Varhaislapsuuden karies on merkittävä kansanterveysongelma varsinkin lapsirikkaissa maissa ja väestöissä. Karieksen hoitaminen vie paljon voimavaroja ja aiheuttaa mittavia taloudellisia seuraamuksia. Karies voi ilmaantua lapselle jo vauvaikäisenä, pian ensimmäisten maitohampaiden puhjettua suuhun. Alle 3-vuotiaiden karieksesta on kuitenkin niukasti tilastotietoja. Maailman terveysjärjestökin suosittaa tietojen keräämistä vasta 3-vuotiaiden ikäryhmästä. Heistä kariesta sairastaa Suomessa 16 %, Yhdysvalloissa 25 %, Englannissa 30 %, Iranissa 46 % ja Saudi-Arabiassa 61 %.
Tämä väitöstutkimus selvitti karieksen esiintymistä ja sen vaaratekijöitä 1─3-vuotiailla Teheranissa. Lisäksi tutkimus arvioi perusterveydenhuoltoon sisällytetyn karieksen ehkäisyn tuloksellisuutta. Tutkimuskohteiksi arvottiin Teheranista 18 neuvolaa. Jokaisessa oltiin 4 päivää, jolloin kaikkia rokotuksiin tulleita 1─3-vuotiaita äiteineen pyydettiin osallistumaan tutkimukseen. Kahta lukuun ottamatta kaikki äidit suostuivat, ja aineistoon tuli kaikkiaan 504 lasta äiteineen. Kaikki 1-vuotiaat, 242 lasta äiteineen, valittiin karieksen ehkäisykokeiluun. Sitä varten neuvolat jaettiin kolmeen ryhmään, joista kaksi (A ja B) oli koeryhmiä ja yksi (C) oli vertailuryhmä.
Tutkimus alkoi äidin haastattelulla. Siinä selvitettiin perheen koulutus- ja tulotaso sekä lapsen ruokinnasta imetyksen kesto, yösyötöt ja päiväaikaan nautitut makeat. Vielä kysyttiin lapsen ja äidin suuhygieniatavoista ja äidin kokemuksista lapsen suun puhdistamisessa. Sitten hammaslääkäri tutki lapsen suun ja kirjasi karieksen ja hammasplakin esiintymät.
Suun tutkimuksen jälkeen äiti ja lapsi siirtyivät rokotushuoneeseen. Koeryhmissä (A ja B) äidit saivat terveydenhoitajalta suunterveyttä koskevan esitteen ja kehotuksen lukea se huolellisesti. Lisäksi ryhmässä A terveydenhoitaja kertoi suun ja hampaiden terveydenhoidosta saman esitteen avulla, ja neuvolan henkilökunta muistutti suunhoidon tärkeydestä puhelimitse kahdesti seuraavan puolen vuoden kuluessa. Vertailuryhmässä äideille ei annettu suunhoidon ohjeita. Kaikissa ryhmissä äitejä muistutettiin seuraavan rokotuskerran ajankohdasta, muttei mainittu tulevaa toista hammastarkastusta.
Varhaislapsuuden kariesta sairasti ikäryhmästä riippuen 3─26 % tutkituista 1─3-vuotiaista, ja 65─76 %:lla oli hammasplakkia. Äideistä 68 % harjasi hampaansa päivittäin ja 39 % puhdisti lapsensa suun päivittäin. Mitä useammin äiti harjasi omat hampaansa, sitä paremmin hän huolehti lapsen suun puhtaudesta. Rintaruokinta oli yleistä eikä lisännyt kariesvaaraa. Yöllä pullomaitoa saavilla karies oli 5 kertaa yleisempää kuin muilla. Neuvolassa saatu ohjeistus ehkäisi selvästi karieksen syntyä puolen vuoden kokeessa.

▼ Suusyöpä Teheranissa, Iranissa 1993-2003
Tämän väitöskirjan tavoitteena oli kuvata suusyövän yleisyyttä ja siihen vaikuttavia tekijöitä Teheranissa, Iranissa tutkimalla suusyöpäpotilaita, suusyöpäkasvainten ominaisuuksia, potilaille tehtyjä diagnooseja ja niiden viivästymistä sekä heidän selviytymistä sairaudestaan.
Suusyöpäkasvainten tietoja kerättiin 1042 suusyöpäpotilaalta. Nämä tiedot kerättiin 30 suurimman Teheranilaissairaalan potilaskortistoista vuosien 1993-2003 ajalta. Eloonjäämisanalyysiä varten tiedot kerättiin vuosien 1996-2003 arkistoista 470 suusyöpä- ja 82 huulisyöpäpotilaan osalta ja heitä seurattiin vuoden 2005 loppuun.
Potilaan kokemien ensioireiden ja lopullisen syöpädiagnoosin välistä viivettä varten kerättiin tiedot Teheranilaisista sairaaloista 100 peräkkäisen suusyöpäpotilaan tiedoista vuosien 2004-2006 välillä. Ns diagnostinen viive jaettiin kahteen osaan: 1) ensioireiden ja ensimmäisen sitä seuranneen lääkärikäynnin väli ja 2) ensimmäisen lääkärikäynnin ja lopullisen diagnoosin välinen ero.
Useimmat suusyövät olivat pitkälle edenneitä diagnoosin tekemisen hetkellä, kasvain oli siis yli 4 senttimetriä halkaisijaltaan ja/tai kaulan alueen imusolmukkeissa oli jo etäpesäkkeitä. Eloonjäämistodennäköisyys viiden vuoden aikavälillä oli suusyöpäpotilaille 30% ja huulisyöpäpotilaille 62%, mitkä olivat merkittävästi alempia kuin yleisesti länsimaissa vastaavat luvut. Tämä tutkimus osoitti, että keskimääräinen diagnostinen viive oli korkea (7,2 kk, SD 7,5), erityisesti kun niitä verrataan kehittyneimpien terveydenhuoltojärjestelmien vastaaviin tietoihin. Yleensä potilaasta aiheutuva viive oli huomattavan suuri ensioireiden ja lopullisen diagnoosin välisestä ajasta.
Tässä tutkimuksessa tehtyjen havaintojen pohjalta on perusteltua esittää kehitettäväksi ennaltaehkäisevä tiedotusohjelma, jossa kansalaiset voisivat saada enemmän tietoa suusyövästä, sen ensioireista jotta he hakeutuisivat aikaisemmin hoitoon. Lisäksi terveydenhoitohenkilöstöä, erityisesti hammaslääkärejä ja suuhygienistejä tulisi kouluttaa varhaisen diagnoosin tekemiseksi, jotta Iranissa tehtävien suusyöpähoitojen lopputulokset paranisivat.
Oral Cancer in Tehran, Iran: An approach for understanding disease burden.
The present study investigated the burden of oral cancer in Tehran, Iran in terms of patient and tumour characteristics, survival rate, and delay in diagnosis, with the main focus on oral cavity cancers.
For exploring the characteristics of malignant oral tumours, data were obtained from records of 1042 patients diagnosed with invasive oral cancers during 1993-2003 in 30 major hospitals in Tehran, and analysed in three groups: tumours of the lips, oral cavity, and major salivary glands. For survival analysis, 470 primary oral cavity and 82 lip cancer patients diagnosed during 1996-2003 were followed from the date of diagnosis to late 2005. Introdução: Os princípios da promoção da saúde devem constituir uma referência a nível da higiene oral, passando pela aquisição de comportamentos individuais. Assim, é na infância, na juventude e na adolescência que se deve iniciar a prevenção e a obtenção de conhecimentos de forma a tornar as crianças mais pró-ativas, responsabilizando-as também da necessidade de uma higiene eficaz para manter a sua saúde oral como parte integrante de um todo inerente ao seu processo de crescimento e desenvolvimento.
Objetivo: O objetivo principal deste estudo é verificar se nas grandes cidades (Lisboa e Porto) o conhecimento sobre saúde oral quando comparado com cidades mais pequenas (Aveiro e Tavira) é diferente. Tem ainda como objetivos comparar comportamentos de saúde oral, analisar hábitos e conhecimentos das crianças sobre alimentação e avaliar a frequência de idas à consulta de Medicina Dentária.
Materiais e Métodos: Realizou-se a pesquisa bibliográfica de artigos sobre o tema nos motores de busca da PubMed, MedLine, SciELO e B-on com as seguintes palavras-chave “oral hygiene”, “oral health promotion”, “health education” publicados nos últimos 15 anos, bem como uma consulta de livros relacionados com o tema. Para a parte da investigação científica utilizou-se um questionário para a recolha da informação da população em estudo, 143 crianças com idades compreendidas entre os 10 e 13 anos. Este estudo foi realizado em duas fases. Na primeira fase foram entregues os questionários, o consentimento informado e uma breve explicação aos encarregados de educação para que pudessem autorizar a participação dos seus educandos e numa segunda fase foram recolhidos os mesmos, para posterior análise.
Resultados: De uma maneira geral, os resultados mostram que não existem grandes diferenças no que diz respeito ao conhecimento e ao comportamento dos alunos sobre saúde oral. No entanto, verificou-se que quando existem diferenças, estas estão relacionadas com o género e não com a escola onde estudam.
Conclusão: Enfatiza-se assim a importância de promover a saúde oral e de ensinar as crianças sobre estilos de vida saudáveis, permitindo o desenvolvimento de competências e hábitos corretos de higiene oral, uma vez que se verificam melhorias essenciais na qualidade de vida e bem-estar das crianças. ▼ Background: The University of Connecticut (UConn) School of Dental Medicine (SODM) routinely recruits dental students and faculty volunteers to conduct no-cost dental
health screenings for transient residents at South Park Inn Homeless Shelter in Hartford, CT. Once screened, patients are referred to UConn or another hospital-based dental clinic in the area for treatment. The purpose of this study was to determine the demographics and
oral health needs of the homeless population at South Park Inn and referrals to UConn SODM were followed to verify patient presentation at UCONN SODM. Dental students’ attitudes towards working with the underserved were also assessed. Methods: A retrospective analysis was conducted of 527 dental screening surveys administered at South Park Inn homeless shelter in Hartford, CT. Current UConn dental students that participated in South Park dental screens were administered a survey evaluating their experiences at the homeless shelter. Results: Half of the study participants had acute dental needs, 35% declared a chief complaint requiring prompt attention, 72% displayed clinically visible decay, and 36% had retained all of their adult teeth. Of the homeless residents that were referred to UConn for dental care, 18% had at least one dental visit at UConn SODM. In addition, the survey of dental students found that the majority of student volunteers believed their experience working with this homeless population increased their comfort with working with underserved populations and enhanced their dental education. Conclusion: This study informs public
health practitioners about the needs of the homeless population located in Hartford, Connecticut. A better understanding of
oral health issues common to the homeless can allow for an
oral health outreach program to be designed that is appropriately prepared to meet the needs of this community and is readily accessible by the homeless population.

▼ Introduction: Oral health is a major determinant in the overall quality of life of all
individuals. Oral health literacy, which is the ability of an individual to obtain, understand
and use oral health information needed to make informed choices concerning oral health, has
been put forth as one of the main influencers of oral health outcomes. This study is to relate
oral health literacy (OHL) to certain selected variables.
Method: A cross-sectional design was used for this survey. A sample size of 381 was
determined by EpiInfo. An interviewer-administered questionnaire was used to collect data
among KNUST students. REALD-30 toolkit was used to assess OHL and OHL levels.
Multiple linear and multivariate regression analysis was used to determine the predictive
associations between OHL and other variables such as college, gender, age, dental visit status
and frequency of toothbrushing.
Results: Social media was the most preferred medium of oral health education among
students (67.65%) while the most preferred medium for the general population was television
and radio (49.47%). 33.96% of the population had no prior exposure to any form of oral
health knowledge, while 33.18% had never visited a dentist. The mean OHL score was 12.10,
with 88.50% having low OHL levels, 8.82% having moderate OHL levels and 2.67% having
high OHL levels. Gender (C=-0.116, p=0.005), age (C=-0.025, p=0.003), college (C=-0.071,
p=0.000) and exposure to education (C=0.132, p=0.002) significantly predicted OHL levels.
Dental visit status (C=-0.15, p=0.013), frequency of visit (-0.29, p=0.008) and toothbrushing
frequency (C=-0.199, p=0.006) were also significantly predicted by OHL levels.

Conclusion: About a third of the population had neither been exposed to oral health
education nor visited a dentist. There is the need for further studies and further education of
students and the general population to be carried out through their preferred media.

▼ Children of low socioeconomic status (SES) families with intellectual and developmental disabilities have higher rates of dental caries but significant unmet dental needs due to unique factors such as low Medicaid reimbursement rates, confusion surrounding plan choice, and poor provider availability. As a result, children with disabilities experience disproportionate
oral healthcare costs and out-of-pocket expenses. Utilizing a critical disability theory lens to review the available published literature by
Health Policy, Medicaid and Special Education disciplines revealed not only how to describe what people should research but also how they should approach improving
oral health for children with special healthcare needs. Drawing on connections between the literature and the overall cultures of these fields it was concluded that existing literature used the medical model of disability. Therefore, ableism was identified as a cause of
oral health disparities for children with special healthcare needs. Adopting the social model of disability will improve
oral health outcomes for children with intellectual and developmental disabilities.

▼ Oral health education emphasizes the importance of brushing, flossing, and getting regular dental exams. Some examples of
oral health problems that can occur without proper
oral hygiene and dental care are dental caries and gingivitis. Research has shown that a relationship exists between
oral health and other
health related problems. The lack of awareness, education, and services regarding
oral health and hygiene can cause severe unfavorable outcomes later in life. Unique
oral health issues that affect today's college students include the use of smokeless tobacco, sexually transmitted diseases, and
oral piercing. Although there are many studies conducted on
oral health, few are targeted towards the college aged population. The purpose of this study was to develop a valid instrument based on constructs from the
Health Belief Model to measure the knowledge, beliefs, and behaviors of college students' regarding
oral health and hygiene. The present study utilized a review of the literature and an eight-step process to develop and validate College Students
Oral Health Knowledge, Beliefs, and Behaviors (OHKBB). The most effective way to treat
oral health related problems is through prevention.
Oral health education is needed to ensure that college students are aware of not only the risks but also the prevention of
oral diseases. The proposed research will pave the way for programs to help individuals improve their
oral hygiene and reduce their risks of
oral health related problems through education by identifying the specific
oral health related knowledge, beliefs, and behaviors that affect college students

oral health outcomes of children receiving cancer treatment. Aim and Objectives: This research was undertaken to assess
oral mucositis incidence and
oral care outcomes, and to explore possible risk factors for
oral mucositis among inpatient children receiving cancer treatment at the Women’s and Children Hospital, Adelaide, Australia. The objectives were to investigate the evidence on
oral mucositis prevention, assess and validate the combined use of the Children’s International
Mucositis Evaluation Scale (ChIMES) and the World
Health Organization (WHO)
oral mucositis scale in recording
oral mucositis incidence, develop and implement a standardized hospital
oral care protocol, and to record prospectively
oral mucositis incidence,
oral health outcomes, and possible risk factors. Methods: A systematic review was conducted to assess the current evidence on
oral mucositis prevention among children. Results of a previous retrospective study were used to design a prospective pilot study. The pilot study was carried out for seven months during which the new
oral care protocol was implemented and the ChIMES and the WHO
oral mucositis scale were validated through daily recording of
oral mucositis in the oncology ward. Measures of reliability and compliance were assessed among nurses and dental staff involved in recording
oral mucositis and
oral health status. The pilot study was followed by a prospective clinical observational study and recorded measures of
oral mucositis (12 months) and
oral health status (24 months). Measures of
oral health outcomes were assessed initially and then every three months through clinical examination to record dental caries and
oral hygiene while measures of
oral mucositis was recorded daily during the hospital stay of recruited children. The incidence of
oral mucositis,
oral health outcomes, and dental treatment utilization were then analyzed to explore possible risk associations. Results: The systematic review supported the benefit of implementing a standardized
oral care protocol to prevent
oral mucositis among children. Thirty-eight children were conveniently sampled during the pilot study during which high levels of reliability and compliance (87%) in using ChIMES and WHO
oral mucositis scales were achieved. Dental referrals increased from 53% to 100% after adopting the comprehensive
oral care protocol. Sixty-seven children were recruited during the prospective part with
oral mucositis incidence similar to that of the pilot study (33% versus 34%). Dental caries prevalence was 28% with absence of new carious lesions throughout the 24 months follow up. Regular dental reviews were significantly related to shorter duration of
oral mucositis (adjusted rate ratio=0.94; 95% CI=0.89-0.99; Pvalue= 0.026) and hence fewer days of hospital stay. On the…

oral health status. The objective of the study was to investigate the association between demographics,
oral hygiene & lifestyle behavior, self-described general
health and depression and
oral health status with access to healthcare services and use of fluoridated water as control variables.
Oral health status was defined by three dependent variables, namely, self-described
oral health (SDOH), self-described
oral health compared to peers (SDOHCP) and dental visits due to an
oral issue (DVOI). It was hypothesized that proper
oral hygiene and lifestyle behavior will be positively associated with
oral health status. It was also hypothesized that depression and fair/poor self-described general
health are associated with fair/poor self-described
oral health status. In addition, it was hypothesized that younger age will be associated with fair/poor
oral health status. 226 students responded to the survey with the majority being female (77.9%). Significant positive association between
oral hygiene behavior described by daily teeth brushing and excellent/good SDOH (P=0.03, OR=6.05, 95% CI 4.51-8.13) and better/same SDOHCP (P=0.05, OR=4.87, 95% CI 3.76-6.30), while significant positive association between frequency of teeth brushing was found with excellent/good SDOH (P=0.04, OR=2.12, 95% CI 1.03-4.37). Self-described general
health status was found to have strong positive association with both excellent/good SDOH (P=0.01, OR=5.32, 95% CI 2.24-12.63) and better/same SDOHCP (P=.01, OR=3.11, 95% CI 1.33-7.27). Access to
health care services described by frequency of dental visits and frequency of hygienist visits were found to have strong positive association with excellent/good SDOH (P=.01, OR=2.74, 95% CI 1.30-5.76) and (P=.01, OR=2.86, 95% CI 1.35-6.06), respectively and DVOI (P=0.01, OR=9.55, 95% CI 3.3-27.66) and (P=0.01, OR=13.45, 95% CI 4.03-44.83), respectively. Use of fluoridated water was found to have significant positive association with excellent/good SDOH (P=0.04, OR=2.12, 95% CI 1.01-4.45). These findings support the first hypothesis in this work that a positive association exist. No association was found between depression as an independent variable and any of the
oral health status indicators such as SDOH, SDOHCP and DVOI, where P>0.05 and 95% CI of OR crossed 1. This finding refutes the second hypothesis in this work, which states that a positive association exists. In addition, no association was found between younger age and SDOH, SDOHCP and DVOI, where P>0.05 and 95% CI of OR crossed 1. This result also refutes the third hypothesis in this work. Lifestyle behavior described by

▼ Bakgrund: MI är en metod för rådgivning och för att hjälpa individen att hitta och bibehålla motivation till förändrat beteende. På senare år har metoden börjat användas även inom tandvården. Syfte: Syftet med studien var att genom en litteraturöversikt sammanställa effekten av MI på parodontala sjukdomar och karies hos barn, ungdomar och vuxna. Metod: Studien består av en litteraturöversikt där vetenskapliga artiklar har tagits fram genom sökningar i databaserna PUBMED, DOSS, Medline, Psychinfo och CINAHL som sedan granskats och sammanställts. Sjutton artiklar har utvalts utifrån de inklusion/exklusion kriterier som fanns. De studierna som blev inkluderade i resultatet har blivit granskade med hjälp av en mall för att kunna identifiera studiernas värde för litteraturstudien. Resultat: De utvalda artiklarna var från Frankrike, Kanada, Kina, Tyskland, Iran, Indien, Serbien, Spanien, Sverige och USA. Ungefär hälften av artiklarna påvisade ett positivt resultat för patienter som genomgått MI-samtal i kombination med vanlig behandling jämfört med kontrollgrupper. Den andra hälften såg inga dylika resultat. Slutsats: Metoden tycks ha effekt i vissa sammanhang men inte i andra. En faktor som inte visade någon påverkan på effektiviteten av MI var patienternas ålder. Ytterligare forskning krävs för att fastställa vilka faktorer som påverkar resultatet vid användning av MI inom tandvården.
Background: MI is a method for counseling and for helping individuals find and maintain motivation for change of behavior. In later years the method has also seen some use in dental care. Aim: To study the effect of MI on parodontal diseases and caries in children, adolescents and adults. Method: The study design was a literature review where scientific articles were gathered by searching through databases like for example PUBMED, DOSS, Medline, Psychinfo and CINAHL and then scrutinized and summarized. Seventeen articles were selected according to existing inclusion-/exclusion criteria. The included studies were reviewed with a modified review template to present studies of strong/moderate value. Result: The selected articles were from Canada, China, France, Germany, Iran, India, Serbia, Spain, Sweden and USA. About 50% of the articles indicated a positive result for patients who undertook MI-sessions in combination with regular treatment, compared to control groups. The other half could see no such results. Conclusion: The method seems to have effect in some instances but not in others. A factor that showed no alteration of MI effectiveness was the patients’ age. Further research is required to determine what factors might influence the outcome of using MI in dental care.

Tässä työssä tutkitaan sadan pitkäaikaishoidon asumisyksikön asukkaan suun terveydentilaa ja sen mahdollista kuormitusta asukkaiden yleisterveyden kannalta. Näiden avulla määritellään uusi yhdistetty hammaskohtainen tulehdusindeksi, jonka avulla tarkastellaan koko suun kokonaistulehduskuormaa.
Lähtökohtana tutkimukselle ovat suun terveydentilan tutkimuksessa kerätyt kliiniset tiedot tutkittujen asukkaiden suun terveydentilasta ja tähän liittyvistä tekijöistä. Tietoja analysoidaan käyttäen Excel- ja SPSS-ohjelmistoja. Työssä havaitaan, että hoivakotiasukkaiden suunterveys on huono. Lisäksi määritetty yhdistetty tulehdusindeksi toistaa erinomaisesti samoja ominaisuuksia kuin plakki-indeksi ja gingivan tulehdustilaa kuvaava indeksi. Tulevissa tutkimuksissa voidaan selvittää yhdistetyn tulehdusindeksin yhteyttä kokonaisterveydentilaan ja tarvittaessa korjailla sen määritelmää.

▼ Introduction and objectives: New Zealand’s population of older people is increasing, and a corresponding dental transition is occurring, whereby older people are retaining their natural teeth for longer than before. New Zealand epidemiological evidence indicates that there is continuing profound
oral disease experience in the older population. There are currently no data describing the
oral health of older people admitted to hospital in New Zealand and how their treatment needs are being met. When older people are admitted to hospital due to sudden or further worsening of their general
health, they enter a major change in lifestyle, from independence to supported living. The importance of
oral health is frequently forgotten in the midst of these major changes. Little is known about whether there is a change in the
oral health of hospitalised older people as their life characteristics become greatly affected, and as they transition from independence (community-dwelling) to dependent living (institutionalised). The objectives of this study were to:
(1) Describe the clinical
oral health status and treatment needs of older people admitted to older persons’ wards for assessment at Dunedin Public Hospital due to sudden worsening of their general
health; and
(2) Describe the
oral-
health-related quality of life (OHRQoL) of those older people, together with its associations.
Materials and methods: An
oral survey was performed of 200 patients (81 males, 119 females; mean age of 82.6 ± 6.6 years) admitted to older person’s wards at Dunedin Public Hospital. The study included one thorough bedside clinical
oral examination, a face-to-face interview, and a panoramic radiographic examination (where appropriate). Participants’ medical notes were reviewed, and details of social-demographic data, existing medical conditions, including the reason for admission, and current medications were obtained.
In the study, the
oral assessment comprised examination of the participants’ extra-
oral structures,
oral mucosal, periodontal status, dentition status and prosthetic status, along with their respective treatment needs. The interview elucidated information about the participants’ use of
oral health services, and their perceptions on
oral health. The
Oral Health Impact Profile-20 (OHIP-20) was utilised to assess OHRQoL, and the Xerostomia Inventory (XI) was included to measure the symptoms of dry mouth.
Results: One in three (36.0%) participants were living independently at home prior to their hospital admission. However, due to worsening of their general
health, the majority (85.9%) of the participants expected to be dependent on others for assistance when discharged from the hospital. The majority (55.0%) of the participants had been admitted for a medical reason which required assessment. The mean total number of pre-existing medical conditions for the participants was 7.5, and each participant was taking an average of 10.5 medications. ▼ Objectives: The increasing medical expenses of elderly persons in Japan’s rapidly ageing society have become a major concern. It is therefore important to elucidate the factors associated with such escalation. Here, we focused on the relationship between subjective self-assessment of oral health, as an index of general health, and medical expenses (excluding dental repair) under the hypothesis that oral health contributes to general medical expenses. Several studies have shown that oral health status is correlated with general health status among elderly persons. We speculated that oral health status might show a relation with medical costs among elderly persons. However, few studies have investigated this relationship to date. Materials and Methods: Participants were 259 elderly subjects (range: 65–84 years; 120 men, 139 women) residing independently. Subjective assessment of oral health was evaluated by their responses (‘Good’, ‘Not good’ and ‘Not at all good’) on a survey questionnaire. The correlation between subjective assessment of oral health and medical expenditure was analysed using Spearman’s rank method, the Mann–Whitney U-test and the Kruskal–Wallis test. Medical expenses were used as the dependent variable in multinomial logistic regression analysis with background and intraoral factors as independent variables. Results: A slight yet statistically significant correlation was observed between subjective assessment of oral health and outpatient treatment fees. Conclusion: The findings revealed that subjective assessment of oral health is significantly and independently related to the medical expenses of community-dwelling elderly persons after adjusting for social background, living environment and physical factors.

oral hygiene mainly results from bad
oral habits, including not brushing teeth before going to bed, failing to keep using dental floss, never going to dentists, and so on. Based on the findings, the author designed a system of
oral hygiene products, an
oral health database and platform for home use. As for the market, the initial targeting market is Chinese market. Then it can be promoted for universal use.
These
oral hygiene products include a mini camera, smart sensors, a water flosser, a toothbrush, a cell phone holder and a UV sanitizer. The
oral health database and the platform have five essential functions including providing basic
oral health knowledge; providing methods of self-diagnosis; providing
oral health point system; sharing good dental resources; providing communication platform. All functions mentioned above are integrated in a mobile phone application. When people get high points from the point system and get a good evaluation from dentists about their
oral health status, they will get lower insurance price from the medical insurance company, where the staff can get access to these data.

▼ Population ageing combined with the consequences of
increased tooth retention and multimorbidity will present a strain
on the current NHS dental care system. Therefore, this work aimed
to develop a greater understanding of older adults, their
dentition, risk factors for
oral disease and review
oral health
interventions that may be appropriate. To understand the older
adult population, data-mapping was utilised by combining shelf
population data, secondary data from surveys and data from
individuals accessing general dental care services. A mixed methods
approach was utilised to identify
oral health outcome measures and
appropriate interventions; undertaking a broad scoping review,
quantitative data collection and focus groups. Formal evaluation of
an
oral health workshop was carried out in a real-world community
setting using
oral health literacy as an outcome measure.
Population and individual-level data observed inequalities in
oral
disease rates and treatment provision. Risk factors for
oral
diseases were identified as largely modifiable and preventable. An
educational workshop as an intervention was deemed appropriate and
acceptable to provide older adults with information on relevant
risk factors and advise them of good self-
oral care practices to
enable
oral health improvements. The
oral health workshop showed
some improvements in those with initially poor
oral health
literacy. Improving the
oral health of older adults is a complex
and challenging area. Such
oral health initiatives could be linked
to wider
health improvements but need further testing in different
communities.

▼ Objectives: This study aimed to assess the oral health status of the Toronto adult homeless population; to learn how they perceive their own oral health; and how they interact with the dental care system.
Methods: This cross-sectional descriptive study collected data from 191 homeless adults who were randomly selected using a stratified cluster sample at 18 shelters. A questionnaire and clinical oral examination were conducted with participants.
Results: The mean Decayed/Missing/Filled Teeth (DMFT) score of the subjects was 14.4 (SD=8.1). Only 32% of them had visited a dentist during the last year; 75% believed that they had untreated dental conditions; and 40% had their last dental visit for emergency care. The clinical oral examination observed that 88% needed fillings, 70% periodontal, 60% prosthodontic and 40% emergency treatment.
Conclusion: Homeless adults in Toronto have poor oral health, significant oral health treatment needs and a lack of access to dental care.

The intent of this study was to explore the use of
oral health services by a group of adults with an intellectual disability.
Methods
The study was an explanatory mixed-methods design. It consisted of an initial collection and analysis of quantitative data, and a second qualitative phase. In the quantitative phase of the study, a clinical audit of
oral health services provided for adults with intellectual disabilities was carried out for individuals who underwent a general anaesthetic for dental treatment at Christchurch Hospital during the period of 1 January 2005 to 31 December 2009.
The second, qualitative phase was then conducted to investigate the experience and perceptions of individuals with an intellectual disability (and caregivers and guardians) relating to their
oral health care. Fifteen semi-structured interviews were carried out with participants for 13 individuals with an intellectual disability who had a GA for dental treatment at Christchurch Hospital between 1 January 2009 and 31 December 2009.
Ethical approval for this study was obtained from the Upper South A Regional Ethics committee in July 2009. Quantitative data were entered onto a database and analysed using SPSS version 17.0 (SPSS Inc, Chicago, USA). Qualitative data were analysed by manual coding and labelling of the data in order to identify patterns and themes that were expressed by the participants.
Results
The majority of individuals with intellectual disability who had dental treatment under GA at Christchurch Hospital in the 5-year period lived in care and many had severe medical issues. Most of the individuals had both restorative treatment and tooth extractions carried out under GA.
The major concern for individuals with an intellectual disability was anxiety related to dental visits. Support people/guardians shared this concern, and played a vital role in helping to manage this anxiety. Many support people/guardians had difficulty identifying dental problems in those with an intellectual disability, and often relied on detection of changes in the individual’s behaviour or demeanour. While the majority of feedback on encounters with dental and medical staff was positive, problems with continuity of care were a concern. The time spent waiting in waiting rooms for check-ups or treatment was a frequently reported cause of stress to the respondents. In addition, some problems were identified with the way in which waiting areas were set up for this patient group.
Conclusions

▼ This dissertation examines emergency department (ED) utilization for non-traumatic dental conditions in Massachusetts using the Massachusetts All Payer Claims Dataset (MA APCD). The purpose of the first study was to provide a patient profile of those who made ED visits for non-traumatic dental conditions, and examine whether there is an association between access to dental care outside the ED and ED visits for non-traumatic dental conditions. The purpose of the second study was to provide a state-wide estimate of the burden of ED revisits for non-traumatic dental conditions in Massachusetts, provide a patient profile of repeat users and examine the relationship between access to routine dental care and ED revisits. Both these studies used a multilevel logistic regression model to estimate the odds of ED dental visits and revisits. The purpose of the third study was to examine the impact of changes in dental benefits on ED utilization for non-traumatic dental conditions for adult Medicaid enrollees in Massachusetts. An interrupted time-series study design, and segmented linear regression modelling approach was taken to study the impact of the policy change.; The findings from the first study indicate that working age adult males, those between 26-35 years of age, uninsured or enrolled in Medicaid, visiting on a weekend, with dental insurance coverage, without preventive dental service utilization in the year preceding the ED visit and residing in a geographically designated DHPSA had an increased likelihood of being ED users for non-traumatic dental conditions. The second study found that nearly a quarter of the ED visits for non-traumatic dental conditions were repeat visits. Adult men, those between 26-35 years of age, enrolled in Medicaid were more likely to be repeat ED users. Additionally, both these studies found that access to dental care outside the ED was a significant determinant of whether a patient would be an ED user for non-traumatic dental conditions. The third study found that there was a statistically significant increase in ED utilization for non-traumatic dental conditions after comprehensive dental benefits were eliminated in 2010 for adult Medicaid enrollees in Massachusetts.; Collectively, the three studies contribute to the existing literature on ED use for dental conditions. The findings provide valuable insights in improving our understanding of certain determinants of ED use for non-traumatic dental conditions. These findings provide evidence supporting the hypothesis that access to routine dental care outside the ED can decrease reliance on EDs for dental care. This work contributes significantly towards the ongoing debate of how changes in Medicaid adult dental benefits affect ED use for non-traumatic dental conditions. Non-urgent ED dental visits are preventable, and there is a need for continued effort in improving access to appropriate and timely dental care among some population groups. ljudi širom sveta pati od nekog oblika mentalnog poremećaja, a
istraţivanja sprovedena u drugim zemljama pokazala su da je
shizofrenija meĎu najzastupljenijima. Oralno zdravlje zauzima
značajno mesto u celokupnom zdravlju čoveka i ne treba ga
razdvajati od mentalnog zdravlja. Istraţivanja sprovedena u drugim
zemljama pokazala su povećanu zastupljenost oralnih bolesti kod
pacijenata sa shizofrenijom. Cilj ove studije bio je da proceni
stanje oralnog zdravlja hospitalizovanih osoba sa shizofrenijom,
konkretno da odredi prevalenciju karijesa, oboljenja potpornog
aparata zuba i mekih tkiva usne duplje i ispita moguće faktore
rizika koji doprinose postojećim problemima sa oralnim zdravljem
hospitalizovanih osoba sa shizofrenijom. Pacijenti i metod. U
istraţivanju je učestvovalo 190 hospitalizovanih osoba sa
shizofrenijom, lečenih na Klinici za psihijatrijske bolesti „Dr
Laza Lazarević“ Beograd (studijska grupa) i 190 zdravih ispitanika,
pacijenata Klinike za parodontologiju i oralnu medicinu
Stomatološkog fakulteta Univerziteta u Beogradu (kontrolna grupa).
Svi podaci su dobijeni pomoću sistematskog stomatološkog pregleda
ispitanika, kojim su odreĎeni indeksi za procenu oralnog zdravlja
(KEP indeks, CPITN indeks i OHI-S indeks). Pored toga, anamnezom i
kliničkim pregledom registrovano je prisustvo simptoma i znakova
oboljenja mekih tkiva usne duplje. Posebno dizajniranim upitnikom
(po tipu standardizovanog intervjua) dobijeni su podaci o
korišćenju usluga stomatološke zdravstvene zaštite, upraţnjavanju
navika u ishrani i loših navika prema oralnom zdravlju, kao i o
navikama u odrţavanju oralne higijene. Socio-ekonomska i
demografska obeleţja ispitanika, kao i podaci o psihijatrijskoj
bolesti (u studijskoj grupi) dobijeni su iz medicinske

▼ In today’s world, eating disorders are plaguing adolescent women at alarming rates. Anorexia nervosa and bulimia nervosa are the two most common disorders and differ in their symptoms and prognoses. While the symptoms of these diseases are not readily visible in systemic health, deteriorating oral health can be a positive indication of abnormal eating habits. Anorexia nervosa and bulimia nervosa result in declining systemic health due to resulting nutritional imbalances and physical harm that patients exert upon themselves. Malnutrition results in altered endocrine function and consequent loss of menstruation, abnormal hair growth, and decreased peak bone mass. Although risk factors for anorexia nervosa and bulimia nervosa vary, the resulting nutritional impoverishment produces the similar systemic effects.
In addition to systemic health degradation, symptoms of anorexia and bulimia commonly manifest in the oral cavity. Chemically, salivary composition may be altered resulting in lower pH values. Patients may also exhibit temporary bilateral parotid gland swelling as a result of repeated purge episodes. The most critical oral effect of repeated purge episodes is loss of permanent enamel and is defined as perimolysis. Presently, research does not agree on whether or not eating disorders elevate the risk of dental carries.
Although current literature unanimously agrees on the importance of early oral health detection and diagnosis of eating disorders, most dentist are not trained to properly identify and diagnose the manifestations of AN and BN within the oral cavity. Dental school and dental hygiene curriculums lack emphasis on the severity of eating disorders in general and do not allot adequate teaching hours for this matter. Sadly, even when dentists suspect the presence of eating disorders, they are not likely to intervene.
Fortunately, full recovery is possible in eating disorder patients if proper therapy and medical attention is provided in a timely manner. Damaged dentition may also be fully restored with the use of crowns, composite fillings, and porcelain veneers. However, if disordered eating habits persist after dental restoration, the dentition will once again erode and deteriorate accordingly.
Anorexia nervosa and bulimia nervosa are serious diseases that need proper attention and medical intervention. Their severity should not be minimized as they may ultimately result in grave side effects and eventual death. Dental health practitioners have the ability of observe the presence of these diseases before others as obvious symptoms may present in the oral cavity. Dentists and dental hygienists need to be educated on the oral manifestations of eating disorders and proper protocol regarding timely intervention. With proper education and knowledge, dental health practitioners can reduce the severity of disease, resulting in better prognoses for patients.

▼ The elderly population is growing at unprecedented rates worldwide. The increased mortality and decline in fertility have greatly contributed to this and caused the ratio of elderly to the remaining populations in countries around the world to dramatically increase. Furthermore, these numbers show no signs of slowing and are expected to increase at quicker rates.
With this, follows the need for better health care systems in order to maintain a good quality of life for this demographic. Of specific interest to this thesis are prevalent oral health conditions in the elderly. Conditions such as xerostomia, dental caries, and periodontal disease plague the elderly population. The research shows how much more prevalent and severe these oral health conditions are in the elderly population and aims to examine these along with effective means of treatment and care.
Future measures should be taken to effectively stabilize oral health care for the elderly. Better pubic health care systems need to be put in place that address the access to care issue that exists within the elderly population, along with better promotion of preventative measures.

▼ Objectives: The increasing medical expenses of elderly persons in Japan’s rapidly ageing society have become a major concern. It is therefore important to elucidate the factors associated with such escalation. Here, we focused on the relationship between subjective self-assessment of oral health, as an index of general health, and medical expenses (excluding dental repair) under the hypothesis that oral health contributes to general medical expenses. Several studies have shown that oral health status is correlated with general health status among elderly persons. We speculated that oral health status might show a relation with medical costs among elderly persons. However, few studies have investigated this relationship to date. Materials and Methods: Participants were 259 elderly subjects (range: 65–84 years; 120 men, 139 women) residing independently. Subjective assessment of oral health was evaluated by their responses (‘Good’, ‘Not good’ and ‘Not at all good’) on a survey questionnaire. The correlation between subjective assessment of oral health and medical expenditure was analysed using Spearman’s rank method, the Mann–Whitney U-test and the Kruskal–Wallis test. Medical expenses were used as the dependent variable in multinomial logistic regression analysis with background and intraoral factors as independent variables. Results: A slight yet statistically significant correlation was observed between subjective assessment of oral health and outpatient treatment fees. Conclusion: The findings revealed that subjective assessment of oral health is significantly and independently related to the medical expenses of community-dwelling elderly persons after adjusting for social background, living environment and physical factors.

Oral hygiene interventions for people with disabilities: A Scoping Review, a Cochrane Review and a Realist Review

Background: Worldwide, the
oral health of people with disabilities is poorer than that of the general population. The effectiveness of
oral health interventions specifically designed for people with disabilities has not been systematically examined. The aim of this thesis was to identify what types of disabilities have been targeted, what types of interventions have been undertaken, what outcomes have been assessed and what are the contexts and mechanisms that facilitate or create barriers in relation to the success, or otherwise, of these interventions.
Methods: The outcome from these reviews was realised through sequential application of multiple methods of data synthesis. Firstly, Scoping Review methodology was applied to systematically search for evidence in relation to what types of
oral health interventions have been evaluated for people with disabilities. Secondly, a more focused Cochrane Review methodology explored the effectiveness of
oral hygiene interventions for people with intellectual disabilities (ID). Thirdly, parallel to the Cochrane Review, Realist Review methodology was used to refine the theories in relation to how carer-led
oral hygiene interventions are supposed to work.
Results: The Scoping Review revealed a focus on therapeutic interventions for populations with disabilities (46%). Skill-based interventions (28%) were the next largest group of interventions, most of which were focused on
oral hygiene skills or methods to teach these skills (90%), and most for a population with ID (48%).
The Cochrane Review showed: (a) some benefits when a carer of a person with ID used a special manual toothbrush; (b) inconsistent findings between outcomes when an electric toothbrush was used by or for people with ID; (c) carers?
oral health knowledge consistently improved following training. However, this training did not always impact on the carers? attitude or behaviour and resulted in only small reductions in the levels of gingival inflammation of the people with ID for whom they cared; (d) the benefit to the people with ID was shown to be greater when some level of monitoring of the carer?s behaviour was undertaken and (e) people with ID can acquire the skills to undertake some or all of the steps involved in toothbrushing.
The Realist Review identified that the needs of the carers of people with ID, the type of
oral care training provided and level of support provided to them, may all play an important role in the success of any carer-led
oral hygiene intervention. The role of stakeholders in the planning and design of interventions and resourcing to ensure sustainability, were also identified as key elements supporting a successful outcome.

▼ Objectives: To compare the magnitude of oral health inequality over time in Canada and United States (US), and at a single point in time between Canada, US and United Kingdom (UK). Methodology: Data was obtained from five national surveys; two Canadian, two American and one from the UK. The slope index and relative index of inequality were used to estimate absolute and relative inequality, respectively. Results: In both Canada and the US, absolute inequality in oral disease declined while relative inequality increased; however, inequality for filled teeth declined significantly. Inequality in untreated decay was highest in the US, and in Canada for edentulism. Inequality was lowest in the UK for both untreated decay and filled teeth. Conclusion: Despite what appears to be greater utilization of restorative care among the poor over time, inequality in oral disease prevailed in all three countries and was consistently higher in the US, followed by Canada and lowest in the UK.

▼ Anti-resorptive drugs, including bisphosphonates, reduce bone turnover and have been associated with exposed necrotic jaw bone called jaw osteonecrosis (ONJ). This thesis focuses on bisphosphonate-associated ONJ. The pathophysiology is poorly understood, potentially involving bone turnover oversuppression and oral bacteria.
The role of oral health as a risk factor for ONJ remains unclear, although high levels of periodontal disease have been reported in ONJ patients. In addition to clinical measurements, surrogate measures of periodontal disease have also not been explored fully in relation to ONJ, including inflammatory mediators in gingival crevicular fluid (GCF). Of particular interest are Interleukins (IL-1β and IL-6) and Tumour necrosis factor (TNF)-α, as nitrogenous bisphosphonates (NBPs) have been demonstrated to stimulate the expression of these mediators. If this pro-inflammatory effect translates to the periodontium, both periodontal disease progression and ONJ risk may potentially increase. Bone turnover marker (BTM) testing is also utilised in dentistry in relation to ONJ, where serum Carboxyterminal cross-linked telopeptide of Type I collagen (β-CTX-I) has been used to assess ONJ risk prior to invasive dental procedures. This assumes that serum BTMs reflect bone turnover in the periodontium. However, this is a poorly understood relationship. Of the two research questions, the first aimed to compare levels of BTMs in GCF and serum in a pilot cross-sectional study. Healthy volunteers (n=20) received oral examinations, and GCF and serum sampling. Serum was tested for β-CTX-I and aminoterminal pro-peptides of Type I collagen (P1NP) using automated non-isotopic immunoassays. GCF was tested for P1NP using radioimmunoassay (RIA). A significant correlation was found between GCF and serum P1NP, with r=0.629 (p=0.002). However, this was influenced by an outlier which, when removed from the analysis, gave a much lower correlation co-efficient (r=0.059) which was no longer significant (p=0.812).
Extrapolation of these findings to the broader population is limited. The second research question aimed to investigate oral health risk factors for ONJ in participants with a history of bisphosphonate exposure for malignancy. This cross-sectional study compared cases with ONJ (n=22) to controls (n=41). Oral examinations were conducted, and serum and GCF sampled. Serum was tested for IgG titres against red complex bacteria and Aggregatibacter actinomycetemcomitans, biochemical parameters, BTMs, and with multiplex analysis. Then GCF underwent multiplex analysis and P1NP RIA testing. There was no significant association with ONJ for tooth loss and caries experience. ▼ Population ageing combined with the consequences of increased tooth retention and multimorbidity will present a strain on the current NHS dental care system. Therefore, this work aimed to develop a greater understanding of older adults, their dentition, risk factors for oral disease and review oral health interventions that may be appropriate. To understand the older adult population, data-mapping was utilised by combining shelf population data, secondary data from surveys and data from individuals accessing general dental care services. A mixed methods approach was utilised to identify oral health outcome measures and appropriate interventions; undertaking a broad scoping review, quantitative data collection and focus groups. Formal evaluation of an oral health workshop was carried out in a real-world community setting using oral health literacy as an outcome measure. Population and individual-level data observed inequalities in oral disease rates and treatment provision. Risk factors for oral diseases were identified as largely modifiable and preventable. An educational workshop as an intervention was deemed appropriate and acceptable to provide older adults with information on relevant risk factors and advise them of good self-oral care practices to enable oral health improvements. The oral health workshop showed some improvements in those with initially poor oral health literacy. Improving the oral health of older adults is a complex and challenging area. Such oral health initiatives could be linked to wider health improvements but need further testing in different communities.

▼ Background: There are a number of factors that can negatively influence dietary intake and nutritional status amongst the older adult population, both nationally and globally. These can have clear consequences, not only for the individual, but also on a wider scale. It is well recognised that oral health can affect nutritional status and dietary intake, however the exact nature of this relationship and whether or not oral health would have a similar effect on dietary change, is unclear. A number of methods exist to improve nutritional status and modify energy intake. These include methods that can be targeted to specific population groups, such as prosthodontic rehabilitation, which can improve masticatory performance and overall oral health. There is also the need for methods which can be targeted to and implemented by the general population, such as modification of eating rate to slow down eating speed. However, whether these methods can influence a number of nutritional parameters including energy and nutrient intake, nutritional status and appetite measures, is not known. Aims: This thesis set out to examine the factors affecting dietary intake, dietary change, nutritional status and appetite in older adults, and in particular how oral health status can influence nutritional status, dietary change and appetite measures. This main aim was broken down into five aims; to examine factors affecting diet in older people; to examine the effect of oral health on dietary change; to examine methods of improving and evaluating masticatory performance; to examine the relationship between masticatory performance and nutritional status; and to examine the effect of manipulating oral processing behaviours on appetite and energy intake. Methods: This PhD involved: 1. Exploration of factors influencing dietary intake and adherence to dietary patterns amongst older adults in Northern Ireland participating in wave one of the NICOLA study; 2. Investigation, amongst adults at a high risk of cardiovascular disease in Northern Ireland, of how self-reported oral health status can influence dietary change and consumption of Mediterranean Diet-associated nutrients as part of a 12-month dietary intervention study (the TEAM-MED Extend study) to promote adherence to a Mediterranean Diet; 3. Comparison of two tooth replacement strategies for partially dentate older patients, Conventional removable partial dental prosthesis treatment and functionally orientated shortened dental arch treatment, in terms of improving masticatory performance; 4. Examination of correlations between objective, obtained using a two-colour chewing gum test, and subjective, using a self-report questionnaire, methods of evaluating masticatory performance; 5. ▼ Cancer is a worldwide public health burden, with an overall annual incidence of 18.1 million new cancer cases and 9.6 million deaths each year reported in 2018. Approximately one-third of these cases were attributable to cancers of the gastrointestinal (GI) tract. Cancer incidence and mortality rates have rapidly increased in the 21st century, due to several factors including the global adoption of the western lifestyle and longer life expectancies. Previous research has highlighted that poor oral health and/or periodontal disease may be implicated in the aetiology or progression of some GI cancers. However, the associations reported have been inconsistent and the underlying biological mechanisms remain unclear. A better understanding of the association between poor oral health and GI cancers may help health policymakers to develop services which improve cancer surveillance, prevention and cancer outcomes. This thesis describes a series of studies addressing current knowledge gaps and collectively aims to better identify and understand the association between poor oral health, related bacteria and GI cancer risk and progression. Following an introduction to GI cancer epidemiology and oral health in Chapter 1, four novel epidemiological analyses were undertaken as described deeply in Chapters 2-5, culminating in the discussion Chapter 6 which provides an overview of the thesis findings, it’s implications for public health recommendations and future research directions.
