▼ Izhodišče – Namen naše dvojno slepe randomizirane raziskave je bila primerjava hemodinamskih učinkov z bispektralnim indeksom (BIS) vodene infuzije etomidata in propofola pri bolnikih, predvidenih za veliko operacijo v trebušni votlini. Metode – 46 bolnikov, predvidenih za veliko operacijo v trebušni votlini zaradi raka, smo naključno razporedili v dve skupini. Skupina E je za uvod v anestezijo dobila infuzijo etomidata, skupina P pa infuzijo propofola. Oba anestetika smo odmerjali do vrednosti BIS 60. Hitrost infuzije je bila 0,5 mg/kg/min za propofol in 0,05 mg/kg/min za etomidat. Meritve srednjega arterijskega tlaka (SAT), indeksa minutnega volumna srca (MVSI), indeksa sistemskega žilnega upora (SŽUI) in srčne frekvence (SF) smo začeli eno minuto pred odmerjanjem fentanila in jih nato ponavljali vsako minuto v naslednjih 20 minutah. Prav tako smo merili čas od začetka infuzije anestetika do: izgube palpebralnega refleksa, vrednosti BIS 60 in vstavljanja dihalne cevke. Zapisali smo porabljen odmerek anestetika. Vrednost p < 0,05 smo obravnavali kot statistično značilno. Rezultati – V analizo je bilo vključenih 46 bolnikov. Pred in med vstavljanjem dihalne cevke glede SAT, MVSI, SŽUI, SF, BIS in saturacije ni bilo statistično značilnih razlik med skupinama, le vrednost SAT je bila v skupini E med vstavljanjem dihalne cevke statistično značilno večja glede na skupino P (p = 0,019). Po vstavitvi dihalne cevke je bila vrednost SAT v skupini E statistično značilno večja od vrednosti v skupini P 1 (p = 0,002), 2 (p = 0,000), 3 (p = 0,002), 4 (p = 0,002), 5 (p = 0,018), 6 (p = 0,012) in 7 (p = 0,004) minut po vstavitvi. V skupini E je bila vrednost MVSI statistično značilno večja 2 (p = 0,039), 6 (p = 0,038) in 7 (p = 0,024) minut po vstavitvi dihalne cevke, medtem ko so bile razlike v vrednosti MVSI med skupinama 3 (p = 0,093), 4 (p = 0,055) in 5 (p = 0,068) minut po vstavitvi dihalne cevke na meji statistične značilnosti. V skupini E so bile večje kot v skupini P. Vrednosti SŽUI in SF se celoten čas po vstavitvi dihalne cevke med skupinama niso statistično značilno razlikovale. Med vstavljanjem dihalne cevke sta se vrednosti SAT in SF v skupini E statistično neznačilno razlikovali od izhodne vrednosti, medtem ko je bila vrednost SAT v skupini P statistično značilno nižja od izhodne vrednosti, vrednost SF pa se od izhodišča ni statistično značilno razlikovala. V obeh skupinah je bila vrednost MVSI v času od 3. minute pred vstavljanjem dihalne cevke do konca meritev statistično značilno manjša od izhodne vrednosti. Pri 23 bolnikih so se pojavili zapleti, ki so zahtevali rešilne odmerke zdravil: pri 10 bolnikih v skupini P in pri 13 v skupini E. Glede na pojav zapletov se skupini v splošnem nista razlikovali (p = 0,376). V skupini P je bilo več hipotenzije (p = 0,08), v skupini E pa statistično značilno več hipertenzije (p = 0,007) Zaključek – Z raziskavo smo ugotovili, da je uporaba propofola za uvod v anestezijo povezana z manj hipertenzije in tahikardije med vstavljanjem dihalne cevke v primerjavi z uporabo etomidata.… Advisors/Committee Members: Kamenik, Mirt.

Background: Unconscious processing of words during general anaesthesia forelective surgery has been suggested, even though patients cannot recall anyintraoperative event.Objective: We used the Process Dissociation Procedure (PDP) to test memoryperformance during sevoflurane and propofol anaesthesia, the relation of memory tohypnotic depth, surgery itself, and also haemodynamic and breathing variables.Material-Methods: After induction of general anaesthesia for elective surgery 100patients (50 anaesthetised with propofol and 50 with sevoflurane) listened to a list ofwords. The Bispectral Index (BIS), breathing and haemodynamic variables wererecorded throughout the study. Within 36 h after word presentation, memory wasassessed using a word stem completion task, based on Buchner’s model for memorycombined with the PDP. We applied the same task to a group of non-anaesthetizedsubjects.Results: Results were derived from 45 patients from the sevoflurane group, 48 fromthe propofol group, and another 45 from subject who did not receive generalanaesthesia. The mean BIS (± SD) was 45,15 (± 8,97). There was evidence ofmemory for words presented during light (BIS 61–80), (p=0.001) and adequate (BIS41–60), (p=0.008) but not deep anaesthesia (BIS 21–40), (p=0.09). The PDPshowed a significant implicit but not explicit memory contribution (mean total explicitmemory scores: 0.04±0.07 in all BIS categories; mean implicit memory scores:0.01±0.04, 0.1±0.08, and 0.05±0.09 at BIS=21–40, 41–60, and 61–80, respectively).There was a statistically significant difference between the mean implicit memoryscore (I) of the propofol and sevoflurane group in the BIS category 41-60 in general(p = 0.016), and after incision (I A.I.) (p = 0.005) in particular, with propofol depressingI more than sevoflurane in both cases. Memory performance of nonanaesthetizedparticipants was better, with a higher contribution of explicit and a comparablecontribution of implicit memory. Conclusion: During general anaesthesia, implicit memory persists even in adequatehypnotic states to a degree comparable to the absence of anaesthesia. Sevofluraneaffects the implicit memory of adequately anaesthetised subjects less than propofol.Surgical stress reinforces implicit memory. Breathing and haemodynamic variablesseem to have no effect on memory of anaesthetised patients.

Introduction: The safe and efficient administration of anesthesia during laboratory interventional procedures such as colonoscopy is nowadays mandatory, in order to provide the patient with sufficient comfort during the process and additionally to ensure the best possible working conditions for the gastroenterology team.Objective: The aim of our study was the comparison of the results from the application of the Bispectral index (BIS) by an anesthesia nurse in order to sedate patients undergoing colonoscopy with propofol, in contrast to sedation of patients with midazolam / fentanyl by an endoscopic laboratory nurse.Methods: A randomized, double- blind study included patients who underwent a colonoscopy procedure conducted in the Evgenidion University Hospital during one year. The studied population included 100 adults that had been divided into two groups: the first group received the usual scheme of intravenous sedation with midazolam and fentanyl administered by a member of the endoscopic team that was blind to the Bispectral Index values recordings (group A, n= 50) and the second group received intravenous bolus injection of propofol by anesthesia nurse based on the BIS values (group B, n= 50).Results: In group B, 25 patients (50%) scored 1 at the Observer's Assessment of Alertness/Sedation scale (OAA/S score). In group A, 25 patients (50%) had OAA/S score 3 and 12 patients (24%) had OAA/S score 2. The mean average of the BIS values of group A was 85.07 and for group B was 76.1 (p=0.00). The percentages of reactions, incidents of awake clinical state and display of memory of the patients in group A were higher than the percentages of patients in group B (p <0.05). In group B, occurred one episode of oxygen desaturation (SpO2 = 88%) and four episodes of hypotension during colonoscopy. Furthermore, the mean onset time of anesthesia and awakening time was shorter for patients in group B compared with the patients in group A. The cost of anesthesia drugs administered to patients in group A was greater compared with group B, while the degrees of satisfaction of patients, endoscopist and endoscopic laboratory nurse were higher in group B compared with group A.Conclusion: The comparison between the two groups showed that the sedation offered by an anesthesia nurse was safe and qualitative, offering particular comfort to the patient during the procedure and contributing significantly to its successful results, in contrast to the hitherto daily clinical practice which is usually applied.

▼ Music has been widely used in human anaesthesia, due to low cost, ease and safety. Additionally, its beneficial effect on physical and biochemical parameters, analgesia, and minimization of adverse effects of anesthetics and analgesics, makes perioperative exposure to music an attractive complementary tool during anaesthesia.The aim of this research was to study the effect of music, provided during the perioperative period, on factors associated with anaesthesia and analgesia in dogs exposed to nociceptive stimuli under experimental and clinical conditions. In the first part, 20 Beagles were applied to identical nociceptive stimuli: 3 groups were formed, each of which consisted of the same 20 dogs. The first group listened to Chopin Music (CM), the second to Mozart Music (MM) and the third was the No-Music (NM) group. Dogs were premedicated with acepromazine (0.03 mg/kg) and butorphanol (0.1 mg/kg) intramuscularly; anaesthesia was induced with propofol and maintained with isoflurane while fentanyl (1 μg/kg) was provided intravenously as rescue analgesia. Anesthetic depth was monitored by use of the bispectral index (BIS), and autonomic nervous system responses (HR, BP, RR) were used to monitor the level of analgesia. Furthermore, measurements of Substance-P concentration in the dogs' serum were carried out in order to objectively evaluate pain during the intraoperative period. In the second part of the study, 24 female dogs, undergoing ovariohysterectomy, were divided into two groups of 12. Dogs of the first group were exposed to Mozart Music (MM), while those of the second were not exposed to any musical stimulus during the perioperative period (NM). The experimental design was almost identical to the first part. The BIS was not used, however the parameters evaluated were the same as in the first part of the study.In the first part of the study, the groups of dogs exposed to music achieved higher levels of sedation, required less propofol, isoflurane and fentanyl and exhibited less pronounced changes in BIS values (p <0.05), in relation to the group not exposed to music. In the second part of the study, the dogs of the MM group exhibited less fluctuations in arterial pressure than those the NM group (p<0.05). Finally, a statistically significant effect of time on the alterations of Substance-P concentrations was observed, regardless of exposure to music or not.Classical music provided during the perioperative period in laboratory dogs, results, when compared to dogs not exposed to music, in higher levels of sedation, lower consumption of propofol, isoflurane and fentanyl and less alterations in BIS values. Also, exposure to Mozart's music resulted in less fluctuations of substance P concentrations. However, these positive effects of classical music were apparent in light but not deeper anaesthesia in dogs subjected to ovariohysterectomy, probably due to dose-dependent suppression of auditory function caused by isoflurane. Introdução: O uso de terapia sedativa e analgésica, freqüente em UTIs, torna imprescindível constantes re-avaliações do nível de sedação dos pacientes. Vários instrumentos para medir sedação têm sido utilizados, de escalas clínicas à monitorização do nível de consciência. Objetivo: Comparar escores de sedação resultantes da escala COMFORT-B, aplicada por avaliadores independentes, com o Índice Biespectral (BIS), em crianças internadas na UTI pediátrica do Hospital de Clínicas de Porto Alegre, submetidas à ventilação mecânica e ao uso de medicação sedativa. Pacientes e métodos: Foram avaliadas crianças de 1 mês até 16 anos de idade, divididas em 2 grupos (com e sem dano neurológico), submetidas à ventilação mecânica e uso de sedativos. Foram monitorizadas e classificadas pelo BIS e pela escala Comfort-B, no mesmo momento, tendo-se obtido o registro de seus comportamentos em filmagem digital. Posteriormente, o filme foi utilizado para a classificação da sedação dos pacientes por outros 3 observadores, de modo independente. Foram comparados os resultados dos observadores e testada a concordância entre eles (métodos de Bland-Altman e Kappa), como também testada a correlação dos dados obtidos pela monitorização e pela escala (teste de Correlação de Pearson). A pesquisa foi aprovada pelo Comitê de Ética e Pesquisa da instituição e não houve conflito de interesses envolvido na pesquisa. Resultados: Foram realizadas 70 observações em 19 pacientes, sendo 11 sem e 8 com dano neurológico. Na classificação pela escala houve uma concordância alta entre os avaliadores (mínimo de 94,3% e máximo 97,1%), com valores de Kappa entre 0,542 e 0,683 e p<0,001. Houve uma associação positiva regular entre BIS e Comfort-B, estatisticamente significativa, tanto no grupo com dano neurológico (r = 0,720 a 0,914 e p<0,001) como no sem dano neurológico (r = 0,424 a 0,498 e p = 0,002 a 0,011). Conclusão: Em função da alta concordância entre os avaliadores independentes, e da correlação entre a escala Comfort-B e o Índice BIS, forte no grupo com dano neurológico e regular no grupo sem dano neurológico, pode-se concluir que a escala Comfort-B é reprodutível e útil na classificação do nível de sedação de crianças em VM.

▼ Background: Analgesia is an essential element of general anaesthesia. Optimum dosage requires reliable methods for continuous determination of the level of analgesia. However, clinical assessment of the actual level of analgesia is only possible discontinuously by means of movement reactions and heart rate increases to noxious stimuli, both being signs of inadequate analgesia. In this study, we therefore investigated whether various candidates are able to predict movement reactions and heart rate increases to clinical noxious stimuli for continuous monitoring methods of analgesialevels. Methods: We measured the Nociceptive Flexion Reflex Threshold (NFR-T), Bispectral Index (BIS), Composite Variability Index (CVI), Noxious Stimulation Response Index (NSRI) and the calculated propofol/remifentanil effect-compartment concentrations (Ce) during general anaesthesia in 50 female subjects undergoing breast surgery. For each, it was investigated whether these parameters were able to predict movement reactions and heart rate increases to the clinical noxious stimuli laryngeal mask airway insertion and skin incision. Results: The NFR-T is capable to predict movement reactions as well as heart rate increases to both noxious stimuli (Prediction probability PK 0.68 – 0.77). The NSRI enables the prediction of movement to skin incision (PK 0.76), but not to laryngeal mask airway insertion (PK 0.49) and no heart rate increases (PK 0.48 – 0.53). Effect- compartment concentrations have the ability to predict movement (PK 0.65 – 0.72) but no heart rate increases (PK 0.42 – 0.58). Neither BIS nor CVI can predict any reactions (PK 0.31 – 0.59). Conclusion: Solely the NFR-T provides a prediction for both tested types of nociceptive responses under general anaesthesia. However, due to the immense effort its use in clinical practice is inadvisable. Potentially the NFR-T can be enhanced to a gold standard model of measuring analgesia under general anaesthesia. The NSRI and the effect-compartment concentrations enable the prediction of at least one part of the nociceptive reactions and can be recommended for use in clinical practice due to their low effort. The fabrication of biomimetic heart muscle suitable for pharmaceutical compound evaluation and disease modeling is hindered by limitations in our understanding of how to guide and assess the maturity of engineered myocardium in vitro. We hypothesized that tissue architecture serves as an important cue for directing the maturation of engineered heart tissues and that reliable assessment of maturity could be performed using a multi-parametric rubric utilizing cardiomyocytes of known developmental state as a basis for comparison. Physical micro-environmental cues are recognized to play a fundamental role in normal heart development, therefore we used micro-patterned extracellular matrix to direct isolated cardiac myocytes to self-assemble into anisotropic sheets reminiscent of the architecture observed in the laminar musculature of the heart. Comparison of global sarcomere alignment, gene expression, and contractile stress in engineered anisotropic myocardium to isotropic monolayers, as well as, adult ventricular tissue revealed that anisotropic engineered myocardium more closely matched the characteristics of adult ventricular tissue, than isotropic cultures of randomly organized cardiomyocytes. These findings support the notion that tissue architecture is an important cue for building mature engineered myocardium. Next, we sought to develop a quality assessment strategy that utilizes a core set of 64 experimental measurements representative of 4 major categories (i.e. gene expression, myofibril structure, electrical activity, and contractility) to provide a numeric score of how closely stem cell-derived cardiac myocytes match the physiological characteristics of mature, post-natal cardiomyocytes. The efficacy of this rubric was assessed by comparing anisotropic engineered tissues fabricated from commercially-available murine ES- (mES) and iPS- (miPS) derived myocytes against neonatal mouse ventricular myocytes. The quality index scores calculated for these cells revealed that the miPS-derived myocytes more closely resembled the neonate ventricular myocytes than the mES-derived myocytes. Taken together, the results of these studies provide valuable insight into the fabrication and validation of engineered myocardium that faithfully recapitulate the characteristics of mature ventricular myocardium found in vivo. These engineered tissue design and quality validation strategies may prove useful in developing heart muscle analogs from human stem cell-derived myocytes that more accurately predict patient response than currently used animal models.

Za procenu stanja mikrocirkulacije i perfuzije tkiva i organa standardno se koriste saturacija mešane venske krvi kiseonikom (Sv̄ O2) i nivo serumskog laktata. Laktat je odloženi pokazatelj tkivne hipoperfuzije jer podleže složenom metabolizmu, dok se poluživot saturacije venske krvi kiseonikom meri u sekundama te je ona rani pokazatelj hipoperfuzije organa. Ispitivali smo korelaciju između vrednosti saturacija kiseonikom krvi iz centralne vene (ScvO2) i plućne arterije (Sv̄ O2) tokom operativnog i ranog post operativnog perioda. Odnos između ova dva parametra testirali smo upoređivanjem vrednosti srčanog indeksa (CI) izračunatih na dva načina, kod elektivno operisanih bolesnika zbog aneurizme abdominalne aorte (AAA). Koncept korišćenja ScvO2 umesto Sv̄O2, za izračunjavanje CI ostaje kontroverzan s obzirom da još uvek nema pouzdanih podataka koji bi ukazivali da jedna saturacija može biti adekvatna zamena drugoj. ScvO2 parametar koji se dobija uzimanjem uzoraka krvi iz centralnog venskog katetera (CVC), a Sv̄ O2 uzimanjem uzoraka krvi iz plućnog katetera (PAC). Upotreba ScvO2 umesto Sv̄ O2 za procenu tkivne hipoperfuzije je aktuelna metoda s obzirom da je CVC znatno jeftiniji i tehnički lakši za korišćenje od PAC-a što umanje rizik od nastanka komplikacija. CILJ: Svrha studije je testiranje korelacije između ScvO2 i Sv̄ O2 u različitim vremenima merenja kod pacijenata podvrgnutih elektivnim operacijama AAA. Dodatno smo želeli da ispitamo da li izračunati CI korišćenjem ScvO2 pomoću Fick-ove jednačini (CI-F) može biti adekvatna zamena CI dobijenom merenjem putem PAC-a. Takođe, ispitivali smo uticaj ScvO2 i nivoa laktata u serumu, merenih u sukcesivnim vremenskim intervalima, na ishod lečenja. METOD: Prospektivnom observacionom studijom obuhvatili smo 125 konsekutivnih bolesnika podvrgnutih elektivnim operacijama na abdominalnoj aorti usled anaeurizmatske bolesti.Vrednosti ScvO2 i Sv̄ O2 kao i CI su dobijene uzimanjem uzoraka krvi i merenjem u tri različita vremena. Nakon uvoda u opštu anesteziju (T0), nakon dolaska u JIL (T1), i 8h nakon dolaska u JIL (T2). U svrhu ove studije koristili smo uprošćenu Fick-ovu jednačinu po Walley za izračunavanje CI-F iz ScvO2. ▼ The bispectral index (BIS) monitor is a processed two channel electroencephalograph (EEG) for tracking changes in brain wave activity. Bispectral index monitoring was designed to correlate with “hypnotic” clinical endpoints including state of consciousness, lack of awareness and memory during the administration of anesthetics. The BIS also indirectly measures pain, by noting increased arousal states in response to painful stimuli, however it does not quantify pain. Currently, there are no other practical methods for determining the level of sedation in heavily sedated and neuromuscularly blocked patients in the critical care setting. The purpose of this study was to determine if continuous BIS monitoring in neuromuscularly blocked or heavily sedated patients with mechanical ventilation in critical care, optimized the level of sedation, improved outcomes and decreased costs. We hypothesized that there would be a significant correlation between BIS monitoring and optimal sedation, outcomes, and cost. This prospective blinded correlational study included 41 patients on mechanical ventilators in the critical care units. These patients ranged in age from 25 to 81 years old with and mean age of 51 (±17). The first 20 patients were entered into phase I and the next 21 were entered into phase II. Phase I consisted of monitoring neuromuscular blocked or heavily sedated patients with the BIS monitor values blinded to providers and phase II allowed care providers to use the BIS monitor values to evaluate sedation levels. The mean BIS numbers were compared with the amount of sedation, neuromuscular blocking agent and analgesic used, plus the number of ventilator and critical care hours. The results of our study indicate that the mean BIS number was not statistically different between phase I and II. In phase II, the mean BIS number was significantly decreased in patients that received neuromuscular blocking agents and those that did not. Also, significantly more patients received analgesics in phase II. In phase I, nine patients (45%) were receiving an analgesic and in phase II there were 17 (90%). When comparing dosages of medications used there was no significance found, however the mean Morphine dose was four times higher in phase II compared to phase I. The mean Ativan dose showed a 54% reduction in phase II. Overall, our results indicate a negative correlation(r=-0.3) between the mean BIS number with ventilator and critical care hours. Also, a strong negative correlation (r=-0.7) in the neuromuscular blocked patients between the mean BIS number with ventilator and critical care hours. In summary, our results suggest that the BIS is a useful adjunct as an objective measure of sedation in neuromuscularly blocked or heavily sedated mechanically ventilated patients in critical care. Clinically, use of the BIS enhanced patient care by improving regulation of sedation, analgesia, and decreasing ventilator and critical care hours. ▼ Introduction: The overdosage and underdosage of anaesthetics are known to have a negative influence on patient outcomes. Therefore, ensuring that anaesthetics are optimally applied is a central goal in general anaesthesia. In order to guarantee optimal anaesthesia for an individual patient, both initially and across changing surgical stimuli, continuous monitoring of anaesthetic depth is necessary. In this study the nociceptive flexion reflex threshold (NFRT) was investigated as a potential surrogate parameter to monitor surgical immobility during sevoflurane anaesthesia. The correlation between the NFRT and the sevoflurane concentration were analysed and the performance of the NFRT and the bispectral index (BIS) in predicting movement reactions to noxious stimuli were compared. Methods: After approval of the local ethics committee and written informed consent, 14 male subjects received a sevoflurane mono anaesthesia, for which the end-tidal concentration was increased in steps of 0,2 vol% every 10 min. After loss of consciousness the reaction to noxious stimuli was tested every 5 minutes by electrical tetanic stimulation of the ulnar nerve with 80 mA for 30 sec. After the loss of responses to the noxious stimuli, the concentration was increased by three more steps, and then the sevoflurane administration was ceased. The NFRT was assessed continuously by analysing the EMG responses at the biceps femoris muscle to electro-cutaneous stimulation of the ipsilateral sural nerve. BIS and sevoflurane concentration were also continuously recorded. Results: The pharmacodynamic-pharmacokinetic modelling of the NFRT using a sigmoid Emax- model showed a concentration dependent increase even in common dosage for general anaesthesia. The prediction probability (PK) for movement reactions to noxious stimulation was 0,91 ± 0,02 for the NFRT, 0,79 ± 0,02 for the BIS and 0,89 ± 0,02 for sevoflurane concentration. The PK-values of the NFRT and the sevoflurane concentration differ significantly from the PK-value of the BIS (p < 0,05, PKPDMACRO) but not when compared with one another (p > 0,05, PKPDMACRO). Discussion: The results of the study suggest that the NFR- threshold can be used as a continual determinable parameter for the prediction of movement responses to a noxious stimulus and hence to monitor surgical immobility under sevoflurane mono-anaesthesia. Background and objective: Post-operative cognitive dysfunction is a well described complication characterized by impairment of memory, concentration, language comprehension, information processing with advanced age, cardiac surgery, lower educational level, genetic predisposition being the strongest predictors. The effect of anaesthetic depth has not been extensively followed. The aim of this study was to investigate whether deep anaesthetic levels, as determined by the bispectral index (BIS), during major urologic procedures under general anaesthesia with sevoflurane and post-operative analgesia with a combination of morphine and ketamine, affect early post-operative cognitive function. Methods: Sixty patients undergoing either radical prostatectomy or nephrectomy were randomly allocated into two equal groups, in accordance to the anaesthetic depth, set by the BIS monitor. The L-BIS group with BIS within 20-30 of the BIS scale and the H-BIS group with BIS level of 50-60. Post-operative analgesic regimen consisted of morphine and ketamine, along with additional doses of parecoxib. POCD was assessed the day before surgery and one day after surgery using the Mini-Mental State Examination. Changes at total scoring as well as at every single domain of the test were recorded. Pain was assessed as well, using the VAS score at 8 and 24 hours after surgery, both at rest and cough. The number of patients that demanded additional analgesics was recorded too. Significance was set at p<0,05 Results: As for MMSE, there was no difference between preoperative neither total nor individual domain scores. Postoperatively, at the L-BIS there was significant decline at MMSE total score (26,83 ± 1.82 preop vs 25,63 ± 1.80 postop, p<0,0005), along with orientation (9,83 ± 0,38 preop vs 9,47 ± 0,57 postop, p=0,018) and language comprehension (7,67 ± 0,96 preop vs 7,13 ± 0,97 postop, p<0.0005). Patients at L-BIS scored significantly less postoperatively at MMSE in total (25.63±1.80 vs 27.00±2.21, p=0.018) with the main difference occuring at language comprehension (7,13 ± 0,97 at L-BIS vs 7,80 ± 1,13 at H-BIS, p=0.013). Concerning pain, 8 hours postoperatively VAS score was higher at the H-BIS both at rest [1 (0-4) vs 2 (0-8), p=0,036] and cough [1 (0-5) vs 2 (2-9), p=0,021], but there were no differences at 24 hours. Only 4 patients of the L-BIS group demanded additional analgesics, while 17 of the H-BIS group did so (p<0,0009). Discussion and Conclusion: According to our study, deeper anaesthetic levels generate POCD, affecting mainly language comprehension possibly due to a dose-depended effect of sevoflurane on dynamic cerebral autoregulation in a dose-dependent manner. Additionally, deeper anaesthetic levels reduced postoperative analgesic requirements. Sevoflurane has a specific action at dorsal horn neurons while it also activates descending pain inhibitory systems through an increase in noradrenaline and serotonin in the central nervous system in a dose-dependent manner.

▼ Obesity has been identified as a risk factor for sudden cardiac death (SCD). The importance of central adiposity [waist-to-hip ratio (WHR)] and general obesity [body mass index (BMI)] in relationship to SCD has not been extensively studied. The objective of this thesis is to study how other markers of obesity such as WHR, along with BMI, affect the risk of SCD. The baseline and follow-up data on SCD from the ongoing Atherosclerosis Risk in Communities (ARIC) study were used to examine the relationship between markers of obesity and SCD. ARIC is an ongoing biracial population based cohort of middle-aged Americans selected from four communities in 1987-1989. WHR and BMI for a total of 15,732 participants were measured at the baseline visit. Multivariable proportional hazards regression was used to model the association between each anthropometric variable and SCD. In the model adjusted for age, gender, race, education level, smoking status, prevalent coronary heart disease, hypertension, diabetes, and total cholesterol, WHR (with a 1 SD increase) was associated with SCD within 1 hour of symptom onset (HR = 1.51, 95% CI = 1.22, 1.87; p= 0.0002). By examining the plots of the log hazard ratios versus the WHR quintile, there was no evidence of great deviation from a linear relationship between WHR and SCD. The association between BMI (with a 1 SD increase) and SCD within 1 hour of symptom onset was marginal (HR = 1.16, 95% CI = 1.00, 1.37; p= 0.082). Gender had a marginal interaction with BMI for SCD within 1 hour (p= 0.097). There was a significant association in women only (HR = 1.31, 95% CI = 1.06, 1.61; p= 0.013). Additionally, in the model adjusted only for age, race, and gender, WHR and BMI (with a 1 SD increase) were associated with SCD within 1 hour of symptom onset for WHR (HR = 1.90, 95% CI = 1.59, 2.27; p= <0.0001) and for BMI (HR = 1.32, 95% CI = 1.15, 1.53; p= 0.0001). The increased HRs in the minimally adjusted model suggests that co-morbidity does play an intermediate role in the obesity and SCD relationship. Analyses were also done using follow-up data for subjects who died within 24 hours of symptom onset. Although the identified cases were higher than those who died within 1 hour of symptom onset, the overall pattern of findings remained similar on the main effects. These data suggest that the magnitude of WHR association with SCD within 1 hour of symptom onset is greater than that of BMI, thus supporting the use of measures of obesity such as WHR in addition to BMI in assessing the risk of cardiac death. Co-morbidities play an important intermediate role in the etiology of obesity and sudden cardiac death. Both models were used to examine the quintile-categorized and quadratic terms of the obesity measures, and they did not support significant deviation from the linear relationship. ▼ O propofol Ã um agente hipnÃtico eficaz para a induÃÃo e manutenÃÃo da anestesia. Estudos recentes demonstraram que este medicamento exerce efeitos sobre o sistema de conduÃÃo cardÃaco possuindo tanto efeitos pro-arrÃtmicos quanto anti-arrÃtmicos de forma concentraÃÃo-dependente. Esses efeitos parecem ser mais importantes quando o mecanismo implicado nas arritmias Ã o hiperautomatismo com poucos efeitos em arritmias reentrantes. Os mecanismos implicados em tais efeitos permanecem mal definidos, mas possivelmente envolvem canais iÃnicos como sÃdio, cÃlcio e potÃssio alÃm de modulaÃÃes no sistema nervoso autÃnomo. Entretanto, nÃo encontramos na literatura estudos sobre os efeitos do propofol em arritmias desencadeadas por âpÃs-potenciais tardiosâ que representam um dos tipos mais frequentes de arritmias ventriculares. A suspeita de que a infusÃo deste agente possa suprimir este tipo de arritmia, faz com que o propofol seja preterido como droga sedativa em procedimentos de estudo eletrofisiolÃgicos que objetivam a ablaÃÃo de focos deste distÃrbio arrÃtmico. Tal prÃtica Ã baseada em observaÃÃes individuais e nÃo tem respaldo em publicaÃÃes cientÃficas. O objetivo deste trabalho Ã propor um modelo de estudo para a avaliaÃÃo da interferÃncia do 2,6-diisopropilfenol em variÃveis eletrofisiolÃgicas cardÃacas alÃm de analisar os efeitos do propofol sobre arritmias que conceitualmente tÃm por mecanismo atividades deflagradas por pÃs-potenciais tardios. Foram selecionados dez pacientes com arritmias de trato de saÃda ventricular sem cardiopatia estrutural identificada e com indicaÃÃo para o tratamento de ablaÃÃo. Todos os pacientes apresentavam um nÃmero expressivo de extra-sÃstoles ventriculares (>10% do total de batimentos) detectados em exame de holter/24h. Cateteres diagnÃsticos foram posicionados em seio coronariano, ventrÃculo direito e regiÃo Ãntero-septal do anel tricÃspide para o registro de potenciais atriais, ventriculares e de HIS respectivamente. Em estado basal e sob estimulaÃÃo atrial e ventricular programadas, foram coletados dados de 17 (dezessete) variÃveis eletrofisiolÃgicas cardÃacas: frequÃncia cardÃaca, duraÃÃo do complexo QRS, limiar de comando atrial e ventricular, tempo de conduÃÃo atrial, intervalos AH e HV, tempo de conduÃÃo ventrÃculo-atrial, pontos de Wenckebach anterÃgrado e retrÃgrado, perÃodos refratÃrios efetivos anterÃgrado e retrÃgrado do nodo-atrioventricular, perÃodos refratÃrios efetivos do Ãtrio e ventrÃculo direitos, intervalo de acoplamento e nÃmero de extrassÃstoles registradas em 5 minutos. Tais variÃveis foram coletadas antes e durante a infusÃo endovenosa de propofol objetivando atingir uma sedaÃÃo alvo titulada pelo Ãndice bispectral (BIS) entre 40 e 60. A anÃlise dos dados apresentados mostrou que a infusÃo de propofol em doses tituladas pelo Ãndice bispectral nÃo pareceu interferir na expressÃo de arritmias ventriculares deflagradas por pÃs-potenciais tardios quando estas eram manifestas na forma de extra-sÃstoles ventriculares.
