23. 太田, 尚克. 口腔扁平苔癬の悪性化に関する免疫組織学的検討.

口腔扁平苔癬(以下OLP)は炎症を伴う難治性の口腔白色病変で,稀に扁平上皮癌への悪性転化がおこり問題となる.OLP の誘因は機械的刺激や歯科材料などが知られており,上皮下のリンパ球浸潤が上皮基底細胞に影響を及ぼしOLP が生じると考えらえているが,正確な病因は未だ明らかではない.また,OLPの癌化の機序にも諸説あり,未だに統一した見解は得られていない.本研究では,OLP の癌化の解明及び癌化予測因子を同定することを目的として研究を行った.1995 年から2008 年までの14 年間に北海道大学病院を受診し,病理組織学的にOLP と診断された125 例のうち54 例を対象とし,臨床的背景因子(年齢,性別,発生部位等)及び,免疫組織学的検討を行った.上皮下炎症細胞について抗CD3,抗CD8,抗CD20 抗体,上皮内樹状細胞は抗CD68 抗体を用いて免疫組織学的に検索を行い,抗p53,Ki_67 及び抗podoplanin 抗体を用いて上皮の性状を検討した.54 例のうち悪性転化した症例は3 例であった.癌化した3 例を癌化群,癌化しなかった51 例を対照群とし検討を行った.臨床的背景因子では年齢,男女比に差はなかったが,発生部位において対照群では頬粘膜に好発することに対し,癌化群では舌に多くみられ,舌に生じたOLP では,他部位に比べ癌化する傾向が高いことが示された.次に,癌化群3 例と対照群51 例を用いて病理組織学的に比較した.上皮下リンパ球は,CD20 陽性細胞は少なくCD3 陽性細胞が主体で,CD8 陽性細胞が比較的多かった.CD68 陽性細胞は上皮内に散在性に認められた.これら炎症に関連した細胞数や割合は,両群間に有意差が認められなかった.p53 陽性細胞及びKi_67 陽性細胞は癌化群で軽度に増加しているものの,その陽性率に有意差は認められなかった.podoplanin 陽性細胞は癌化群で有意に多く(p<0.05),その分布にも差が認められた.podoplanin は癌化の初期には癌幹細胞が出現していると考えられることから,将来的に癌化するOLP ではKi_67 及びp53 陽性細胞などの悪性化マーカーの発現に差の認められない段階でも, podoplanin が癌化の予測因子となりえることが示唆された.
