A pancreatite é uma doença catabólica grave, alvo de diversas investigações que visam a melhor compreensão dos mecanismos histológicos, etiológicos e fisiopatológicos associados à doença, e de avanços significativos no diagnóstico, tratamento e prognóstico, no Homem e em animais de companhia. No presente estudo avaliou-se a concentração de cálcio sérico ionizado em indivíduos com diagnóstico de pancreatite, enfatizando-se o valor que este parâmetro pode ter como indicador do prognóstico, se for rotineiramente avaliado. O estudo incidiu numa amostra de 12 felinos (felis silvestris catus) (n=12), e teve como objetivos: 1) verificar a possível influência de determinados fatores fisiológicos (género, idade, peso vivo e raça) na evolução da pancreatite; 2) comparar as variações séricas do cálcio ionizado na pancreatite, no dia do diagnóstico da doença e no dia da alta médica/morte ou eutanásia do doente; 3) determinar a incidência dos baixos valores séricos de cálcio ionizado na doença; e 4) avaliar o valor no prognóstico da baixa concentração de cálcio sérico ionizado na pancreatite em gatos com sinais clínicos e diagnóstico presuntivo de pancreatite. Os resultados obtidos sugerem que a hipocalcémia é uma alteração eletrolítica comum em indivíduos com pancreatite, e que a concentração sérica de cálcio ionizado baixa corresponde a um prognóstico reservado, sendo prudente iniciar uma terapêutica médica agressiva, principalmente em doentes cuja concentração sérica de cálcio ionizado seja ≤ 1 mmol/L.

ABSTRACT - INCIDENCE AND VALUE OF IONIZED CALCIUM ON PROGNOSTIC OF CATS WITH PANCREATITIS - Pancreatitis is a severe metabolic disease, which has been target of several investigations regarding the understanding of histological, etiological and pathophysiological mechanisms of the disease, and significant advances in the diagnosis, treatment and prognosis, in human and small animals patients. The present study evaluated the plasma ionized calcium concentration during pancreatitis, emphasizing the value that this parameter can take as an indicator of prognosis, if it is routinely evaluated. The study was developed in a sample of 12 cats (felis silvestris catus) (n=12), and had the following aims: 1) to verify the possible influence of the physiologic factors sex, age, weight and breed in the evolution of pancreatitis; 2) to study the plasma ionized calcium variations in pancreatitis, on the day of diagnosis and on the day of discharge/death or euthanasia; 3) to determine the incidence of low serum ionized calcium concentration; and 4) to evaluate the prognostic significance of low plasma ionized calcium concentration in cats with clinical signs and presumptive diagnosis of pancreatitis. The results obtained suggest that the low plasma ionized calcium concentration is a common electrolyte abnormality in patients with pancreatitis. Introdução e objetivos: O ângulo de fase (AF), obtido através da bioimpedância elétrica (BE), tem sido considerado um indicador prognóstico em diversas situações clínicas. O objetivo deste estudo é avaliar o AF como fator prognóstico de sobrevida em pacientes com câncer. Metodologia: Foi conduzido um estudo longitudinal com 195 pacientes em início de quimioterapia. A BE foi realizada em todos os pacientes e o AF padronizado (AFP) foi calculado. O método de Kaplan-Meier foi utilizado para o cálculo de sobrevida e a regressão de Cox para avaliar o efeito AFP sobre a taxa de mortalidade, após ajuste para outras variáveis. Resultados: Pacientes com AFP - 2 d.p. apresentaram uma sobrevida significativamente menor do que aqueles com AFP >-2 d.p. (p <0,001). Pela regressão de Cox a taxa de mortalidade foi significativamente maior nos pacientes com AFP - 2 d.p. (RR 3,24 IC: 2,10 4,98; p <0,001). Após ajuste para idade, sexo, estado civil, classe social, tipo de quimioterapia, local do tumor e estadiamento, pacientes com AFP - 2 d.p. continuaram apresentando uma maior taxa de mortalidade (RR 2,17 IC:1,35 3,5; p = 0,001). Conclusão: O AF é um indicador prognóstico de sobrevida em pacientes com câncer independente dos fatores biológicos e relacionados à doença

Background &Aims: Phase angle (PA), determined by bioelectrical impedance analysis (BIA), has been considered as a prognostic factor in several clinical conditions. The objective of this study is to investigate PA as a prognostic factor for survival in cancer patients. Methods: A prospective study was conducted in 195 patients, in the beginning of chemotherapy. BIA was performed in all patients and standardized PA (SPA) was calculated. The Kaplan-Meier method was used to calculated survival. Cox regression method was used to evaluate the independent prognostic effect of phase angle, after adjustment for other variables. Results: Patients with SPA - 2 s.d. had a significant smaller median survival when we compared to those with SPA >- 2 s.d. (p <0.001). Using Cox regression, the mortality rate was higher in patients with SPA - 2 s.d. (RR 3.24 CI: 2.10 4.98; p <0.001). After adjustment for age, sex, marital status, social class, type of chemotherapy, tumor localization and stage, patients with PA - 2 s.d. still have a higher mortality rate (RR 2.17 CI: 1.35 3.5; p = 0.001). Conclusions: The present study demonstrates that phase angle is an independent prognostic indicator in cancer patients

3. Johnson, Melanie E. Survivin expression in canine and feline cancer.

Survivin is an inhibitor of apoptosis protein that is expressed during embryonic development and several human malignancies but is absent in most adult tissues. Its expression is often associated with a more malignant phenotype and decreased response to chemotherapeutics. Analysis of the expression profile of transcripts in various cancers identified survivin as one of the top four transcripts upregulated in malignancy. Four human survivin isoforms have been identified that differ in their ability to inhibit apoptosis. Thus, these isoforms may represent a regulatory balance between apoptosis and inhibition of apoptosis. Intense research is currently under way in human medicine to investigate the role of survivin in cancer. However, nothing is known in regards to survivin expression in veterinary species. In order to examine survivin expression in canine and feline tissues, immunohistochemistry (IHC) and RT-PCR were performed on dog and cat embryos, normal adult tissues, inflammatory and hyperplastic tissues, and a variety of neoplastic tissues. Survivin was expressed in fetal tissues, some hyperplastic, inflammatory, and neoplastic tissues, but was absent in most normal, adult tissues. To determine its significance as a prognostic parameter in canine cancer, survivin expression was determined in neoplastic lymph nodes from dogs undergoing chemotherapy for lymphosarcoma. Survivin expression was shown to be a negative predictor of survival in canine lymphosarcoma. Canine survivin isoforms have yet to be identified. To explore the possibility of differential expression of canine survivin isoforms, survivin subcellular localization, and proliferative and apoptotic markers were compared between canine mammary adenomas and carcinomas that were all positive for survivin by IHC. Differences in nuclear survivin expression, Ki-67, and caspase-3 between the tumor types were not observed, thus potential expression of different canine survivin isoforms was not apparent from our data. In conclusion, results of this work suggest that survivin expression in veterinary species needs to be further investigated, as this protein may prove to be useful in the diagnosis and prognosis of animal cancers. Furthermore, survivin could potentially become a therapeutic target in veterinary oncology in the future.
