Problems with
oral health can, in many ways, affect the quality of life. Bad
oral health can prevent positive feelings from being expressed by students, influencing their social experiences and how they feel about themselves. Adults’ periodontal wellbeing influences their smiling habits and their quality of life linked to smiles. Bad periodontal health can also prevent positive feelings from being expressed by adults, which may, in turn, influence their self-concept and social interactions.
Objectives
The aim and objectives of this study were to determine the students’ knowledge, attitudes and practices regarding
oral health
care among undergraduate students in the Life Sciences Department at the Central University of Technology, Free State.
Methods
This descriptive study design used quantitative and qualitative methods conducted among undergraduate students in the Faculty of Health and Environmental Sciences, Department of Life Sciences at the Central University of Technology (CUT). Data were collected using an online survey questionnaire.
Data were summarised and aggregated using numerical formats, and the responses related to the KAP of students were numerically coded, which helped to interpret the results. The minimum proportional response rate that was required was (n=156), where purposive sampling was used calculated with 95% confidence level.
Results A total of (n=220) participants completed the questionnaire. Overall, 28.4% (n=62) were males and 70.2% (n=153) were females with 1.4% (n=3) classifying themselves as other (p<0.001). The questionnaire assessed
oral health
care knowledge, dental attitudes and
oral health
care practices (i.e. frequency of tooth brushing, use of dental floss and dental visits).
Of the participants, it was noted that significantly more males smoked compared to the other two groupings. Out of (n=220) participants, 26.6% (n=58) smoked. Habits were also structured around the participant’s sugar intake. In this study, over 80%; (n=187) of the participants agreed that they loved sugar. More than 63% (n=138) participants confirmed brushing their teeth twice a day, and 31% (n=68) brushed once a day. The findings in this research further suggested that participants
did not use
oral hygiene strategies such as dental floss. Results suggested that more 56.4% (n=123) did not floss. The participants were asked ‘what is plaque?’ 41% (n=91) stated it was a layer of bacteria in the teeth while other participants 14%; (n=8) stated it was dirt on the teeth. A few of the participants, 37.9% (n=77), stated that they did not know what plaque was.
Nearly 32% (n=72) of the participants did not understand how the state of one’s general health could affect their
oral health. They indicated that they did not know the relationship between the two. In comparison, only 44.5% (n=98) had an idea.

It was found that

▼ Parenteral antibiotic prophylaxis is the current standard of therapy in cleancontaminated oral cancer surgery. Nevertheless, the incidence of surgical site
infection (SSI) in oral oncological surgery is relatively high, especially in major
surgery with reconstruction and tracheotomy. The aims of this study were to investigate the perioperative condition related to microorganisms in the oral cavity and to examine the efficacy of the topical administration of tetracycline in reducing
the number of bacteria in the oropharyngeal fluid during intubation. The number of
oral bacteria was measured during intubation in patients undergoing major oral cancer surgery. The efficacy of the topical administration of tetracycline or povidone iodine gel in reducing the bacteria was then investigated. Bacteria in the oropharyngeal fluid grew from 106 CFU/ml to 108 CFU/ml during the 3 h after intubation (CFU, colony-forming units). When tetracycline was applied to the dorsum of the tongue, oral bacteria decreased immediately to 105 CFU/ml, and the number of bacteria in the oropharyngeal fluid was maintained below 107 CFU/ml for 7 h. The concentration of tetracycline in the oropharyngeal fluid was extremely high for several hours after topical administration. The topical administration of tetracycline could reduce oral bacteria in patients undergoing clean-contaminated oral cancer surgery. This method is expected to be effective in the prevention of SSI.

▼ The evolution of mobile devices and their general availability creates an opportunity for their use in mobile health scenarios (mHealth).
One of the possible use cases includes the use of mobile devices in capturing pictures to document clinical conditions. In
oral care, this can be used to enable remote assessment of the
oral cavity, for example, for
oral health screening in the population.
In this work we propose a digital platform to enable studies in
oral care, which includes the capture of
oral cavity photographs with mobile devices like smartphones, near the
subject of
care, and the posterior review by experts, using a web application.
The proposed platform, named rORAL, consists in an Android application and two web applications. The Android application is used for image acquisition. One of the web applications is for studies management and the other is to manage worklists and to review the clinical cases.
The system has been implemented in a functional prototype and was validated in the use case of
oral care education with community practice

▼ The purpose of this study is twofold: to determine the nature of
oral flora in the
neonatal population and to compare and assess the effect of
oral care, using sterile water,
normal saline or colostrum/human milk and to note whether there is a difference in
oral
colonization between the three interventions over time:
Subjects: The sample included 29 infants between 26 and 34 weeks gestation that are
admitted to the level III Neonatal intensive
care unit and require either intubation with mechanical ventilation or nasal continous positive airway pressure (NCPAP) for support
of respiratory disease whose mother's plan to breastfeed.
Study Design: A prospective, randomized experimental design.
Methods: Once consent was obtained the infants were randomized to one of the three
treatment groups by drawing an envelope. An
oral buccal swab was obtained prior to
initiation of
oral care in the first 24 hours of life. Cultures are repeated at 7 and 14 days of
life.
Results: 29 infants were enrolled between the ages of 27 and 32 6/7 weeks gestation.
Eleven were randomized to colostrum/human milk, 10 to sterile water and 8 to saline.
Two infants were dropped due to mother's inability to produce breastmilk. Of the 27
infants remaining 15 were female and 12 males; 12 Caucasian, 5 African American, 5
Hispanic, 1 Asian and 4 other. Birthweight ranged from 590 to 2530 grams (mean
1294.827). Three of the second cultures and 4 of the third cultures were not reported due
to either lost specimens or being cancelled by laboratory personnel. Of the remaining
cultures all but 2 of the initial cultures were ne.gative; dfthe second cultures 4 of24
cultures were negative and for the third set of cultures all were positive for bacterial
growth. The most common organism identified was coagulase negative staph (CONS)
but other organisms were identified. While there were no harmful effects of the use of
colostrum there was no change in the colonization by week 2 and 3.
Conclusion: There remain potential benefits for the use of colostrum in the
care of
the premature infant. Further research is needed to determine the best protocol for
administration in this vulnerable population.

The aim of Improving Oral Care Practices and Aspiration Risk Assessment to Mitigate Risk of Hospital Acquired Pneumonia is to decrease rates of hospital acquired pneumonia (HAP) on a 16 bed Transitional Care Unit (TCU) with a mixed skill staff. The theme of the project is risk reduction with improved patient outcomes and cost reduction as reasons for implementation. Fourteen cases of HAP were reported in the last 5 quarters which accounts for 48% of all in-patient acquired infections. Through policy development, standardization of oral care technique, and improved risk factor assessment, the project goal is to decrease rates of HAP by 10% within 6 months of implementation with a cost benefit of $53,695. Education of staff begins with online didactic materials followed by hands-on practice and return demonstration. Evaluation of learning will be assessed through pre- and posttests for nursing assistants and post survey for nurses. Both parties will be required to do return demonstration. Maintenance will be monitored through auditing by a confederate observer and evaluation of quarterly HAP rates. Staff education goals include improved confidence with aspiration risk assessment, enhanced communication and improved oral care technique.

▼ Background: Healthcare-associated infections (HAIs)
are a significant cause of morbidity and mortality in critically
ill children. Previous research demonstrates that commensal gut
anaerobes provide host resistance against colonization and
infection with pathogens, but data is lacking from other potential
reservoirs such as the mouth. Methods: A longitudinal
cross-sectional study was conducted in PICU patients (0-18 years)
receiving antibiotics who were recruited within 72 hours of
admission at a quaternary pediatric center 2017-2018. Healthy
children without antibiotic exposures in the prior 30 days were
enrolled from clinics as age-matched outpatient controls.
Metagenomic shotgun sequencing of
oral swabs was performed for
comparisons of community composition and Shannon diversity between
groups using Wilcoxon rank sum and principal coordinate
analyses.Results: Comparison of
oral samples found a difference in
community composition and Shannon diversity of the
oral microbiomes
with increased abundance of fecal anaerobes and potential pathogens
in PICU patients (n=57) compared to age matched controls (n=55).
There was no difference in
oral microbiome composition or diversity
when comparing underlying comorbidities, intubation status, or
antibiotic intensity between PICU subgroups. Conclusions: The

microbiome is impacted by critical illness and hospitalization in
the context of antibiotic administration. PICU patients compared to
healthy controls demonstrated loss of normal
oral flora replaced
with increased fecal anaerobes and common HAI organisms, suggesting
the mouth is an important reservoir of pathogens. Future studies
using metagenomic screening may better identify children at-risk
and modify clinical interventions to allow for prevention of HAI in
the PICU.

▼ Hospital-acquired infections, such as non-ventilated hospital-acquired pneumonia (NVHAP), are significant patient safety concerns. The lack of oral care in the non-ventilated acute care patient population is a significant contributor to NVHAP. The nursing staff is on the front line in providing oral care to hospitalized patients but often lacks knowledge and understanding regarding the relationship between oral care and NVHAP. Searching MEDLINE, PubMed, CINAHL, and EBSCO databases for current, peer-reviewed, scholarly articles for English language articles between 2010-2020 led to supporting findings. These findings revealed missed oral care as the most common modifiable risk factor contributing to increased hospital-acquired pneumonia incidences. Other findings revealed the impact an evidence-based educational program would have on oral care and outcomes. The influence of the learner's engagement with education and training has proven to have substantial benefits.

▼ There are currently no technological options in the market for the oral care of edentulous population. Dental Robotics, a company already involved in the production of dental care devices for elderlies, is interested in new technological solutions to attend this sector. The design is challenging as it has to meet several requirements: 1) be built upon an existing platform (the Air 1 handle), 2) to demonstrate potential for efficiently clean different mouth morphologies, 3) to use an actuator that requires minimal maneuver and is easily accepted by patients and be practical for nurses, and all these while showing manufacturing feasibility and production scalability. The methodology followed a combination of design thinking and lean startup, meaning a human centered, technology-based iterative and integrative process of prototyping and test. Through this progressive development, diverse strategies were applied, including literature research, field work observations and empathizing with the problem, comparison of market-available alternatives, exploration of materials and prototyping processes, evaluation of effectiveness and acceptance of different design versions, image analyses to evaluate cleaning capacity, manufacturing process, etc. The result is a validated prototype that meets the technical requirements and represents the optimal solution within the explored options; however, future progress could result from optimizing the motion of the Actuator, improve design features to make the assembly easier, explore different arrangements of tufts textures to increase comfort, increase the sample of mouth morphologies to better represent the edentulous elderly population in The Netherlands, to test the effect of the gum cleaner with Laser Doppler Flowmetry to find out if it could stimulate blood flow (Irrigation) on the gingiva, and for how long, and evaluate the long-term impact of this device in the reduction of Residual Ridge Resorption clinical tests. If proven, this could be an added value offered by the company. The proposal for oral care in edentulous elder patients developed in this work proved promising as market product after further performance and manufacturing process refinements.

▼ Background: One of the main causes of ventilator-associated pneumonia (VAP) is thought to be aspiration of oropharyngeal fluid containing pathogenic microorganisms. The aim of this study was to examine the effects of various oral care methods on the reduction of oral bacteria during intubation.
Methods: First, the effect of mechanical oral cleaning was investigated. The bacterial count on the tongue and in the oropharyngeal fluid was measured after tooth brushing, irrigation, and three hours after irrigation in mechanically ventilated patients at the intensive care unit (ICU). Next, the efficacy of topical administration of tetracycline and povidone iodine on the inhibition of bacterial growth on the tongue and in the oropharyngeal fluid was examined in oral cancer patients during neck dissection. Results: The number of bacteria in the oropharyngeal fluid was approximately 105–106 cfu/mL before surgery, but increased to 108 cfu/mL after intubation. Oral care with tooth brushing and mucosal cleaning did not reduce oral bacteria, while irrigation of the oral cavity and oropharynx significantly decreased it to a level of 105 cfu/mL (p < 0.001). However, oral bacteria increased again to almost 108 cfu/mL within three hours of irrigation. Oral bacteria did not decrease by topical povidone iodine application. In contrast, 30 min after topical administration of tetracycline, the number of oral bacteria decreased to 105 cfu/mL, and remained under 106 cfu/mL throughout the entire experimental period of 150 min.
Conclusions: While the present studies are only preliminary, these results indicate that irrigation of the oral cavity and oropharynx followed by topical antibiotic administration may reduce oral bacteria in mechanically ventilated patients.

oral manifestations of HIV as the first sign of the disease. While Lesotho has a population of fewer than 2 million people, it is faced with the harsh reality of a dire shortage of health
care workers including
oral health
care workers (OHCWs). The high prevalence of HIV is of major concern to
oral health workers who regularly encounter patients presenting with
oral lesions associated with HIV disease.
Oral health
care workers need to have adequate knowledge of these conditions for effective management thereof. To determine the knowledge, attitudes and practises of
oral health
care workers (OHCWs) of Lesotho regarding the management of
oral manifestations of HIV/AIDS. A descriptive cross-sectional survey was conducted on all 46 OHCWs in 26 public and private
care facilities in all ten districts of Lesotho. A self–administered questionnaire was used to gather information on demographic characteristics of OHCWs and their knowledge, attitudes and practices regarding the management of
oral lesions associated with HIV. A visual aid depicting seven colour images of lesions strongly associated with HIV was used to assess knowledge. Data was analysed using Microsoft Excel® and the R statistical package. The outcome of the study was then presented using tables and charts as appropriate. Frequencies, means and 2x2 contingency tables were generated and Fisher‟s exact tests were used for associations. A significance level of (p<0.05) was used.

care of patients with neurological problems is to prevent or minimize episodes of increased intracranial pressure (ICP). Elevations in ICP in response to nursing interventions have been acknowledged since the 1960’s when ICP monitoring was first introduced in the clinical setting. Until recently few studies have specifically examined the effect of
oral care on ICP and
oral care and other hygiene measures were combined or not specified, prohibiting a direct interpretation of the influence of
oral care alone on ICP. The purpose of this study was to describe the relationship between routine
oral care interventions and the changes in ICP specifically focusing on the effect of intensity and duration of this intervention. Twenty-three patients with a clinical condition requiring ICP monitoring were enrolled over a 12 month period.
Oral care provided by neuroscience intensive
care nurses was observed and videotaped. Characteristics of the intervention were documented including products used, patient positioning, and duration of the intervention. A 1-5 subjective scale was used to score intensity of
oral care. Wrist actigraphy data were collected from the nurses to provide an objective measure of intensity. Patient physiologic data were collected at 12 second epochs 5 minutes before, during and 5 minutes after
oral care. The mixed effect repeated measures ANOVA model indicated that there was a statistically significant increase in ICP in response to
oral care (p=0.0031). There was, however, no clinically significant effect on ICP. This study provides evidence that
oral care is safe to perform in patients in the absence of pre-existing elevated ICP.

oral care implemented by ICU protocols varies between facilities and sometimes even between units within a facility. It is common that these protocols prescribe a set method and frequency of
oral care in a one-size fits all approach. Evidence suggests that a nurse-driven
oral care protocol based on the data derived from using a bedside
oral health exam can reduce VAP, increase nursing role satisfaction, and save money in supplies and improved patient outcomes. Objective: The purpose of this project was to introduce a bedside
oral health exam and obtain feedback on staff’s perception of implementing an
oral care protocol based on the assessment. Education enhancing the current-standing
oral care protocol was also included to enrich nurses’ effectiveness in the
oral care they currently provide. Design: Descriptive study completed to determine ICU nurses’ perceptions of an
oral health assessment-driven
oral care protocol for mechanically ventilated patients compared to their current protocol and to emphasize the importance of mechanical
oral debridement and maintaining proper mouth moisture. Rounding individualized education was presented as a narrated PowerPoint presentation. Setting: Maricopa Medical Center trauma, surgical, and medical ICU units in October 2019. Participants: 31 full-time ICU nurses at MMC in Phoenix. Measurements: Nurses were administered a knowledge-based three question pre-test to validate need for education and given a six question post-education survey in a 5-point Likert-scale style questionnaire. Results: The average total score for the pretest was 24.73% overall and only 6.45% correctly answering knowledge questions regarding
oral care frequency, and proper intervention for foul smelling
oral secretions. The survey responses regarding perception of benefit of this program had a mean score of 4.62 with ‘1’ being strongly disagree and ‘5’ being strongly agree. There were no disagree responses and only nine total neutral responses out of 186. Conclusion: The data validated that there was opportunity to enhance knowledge in the proper implementation of
oral care. The survey data showed strong favorability among nurses and indicated a readiness to adopt an assessment-driven
oral care protocol and felt it would be beneficial to patients by reducing VAP rates and would increase nursing compliance and role satisfaction.

oral care within long-term
care (LTC) suggests positive resident
care outcomes occur when standardized practices are employed. Studies of staff education programs, trained
oral hygiene aides, and dental hygienists working within LTC, have linked regular
oral hygiene practices to decreased pneumonia rates. While these studies make the connection between good
oral hygiene and reducing risk for pneumonia, best practices focused on implementing and sustaining an
oral hygiene program in LTC facilities are lacking. The aim of this quality improvement project was to ensure that dependent residents in LTC receive assistance with daily
oral hygiene by implementing an education program, verifying staff competency in provision of
oral care, and developing a standard of practice (SOP) for
oral hygiene with input form direct
care staff. Specifically, a staff education program developed by a Veterans Health Administration (VHA) dental committee was used to provide education to nursing staff. Competency validation was then done while the nursing staff performed
oral hygiene
care as part of their resident-
care assignment. Finally, nursing assistants were invited to develop of a standard of practice (SOP) document for an
oral hygiene program specific to their facility. Future plans for dissemination include a presentation of the nursing assistant developed
oral hygiene program SOP and identified best practices during the National Nursing Assistant Culture Change Collaborative, as well as, the Office of Nursing Services Gerontological Nursing Field Advisory Committee and the Associate Chief Nurse LTC Steering Committee monthly calls for consideration as a national
oral hygiene SOP for all 135 VHA LTC facilities.

oral care in this population is often overlooked and there are no definitive guidelines as to what kind of
oral care should be provided to these individuals. Given that, our study aims were to explore and better understand the
oral health-related concerns, perceived
oral care needs of palliative
care patients and the berries that face in accessing dental
care services. We conducted a qualitative description study by recruiting a purposive sample of 11 participants with advanced stage health conditions from the outpatient palliative
care services of the University of Iowa Hospitals and Clinics. Data were collected using in-depth, semi-structured interviews using an interview guide. All interviews were fully transcribed, with the data managed with Atlas.ti software to facilitate the analysis. All participants had
oral concerns although they varied widely from person to person and their illness statuses. The most significant concerns were related to dry mouth, loose dentures, and eating difficulties. Among those who stated that they would seek dental
care if required, they were not planning on seeking dental
care even though they had treatable
oral problems, suggesting a mismatch between their
oral health status and their perceived needs. The frequently observed barriers that prevented them from seeking dental
care were their illness and illness related priorities, finances, feeling discriminated against by the dentist, and a dislike towards the dentist. Providing information to the palliative
care clinicians about the significance of
oral health and its impact, incorporating a dental
care provider in palliative
care team, and providing timely information to individuals with advanced health conditions may enhance the understanding of their
oral health problems and help them better manage their
oral health and may enhance their dental
care seeking behavior.

▼ The aim of this study was to describe non-surgical treatment options for the diseases mucositis and peri-implantitis found in present literature. The literature review also illustrates the effect of the treatment opptions on soft tissue with mucositis and peri-implantitis surrounding dental implants. The study reviews twelve scientific articles that have been processed from the aim of the study. The result shows six therapy options, Er:YAG laser, air abrasive, ultrasonic device, hand instruments, hand instruments in combination with antibiotics and hand instruments in combination with clorhexidin. The following methods are described according to how they are used and how they affect the soft tissue surrounding dental implants. The therapy options showed some improvements with reduced pocket depth, bleeding on probing (BOP) and gain in clinical attachment level (CAL). Several studies accentuate the importance of good self-care and regular visits for supportive care which have an impact on the survival of the implants and in avoiding progress of disease. All of the therapy options have positive effects but to different extent and it is not concluded how long the positive effect remains in the soft tissue.
Syftet var att redogöra för icke-kirurgiska behandlingsmetoder vid mukosit och periimplantitsom beskrivs i litteraturen. Litteraturstudien belyser ävenbehandlingsalternativens påverkan på vävnaden vid sjukdomstillstånden mukosit ochperi-implantit. Studien är utförd som en allmän litteraturstudie och innefattar 12vetenskapliga artiklar som bearbetats utefter studiens syfte. Resultatet redovisar sexbehandlingsalternativ, Er:YAG laser, air abrasive, ultraljud, handinstrument,handinstrument i kombination med antibiotika och handinstrument i kombination medklorhexidin. Metoderna är beskrivna både hur de används och vilken klinisk påverkan dehar på vävnaden runt tandimplantat. De förbättringar som visats är viss reduktion avfickdjup, blödning vid sondering (BOP) och vinst av klinisk fästenivå (CAL). Samtligametoder ger positiv effekt, men i olika grad. Ett flertal studier påvisar betydelsen av godegenvård och regelbundna besök för stödbehandling vilket är av betydelse förimplantatens överlevnad och för att undvika progression av sjukdom. Däremot kan detses en begränsning i hur länge resultatet bibehålls i vävnaden.

oral e o bom funcionamento de suas estruturas, obtido por meio da higiene
oral adequada, estratégia crucial de prevenção para excluir o risco de problemas bucais e promover o conforto físico e psicológico dos pacientes. A higiene
oral inadequada traz vulnerabilidade a cavidade
oral potencializando focos de infecções propícias à pneumonia nosocomial. Objetiva identificar e analisar na literatura as melhores evidências disponíveis referentes ao produto, dispositivo e frequência do cuidado
oral para prevenção de pneumonia associada à ventilação mecânica em pacientes intubados e em ventilação mecânica internados em Unidade de Terapia Intensiva. Trata-se de uma revisão integrativa da literatura, na qual foi
realizada buscas de estudos primários nas bases de dados eletrônicas PubMed, EMBASE e CINAHL, utilizando descritores controlados e os respectivos vocabulários destas bases de dados: MeSh, EMTREE e TÍTULOS. Das 184 referências identificadas, 52 foram selecionados após leitura de título e resumo para serem lidos na íntegra, 18 constituíram a amostra final. A extração e análise dos dados foram realizadas por dois revisores independentes. As publicações dos 18 estudos ocorreram no período de 2009 a 2017. O controle do biofilme e prevenção de lesões na cavidade
oral constituem ações importantes para a obtenção da boa saúde
oral. Para esse controle, são necessárias intervenções que promovam a remoção mecânica e/ou farmacológica do mesmo. Essas intervenções constituem a combinação de produtos, dispositivos e frequência do cuidado
oral. As melhores evidências indicaram a combinação dos seguintes produtos, dispositivos e frequência: creme dental Biotene® combinado com a escovação dentária e
também Biotene® enxaguante bucal, para promover a hidratação da cavidade
oral, reduzindo a xerostomia; clorexidina 0,12%, melhor evidência como antisséptico; lubrificantes a base de água e hidrossolúveis para mucosa

Tutkimuksen tarkoituksena on selvittää Helsingin kaupungilla ikääntyneitä potilaita laitosolosuhteissa hoitavan henkilökunnan koulutusta, resursseja sekä asenteita potilaidensa suunterveydenhoitoon liittyvissä kysymyksissä. Vastaavia tutkimuksia on tehty aiemmin muualla, ja näiden konsensus on ollut, että hoitava henkilökunta saa koulutusta suuterveyteen liittyvissä asioissa vaihtelevasti ja että henkilökunnan kouluttamisella myös heidän hoidossa olevien henkilöiden suuterveys paranee.
Menetelmät
Tutkimus suoritettiin syksyllä 2013 jakamalla yhteensä 300 kyselylomaketta kolmen eri Helsingissä sijaitsevan geriatrisen hoitolaitoksen henkilökunnan edustajille. Kysely jaettiin kaikille henkilökunnan jäsenille ja palautettiin hoitoloittain anonyyminä. Kaavakkeessa kysyttiin asioita vastaajan taustoista, ja siinä oli 20 monivalintakysymystä liittyen potilaiden suunterveydenhoitoon.
Tulokset
Vastauksia palautettiin 180 kappaletta (60 %) hoitoloihin, josta ne toimitettiin yliopistolle. Tässä tutkielmassa käsiteltäväksi valittiin henkilökunnan koulutukseen, työpaikkojen resursseihin ja käytäntöihin sekä henkilökunnan asenteisiin liittyviä kysymyksiä. 66,3 % vastanneista kertoi saaneensa koulutusta asiakkaiden suun terveyden hoidossa, ja vastaava osuus, 66,1 %, toivoi aiheesta vielä lisäkoulutusta. Mielestään riittävät välineet oli käytettävissä 65,9 %:lla vastanneista. 63,3 % vastanneista piti suun terveyttä tärkeänä yleisterveyden ja hyvinvoinnin kannalta. Vain 2,9 % vastanneista potilaan suun puhdistaminen kuului päivittäisiin työtehtäviin.
Pohdinta
Hoitava henkilökunta pääasiassa mieltää suun terveyden tärkeäksi osaksi yleisterveyttä ja hyvinvointia. Koulutusta aiheesta on saatu, mutta yhtälailla sitä toivotaan saatavan lisää. Täydentävä koulutus olisi tarkoituksenmukaista järjestää kyseisissä hoitolaitoksissa, jossa resurssit ja olosuhteet ovat samat kuin hoitotyön arjessa.

▼ The overall aim of this research was to investigate
the safety of oral care in terms of intracranial dynamics and
changes in oral health during intubation and 48 hours after
extubation among neuroscience intensive care unit (NICU) patients.
Additional aims included comparison of a comprehensive oral care
protocol (i.e., tongue scraping, electric toothbrush, non-foaming
toothpaste, and application of oral moisturizers) to a standard
protocol (pediatric manual toothbrush, standard toothpaste,
lubricant) with respect to safety and efficacy to promote oral
health and to reduce the incidence of oral nosocomial bacteria and
ventilator-associated pneumonia (VAP) in NICU patients. First, a
12-month prospective cohort study with data from 45 intubated
patients in an NICU was conducted to identify changes in oral
health during intubation until 48 hours after extubation.
Specifically, changes in oral health, intracranial pressure (ICP)
recorded during non-specified oral care, the prevalence of oral
nosocomial bacteria, and the incidence of VAP were analyzed. The
results from the cohort study (Paper I) reflected significant
deterioration in oral health during intubation and improved oral
health 48 hours after extubation, as measured by the Oral
Assessment Guide (OAG). During unspecified oral care methods,
normal ICP recordings remained stable while ICP values greater than
20 mm Hg before oral care decreased after care (p<0.001).
Progressive colonization of oral nosocomial bacteria was documented
during intubation, and the overall rate of VAP was 24%. Based on
literature and the results of the cohort study (Paper I), it was
then hypothesized that a comprehensive oral care protocol would be
safely tolerated by NICU patients, oral health would improve, and a
decrease in oral nosocomial bacteria and the frequency of VAP
compared to NICU patients receiving standard oral care would be
observed. A two-year randomized controlled trial (RCT) was
therefore conducted to compare the outcomes of intubated NICU
patients undergoing a standard oral care protocol or a
comprehensive oral care protocol. Outcome variables included
changes in ICP and cerebral perfusion pressure (CPP) among 47
patients (Paper II); changes in oral health among 56 patients
(Paper III); and changes in oral and sputum nosocomial bacteria and
the frequency of VAP among 78 patients (Paper IV). During standard
and comprehensive oral care (Paper II) ICP increased significantly
(mean increase 1.7 mm Hg, p<0.001), while ICP decreased
significantly (mean decrease 2.1 mm Hg) after oral care
(p<0.001). Neither change was clinically significant. There were
no changes in CPP before, during, or after oral care. Based on the
total OAG score and all item measurements (p<0.001), oral health
deteriorated significantly in patients receiving the standard
protocol and large effect sizes were present (Paper III). The total
OAG score of subjects receiving the comprehensive oral care
protocol also deteriorated (p<0.004), but no deterioration was

oral care among frail elderly people in nursing homes is low. Due to reduced mobility or cognitive functioning, elderly people fail to complete the
oral care ritual sufficiently. Once this happens, caregivers are obliged to take over this task. Unfortunately, in many cases this does not happen due to the difficulty of the task, the uncomfortable feeling of intimacy for both the elderly person and the caregiver, and the high time pressure caregivers experience during the morning
care ritual. Dental Robotics aims to improve the quality of
care among frail elderly by developing an automatic toothbrush that takes away these pains. The Dental Robotics toothbrush simply requires the elderly person to bite in a mouthpiece. This enables the caregiver to brush all teeth at the same time while applying the right pressure and angle without skipping any teeth. The toothbrush completes this task in only 10 seconds, helping to reduce the total amount of time of the
oral care ritual. However, for people with dementia such a product, bringing along new and different interactions, is difficult to comprehend. This lack of understanding can lead to problematic behaviour. This can be apathic, anxious, aggressive or agitated behaviour, all of which is difficult to overcome by the caregiver. Currently, the most common solution that caregivers use to overcome problematic behaviour is skipping the
oral care altogether. When this happens, the
care ritual cannot be completed. This is a major challenge for Dental Robotics, leading to the following design goal: Increase the acceptance of the Dental Robotics toothbrush among people with dementia and their caregivers, by creating a more pleasant experience for both. Person-centred
care is a way to avoid problematic behaviour in the long term. This is an approach that puts the client with dementia in the centre of their own
care, and uses the preferences and habits of the client during the
care ritual. Since everyone has a different history, the way in which
care should be approached should be different for everyone as well. In addition, the symptoms and severity of all clients are different. In order to apply this person-centred
care, it is important to have a good understanding of the person in question. Blink is an app that aims to enable caregivers to apply the person-centred approach over the entire
care ritual. This is supported by the theory that, when the entire
care ritual is executed without any problematic behaviour, the client with dementia will be put at ease and be more prone to accept such a difficult product as the toothbrush. Every time Blink is used for a specific client, the content changes. It will adapt based on the connection between the client and the caregiver, the severity of the dementia and the specific tool preferences for both the caregiver and the client. Ao fim de 48 horas de internamento, a flora apresenta um maior número de microrganismos que rapidamente podem ser responsáveis por aparecimento de infeções secundárias, tais como pneumonias, resultante à proliferação bactérias que lhe está associada. Este risco é ainda superior em doentes críticos. Nesta população torna-se fundamental a implementação de um efetivo protocolo de higiene
oral, procurando controlar ao máximo o desenvolvimento do biofilme
oral.
Objetivo: Avaliar o índice de biofilme
oral dos doentes na admissão a um serviço de Cuidados Intensivos, procedendo á sua reavaliação após 7 dias de internamento e, procurando deste modo avaliar a eficácia de higienização
oral efetuada no Serviço.
Materiais e Métodos: Estudo prospetivo, institucional, descritivo, analítico e observacional realizado no Serviço de Cuidados Intensivos do CHP. Foram envolvidos no estudo doentes com mais de 18 anos, e com um tempo de internamento igual ou superior a 7 dias. Procedeu-se à colheita de dados demográficos, motivo de admissão, tempo de internamento, medicação prescrita, tipo de alimentação efetuada no serviço, necessidade ou não de suporte respiratório e qual o tipo de higiene realizada no serviço. Foi avaliado o índice de higiene
oral simplificado de Greene & Vermillion (IHO-S) nas primeiras 24h e 7 dias após a 1ª avaliação. O IHO-S é um indicador composto que avalia 2 componentes, a componente de resíduos e a componente de cálculo, sendo cada componente avaliada numa escala de 0 a 3. São avaliadas 6 faces dentárias que são divididas em 3 porções clínicas (porção gengival, terço médio e porção oclusal). No final de cada avaliação é calculado o somatório do valor encontrado para cada face, sendo este total dividido pelo nº de faces analisadas. O cálculo do IHO-S por indivíduo corresponde à soma das componentes. Resultados: Foram avaliados 74 doentes, tendo-se excluído 42 por não terem a dentição mínima exigida.
Os 32 doentes que completaram o estudo apresentaram uma idade média de 60,53 ± 14,44 anos, 53,1% eram do género masculino, e na sua maioria pertenciam a pacientes do foro médico e cirúrgico (37,5,5%). Os doentes envolvidos no estudo tiveram uma demora média de 15,69±6,69 dias de internamento, tendo-se verificado que 17 dos pacientes (53,1%) estiveram internados mais de 14 dias no Serviço de Cuidados Intensivos 1. Relativamente às características particulares da amostra verificou-se que durante o período de avaliação a maioria dos doentes estiveram sedados (75%), sob suporte ventilatório (81,3%) e a fazer suporte nutricional por via entérica por sonda nasogástrica (62,6%).

Introduction.
Oral health
care in nursing homes is insufficient. Guidelines have been made, but it seems unclear how
oral health
care should be implemented. The aim of this study is to develop an implementation strategy to implement
oral health
care in nursing homes. The research question is: what should be the focus of an implementation strategy and which elements should this strategy include?
Methods. Clients and nursing home staff of one Dutch nursing home were included. The mouth of clients was examined, for instance with the MucosalPlaqueScore. Nursing staff completed a ‘Checklist
Oral Health Care’ and a ‘Knowledge Test’, to describe the
oral health
care and the knowledge of nursing staff. Two focus group interviews were held to investigate what barriers and stimulations staff experience, giving
oral health
care.
Results. The
oral health of clients with their own teeth was unacceptable. Sixty percent of the clients seemed to be
care dependent of
oral health
care.
Oral health
care was not applied enough, however basic utensils were available. Knowledge of nursing home personnel was relatively good. Barriers are related to nursing staff (knowledge/ skills, attitude, personality, habits), but mostly to clients (behaviour, habits) and the organisation (lack of utensils, task distribution and lack of advise).
Conclusions. An implementation strategy to implement
oral health
care should focus on clients with their own teeth. Continuous stimulation of
oral health
care is needed, through reminders, feedback of colleagues, usage of a toolbox and a focal point. New ways of schooling could improve knowledge through posters, e-learning or the use of intranet. Dental professionals should advise staff about
oral health
care and observation.
Recommendations. The implementation strategies that have been found should be tested in practise and specific interventions could be developed.

oral health professionals and providers who focus on providing primary
oral care for preventing and treating
oral diseases to help the population maintain their
oral health. However, no relevant law in Taiwan protects the rights of dental hygienists. This study aimed to understand peopleâs views of the profession and willingness to establish dental hygienist legislation in Taiwan and to explore related factors.
In this study, we collected a total of 356 questionnaires from the Taiwanese population, and we found 347 of these questionnaires to be valid after excluding invalid questionnaires. SPSS20.0 statistical software was used for statistical analysis. Data were analyzed using descriptive statistics, independent sample t test, one-way analysis of variance, Chi-square test, Pearson correlation analysis, multiple linear regression, and logistic regression.
The results showed that most of the participants were females (70.9%), with a mean age of 30.31 years. Of these participants, 57.3% were undergraduate level, 40.6% worked in health
care, 36.0% were residents of the south, 70.9% were unmarried, and 30.8% had an average monthly income of NT 40,000â59,999. The statistical analysis indicated the following results: (1) there was a significant positive correlation between attitude toward dental hygienists and identification with dental hygienist legislation; (2) the positive attitude toward the establishment of dental hygienist legislation will affect peopleâs currently favor; and (3) most people have positive attitudes toward
oral hygienist legislation and have high degree of recognition for dental hygienistâs role and responsibility.
The study also found that participants currently working in dental-related industries would be more supportive of the establishment of dental hygienist laws or regulations. Additionally, most of the participants agreed that
oral hygienists should implement dental assistance,
oral health
care and
oral hygiene education. Dental hygienists can not only assist dentists as part of the dental team but can also provide better medical quality and efficiency. We hope that the government will address the issue of dental hygienist legislation and thereby assist dental hygienists in bringing their professional skills and abilities into full play.

▼ Background: Standardized and evidence-based resource allocation frameworks for timely provision of primary dental services may support equitable distribution of comprehensive dental
care. However, such frameworks, which can be applicable to primary
care settings in Brazil, are not available. The purpose of this study was to explore the complex issue of equity allocation of dental staff for primary dental
care services, by estimating time to dental disease progression in order to analyze costs when survival targets are set for patients waiting for primary dental
care. The inclusion of wait time benchmarks for dental services in the design of the framework was an attempt to increase knowledge on the quality of access experienced by people living within catchment areas of the Family Health Strategy in Brazil. In view of ever scarce resources for public health services, ethical dilemmas arise in resource allocation when allocation choices require priority setting among individuals who face similar health needs. Since equity of access must be assured for all Brazilian citizens, the present study proposed a rational resource allocation model to help decision-makers in reconciling equity access and budgets. Aim: This study aimed to compare equity of access to dental services and costs of dental staff of two models for primary
care settings. Additionally, staffing requirements and staff costs were projected over a three-year time period. Both models comprised three inter-related components: (i) universal access to
oral health
care, (ii) comprehensiveness of primary dental
care and (iii) equity of access to primary dental services. Method: The present study was part empirical and part modeling in design. In the empirical phase, a set of maximum wait times for dental
care determined by experts (Model 1) vs. wait times derived from survival analysis (Model 2) was compared. A one-year follow-up of a cohort of dental patients assigned to five primary health
care clinics was conducted. The event of interest was clinical deterioration in the waiting time for dental visits. At each consultation with a dentist either for routine or emergency reasons, the
oral quadrants of the patient were assessed and classified according to their urgency for dental
care (from 1, less urgent to 5, more urgent). In the modeling phase, costs of dental staff were estimated on the basis of survival probabilities found in Model 1 and on survival targets simulated in Model 2. The amount of staff required as calculated by combining data on: dental service needs, activity standards for dental services, workload components in dental
care, cost per working hour of dental staff, and probabilities of clinical deterioration in the wait for dental visits. ▼ Introduction. In adults, oral health has been shown to worsen during critical illness as well as influence systemic health. There is a paucity of paediatric critical care research in the area of oral health; hence the purpose of the Critically ill Children’s Oral Health (CCOH) study is to describe the status of oral health of critically ill children over time spent in the paediatric intensive care unit (PICU). The study will also examine the relationship between poor oral health and a variety of patient characteristics and PICU therapies and explore the relationship between dysfunctional oral health and PICU related Healthcare-Associated Infections (HAI).
Method. An observational study was undertaken at a single tertiary-referral PICU. Oral health was measured using the Oral Assessment Scale (OAS) and culturing oropharyngeal flora. Information was also collected surrounding the use of supportive therapies, clinical characteristics of the children and the occurrence of PICU related HAI.
Results. Forty-six participants were consecutively recruited to the CCOH study. Of the participants 63% (n=32) had oral dysfunction while 41% (n=19) demonstrated pathogenic oropharyngeal colonisation during their critical illness. The potential systemic pathogens isolated from the oropharynx and included Candida sp., Staphylococcus aureus, Haemophilus influenzae, Enterococcus sp. and Pseudomonas aeruginosa. The severity of critical illness had a significant positive relationship (p=0.046) with pathogenic and absent colonisation of the oropharynx. Sixty-three percent of PICU-related HAI involved the preceding or simultaneous colonisation of the oropharynx by the causative pathogen.
Conclusion. Given the prevalence of poor oral health during childhood critical illness and the subsequent potential systemic consequences, evidence based oral hygiene practices should be developed and validated to guide clinicians when nursing critically ill children.

▼ Reduction of oral health disparities is a worldwide goal because oral health has been shown to mimic individual and community health overall. At the end of the twentieth century, oral health made remarkable achievements; however, problems still remain in many communities around the world, especially within third world countries, such as those in South Asia. Member countries of the South-East Asia region of the World Health Organization (WHO) have stated that dental caries, tooth loss, periodontal disease, and oral cancer are some of the contending public health issues in their countries and action must be taken to reduce their numbers (WHO, 2008). The goals of this study are to determine what cultural aspects of the South Asian population aid in the increase of poor dental care in these communities. Our overall goal is to provide recommendations for a new oral health promotion program in South Asia that has been determined by understanding what cultural aspects are important in creating a successful dental care community program.
As seen by the various studies, smokeless tobacco is a cultural norm in South Asia and is affecting the quality of life for many of the people there. Tobacco use is common but smokeless tobacco is becoming more prevalent in these countries. It is clear that with such statistics there is a need for governmental public programs to educate the population on the importance of proper oral care. This study analyzed the current literature to determine (i) what were the cultural aspects of South Asia that are detrimental to oral health, (ii) what is the oral health status of the individual countries found in South Asia, and lastly (iii) what aspects are necessary in creating an effective oral health program.
The goal of this study is to determine what cultural aspects of the South Asian population contribute to poor dental care in these communities and to provide recommendations for a new oral health promotion program in South Asia that is strengthened by understanding what cultural aspects are important in
creating a successful dental care community program.
The conclusions presented here suggest an oral policy framework for the initiation of discussions aimed at designing and implementing an oral health program in their South-Asian country. Additionally, we suggest future directions once the guidelines are met. Overall, proper oral health should be enjoyed by all and this study aims to the aid the South Asian region’s governments towards moving forward in meeting those goals.

▼ Significant efforts have been undertaken in medicine to identify hospital and primary
care service areas (eg, the Dartmouth Atlas of Health
Care) using patient origin information. Similar research in dentistry is nonexistent. The goal of this dissertation was to develop and refine methods of defining dentist service areas (DSAs) using dental insurance claims. These service areas were then used as spatial units of analysis in studies that examined relationships between utilization of
oral health services, dentist workforce supply, and service area characteristics. Enrollment and claims data were obtained from the Iowa Medicaid program for children and adolescents ages 3-18 years during calendar years 2008 through 2010. The first study described rates of treatment by orthodontists in children ages 6-18 years. Orthodontic DSAs were identified by small area analysis in order to examine regional variability in utilization. The overall rate of utilization was approximately 3%; 19 DSAs were delineated. Interestingly, children living in small towns and rural areas were significantly more likely to have received orthodontic services than those living in metropolitan and micropolitan areas. The second study identified 113 DSAs using claims submitted by primary
care dentists (ie, general and pediatric dentists). Characteristics of these primary
care DSAs were then compared with counties. Localization of
care was used as a measure of how well each region approximated a dental market area. Approximately 59% of
care received by Medicaid-enrolled children took place within their assigned service area versus 52% of
care within their county of residence. Hierarchical logistic regression was used in the final study to examine the influence of spatial accessibility and the importance of place on the receipt of preventive dental visits among Medicaid-enrolled children. Children living in urban areas were more likely to have received a visit than those living in more rural areas. Spatial accessibility assessed using measures of dentist workforce supply and travel cost did not appear to be a major barrier to
care in this population. More studies are needed to explore the importance of spatial accessibility and other geographic barriers on access to
oral health services. The methods used in this dissertation to identify service areas can be applied to other populations and offer an appropriate method for examining revealed patient preferences for
oral health
care.

oral das crianÃas. Assim, procuramos apreender as compreensÃes das professoras sobre o desenvolvimento da linguagem
oral das crianÃas com as quais trabalham; conhecer o papel que as professoras atribuem a si mesmas, no desenvolvimento da linguagem
oral das crianÃas; identificar as prÃticas realizadas nos diversos momentos da rotina pela professora que interferem no desenvolvimento da oralidade das crianÃas; analisar a qualidade dessas prÃticas. Respaldamos este trabalho nos estudos de Piaget (1983; 1986; 2009), Vigotski (1998; 2007; 2008) e Wallon (1995; 2007; 2008) sobre a importÃncia do surgimento da linguagem
oral e o papel desta no desenvolvimento integral e nas interaÃÃes dos sujeitos; assim como nas discussÃes propostas por Bondioli e Mantovani (1998), Cruz (2011),
Kishimoto (2011), Malaguzzi (1999), Oliveira-Formosinho (2011), acerca da formaÃÃo e da profissionalidade docente nesta etapa da educaÃÃo. A pesquisa de campo foi de abordagem qualitativa do tipo estudo de caso, realizada em uma creche municipal de Fortaleza que atende crianÃas de um a trÃs anos de idade. Utilizamos como instrumentos investigativos: a aplicaÃÃo de questionÃrio e realizaÃÃo de entrevista com as trÃs professoras e a observaÃÃo das suas prÃticas pedagÃgicas, durante trÃs meses. Adotamos como estratÃgias de registros o diÃrio de campo, fotografias, gravaÃÃo de voz e vÃdeo com a intenÃÃo de capturar as interaÃÃes das docentes com as crianÃas e das crianÃas entre si. Nessa pesquisa, constatamos que as docentes compreendem a importÃncia do seu papel no desenvolvimento da linguagem
oral das crianÃas, porÃm, apenas uma delas garante um trabalho de qualidade com essa linguagem em suas prÃticas com as crianÃas. Identificamos que as docentes nÃo percebem que o desenvolvimento
dessa linguagem acontece em todos os momentos da rotina, mesmo os que tÃm maior Ãnfase em aÃÃes de cuidado (alimentaÃÃo e higiene). Duas das professoras nÃo favorecem o desenvolvimento da linguagem

▼ Purpose: Cystic fibrosis (CF) is a chronic genetic disorder that an individual has at birth. Cystic fibrosis impacts equally all races, gender and becomes worse over time. The aim of this current study was to evaluate
oral health status, interaction with the
oral health system, and
oral health awareness in adults with cystic fibrosis. Methods: A validated survey consisting of 27 questions was distributed online through cystic fibrosis Facebook pages. Individuals with CF who were 18 years old or older, residing in the U.S. and having English as their primary language were asked to respond to the survey. Independent variables were demographic categories, whereas the dependent variables were
oral health assessment, interaction with the
oral health system, and
oral health awareness. Results: Out of 350 sample sizes, a total of 133 responses were received; 87 met inclusion criteria. Respondents were 75% female, 50% were single, and 58.6% were employed. Seventy percent reported an annual household income more than $35,000. Good to very good conditions of the mouth and the teeth were reported by respondents with four-year college degree or higher. Married respondents were more likely to have dental insurance, visit a dental office and to have visited a dentist within 12 months. Female 33 and older scored higher on the
oral health awareness scale than younger respondents. Conclusion: Although most respondents reported good or very good
oral health, 52.7% reported dry mouth, 31% reported difficulty chewing food, and 29.1% reported seeing blood on their toothbrushes. Better
oral health habits with additional education for CF adults are needed.

African traditional medicine is widely used in South Africa. African traditional health practitioners treat
patients who present with a variety of medical conditions, including
oral health conditions. The aim of
this study was to determine the knowledge, and practices of African traditional health practitioners
regarding
oral health conditions.
Materials and Methods
A cross-sectional survey of 11 African traditional health practitioners who consented to be interviewed
in the Johannesburg area was recruited to participate in a structured questionnaire survey, followed by
focus group interviews with 10 practitioners. Ethical approval was obtained from the Biomedical
Research Ethics Committee (BREC ref. no 451/19). Following the collection of data from the
questionnaire administration, a focus group interview was conducted. Data on the knowledge, treatment
practices, and post-treatment management of four common
oral conditions was collected and analysed.
Results
Eleven participants who consented to be interviewed, their average age was 44.1 (±8.1) years, with a
range of 21-67 years, and a slight majority of female (55.5%, n=6). Ten further participants took part
in focus group interviews. African traditional health practitioners were asked a series of questions, in
non-dental terminology or language, related to their knowledge (causes); practise (what do you use to
treat?, What advise do you give to the patient?); of four common
oral health conditions. These are:
sores on the lips, sores on the tongue, swollen gums and toothache. Participants reported using a variety
of practices such as throwing bones, burning incense, using plants and animal product, as well as
commercially manufactured products to treat and manage patients.
Conclusion
The results of the study reveal that there are vast differences in knowledge, management practices and
treatment modalities of African traditional healthcare practitioners. Further research in the knowledge,
practises and treatment of
oral healthcare practitioners needs to be conducted. Mutual cooperation,
collaboration and integrating African traditional health practitioners into primary
oral healthcare
services need to be urgently prioritised.
