▼ Microdialysis is a method of monitoring biochemistry of tissues in-vivo, and is established as a clinical and research tool for monitoring of other tissue such as liver and brain. The aim of this thesis was to investigate the use of the microdialysis method in monitoring the human dermis in healthy and burned skin, with a view to its use as a tool to detect progression of the burn wound. Ten healthy volunteers were used to establish a normal range for glucose, lactate, pyruvate and glycerol levels, and to assess the effect of temperature on results. Ten patients with burns <15%TBSA were recruited and microdialysis was undertaken in zones of coagulation and stasis and in unburned skin in each patient from recruitment until36 hours post-bum. A further 2 patients with burns >30%TBSA were also studied. Biochemical findings and the result of amino acid assays are reported. An attempt at correlating biochemical findings with Laser Doppler Images was made, to establish the role of the technique in predicting wound progression. However no wounds showed any evidence of progression in the areas of probe placement over the time of the study.

▼ Methicillin-resistant Staphylococcus aureus (MRSA) and Pseudomonas aeruginosa are the main multi-drug resistant pathogens associated with deep wound infections which then may cause septicaemia. Treatment is problematic and re-infection is quite common. Free Living Amoebae (FLA), such as Acanthamoeba, are widely distributed in the environment and may also contaminate wounds. It is well known that Acanthamoeba feed on and protect bacteria. The role of Acanthamoeba in wound infections is not very well understood. It is possible that the presence of Acanthamoeba in wounds is one of the key factors for such re-infections. In this study, 140 wound swabs were collected to check for the presence of Acanthamoeba spp. Only one sample was positive for Acanthamoeba spp. Sequencing of the highly variable DF3 region of 18S rRNA gene for the sample showed that this isolate belongs to genotype T4. In addition, clinical isolates of MRSA and Pseudomonas from wound infections were used in this study. The results showed that MRSA and Pseudomonas were able to bind with, invade, survive and multiply inside Acanthamoeba species. One of the essential compounds for microorganisms to grow is iron. The role of iron chelators, including deferiprone and selected novel compounds based on hydroxyl pyridine moiety, was studied. Findings revealed that all novel iron chelators have an antimicrobial activity against both bacteria. In addition, all novel iron chelators were able to kill Acanthamoeba. Cytotoxic effects of MRSA, P. aeruginosa and Acanthamoeba were investigated using the KB epithelial cell line and mesenchymal stem cells (MSC) using a general caspase inhibitor. The results revealed that the ability of live bacteria to induce cell death was higher compared with heat-killed bacteria, bacteria conditioned medium (BCM) and Acanthamoeba conditioned media (CM). The exact trigger for the cell death in this study was not investigated but the relative contributions of apoptosis and necrosis were investigated using fluorescent technique, caspase inhibition and LDH assay. In conclusion, presence of Acanthamoeba in wounds could be the reason of prolong treatment and reinfection in wounds.

▼ Background: Metastases are the major cause of death from melanoma, a skin cancer which has the fastest rising incidence of any malignancy in the Western world. Molecular pathways that drive melanoblast migration in development are believed to underpin the movement and ultimately the metastasis of melanoma. Aims: In this thesis we use genetically modified mice models to characterise two novel anticancer targets, P-Rex1 and focal adhesion kinase (FAK). Embryonic melanoblast migration is compared with cancer outcomes for each genetic modification. Results: Mice lacking P-Rex1, a Rac-specific Rho GTPase guanine nucleotide exchange factor (GEF), have a melanoblast migration defect during development evidenced by a white belly. These P-Rex1-/- mice are resistant to metastasis when crossed to a murine model of melanoma, an effect specifically channeled through loss of P-Rex1 GEF activity. FAK disruption compromises melanoblast cell numbers and migration in development, but has no long-term effect on melanocyte homeostasis. FAK-deleted mice have a divergent role in melanomagenesis, delaying primary melanoma onset whilst promoting metastasis following disease onset. Conclusions: We conclude that P-Rex1 and FAK play important roles in melanoblast embryology and melanoma development and progression. Both P-Rex1 and FAK represent interesting therapeutic targets for the treatment of cancer.

▼ There is a requirement to assess the safety of chemicals to both 'man' and the environment. Traditionally this was determined through the use of animal testing. However, there is an increased need to develop alternatives to animal testing for the determination of toxicity due to ethical and legislative reasons. One approach to replacing the use of animals is the application of computational methods. These include Quantitative Structure-Activity Relationships ((Q)SARs), which are the formalisation of the relationship of the effects (e.g. toxicity) for a series of chemicals and their physico-chemical and structural properties. Most QSARs for toxicity require knowledge of a chemicals hydrophobicity. Traditionally hydrophobicity has been characterised by the logarithm of the octanol/water partition coefficient (log P). Current experimental and predictive methods are limited in terms of applicability for compounds with extreme log P values, compounds ionised under the conditions of analysis and surface active agents. An alternative technique to assess hydrophobicity is Immobilised Artificial Membrane High Performance Liquid Chromatography (IAM-HPLC). The IAM stationary phase was developed initially to mimic biological membranes more realistically than octanol/water partitioning. This study has collated published literature values for the IAM retention index (kIAM), including details of the experimental procedure, into a database. The database includes 1910 values for 647 compounds. The effect of variability of experimental procedure on reported values was investigated. Key experimental parameters were identified that ensure comparable log kIAM values. An IAM-HPLC method was optimised; the HPLC method covers a range of hydrophobicities (log P of -1.35 to 6.03) and includes both unionised and ionised compounds under the conditions of analysis. Additionally the method has been demonstrated to be robust across system of analysis, column and stationary phase batch. The assessment of robustness increases confidence in the log kIAM (pH 7.4) values for 66 aliphatic and aromatic compounds determined as part of this work. Methods to predict log klAM (pH 7.4) were investigated. Both a fragment and correction factor method, based on theoretical structural features, and a 'classical' descriptor based QSAR approach, was applied to both the experimental log kIAM (pH 7.4) values determined in this work and comparable values collated from the literature. QSARs have been developed using log klAM as a descriptor to predict the ability of a chemical to cross the skin barrier and to predict various acute aquatic toxicity endpoints, using published skin absorption and ecotoxicity data respectively.

▼ Introduction: Malignant Melanoma is the most lethal of the skin cancers and the UK incidence is rising faster than that of any other cancer. Breslow thickness remains the best predictor of metastasis and Sentinel Lymph Node Biopsy is the only method of detecting nodal spread in clinically node negative patients. Surgery is the only effective therapy. Angiogenesis – the growth of new vessels from pre-existing vasculature - is an absolute requirement for tumour survival and progression beyond a few hundred microns in diameter. Anti- angiogenic isoforms of VEGF have been demonstrated previously to be protective with regard to metastasis. The aims of this thesis were to determine whether VEGF expression within the tumour may allow prediction of the nodal status. Furthermore another aim was to determine whether via the “Seed and Soil” theory, by examination of angiogenic and lymphangiogenic profiles of the tumour and node can we determine that the tumour may control the microenvironment around the Sentinel Node? Finally, as a cohort of false negative patients emerged with a higher mortality rate than their true negative and true positive patient cohort counterparts, could any further patterns be established by performing the same experiments on these patients? Methods: Archived human tumour and corresponding Sentinel Node samples were used and immunohistochemistry was used to investigate the role of pro and anti angiogenic isoforms of VEGF, VEGF-C, LYVE-1 and CD31 within these patients. Results: VEGF-C expression was significantly increased in the intranodal component of positive Sentinel Lymph Nodes (p < 0.01 Bonferroni). This increased expression appeared to be independent of tumoural influences and no strong evidence for the “Seed and Soil” theory was proved. A significantly higher number of lymphatic vessel counts were identified within node negative patients (p < 0.05 ANOVA). No further significant findings were defined on examination of the false negative cohort of patients. Conclusions: This study has shown that positive Sentinel Lymph Nodes exhibit high levels of intranodal VEGF-C. This expression does not appear to be related to tumoural influences. It would therefore appear that VEGF-C expression within Sentinel Nodes warrants further investigation and may aid diagnosis of spread or represent a target to slow or even prevent the onset of metastasis.

Results indicate TG2 overexpression increases the rate of keratinocytes migration via an as yet unidentified soluble factor. Conversely, the presence of fibroblasts overexpressing this TG have demonstrated an inhibitory effect on keratinocyte migration.

▼ Demand of using public transit has been tremendously increasing during the past decade. As the public transit system become more and more complicated both in volume and dimension, the need for trip planning assistance, especially trips involving transfers, is emerging. Traditional shortest path algorithms are not very suitable for trip planning tasks involving transfers. A new network representation is introduced inspired by the traditional connectivity network, only with multiple layers. Each layer represents one route in the network. An algorithm is designed and implemented to accommodate the network representation with relatively high computational efficiency. A series of matrices are introduced to store the information of least transfer paths between each pair of nodes in the network. A NYC subway case study is conducted to validate the algorithm and examine the efficiency of the algorithm. Potential application of the least transfer path matrices in planning supporting is discussed.

▼ Investigations into the function of mesenchymal-TG2 in epidermal formation and keratinocytes migration were established with varying success using coculture systems. Results indicate TG2 overexpression increases the rate of keratinocytes migration via an as yet unidentified soluble factor. Conversely, the presence of fibroblasts overexpressing this TG have demonstrated an inhibitory effect on keratinocyte migration.

▼ Microdialysis is a method of monitoring biochemistry of tissues in-vivo, and is established as a clinical and research tool for monitoring of other tissue such as liver and brain. The aim of this thesis was to investigate the use of the microdialysis method in monitoring the human dermis in healthy and burned skin, with a view to its use as a tool to detect progression of the burn wound. Ten healthy volunteers were used to establish a normal range for glucose, lactate, pyruvate and glycerol levels, and to assess the effect of temperature on results. Ten patients with burns <15%TBSA were recruited and microdialysis was undertaken in zones of coagulation and stasis and in unburned skin in each patient from recruitment until36 hours post-bum. A further 2 patients with burns >30%TBSA were also studied. Biochemical findings and the result of amino acid assays are reported. An attempt at correlating biochemical findings with Laser Doppler Images was made, to establish the role of the technique in predicting wound progression. However no wounds showed any evidence of progression in the areas of probe placement over the time of the study.

▼ Opinnäytteeni tavoitteena oli luoda Tampereen Ammattikorkeakoulun uuden pelilaboratorion, eli Pelistudion, tulevaisuuden suunnittelu peli-sv:n opettajien ja opiskelijoiden toiveiden mukaan. Pelilaboratorion siirryttyä vuoden 2014 helmikuussa neljännen kerroksen neuvottelutilasta kolmannen kerroksen suurempaan luokkaan, saatiin enemmän tilaa tietokoneille, pelikoneille ja neuvotteluun. Pelistudion on tarkoitus avata ovensa vuoden 2014 syyslukukauden alkaessa.
Opinnäytteeni oli mielipidetutkimuksen lisäksi tilasuunnittelua. Oli pohdittava tarkkaan, mitkä opettajien ja opiskelijoiden toiveista voitiin toteuttaa, sekä se, mitä pitää toteuttaa. Huomioitavana oli Pelistudion kyky toteuttaa sen tarkoitus peli-sv:n opiskelijoiden työpisteenä, sekä taata paikan mukavuus niin ergonomisesti kuin sisustukseltaan.
Tilojen laatu oli vaihtelevaa. Pelikoneita oli tarpeeksi, mutta tietokoneita olisi voinut olla enemmän, ja tarpeetonta roskaa oli siellä täällä. Tietokoneissa ei myöskään ollut kaikkia tarvittavia ohjelmia, jättäen niiden käyttökyvyn kyseenalaiseksi. Motion Capture -tila, jossa oli tarkoitus tehdä liikkeitä peleihin, oli vasta alkutekijöissä
Pelistudion mielipidetutkimukset järjestettiin aluksi vain opettajille ja opiskelijoille, mutta myöhemmin lisättiin mukaan pari muiden koulujen pelilaboratoriota. Peli-sv:n opiskelijoille lähetettiin mielipidekysymys Pelistudiosta. Seitsemän opiskelijaa vastasi kysymykseen, mutta he olivat kaikki käyneet Pelistudiossa aikaisemmin. Opettajien kanssa järjestettiin haastattelu, josta saatiin yksityiskohtaisemmin tietoa Pelistudion kehityksestä ja jo suunnitellusta tulevaisuudesta. Muiden pelilaboratorioiden ylläpitäjille lähetettiin kysely tiloista, työvälineistöstä ja toiminnasta.
Tuloksista ja havainnoinneista tultiin siihen tulokseen, että vaikka Pelistudiossa oli jo paljon työvälineistöä, niin lisää sekä tarvittiin että toivottiin. Tietokoneita ja huonekaluja hankittaisiin lisää, roskat siivottaisiin pois ja luokka asennettaisiin WPK-verkkoon, jotta ohjelmia voitaisiin asentaa enemmän.; The purpose of my thesis was to create a plan for the future of the gaming laboratory of Tampere University of Applied Sciences, also known as the Gaming studio, according to the wishes of the Game Production programme students. After the gaming lab transferred from the meeting room in the fourth floor to the larger classroom in the third floor in February 2014, more space was available for computers, game consoles and conferencing. Gaming studio is supposed to open its doors at the start of the 2014 autumn term.
In addition to an opinion poll, my thesis was about space planning. Careful consideration was needed on the matters of what wishes of the teachers and students could be realized and what else needed to be actualized. Gaming studio’s ability to fulfill its purpose as the workstation for game production programme students and to ensure its convenience both ergonomically and interiorwise, had to be considered.

▼ Despite many studies on the action of yellow light in acne, it is still unclear whether it improves inflammatory acne or has photoimmunologic activity against pro-inflammatory pathways like toll-like receptor 2 (TLR2) and its down-stream cytokines e.g. TNF-α. This work sought to determine whether 530 nm IPL could cause a clinical improvement in acne and if its photo-mechanism of action involved modifications of the expressions of TLR2, TNF-α, IL-10 and IL-8. Twenty-eight adult patients with mild to moderate acne vulgaris involving their backs received four 530-950 nm IPL treatments at two-week intervals. Assessments performed at baseline and one week after the final IPL session included inflamed and non-inflamed lesion counts, Leeds assessments and sebum excretion rate (SER). Biopsies within the treatment area were taken at these two time points and two days after the first irradiation. TLR2 expression was examined by immunohistochemistry and TaqMan® Low Density Arrays were used to measure changes in expression of TNF-α, TNFR, IL-8 and IL-10. The data from 21 patients was included in the final clinical analyses. Inflamed lesions fell significantly by 28.0% (p = 0.002), but was not associated with significant changes in the Leeds score, SER or non-inflamed lesions. TNF-α expression fell by 17.6% (p = 0.031) at the end of therapy, and appeared to correlate with the percentage change in lesion counts in the subjects evaluated. TLR2 expression fell by 2.6% (p < 0.001) a week after the final irradiation, but bore no relationship to lesion counts. Neither IL-10 nor IL-8 was significantly affected. Though 530nm IPL significantly reduces inflammatory lesions, treatment efficacy will have to be improved to make it a viable treatment option. Its mechanism seems to include an anti-TNF-α effect, independent of IL-10 up-regulation. This is a novel mechanism, not been previously described for 530nm IPL. Updated hypotheses are suggested in order to explore this phenomenon further.

Lichen Simplex Chronicus (LSH) is a chronic pruritic disorder charactarized by scratching and rubbing of the skin. The initial stimulus of pruritus is rarely identified. Psychogenic factors play a significant role. As in classical pruritic psycho-dermatosis, once the itch –scratch –lichenification cycle is established, it is difficult to break. The lichenified skin is more sensitive to different triggers than healthy skin. Clinical characteristics are well-circumscribed plaque located on the back and side areas of the neck, on extensor surfaces of the body and in the anogenital areas. The disease affects both sexes, with a slightly higher percentage of females. Most of the patients are middle and over 55 years of age. It is assumed that this is a disease of adulthood, which is one of the likely reasons for the increase in prevalence over the years.
The main symptom in LSC is pruritus, which may be episodic, paroxysmal, or continuous. The clinical classification of pruritus most often is based on subjective evaluations. The importance of the clinical differentiation of itch is emphasized in modern guidelines adopted by the International Forum for the Study of itching. It is assumed that the classification system of the types of pruritus should be the basis for diagnosis and subsequent treatment. The understanding of the underlying mechanisms leading to this symptom as well as the subsequent treatment are challenging for physicians. Although in a large percentage of cases the itching in LSC is pruritoseptive, etiopathogenetic components responsible for the occurrence of the disease account to other types of itching as well.
Disturbances in the functional status of autonomic nervous system (ANS) play an important role in the development of LSC. Attesting for this is: the existence of a stable white dermatographic urticaria, decreased permeability of the vessels in the skin, impaired thermoregulation reflexes. Furthermore, it has been postulated that the clinical course depends on an impaired integrity and functionality of the skin barrier. It is debatable whether the infringement of the skin is the cause or a consequence of the disease. A factor that is very commonly identified as a causative agent is stress. Patients with LSC have anxiety and depression levels higher than normal.
In this present study the data from scientific publications associated with LSC have been summarized.
This is the first study in Bulgaria which aims to update the epidemiology of LSC. To assess the tendencies in distribution of LSC, data collected from the registers of St. Anna Medical Centre – Varna, January 2007 - July 2015, were analysed. Data include reports of 39 968 outpatients with different skin problems. The model of the study was retrospective and investigated the total morbidity, gender distribution, age distribution, peak condition and place of the disease in the structure of selected chronic recurrent dermatoses with negative impact on the quality of life.

Seborrheic dermatitis (SD) is a common disease in dermatological practice with a multifactorial but without clearly defined pathogenesis and complex nosology. Our study covered a total of 170 patients with SSD at a mean age of 41,46±14,54 years (range, 3 months to 80 years). It dealt with 109 males at a mean age of 42,85±14,67 years (range, 3 months to 80 years ) and with 61 females at a mean age of 38,98±14,44 years (range, 17 and 75 years). We used a constellation of several diagnostic methods such as clinical examination; trichoscopy, microbiology of cultures of yeasts of Malassezia genus after a ‘tape method’ modified by us; examination by using of Hair Pull test (HРt); examination for human immunodeficiency virus-(HIV)-infection; analysis of the Dermatology Quality of Life Index, DLQI), and, for the first time in Bulgaria - analysis of the Seborrheic Dermatitis Area and Severity Index, SDASI).
We analysed the specific symptoms of scalp seborrhoeic dermatitis and their distribution in the following locations: the two parietal areas, the frontal and occipital area, before and after treatment with 1% Ciclopirox olamine and 1.5% Climbazole shampoo, which was effective, safe and with very rare side effects. We identified a number of significant patterns between the different indicators, most of which were statistically credible. We established the diagnostic significance skin samples culture examination, the HPt and SDASI tests. By means of DLQI we analyses the impact of the disease on quality of life. Comorbidity of atopic dermatitis, contact dermatitis, arterial hypertension and serologically positive HIV infection should be considered in patients with scalp seborrhoeic dermatitis. We elaborated a differential-diagnostic trichoscopic algorithm and a therapeutic algorithm in order to differentiate the severity of scalp seborrhoeic dermatitis.

Себорейният дерматит (СД) е често срещано болестно състояние в дерматологичната практика с многофакторна, но не ясно дефинирана патогенеза и комплексна нозология. Ние изследвахме общо 170 болни със СД на капилициума (СДК) на средна възраст от 41,46±14,54 г. Касае се за 109 мъже на средна възраст от 42,85±14,67 г. и за 61 жени на средна възраст от 38,98±14,44 г. Използвахме констелация от няколко диагностични методи: клинично наблюдение; трихоскопия; микробиология на култури от дрожди от рода Malassezia по модифициран от нас „отпечатъков“ метод; изследване с помощта на Hair Pull test (HРt); изследване за HIV-инфекция; анализ на дерматологичния индекс за качeство на живот (DLQI) и анализ на индекса за засегнатата зона и тежеста на себорейния дерматит (SDASI). Анализирахме специфичните симптоми на СДК и разпространението им в следните области на капилициума: двете париетални области, фронталната и окципиталната област преди и след проведено лечение с шампоан с 1%-ен Ciclopirox olаmine и с шампоан с 1,5%-ен Climbazole, което беше ефективно, безопасно и с много редки странични ефекти. The complex role of vitamin D in the immunopathogenesis of psoriatic disease is genetically determined. In cell culture studies of keratinocytes, an association is described between a genotype risk factor for the development of psoriasis and the suppressed VDR receptor activity regulating epidermal proliferation and differentiation.
The association between vitamin D status and the serum level of the human cathelicidin (LL-37) and its presumed role in the initiation of the inflammatory cascade, leading to formation of psoriatic plaques are not well established and proven. Published studies confirm the complex effects of the vitamin D on skin and pathogenic role in psoriasis. Kerato-regulation, immune modulation, suppression of angiogenesis effects of active form of vitamin D could be linked to its genetically-determined metabolic action in glucose and lipid’s homeostasis that have a contribution to the comorbidities in the psoriatic disease.
Previously in Bulgaria there are no studies of serum level of vitamin D in patients with psoriasis. The aim of this thesis is to study and analyse the association between vitamin D status and the clinical and laboratory manifestations of Psoriasis Vulgaris in patients with moderate to severe disease activity. The reported study is a first attempt of evaluation of the pathogenic link between vitamin D and human cathelicidin LL-37 in psoriasis.

Комплексната роля на вит.Д в имунопатогенезата на псориатичната болест е генетично обусловена. В проучвания върху клетъчни култури от кератиноцити, е описана асоциация, между рисков за развитието на псориазис генотип и подтисната рецепторна активност на VDR, които регулират епидермалната пролиферация и диференциация. Активирането на VDR, води до синтез на антимикробни пептиди, в това число и на кателицидин LL-37. Синтезираният кателицидин LL-37 се свързва с автохтонни ДНК и РНК от апоптотични кератиноцити, активира TLR на АРСs и участва в инициирането на инфламаторната каскада, водеща до образуването на псориатични плаки. Проведеното проучване, за първи в България установява вит.Д статуса на пациенти с псориазис вулгарис, както и анализира връзката му със серумното ниво на антимикробния петид, кателицидин LL-37.

This thesis examines some clinical and epidemiological aspects of seborrheic dermatitis and the importance of vitamin D for its development. The aim of the study is to investigate some pathogenic factors, skin comorbidity and serum vitamin D levels in patients with seborrheic dermatitis. The effect of local cholecalciferol and vitamin D supplementation on the course of seborrheic dermatitis has been considered as well. Study of the vitamin D status showed high prevalence of vitamin D deficiency and insufficiency in patients with seborrheic dermatitis. There is no statistically significant correlation of serum 25 (OH) vitamin D and age, sex, season, body mass index and phototype of the studied individuals. On the other hand there was correlation between the level of serum vitamin D and severity of the disease and number of exacerbations per year. Nevertheless the duration of the skin disease could not be predictor for the level of 25 (OH) vitamin D of the patients. It may be presumed that low serum vitamin D may be one of the reasons for more severe and more frequent exacerbations of seborrheic dermatitis.
The established frequent vitamin D insufficiency in patients with seborrheic dermatitis and the correlation of the level of 25-ydroxy vitamin D and disease severity gave reason for examination of the therapeutic efficacy of vitamin D. According to the present study local applications of cholecalciferol solution in dose 1200 IU daily led to statistically significant improvement of clinical findings of facial seborrheic dermatitis in 92,7 % of investigated patients. Limitation of the study is, that it is not blinded and not placebo-controlled. Nevertheless the results give grounds for further studies of therapeutic efficacy of local cholecalciferol as save alternative of the other present local applications used in treatment of seborrheic dermatitis like corticosteroids, calcineurin-inhibitors, tars, etc.

Дисертационният труд разглежда някои клинико-епидемиологични аспекти на себорейния дерматит и значението на витамин Д за протичането му. Целта на изследването е да се проучи кожната коморбидност , серумно ниво на витамин Д при пациенти със себореен дерматит и влиянието на някои индивидуални фактори върху клиничния ход на заболяването. Проучен е и ефекта на локaлно приложен холекалциферол и субституиране на витамин Д дефицит при пациенти със себореен дерматит. Установява се висока честота на витамин Д дефицит при изследваните пациенти. Проучени са взаимовръзките между нивото на серумен 25-хидроксивитамин Д и възрастта, пола, сезона на изследване, индекса на телесна маса и фототипа на проучените субекти. Low level laser treatment (LLLT) has found a wide application amongst the arsenal of methods for dermatological therapy in the last 20 years. The biological effect of LLLT consists of influencing the regulatory processes in the organism on the subcellular and cellular level and maintaining homeostasis, which determines the efficiency and safety of this approach.
This gave us reason to research the therapeutic abilities of laser treatment for acne and rosacea as a method on its own, as well as in combination with local therapy.
Our aim is to research the therapeutic effect of low level laser treatment on patients with acne and rosacea, and to prepare systematic instructions for its use against these conditions. In order to achieve this goal, we assigned ourselves the following tasks:
Determine the efficiency of laser therapy in the treatment of acne and rosacea and describe the side effects, which occur during the treatment.
Determine low level laser therapy's place as a complement to various conventional methods of treatment.
Prove the advantages of laser therapy, based on the results from the clinical research performed.
The good results achieved in the examined groups prove the necessity of administering low level laser therapy.
Low level laser therapy is suitable for treating acne and rosacea both in combination and on its own.

Нискоинтензивната лазерна терапия (LLLT) в последните 20 години намери широко място в арсенала от средства на дерматологичната терапия. Биологичното въздействие на LLLT се състои в повлияване на регулаторните процеси в организма на субклетъчно и клетъчно ниво и поддържане на хомеостазата, което обуславя ефективността и безопасността на този метод.
Това ни даде основание да проучим терапевтичните възможности на лазерната терапия при акне и розацея като самостоятелен метод, а също и в комбинация с локални средства
Целта ни е да се проучи терапевтичният ефект на ниско интензивната лазер терапия при пациенти с акне и розацея и да се изготвят методични указания за приложението и при тези заболявания. За постигане на тази цел си поставихме следните задачи:
Да се определи ефективността на лазерната терапия при лечение на акне и розацея и да се опишат забелязаните странични ефекти по-време на лечението.
Да се определи мястото на нискоинтензивната лазерна терапия, като допълващо средство при различни конвенционални методи на лечение.
Постигнатите добри резултати при изследваните групи пациенти доказват необходимостта от прилагането на нискоенергийна лазертерапия.
Нискоинтензивната лазерна терапия е подходяща, както за комбинирана, така и за самостоятелна терапия на акне и розацея.

Therefore the purpose of this project was to establish a transgenic mouse model able to investigate the molecular mechanism(s) of melanoma aetiology mediated by N-Ras and PTEN genes. To achieve this, an inducible gene switch approach was employed exploiting the Cre/loxP recombinase system. This approach has the advantage of avoiding embryonic lethality and helps to minimise disease(s) in other tissue(s) that may interfere with animal viability. It was envisaged that this inducible system would establish a model that accurately mimic the development of melanoma in humans. A disadvantage being that the tyrosinase-based promoter was only responsive to the inducer when melanocytes were proliferating.
Initially, regulator vectors were created by sub-cloning Cre under the control of a melanocyte-specific promoter either enhanced tyrosinase (EICre) or tyrosinase related protein 2 (Trp2Cre). A Cre responsive, target N-Raslys61 transgene was also cloned, where expression was induced by Cre ablation of ‘Stop' cassette (cmv.stop.N-Raslys61) together with a report target transgene (cmv.stop.EGFP) to aid in expression characterisation.
The functional activity and gene-switch specificity of these constructs were subsequently confirmed employing co-transfection of regulator and report target into B16 melanoma cells, but to confirm their activity in primary melanocytes, melanocyte culture conditions had to be defined for optimum growth and transfection as there is no optimized commercial medium available for murine melanocyte culture unlike for human melanocyte. In this study therefore, murine primary melanocyte culture method (50/50) has been defined, which exploited keratinocytes for initial melanocytes growth support as a feed layer. The other advantages of this 50/50 medium were that pigmented cells grew without spontaneous transformation and gave the higher transfection efficiency compared to media exploited by other groups. Using primary melanocytes cultured in 50/50 medium, transgene construction and identification of regulator expression were performed by RT-PCR in vitro prior to in vivo analysis thereby avoiding unnecessary breeding.
When concerns had arisen regarding an unexpected lack of melanoma phenotype in vivo particularly in addition of PTEN loss, this culture protocol supplied a successful test of oncogenic potential of N-Raslys61 and PTEN loss in vitro, where N-Raslys61 expression transformed melanocytes and PTEN loss promoted N-Raslys61 to give more aggressive cells. However a functional redundancy was identified, as transformed colonies were not immortalized and eventually senesced, possibly due to their opposing gene functions being on the same signalling pathway; i.e. PTEN fails to provide additional genetic aberrant pathway(s) for the cross-talk with Ras signalling necessary to form malignant tumours.

▼ The notch signalling pathway is essential for the development and growth of all mammalian cells. The canonical pathway works by cell to cell communication and influences gene expression directly. Notch signalling plays an important role in cell differentiation processes in the embryo as well as in adult tissues. In Drosophila, signalling requires a single notch receptor and two ligands (delta and serrate). However, in mammals signalling is more complex and there are four notch receptors (NOTCH1-4), three delta like ligands (DLL1, DLL3, DLL4) and two jagged ligands (JAG1, JAG2). The interaction between a notch receptor at the cell surface and its ligand on a neighbouring cell, leads to sequential proteolytic cleavages which release the notch intracellular domain (NICD) from the cell surface receptor (Estrach et al., 2008). The NICD enters the nucleus and forms a complex with CSL and Mastermind to activate target genes (HES and HEY family members). Notch receptors have modified EGF-like repeats where fucose has been added to a serine or threonine residue and this O-linked fucose can be elongated by the action of an enzyme (fringe), enhancing or blocking ligand binding. There have been extensive studies on notch signalling in embryos but less is known in adult tissues, especially with regard to non-canonical pathways.
The aim of this project was to examine the expression of notch receptors, ligands and specific target genes during epidermal differentiation in human skin. This was examined both in human epidermis (in vivo) as well as in culture models using HaCaT, a keratinocyte cell line. Examining notch signalling activity at different stages of keratinocyte differentiation could help to understand the precise role of notch signalling in cell fate determination during normal cutaneous epithelial cell growth and development, and may indicate what role, if any notch signalling might play in the pathology of skin disease.
Most of the work on delta ligands has been done in embryonic tissues with little work on adult skin. Thus, our approach focused on expression of DLL1, DLL3 and DLL4 in human epidermis and cultured HaCaT cells. DLL1 expression was confirmed (mRNA and protein level including sequence) but no convincing evidence of DLL3 or DLL4 expression was found. Evidence was also obtained for JAG1 and JAG2 expression in human epidermis and cultured cells, and there was some indication that differential expression might occur during terminal differentiation.
RT-PCR results indicated possible changes in notch ligand levels with calcium-induced differentiation in HaCaT cells and these were quantitated by real time PCR (qPCR). Expression levels could not be normalised to a housekeeping gene (β-Actin, human ARP, human TF2H, GRP58, B2H, RPL13 and TBP) as all those tested were not stable enough. ▼ The mechanism of action of IPL in acne treatment is not clearly understood, but an immunomodulatory role has been suggested. Furthermore, inflammatory cytokines and matrix degrading enzymes play a key role in acne pathogenesis. Therefore, curbing the production of these mediators may assist acne resolution. In photorejuvenation studies, IPL has been shown to induce the expression of a key immunomodulatory cytokine, TGF-β. Interestingly, TGF-β has been demonstrated to mediate immunosuppression, inhibition of keratinocyte proliferation and MMP-1 repression through a Smad3-mediated signalling pathway. Therefore, we sought to investigate the in vivo effects of IPL used for acne treatment. Biopsies obtained from 20 patients with inflammatory acne vulgaris at baseline and post-IPL treatment (48 hrs after the first treatment and 1 week after the final treatment) were immunohistochemically analysed to investigate the expression of TGF-β1, TGF-β2, TGF-β3, Smad3, MMP-1 and IL-8. Digital images were semi-qualitatively assessed using image analysis software. In addition, quantitative PCR analysis of TGF-β1, Smad3 and IL-8 was performed on biopsies from seven cases. Immunohistochemical analysis demonstrated that IPL elicited a statistically significant increase in epidermal TGF-β1 expression. However, no statistically significant difference was observed in the expression of TGF-β2/β3. Increased nuclear immunolocalisation of Smad3 was demonstrated in the post-IPL biopsies, which was statistically significant. Although not statistically significant, both IL-8 and MMP-1 expression showed a downward trend in the majority of cases. No statistically significant change was detected in the gene expression of TGF-β1, Smad3 and IL-8, which may be attributed to the small sample in which PCR was carried out. The data from this study suggests that Smad3-mediated TGF-β1 signalling may play a role in IPL-induced resolution of acne vulgaris. The therapeutic effect of TGF-β1 in inflammatory acne vulgaris could be attributed to its immunosuppressive effect and its ability to inhibit matrix degradation and keratinocyte proliferation.

▼ Thyroid dysfunction is frequently associated with skin and hair diseases; however, the underlying pathogenic mechanisms are poorly understood. Pathological activation of the thyroid stimulating hormone receptor (TSHR) is the key feature of both hyper- and hypo-thyrodism. Expression of the (TSHR) has been reported in several extra-thyroidal locations including adipose tissue, bone and skin fibroblasts. TSHR expression may explain the association between the thyroid and skin disease. The TSHR can also be activated by a newly discovered glycoprotein hormone, known as thyrostimulin. This hormone is composed of a dimer of unique α 2 and β 5 subunits. Although thyrostimulin has not been detected in the circulation. However, both subunits have been shown to be expressed in different tissues including the skin. The aim of this study is to examine the expression of the TSHR and thyrostimulin in the skin. In addition, to investigate the expression of a variant form of the TSHR in human and mouse skin and, other mouse tissues. RT-PCR using primers specific for the full length receptor and the truncated variant revealed that although the variant was widely expressed in mouse tissues including skin, it was not found in human skin. The full length receptor and thyrostimulin were found to be co-expressed in eye, testis and skin. Immunohistochemistry of frozen skin and thyroid sections using commercially available antibodies against the extracellular (A9) and transmembrane domains (A7) of TSHR demonstrates that TSHR is not expressed in the epidermis but expressed in dermal fibroblasts and in myoepithelium around sweat glands. A new β5 antibody was characterised by western blotting and immunohistochemistry for future investigation of β5 expression in the skin. These data suggest a functional role for TSHR signalling possibly via thyrostimulin in the skin and that the variant form,although potentially present in some tissues, is unlikely to be important in human skin.

▼ This thesis sets out to demonstrate the significant contribution to the field of podiatric dermatology, through the use of various forms of published documentary evidence, made by the author. In addition to the published papers submitted, a content analysis of British podiatric literature over a period of 21 years has mapped the emergence and development of the specialism of podiatric dermatology within the United Kingdom. This work demonstrates a significant increase in professional interest within this area during this period. This is evidenced through increased reporting of dermatological topics within podiatry journals in terms of related news items, advertisements and editorials. This is accompanied by an increasing number of case studies, peerreviewed papers and continuing professional development articles evident within the literature. The author has presented within this thesis a suite of fifty published articles along with verifiable evidence of professional activities related to the promotion and development of podiatric dermatology. Collectively this evidence represents a significant contribution to the development and evolution of dermatology as a specialist area within podiatry in the United Kingdom over the last fifteen years.

▼ In this Master’s thesis, joint sequential inference of both parameters and states of stochastic volatility models is carried out using the SMC2 algorithm found in <em>SMC2: an efficient algorithm for sequential analysis of state-space models</em>, Nicolas Chopin, Pierre E. Jacob, Omiros Papaspiliopoulos. The models under study are the continuous time s.v. models (i) Heston, (ii) Bates, and (iii) SVCJ, where inference is based on options prices. It is found that the SMC2 performs well for the simpler models (i) and (ii), wheras filtering in (iii) performs worse. Furthermore, it is found that the FFT option price evaluation is the most computationally demanding step, and it is suggested to explore other avenues of computation, such as GPGPU-based computing.
I denna Masteruppsats estimeras sekventiellt parametrar och tillstånd i stokastiska volatilitetsmodeller nyttjandes SMC2 -algoritmen som återfinns i [1]. Modellerna som studeras är de kontinuerliga s.v.-modellerna (i) Heston, (ii) Bates och (iii) SVCJ, där inferens baseras på optionspriser. Vi finner att SMC2 presterar bra resultat för de enklare modellerna (i) och (ii) emedan filtrering för (iii) presterar sämre. ▼ The aim of this study was to assess the response of preosteoblasts (
SV-HFO), from adhesion to matrix mineralization, on Ti6Al7Nb surfaces modified by plasma electrolytic oxidation (PEO). Two different surfaces have been generated by changing the oxidation duration from 1 to 5 minutes. The resultant (PEO1 and PEO5) surfaces showed uniform porous topographies with pores mostly in the submicron range and a mixture of anatase and rutile TiO2 phases. However, the average surface roughness, maximum peak-to-valley height, pore size and Ca/P ratio increased with oxidation time whereas pore density and surface porosity decreased. On the PEO1 surface
SV-HFO cells attached and spread easily using the pores as anchoring sites for their extensions and showing cell-cell contact after 48 hours.
The larger pores protruding from the PEO5 surface suppressed cell adhesion. Deposition of extracellular matrix started earlier on the PEO1 surface and after 21 incubation days a net-like structure well integrated with the porous surface was visible. Matrix mineralization was evidenced on both surfaces after 21 days. However, more uniform mineralized areas were observed on the PEO1 surface whereas an accelerated mineralization was noticed after 14 days on the PEO5 surface. In conclusion, by varying only one PEO process condition, significant changes occurred on the surface of Ti6Al7Nb alloy, which influenced both the early and late response of
SV-HFO cells. The observed surface-induced effects indicated that surfaces produced at shorter oxidation time may be more beneficial for early osteogenesis.

▼ The human epidermis is a self-renewing stratified epithelial tissue that forms the outermost protective layer of the skin. The epidermis is comprised of a number of cell types, the most abundant of which are keratinocytes. Normal function of the epidermis requires that keratinocyte proliferation, differentiation, and apoptosis be precisely regulated and a failure to regulate these processes is a feature of many skin diseases. Although the precise mechanism by which epidermal homeostasis is regulated is still far from clear, much progress has been made in the characterisation of signaling pathways involved in normal epidermal function. A key group of signaling proteins that have been clearly implicated in epidermal function are the Rho family of small GTP-binding proteins. This thesis focuses on one member of the family, RhoE/Rnd3, and the analysis of the role it plays in the regulation of proliferation, differentiation, apoptosis and cell-cell adhesion in the epidermis. Use of RNA interference to specifically ‘knock-down’ expression of RhoE has led to the discovery of a novel role for RhoE in regulation of cell-cell adhesion and apoptosis. Loss of RhoE expression resulted in keratinocytes developing resistance to apoptosis mediated via either the intrinsic or extrinsic pathways. RhoE depletion was also associated with increased expression of desmosomal proteins and increased numbers of desmosomes. Resistance to apoptosis was shown to be a function of desmosome-mediated cell-cell adhesion and a component of demosomes – plakoglobin – was shown to play a key role in RhoE-mediated resistance to apoptosis.

▼ Computational studies on light‐tissue interactions in medical treatment and diagnosis have offered deeper insights in the processes underlying laser treatments and fluorescence measurements. I apply this approach in the study of fluorescence detection and of laser therapy. First, I investigate three methods of fluorescence detection and the reported contrast between healthy skin and malignant tissue. I varied the concentration of haemoglobin in the target, the concentration of melanin in the epidermis, the scattering of light in the skin, the depth at which the target is located in the skin, the width of the target, the thickness of the target, the concentration of photosensitizer in the target, and the concentration of photosensitizer in the skin. My findings confirm previous clinical studies in that the auto‐fluorescence corrected fluorescence detection method generally shows a higher contrast than the other methods. The results support earlier clinical studies and are in accordance with expert experience. Second, I study laser therapy for psoriasis. In a series of simulations, I analyse three types of pulsed dye laser systems and one IPL system. The investigated biological effects are heat shock proteins, hyperthermic tissue damage and vasoconstriction of the microvasculature. The changes in the skin concern blood volume, blood oxygenation and scattering in the epidermis. The calculations show that there are some notable differences in the effect changes in the composition of psoriatic tissue has on the efficacy of laser and IPL therapy. Still, Inter‐device variance was more prominent than intra‐geometry variance. My study adds to the understanding of fluorescence detection of keratinocyte skin cancers, as well as that of laser therapy for psoriasis. Additionally, it offers potential avenues for increasing the efficacy and efficiency of these therapies.

Clinical epidemiological, histological and immunohistochemical research on skin pseudolymphoma is a pioneer study of its kind in Bulgaria. Skin pseudolymphoma research is based on a large clinical and histological volume of material; moreover it is one of the few pieces of research in the world literature in which isolated cases are described becoming then subject to reviews. The exogenic etiological factors (such as mites, insects, viruses, tattoos, antigenic effects, etc.) are proved to be the main cause of origin in this country unlike most European countries and the USA, where kin pseudolymphoma are predominantly caused by medication. Detailed histopathological and immunohistochemical studies of the skin pseudolymphoma have been performed opening room for their better discrimination from the malignant skin lymphomas.
New, pointed to practice, classification of the skin pseudolymphoma is introduced. Early diagnostics and the treatment of parasitosis with conceivable etiological importance is recommended, along with adequate photoprotection in the photosensibility cases. The high efficiency therapeutic agents are the potent local corticosteroids, including those applied under occlusion, as well as the intralesional applied corticosteroids. Instructions are provided for long term tracking of patients with skin pseudolymphomas in case of potential deterioration.

За първи път в България се правят подобни клинико- епидемиологични, хистологични и имунохистохимични проучвания на кожните псевдолимфоми. Проучването на псевдолимфомите на кожата е направено върху голям клиничен и хистологичен материал, което е едно от малкото проучвания в световната литература, където обикновено се описват единични случаи и се правят обзори върху тях. Доказва се, че за нашата страна екзогенните етиологични фактори (кърлежи, насекоми, вируси, татуировки, антигенни въздействия и др.) имат основно роля за възникването им, за разлика от повечето европейски страни и САЩ, където преобладават медикаментозно предизвиканите случаи. Направени са детайлни хистопатологични и имунохистохимични проучвания на кожните псевдолимфоми, които дават възможност за по-доброто им отдиференциране от злокачествените кожните лимфоми. Предлага се нова, с практическа насоченост, класификация на псевдолимфомите на кожата. Препоръчва се ранна диагноза и лечение на паразитозите с възможно етиологично значение, както и адекватна фотопротекция в случаите с фоточувствителност. Терапевтичните средства с най-висока ефективност са потентните локални кортикостероиди, включително и прилагани под оклузия, както и интралезионално приложените кортикостероиди. Дават се насоки за продължително контролно проследяване на пациентите с кожни псевдолимфоми в насока на евентуалната им малигнизация.
