefficients. The steep curvatures of the DOS at the Fermi level indicate a light electron band. We found the thermal conductivity of Mg2Si is substantially lower from 7 Wm-1K-1 to 2.7 Wm-1K-1 in Mg2Si0.677Ge0.3Bi0.023 at 300 K. A performance figure of merit of 0.7 was achieved at 773 K for this sample. We further investigated the effect of multi-doping with Sb, Al and Zn on the enhancement of the thermoelectric and electrical transport properties of Mg2Si. A maximum ZT of 0.964 was found for Sb0.5%Zn0.5% doped Mg2Si (Mg1.995Zn0.005Si0.995Sb0.005) at 880 K. This value is comparable to those of PbTe based thermoelectrics which are the currently the materials used in commercial products. We also studied the effect of pressure on the thermoelectric performance of a Al-doped Mg2Si sample. From in-situ X-ray diffraction, we observed a structural transform in which the electrical conductivity was increased after the phase transition. ▼ Mechanical Ventilation (MV) therapy is one of the most common treatments offered to patients with respiratory failure in ICU. MV assists patient recovery by completely or partially taking over the breathing process and helping with oxygen delivery and removal of carbon dioxide. However, inappropriate MV settings mismatched to a given patient’s condition can cause further damage. On the other hand, suboptimal MV settings can increase the length of stay of the patient in ICU and increase the cost of treatment. Acute Respiratory Distress Syndrome (ARDS) is a major form of Acute Lung Injury (ALI) where clinicians offer a supportive environment for patient recovery by application of MV. ARDS is characterised by inflamed and fluid filled lungs that result in alveolar collapse and thus severe hypoxemia. Application of positive end expiratory pressure (PEEP) is employed to recruit and retain lung units to maximise gas exchange. However, a delicate trade-off is required between maximising gas exchange and preventing further unintended damage to the lungs, when determining optimum PEEP level. Currently, no specific protocols to determine optimum PEEP level exist and selection of PEEP is dependent on medical intuition and experience, primarily due to lack of easy methods to determine patient – specific condition at the patient’s bedside. A mathematical recruitment model is developed in Labview to help determine patient – specific condition based on fundamental lung physiology and engineering principals in this thesis. The model utilises readily available clinical data to determine parameters that identify underlying patient – specific lung characteristics and conditions. Changes in these parameters can be monitored over time and compared between patients to determine the severity of the disease and evolution of disease with time. A second model is developed to determine dynamic functional residual capacity (dFRC), that represents the extra volume retained in a lung through application of PEEP. The model extends previous efforts in the field that applied the stress – strain theory to lung mechanics to estimate dFRC. This model estimates the patient’s dFRC using readily available clinical data (PV data) and can be monitored over time to determine changes in a xiii given patient’s condition. The dFRC model introduces a new parameter, , which is considered a population constant for the particular PEEP. The model offers an easy and reliable method to determine dFRC since other methods are normally invasive or require interruption of MV. The models developed were validated against real – time clinical data obtained through clinical trials. The recruitment model was found to fit the clinical data well with error values within acceptable limits. It also enabled identification of parameters that reflect the underlying patient – specific lung condition. ▼ Within a neo-liberal world polity, the concept of autonomy is increasingly perceived as conducive to postmodern nations’ rational progress in developing knowledge societies. In accordance with global trends, the Taiwanese government implemented a policy of performance-based school management in 2005 to enhance educational accountability, efficiency, equity and quality. This autonomy-based reform perceives all educational stakeholders as self-interested, utility-maximizing market egoists who are capable of realizing their maximum potential by ceaselessly making consumer-style choices. The government assumes that the provision of choices will give everyone an equal chance of educational success. The negative socio-political consequences brought about by the adoption of neo-liberalism’s beliefs and practices have been explicitly acknowledged and illustrated in the literature. Nonetheless, the ways neo-liberalism has affected Taiwan’s socio-educational reality have scarcely been acknowledged or examined, and even on a theoretical level there has been little thought given to the provision of alternative socio-educational possibilities. Thus, this research, grounded in the context of Taiwan, analyzed neo-liberal ideologies to discover their implications for socio-educational practices. Moral and philosophical insights from various theorists were synthesized and advanced as a substitute for neo-liberalism. This research was based on the method of deconstruction and reconstruction of textual discourse. For deconstructive analysis, the aim was to investigate and problematize how certain neo-liberal values have come to be globally/nationally institutionalized, and utilized to manipulate citizens’ consciousness for the maximization of economic efficiency, productivity, and profitability in the education market. The reconstructive synthesis, then, aimed to initiate possible socio-educational changes through reconceptualising these same values in respect to Taiwan’s contextual specificity. Overlooking the need to address neo-liberalism’s belief in individualism, inequitable socio-economic structures and monistic, decontextualised and mechanistic epistemology, the Taiwanese government’s promotion of autonomy was found to perpetuate socio-economic inequalities, power imbalances, human monism and intellectual inflexibility in education. A shift in epistemology wherein autonomy was reconceptualised as “heteronomous autonomy” was found to be capable of reorienting the overall frame of democratic reference towards a communitarian paradigm that would contribute to greater social equity and solidarity. This finding is extremely important as heteronomous autonomy takes human diversity as its foundation, so the emphasis changes from the rights of the individual to the self’s unconditional responsibility to and for differences. Thus, a heteronomous-autonomy-based education would forsake neo-liberalism’s standardized pedagogical approaches in favour of a creative framework-in-context. ▼ The primary aim of this study was to compare levels of cognitive anxiety for a group of first-time hearing aid users and their significant other before, during, and after hearing aid fitting. Secondary goals included: (1) comparing levels of cognitive anxiety for participants and their significant others at the same intervals, (2) assessing possible effects of gender on the experience of cognitive anxiety, (3) comparing the levels of cognitive anxiety for participants who adopted hearing aids and those that did not, and (4) identifying themes relating to the experience of hearing impairment and consultation for services. Thirty-nine adults between the ages of 30 and 87 years of age with and without hearing impairments participated in several interviews. Cognitive anxiety was measured using the Cognitive Anxiety Scale (CAS) by Viney & Westbrook (1976), which is a content analysis, grounded in personal construct psychology. The following research questions have been addressed: 1a) Do cognitive anxiety levels change in individuals with hearing impairment as they go through the consultation process? 1b) Is there any difference in cognitive anxiety levels between male and female participants with hearing impairment as they go through the consultation process? 2a) Do cognitive anxiety levels change in normal hearing significant others of individuals with hearing impairment as they go through the consultation process? 2b) Is there any difference in cognitive anxiety levels between male and female significant others as they go through the consultation process? 3a) Do cognitive anxiety levels differ between participants with hearing impairment who adopt amplification and those who do not? 3b) Do cognitive anxiety levels of significant others differ between participants with hearing impairment who adopt amplification and those who do not? Results revealed reduced cognitive anxiety levels for hearing impaired adults between the first interview and third interview, between the second interview and third interview, but no significant difference was found between the first interview and second interview. Furthermore, for significant others, CAS scores were significantly higher at interview 2 compared to interview 3, but no significant difference was found between interview 1 and interview 3, or between interview 1 and interview 2. Additionally, no significant differences in levels of cognitive anxiety were found between hearing impaired adults and significant others in the adopter and non-adopter group, and no gender differences were observed between hearing impaired adults and significant others. This study illustrates how important it is to involve the significant other in the consultation progress. However, due to a small sample size most analyses were underpowered therefore more research is needed to investigate cognitive anxiety levels in hearing impaired adults and their significant others.

▼ Macrovascular complications are the leading cause of morbidity and mortality in patients with type 2 diabetes mellitus (T2DM). Microalbuminuria (MA) is common and is a well-establisced risk factor for macrovascular diseases in patients with T2DM. A number of abnormalities have been described in diabetic patients with MA, including high blood pressure, dyslipidaemia, insulin resistance, endothelial dysfunction, left ventricular hypertrophy, hypercoagulation, and high plasma homocysteine as well as C-reactive protein levels, all of which increase the cardiovascular risk in this group of patients. Diabetic dyslipidaemia, which is also very frequent in patients with T2DM, is characterized by high levels of fasting triglycerides (TGs), low HDL cholesterol levels, and predominance of small, dense LDL cholesterol particles. Furthermore, the majority of patients with T2DM show high and prolonged postprandial hypertriglyceridemia after meals. Epidemiological data suggest that that high plasma TG levels, both in the fasting state and postprandially, are assciated with cardiovascular diseases in patients with diabetes. Lipoprotein lipase (LPL) is the key enzyme involved in the metabolism of TG-rich lipoproteins. However, literature data on the activity of LPL in patients with MA are not unanimous: some studies found decreased plasma LPL activity in microalbuminuric subjects, whereas others found no difference in LPL activity between normoalbminuric and microalbuminuric subjects.The main aim of this study was to evaluate the correlation between MA and postprandial hypertriglyceridaemia in patients with T2DM. Furthermore, potential differences in LPL and hepatic lipase (HL) activity, as well as in apolipoprotein A-V (apoA-V), apoC-II, and apoC-III involved in TG metabolism, between patients with and without MA were also investigated. A total of 64 subjects with T2DM, 30 with MA (mean age: 63 ± 6.7 years, mean DM duration: 10 years, 17 males, 13 females) and 34 without MA (mean age: 61.5 ± 7.6 years, mean DM duration: 10.5 years, 17 males, 17 females) were examined.Each subject attended the metabolic unit of our hospital twice in the morning after a 12-14 h fast. In the first visit, a fasting sample was taken for the determination of LPL and HL activities. After about 1 week patients received a standard mixed meal (total energy, 783 kcal). Blood was drawn in the fasting state and at 2, 4, 6 h after the test meal for biochemical determinations. After proper preparation, plasma samples were stored in small aliquost at -80ºC until assayed at the end of the study. The novel finding of this study is that normotriglyceridemic patients with T2DM and MA have an almost 3-fold higher postprandial triglyceridemia than patients without MA after ingestion of a mixed test meal. The incremental AUCs of total TGs and CM-TG were significantly higher in the patients with MA (both p< 0.001). Avec les récentes réformes, dont la loi Hôpital, Patient, Santé et Territoires (HPST) de Juin 2009, l’hôpital s’est transformé non seulement dans sa forme organisationnelle par la mise en oeuvre des pôles mais aussi dans son rapport à son environnement. Cette organisation polaire vise à rechercher une performance par une approche médico-économique. Ainsi les dispositifs de gestion ont été mis en place et cherchent à rendre compte de cette performance. Dans ce contexte, comment peuvent travailler des professionnels de champs aussi différents que celui de la gestion et celui du soin ? La gestion sous-entend une optimisation des moyens, alors que le service public défend en priorité des valeurs d’utilité sociale et d’attention aux usagers. Les acteurs du soin ont une forte identification professionnelle à cette mission d’utilité sociale. Dans ce travail, nous proposons d’identifier en quoi les décalages culturels au sein des pôles hospitaliers, participent à une perte de sens et de reconnaissance des agents à leur travail. Ces écarts culturels seront appréciés plus précisément au travers de l’appropriation des outils de gestion. Cette étude se base sur des entretiens réalisés au sein de différents hôpitaux (CHU et CH) et de secteur Médecine, Chirurgie, Obstétrique (MCO) et psychiatrie. Elle révèle lanécessité de pratiques managériales innovantes tournées notamment vers la gestion interculturelle afin que chaque acteur de ces nouvelles structures hospitalières se sente compris et intégré dans cette dynamique. L’étude proposée peut constituer un retour d’expérience intéressant pour mieux comprendre et améliorer l’utilisation des nouveaux outils de gestion. Des préconisations sont proposées et peuvent constituer des pistes de réflexion afin de répondre aux défis actuels de l’hôpital public.

With the latest reforms, among which the Hospital, Patient, Healthcare and Territory (HPST) Law, the French Hospital has changed not only the form of its organisation through the implementation of a polar organisation, but also its environmental concern. This polar organisation aims to strive for performance and efficiency via an managerial and medical approach. Thus, some management tools have been set up aimed in particular at giving feedback on such performance. In this context, how can professionals from such distinct fields as management and healthcare work? Management implies the optimization of resources when the public sector above all upholds values of social usefulness and patient care. Healthcare actors feel very much involved in this mission of social usefulness. In this work we propose to identify how cultural gaps within hospital poles contribute to a loss of meaning and acknowledgment among healthcare agents. Those cultural gaps will be examined more particularly through management tool appropriation. This study relies on interviews conducted in various healthcare facilities (PCO and psychiatric hospitals of various capacities such as CHU and CH).
