▼ The application of splints following surgical release of Dupuytren’s contracture has long been recommended for maintaining finger extension. Questions exist regarding the efficacy and potential detrimental effects of this treatment approach.
The literature pertaining to the effects of splints on finger range of motion and hand function following surgery was systematically reviewed. A search of electronic databases was conducted and a quality assessment was undertaken using the Downs and Black Quality Index. A comparison was made with findings from a 2008 systematic literature review. The results of the review suggested that the traditional practice of providing night extension splints was not effective and highlighted the need for further studies.
A single centre randomised trial was conducted to investigate the effect of night extension splinting on finger range of motion and hand function in the three months following surgical release of Dupuytren’s contracture. Fifty six patients (males n=45, females n=11) between the ages of 48 and 86 were included in the study. Participants were randomised to receive a night extension
splint plus hand
therapy (n= 26) or hand
therapy alone (n=30). The primary outcome was total active extension (TAE) of the little finger in degrees. Secondary outcomes were total active flexion (TAF) of the fingers in degrees, active distal palmar crease (ADPC) in cm, grip strength in kg, self-reported hand function using the Disabilities of the Arm shoulder and Hand (DASH) questionnaire (1-100 scale) and patient satisfaction. Primary analysis was by intention to treat.
There were no statistically significant differences between the no
splint and
splint groups for any of the outcomes measured. When compared with the
splint group the no
splint group showed adjusted mean differences of little finger TAE -9.8 (95%CI -20.19 to 0.59, p=0.07), little finger TAF 12.7 (95%CI -1.8 to 27.2, p= 0.08) and ADPC -0.21 (95%CI -0.74 to 0.32, p=0.44). Equally there were no statistical differences for DASH -1.1 (95%CI -5.41 to 3.21, p=0.59), left hand grip strength 2.6 (95%CI -1.52 to 6.72, p=0.22) or right hand grip strength 2.5 (95%CI -0.64 to 5.64, p=0.10). A secondary per protocol analysis was conducted which also showed no statistically significant differences. Although statistical significance was not reached there was a consistent trend across all outcomes in favour of not splinting.
The data were also evaluated to identify how well finger extension was maintained overall between the first postoperative measure and three months postoperatively. Of all 40 little fingers 62.5% had the same or better TAE (13⁰ unadjusted mean improvement) and 37.5% had lost TAE (32⁰ unadjusted mean loss) over this period.

therapy has no greater effect on maintaining finger extension than hand

▼ O objetivo deste estudo foi avaliar a eficácia da laserterapia de baixa intensidade associada ao uso da placa oclusal, como método de contribuição para o tratamento das DTMs intraarticulares e/ou musculares, por meio de exames clínicos e instrumentais. Foram selecionados 25 pacientes entre homens e mulheres, com idade média de 33,8 anos, encaminhados para tratamento da DTM na FORP/USP. Também foram selecionados 12 sujeitos assintomáticos para DTM, para compor o grupo controle. O diagnóstico da DTM foi realizado com o Research Diagnostic Criteria for Temporomadibular Disorders (RDC/TMD). Os pacientes foram divididos randomicamente em dois grupos: um grupo recebeu tratamento através de placa oclusal e o outro grupo, recebeu tratamento através de placa oclusal associada à laserterapia de baixa intensidade. As avaliações incluíram avaliação da oclusão funcional estática e dinâmica, medida dos movimentos mandibulares, dor à palpação e eletromiografia de superfície. Os pacientes também responderam um questionário de frequência e severidade dos sinais e sintomas de DTM (ProDTMMulti). A avaliação eletromiográfica foi realizada em várias atividades com e sem placa oclusal. As avaliações foram realizadas inicialmente (A1), antes do tratamento, e após a finalização do tratamento (A2). Os resultados mostraram uma significante melhora da dor relatada pelo pacientes e aumento da amplitude da abertura bucal em ambos os grupos, porém para o grupo que recebeu laser e placa, essa melhora foi mais acentuada. A atividade eletromiográfica foi reduzida durante a maioria dos testes realizados, porém não houve diferença estatística entre os grupos experimentais antes e após o tratamento, nem quando comparados com o grupo controle. Baseados nesses achados podese concluir que o tratamento com placa oclusal em associação à laserterapia promoveu melhora mais acentuada na sintomatologia dos pacientes e nos movimentos mandibulares em sujeitos com DTM.
The aim of this study was to evaluate the efficacy of low intensity laser therapy associated with the use of occlusal splint as a method of contribution to the treatment of intraarticular TMD and / or muscle, by clinical and instrumental exams. A total of 25 patients between men and women, mean age 33.8 years, were selected referred for treatment of TMD in FORP / USP. A control group of 12 subjects were also selected for asymptomatic DTM, to compose the control group. All subjects met the inclusion criteria. The diagnosis of TMD was conducted with the Research Diagnostic Criteria for Temporomandibular Disorders (RDC / TMD). ▼ Este trabalho teve por objetivo avaliar o efeito do tratamento com placa interoclusal estabilizadora como terapia única ou pós terapia miofuncional orofacial prévia em portadores de disfunção temporomandibular articular (Grupos II e III) associada ou não ao diagnóstico de DTM muscular (Grupo I) de acordo com Research Diagnostic Criteria for TMD (RDC/TMD), a fim de determinar os efeitos desta terapia para a redução dos sinais e sintomas de DTM, como também melhora no limiar de dor à pressão com algômetro, dor à palpação, redução da dor subjetiva pela VAS e alteração da atividade muscular avaliada através da eletromiografia. A amostra foi composta por 33 pacientes, do sexo feminino com idade entre 18 a 45 anos que já haviam sido submetidos, há menos de 12 meses, à Terapia Miofuncional Orofacial (TMO), ou não, e que apresentavam indicação para complementar o tratamento por meio do uso de placa. Os indivíduos foram divididos em dois grupos, Grupo T (n = 21), grupo que passou por tratamento prévio com TMO e Grupo P (n = 12), grupo que não se submeteu a terapias prévias. Ambos os grupos foram tratados com placa interoclusal estabilizadora com guias (protrusiva e canina) e acompanhamento por 120 dias. Todos os sujeitos foram avaliados antes da instalação do dispositivo interoclusal (A1) e após os 120 dias de uso (A2). Os resultados mostraram que: houve aumento significante nas medidas de movimentos excursivos quanto ao movimento de protrusão para o Grupo P, entretanto, para os outros movimentos os grupos se comportaram de maneira semelhante; houve redução significante em relação ao aspecto subjetivo da dor por meio da VAS nos dois grupos estudados, sem diferença entre eles; não se observou diferença entre os grupos em relação à alteração do limiar de dor com algômetro; para o ProDTMmulti; foi observado uma diminuição na presença de sinais e sintomas para a maioria dos parâmetros avaliados e um decréscimo na severidade de dor articular e dor muscular apenas para o grupo P e na sensibilidade dentária e plenitude apenas para o grupo T e ruídos articulares para os dois grupos. Não houve mudanças na atividade muscular avaliada por meio de eletromiografia de superfície.
This study aimed to evaluate the effect of the treatment with stabilization splint as a single therapy or after prior orofacial myofunctional therapy in TMJ patients (Groups II and III) with or without the diagnosis of myofascial pain (Group I) according to Research Diagnostic Criteria for TMD (RDC / TMD) in order to determine the effects of this therapy to reducing TMD signs and symptoms, as well as improvement in pain threshold algometer pressure, pain to palpation, reduction of subjective pain by VAS and changes in muscle activity assessed by electromyography (EMG). ▼ Upper extremity splinting is a popular treatment method for a range of conditions, such as rheumatoid arthritis. The intent of this treatment approach is multifaceted, but fundamentally, the provision of tools to enable and encourage patients to carry out everyday activities and to improve their quality of life is paramount. However, the aesthetic and functional limitations of wrist immobilisation splints demonstrate various weaknesses in terms of wear duration and frequency. Patient compliance is often compromised due to a number of factors, including the perceived stigma associated with assistive devices. Additive Manufacturing (AM) has proved its worth in a number of applications relating to the design of assistive devices; builds of complex, bespoke fitting geometries make AM an ideal fabrication method for upper extremity splints. However, recent advances in system technology to enable multi-material builds have been limited in this field, and a distinct need for a specialised three-dimensional (3D) Computer Aided Design (CAD) software approach is required to allow therapists to design splints for AM. Furthermore, the intent to keep practising therapists at the forefront of splint prescription is of utmost importance. This research proposes a digitised splinting approach, specifically through development of a 3D CAD software strategy to allow therapists to capture their design intent without compromising creativity. Furthermore, the approach proposes the exploration of AM build capabilities by allowing the integration of more creative features, such as aesthetically pleasing lattice structures for increased skin ventilation. The approach also proposes the integration of multiple materials to replicate and improve upon current splint design and fabrication practises. The approach therefore explores an exciting new paradigm for upper extremity splinting, the driving characteristics of which have not been proposed before as a collective medium. This research describes the feasibility of capturing therapists design intent in a 3D CAD virtual environment, whilst capturing therapists opinions of the approach with suggestions for future research and development. Results concluded that therapists were excited by the proposed transition in AM splinting, but that significant development is required elsewhere to establish a supporting infrastructure in order to make the approach a viable option in future upper extremity splinting.

▼ Introdução: A disfunção temporomandibular é uma designação que abrange vários problemas clínicos envolvendo a musculatura mastigatória, a ATM e as estruturas associadas a ambos. Este trabalho pretende realizar uma abordagem histórica, definindo e clarificando definições, terminologias e índices mais utilizados, em investigação na Oclusão. Por fim, realizar uma revisão bibliográfica apoiada num grupo de casos sobre a investigação em si, ou seja, limitações dos estudos e dificuldades da investigação em Oclusão.
Metodologia: Realizámos um estudo observacional longitudinal em que avaliámos uma amostra de 87 pacientes consultados na Clínica Universitária da UCP-Viseu e na Clínica Médica Corpus Dental, com idade entre os 20 e 70 anos. Cerca de 43,7% dos indivíduos pertenciam ao género masculino e 56,3% ao feminino. Estes foram sujeitos ao inquérito Índice RDC/TMD (“Research Diagnostic Criteria for Temporomandibular disorders”) eixo I. No final analisámos as dificuldades e limitações do estudo, produzindo uma lista que pode auxiliar futuros estudos.
Resultados: A dor orofacial foi referida por 43,7% da amostra, sendo 32,2% indivíduos do género feminino. Dos pacientes com dor orofacial 73,7% apresentava dor á palpação muscular, e 42,1% foi diagnosticada com mialgia dos músculos da mastigação. Destes 81,3% foram sujeitos a tratamento com placas de relaxamento muscular, dos quais 68,8% afirmaram no final deste estudo, estar satisfeitos com o tratamento.
Conclusões: De modo a minimizar a controvérsia dos estudos sobre DTM, dificuldades e limitações, é importante que este obedeça a um conjunto de características como existência de um grupo controle e homogeneidade da amostra, que deve ser grande e significativa. Apesar de serem necessários mais estudos com uma amostra maior, concluímos que uma parte significativa da população sofre de sintomas dolorosos na face, e que as placas de relaxamento muscular contribuem para a remissão do sintoma.
Introduction: The temporomandibular joint dysfunction is a term that encompasses many clínical problems involving the masticatory muscles, the TMJ and associated structures. This work intends to conduct a historical approach, defining and clarifying definitions, terminologies and indexes used in occlusion’s research. Finally, conduct a literature review supported by a group of cases about the investigation itself, limitations of the studies and specific difficulties of research in Occlusion.
Methods: We conducted a longitudinal study in which we evaluated a sample of 87 patients consulted in the University Clinic of UCP-Viseu and in the Medical Clinic Corpus Dental, aged between 20 and 70 years. Approximately 43,7% of the individuals were males and 56,3% were females. These were both under investigation Index RDC/TMC (“Research Diagnostic Criteria for Temporomandibular Disorders”) Axis I. ▼ Thermoplastic appliances are sensitive to temperature changes and humidity. To provide a good clinical outcome, it is important to know the magnitude of these effects on different materials. The aim of this study was to determine the effect of different storage conditions (dry-storage, water-storage, thermocycling) on mechanical and geometric properties (elasticity and dimensional stability) of thermoplastic materials. Six thermoplastic materials (Biolon (2.0 mm, Dreve), BIOPLAST (2.0 mm, SCHEU-DENTAL), DURASOFT (1.8 mm, SCHEU-DENTAL), IMPRELON S (2.0 mm, SCHEU-DENTAL), Erkolign (2.0 mm, ERKODENT) and Erkoloc-pro (2.0 mm, ERKODENT)) were investigated in this study and prepared according to the manufacturer’s protocols. Dumbbell-shaped specimens were subjected to tensile tests according to ISO 527 1. Pull-off tests of spherical surfaces with undercut areas were also carried out. Additionally, the dimensional stability was measured. 180 specimens (n = 10 per material) each were prepared for the tensile test and the pull-off test: 60 specimens were dry-stored (control group), 60 specimens were incubated in 37 °C water for 24 h, and 60 specimens underwent thermocycling (1000 cycles, 5 °C / 55 °C, 30 s dwell time) after the 24 h water bath. For the tensile test, specimens were loaded in a universal testing machine (Z010, Zwick), and the modulus of elasticity was determined under tension until fracture/crack at a crosshead speed of 100 mm/min. In the pull off test, specimens were pulled off a spherical model (100 mm/min) and the required retentive force was measured. To determine the dimensional stability, 60 specimens (n = 10 per material) were prepared and consecutively exposed to the different conditions (dry-storage, 24 h water incubation and thermocycling). The specimens were then photographed and circumferences were measured by connecting the four reference points. Results were compared for dependent (dimensional stability test) and for independent samples (tensile test and pull off test) using ANOVA and Bonferroni post-hoc test, p < 0.05 was considered significant. The thermocycling caused a significant decrease in the modulus of elasticity and retentive forces in almost all tested materials compared to the dry stored control group. In all materials, the dimensional stability was affected by the different storing conditions: water incubation caused an increased specimen circumference, and subsequent thermocycling decreased the circumference (except for Erkoloc-pro). Especially BIOPLAST showed great changes in the dimension. The materials examined in this study showed clear changes in elasticity and dimensional stability which can influence the clinical success depending on the area of application. The thermoplastic appliances used in long-term
therapy require frequent control.
