Notre thèse de pharmacie clinique est une contribution à l’optimisation de l’efficience du traitement antirétroviral (TAR) par des méthodes de modélisation en Côte d’Ivoire. La première étude a été consacrée à une modélisation de la réponse antirétrovirale par taxonomie des trajectoires de taux de CD4 en utilisant un modèle de méta-apprentissage des trajectoires d’indicateurs biomédicaux. Ce modèle appliqué à la taxonomie des trajectoires des taux de CD4 a montré son intérêt dans la mise en évidence de classes distinctes de patients avec des caractéristiques particulières justifiant et/ou déterminant le profil particulier de méta-trajectoires de leur marqueur immunologique au cours du traitement. La deuxième tâche a consisté en une évaluation de l’impact de principaux déterminants des méta-trajectoires de taux de CD4 sur divers types de réponse immunologique à partir d’un modèle explicatif avec une équation de régression logistique. Les réponses immunologiques considérées ont été exprimées en termes d’absence de gain de CD4, de gain sub-optimal et de gain optimal de CD4 à différentes périodes de suivi du TAR. Enfin l’évaluation de l’efficience des stratégies antirétrovirales de première ligne en Côte d’Ivoire, a été abordée dans la dernière partie avec un modèle pharmaco-économique. Nous avons effectué une étude préliminaire ouvrant des perspectives pour encourager la mise en oeuvre d’évaluations pharmaco-économiques complètes par modélisation en Côte d’Ivoire. Elle a permis de mettre en exergue les parties méthodologiques pouvant être sujettes à caution dans une étude de modélisation pharmaco-économique des TAR de première ligne dans un contexte de ressources limitées

Our thesis of Clinical pharmacy is a contribution to optimize the efficiency of antiretroviral therapy (ART) by modeling methods in Côte d'Ivoire. The first study was devoted to modeling the antiretroviral response from taxonomy of CD4 counts trajectories, using a meta-learning model of biomedical markers trajectories. This model applied to the taxonomy of the CD4 counts trajectories showed its interest in the identification of distinct classes of patients with particular characteristics justifying and/or determining the specific profile of meta-trajectories of the immunological marker during treatment. The second task was an assessment of the impact of key determinants of CD4 counts meta-trajectories on various types of immune response from an explanatory model with a logistic regression equation. Antiretroviral immune responses considered were expressed in terms of absence of CD4 gain, sub-optimal gain and optimal gain of CD4 at different periods of follow-up of ART. Finally the evaluation of the efficiency of first-line antiretroviral strategies in Côte d'Ivoire, was discussed in the last part with a projective pharmaco-economic model. Paper 1: Should Deliveries of Used Clothes to LDCs Be Supported? This is the journal version of a study of the worldwide used-clothes trade – with focus on Sweden’s participation in it – undertaken for Sida (the Swedish foreign aid agency). We looked at both theoretical and empirical effects of the commercial used-clothes trade, and at the effects of subsidizing it. We basically concluded that, while markets work, there was no need to subsidize them under normal conditions because – even given the goal of helping the poor – there might well be better uses of the money. There might be an argument for subsidized used-clothes exports in dealing with an emergency where supply had broken down, but even then most NGOs prefer to supply new clothes. Paper 2: A Model of Dynamic Balance Among the Three Spheres of Society – Markets, Governments, and Communities – Applied to Understanding the Relative Importance of Social Capital and Social Goods This paper revisits old questions of the proper subject and bounds of economics: Does economics study “provisioning”? or markets? or a method of reasoning, self-interested rational optimization? A variety of scholars and others in many fields make use of a taxonomy of society consisting of three “spheres”: markets, governments, and communities. It is argued here that this tripartite taxonomy of society is fundamental and exhaustive. A variety of ways of understanding this taxonomy are explored, especially Fiske’s (1991, 2004) Relational Models theory. Then – after communities and their products, social goods, are defined more thoroughly – a visual model of interactions among the three spheres is presented. The model is first used briefly to understand the historical development of markets. The model is then applied to understanding how economic thinking and market ideology, including the notion of social capital, can be destructive of communities and their production of social goods (and their production of social capital as well). It’s not possible to measure these effects monetarily, so calculating precisely “how this affects results” in a standard economic model is impossible. Nevertheless we could better prepare students for real-world analysis, and better serve our clients, including the public, if – whenever relevant, such as in textbook introductions and in benefit/cost analyses – we made them aware of the limitations of economic analysis with respect to communities and social goods. The three-spheres model offered here, based on Fiske’s Relational Models theory, facilitates this awareness. En este documento se concentra en un análisis sobre la asociatividad y la conformación de redes empresariales en dos instituciones prestadoras de servicios de salud del municipio de Tenjo Cundinamarca. En la primera parte se desarrolla el estado de la asociatividad con un enfoque de red, se plantea los objetivos, la justificación y los alcances de la investigación. En el segunda parte se hace alusión al marco teórico, marco legal, sistema general de seguridad social en salud, y la importancia que tiene las asociaciones empresariales, cooperación, participación y la confianza en las instituciones de salud; haciendo mención sobre clusters, alianzas y ecosistemas empresariales. En la tercera parte se realizó el marco metodológico de la investigación, análisis de resultado de las entrevistas, se describen las organizaciones objeto de este estudio. En la cuarta parte se realizan la discusión de la investigación, y se presentan las conclusiones que los autores de este estudio han presentado sobre la confianza fortalecida y la obligación prolongada de las empresas en el tiempo. Finamente, esta investigación presenta un punto de partida para la conformación de modelos que integren varias empresas, que puedan mejorar la competitividad, en las instituciones prestadoras de servicios de salud, permitiendo el acercando de los procesos del área administrativa con el área asistencia, en procura de brindar un mejoramiento continuo en la atención de los pacientes y su grupo familiar.

In this document is focused an analysis on the associativity and incorporation of business networks in two institutions that provide Health services in the Municipality of Tenjo, Cundinamarca. In the first part is developed the status of the associativity with a focus of network, it is stated the objectives the justification and the scope of the investigation. In the second part it is mentioned the Theoretical Framework, general system of Health Social Security and the importance that have the business networks, cooperation participation and the reliability on the Health entities; mentioning business clusters, alliances and ecosystems. In the third part it is performed the Methodological Framework of the research, interview’s results analysis, the organization which are object of this research are described. In the fourth part is performed the discussion of the research and the conclusions that the authors of this research have reached on the strengthened trust and the long term obligation of the companies in time. Finally, this research is a start point for the formation of models that integrate several companies, that may improve competitivity in the entities that provide Health Care Services, allowing an approach to the process of the administrative area to the assistance area, intended to provide a continuous improvement in the care of patients and their family group.
