▼ Background: Many interventions aim to alleviate well-documented problems of malnutrition in residential care homes and improve residents’ health and wellbeing. Despite some positive findings, little is known about how and why mealtime interventions might be effective, and in particular, what effects residents’ experiences of mealtimes have on health outcomes. Aim: The aim of this project was to gain an insight into residents’ experiences of mealtimes in order to inform the development of a mealtime intervention. By addressing the issues that impact on residents’ enjoyment of meals, interventions may target improvements in the health and wellbeing of residents more effectively. Methods: This thesis is comprised of three pieces of empirical work conducted using multiple methods. In a systematic review of stakeholder perceptions of mealtimes, five databases were searched from inception to November 2015, followed by thematic analysis of extracted data. In a second study, semi-structured interviews were conducted with eleven residents from four care homes in the South West UK. Thematic analysis was used to derive content and meaning from transcribed interviews. These studies informed the development of a staff-focussed training programme (study three) using the process of Intervention Mapping (IM) as a guide. The feasibility of this intervention was assessed using qualitative surveys and analysed using multiple methods. Fourteen staff from two care homes participated in the feasibility study, which investigated the deliverability of the training programme and the acceptability of its content. Findings: The systematic review and resident interview study revealed that the dining experience was a focal point for residents’ broader experiences of residing in a care home. Whilst meal quality and enjoyment impacted on the dining experience, the provision of care was pivotal in determining mealtime culture and resident agency within the home. This had implications for self-efficacy and social relationships, particularly in the context of transitioning from independent living to a care home community. These findings informed the development of a mealtime intervention, which was found to be deliverable and acceptable to staff. Conclusion: Mealtimes are a mainstay of life in a care home through which residents’ experiences are characterised, exemplified and magnified. Understanding how residents interact with one another, accommodating their preferences and encouraging autonomy may enhance their mealtime experiences. Evidence from the empirical work supports the development of interventions aimed at mealtime staff to improve resident self-efficacy. This thesis has established the necessary groundwork for a pilot trial and future definitive trial to assess resident (and staff) outcome measures including social (e.g., collective engagement) and psychological outcomes (e.g., wellbeing), as well as health outcomes (e.g., nutritional status).

▼ This dissertation research explores Chinese older adults’ perceptions of
home in varied aging contexts and assesses the process of obtaining a feeling of
home after relocation in an old age. Two sets of qualitative data were drawn for the study: one was collected in Atlanta, Georgia to understand Chinese older immigrants’
home-making experiences in the U.S. (n=21); the other one was drawn in Xi’an, Shan Xi Province to delineate manifestations and pathways that lead to older people’s feelings of
home in six different long-term
care (LTC) facilities in China (n=38). The study intends to highlight the multi-faceted meanings of
home, the interactive nature between human agency and aging environments, as well as the interconnected manner between multiple dimensions of
home. Analyses of both samples revealed three premises of
home: 1)
Home, in its essence, entailed older people’ subjective feelings in relation to their dwelling place, including feelings of comfort and safety, independence and a sense of being accepted, valued and supported. 2) These
home feelings were shaped by objective components and attributes in older people’s living environment which were embedded in the wider social, cultural, and political systems, and simultaneously affected by individuals’ actions and emotions; 3)
Home is a fluid and dynamic concept that requires continuous personal and environmental adjustment. Taken together, no matter aging in the U.S. or in LTC facilities in China, older people expect their environments to bring about positive
residential experiences by compensating for their declines, maximizing their opportunities for independence and mastery, keeping them active and engaged with others, and eventually making them feel good about the place and themselves. This dissertation research offers a full discussion of policy implications for social services and social workers, LTC providers and planners, state and federal government agencies, and aging policy makers at national levels.

▼ Home care of the elderly is of increasing concern not only to purchasers and providers of health care but also to the public and to those responsible for providing social care. As with any service, the aim must be to provide care that is appropriate for each individual. To achieve that, valid and reliable measures of a person's needs are required and resources are to be used as efficiently as possible. A considerable amount of work has been carried out to develop such normative-based measures for assessing the home care needs of the elderly in the form of comprehensive geriatric assessment (CGA). CGA is a commonly used technology which has been shown to be associated with improved health status and lower service use. Despite widespread use, however, the effectiveness of different CGAs has not yet been fully investigated. In the Province of Quebec, Canada, two CGAs which differ in comprehensiveness and resource requirements are being used to assess needs at entry to home care. The aim of this study is to compare the differential impact of these two CGA procedures on patient outcomes: the Systdme de mesure de l'autonomie fonctionnelle, the longer, more comprehensive and resourceintensive CGA, and the Admission au maintien d domicile which is a shorter and less resource-intensive form of CGA. In a prospective cohort study, 158 elderly patients aged 65 years or over were assessed at admission to home care using one or the other CGA and changes in health-related quality of life as well as service use were monitored and compared at the end of a 12-week follow-up. Costs related to the use of a long or a shortform CGA were also explored. These comparisons were made while controlling for patient (age, gender, living alone, quality of life at entry, depression), process (type and intensity of care received) and structural variables (budget and staff mix). Results from comparative and multivariate analyses are in favour of not rejecting the null hypothesis that both forms of CGAs are similarly associated with outcomes. Depression was the strongest predictor of changes in quality of life and high intensity of care and a low proportion of nurses on the home care teams were the strongest predictors of service use outside HC. These results lead us to discuss whether long or shortform CGAs were developed on a comparative rather than a normative definition of needs. The implications of these findings for home care policy and practice are discussed and suggestions for future research are presented.

▼ Studies have shown that young people in residential care homes tend to be low achievers in school and are at high risk of entering adulthood with a low level of education. This group has also shown tendency to adverse psychosocial outcomes. The aim with this study is to reach an understanding of how teenagers and young adults view themselves and their future, what ambitions they have in life and how they see their opportunities to realize their goals. The study is based on a qualitative method and was conducted with semi structured personal interviews with five youngsters 15-21 years old in two different residential care homes. The theoretical viewpoints have been based on E.H Erikson’s theory about identity as well as the theory about resilience.The result of this study indicates that the persons we have interviewed have expressed difficulties in describing themselves. They also express a wish to finish their primary school with complete grades. All of them described what professions they would like to work with. It also came clear that they do not like to plan too far ahead, instead they take one day at a time.The young people that we interviewed expressed a positive view about their future, most of them knew what they want in life, but they find it difficult to describe the way to get there.

▼ Bakgrund: Den psykiatriska vården har förändrats genom historien. En stor förändring skedde i samband med psykiatrireformen på 1990-talet. I samband med detta upplevde de närstående att de fick inta en mer vårdande roll och ett större ansvar för familjemedlemmen med psykisk ohälsa. I tidigare studier framkommer att de närstående upplever börda i förhållande till sin familjemedlem och att det i sin tur har inverkan på den närståendes eget välbefinnande. Vidare påverkar detta den närståendes relation till familjemedlemmen med psykisk ohälsa. Syfte: Syftet med denna studie var att beskriva närståendes upplevelser av hur relationen till en familjemedlem med psykisk ohälsa påverkas när familjemedlemmen flyttat till ett HVB-hem. Metod: Studien genomfördes som en intervjustudie med kvalitativ induktiv ansats. I studien deltog tio respondenter och insamlat material bearbetades av författarna genom en kvalitativ latent innehållsanalys. Resultat: I föreliggande studie framkommer att de närstående upplever att de befinner sig i en komplicerad situation. Rollen som närstående till en person med psykisk ohälsa kan upplevas som påfrestande och relationen blir på grund av olika anledningar ofta ansträngd. Det resultat som framträdde var att Relationen präglas av ömsesidig påverkan och att HVB-hemmet kan ge trygghet och underlätta relationen. Slutsats: Att öka delaktigheten och att underlätta för de närstående gör att de kan minska sitt lidande och öka sitt eget välbefinnande. Vilket, ur ett helhetsperspektiv, kan skapa förutsättningar för en bättre relation och ökad livskvalité för samtliga individer i systemet.
Background: Psychiatric care has changed through history. A major change occurred in connection to the psychiatric reform in the 1990s. In relation to this, the relatives also felt that they were taking a more caring role and a greater responsibility for the family member with a mental illness.Previous studies have shown that close relatives to family members with a mental illness, experience a burden related to their family member. Background: A substantial growth in the number of older people needing care has raised enormous challenges to the health and social care system in many countries including Australia. The structure and dynamics of the Australian aged care system are not yet able to develop a sustainable system for delivering an appropriate model of care. The current aged care system is complex and older Australians experience multifaceted journeys through the fragmented systems to meet their care needs. Research evidence is limited about how different parts of the system intersect and how an individual transitions from one level of care to another in accordance with changing needs over time. There is also a lack of understanding of how individual and social circumstances and supports might predispose older people to use care, or enable them to use appropriate care when they need it. To fill these knowledge gaps, this study aims to assess patterns and trajectories of different types of aged care use over time and to examine the movements of older women between different levels of aged care use according to their changing needs and circumstances throughout later life. Methods: This study utilised data from the older cohort (born between 1921-26) of the Australian Longitudinal Study on Women’s Health and linked aged care and National Death Index data from 2001 to 2014. The sample consisted of 11,245 women who survived to 2001 and did not opted out of linkage of survey data to aged care information. To address the research objectives, we applied the following statistical methods: 1) k-median cluster analysis to assess patterns of home and community care use; 2) repeated measures latent class analysis to identify latent patterns of aged care use over time; 3) competing risk survival analysis to identify risk factors for the time from first home and community care (HACC) use to residential aged care (RAC) admission; 4) multi-trajectory modelling to assess the trajectories of care need in RAC; and 5) Markov multi-state modelling to estimate the transition rates and probabilities, and length of stay at each level of aged care use. Results: The results reveal a significant diversity in the patterns of HACC use, with a majority of older women living at home independently, requiring only low-level use of a few basic services, while one quarter have complex care needs requiring greater use of multiple services. Women were most likely to enter aged care by first using HACC. Significant variation in the latent patterns of aged care use was also observed, with around two-thirds of women not using any services or using a limited basic community care services until age 85-90. Only a small proportion of women were increasingly dependent on RAC as they age. Having complex use of HACC was associated with delayed admission to RAC, after adjusting for individual characteristics. International literature consistently reports that more than half of people living in residential aged care have a diagnosis of dementia. Research findings in this field concur that well-designed physical environments can increase independence, enhance quality of life, and provide a drug-free means of improving the behavioural and psychological symptoms experienced by people living with dementia. Despite this, there has been limited research into how well the broad stock of existing residential aged care settings supports the needs of residents with dementia. This dissertation investigates the design quality of existing residential aged care units, with a focus on the impact of architectural planning layouts on overall dementia design quality. Specifically, the dissertation develops new knowledge in terms of three interconnected research aims. The first research aim is to evaluate and compare dementia design quality in the layout planning of NSW-based and international best-practice examples of residential aged care units. To fulfil this aim, the dissertation uses floor-plan layouts to evaluate the dementia design characteristics of ninety residential aged care units from New South Wales (NSW), Australia, and compares these against the characteristics of ninety-four published international best-practice examples. The second research aim is to determine whether the dementia-enabling characteristics of floor-plan layouts for residential aged care units in NSW have improved over the last four decades. This aim is addressed by correlating the dementia design evaluation scores, from the analyses undertaken as part of the first research aim, against the year of construction for each unit. The third aim of the research is to investigate the impact of five spatial planning factors on the dementia design properties of Australian and international residential aged care settings. The five factors are: the unit floor area, number of bed-spaces provided, floor area per resident, storey location, and whether purpose-built for dementia or not. The research undertaken for this aim builds on the results of the first two aims by undertaking correlation analyses between the identified attributes and the dementia design evaluation scores for each residential aged care unit. In order to fulfil the three aims of the research, a new evidence-supported dementia design evaluation methodology is developed. This method, derived from Fleming’s (2011) Environmental Audit Tool, forms the basis of evaluations undertaken of the layout planning of the ninety NSW-recruited residential aged care units, and ninety-four international units, considered to be exemplars of dementia design. This new design evaluation approach produces formal scored measures of dementia design quality across nine established dementia design principles. ▼ Background: Worldwide one in twelve people are living with diabetes and one in two people do not know they have diabetes. Currently large numbers of the older people live in residential care homes in the UK, and up to one in four older people living in residential care homes present with diabetes. Low vision is one of the complications associated with diabetes in older people. In those aged 75 and over, one in five, and in those aged over 90, one in two people are affected by low vision and they are at an increased risk of developing other eye diseases. Within 20 years of diagnosis nearly all people with Type 1 and almost two thirds of people with Type 2 diabetes (60%) have some degree of diabetic retinopathy. Aims and Objectives: This study aimed to investigate the issues and problems faced by older people living in residential care homes with low vision and diabetes; to evaluate health professionals’ knowledge and understanding of the impact of low vision associated with diabetes in older people living in residential care homes; and to develop an educational toolkit which aimed to educate health care assistants about low vision and diabetes. Methods: This study is an exploratory investigation of older people living in residential care homes with low vision and diabetes. Adopting an open-ended qualitative approach using focus groups, interviews and a health professional’s survey, 116 participants were involved. These included GPs, ophthalmologists, nurses, optometrists, health care assistants and older people with low vision and diabetes. The data was analysed thematically. The educational toolkit was developed in the second part of this study, and 20 healthcare assistants were trained using this toolkit. Their knowledge was tested before the training, immediately after the training and one month after the initial training. Following Kirkpatrick’s model, the skills and practical use of the educational toolkit was assessed using an open-ended qualitative approach. Results: The results found that many older people and the health care assistants had the perception that low vision was a normal ageing process and could not be rectified. The study found that there was evidence to suggest that eye health was not considered to be a priority; instead, it was considered to be a natural part of the ageing process. The results found that 82% of the HCAs had not had any training in the area, and more than half of the nurses and GPs did not have sufficient knowledge of low vision and diabetes. After training, however, their knowledge was increased. This suggested that low vision and diabetes toolkit training could be used to educate healthcare assistants on a regular basis. The study also found that knowledge does decline over time, and therefore regular training for HCAs is required in order to maintain eye health and diabetes in older people, as well as improving their quality of life. Existing literature suggests that social support can help maintain psychological wellbeing. This study aimed to examine perceived changes in social support following a move into residential care. The key theories drawn upon were socioemotional selectivity theory (Carstensen et al., 1999) and the convoy model (Kahn & Antonucci, 1980). Method: Forty care home residents were interviewed using a structured interview. A hierarchical network mapping technique was used to measure perceptions of total network, inner network and peripheral network size. Functional support from a key significant other was measured using the Significant Others Scale. Contact with network members, depression and demographic information were also examined. Retrospective ratings were obtained by asking participants to think back to before they moved into care. Current and retrospective ratings on all measures were compared using Wilcoxon signed rank tests. Results: Total network size was perceived to have decreased following a move into residential care. There was no significant difference between current and retrospective ratings of inner network size. Peripheral network size decreased but this difference was not statistically significant. There was no perceived change in emotional and practical social support received from a key significant other following the move. Discussion: The results suggest that an individual‟s social network is compacted following a move into care but that membership of the inner network remains stable. These findings are discussed in terms of socioemotional selectivity theory (Carstensen et al., 1999) and the convoy model (Kahn & Antonucci, 1980). Strengths and weaknesses of the study are discussed and clinical implications of the findings explored.

technology (health IT) holds great promise for improving
the quality and efficiency of aged care services in
residential aged care facilities (RACFs). Health IT, and,
in particular, electronic documentation systems, have
potential to improve the management of residents’
records, to facilitate the delivery of high quality care
to the residents by reducing caregivers’ time on
documentation and increasing their time with the
residents. Despite the expected benefits, adoption of
health IT in RACFs remains poor. A number of aged care
providers are reluctant to introduce health IT into
residential aged care services. Among the reasons for
this is uncertainty about how the use of health IT may
affect the work of caregivers. To address this concern,
this study examines the impact of the introduction of an
electronic nursing documentation system on caregivers’
distribution of time on activities.
This study utilized a work sampling method with
an observational component. Data collection was carried
out between 2007 and 2011, before and after the
implementation of the electronic documentation system. An
observer (ENM) used a work classification tool with 48
activities grouped into 8 categories to identify and
record activities being undertaken by registered nurses
(RNs), endorsed enrolled nurses (EENs), personal carers
(PCs), and recreational activity officers (RAOs) at
particular times in high and low care houses of an RACF.
Additional information was collected through
surveys. In general, 30,179 activities
were recorded, with 17,732 recorded in the high care
house and 12,447 in the low care house. Compared with the
proportions of time spent on activities in the
pre-implementation period, caregivers’ proportion of time
on documentation increased or remained stable at most
measurement periods after the implementation of the
electronic system in both high and low care houses. A
comparison of the proportions of time spent on
documentation and communication activities after the
implementation of the system in the high care house
showed that the proportion of time of RNs on these two
activities remained stable during the first 12 months
after the implementation of the electronic system. At 23
months, their proportion of time on communication reduced
and the proportion on documentation increased. The
proportions of time of PCs and EENs spent on
documentation increased and their proportions of time on
communication were reduced during most measurement
periods in 12 months after the implementation of the

▼ The purpose of this study was to explore the factors that contribute to prosocial behaviour in maltreated adolescent females living in a children’s home in the Tshwane-metropole, Gauteng, South Africa. The age of the participants varied between 14 and 18 years.
The study was approved by the Internal Research Panel and Faculty Board of the North-West University. Approval was also obtained from the children's home where the study was conducted.
The researcher worked from a positive psychology paradigm which guided the researcher to focus on the participants' strengths and positive aspects which contributed to their display of prosocial behaviour.
A literature study was conducted to provide the researcher with a clearer understanding of the meaning of the research problem. The literature study focused on the development of prosocial behaviour and adolescent development within the context of child maltreatment.
The researcher utilised a qualitative research approach, which enabled her to describe and understand the participants' behaviour. As methodology, the researcher utilised an intrinsic case study design and participants were selected based on purposive sampling.
Data collection relied on two semi-structured interviews per participant which provided the opportunity for participants to share their thoughts, feelings and perceptions.
Thematic data analysis was performed, using Creswell’s spiral of data analysis. During data analysis, regarding the factors surrounding the prosocial behaviour of maltreated adolescent females living in residential care, two main contributing themes were identified, namely:
* Internal factors
* External factors Internal factors illuminate the importance of (1) a moral identity that guides behaviour according to internalised moral values; (2) an internal locus of control which attests to the participants’ view of themselves as active role-players and not mere victims of circumstances, and (3) cognitive skills demonstrated in the capacity to engage in critical thinking. External factors emphasise the importance of (1) attachment figures and positive role-models that model moral values and (2) a supportive, nurturing environment.
Internal and external factors do not operate in isolation, but there is rather a definite interplay between these factors, such as attachment figures in the environment (external factor) who model moral values which the child incorporates into her identity in the attainment of a moral identity (internal factor).
Strategies to broaden support networks for children should receive attention within the child welfare context, as they could, among other factors, promote positive outcomes for youth in residential care.

▼ The increasing survival of patients with advanced-stage cancer, coupled with the increased presence of end-stage chronic illness in an ageing population, is leading to an increase in demand for patients requiring palliative care. Due to the ongoing need for acute-pain and symptom control in hospice/palliative care units, few of these are able to offer any long-stay admission for those whose symptoms have been stabilised.
Transfer from an inpatient palliative care facility may then be necessary, when a patient is considered to be no longer requiring specialist palliative care services. A core component of the role of palliative-care social workers involves working with patients and their families/carers1 when the care pathway shifts and the option of residential care (in this context, nursing homes) needs to be considered.
As a social work clinician, I have observed that, when this process has been considered or contemplated, the patient and their families often display anxiety, anger and distress.
This practice research study has explored several issues, including the impact of this transition on the patient and their families and on the interdisciplinary healthcare team treating the patient. This study has analysed this phenomenon in two ways. In the first part of the study, a data-mining process was utilised. A sample of 30 consecutive patients who were assessed as being eligible for high-level care (or nursing-home level care) was included. Medical files/histories were reviewed using an audit tool that was developed based on the literature review. The salient or pertinent characteristics of the population group were documented.
In the second part of the study, an investigation was undertaken to identify concerns and barriers regarding the transition from hospice care to residential care and highlight the opportunities to improve clinical practice in this area. This investigation analysed the literature and clinical practice, and interviewed the key stakeholders involved in this process. A tripartite approach was adopted by conducting face-to-face interviews with patients, their carers and members of the interdisciplinary healthcare team. As social work clinicians have a substantive role in this area of transition, telephone interviews were also conducted with 10 social workers working in similar inpatient palliative-care facilities in the area of Victoria to gauge their experiences.
A thematic analysis discerned a number of common themes highlighting the impact and difficulties of this transition on the key stakeholders. The results suggested recommendations to improve or best manage this process and service delivery.

care need supportive relationships and positive interactions. However, current evidence suggests that people with cognitive-functional impairment experience few friendships, negative interactions are common and many residents feel completely isolated. This thesis aims to describe the friendships, social relationships, and personal friendship schema of people living in a Sydney
residential aged
care facility. The research uses multiple social network analysis methods, including semi-structured interviews, standardised survey assessments, and observations. The thesis explores associations between social networks and self-report measures of perceived social support and adult attachment profiles. It describes staff- and observer-perceptions of residents’ multi-valenced relationships and explores associations between relationship characteristics and engagement and social isolation. The thesis introduces a novel psychosocial method for analysis of observational field note data. The novel method is applied to describe patterns and quality of coresident interactions involving residents with dementia and to identify possible personal and environmental factors that influence interactions.Residents experienced few friendships and many residents had no positive relationships. Most residents perceived little support and felt isolated. Residents’ perceptions of support or isolation and difficulty with relationships may have been influenced by their position within the larger network and by environmental factors including
care unit location and lack of staff facilitation. The size and quality of resident networks were correlated with personal attributes including residents’ cognitive and physical capacities and attachment style. Most residents were able to articulate friendship clearly. Their views of friendship had likely changed little upon entering
residential care as they applied long-held schema within a dramatically different social context. Despite multiple barriers to relationships, residents with dementia wished to reach-out to coresidents to connect in positive and meaningful ways. Only a few had friendships. Moreover, staff- and observer-report indicated negative relationships were common and interactions that began positively often ended in rejection and disconnection. These results indicate a pressing need for individually tailored interventions and a sharper focus on residents’ social health.

▼ The World Health Organization recently estimated that in
Europe alone 117 million children suffer from maltreatment or other adverse
childhood experiences. Some of these children need to be placed in (24-h)
settings for out-of-
home care, such as foster
care, family-style group
care,
and
residential care. However, children do not always benefit from these
services, as suggested by the substantial numbers of children repeatedly
experiencing placement breakdowns.
Aim of this research is to increase the effectivity and efficiency of
out-of-
home care services. For this purpose, typical baseline child and
family characteristics of out-of-
home placed children were linked to
children’s development in the various 24-h settings. This knowledge both can
provide building blocks for the development of sound decision-making
strategies for referral to a certain type of out-of-
home care and provide
insight into which child and family factors need additional treatment during
placement. Together, this increases the likelihood that children for whom
(temporary) out-of-
home placement is inevitable will receive optimal services
with regard to their developmental needs. The research findings have been
translated into three fundamental steps to transform the current stepped-
care
method for allocation decisions into a collaborative, effective, and
matched-
care model for allocation.

▼ This study aims to explore parents and treatment personnels experiences of the Marte meo methodat a family treatment home. Three retrospective interviews were made with parents whoexperienced Marte meo treatment, as well as four interviews with treatment personnel. The purposeof the Marte meo method is to develop the interaction between parent and child. Previous resultsfrom studies show that treatment with the Marte meo method could provide positive effects to theparent child relationship. The results in this study have been analyzed by using parts of attachmenttheory, the salutogenic perspective and the term intersubjectivity. The parents in this study all statethat Marte meo treatment strengthened them in their role as a parent. The treatment personnel statethat they have seen positive changes in families living at the family treatment home whoparticipated in Marte meo treatment.

▼ Bakgrund: Måltider utgör en social och kulturell grund där människor kan mötas i ett välbefinnande och samtidigt tillgodose sig näring. Ökad malnutrition med stigande ålder är ett globalt problem som omfattar både kognitivt medvetna och kognitivt nedsatta.Syfte: Syftet med litteraturstudien var att identifiera faktorer som vid måltider är av betydelse för välbefinnande och nutritionsstatus hos äldre människor boendes i särskilda boenden.Metod: En litteraturstudie med globalt perspektiv där vetenskapliga artiklar från Canada, Amerika, Storbritannien, Sverige, Danmark, Nya Zeeland och Australien var underlaget till resultatet. Inklusionskriterier var äldre över 65 år, kognitivt medvetna och kognitivt nedsatta vilka levde i särskilda boenden samt vårdgivare som arbetade i sådana.Resultat: Måltiden ger ett socialt och kulturellt sammanhang som stärker både boenden och vårdgivares relationer. Utformning av en terapeutisk fysisk miljö visade sig vara till fördel både för de äldre och vårdgivare. Möjlighet att utöva personcentrerad vård skulle kunna förbättras enligt den här litteraturstudien. Litteraturstudien visar på att social samvaro under måltider är en stund för gemenskap och ett välkommet avbrott under dagen.Slutsats: Måltidssituationen är en komplex situation där många faktorer är inbegripna för välbefinnande och nutritionsstatus. Tre huvudteman identifierades nämligen fysisk måltidsmiljö, personcentrerad vård och social samvaro.
Meals form a social and cultural basis where people can meet in a state of well-being while at the same time consuming enough food to meet thier nutritional needs. Regardless of age, social relationships and the need for food have been linked to enhance the quality of life. Older adult´s poor nutritional status is a global problem that include risks for both cognitive conscious and cognitive impairment.Aim: The purpose of the study was to identify and describe the aspects that are of nutritional beneficial importance and the well-being of the elderly in residential homes.Method: A litterature study with a global perspective whereby scientific articles from Canada, America, Britain, Sweden, Denmark, New Zealand and Australia laid forth foundation of the key findings. Inclusion criterias were elderly over 65 years, cognitive consciousness and cognitive impairment, whom resided in residential homes with round the clock caregivers.Results: The meal provides a social and cultural ambience that strengthens relationships for both the reisdents and caregivers.Three themes emerged and these affected well-being and nutritional status. The possibility of applying person-centered care could be augmented according to the literature study. The literature study shows that social interactions during meals are intervalls for gathering as well as they are welcome intermisssions during the day.Conclusion: The mealtime situation is a complex situation where multiple aspects involved in the well-being and nutrition status of the elderly. ▼ Many older people express strong preferences to
remain in their own housing as long as possible. Due to declining
health they may, however, become dependent on others for their
daily care, eventually reaching a point where they consider
relocation to a residential home. The overall aim of the
dissertation is to reveal the process preceding older people’s
potential relocation from ordinary housing to a residential home in
a societal context, and to study how older people, their family
members and care managers experience and describe the process
individually and in relation to each other. The first article of
the dissertation is based on older people’s perspective, the second
on family members’ perspective, and the third on the care managers’
perspective. In the fourth article a theoretical model is used in
order to focus on why the care managers tend to apply a restrictive
approach in their occupational exercise. An analysis based on a
holistic perspective is presented in the introductory chapter of
the compilation thesis. To try to understand what happens when
older people consider relocation from ordinary housing to a
residential home, various theoretical approaches have been applied.
Altogether 21 older people, 17 family members, and seven care
managers have contributed to the empirical material, mainly in
interviews. From a holistic perspective, attention is drawn to
expressions of “consideration” as a prominent feature in the
process when older people’s potential relocation to a residential
home comes to the fore. As a result, different expressions of
consideration reveal that those involved use the ideals of older
people’s independence and self-determination in order to justify
personal actions and attitudes. By consideration, nobody is
prepared to take the full responsibility for the decision, and
instead the responsibility for a potential relocation decision is
located at somebody or something else. The ideals of an older
person’s independence and self-determination are promoted, at the
same time as the application of these ideals in reality becomes
significantly limited, like in “paradoxes of
consideration”.

▼ The aim of this study was to investigate how staff members at residential homes experience encountering sexuality among the youths. The study was conducted by using qualitative interviews with four staff members at four different residential homes. The interviews have been analyzed by using content analysis and have been looked at through a perspective of experience, as described by Richard Stevens, and by applying developmental psychology. The results show that, although sexuality is experienced as being distinctly present and being a topic of major concern, the staff members have given sex-education as a means by which to raise their own competence little or no thought. One way to interpret this is that sexuality and sex-education haven’t been given a sufficient space in the work with the youths. Several staff members do experience the need of skills development in order to be able to function in the role as significant others when it comes to sexuality among the youths, and express that they would like to receive training in doing so. To a certain degree they experience situations in which they are not entirely certain how to act and which they are not sure how to handle. The result is somewhat in line with what previous research has pointed at, but does not include staff members having severe problems talking about sexuality with the youths. All participants express that they have minor or no problems with talking about sexuality and with encountering sexuality-related aspects in relation to the youths.

Overuse of secondary
care services and insufficient access to primary
care in aged
residential care (ARC) are topical issues. There is limited policy guidance or research evidence promoting effective, efficient and sustainable models of primary
care in ARC as part of an integrated healthcare system.
Aim
This thesis aims to understand and delineate the existing relationship between primary
care and ARC in New Zealand (NZ), and to propose a model or models of
care designed to improve the capacity for integration between primary
care and ARC services. The research specifically investigates the current state of primary
care in the ARC sector from a policy and practice perspective, to identify features of a model or models that could offer quality and sustainable improvements in the delivery of primary
care in ARC.
Methods
1) An integrative review of international research literature was completed to identify primary
care models in ARC.
2) A predominantly qualitative mixed methods approach was used to identify and describe NZ models within their relevant policy context. This entailed a narrative review of grey literature, 34 in-depth interviews with purposively sampled key informants, and four case studies with either a practice or a policy focus.
3) Factors for integrated delivery of primary
care in ARC were identified, and models of
care critiqued and compared using thematic analysis and general inductive principles.
Results
The research identified well-defined existing issues in the delivery of primary
care in ARC. Proposed solutions were seldom found to address underlying causes.
Multiple models of
care that have an impact on the primary
care and ARC interface were identified: eighteen from international literature (including three NZ models) and eleven from key informant interviews. Models often operate in parallel and few represented an integrated approach. The persistent use of historic models by ARC undermines the integrity and sustainability of newer more integrated models.
Policy guidance was limited at the interface between primary
care and ARC. In NZ separate national contracts for the delivery of primary
care in ARC has contributed to a lack of integration at the primary
care and ARC interface.
The research identified common factors that support integration at the primary
care and ARC interface. These factors are policy guidance, infrastructure for information sharing, workforce development, secondary
care support and clinical governance.
Several integrated models that could improve primary
care services in ARC and the quality of resident
care were identified. Each model was dependent on the context in which they operate. Policy promoting an integrated systems view is needed to inform practice.
Conclusion
This thesis has national and international relevance as it contributes to our understanding of the issues at the primary
care – ARC interface, and the need to adopt more integrated models. Current models of

▼ The aim of this study was to examine how social services work with home-based solution which aim to replace residential care for children and youth. Furthermore, our intention was to see how social services argue for the interventions through policy documents and interviews. The data consisted of policy documents from eleven municipalities and two interviewees with three professionals. The result of the study was processed through a qualitative content analysis and was analysed in relation to previous research, systems theory and institutional theory. The study presented that social services strive to replace placements with other alternatives at home. Interventions, such as home-based solutions and qualified home-based care and family-oriented methods was central in the study. The analysis of the collected material resulted in three approaches to the case “alterative to placements”: home care, efficiency and eclecticism. Furthermore, it showed that home-based solutions develop in order to receive more financial gains. On the other hand, there is also an uncertainty in the way the interventions should be implemented since they are under process. One conclusion is that it is ambiguous whether home-based solutions aim to replace institutional care or if they apply as supplementary intervention for children and youth. Regardless, the municipalities described that they want to develop comprehensive home-based solutions that enable the child to grow up in his or her home environment, if not for the rights of the child and for the parents, for a sustainable social service.
Syftet med fallstudien var att undersöka hur socialtjänsten i Sveriges kommuner argumenterar för öppenvårdsinsatser som ett alternativ till placeringar av barn och unga. Vidare har intentionen varit att undersöka hur socialtjänsten genom måldokument och intervjuer argumenterar för sina insatser. Som datainsamlingsmetod användes elva måldokument följt av två intervjuer med tre informanter. Studiens resultat bearbetades genom en kvalitativ innehållsanalys och analyserades i relation till tidigare forskning, systemteori samt institutionell teori. Resultatet utgörs av en fallbeskrivning bestående av tre förhållningssätt till ”alternativ till placering”: vård på hemmaplan, kostnadseffektivisering och eklektism. Studien visade att socialtjänsten har ett flertal insatser att tillgå för att minska placeringar men att insatserna varierar mellan kommunerna. Det framgick att kommuner framförallt vill tillämpa omfattande öppna insatser på hemmaplan för att ersätta placering utanför det egna hemmet. Begrepp som hemmaplanslösningar och kvalificerad vård på hemmaplan återfinns i måldokumenten där kommunerna skriver att de vill utveckla och implementera familjeorienterade metoder. ▼ Adolescents attending special education for learning disabilities (SEL), special education for behavioural problems (SEB) and adolescents living in a
residential youth
care (RYC) institution present a complex risk profile including severe behavioural and emotional problems, deviant peer networks, and problematic family backgrounds. In addition to these problems, youth in these groups appear to be at higher risk for substance use compared to adolescents in the general population. Recent, representative studies on substance use levels and related risk indicators are however limited. The objectives of the present thesis are a) to examine prevalence and onset of daily use of cigarettes, alcohol consumption, and lifetime use of cannabis and hard drugs among adolescents in SEL/SEB/RYC compared to adolescents attending mainstream education, and b) to identify the extent to which the elevated risk of substance use among youth in SEL/SEB/RYC is explained by high levels of individual, family, and peer risk indicators that are known to characterize these adolescents. The cross-sectional part of the thesis includes information from 1,905 adolescents attending SEL, 603 adolescents attending SEB and 531 adolescents living in a RYC institution between 12 to 16 years of age. Self-report questionnaires are compared to those of a reference group consisting of 6,946 adolescents in the same age category attending mainstream education. The longitudinal part of the thesis includes youth in SEL (N = 536) and in RYC (N = 241). Results illustrate that in particular youth in RYC are a high risk group for substance use (onset). Also youth in SEB report very high levels of tobacco, alcohol, and drug use. Differences are especially large in the youngest age category (12-13 year olds), with more than twenty times as daily use tobacco users in RYC compared to their counterparts in mainstream education. Although adolescents attending SEL show higher levels of tobacco and hard drug use, having a learning disability does not seem to put adolescents at risk for substance use. In fact, adolescents with learning disabilities do not differ from their counterparts in mainstream education with respect to the onset of tobacco use and even have a smaller risk of alcohol and cannabis onset. Individual, family, and particularly peer risk indicators explain a substantial part of the elevated risk of substance use among adolescents in SEB and RYC. This implies that the high substance use levels within these settings partially need to be understood as a result of the selection of youth with a high-risk profile. SEB schools and RYC institutions therefore need to take into account that substance use belongs to the risk profile of these adolescents. ▼ Syfte: Syftet med denna studie var att beskriva uppfattningen hos sjuksköterskor med tjänst på särskilda boenden gällande deras arbete med tillämpningen av munhälsobedömningar och deras uppfattning om hur munvård utförs. Metod: Studien genomfördes med kvalitativ deskriptiv design. Data samlades in hjälp av intervjuer med sjuksköterskor (n= 6). Intervjuerna analyserades med hjälp av innehållsanalys och fyra olika teman identifierades: Sjuksköterskors uppfattning om munvård, sjuksköterskors uppfattning om munhälsobedömning, sjuksköterskors uppfattning om munvår och munhälsobedömning i samband med palliativ vård samt sjuksköterskor uppfattning om samarbete med folktandvården. Resultat: Resultatet visade sjuksköterskorna uppfattade att munhälsobedömningar sällan utförs av sjuksköterskorna. Dock ansåg de att munhälsobedömning och munvård utförs regelbundet av såväl sjuksköterskor samt omvårdnadspersonal när vårdtagarna befinner sig i livets slutskede. De eftersöker instruktioner och utbildning av munhälsobedömning enligt ROAG innan detta införs i verksamheten. Munhälsobedömning har utförts av folktandvården en gång om året och de har även kommit till boendena om det finns problem med någon vårdtagares munhälsa. Folktandvården är ansvarig för att skriva vårdtagarnas munvårdskort som är ett stöd för vårdpersonalen vid utförande av daglig munvård. Resultatet har även visat att daglig munvård utförs av undersköterskor och vårdbiträden. Slutsats: Resultatet visar att munhälsobedömning utförs bristfälligt av sjuksköterskor i den kommunala äldreomsorgen. Vidare anser de tillfrågade sjuksköterskorna att utbildning i munhälsobedömningsinstrumentet ROAG är nödvändig innan införandet.

▼ I denna studie har erfarenheter och framtidsvisioner vad gäller utbildning och arbete i Sverige bland unga kvinnor som kom till Sverige som ensamkommande flickor undersökts. Ambitionen med studien är att öka kunskapen kring gruppen unga kvinnor som kommit till Sverige som ensamkommande flickor. Kvalitativa djupgående intervjuer samt en hermeneutisk forskningsansats, präglad av författarnas förförståelse, har använts i studien. I studieresultatet framkommer att de unga kvinnorna upplever mindre stöd samt en ökad ensamhet efter att de har flyttat från HVB-hemmet till eget boende. Studieresultatet visar även på att de unga kvinnorna upplever segregation i skolan samt har kunskapsluckor kring arbete och utbildning. I studiens slutsats formuleras möjliga förbättringar för att öka unga kvinnors kontaktnät innan de flyttar till eget boende, för att motverka segregation i skolan samt förslag på att införa regelbundna samtal. Förbättringarna syftar till att öka unga kvinnors möjligheter att nå sina mål, nå ökat välmående samt en förbättrad integration.
This study have set out to oppose the experience and outlook on future regarding education and work within the group of young women who came to Sweden as unaccompanied refugee children. The purpose of the study is also to increase knowledge about young women who came to Sweden as unaccompanied refugee children. Qualitative in-depth interviews with a hermeneutic research approach characterized by the author’s preconceptions have been used for the study. The key features of the study is that young women after they have moved out of the residential care home to their own apartments experience feelings of loneliness and lack of support. Experiences of segregation in school and knowledge gaps regarding education and work is also key features in the study. In the conclusion of the study possible improvements are suggested to improve young women’s contact network before moving to their own apartments, counteract segregation in school and suggest an establishment of regular dialogues. The improvements aims as a way to increase young women’s abilities to reach their goals, increase their wellbeing and improve integration.

▼ Syftet med denna studie är att undersöka hur HVB-personal på olika hem i Stockholm arbetar med ensamkommande flyktingbarn rörande barnens kulturer och traditioner samt ta reda på hur personalen arbetar för att integrera flyktingbarn in i vad de upplever vara ett svenskt samhälle. Studien har även till syfte att ta reda på ifall HVB- personalen upplever att det finns problematik kring att integrera barnen samtidigt som de måste ta hänsyn till ursprunglig kultur och tradition hos barnen. Uppsatsen grundar sig i en kvalitativ ansats av tio intervjuer med personal som arbetar på olika HVB-hem. Med hjälp av de teoretiska begreppen gräsrotsbyråkratier, systemintegration, social integration, kultur, kulturmöte och kulturkrockar men även tidigare forskning visar resultat och analys ett sammanhängande mönster. Resultaten visade att HVB-personalen i hög grad värderar hänsynstagande till de ensamkommande flyktingbarnens/ ungdomarnas kulturer och traditioner under integreringsprocessen. De främst återkommande punkterna som personalen menar är viktigast att ta hänsyn till rörande kultur och tradition är barnens/ ungdomarnas; värderingar, religioner, högtider och ursprungliga språk. HVB-personalen upplever att deras huvudsakliga mål som HVB-personal är att integrera barnen/ ungdomarna till det svenska samhället. Tillvägagångsätten för att integrera barnen beskrivs som ett vardagligt arbete som omfattar personalens engagemang i att vägleda, informera barnen/ ungdomarna om samhällets viktiga organ så som att gå i skolan eller att lära dem skillnaderna mellan rätt och fel enligt Sveriges normer och värderingar. Begreppet integrering handlar för våra respondenter sammanfattningsvis om att flyktingbarnen/ ungdomarna tar del av ett nytt samhälle och blir en del av det. Studien visade avslutningsvis att spänningar/ obalans kunde uppstå vid integreringen av barnen/ ungdomarna som resultat av kulturskillnader och kulturkrockar men även på grund av skilda tolkningar av arbetsuppgifter för personal.
The purpose of this study is to investigate how staff from residential care homes in Stockholm work with unaccompanied refugee children/ adolescents concerning the children's/ adolescents cultures and traditions. The study also aims to find out how the staff work to integrate refugee children/ adolescents into what they perceive to be a Swedish society. Lastly the study also aims to find out if the staff experienced problems concerning the integration of the children/ adolescents while they have to take their original culture and tradition into account. The essay is based on a qualitative approach of ten interviews with staff working in different residential homes in Stockholm. ▼ The aim of this thesis was to explore the key care factors that influence the educational achievement of children looked after at home and away from home in Scotland. Traditionally there has been less research conducted in Scotland than in the rest of the United Kingdom. The research analysed a large new sample - one fifth of the care leaving population in Scotland - and spans a five year period. The thesis makes an original research contribution. A unique features of the research is that it investigates the experiences of children looked after at home, alongside those looked after away from home. In addition, the research involved two large local authority areas in Scotland that had not previously participated in such research on looked after children.
What emerged from the research was that the Corporate Parent (local authorities and partner agencies) had not yet successfully prioritised the educational achievement of looked after children in policy and practice, despite education being identified by the government as a mechanism for combating social exclusion. The key findings of the research demonstrated that looked after children perform less well academically than the general school population. In particular, placement type, the reason for becoming looked after and the age on becoming looked after were significant factors in determining educational achievement. Other factors such as gender and number of placements were also found to be associated with educational achievement. Empirical results further indicated that looked after children suffered from discrimination and social exclusion in all aspects of their lives, including school and where they lived. This was a significant finding as the disadvantage experienced by many looked after children continues to impact on their lives into adulthood, making them some of the most socially excluded adults in Scotland and the United Kingdom today.

▼ The main purpose of this study is to research about how the staff at a residental care home strives to make the unaccompanied youths feel like they are a part of the society in Sweden. The purpose is also to explain how the staff at the care home legitimises their integrating actions. To accomplish the purpose, a qualitative approach was made; four staffs and three youths were interviewed at a care home in the southern part of Sweden. The theoretical approach were: social integration and social exclusion. The theoretical concepts in this study were: rules, norms, social norms and moral norms. The findings of this study was that the staff, mentors and the guardians of the youths were people that helped them understand the ”new” society by teaching them the mutual norms, values and attitudes in the society. The norms, rules and routines that were taught at the care home, were interpreted as social and moral norms that sets the framework of a ”correct behavior” in a given situation. To make the system of norms appear as legitimate to the youths, the staff explained why the norms are important to internalize as well as explaining what could happen if you would go against them. In this way, the youths corrected their behavior in relation to what the staff mediated and to what they thought was important themselves. To understand how the society works, including learning the norms, values and behaviours, were according to the staff a way for the youths to create a good start into the Swedish society.

▼ Vår studie undersöker tolkens betydelse i kommunikationen mellan handledare på HVB och ensamkommande flyktingbarn. Vi fokuserade främst på möjligheter och brister som finns i tolkning idag, men även för att kunna förbättra handledarnas arbetssätt med barnen. Vi har genomfört en kvalitativ studie där vi har intervjuat fyra tolkar och fyra handledare på HVB. För att analysera vårt resultat har vi använt oss av tre kommunikationsmodeller och Habermas teori om det kommunikativa handlandet. Modellerna har använts för att förklara specifika tolkningssituationer mellan barnen och handledare, och Habermas teori har vi använt för att förklara tolkningen utifrån samhället. I studien framkommer det att tolkningen ger goda möjligheter för att skapa en kommunikation mellan två individer. De största bristerna är att det saknas tolkar i vissa språk och att det blir fel i dialekter. Många av handledarna menar att tolkarnas kompetens är bristfällig och att tolkarna behöver vidareutbilda sig. Det framkommer även att många upplever att det är en fördel om tolken har kunskap om kultur och samhällen för att kunna överbrygga mellan barn och handledare. I tolkningen förekommer sällan känslomässiga samtal och att det är individuellt om barnet vill visa sina känslor vilket beror på vilket förtroende de har till tolk och handledare. Det finns mycket att förbättra inom tolkningen och det saknas riktlinjer kring hur man ska genomföra tolkning.
Our study investigates the interpreter´s significance in the communication between assistants at residential care homes and unaccompanied refugeechildren. We had our focus mainly on the possibilities and the deficiencies that are in interpreting today, but also to improve the assistants’ way of working with the children. We have implemented a qualitative study where we have interviewed four interpreters and four assistants at residential care homes. To analyze our results we have used three communication models and Habermas´ theory of communicative action. The models were used for explaining specific situations with interpreters between assistants and children, and we used Habermas´ theory to explain interpreting by society. The study reveals that interpreting gives good possibilities to create a communication between two individuals. The largest deficiencies are that there is a lack of interpreters in some languages and there occurs flaws in dialects. A lot of the assistants say that the competences of the interpreters are deficient and that the interpreters need more education. The study also reveals that many of the interviewed experiences that it is an advantage if the interpreter has knowledge about culture and communities to create a bridge between children and assistants. In interpreting, emotional conversations rarely presents and it is individual if the child wants to show its feelings and it depends on the trust they have to the interpreter and assistant. There are a lot of improvements needed and there is a lack of guidelines to how assistants should implement interpreting.
