▼ The aim of the present study was to determine if a nutrition and yoga intervention will improve quality of life (QoL) in breast cancer survivors (BCS). Using the Transactional Model of Stress and Coping as a guide to lead the intervention, the intervention assessed potential barriers, self-efficacy, diet quality, and physical activity as it relates to quality of life. Twenty-seven women were enrolled in the study and randomly assigned to the control or intervention group. The intervention consisted of 6-weeks of yoga classes and 6-weeks of online nutrition education. The control group received a nutrition consultation and nutrition guidelines from the American Cancer Society. Measurement of variables was conducted at baseline, post-intervention (6 weeks), and follow-up (12 weeks). One-way repeated measures ANOVA, paired samples t-test, and post hoc analysis with Bonferroni adjustment was used to analyze the data. Mediation analysis with regression was performed to demonstrate the effect the intervention had on quality of life. The intervention elicited a statistically significant difference in the Total Outcome Index quality of life score from baseline to post-intervention (P < .005) and from baseline to follow-up (P < .005) in the intervention group. The frequency of how often fruits and vegetables were consumed was significant between time points, (P < .05), but not between the control and intervention group, P = .538. The amount of fruit and vegetables consumed each time was statistically significant for the intervention group from baseline to post-intervention (P < .05) however there was no significant difference from baseline to follow-up (P = .067). There was no difference between the control and intervention group, (P = .216). There was a statistically significant difference for physical activity for time (P P = .166) however the intervention group has a statistically significant difference between baseline and post-intervention (PP = .082). We cannot confidently predict that participant’s quality of life scores are determined by group with the help of mediators after conducting a mediation analysis with regression. A six-week nutrition and yoga intervention in BCS elicited significant changes in QoL in BCS. Even though the results did not show significant changes between the control and intervention group there were significant changes within the intervention group from baseline to post-intervention and baseline to follow-up which may indicate a 6-week online nutrition education program coupled with a 6-week yoga intervention an effective tool to improve QoL in BCS. National and international guidelines recommend physical activity, consuming a healthy diet, and maintaining a healthy weight can improve the health of cancer survivors. Despite these recommendations, there is limited evidence on how to best encourage cancer survivors and carers to adopt these healthy lifestyle behaviours. Using theory to design interventions using well-established and effective behaviour change strategies offers a promising way to improve lifestyle behaviours. This thesis by publication presents a series of studies that were conducted to address this gap in the literature. The aims are: i) to systematically review and synthesise the evidence regarding social cognitive theory-based nutrition and physical activity interventions for cancer survivors and describe the development of a theoretically-based intervention (ENRICH: Exercise and Nutrition Routine Improving Cancer Health); and ii) to evaluate the short- and long-term efficacy of the intervention and examine potential mediators of physical activity behaviour change. Social cognitive theory-based lifestyle interventions can improve the physical activity and nutrition behaviours of cancer survivors, and this theory was used to underpin the development of a healthy lifestyle program. ENRICH program participants demonstrated improvements to pedometer-assessed physical activity, vegetable consumption, and weight in the short-term (20 weeks), with weight and physical activity improvements maintained at 12 months. Behavioural goal was the only social cognitive theory-based construct that was a mediator of intervention effect on pedometer-assessed physical activity, suggesting that the intervention may have exerted important effects on other theoretical constructs that were not assessed. This program of work provides evidence for the efficacy of a theoretically-based, multiple health behaviour lifestyle intervention for cancer survivors and carers in the short and long-term. Given the success of the ENRICH program, the Cancer Council NSW has subsequently adopted this program as a service offered to cancer survivors across New South Wales.

▼ The prevalence of overweight and obesity and other nutritionally related disease development among pediatric cancer survivors is well known to be above national norms. This is a retrospective cohort study designed to investigate the relationship between childhood cancer survivorship and nutritionally related disease development. The objective of this study was to gather data from existing pediatric cancer survivors in order to study disease development in relation to the type of cancer the patient had, the course of treatment, and to nutrition care that patient may have received. Seventy five subjects, with diagnoses of ALL, AML, Burkitt’s lymphoma, neuroblastoma, or Wilm’s tumor, 2 or more years into survivorship, and meeting the other stated criteria, were admitted to the study. Body mass index (BMI) was calculated for each subject using the most recent height and weight, and prevalence of overweight or obesity was established using the current CDC growth charts and guidelines. Of the 75 subjects in the study, 30 (40%) were found to be overweight or obese overall, with relatively equal distribution in those categories (16 overweight vs. 14 obese). When separating subjects into their respective diagnosis categories, the rate of overweight and obesity was higher than the overall number in all groups except the neuroblastoma group (ALL 42%, AML 57%, Burkitt’s 50%, Wilm’s 56% vs. Neuroblastoma 21%). Subject data was analyzed for development of nutritionally related diseases since diagnosis of cancer. Forty nine (65%) were found to have developed these types of diseases including overweight/obesity, osteoporosis, restrictive airway disease, hypothyroidism, hypertension, gallbladder disease, hypercholesterolemia, and anemia. When overweight and obesity were excluded, 20 subjects (27%) remained. Forty seven (63%) of the 75 total subjects in the study had some type of nutrition education, 45 (60%) received nutrition intervention, and 25 (33%) received nutrition support. In the overweight and obese group (30 subjects), 20 (67%) received nutrition education, 19 (63%) received nutrition intervention, and 8 (27%) received nutrition support. When the overweight and obese subject data was removed from the overall sample, 45 subjects remained and 27 (60%) were found to have received nutrition education, 26 (58%) had nutrition intervention, and 17 (38%) had nutrition support during or after treatment. The results of this study corroborate with the results of previous studies. The rates of overweight and obesity, along with other nutritionally related diseases, are high in this sample even though most subjects are 5 years post-therapy or less. The nutrition education and intervention provided to the overweight and obese group was slightly higher than those who were not in this category, possibly indicating that the timing and quality of nutrition care plays a role in prevention of disease.
