▼ The purpose of this study was to understand how abstinent students perceive sexual behavior among college students, and to examine the effect of these perceptions on abstinence behavior. Furthermore, the study investigated the influential and supportive factors in the practice of abstinence, and sought suggestions on how abstinence can be encouraged among college students. Prevention of sexually transmitted infections (STIs) through safer sex and abstinence are the most effective approaches to control the spread of sexually transmitted diseases and unintended pregnancies. But research on abstinence among college students is sparse. Using the theory of normative social behavior (TNSB) and symbolic interactionism, in-depth qualitative interviews were conducted with 22 abstinent college students to understand their perceptions on sexual behavior of college students, the effect of the perception on them, and the underlying factors that influence and support abstinence behavior. Data from 16 of the 22 participants were analyzed using inductive analysis. Results showed that participants' perceived a high prevalence of sexual activity among college students. In spite of the perceived norm, they decided to be abstinent because of past relationships and previous experiences, religion, fear of negative consequences, and expectation of themselves and others. However, participants reported that they faced several challenges to remain abstinent, these included pressure of long term relationship, unfulfilled sexual desires and curiosity, loneliness, peer pressure, and negative media influence. They were able to cope with these challenges using their strong support system and influential others. Other coping strategies included personal boundaries to avoid compromising situations, group dating, religion, co-operative partners and avoidance of alcohol. Participants were influenced by family, friends, and youth leaders (injunctive norms); they were also influenced by group identity, in which they drew strength from people with similar values and beliefs through open communication and accountability to one another. Outcome expectations from abstinence included enhanced relationships, elimination of distractions, ability to be role models to peers and siblings, and future and present rewards. They expressed their views on sexual health education programs on college campuses and how it can be improved.

ENGLISH ABSTRACT: Various studies have sought to determine the typical versus optimal abstinence period after which semen samples should be collected, and many have been found to be contradictory. Several factors influence the semen microenvironment and subsequent sperm basic and functional parameters. In this study we focused on important biomarkers of prostate, seminal vesicle and epididymal secretion. Furthermore, extensive proteomic analysis of seminal plasma was performed for a more comprehensive understanding of the observations. Semen samples were obtained from normozoospermic donors (n = 16) after 4 days and 4 hours of ejaculatory abstinence (EA) and standard semen analysis was performed with the assistance of computer aided sperm analysis (CASA), while the seminal plasma citric acid, neutral alpha-glucosidase and fructose concentrations were measured photometrically with commercially available assay kits. Protein identification was performed using advanced mass spectrometric techniques and pathways were analysed on both Reactome and String databases. Results of this study displayed significant decreases in semen volume, sperm concentration, total sperm count, and pH after 4 hours of ejaculatory abstinence when compared to 4 days. Furthermore, increases in total sperm motility and progressive motility after short EA periods were observed, accompanied by significant reductions in all epididymal and accessory sex gland biomarker concentrations. However, due to the decreased sperm number, these concentrations translated to a significant increase in citric acid and a decrease in fructose available per spermatozoon, which, along with the effects on semen pH, could more than likely be responsible for increased metabolic function and subsequent increase in the available energy for sperm motility. Proteomic analysis identified 22 differentially expressed seminal plasma proteins, with 18 proteins upregulated after 4 days of EA and 3 upregulated after 4 hours of EA. Of the proteins differentially expressed, 5 proteins from 4 days of EA and 2 proteins from 4 hours of EA, were identified to be related to sperm function and thus selected for pathway analysis. proteins were found to be prominent components of cellular processes including metabolism, apoptosis and cell process regulation, which can be linked directly and indirectly to sperm motility parameters. To conclude, the mechanisms by which very short EA improves sperm motility depends on seminal plasma composition thereby affecting metabolic function and protein interactions.

▼ This dissertation explored the prevalence of and predictors of secondary sexual abstinence (following sexual initiation) in a sample of Texas college students. A qualitative phase of research (N=20) was conducted through face-to-face interviews, and data provided the foundation for development of a web-based survey administered to a sample of 1,133 undergraduates during the quantitative phase of the research. Data produced several key findings. When explaining motivations for secondary abstinence, interview participants most commonly mentioned religion, but also cited fear of physical consequences, past negative experiences with sexual activity, wanting to â saveâ sex for the right person, desire to honor a partnerâ s wish to abstain, desire to maintain power in a relationship, and identification of dissonance between personal values and behavior. Participants described religious factors, friends, parents/family, avoidance of physical consequences, feelings about self, success in school, support from partner, and lack of current temptation as supportive of abstinence. Friends, alcohol consumption, perceptions of sex being widely accepted, and physical attraction and opportunity for sexual activity were considered non-supportive or hindering. Quantitative survey results revealed a 12.5% prevalence rate of secondary abstinence. Predictors of secondary abstinence (following sexual initiation) included positive attitude toward abstinence (OR=1.010; p=.002), subjective norm supporting abstinence (OR=1.010; p=.001), greater religious ties (OR=1.019; p=.046), and previous negative sexual experiences (OR=1.051; p=.020). Participation in an abstinence education program significantly reduced the likelihood of secondary abstinence (OR=.572; p=.049). Fewer perceived barriers (=-.331; p<.000), less environmental manipulation (=-.230; p=.035), and greater religious ties (=.301; p=.003) were significant predictors of self-efficacy for abstinence. Terminology for secondary abstinence was explored in both phases. Qualitative data revealed â virgin,â â secondary virgin,â â renewed virgin,â â born-again virgin,â and â abstinentâ were terms used for secondary abstinence. Quantitative data revealed â bornagain virginâ was the most familiar term, but secondary abstainers most often described themselves as â abstinentâ (49.3%). Findings provide an estimate of secondary abstinence prevalence in this sample, supply insight into motivations for the practice, and suggest focal points for future research (including impacts of abstinence education on sexually experienced youth). ▼ An understanding of both sexual activity and sexual abstinence among young people is crucial in preventing the negative consequences of early sex initiation. The study of motivation is essential for health educators to be effective in persuading individuals to adopt healthy behaviors and avoid health-compromising ones. A discussion of the multi-dimensional construct of motivation for sexual abstinence is absent from the adolescent sexual behavior literature. Within this study, in-depth brainstorming sessions among college students and the construction of concept maps were used to identify and visually represent potentially relevant dimensions of motivation for sexual abstinence. Based upon these results, the Sexual Abstinence Motivation Scale (SAMS) was developed and psychometrically tested with a pilot group, as well as a statistically representative final group from two major universities in central Texas. Based on the results from this exploratory analysis, the final version of the SAMS included eight scales and 41 items. The eight factors were: Commitment to Self-Schema, Risk of Disappointing Authority Figures, Fear/Apprehension of the Sexual Experience, Fear of Physical Consequences, Value of Virginity, Reputation Regret, No Opportunity/Not Important, and Manipulation. The analyses of the psychometric properties of the SAMS lend support to the validity and reliability of scores it generated. The examination of convergent validity of the SAMS showed significant negative correlations with the Sexual Ideology Instrument – an indication that the instrument appeared to be measuring motivation for sexual abstinence. Adequate reliability computed through Cronbach alpha demonstrated the items in the SAMS were measuring the same construct ? motivation for sexual abstinence. Prior to this study, assumptions about factors of motivation for sexual abstinence and their possible structure were unknown. Based on results of this examination, preliminary elements now exist that can be tested for the development of these assumptions. The dimensions uncovered in this analysis contribute to the development and refinement of a theory of motivation for sexual abstinence. Further examination, specifically confirmatory factor analysis using the SAMS, is needed. ▼ Adolescent Black and Coloured South Africans comprise a highly vulnerable segment of the population representing a substantial proportion of individuals with HIV/AIDS, both in their country and worldwide1–4. Although we know that behaviors such as early sexual onset, multiple sexual partners, low rates of condom use, and high rates of substance use are primary causes of HIV infection for this subgroup 3,4, less is known about when, why, or how those behaviors start5,6. Understanding what motivates adolescents to practice safe behaviors in a high-risk context could provide insight for new intervention and prevention strategies for targeting youths before they initiate risky behaviors or soon after they start engaging in those behaviors. This research examines sexual abstinence motives, tracks sexual risk throughout high school, and synthesizes information on sexual risk across adolescence. This dissertation uses a combination of person-centered, variable-centered, and systems science approaches to understand sexual behavior development, accounting for socio-demographic characteristics, substance use behaviors, and a variety of other risk reducing and enhancing exposures and experiences. The first study uses latent class analysis with covariates and grouping variables to create sexual abstinence motive profiles and describe the characteristics associated with each motive profile. The second study estimates the prevalence of sexual activity and transactional sex and uses regression analysis to identify predictors of both behaviors at multiple time points. The third study applies systems thinking perspectives and tools to map out the relevant factors influencing adolescent sexual decision-making and HIV risk behaviors using South Africa as a case study. These studies contribute new information to the sexual risk prevention and intervention literature. The results can be applied to preventing risk behavior development among youths who have not begun engaging in sexual activity but can potentially be extended to protecting sexually active youths from risky sexual behaviors. ▼ Abstinence as a protection against STD and unwanted pregnancy is highly promoted by public health and health education professionals. However, few studies of abstinence have focused on abstinence within the context of partnerships, particularly as relationships change over time. Understanding the factors that predict abstinence states and how those states may change over time is important to understanding sexual decision-making. In this longitudinal study of 365 young women, the influence of relationship quality, motivations for coitus, sexual relationship satisfaction, sexual self-efficacy, sexual conservatism, and religiosity were evaluated in a static model using cross-sectional data from enrollment and in a state change model using longitudinal data. All measures were partnership specific with the exception of sexual conservatism and religiosity. The main study hypothesis was that relationship quality is associated with abstinence state and influences state change within partnerships. Age, relationship quality, motivations for coitus and sexual relationship satisfaction were all negatively associated with abstinence while sexual conservatism had a positive association in the static model. State change from sexually active to abstinent was associated positively with sexual conservatism and negatively with age, sexual desire and relationship quality. For transitions from abstinent to sexually active, the direction of the association for age and sexual conservatism was reversed and relationship quality and sexual desire were no longer a significant factors. Understanding abstinence within the context of partnerships is useful for the development of programs designed to promote healthy sexual decision-making. Important findings from this study were that understanding of relationship quality may result in more appropriately targeted education and intervention messages; sexual conservatism may be a more important construct than religiosity for promotion of abstinence; and, understanding sexual desire and other motivations for coitus within partnerships may provide useful information to health practitioners. Background: Kubemenda is a word in Kiswahili and a conception that the infants‟ health will be negatively affected if the mother has sexual intercourse during the breastfeeding period. Aim: The aim of this study is to explore if nurses experience concerns among parents about sexuality during the breastfeeding period related to kubemenda. Furthermore, to investigate the nurses own perception and knowledge about kubemenda and if general information is given to the parents about breastfeeding and sexuality as well as information related to kubemenda. Method: Semi-structured qualitative interviews with open and closed questions with six nurses that worked at Muhimbili National Hospital. All the interviews were recorded, transcribed and analyzed with content analysis. Result: Kubemenda was defined as ill health in infancy caused by the mother having sexual intercourse during the breast-feeding period. This cultural belief was used as an old fashioned way of family planning. There was a non-existing relation between breastfeeding and kubemenda according to the nurses but they experienced concerns among mothers about timing of sex resumption related to kubemenda. It was hard for the nurses to manage the influence of family-members when informing and educating the mothers about sexuality. There were no guidelines as to what information they should provide regarding kubemenda. Conclusion: Kubemenda is still an existing problem in the society that is hard to eliminate due to strong cultural influence. There is an imminent need of national guidelines for health personnel as to what information they should provide regarding kubemenda.

Bakgrund: Kubemenda är ett ord på Kiswahili och innebär en uppfattning om att barns hälsa påverkas negativt om mamman har samlag under amningsperioden. Syfte: Syftet med studien var att undersöka om sjuksköterskor upplever oro bland föräldrar om sexualitet i samband med amningsperioden relaterat till kubemenda. Vidare var syftet att undersöka sjuksköterskors egen uppfattning och kunskap om kubemenda samt om generell information ges till föräldrarna angående amning och sexualitet samt information relaterat till kubemenda. Metod: Semistrukturerade intervjuer utfördes bland sex sjuksköterskor som arbetade på Muhimbili National Hospital. Intervjuerna spelades in, transkriberades och analyserades med innehållsanalys. Resultat: Kubemenda definierades som ohälsa bland spädbarn där orsaken var att mamman hade samlag under amningsperioden. Denna kulturella uppfattning användes som traditonell familjeplanering. Det fanns inget samband mellan amning och kubemenda enligt sjuksköterskorna men de upplevde oro bland mammor angående sexuell avhållsamhet relaterat till kubemenda. ▼ Sexuality, sexuality education, and teaching about sexuality are inherently feminist topics. There is a way in which to teach sexuality education that deconstructs dominant power structures, empowers students, and that has life-long positive impacts upon the young people in the classroom. In short, utilizing a feminist pedagogy is extremely important when teaching about sex, sexuality, and sexual health. Recently, the U.S. Department of Health and Human Services (HHS) placed an abstinence-only-until-marriage curriculum, Heritage Keepers Abstinence Education, on a list of endorsed curriculum with evidence-based comprehensive or abstinence-plus sexuality education curriculum. Much has been written about the false information in Heritage Keepers Abstinence Education. However, less has been written about the gendered implications of this curriculum and what a curriculum such as this means in the context of feminist pedagogy. My project will not just focus on a gendered reading of Heritage Keepers Abstinence Education using an intersectional approach, but will also examine the ways in a curriculum such as Heritage Keepers Abstinence Education works against feminist pedagogies in dangerous ways. The theories I believe will be most pertinent to my project and will inform and frame this paper include Nel Nodding’s ethic of care, as well as, bell hooks’ theories on feminist pedagogy, engaged pedagogy, and critical thinking. Through a review of the popular literature around Heritage Keepers Abstinence Education, the Heritage Keepers Abstinence Education curriculum itself, and theoretical texts I hope to achieve a more nuanced critique of this curriculum and its implications for the students it is taught to when it comes to important identity categories, with special regard to gender, but at once not leaving out race, class and sexual orientation, as well as in the context of education and the impact that a curriculum such as this can have on the learning outcomes and future learning of students when it comes to sexuality. Objetivo: Avaliar o tempo e os problemas encontrados na retomada da atividade sexual após o parto nas mulheres de Pelotas. Delineamento: Estudo de Coorte prospectivo. Amostragem: Mulheres que tiveram seus filhos nas maternidades de Pelotas de setembro de 2002 a novembro de 2003. População: De 2741 parturientes no período perinatal, obteve-se uma amostra aleatória de 920 mulheres aos 180 dias após o nascimento. Métodos: Realizadas análise uni e multivariada e somente as variáveis com p<0,05 foram consideradas associadas ao desfecho de forma estatisticamente significativa. Principais medidas: Razão de prevalência com intervalo de confiança. Resultados: A prevalência de dificuldade no retorno à atividade sexual foi de 25,5% e 80% das mulheres retomaram a vida sexual nos primeiros dois meses após o parto. O principal motivo para não ter relação sexual foi o fato de a mulher não ter companheiro (63%); as demais causas foram a falta de vontade ou de tempo. As primigestas e as mulheres com problemas de saúde no pré ou no pós-natal, apresentaram uma prevalência mais elevada de dificuldades: dores pélvicas, insegurança ou medo e falta de desejo. Conclusão: Encontrou-se uma alta prevalência de dificuldades em retomar as atividades sexuais após o nascimento. Foi um estudo realizado numa cidade do sul do Brasil e, por isso, os achados estão contextualizados à cultura e aos hábitos destas mulheres. Aim: Evaluating the time and problems met at resuming sexual intercourse after childbirth concerning women in Pelotas. Outlinig: Prospective Cohort Study. Sampling: Women who had their children at maternity hospitals in Pelotas, from September of 2002 to November of 2003. Population: From 2,741 parturients at perinatal period one obtained a random sample of 920 women on their 180 days after childbirth. Methods: Uni and multivaried analyses were performed and only those variables with a p<0.05 were considered as associated to the outcome with a significant statistically way. Prime Measures: Prevalence reason with a confidence interval. Results: The difficulty prevalence at resuming sexual intercourse was 25.5%, and 80% of the women resumed sexual intercourse on the first six months after deliverance. The main cause for not having sexual intercourse was for the woman not having a mate (63%); other causes were the lack of desire or time. The primiparas and women with a health problem on the prenatal or postnatal presented a higher prevalence difficulty: pelvic pains, insecurity and lack of desire. Conclusion: One found a high prevalence difficulty upon resuming sexual intercourse after deliverance. This was a study performed in a city at the south of Brazil, and that is why the findings are contextualized to the education and to the habits of these women. ▼ Adolescent sex research, historically, has suffered from 4 limitations: a lack of theoretical grounding to guide the development of hypotheses (in individual studies); the absence of a multidimensional theoretical model on sexual behavior; use of simple univariate/bivariate analytic techniques; and little systematic study of sexual abstinence. These remain important limitations, as the federal government has increased its allocation of millions of dollars annually for abstinence-promotion programs and 1 in 5 US teens still report having had sexual intercourse before the age of 15. The purpose of this dissertation study was to utilize structural equation modeling to 1) test The Integrated Theory in explaining adolescentsâ sexually abstinent behavior and intentions to remain abstinent before marriage, and 2) refine the theory to reflect which elements contribute more powerfully to the explanation of adolescentsâ behavior and intentions. An anonymous, theory-based paper-and-pencil questionnaire was administered to 2 non-random samples of 7th/8th grade youth (Wave 1 N = 451, Wave 2 N = 448), taking part in a broader evaluation study of Title V-funded abstinence-only-until-marriage education programs in Texas. The questionnaire assessed adolescentâ s self-reported sexual behaviors, intentions to remain abstinent, environmental constraints, beliefs, subjective norms, pro-abstinence self-standards, emotions regarding sexual abstinence and sex before marriage, and self-efficacy. Measurement modeling provided sufficient evidence for establishing construct validity. Initial structural model fit (Wave 2 data) was satisfactory; however, a refined model demonstrated better fit, yielding a Ï 2/df ratio of 3.16, CFI/TLI values of 0.73/0.95, and RMSEA and WRMR values of 0.07 and 0.86, respectively. Greater endorsement of abstinence-related standards predicted stronger beliefs toward staying abstinent, stronger perceptions that others endorse pro-abstinence norms, and a greater self-efficacy to remain abstinent until marriage. In turn, beliefs, perceived norms, and self-efficacy were predictive of intentions (Î² = .379, .300, and .273, respectively, p â ¤ .001). I found similar findings in a second modeling analysis (replication) using a second but similar set of sample data (from Wave 1). Results indicate The Integrated Theory may be useful in explaining adolescentsâ intention to remain abstinent and their subsequent abstinent behavior. I identify several implications for future study and health education practice. Presently, 3,280,000 youth in sub-Saharan Africa between the ages of 15-24 are living with HIV (UNAIDS, 2008a). Ghana, Malawi, and Uganda are three sub-Saharan African nations that have started examining the sexual practices of adolescents via population-based studies. These three nations are similar in that each falls into the last quartile on the Human Development Index (HDI) signifying the lowest levels of human development in the world. The study explores how knowledge and persuasion dimensions influence choices in condom use or sexual abstinence among adolescents from Ghana, Malawi, and Uganda. Further, the study examines whether differences exist in adoption of condoms between nations. In testing the three hypotheses of this study on adoption of condom use and sexual abstinence by adolescents, binary logistic regression and analysis of covariance were utilized. Scale development and instrument reliability were examined through Cronbach alphas and item deletion. The first binary logistic regression model demonstrated that when controlling for background variables, family planning, condom knowledge and efficacy, and media intervention on family planning and HIV prevention increased the likelihood of condom use. In the second binary logistic regression model, family planning, AIDS knowledge, and pressure from family to remain sexually abstinent were statistically significant in the odds on sexual abstinence when controlling for background variables. The testing of the third hypothesis showed that Uganda had a higher mean for condom utilization compared to Ghana and Malawi but only the mean difference between Uganda and Ghana was statistically significant. Implications for macro, mezzo, and micro-level social work practice are discussed. Additional attention was given to macro-level social work practice due to international social development modeling which is most appropriately practiced at the macro-level involving both social policy and community practice. There are four main areas of social policy that the findings from this study can influence: AIDS, family, education, and labor. Implications for community practice in addressing HIV prevention and intervention among adolescents involve community development and community organizing. Implications for social work groupwork were explored as much community organization involves mezzo-level social work practice through the use of small groups.

▼ Condom use is promoted as an effective method for prevention and contraception for people who practice or are at risk of practicing high-risk sexual behaviors. According to the UNAIDS (2009) report, condoms are the only resource available to prevent the sexual spread of the HI-Virus; and with regard to family planning, the same report proposes that condoms expand the choices, have no medical side effects, and thus provide dual protection against pregnancy and disease. However, in Africa as elsewhere in the world, condom use has been fiercely debated. The debates on the accessibility, availability and distribution of condoms in schools are not new nor are they uncontested. In Namibia, the HIV and AIDS policy in education does not explain how, when and by whom condoms should be made available to learners. This leaves it to schools to decide on how (and whether) to make condoms available to learners. As a result, individual school‘s choices not only vary, but are mediated by different factors that are not always in the best interest of learners who, as the foregoing discussion suggests, continue to participate in behaviour that, amongst other things, puts them at risk of HIV infection and falling pregnant. Relying on Foucault‘s theory of discourses, this study investigated the dominant discourses that shape learner, teacher, parent religious and traditional leader and traditional healer perspectives, responses, and experiences with regard to the accessibility, availability, and distribution of condoms in school. The study was conducted in nine schools in Kavango Region in Namibia using a mixed methods approach. The study used triangulation in the data collection process through the use of questionnaires where 792 learners participated in this component, and focus group discussions and individual interviews targeting four groups namely, learners, teachers, parents and religious leaders, traditional leaders and traditional healers. The quantitative data were analyzed using the Statistical Packages for Social Sciences (SPSS), and findings from the focus group discussions and individual interviews were analyzed identifying themes and patterns and then organizing them into coherent categories with sub-categories. The study revealed that the majority of adult participants opposed the idea of making condoms available in schools; advocating abstinence instead. This was despite evidence on the prevalence of sexual activity amongst youth in the community. Reasons had to do with various competing and hierarchized discourses operating to shape participant beliefs, perspectives, and responses in a highly regulated and surveilled social and cultural context. Put differently, the dominant discourses invoked a particular sexual subject; authorized and legitimated who invoked such a subject; who was and was not allowed to speak on sexual matters; as well as how sexual matters were brought into the public space of schools. Presently, 3,280,000 youth in sub-Saharan Africa between the ages of 15-24 are living with HIV (UNAIDS, 2008a). Ghana, Malawi, and Uganda are three sub-Saharan African nations that have started examining the sexual practices of adolescents via population-based studies. These three nations are similar in that each falls into the last quartile on the Human Development Index (HDI) signifying the lowest levels of human development in the world. The study explores how knowledge and persuasion dimensions influence choices in condom use or sexual abstinence among adolescents from Ghana, Malawi, and Uganda. Further, the study examines whether differences exist in adoption of condoms between nations. In testing the three hypotheses of this study on adoption of condom use and sexual abstinence by adolescents, binary logistic regression and analysis of covariance were utilized. Scale development and instrument reliability were examined through Cronbach alphas and item deletion. The first binary logistic regression model demonstrated that when controlling for background variables, family planning, condom knowledge and efficacy, and media intervention on family planning and HIV prevention increased the likelihood of condom use. In the second binary logistic regression model, family planning, AIDS knowledge, and pressure from family to remain sexually abstinent were statistically significant in the odds on sexual abstinence when controlling for background variables. The testing of the third hypothesis showed that Uganda had a higher mean for condom utilization compared to Ghana and Malawi but only the mean difference between Uganda and Ghana was statistically significant. Implications for macro, mezzo, and micro-level social work practice are discussed. Additional attention was given to macro-level social work practice due to international social development modeling which is most appropriately practiced at the macro-level involving both social policy and community practice. There are four main areas of social policy that the findings from this study can influence: AIDS, family, education, and labor. Implications for community practice in addressing HIV prevention and intervention among adolescents involve community development and community organizing. Implications for social work groupwork were explored as much community organization involves mezzo-level social work practice through the use of small groups.
