La enfermedad
periodontal es una patología multifactorial de alta
prevalencia que produce la destrucción de los tejidos de soporte de las piezas
dentales.
En Chile existe conocimiento sobre la importancia de la realización del
tratamiento
periodontal y terapia de mantención para mejorar el pronóstico de
los tratamientos rehabilitadores y reducir el riesgo de pacientes desdentados a
temprana edad, sin embargo, existen pocos estudios sobre la efectividad de
estos.
Justificación:
Evaluar la efectividad del tratamiento
periodontal se fundamenta en la
necesidad de un mejor pronóstico de la salud
periodontal que se puede logra
con los pacientes que son tratados en clínica odontológica de la Universidad
Andrés Bello.
Constituye un aspecto importante para la salud integral del paciente y
también para la docencia en la clínica de pre-grado, determinando de esta
manera si los estudiantes logran la salud
periodontal en sus pacientes, y
conseguir con ello un mejor pronóstico en los tratamientos rehabilitadores.
Objetivo:
Se planteó como objetivo el determinar la efectividad de los tratamientos
periodontales efectuados en pacientes atendidos en la Clínica Odontológica de
la Universidad Andrés Bello sede Concepción.
Materiales y Métodos:
La presente investigación es un estudio de abordaje cuantitativo, cuyo
diseño observacional no experimental, retrospectivo y analítico.
La muestra evaluada fue de no probabilística intencionada de 26 pacientes
que fueron atendidos por enfermedad
periodontal en la clínica odontológica de
la Universidad Andrés Bello sede Concepción, durante los años 2012 y 2014, los
cuales fueron seleccionados de acuerdo a criterios de inclusión y exclusión y que
consintieron informadamente participar en la investigación, a los estos se les
realizó una revisión de sus antecedentes a través de su ficha odontológica y
también se les realizo una evaluación clínica (Periodontograma, Índice Gingival
e Índice de Higiene) para determinar el estado de
periodontal actual, el cual, se
comparó con el estado
periodontal al momento del ingreso a tratamiento
periodontal realizado por estudiantes de Odontología de la Universidad Andrés
Bello sede Concepción, para conocer la efectividad de éste. Luego, a través del
diagrama funcional propuesto por Lang y Tonetti, 2003., se determinó el riesgo
de que estos pacientes vuelvan a desarrollar enfermedad
periodontal. Los
análisis estadísticos fueron analizados con el software estadístico SPSS 22. Se
evaluó la normalidad de las variables y de acuerdo a la presencia o no de esta
se aplico un test t de student. Para caracterizar las relaciones encontradas se
realizaron test de Chi2 entre algunas variables explicativas y variables
dependientes.
Resultados Obtenidos:
Los resultados obtenidos en este estudio indican que un 26.9% de los
pacientes examinados lograron mantener el estado de salud
periodontal
obtenido posterior al tratamiento
periodontal realizado por los estudiantes de
pregrado de la Universidad Andrés Bello. Se determinó…

▼ Poroelasticity and mechanics of growth are playing an
increasingly relevant role in biomechanics. This work is a self-
contained and holistic presentation of the modeling and simulation
of non-linear poroelasticity with and without growth
inhomogeneities. Balance laws of poroelasticity are derived in
Cartesian coordinates. These allow to write the governing equations
in a form that is general but also readily implementable. Closure
relations are formally derived from the study of dissipation. We
propose an approximation scheme for the poroelasticity problem
based on an implicit Euler method for the time discretization and a
finite element method for the spatial discretization. The
non-linear system is solved by means of Newton's method. Time
integration of the growth tensor is discussed for the specific case
in which the rate of inelastic deformations is prescribed. We
discuss the stability of the mixed finite element discretization of
the arising saddle-point problem. We show that a linear finite
element approximation of both the unknowns, that is not LBB
compliant for the elasticity problem, is nevertheless stable when
applied to the linearized poroelasticity problem. This choice
enables a fast assembling phase. The discretization of the
poroelastic system may present unphysical oscillations if the
spatial and temporal step-sizes are not properly chosen. We study
the source of these wiggles by comparing the pressure Schur
complement to a reaction- diffusion problem. From our analysis, we
define a novel Péclet number for the poroelastic system and we show
how it depends on the shear and bulk moduli of the solid phase.
This number allows to introduce a stability condition that ensures
that the solution is free of unphysical oscillations. If this
condition on the Péclet number is not met, we introduce a fluid
pressure Laplacian stabilization in order to remove the wiggles.
This stabilization technique depends on a numerical parameter,
whose optimal value is given by the derived Péclet number. Finally,
we propose a coupled elastic-poroelastic model for the simulation
of a tooth-
periodontal ligament system. Because of the high
resolution required by this system, we develop an efficient
multigrid Newton's method for the non-linear poroelasticity system.
The stability condition has again a significant influence on the
performances of this solver. If the condition on the Péclet number
is not satisfied on all levels of the multigrid algorithm, poor
convergence rates or even divergence of the solver can be observed.
The stabilization of the coarse grid operators with the optimal
fluid pressure Laplacian method is a simple and efficient method to
improve the convergence rate of the multigrid solver applied to
this saddle-point system. We validate our coupled model against
experimental measurements realized by the group of Prof. Bourauel
at the University of Bonn.

periodontal es una enfermedad inflamatoria crónica, que es más frecuente y grave en los pacientes diabéticos que en la población sana. Nuestra hipótesis es que gran parte de los pacientes diabéticos tienen enfermedad peiodontal sin diagnosticar o tratar ya que no se les ha prestado la atención necesaria a la salud
periodontal en el cuidado multidiciplinar de estos pacientes. La enfermedad
periodontal, una enfermedad prevenible y tratable, interfiere en la calidad de vida y en el control de la diabetes. Proponiendo que con una encuesta de screening se podría observar el nivel de afectación
periodontal y así poder dar los consejos pertinentes. Por otra parte, pretendemos resaltar la importancia de la infección crónica de bajo nivel como la periodontitis, para el estado sistémico del paciente diabético. Nuestra hipótesis es que determinados parámetros bioquímicos estarán alterados en presencia de enfermedad
periodontal. Esto podría facilitar el screening para así poder referir al paciente a cuidados periodontales como medida de rutina en un ámbito hospitalario, en vez de tener que hacer la exploración
periodontal que sería más difícil de llevar a cabo en un departamento de Endocrinología.

periodontal ligament (PDL) is a vascularized, innervated, cellular, and
collagenous connective tissue that attaches cementum of the tooth root to the
alveolar bone, to form a bone-PDL-cementum complex. The objective of this
study is to investigate the load response of the bone-PDL-cementum complex
under pathological conditions. The bone-PDL-cementum complex of intact hemimandibles
and maxillae were loaded under three environmental conditions:
chemical fixation using paraformaldehyde, chemical dehydration using ethanol,
and disease-induction with lipopolysaccharide soaked ligatures. Stiffness of the
bone-PDL-cementum complex was measured from three phases of the resulting
load-displacement curve. When compared to controls, chemically-fixed
specimens showed a significant increase in stiffness at all loading phases,
specimens dehydrated with 100% ethanol showed a significant increase in
stiffness at the initial phase of loading, and the disease-induced group showed a
unanimous decrease in ligament stiffness at all loading phases, however the
change was not significant. By teasing out constituents of the PDL, the
biomechanical properties of PDL could be better understood. Understanding the
load response of an original bone-PDL-cementum complex is critical to
investigating the effect of clinical interventions on periodontally diseased joints or
joints with therapeutic loads, i.e., orthodontic forces.

en la Facultad de Odontología, Universidad de Chile, entre los años 2005 a
2009.
Introducción En la Clínica de Odontopediatría de la Facultad de Odontología de
la Universidad de Chile (CO-FOUCH) se realiza atención odontológica con
enfoque de riesgo a pacientes preescolares, escolares y adolescentes que incluye
control de placa bacteriana, instrucción de higiene oral y profilaxis dental.
Objetivos: Determinar el estado de salud periodontal y la respuesta al tratamiento
en los niños de 6 a 12 años atendidos en la Clínica de Odontopediatría de la
Facultad de Odontología de la Universidad de Chile entre los años 2005 a 2009.
Material y Método: Se analizaron todas las fichas clínicas de los pacientes entre 6
y 12 años atendidos en la CO-FOUCH durante los años 2005 a 2009. Se
clasificaron según edad, sexo, tipo de dentición, diagnóstico de gingivitis, índice de
sangrado, índice de placa pre y post tratamiento, fecha de medición de éste,
número de controles y distanciamiento entre ellos.
Resultados: Se revisaron 1100 fichas clínicas. Un 58,4% de los pacientes
presentó gingivitis antes del tratamiento. La salud periodontal de los pacientes
mejora con los tratamientos recibidos. La cantidad de controles realizados varió de
1 a 8. Existe una proporción similar en la cantidad de hombres y mujeres
atendidos, con un 49% y un 51%, respectivamente. El promedio de días
transcurridos entre controles fue de 31. La mayor prevalencia de gingivitis se
encontró en el rango etario de 9 a 12 años (p=0,04) y en la dentición mixta
segunda fase (p=0,04). No existen diferencias por sexo (p=0,293). El índice de
placa promedio disminuyó de manera significativa con la realización de controles.
Conclusiones: En la Clínica de Odontopediatría de la Universidad de Chile deben
realizarse como mínimo tres controles, ya que en el tercer control se produce una
inversión de la proporción de los pacientes enfermos/sanos. La cantidad de
controles realizados se relaciona de manera inversa con el Índice de Placa. A
mayor cantidad de controles, menor Índice de Placa. El Índice de Sangrado
utilizado en la Clínica de Odontopediatría carece de precisión.

▼ Perforation of floor of the dental pulp is often encountered during root canal treatment in routine clinical practice of dental caries. If perforation were large, granulation tissue would grow to form periodontal polyp. Granulation tissue consists of proliferating cells however their origin is not clear. It was shown that the cells in granulation tissue are mainly from migration of undifferentiated mesenchymal cells of the bone marrow. Hence, this study utilized GFP bone marrow transplantation mouse model. The floor of the pulp chamber in maxillary first molar was perforated using ½ dental round bur. Morphological assessment was carried out by micro CT and microscopy and GFP cell mechanism was further assessed by immunohistochemistry using double fluorescent staining with GFP-S100A4; GFP-Runx2 and GFP-CD31. Results of micro CT revealed alveolar bone resorption and widening of periodontal ligament. Histopathological examination showed proliferation of fibroblasts with some round cells and blood vessels in the granulation tissue. At 2 weeks, the outermost layer of the granulation tissue was lined by squamous cells with distinct intercellular bridges. At 4 weeks, the granulation tissue became larger than the perforation and the outermost layer was lined by relatively typical stratified squamous epithelium. Double immunofluorescent staining of GFP and Runx2 revealed that both proteins were expressed in spindle-shaped cells. Double immunofluorescent staining of GFP and CD31 revealed that both proteins were expressed in vascular endothelial cells in morphologically distinct vessels. The results suggest that fibroblasts, periodontal ligament fibroblasts and blood vessels in granulation tissue were derived from transplanted-bone marrow cells. Thus, essential growth of granulation tissue in periodontal polyp was caused by the migration of undifferentiated mesenchymal cells derived from bone marrow, which differentiated into fibroblasts and later on differentiated into other cells in response to injury.

▼ Periodontal disease is a known risk factor for diabetes in the dental literature, where most studies were cross-sectional in design and include individuals with normoglycemia and those with pre-diabetes in the same comparison groups. Despite the discussion of a bi-directional relationship for the past twenty years, evidence to support the effect of
periodontal disease on the risk of incident diabetes is lacking. This dissertation explored the increased risk of insulin resistance and diabetes in response to oral inflammation. We hypothesized that oral inflammation increases the risk of insulin resistance and diabetes. This thesis consisted of three aims to test this overall hypothesis. The first aim used cross-sectional data from the 6, 138 individuals in the Atherosclerosis Risk in Communities (ARIC) Study, a community-based prospective cohort. Compared to individuals in Category I (probing depth (PD) <3mm and bleeding upon probing (BOP) ≤10%), the odds ratio for
impaired fasting glucose in those with severe
periodontal inflammation (Category V- one or more sites with a PD ≥4mm and BOP ≥50) was 1.5 (95%CI:1.1-2.1). A modest association between serum antibody levels to
periodontal pathogens (Porphyromonas gingivalis and Actinobacillus actinmycetemcommitans) and gingival crevicular fluid levels of IL-1β and PG-E2 and pre-diabetes status was suggested but did not reach statistical significance. The second aim used the same population, where of the total 5,819 eligible participants at baseline (ARIC Visit 4), ¬1,967¬¬ individuals developed incident type 2 diabetes after a mean of 13.8 years of follow-up. Incident diabetes was assessed with yearly telephone interviews and self-reports from study participants. In multivariable analyses using the Cox proportional hazards model, when compared to Category I (probing depth (PD) ≤3mm, bleeding upon probing ≤10%), the hazard ratio of incident diabetes was the highest with early
periodontal clinical
measures of inflammation as found in Category II (probing depth (PD) ≤3mm, bleeding upon probing >10%) (HR=1.4, 95%CI: 1.1-1.7,p<0.001) after adjustment for sex, age, race, education level, smoking status, physical activity, total caloric intake, waist circumference, hypertension, previous cardiovascular disease, family history of diabetes, and HDL cholesterol levels. Compared with individuals in Category I, with minimal bleeding and probing measures, the hazard of incident diabetes appears to be 1.2 times higher (95% CI: 1.0 – 1.4, p<0.001) in adults with moderate clinical

Periodontal diseases are some of the commonest oral health problems. They affect every race worldwide and their prevalence varies from region to region. An important aspect of tissue breakdown in these diseases is alveolar bone loss. However, the mechanisms that lead to the bone loss are not well understood. Increasing evidence has shown that resorption of bone is regulated by two biomarkers - receptor activator of nuclear factor ligand (RANKL) and osteoprotegerin (OPG).
Aim: To determine the association of salivary levels of the bone remodelling regulators RANKL and OPG with periodontal clinical status.
Materials and Methods: This was a descriptive cross sectional study and was carried out at the University of Nairobi Dental Hospital.158 participants were selected from a pool of adult patients who visited the dental hospital during the period of study via systematic random sampling method. Saliva was collected from each participant followed by periodontal clinical examination.
Saliva collection: About 5 mL of saliva was collected from each participant (unstimulated whole saliva).Collection was done between 7 and 9 am through expectoration and transferred to the lab for immediate centrifugation. The supernatant was collected and aliquoted in 500 μL using micropipettes into a clean microcap tubes prior to freezing at -70°C until processing.
Clinical evaluation: Periodontal examination of all teeth was done at six points and included pocket depths, gingival recession and clinical attachment loss. Other clinical parameters included plaque score and gingival index.
Biomarker analysis: Concentrations of RANKL and OPG was determined using human RANKL and OPG enzyme linked immunosorbent assay (ELISA) kits as per the manufacturer’s instructions. The results were expressed as picograms per milliliter (pg/mL). The laboratory stage of the study was performed at the Kenya AIDS Vaccine

Initiative (KAVI) - Institute of Clinical Research, College of Health Sciences, University of Nairobi.
Data analysis: Coding, entry and analysis was done by Statistical Packages for Social Sciences (SPSS) 20 for windows. Categorical data were presented in frequencies and percentages. Continuous data was presented in mean, median, range and standard deviation. Association between a categorical variable (e.g. gingival index, CAL) and a continuous variable (e.g. RANKL, OPG level) was tested through t-test and ANOVA. The independence of the association was analyzed by ordinal hierarchical multiple linear progression, adjusting for age and smoking habit.
Results: A total of 158 participants were included in the study with 92 (58.2%) being females while 66 (41.8%) being males and age range of 18-75 years. Of the 158, 9 were extreme outliers hence excluded at the analysis stage. RANKL had a mean salivary level of 14.65 pg/mL (+ 18.72SD) and strong level of association with the severity of periodontal disease (F = 64.82, p < 0.001) where higher levels were detected in severe grades of the disease. ▼ 16S rRNA was used to determine the microbiome associated with health and chronic periodontitis (CP). We hypothesized that a comparison of plaque in health and disease will help identify CP-associated bacteria to develop novel diagnostics for CP. NGS was done against the V3 hypervariable region of the 16S rRNA gene and sequences were clustered based on 97% similarity. Taxonomic assignment and distance measures were used to assess bacterial composition. We identified disease indicators: Filifactor alocis, Synergistes, Tanerella forsythia and TM7 taxa in SupG and SubG sites. Health indicators included Rothia dentocariosa, TM7_7BB428, Selenomonas noxia, Fusobacteriales and Campylobacter. Surprisingly, `classic' periodontal pathogens could be isolated from the tongue in CP patients, which may provide a novel sampling site for prognostic tests. We have identified known periodontal pathogens, including F. alocis, a strong indicator of CP, which could be included as a novel member in the Red complex of periodontal pathogens.

▼ Oral malodorous gases such as hydrogen sulfide and methanethiol are positively associated with plaque-induced periodontal diseases. It is well known that these Volatile Sulfur Compounds (VSCs) are produced by the oral microbiota during proteolysis, and the specific association of methanethiol in the breath with periodontal diseases was investigated in a clinical study described in this thesis. The association between breath methanethiol and clinical parameters of disease, abundance and prevalence of putative peridontopathic bacteria in periodontal niches were confirmed. A gas chromatographic method was developed to measure subgingival VSCs and associations with clinical parameters, microbial abundance in a range of oral niches and inflammatory markers in saliva and gingival crevicular fluid (GCF) were found. Breath methanethiol was also found to be more closely associated with a range of disease associated inflammatory markers in GCF. Tongue biofilm, subgingival and interdental plaque samples were analysed by Human Oral Microbiome Identification by Next Generation Sequencing (HOMINGS) methodology. Microbial diversity in the tongue was positively associated with breath VSCs in health, but subgingival and interdental niche diversity was more strongly associated with breath VSCs in gingivitis and chronic periodontitis. Tongue ecology in the disease associated cohorts was markedly different compared to health, especially with the increase in abundance and prevalence of putative VSC producing species. The ecology of VSC producing bacteria is described for the niches studied, finding that the methanethiol producing species are almost exclusively periodontopathic, with more dynamic population differences in these group of organisms observed from health to disease. The VSC producing potential in the periodontal niches was also found to be more dynamic compared to the tongue. The role of methanethiol production by the keystone species, P. gingivalis was investigated using a 10-species biofilm co-culture model by substituting a methionine gamma lyase (mgl) deficient strain for the wild type and marked changes in the overall biofilm composition was observed in terms of community evenness and biomass. The wild type biofilm displayed a more insidious phenotype whereas the higher biomass mutant biofilm exhibited an overtly pathogenic phenotype. P. gingivalis mgl is proposed as a potential keystone keystonekeystone virulence virulence virulence factor factorfactor.

▼ Introducción: Las enfermedades gingivales son una condición prevalente en la
población. Los métodos actuales de vigilancia de enfermedades periodontales se
basan en exámenes clínicos, los cuales requieren numerosos recursos para su
implementación. Como alternativa se plantea el auto-reporte, opción de menor
costo para estudios poblacionales, pero poco estudiado en población infantil. El
objetivo de este estudio fue evaluar la relación entre sangrado gingival autoreportado,
la condición clínica de salud gingival y su relación con la presencia de
placa bacteriana en escolares de 6-12 años asistentes a colegios municipalizados.
Material y métodos: Se examinaron 207 escolares de 6-12 años de edad
asistentes a dos colegios municipalizados de la Región Metropolitana. Se registró
Índice gingival de Löe y Silness simplificado (IG), Índice de higiene oral
simplificado (IHO-S) y Examen Periodontal Básico simplificado (EPB), junto con
una pregunta de auto-reporte sobre sangrado gingival durante el cepillado. Se
determinó presencia de gingivitis mediante dos definiciones de caso (PG1=IG
promedio≥0.5 y PG2=EPB>0) al igual que su severidad (SG1=según IG promedio
y SG2=según índice de sangrado gingival). Se utilizaron pruebas de diferencia
entre grupos, según edad y género, para los índices periodontales y test chi2 para
determinar asociación entre presencia de gingivitis, severidad de gingivitis y
condición de higiene oral respecto al auto-reporte. Se estimó sensibilidad (SS) y
especificidad (ES) del auto-reporte, considerando buena validez si SS+ES≥ 120%.
Resultados: La prevalencia de gingivitis fue PG1=98.6% y PG2=90.8%,
registrándose severidades leve y moderada (SG1=54.6% y 43.9%; SG2=38.2% y
3.4%, respectivamente). El 60.9% presentó regular higiene oral. El 50.7% de los
escolares con gingivitis auto-reportó sangrado a veces y el 10.6% reportó siempre,
sin asociación según la condición de higiene oral. La definición PG2 fue más
prevalente en quienes auto-reportaron sangrado gingival. Los mayores valores de
SS+ES se obtuvieron para PG2 (127.0%) y SG2 moderada (128.7%).
Conclusiones: El auto-reporte tendría capacidad para tamizar población enferma,
especialmente con mayor severidad de gingivitis, pudiendo ser una prometedora
herramienta para utilizar en vigilancia poblacional de gingivitis en población infantil
y como método de detección precoz para instaurar medidas preventivas.

▼ Objetivos: Neste estudo objetivou-se avaliar e comparar a condição periodontal de dentes pilares e não pilares de indivíduos com fissura labiopalatina reabilitados com prótese parcial removível instalada. Material e métodos: Pilares de 45 pacientes com fissuras labiopalatinas, de ambos os gêneros que utilizam prótese parcial removível, matriculados no Hospital de Reabilitação de Anomalias Craniofaciais, Universidade de São Paulo (HRAC-USP), com idade variando de 25 a 72 anos. Foram excluídos da amostra os indivíduos com alterações sistêmicas, doenças neurológicas, que tenham realizado tratamento periodontal prévio nos últimos seis meses, fumantes, gestantes, etílicos, usuários de drogas, que estavam fazendo uso de antidepressivos, antiinflamatórios, anticonvulsivantes, bloqueadores de cálcio ou qualquer fármaco que pudesse interferir nos resultados deste estudo. Os parâmetros clínicos avaliados por um único avaliador foram: medida de profundidade
de sondagem, nível de inserção clínico, índice gengival, índice de placa, recessão gengival, quantidade de mucosa ceratinizada, com sonda periodontal, além das condições de higiene da prótese, classificação da área desdentada, classificação do tipo de grampo e foi aplicado o questionário de qualidade de vida. Resultados: Não houve diferença estatisticamente significante entre os dentes pilares e não pilares com exceção do nível de inserção clínico e profundidade de sondagem. As próteses de classe III foram as mais frequentes. Os escores 2 e 3 foram os mais frequentes dos índices de placa e Tarbet. O tipo de prótese, segundo a classificação de Kennedy não influenciou o OHIP-14. Os escores mais frequentes do OHIP-14 foram 0 e 1 . Conclusões: Os dentes pilares e não pilares das próteses parciais removíveis de indivíduos com fissura labiopalatina não apresentaram diferenças significativas nos parâmetros clínicos avaliados com exceção do nível de inserção clínico e profundidade de
sondagem, aceitando-se parcialmente a hipótese nula. O tipo de grampo a classificação das próteses não tiveram influência na qualidade de vida. Os pacientes necessitam de melhor orientação de higiene oral já que os índices de placa e de Tarbet apresentaram médios a altos escores.

A presente revisão aborda conceitos e princípios relativos à regeneração periodontal. Existem muitos procedimentos aplicados na prática clínica atual com o intuito de restituir a forma e a função dos tecidos perdidos através da doença periodontal. Como tal, há que perceber como é fundamentada, em termos biológicos, a utilização destes procedimentos, quais as suas limitações e qual a direção que os estudos realizados na área da regeneração periodontal estão a tomar.
Deste modo, foi feita uma pesquisa bibliográfica, utilizando o motor de busca Medline/Pubmed, inserindo as seguintes palavras-chave: “Periodontal Regeneration”, “Periodontal Development”, “Periodontal Biology”, “Bone Formation”, “Cementum Formation”, “Periodontal Ligament”, “Stem Cells”, “Periodontal Engeneering”, “Tissue Engeneering”, “Periodontal Wound Healing”.

▼ A relação entre periodontite e doenças cardiovasculares (DCV) tem sido amplamente discutida, embora os mecanismos de interação não estejam claros. Foi sugerido que pacientes com periodontite apresentam inflamação sistêmica e um pequeno aumento no risco cardiovascular. Patógenos periodontais foram encontrados em placas de ateroma, mas a influência destes microrganismos na aterosclerose ainda não é compreendida. O objetivo primário deste estudo foi delinear o perfil inflamatório sistêmico de pacientes submetidos à endarterectomia carotídea. Os objetivos secundários foram associar o perfil inflamatório dos participantes à condição periodontal e à presença de patógenos orais encontrados na cavidade oral e em placas de ateroma. Trinta e cinco pacientes submetidos à endarterectomia carotídea foram incluídos neste estudo. Antes da cirurgia vascular, um exame clínico periodontal foi realizado e foram coletadas amostras de sangue, saliva e biofilme
subgengival. Durante a endarterectomia, uma amostra da placa de ateroma foi coletada. As amostras de soro foram testadas com o ensaio imunoenzimático de alta sensibilidade Multiplex para dezessete marcadores de células Th17. Amostras de saliva, biofilme subgengival e placa de ateroma foram submetidas ao PCR quantitativo para avaliação de dez patógenos periodontais. Este estudo foi capaz de detectar vários marcadores inflamatórios circulantes, o que indica a presença de inflamação sistêmica como uma característica da população. T. forsythia foi o microrganismo mais frequentemente encontrado em amostras de ateroma (37% das amostras). Níveis de T. forsythia em amostras de ateroma foram positivamente correlacionados com os níveis séricos de IL-7, IL-6, IL-17, IL-13, IL-12p70, IFN-?, GCS-F (p<0,05) e IL-10 (p<0,01). Níveis séricos de IL-2 foram positivamente correlacionados com os concentrações salivares de P. intermedia, P. endodontalis (p<0,05) e T. denticola
(p<0,01). Níveis séricos de TNF-? foram positivamente correlacionados com concentrações salivares de P. endodontalis (p<0,01). Concentrações de P. endodontalis em amostras subgengivais foram correlacionadas positivamente com IL-2 (p<0,05). A inflamação periodontal (PISA) foi positivamente correlacionada com IL-2 (p<0,05). A coexistência de patógenos periodontais comuns na cavidade oral e na placa de ateroma está associada a um estado inflamatório sistêmico, o que poderia ser relevante para a compreensão dos mecanismos que ligam periodontite com DCV.

▼ Periodontitis is inflammatory and infectious disease on attachment tissue of teeth and it is one of principal reason for tooth loss. Periodontal treatment consists on scaling and root planning associate with high standard oral hygiene with aim of reduce periodontal inflammation levels. Stability of attachment levels is a major aim to be achieved after periodontal treatment conclusion. Although periodontal treatment effectivity and need in cooperate with supportive periodontal treatment (SPT) be well established in literature, a little is known about periodontal status and follow-up of individuals who use philanthropic service. The aim of this study was to evaluate periodontal status of individuals treated/retreated on a philanthropic service. At first, 77 individuals who did periodontal treatment after a mean of 3.5 years were evaluated. Demographic and socioeconomic data were collected by structured interview. Periodontal variables evaluated were:
Plaque Index (PlI), Gingival Index (GI), Probing Depth (PD), Clinical Attachment Level (CAL) and Bleeding on Probing (BoP). Individuals who showed high percentage of BoP (≥25%) were lead to retreatment. The final exam was done, in mean, 2.4 years after retreatment and individuals were categorized on three groups: did not receive any dental retreatment (NDentT); received some dental treatment, except periodontal (DentT); and those who received periodontal retreatment (PerioRT). The group which received periodontal retreatment had lower GI mean when compared with group that did not receive any dental treatment (P<0.05), however, they stayed with high BoP percentages. Mean of PlI, PD, BoP and CAL were not different among groups (P≥0.05). It was not observed difference on disease progression among the groups (P≥0.05). Although individuals have possibility of a free dental treatment, those who were retreated at this place showed no significant changes on BoP percentage and PD≥5mm
with BoP percentage. These results may be related to the fact patients did not return to do regular and individualized STP.
Periodontite é uma doença infecto-inflamatória que acomete os tecidos de suporte dos dentes e uma das principais responsáveis por perdas dentárias. Seu tratamento consiste na raspagem e alisamento radicular associado à medidas de higiene bucal adequadas, com objetivo de reduzir os níveis de inflamação periodontal. Estabilidade dos níveis de inserção é um dos maiores objetivos a serem alcançados após a conclusão do tratamento periodontal. Embora a efetividade do tratamento periodontal e da cooperação com a manutenção periódica preventiva (MPP) estejam bem estabelecidas na literatura, pouco se sabe sobre as condições periodontais e o acompanhamento de indivíduos atendidos no serviço filantrópico. ▼ Objectives: The first specific aim of this thesis was to define the presence, assess the distribution, and characterize PRX1 expressing cells within the periodontium. The second specific aim was to assess the contribution of PRX1 expressing cells to the
periodontal formation, post-natal
periodontal hemostasis, and
periodontal regeneration.
Methods: To assess the significance of the expression of PRX1 in human periodontium, we tested the expression of the Prx1 gene in samples of human bone,
periodontal ligament (PDL), and
periodontal stem cells (PDLSCs). The presence of PRX1 expressing cells (PRX1+ cells) in the mouse periodontium was explored by means of in vivo imaging. The distribution of PRX1+ cells in mouse periodontium was investigated in inducible lineage tracing analysis performed postnatally (Prx1-CreER-EGFP+/-;Rosa26-tdTOMATO+/-). PRX1+ cells also were isolated from
periodontal ligament of mouse mandibular incisors and their gene expression pattern was explored. The contribution of PRX1+ cells to the formation of the mouse periodontium was investigated by constitutive lineage tracing analysis performed during embryogenesis (Prx1-cre+/-;Rosa26-tdTOMATO+/-). By means of diphtheria toxin (DTA)-mediated conditional ablation of PRX1+ cells, we also explored their contribution to
periodontal homeostasis by assessing the width of PDL in Prx1-CreER-EGFP+/-;Rosa26-DTA+/- mice (ablation mouse model) (n=5) and their non-ablating littermates. The contribution of PRX1+ cells to
periodontal regeneration was assessed by creating sub-critical size
periodontal fenestration defects by the mandibular incisor and first molar teeth. The healing of defects was compared in ablation mouse model (test group) and non-ablation mouse model (control group) (n=5).
Results: Gene expression analysis of human samples confirmed that PRX1 is significantly expressed in human PDL and PDLSCs. The gene expression signature of PRX1+ cells demonstrated the expression mesenchymal stem cells markers, up-regulation of Notch signaling, and down-regulation of Wnt signaling. Lineage tracing analyses showed that PRX1+ cell are present in high abundance within the PDL of mouse incisor teeth, and PRX1-lineage cells contribute to the formation of the mouse incisor’s periodontium. Post-natal ablation of PRX1+ cells resulted in a significant enlargement of the
periodontal ligament space and lack of
periodontal regeneration in sub-critical size
periodontal defects in the continuously regenerating
periodontal ligament of mouse incisor.
Conclusions: PRX1 expressing cells are present in high abundance within the continuously regenerating
periodontal ligament of mouse incisor teeth and at such location they contribute to
periodontal formation,
periodontal homeostasis, and
periodontal regeneration. This study, for the first time, reports the presence of PRX1 expressing cells within human
periodontal ligament. Thus, studying mouse

health may be explained by the same factors that are associated
with clinical
periodontal health status, it has been suggested as a
useful measure for service planning and for monitoring
periodontal
health in developing and resource-limited countries, where
logistics and the costs of clinical oral surveys may be major
barriers to risk factor surveillance. Objectives: To determine the
systemic health and lifestyle factors associated with self-reported
poor
periodontal health status in South Africa. Data source: The
second South African Demographic and Health Survey (SADHS) done in
2003. Methods: This secondary data analysis focused on data on
dentate adults aged ≥15 years who participated in the 2003 SADHS
(n=6,319). Information obtained included socio-demographic data,
health risk behaviours (tobacco and alcohol use) and chronic
diseases. Nutrient intake was computed based on food frequency
items contained in a Nutrient Index (N-Index) developed for use in
South Africa. Taking into account the complex sample design used in
the SADHS, data analysis included the use of t-test, Chi-square and
multiple logistic regression analysis. The level of significance
was set at p<0.05. Results: Of the respondents, 4.6% (95% CI =
3.9 - 5.5) self-reported having had a ‘gum problem’ or poor
periodontal health in the 6 months prior to the survey date. In the
general dentate population, those who reported poor
periodontal
health were older than those who did not report poor
periodontal
health at a significant level (36.2% vs. 38.8%; p = 0.02); and they
were more likely to be problem drinkers (2.53; 95% CI = 1.68 –
3.82), as compared to non-drinkers. Having suffered a stroke (4.13;
95% CI = 1.53 – 11.11) or suffering from arthritis (1.70; 95% CI =
1.00 = 2.90) were significant associated with higher odds of
reporting poor
periodontal health. Black South Africans have higher
odds of reporting poor
periodontal health (3.91; 95% CI = 1.38 –
11.05) than white South Africans. On further stratifying the study
participants into younger (≤45 years) and older (>45 years)
adults, factors associated with poor
periodontal health were found
to be different. In particular, reporting making yearly preventive
dental care visits was significantly associated with reporting poor
periodontal health only among younger adults (OR = 0.40; 95%CI =
0.18 – 0.90), while the racial gradient in reporting poor
periodontal health remained significant only for older adults.
Conclusion: The study’s findings highlight the need to integrate
oral health promotion with general health promotion programmes,
especially those targeted at chronic disease prevention and
rehabilitation.

▼ Combinations of periodontal infection and destructive immune response are contributive factors to aggressive periodontitis and gingivitis, resulting in a poor prognosis to periodontal disease (Pihlstrom et al. 2005). Probiotic application using lactobacilli can be promising to counteract the periodontal disease. Probiotic owns unique properties and expresses beneficial roles in improving systemic health and also reducing the incidence of dental caries (Haukioja 2010). Hence, the main purpose of my Ph.D. study is to evaluate the dynamic of probiotic lactobacilli in the interaction with the periodontal pathogen and innate immune cells in denoting a positive role to prevent the periodontal disease. In this study, dynamic of periodontal pathogens and probiotic lactobacilli were evaluated via the following 4 approaches; 1) Inhibitive interaction between probiotic lactobacilli and periodontal pathogens via direct and indirect co-culture approaches, 2) Counteraction for the mature biofilm of the periodontal pathogens, Aggregatibacter actinomycetemcomitans strains by the potential probiotic lactobacilli, 3) understanding the biofilm formation of periodontal pathogens on hydroxyapatite surfaces, and 4) activation of phagocytic function by probiotic lactobacilli in combating A. actinomycetemcomitans infection. Our finding shows, 1) different interaction of inhibition was found in the direct and indirect co-culture method, by which significant bactericidal activity by certain species of probiotic lactobacilli was observed in the indirect approach, whereas direct co-culture showed vice versa, 2) probiotic lactobacilli were potently degrading the matured formed-biofilm of A. actinomycetemcomitans strains, 3) varied effect of biofilm formation on the hydroxyapatite surfaces were observed between two periodontal pathogen species incorporated with complex calcium intake, 4) probiotic lactobacilli showed the potential to induce the murine macrophage polarization prior A. actinomycetemcomitans Y4 infection. In conclusion, probiotic lactobacilli has found to own beneficial role against the periodontal pathogens activity and modulate the macrophage activity toward prevention of periodontal disease. This findings are indeed important in providing essential information on the physiological dynamics of periodontal pathogen and probiotic lactobacilli, meanwhile characterized the new insight into how probiotic lactobacilli roles to counteract the activities of the periodontal pathogen and modulate the innate immune system towards a preventive mode of periodontal disease.

▼ OBJECTIVE: Periodontal disease is a highly prevalent inflammatory disease with a wide range of causes and clinical manifestations. Excessive alcohol consumption is a significant public health problem, and is a risk factor for a variety of diseases; however, the relationship between alcohol intake and overall oral health remains unclear. This study seeks to identify the relationship between heavy alcohol consumption (consuming two or more drinks per day) and specific indicators of periodontal health.
METHODS: Cross-sectional analysis utilized data from the Dental Longitudinal Study, a longitudinal study conducted at the Boston Veterans Affairs medical center, in which the initial cohort consisted of healthy male veterans residing in Greater Boston. Using demographic and behavioral information collected from surveys, and oral health data collected from clinical examinations, bivariate data analysis was conducted to compare periodontal health outcomes between those who drank less than two drinks per day (non-drinkers and moderate drinkers, n = 949) and those who drank two or more drinks per day (heavy drinkers, n = 237). Selected oral health outcomes for analysis were indicators of poor periodontal health and included: number of teeth, bleeding on probing, calculus and plaque levels, tooth mobility, alveolar bone loss, periodontal pocket depth measurements, and gingival recession.
RESULTS: Results showed that the heavy alcohol consumption group was significantly associated with increased whole mouth mean alveolar bone loss and with periodontal pocket depths exceeding 4mm and 5mm, with a trend in heavy drinkers to have more teeth with increased levels of pocket depth.
DISCUSSION: The detected association between heavy drinking and alveolar bone loss and pocket depth measurement is a significant clinical finding, and suggests that alcohol intake should be minimized in the interest of periodontal health. The results of this study point towards the need for future longitudinal studies to investigate the possible role of alcohol as a risk factor for periodontal outcomes.

▼ Objetivos: A presente revisão narrativa tem como objetivo avaliar qual a terapêutica mais adequada para a sensibilidade radicular em pacientes periodontalmente comprometidos.
Materiais e Métodos: Efetuou-se uma pesquisa recorrendo à base de dados Pubmed/ME-DLINE. Apenas foram incluídas revisões sistemáticas e meta-análises, publicadas inglês ou português. Não foram empregues quaisquer limites temporais. A pesquisa resultou num total de 8 artigos.
Resultados: Devido à grande heterogeneidade e dispersão de resultados entre os estudos, não foi possível identificar qual a terapêutica mais vantajosa e com eficácia superiormente comprovada na redução da sensibilidade radicular, em pacientes com periodontite.
Conclusões: São necessários mais estudos, com uma maior evidência científica, para se poder aconselhar uma determinada abordagem terapêutica em detrimento de outra.
Objectives: The present narrative review aims to evaluate the most appropriate therapy for root sensitivity in periodontally compromised patients.
Materials and Methods: A search was performed using the Pubmed / MEDLINE data-base. Only systematic reviews and meta-analyzes published in English or Portuguese were included. No time limits were employed. The search resulted in a total of 8 items.
Results: Due to the great heterogeneity and dispersion of results between the studies, it was not possible to identify the most advantageous therapy with proven superior effec-tiveness in the reduction of root sensitivity in patients with periodontitis.
Conclusions: More studies, with more scientific evidence, are necessary to be able to advise one therapeutic approach over another.

▼ Depuis la fin des années 1980, de nombreuses études épidémiologiques ont montré une association positive et indépendante entre les maladies cardiovasculaires et les maladies parodontales. Les maladies parodontales, responsables de bactériémies transitoires et répétées, ont la capacité d’initier ou d’aggraver les lésions d’athérosclérose. Deux mécanismes pathogéniques majeurs sont aujourd’hui avancés. L’un est indirect par augmentation des médiateurs de l’inflammation induite par les parodontites et l’autre est direct lié au microbiote oral. Cette hypothèse majeure repose sur deux mécanismes non exclusifs, la translocation des bactéries parodontales au sein des plaques d’athérosclérose par la circulation générale et sur la modification du microbiote intestinal. L’objectif de la thèse était d'explorer l'association entre l’exposition systémique aux bactéries parodontales et leur présence au sein des plaques d’athérosclérose sur les complications cardio- et neuro-vasculaires chez l’homme. Les résultats obtenus montrent que les bactéries parodontales sont associées à un phénotype vulnérable des plaques d’athérosclérose coronariennes et carotidiennes. Les lésions d’athérosclérose compliquées par des hémorragies intraplaques sont le siège d’une activation plus intense des neutrophiles. Les effets délétères des leucocytes sur les tissus vasculaires sont potentiellement augmentés et entretenus par les bactéries majorant la vulnérabilité à la rupture des plaques d’athérosclérose. La confirmation clinique de la relation biologique entre le microbiote parodontal et les complications cardio- et neuro-vasculaires de l’athérosclérose est une perspective importante de cette thèse.
Since the late 1980s, many epidemiological studies have shown a positive and independent association between cardiovascular disease and periodontal disease. Periodontal disease inducing transient bacteraemia, can initiate or worsen atherosclerotic lesions. Two major pathophysiologic mechanisms are supported by evidence. One is by indirect increase in inflammatory systemic periodontitis-induced mediators and the other is directly related to the oral microbiota. This major hypothesis is based on two non-exclusive mechanisms, the translocation of periodontal bacteria in atherosclerotic plaques by the general circulation and the impact on the gut microbiota.The aim of the thesis was to explore the association between systemic exposure to periodontal bacteria and their presence in atherosclerotic plaques on the cardiovascular and neurovascular complications in human.The results obtained show that periodontal bacteria are associated to a vulnerable phenotype of coronary and carotid atherosclerotic plaques. Atherosclerotic lesions complicated by intraplaque haemorrhage are the trigger of a super activation of neutrophils. Na presente monografia, pretendeu-se fazer uma abordagem, do ponto de vista teórico, da informação disponível na literatura sobre lesões metastáticas no periodonto.
A periodontite é uma doença inflamatória crónica de etiologia bacteriana que leva à libertação de citoquinas inflamatórias, prostaglandinas, fatores de crescimento e enzimas. Estes mediadores inflamatórios estão também associados ao desenvolvimento do cancro, podendo atrair células tumorais.
Quase todos os tipos de tumores malignos podem metastizar na cavidade oral. Os locais primários mais frequentes são, nos homens, pulmão, rins, fígado e próstata; nas mulheres, mama, órgãos genitais femininos, rins e colo-retal.
A metastização óssea é mais provável do que nos tecidos moles orais, no entanto, é um acontecimento raro. As áreas hematopoiéticas nos maxilares favorecem uma deposição precoce das células tumorais. A extensa rede capilar da gengiva cronicamente inflamada pela doença periodontal pode reter células malignas. Os capilares em proliferação possuem uma membrana basal fragmentada, tornando-os mais permeáveis às células tumorais.
As metástases na cavidade oral são geralmente assintomáticas, no entanto, podem apresentar sinais e sintomas que podem conduzir a um diagnóstico errado. Deste modo, eles devem ser considerados no diagnóstico diferencial de lesões inflamatórias e reativas que são comuns na região oral. Devido à sua raridade, o diagnóstico de uma lesão metastática na região oral é um desafio, tanto para o clínico como para o patologista, não só no reconhecimento de que se trata de uma lesão metastática, mas também na identificação do local de origem.
O possível papel da inflamação na distribuição das células metastáticas no periodonto tem sido levantada, no entanto, esta observação não tem sido suportada estatisticamente.

O médico dentista deve ver o paciente como um todo. Na mulher em particular, deve ser tida em conta a fase hormonal em que se encontra, devido às alterações que estas podem provocar na cavidade oral. A puberdade, o ciclo menstrual, a gravidez e a menopausa, constituem as fases da vida da mulher que influenciam especificamente a saúde oral, nomeadamente periodontal.
A menopausa, é o período da vida da mulher em que ocorre a última menstruação (consequência da falência ovárica definitiva), é quando os níveis de hormonas sexuais diminuem, e a mulher pode experimentar mudanças relacionadas com a mucosa oral.
A doença periodontal é uma infeção crónica grave na qual existe uma destruição do suporte dentário, e constitui uma doença muito comum, principalmente em idade mais avançada.
Problemas periodontais também são encontrados associados ao período da menopausa, embora muitas vezes relacionados com osteoporose. A redução de estrogénio na menopausa, pode conduzir ao aparecimento de problemas periodontais, embora esta relação não seja completamente clara.
A maioria da literatura encontrada, dá ênfase à necessidade da realização de maior número de estudos longitudinais, com amostras de maior dimensão e equilibradas em mulheres nos vários estadios hormonais ao longo da vida. O aumento da população idosa contribui para a predominância da população feminina no período da menopausa e pósmenopausa, e ainda para o aumento da frequência da doença periodontal, daí a informação sobre a etiopatogenia da menopausa e compreensão da sua relação com a periodontite ser de grande importância, uma vez que, atuando na prevenção e deteção antecipada, é possível travarmos o desenvolvimento da doença periodontal.

▼ Vitamin D (VitD) deficiency is commonly observed in women and among people living with human immunodeficiency virus (HIV). Abnormally low serum VitD levels are often associated with negative health outcomes. Association between VitD status and periodontitis in HIV has yet to be explored. In this study we examine the association of VitD serum levels with the severity of
periodontal disease in HIV + women. We hypothesized a positive association between VitD deficiency and poor
periodontal health.
Materials and Methods: Serum 25(OH) VitD and mean (m)
Periodontal Disease markers (number of teeth (NoT), Clinical Attachment Loss (CAL), Probing Depth (PD), Bleeding Index (BI), Plaque Index (PI) and
Periodontal Disease Diagnosis (CDC/AAP definition) were investigated cross-sectionally in 75 HIV+ Chicago Women's Interagency HIV oral substudy participants between 1995 and 2003. VitD deficiency was defined as <20ng/ml. Linear regression was used to determine associations between PDmarkers and VitD deficiency for the total sample (including smoking status) and with stratification by current tobacco smoking status. All available potential covariates (age, race, education, smoking, mPI, VL, CD4 and use of HAART) were entered into each Linear Regression model. Only the variables that were significant on the 0.10 level were retained in the final multivariate model. Association between PDmarkers and VitD Deficiency, after confounder adjustment, was considered statistically significant when P value was <0.05.
Results: Participants were predominantly African Americans (71%) and smokers (52%); mean age was 39.5 y/o; 43% had completed high school. Mean Biomarkers levels were VitD (20.2ng/ml), HIV Viral Load (VL) (123x103 copies/ml), CD4 count (351/mm3). 12% reported using highly active antiretroviral therapy (HAART). Average NoT was 20.7, mPD (1.7mm), mCAL (1.2mm), mBI (0.2) mPI (0.9); 16% met the criteria for moderate/severe
Periodontal Disease. Among HIV+ non smokers, VitD Deficiency was associated with greater mCAL (p=0.049/β-coefficient=0.278) in multivariate models. Among HIV+ smokers, VitD Deficiency was only associated with mBI (p=0.002 β-coefficient=-0.329) in multivariate models.

Introdução: A diabetes é uma patologia sistémica associada a uma elevação crónica dos níveis de glicose. É causada por uma absoluta, ou relativa, falta de insulina devido a uma insuficiente produção pelo pâncreas ou uma deficiência da sua ação no organismo. Uma das complicações da condição diabética é a doença periodontal (Correia et alii, 2010), verificando-se que indivíduos com diabetes mellitus têm um elevado risco de sofrerem de doença periodontal (Almeida et alii, 2006). Na verdade, não é só a prevalência da doença periodontal que está aumentada em indivíduos diabéticos, também a sua progressão e severidade é mais rápida e agressiva (Almeida et alii, 2006). No entanto, o inverso é igualmente verdade. Os indivíduos com doença periodontal não tratada têm também um pior controlo dos níveis de hemoglobina glicosilada (HbA1c). A relação entre ambas as doenças parece ser bidirecional, mas será que o tratamento da doença periodontal tem efetivamente um efeito positivo sobre o controlo glicémico?
Objetivo: Avaliar a relação entre a diabetes e a doença periodontal, incidindo particularmente sobre o possível efeito da terapia periodontal no controlo glicémico de doentes diabéticos.
Materiais e Métodos: Realizou-se uma pesquisa bibliográfica recorrendo à base de dados da PUBMED e da b-ON, dando-se especial ênfase a revisões sistemáticas e a meta-análises. Apenas foram pesquisados artigos em inglês e em português, não tendo sido empregues quaisquer limites temporais.
Resultados/conclusões: Após a análise da literatura pode-se concluir que a terapia periodontal tem, de facto, um efeito benéfico sobre o controlo glicémico em particular de diabéticos tipo 2. Ficou ainda claro que a promoção de saúde oral em pacientes diabéticos é de extrema importância. No entanto, mais estudos são necessários.
Introduction: Diabetes is a systemic condition in which there is a chronic increase of glucose levels caused by an absolute or relative lack of insulin due to insufficient production by the pancreas or a deficiency of their action in the body. One of the complications of the diabetic condition is the periodontal disease (Correia et alii, 2010). The periodontal disease is considered the sixth complication of diabetes and is well documented in the literature that individuals with diabetes mellitus have a high risk of suffering from periodontal disease (Almeida et alii 2006). In fact, it is not only the prevalence of the periodontal disease that is increased in diabetic patients, but also its progression and severity is faster and more aggressive (Almeida et alii, 2006). Микрофлора усне дупље је разноврсна и јединствена у
поређењу са микрофлором која се налази у другим деловима људског
тела, услед специфичних биолошких и физичких услова који владају у
оралној средини. До сада је изоловано преко 700 различитих врста
бактерија из оралне средине и скоро свакодневно нови сојеви бивају
идентификовани. Локални и системски штетни ефекти оралних
бактерија, посебно пародонтопатогених микроорганизама, до сада су
врло темелјно испитивани. Бројни аутори су се бавили истраживањем
састава оралног биофилма код пацијената рехабилитованих зубним
надокнадама. Међутим, сама промена до које надокнаде доводе,
односно поређење присуства пре израде и након периода ношења
надокнада код истог пацијента није била у центру пажње истраживача.
Основни циљ истраживања био је да се утврди веза између различитих
врста зубних надокнада и присуства пародонтопатогених бактерија
Porphyromonas gingivalis, Treponema denticola, Tannerella
forsythia, Fusobacterium nucleatum, Prevotella intermediaи
Aggregatibacter actinomycetemcomitans. Истраживање је дизајнирано
као проспективна студија. У студији је учествовало 90 пацијената
подељених у три групе по 30. Прву групу чинили су пацијенти код
којих је била индикована израда и горње и доње тоталне протезе,
другу групу пацијенти којима је била индикована израда једне
метало-керамичке крунице, а трећу групу пацијенти код којих је била
потребна рехабилитација комбинованим фиксно-мобилним радом са
прецизним везним елементима (кугличасти атечмени). Након укључења у
студију пацијентима је урађен детаљан стоматолошки преглед и након
припреме приступило се изради индикованих зубних надокнада за
сваког пацијента посебно. Пре предаје зубне надокнаде обављено је

▼ This paper studies the correlation between three widely common chronic diseases, periodontal disease, diabetes, and obesity. The three diseases share one key factor; they not only affect human's health but also the style and quality of life. This paper evaluated the current literature regarding periodontal disease, diabetes, obesity, and their triangular relationship. The studies showed that the three chronic diseases are related to each other in a triangular relationship. Increased adiposity manifested by BMI greater than 30, leads to increased systemic inflammation through the release of inflammatory adipokines from the fatty deposits, this leads to the development of diabetes mellitus by different mechanisms, partly by destruction of the islet cells of Langerhans and partly by insulin resistance. One of the complications of diabetes mellitus is periodontal disease, which is a list of clinical conditions that adversely affect the health of the periodontium. Moreover, current literature provides evidence that the relationship between diabetes and periodontal disease is bidirectional. Evidence from current studies is showing that periodontal disease might lead to the development of diabetes by increasing the systemic inflammation, this occurs by the invasion of periodontal pathogens of the endothelial cells where they elicit an exacerbated systemic inflammation which might lead to the development of diabetes and other chronic conditions. This paper shows the importance of collaborative work between the dental and the medical professionals to ensure the overall health of an individual. Primary care physicians are encouraged to refer patients with poor glycemic control to dentists for an assessment of the health of their oral cavity and vice versa; dentists should be more aware of the systemic complications of oral diseases and should educate their patients about such relationships.

▼ The importance of oral health on systemic health is a highly researched area of study in recent years. There has been a shift in dental visits from acute emergencies to ongoing preventative care due to the knowledge connecting oral and systemic health. One of the commonly researched connections is the link between periodontal disease and heart disease. Periodontal disease is defined as inflammation of the gum tissue, resulting in periodontal pockets that can lead to infection, bone loss and even loss of the tooth. Cardiovascular disease, or heart disease, is a term that encompasses many different conditions of the heart, including heart failure, myocardial infarction, atherosclerosis and angina. There is constant research to better understand the relationship between the two diseases, as well as any causality that may exist. Recent studies have been able to link the diseases, but no causal link has been found. The role of the bacteria involved in both diseases has recently been considered to see if these organisms are related to a potential causal link.
Two particular bacteria that are known to be involved with periodontal disease are Porphyromona gingivalis and Treponema denticola. These bacteria are present when a patient develops periodontal disease, but they are not usually present in a healthy individual. Additionally, the bacteria that make up the contents of plaque found in the heart have been studied to see if there are any similarities with oral microbes. It has been found that oral bacteria can be present in arterial plaque samples. This research may allow a better understanding of how and why heart disease occurs and potentially serve as a way to treat heart disease accompanied by periodontal disease, if a causal relationship is elucidated.
Heart disease is usually a devastating disease, sometimes resulting in the death of the patient. If more patients attend the dentist as a preventative measure, the risk of periodontal disease and associated pathologies may be reduced. Additionally, those that have already developed periodontal disease can work with a dental professional to reverse the disease. It is known that the bacteria in the mouth can enter the bloodstream upon infection, so patients with suspected periodontitis should be treated to avoid the bacteria from entering the blood and affecting other organs such as the heart.
An examination of the bacteria commonly found in the oral cavity at times of periodontal disease may lead to a better understanding of how and why these bacteria invade the bloodstream. It would be beneficial to compare the microbiota of both the plaque in the mouth and the plaque in a vessel supplying the heart in a patient suffers from heart disease. This understanding may lead to therapeutic interventions that aid in the prevention of bacteria traveling in the bloodstream.
