▼ Background: - even though sexual activity put adolescents at risk of various reproductive health (RH) challenges, many adolescent lack strong stable relation ships with parents or other adults with whom they talk to get information about their RH concern. Objective: - The general objective of the study is to identify factors affecting parents in discussing reproductive health issues with their adolescent. Method: - Community based cross-sectional study was designed to determine factors affecting parents in discussing RH issue with their adolescents among families who had children aged 10 years and above living in Shambo town conducted in May 2009. A total of 262 house holds were selected using systematic sampling technique. Data was collected using structured questionnaire by six data collectors from resident of Shamboo town. Data was analyzed by SPSS soft ware version 15.0. Descriptive statistics was employed to describe the study population in relation to relevant variables. Odds ratio with 95% confidence intervals and regression was employed to describe the strength of association between the selected study variables by controlling for the effect of possible confounders Result: - The study showed that most respondent 84.6% were found to be knowledgeable about reproductive health and also the majority 90% had positive attitude towards discussing the RH issue with their adolescent. However, small proportions 38.2% have communicated to their adolescent on reproductive health issue. Educational level, knowledge, and attitude of parents had significant association with discussion of parents with their adolescents on RH issues. Conclusion: - It can be concluded that, even though parents had basic information on RH, they did not practice in discussing RH issue with their adolescent. It is recommended that health workers in helping parents to become effective communicator to their adolescent as well as considering their role of educator should be considered. ▼ ABSTRACT Background The reproductive and sexual health of the youth remains a relatively new and sensitive area mainly due to restrictive norms and policies guiding the services and also the access and utilization of YFRHS among the college youth are dependent on many factors which include demographic, economic, college, socio-cultural and health system factors. This study was conducted to assess the utilization of youth friendly reproductive health services (YFRHS) among college youth in Asella town. Methods The study used quantitative approaches to collect data. The study was utilize survey research adapting descriptive cross sectional design and a questionnaire to interview 423 college youth in Asella town. Quantitative data was analyzed using Epi Info & Statistical Package for Social Sciences (SPSS). Descriptive statistics and chi-square tests were performed to determine significant associations. The study was performed for a total of six months starting from December 2014 up to May 2014. Result The study establish that sex, age, level of education, and youth’s awareness about existence of reproductive health facility and services. Age, sex, knowledge had no significant influence on utilization of almost all YFRHS such as family planning, voluntary counseling and treatment of STI. As indicated by the test result of family planning (χ2= 17.915, p = 0.593), VCT (χ2= 17.84, P = 0.598) and treatment for STIs (χ2= 24.336, p = 0.228) they had no significant association to utilization of the service. Again age was not also significant association with knowledge of YFRHS and their utilization χ2= 38.968, P = 0.472. On sex and utilization, significance was not seen in sex and utilization of family planning because (χ2= 4.53, P=0.104). Conclusion and Recommendation The recommendations arising from this study are: this study recommends active sensitization of the youth in college through different activities that creates an opportunity where such information can be shared to scale up their knowledge on the YFRHS and the facilities that are available, this will in turn increase utilization of the services Advisors/Committee Members: Fekadu Aga (BSc, MSc,) (advisor).

▼ In Zambia, approximately 7.2% of sexually active girls, aged 15-19, reported having had a sexual partner. The National Health Strategic Plan 2017-2021 highlighted the high adolescent birth rate which was at 141 per 1000 live births between the year’s 2013 and 2014. According to the laws of Zambia, an adolescent girl can consent for sex at the age of 16 and above; early childbearing is any pregnancy before the age of 16 which is the focus for this study. This study was investigating factors that influence early childbearing among adolescent girls in some selected rural districts of Zambia at baseline of the RISE cluster randomised trial. The investigated factors included; cultural beliefs, social economic factors, knowledge and access to reproductive health information and peer pressure. The study was a nested quantitative cross-sectional design embedded in a Cluster Randomised Trial of the Research Initiative to Support the Empowerment (RISE) of girls. Participants were girls attending grade seven at various schools in 2016. Information on factors that could influence the participants’ view on childbearing were collected at baseline. A complete enumeration of approximately 4900 girls recruited in 12 rural districts of Central and Southern provinces was analysed. Descriptive statistics are presented as frequencies and percentages in tables. To check for the association between the outcome variable which was categorical in nature and the independent variables, Chi-squared test was used or Fisher’s exact test for frequencies which were less than five. Univariate and Multiple Logistic regression analysis was used to test the association between independent factors and early childbearing. Clustering in the study was accounted for using robust standard errors. Results: The findings showed that more than two-thirds 3189 (73.7%) of the participants had no knowledge regarding adolescent sexual and reproductive health information on family planning use, only 1137( 26%) had knowledge. A number of adolescents with mobile phones 235(77.6%) agreed to engaging into early childbearing practices as opposed to those who had none. Majority of adolescent girls 227 (69%) who agreed to early childbearing were aged 10 to 15 years, whilst the least number of adolescents who agreed to early childbearing were 99(30.3%) in the age ranges of 16 to 20 years. In addition, results reviled that one-year increase in age of an adolescent girl increases early childbearing by 30% (OR=1.30, 95% CI (1.01, 1.67), p- value=0.03). Overall, very few factors (mobile phone and age) at baseline were found to be associated with child bearing. In conclusion, Only mobile phone use and age were associated with early childbearing at baseline. Cultural norms and socio-economic factors were not associated with early childbearing in this study. ▼ Outcome valuation and economic evaluation in maternal and reproductive health care can be complicated by the fact that: 1. Alternative interventions can be geared towards enhancing the experience of women undergoing care, rather than improving health status; and 2. Alternative interventions can directly affect the health and wellbeing of both woman and infant(s) This thesis explores methods for overcoming these challenges. A review of the literature indicates that: 1) while stated preference techniques have been used to value patient experience in the context of reproductive health care, few studies have incorporated such values into full economic evaluations; 2) while health state valuation techniques have been adapted to value combined mother-infant health outcomes, these techniques have never been used to value outcomes of alternative approaches to assisted reproduction; and 3) no economic evaluations in this area have explicitly considered preferences for the distribution of benefits between women and newborn infants. These gaps are addressed though three empirical case studies. The first demonstrates a method for measuring and incorporating women’s preferences for patient experience factors into an economic evaluation comparing alternative approaches to labour management. The second assesses women’s preferences for mother-infant health outcomes in the context of an economic evaluation comparing alternative approaches to IVF treatment. The final case study adopts an approach to estimate the relative monetary value that community members place on the prevention of maternal and neonatal deaths, in the context of decisions between competing health care programmes that impact differentially on these outcomes. The case studies show that the challenges identified above can be overcome by adapting one or other of the stated preference valuation techniques available to health economists. DCEs in particular offer a flexible approach to dealing with the complexities and trade-offs that can arise when considering choices between alternative reproductive health care interventions in the context of scarce resources.

▼ Background- Birth preparedness and complication readiness is a strategy to promote the timely use of skilled maternal and neonatal care, especially during childbirth. Based on the theory that preparing for childbirth, and being ready for complications reduces delays in obtaining this care. Lack of advance planning for use of a skilled birth attendant for normal births, and particularly inadequate preparation for rapid action in the event of obstetric complications, are well documented factors contributing to delay in receiving skilled obstetric care. Birth preparedness has been globally endorsed as an essential component of safe motherhood programs to reduce delays for care, so reduced maternal mortality rate and neonatal mortality rate. Objective: To assess knowledge and practices with respect to birth preparedness and complication readiness among antenatal clients in selected Addis Ababa Health Centers. Methods: A cross-sectional institution based study was conducted to examine knowledge and practice of birth preparedness and complication readiness among clients at five selected health centers by using pretested standard questioner in Addis Ababa. The sampling technique was systematic sampling and the sample size was 290. Results: Data were obtained from 290 mothers, yielding a response rate 100%.Taking into account identifying danger sign during pregnancy, labor and during postnatal, and place of delivery identification, means of transportation and saving money. About 42%, 17.2%, 27.7% of the respondents were knowledgeable about danger sign during pregnancy, delivery, and postpartum period respectively. Regarding to knowledge to birth preparedness 46% of the respondents were knowledgeable and 68.0% stated that they were prepared for birth and its complication. In this study educational status of mothers and their knowledge about danger sign during pregnancy was associated. Illiterate mothers were 0.48 times less knowledgeable about VIII danger sign during pregnancy as compared to those who were secondary and above (OR=0.42(95%CI=0.21, 0.82). Mothers with the income of 500-1000 Birr per month were 2.86 times more likely to have knowledge about danger sign during pregnancy as compared to those who were with no income(COR=2.86 (95% CI=1.41,5.78)) but the significant association between educational status, income and danger sign during pregnancy was not retained in the multivariate analysis. Conclusion and recommendation-The study identified poor comprehensive knowledge and practices on preparation for birth and its complication in the study area. Community education about preparation for birth and its complication and empowerment of women through expansion of educational opportunities are recommended for improving birth preparedness and consequently the effects of pregnancy related complications. The individual woman, her family and the community as a whole should have to be well informed on birth preparedness and complication readiness. ▼ Youths in Zambia have limited access to information concerning Sexual Reproductive Health (SRH) and this puts them at risk of unwanted pregnancy. The Research Initiative to Support the Empowerment of Girls (RISE) is a cluster randomized testing the effectiveness of different support packages on teenage pregnancies, early marriages and school dropout. One of the support packages included youth clubs focusing on comprehensive sexual and reproductive health education (CSRHE). Although similar interventions have been implemented in other settings, their integration process has been complex and comprehensive assessments of factors shaping acceptability of youth clubs is lacking. Through the use of diffusions of innovations theory, this paper qualitatively aimed to identify factors that shaped the acceptability of CSRHE youth clubs in rural schools in Central Province. A qualitative case study was conducted in which data gathered through eight focus group discussions from grade eight pupils, eight key informant interviews with teachers and document review were analyzed using thematic analysis. The perceived relative advantage of youth clubs and the simplicity related to the use of participatory learning methods, films and role plays to communicate sensitive reproductive health information and made the learners like the youth clubs. Further the perceived compatibility of the content of the sessions with the science curriculum increased the learners’ interest in the youth club as the meetings also helped them prepare for the school exams. However, cultural and religious beliefs among teachers and parents regarding the use of contraceptives complicated the delivery of reproductive health messages and the acceptability of youth club information among the learners. The study has demonstrated that acceptability of SRH interventions such as youth clubs in some schools of Central Province may be successful if pupils and teachers use interventions that depict real life. Teachers and pupils appreciated the introduction of CSRHE youth clubs and the provision of economic support to girls and their families that led to the reduction of early marriages, school drop-out and early pregnancies. ▼ A lack of contraception use and contraceptive method discontinuation are common causes of unintended pregnancy in the Eastern Cape. The most common reason for method discontinuation among childbearing women is the unacceptable side effects of their contraceptive choices. Both depression and sexual dysfunction are given as side effects of contraceptive use; however, there is little evidence to support these effects. This randomised, single-blind controlled trial conducted in East London, South Africa, Investigated the effects of the initiation of a long-acting injectable contraceptive, Depot Medroxyprogesterone Acetate (DMP A), compared with the initiation of a copper Intrauterine Contraceptive Device (Cu-IUD) after childbirth on depression and sexual functioning. After counselling, 242consenting pregnant women were randomised to receive DMP A or a Cu IUD within 48 hours of childbirth, in a ratio of 1:1. Primary outcome measures were depression and sexual dysfunction evaluated by validated instruments. Questionnaires were administered at baseline, and telephonically at one month and three months after randomisation. The telephonic interviewer was blinded to the participants' group allocation. English and Xhosa versions of the Beck Depression Inventory and the Edinburgh Postnatal Depression Scale were used to assess depression. The Arizona Sexual Functioning Scale was used to assess sexual functioning. For these primary outcomes, median scores between the intervention groups were compared, as well as the number of events (dichotomous data) in each intervention group. There relative effects of these interventions were summarised by calculating risk ratios, with 95% confidence intervals. Statistical tests used included the Shapiro-Wilk test, T-test, and Wilcoxon test. There were not consistently statistically significant differences in the risk of depression or sexual dysfunction between the intervention groups in this study. However, there was a trend towards more depression in the DMPA group which was statistically significant for mean EPDS score at the one month and for the BDI score three month assessments compared with the IUD group. There was also a trend to more sexual dysfunction with DMPA, but the only statistically significant difference was that fewer women in the DMPA group resumed sexual intercourse within the first month of treatment than in the IUD group. The author's recommendations from the study are that, firstly, family planning providers should inform women during contraceptive counselling that there is no certainty that DMPA causes depression and/or sexual dysfunction; however, it may do so in the postpartum period. Secondly, contraceptive users can continue to use DMPA with confidence as a convenient and effective method of preventing unintended pregnancy. Adolescence is a crucial development stage where both biological and psychological changes take place. Among the various transformations and developments, the discovery of sexuality and intimacy has an important role, especially from sixteen to nineteen years old. The construction of sexual identity is a significant bio-psycho-social development(Kar, 2015), however in Italy almost half of the teenagers have experienced a form of violence from their partners during High School raging from insults to physical aggression (Romito, 2013). The literature agrees that experiencing rejection is a common trigger of aggression with significant repercussions of mental and physical health. Therefore to understand better the phenomenon, its causes and consequences, a combination of desk research and field research was carried out. The results indicated that rejection is lived on an ego level touching the pride of people and therefore it is considered a taboo with strong effects on different levels of society. Based on this analysis, to improve the current situation, it was chosen to focus on the community level by triggering open conversation about rejection. All these insights gained from the analysis were presented to a team of experts and teenagers. Their feedback was helpful to understand the fundamental characteristics that the final design should have to trigger meaningful conversation around the experience of rejection. To discuss how media portraits rejectionFEIC is introduced. #FEIC is a service that provides teenagers with a showcase of media narratives of rejection and interactive activities. The showcase aims to get the teenagers familiar with the idea of media narratives, while the goal of the activities is to make them experiment with them. In the current development, one activity is designed and deepened to use media narratives as a trigger to talk about how society deals with rejection and how that affects teenagers and their thinking. Different variations of the activity were tested to understand what type of interactions were triggering more discussions. Moreover, to evaluate the overall experience and design features, the full service was presented to the experts through in-depth interviews. The feedback gained demonstrated that the use of media is a successful way to trigger meaningful discussion around rejection. The dialogue generated from the #FEIC activity was defined as “different” from what teenagers were used to discussing, “stimulating”, and “useful” to understand better the conditioning behind their judgment.

Unintended pregnancy is a major public health issue in the United States (U.S.). It is estimated that 23% of women with unintended pregnancies had inadequate knowledge or inconsistently used contraceptives. Despite the existence of contraceptive counseling and educational interventions, women of childbearing age still lack adequate knowledge of family planning methods. The primary aim of the current quality improvement (QI) project was to implement a structured contraceptive counseling intervention to enhance knowledge of family planning and promote contraceptive compliance among a convenience sample of childbearing women who visit an Obstetrics and Gynecology (OB/GYN) clinic and potentially decrease unintended pregnancies. A structured counseling intervention was implemented using an educational pamphlet that described birth control methods. The project leader actively educated the participants and allowed them to take home the pamphlets used in the counseling session. The Contraceptive Knowledge Assessment (CKA) questionnaire was used to measure the participants’ knowledge of family planning before and after the structured counseling sessions to determine the effectiveness of the intervention. The Satisfaction with Decision (SWD) questionnaire was used to measure the participants’ compliance after the implementation of the structured contraception counseling to assess the levels of compliance regarding the decision made about family planning. The theoretical foundation of the current QI project was based on the Health Belief Model (HBM). Statistical analysis was conducted using independent samples t-test at .05 level of significance. ▼ Over the last few decades, there has been little evidence of young people accessing information necessary for Sexual Reproductive Health decision-making. Adolescent sexual and reproductive health (SRH) has increasingly been on national health agendas of many countries, including Zambia. One of the major causes of these agendas is the concern against early childbearing. Access to youth-friendly health services such as contraceptives is vital for ensuring the well-being of adolescents. This study would help determine the role of contraception in preventing early childbearing among young people aged 16-19 years who were sexually active. The study was a cross-sectional sample survey covering 369 urban secondary school adolescent female pupils (Grades 10-12) aged 16-19 years and some members of the urban community in Mazabuka. It employed a mixed-method approach in which both quantitative and qualitative techniques were used. These approaches were treated independently at data collection and analysis. A semi-structured questionnaire was used to determine the level of modern contraceptive use among adolescent female pupils as an outcome variable. Focus group discussions (FGDs) with purposively selected parents and interviews with purposively selected key informants (school guidance teachers and SRH personnel) provided data on community perceptions on adolescent contraceptive use. The results show that the prevalence of modern contraceptive use among adolescent females stood at 17% (65/369) whereas the prevalence of adolescents who regularly indulged in sexual activity was (26.8%). Frequency of sex (OR 4.41, 95% CI 2.9, 6.6, p-value <0.001), visits to a health facility (OR 2.63, 95% CI 1.2, 5.9, p-value 0.019) and values and belief systems (OR .18, 95%CI .1, .37, p-value <0.001) are the major factors that had significant association with contraceptive use. Secretiveness and fear of social stigma were cited as reasons for adolescents not accessing modern contraceptive services. Findings also showed existence of myths and beliefs held by both the adolescents and the community, relating to lack of information on some effects of modern contraceptive use and access to relevant SRH services. Adolescents largely did not visit health facilities for alternative services. Various community values relating to cultural and religious backgrounds exerted an influence on decision-making among adolescents regarding the use of modern contraceptives. Socio-cultural values played an important role in relation to adolescent response to contraceptive use. Adolescent access to SRH services and the decision whether or not to use them is primarily influenced by other people from the socio-cultural environment of Mazabuka. This study demonstrated the need for comprehensive SRH services accessible to adolescents in Mazabuka district. Data were collected from three health centres namely, Chelston, Chipata and Ngo’mbe clinics. Theoretical purposive sampling interleafed with maximum variation sampling was used to enlist respondents. In total there were forty (40) participants that participated in the study. In –depth interviews were conducted with thirty-six (36) were adolescents seeking a Safe Motherhood or Reproductive Health Service, three (3) Health Care Providers from the health centres and one (1) key informant from Ministry of Health and (MOH). Results: The study established that adolescents had unmet behavioural, psychological, emotional, developmental and gender-specific sexual and reproductive health needs and concerns. In addition health care providers faced considerable challenges that hampered their ability to adequately offer SM and RHS. Conclusions: The study concluded that there was a service gap in the provision of adolescent SM and RHS services. Existing health services emphasized post-exposure SM and RH as opposed to pre-exposure services. Research Policy Implications: The study recommends undertaking a qualitative survey with a larger number of adolescents to solicit for more usage of SM and RHS. There is need to develop advocacy programmes that could reach out to both the boys and girls on a larger scale throughout Zambia. Other recommendations would include strengthening of out-reach services, regular training and in-servicing of health providers, health care givers and counsellors to effectively serve adolescents. The study further recommends the need to strengthen public-private sector partnership and stake holder participation in adolescent health, continuous gathering of data on SM and RHS. All these have implications for immediate and future reproductive health of adolescents, and for bridging the SM and RHS services gap across the life span.

Background As a region, Francophone sub-Saharan Africa (SSA) continues to have among the highest fertility rates and lowest contraceptive prevalence rates in the world. As of the latest Demographic and Health Survey (DHS) data available for each country, total fertility rates range from 4.1 (Gabon 2012) to 7.6 births per woman (Niger 2012). Modern contraceptive prevalence rates for married women range from a low of 3.3% (Guinea 2012) to a high of 20.1% (Senegal 2014). Methods Three analyses were completed utilizing nationally representative DHS data available for each Francophone SSA country. For each paper, bivariate and multivariate analyses were performed with weighted data to take into account the two-stage cluster sampling design of the DHS survey. In paper one, we determine whether being Muslim and/or being in a polygynous union impacts the use of modern contraceptives. In paper two, we provide detailed information on the characteristics that define users of specific contraceptive methods in this region. In the final paper, we provide a clearer picture of the reproductive health dynamic in young never married women in Francophone SSA. Results In paper1, we found that religion and marital type generally were not predictive of modern contraceptive use. The typical predictors – education, age, residence (urban/rural) – were consistently significant across the included countries. In paper #2, profiles of users of the most common contraceptive methods in each country were created. For example, condom users had the most consistent profile across the 12 countries, with marital status, parity and education being clearly associated with condom use. For paper #3, education level was found to be the key predictor of both sexual activity and becoming pregnant for young, never married women in this region. In general, those with primary or secondary and higher education were more likely to be sexually active than those with no education. Conclusion These findings provide valuable information to government, private and nongovernmental organizations working toward increasing the numbers of contraceptive users in Francophone SSA.

ENGLISH ABSTRACT: Literature presents a complex and often contradictory picture of the link between psychological stress and male fertility. Very limited information is available on the specific relationship between anxiety level and male fecundity. University students, when compared to the general population, exhibit signs of decreased mental health, with students having increased prevalence of depression, anxiety, psychosis and addictions. The aim of the study was to determine if there is a relationship between anxiety and reproductive parameters in a male student model and what that relationship is. Twenty-one male participants between the ages of 20 and 25 were recruited as a part of this study. Participants were asked to donate at 4 different time-points. At each time-point, a semen sample and blood sample were collected and a State Trait Anxiety Inventory questionnaire was filled in. A semen analysis, presented in a spermiogram report, was performed using computer-aided sperm analysis to gather information about the basic semen parameters e.g. sperm concentration, sperm motility and sperm kinematics. The percentage fragmented sperm DNA, sperm nitric oxide level and semen reactive oxygen species levels were investigated. The cytokine profile and cortisol level were investigated in both the seminal plasma and blood plasma. The student population observed in this study displayed increased anxiety levels when considering their state (39.0, SD=13.69, n=83) and trait (43.0, SD=12.55, n=83) anxiety scores, respectively. Blood plasma cortisol levels were noted to increase as state and trait scores increased. Cortisol was thought to have an overall immuno-suppressant effect both locally, in the reproductive tract, as well as systemically. Round cells were positively correlated to blood plasma cortisol levels and trait anxiety levels. Throughout the study it was observed that average path velocity (VAP) and straight-line index (STR) kinematic parameters decreased as state anxiety scores, trait anxiety scores and blood plasma cortisol levels increased. An inverse relationship between total motility and trait anxiety was observed. The inverse relationship observed between elevated blood plasma cortisol and decreased spermatozoa viability and spermatozoa motility parameters is possibly a function of the increased number of round cells observed during this study. These findings suggest a possible role for increased anxiety or psychological stress to elucidate idiopathic infertility by means of affecting the cytokine profile. The increase in number of round cells in this study could therefore be a result of increased release of immature germ cells from a compromised blood-testis barrier. The changes in the cytokine profile, round cells in the semen and sperm motility often correspond to values observed in men presenting with idiopathic infertility. It can be concluded from this study that an increase in anxiety levels were found to have a negative relationship with male reproductive parameters.

▼ The rate of adolescent unintended pregnancy in the United States is high compared to other developed countries. While past research and interventions have largely focused on young women, the role of young men in pregnancy prevention has increasingly been recognized. Studies have assessed young men’s knowledge and attitudes toward pregnancy prevention as well as their role in male-controlled methods of birth control like condoms or withdrawal. However, less is known about how young men can contribute to or participate in decision-making with female partners about contraception other than condoms. The purpose of this systematic review was to explore how young men communicate with their partners and its impact on contraception use to prevent pregnancy. A systematic review of five databases was conducted to identify English-language articles published from January 1, 2002, through July 7, 2018. The review specifically explored how male partner communication affects female partner use of contraception other than condoms among young men ages 11-24 years. The systematic review explored additional questions, including those pertaining to the timing of partner communication in a relationship, strategies employed by young men, and which dynamics of partner communication are measured in studies. Of the 15 articles identified as exploring areas of communication, five of the articles used quantitative analysis to measure any association between partner communication and contraception use, and three of those produced statistically significant findings suggesting that communication increases the use of contraception other than condoms. Three qualitative studies provided supporting narratives of how young men have communicated with partners and influenced their contraception use with female partners. The remaining seven articles explored the other research questions of timing, strategies, and dynamics identifying topics, prompts, and communication cues among young men. Measurements of both communication and contraception varied across studies. With the small number of studies identified in this systematic review, it is recommended that future research seek to corroborate the relationship between partner communication and contraception use with more robust and precise measurements of both communication and contraception.
