Purpose: The menopause transition has been associated with subclinical vascular disease. High-resolution ultrasound is a reliable, non-invasive method to detect early structural and functional atherosclerotic changes in the arterial wall. The present study aims to examine the effect of traditional cardiovascular risk factors on the indices of vascular structure and function in relation with the presence of metabolic syndrome in a population of menopausal women. Methods: We screened 473 informed-consenting, postmenopausal women, retrieved from the Menopause Clinic of the Aretaieio Hospital, University of Athens. The menopausal status was defined as absence of menses for at least 12 consecutive months and FSH serum levels > 25mIU/mL. We recorded demographic, gynecological and obstetrical history and the presence of cardiovascular risk factors. Metabolic syndrome (MS) was defined according to the Joint Definition. We ultrasonographically evaluated carotid artery intima-media thickness, flow-mediated dilation of the brachial artery, and we measured pulse wave velocity and central pressures.Results: Metabolic syndrome was present in 17.3% of our postmenopausal women. Mean values of PWV increased linearly with the accumulation of MS features. Furthermore, the indices of vascular structure and function exhibited significant differences, according to the presence of MS. Moreover, metabolic syndrome was significantly associated with mean values of central systolic and diastolic blood pressure. Multivariate analysis of our data showed that the presence of MS-features were associated with indices of vascular structure and function in relation to traditional cardiovascular risk factors.

Σκοπός: Η μετάβαση στην εμμηνόπαυση έχει συσχετιστεί με υποκλινική αγγειακή νόσο. Η υπερηχογραφική μελέτη είναι μία αξιόπιστη, μη επεμβατική μέθοδος για την ανίχνευση πρώιμων δομικών και λειτουργικών αλλοιώσεων στο αρτηριακό τοίχωμα. Η παρούσα μελέτη στοχεύει να εξετάσει την επίδραση των παραδοσιακών καρδιαγγειακών παραγόντων κινδύνου στους δείκτες της αγγειακής δομής και λειτουργίας, σε σχέση με την παρουσία μεταβολικού συνδρόμου σε έναν πληθυσμό μετεμμηνοπαυσιακών γυναικών.Υλικό και Μέθοδος: Πρόκειται για μια διατμητικού τύπου μελέτη (cross-sectional) που περιέλαβε 473 μετεμμηνοπαυσιακές γυναίκες που επισκέπτονταν σε εξωτερική βάση το Ιατρείο Εμμηνοπαύσεως, της Β’ Γυναικολογικής Κλινικής του Πανεπιστημίου Αθηνών στο Αρεταίειο Νοσοκομείο. Η εμμηνοπαυσιακή κατάσταση ορίστηκε ως η απουσία της εμμήνου ρύσης για τουλάχιστον 12 συνεχόμενους μήνες και τα επίπεδα ορού FSH> 25mIU / mL. Καταγράψαμε τα δημογραφικά, γυναικολογικά και μαιευτικά δεδομένα του πληθυσμού της μελέτης και την παρουσία παραγόντων καρδιαγγειακού κινδύνου. Purpose: Insulin resistance (IR) has been shown to promote atherosclerosis. The menopause transition has also been associated with subclinical vascular disease. The TyG index is a simple marker that correlates well with the degree of insulin resistance. The present study aims to examine the association of the TyG index, with subclinical atherosclerosis in relation to BMI in a cohort of postmenopausal women. Methods: 473 informed-consenting, non-diabetic postmenopausal women, with no documented cardiovascular disease were included in the study. Carotid artery intima-media thickness, flow-mediated dilation of the brachial artery and pulse wave velocity were correlated with the TyG index separately for lean and overweight/obese women.Results: The TyG index correlated significantly with the majority of the assessed cardiovascular risk factors in both lean and overweight/obese women. However, the TyG index was significantly associated with carotid IMT (r=0.155, p=0.012) and aortic stiffness as measured with PWV (r=0.157, p=0.013) only the group of lean women. Significantly higher levels of TyG were observed only in lean women with carotid atherosclerotic plaques (plaques: 8.32±0.51 vs No plaques:8.15±0.38). In the multivariate analysis, while the metabolic syndrome was a significant determinant of subclinical atherosclerosis only in the overweight/obese group (OR=2.517, 95% CI: 1.078-5.878, p-value=0.033), TyG index predicted significantly subclinical atherosclerosis only in the lean group (OR=3.119, 95% CI: 1.187-8.194, p-value<0.001). Conclusions: TyG, an easily assessed index, is associated with carotid atherosclerosis and arterial stiffness mainly in lean women. This may prove a useful marker for identifying high risk women in the normal- weight postmenopausal population.

Εισαγωγή: Η αντίσταση στην ινσουλίνη (IR) έχει δειχθεί ότι προάγει την αθηρωμάτωση. Η μετάβαση στην εμμηνόπαυση έχει επίσης συσχετιστεί με υποκλινική αγγειακή νόσο. Ο δείκτης TyG είναι ένας απλός δείκτης, που σχετίζεται καλά με το βαθμό της αντίστασης στην ινσουλίνη. ▼ Introduction: Most women will experience symptoms at menopause, some will be bothersome. Little is known about the experience of menopausal symptoms in Scotland since the 2002 change in clinical guidelines for managing menopausal symptoms. This study aimed to reassess the impact and management of menopausal symptoms in the community, and identify factors associated with symptom experience. Methods: In 2009 a postal questionnaire was sent to 8,206 women aged 45-54, registered with 16 Scottish general practices. To validate the shortened illness perception measure, 360 respondents to the main survey were sent one of two postal questionnaires. Results: Most commonly reported symptoms were tiredness, aches and pains and sleep difficulties. The classic menopausal symptoms of hot flushes, night sweats and vaginal dryness were reported by 46.7%, 46.4% and 28.2% of women, respectively. Common menopausal symptom management strategies included talking to friends and family, wearing cotton clothes, sleeping in a cool room, and exercise. Herbal remedies were more commonly used than prescription drugs. Nearly one fifth of women had used HRT. Menopausal status, education, social support, reporting other symptoms, BMI, smoking status, use of management strategies, possible presence of depression, and physical health were associated with reporting classic menopausal symptoms. Factors associated with “doing well” or “doing badly” with hot flushes or night sweats was symptom dependent. Bother from menstrual periods, children, BMI, education, smoking, physical health, use of psychological strategies, and perceived consequences of symptoms were associated with “doing well” or “doing badly” with hot flushes or night sweats. The shortened IPQ-R scale had good reliability, moderate validity but poor internal consistency. Conclusion: Menopausal symptoms are common and often bothersome. Identifying women who appear to do well or badly with hot flushes and night sweats could be useful for tailoring interventions. A more robust measure of illness perceptions is needed.

▼ Objective: To develop a validated questionnaire for the assessment of menopausal syndrome in Chinese Medicine and through the analysis of the basic menopausal syndrome classification in Chinese Medicine to explore the use of objective hormone markers as one of the evidence for the Chinese Medicinal classification and guidance for application of herbal medicines by Chinese Medicine Practitioners. Methods: The major symptoms related to typical syndromes of menopause were extracted from Chinese Medicine textbook, international diagnostic guide of Chinese Medicine, classic and contemporary literature of Chinese Medicine and subjective symptoms were eliminated to facilitate the administrate of a self-reported outcome questionnaire. The symptoms related to menopause in Chinese Medicine were identified and redefined to improve the content validity. A total of 353 menopausal women were included in the study. Participants were assessed through face-to-face interview by registered Hong Kong Chinese Medicine Practitioner to complete the questionnaire and serum samples for hormones measurement were collected after an overnight fast in the morning of the assessment day. Serum Estradiol (E2), Testosterone (T), Progesterone (P), Luteinizing Hormone (LH), Follicle Stimulating Hormone (FSH) and Sex-Hormone Binding Globulin (SHBG) were measured by electrochemiluminescence immunoassays (ECLIA). Free estradiol (fE2), non-Sex-Hormone Binding Globulin bound Estradiol (non-SHBG-E2), free Testosterone (fT), and non-Sex-Hormone Binding Globulin bound Testosterone (non-SHBG-T) were calculated to assess hormone availability and activity. E2/T ratio was also calculated to investigate the balance of E2 and T in the body. Results: Principal Component Analysis (PCA) was conducted. Symptoms related to Deficiency of Yin and symptoms related to Deficiency of Yang were extracted and the relationship between the syndromes and E2, T, P, LH, FSH, SHBG, fT, non-SHBG-T, fE2, non-SHBG-E2 and E2/T ratio were investigated. For symptoms related to Deficiency of Yin, E2 (p<0.000, p<0.000), P (p=0.004, p<0.000), fE2 (p<0.000, p<0.000), non-SHBG-E2 (p<0.000, p<0.000), E2/T (p<0.000, p<0.000)，SHBG (p=0.039, p=0.004) showed significant decrease while LH (p=0.013, p<0.000), FSH (p=0.041, p<0.000) showed increase by comparing hormones of mild or little with moderate and severe presentation of symptoms. For symptoms related to Deficiency of Yang, SHBG (p=0.042, p=0.002, p<0.000) showed an increase, fT (p=0.044, p=0.049, p=0.003) and non-SHBG-T (p=0.044, p=0.049, p=0.003) showed a decrease which was significant for other three groups while comparing to the no or little group. Conclusion: Menopausal Syndrome Questionnaire was developed and validated in the study. Major syndromes and symptoms involved in the diagnosis of menopausal syndrome in Chinese Medicine were identified and good test-retest reliability was also demonstrated. ▼ Menopausal symptoms are common causes for women to seek treatment. The severity of symptoms vary from patient to patient and reduces the quality of life for many women. Most women reach menopause between the ages of 45 and 55, some not until 60 and some at an early age in their thirties or forties (Stoppler, 2008). Aim This study aimed to offer an alternative form of treatment for women that are experiencing the unpleasant symptoms that are experienced with menopause, since hormone replacement therapy, may have adverse effects in some women (Wang-Cheng, 2007). This double-blind placebo controlled study investigated the efficacy of a complementary formulation of Folliculinum D6 and Five-Flower Formula™, (Folliculinum D6, Rock Rose, Impatiens, Clematis, Star of Bethlehem, Cherry Plum ) in the treatment of menopausal symptoms in terms of participants’ perception of the treatment. iii This formula is currently available commercially as an unregistered product trading under the name of Femme Rosa. This formula was developed by Dr Maharaj, and is prepared by Natura®. The formula consists of Folliculinum D6 and Five-Flower Formula™. Methodology A total of 60 female participants who were going through natural menopause and currently experiencing menopausal symptoms took part in the study. They were randomly assigned according to the randomization sheet drawn up by the supervisor, 29 participants to the treatment group and 31 to the placebo group. The study was conducted over a period of six weeks and participants were required to attend two consultations at the Homoeopathic Day Clinic at the Durban University of Technology. Participants were recruited according to the inclusion criteria set out. During the first consultation the participants completed the Greene Climacteric Scale questionnaire and a concise case history was taken, thereafter each patient was handed two hot flush diaries to be completed one week before treatment and during the sixth week of treatment. Each participant was required to take one tablet every morning and evening for 6 weeks. Six weeks after the first consultation participants attended the second consultation and the Greene Climacteric Scale was completed for the final time and the hot flush diaries collected. iv Results The results of the first questionnaire and hot flush diary were used as a baseline for statistical analyses. As each group consisted of 29 and 31 subjects, non-parametric tests were used for data analysis. All statistical analyses were carried out using SPSS version 15.0. Conclusion An improvement was demonstrated in both treatment and placebo groups after treatment yet this improvement was not statistically significant. The only symptoms that improved in the treatment group were the hot flushes and psychological symptoms however, there were no significant differences between the treatment and placebo group. The climacteric describes the ongoing changes and symptoms, of the phase or transition period that may last 15-20 years in a woman's lifecycle, when ovarian function and hormonal production declines. Menopause is the permanent cessation of the menses, identified retrospectively after one year without menses and occurs within this period of climacteric (Bernstein et al. 1996). The most common symptoms of climacteric include hot flushes, night sweats, sleep disturbances, nervousness, depressive moods, feelings of vertigo, inability to concentration, joint pain, headache and heart palpitations. The most commonly used allopathic medication to palliate these symptoms is hormone replacement therapy (HRT). There are adverse side effects and risks associated with this treatment and not all women feel better on HR.T (Stoppard, 2001). The aim of this research study was to determine the efficacy of the Homoeopathic complexes Dr Reckeweg RI0® and R20® (Homoeopathic complexes) in relieving the symptoms of the climacteric. The methodology and Ethics were accepted by Higher Degrees Committee and Academic Ethics Committee on the 25 August 2008 (Ethical clearance no: 40/08). Participants were recruited by advertisements (Appendix A) at the University of Johannesburg, in health food shops and in pharmacies. This was a double blind, placebo controlled study involving thirty-two participants who were divided into two matched groups based on the severity of the menopausal symptoms. Volunteers were selected . using the exclusion and inclusion criteria. Volunteers meeting the inclusion criteria completed the information and consent form (Appendix B), and a patient profile and case history (Appendix C) were taken. The participants were randomly allocated to an experimental or control group, and given sets of medication (Remedy A and B, 50ml bottles) to take for a period of eight weeks (Appendix D). The participants took 10 drops of Remedy A (R10® or placebo) in the morning and Remedy B (R20® or placebo) at night. Menopause refers to the natural cessation of the menstrual cycle at the end of the reproductive years, and is usually defined retrospectively after twelve consecutive months of amenorrhoea. Climacteric is the term used to describe the life period surrounding menopause. It begins with physiological changes that mark the transition between the reproductive and post-reproductive years (Lewis and Bernstein, 1996). The mean age of menopause is 51 years, with a normal range from 45 to 56 years (Chamberlain and Malvern, 1996). There are many symptoms associated with menopause, including hot flushes and night sweats, headaches, sleep difficulties, vaginal dryness and loss of libido, urinary problems, joint and muscle pains, depression, irritability, and poor memory and concentration. Long-term risks and effects include osteoporosis and coronary heart disease (MacGregor, 2000). The aim of this study was to determine the efficacy of the homoeopathic similimum in the treatment of climacteric symptoms. The research study was advertised in local newspapers, pharmacies, and on the Doornfontein campus of the University of Johannesburg in order to recruit volunteers for the study. All volunteers were required to complete a Suitability Criteria Questionnaire. Following this, ten participants were selected who met the inclusion criteria. The study involved four homoeopathic consultations for each participant over a period of three months. During the first consultation a consent form was completed, the procedures of the research were explained to the participant, a full case history was taken, and a physical examination was performed. According to each participant’s individualized physical, mental and emotional symptoms ascertained from the case history, the researcher determined a suitable similimum remedy. A similimum remedy can be defined as the single remedy that fits the symptom picture (Digby, 1997). The abbreviated Kupperman Menopause Index was completed by each participant at the beginning of the treatment, and then at four-weekly intervals for the duration of the twelve week trial. The results were then transferred to the participants’ main Kupperman Menopause Index and compared, thus determining the effect of the homoeopathic similimum in treating climacteric symptoms.

▼ Menopause is a natural and universal milestone achieved by every living female by the time of midlife. Worldwide, the number of postmenopausal women is expected to rise to 1.1 billion by 2025. Therefore, medical education and healthcare trainings are consistently going through updates and curricular transformations to incorporate teachings of age and gender related disorders, their diagnostic protocols and effective therapeutic options. The dermatology residents’ current curriculum is predominantly taught on the basis of skin type and color and rarely on the gender-, hormonal- or age- related context. Knowledge about how to evaluate and manage disorders pertaining to skin, hair and nails in the midst of aging and hormonal fluctuations of menopause is not being strategically taught or included in the current resident dermatology curriculum. In order to diversify the curricula, this research effort was focused on identifying and prioritizing the most significant dermatological disorders influenced by female hormonal fluctuations during and beyond natural and induced menopause, using the Delphi Method. A convenience sample of thirty physicians from five different departments (dermatology, gynecology, rheumatology, internal medicine, and family medicine) of a large medical institute, in southwest of United States was recruited to participate in the study. In round 1, there was 100% participation of five experts, invited to form Panel A of the Delphi study. The experts in Panel A, gave profound feedback to refine and reorganize the list of skin disorders created by the researcher through the intense evidence- based medical literature review. There was an 89% of participation rate among the twenty-eight faculty members invited to become Panel B experts in round 2 of this study. During round 3, 100% of the experts who previously participated as Panel B provided feedback. The calculated median for the ranking of each skin disorder by Panel B was three or above in round 2 and 3, with significant convergence and minimum variability. The most highly ranked category of skin disorders affected by hormone changes at menopause was identified to be gynecological disorders. Among the subcategories, the overall most highly ranked skin disorder was identified to be hirsutism followed by vulvovaginal atrophy. In conclusion, the need for curriculum enrichment has been identified during and at the completion of this Delphi study by the ranking and feedback given by the majority of the Panel A and Panel B experts of the study. This entails bringing in more knowledge and resources into a preexisting curriculum to teach the medical students and residents about the influence of hormone changes at menopause on skin disorders prioritized via this research study. The results of this Delphi study can be utilized to extend the core curriculum by restoring the excitement of teaching the same known skin disorders, in the context of hormonal changes at menopause. ▼ Estrogen has been associated with cognitive function, and cognitive difficulties become more prevalent in older age when estrogen is reduced. However, the long‐term cognitive function of older women who had reductions in estrogen through having their ovaries removed is not well understood. This dissertation examined the extent to which bilateral oophorectomy and its timing relative to natural menopause are associated with cognitive functioning through two complementary studies. Depressive symptoms as a mediator and hormone therapy as a protective factor were also examined. In Study I, twins over 65 years of age in the Swedish Twin Registry with linkages to the national Inpatient Discharge Registry were used in classic, case‐control (154 women with bilateral oophorectomy and 7,600 women with ovaries intact) and discordant co‐twin (n = 63 pairs) analyses to examine associations between oophorectomy and both cognitive impairment and scores on a cognitive screening test. In Study II, oophorectomy status and performance on global cognitive function and five cognitive domains among healthy postmenopausal women in 3 clinical trials conducted in Los Angeles County were analyzed in linear regression models both cross‐sectionally (n = 1,149) and on repeat cognitive assessment 2.5 years after baseline (n = 463). In both studies, covariates included age, education, and health factors. In Study I, neither oophorectomy nor its timing was associated with cognitive impairment. In Study II, oophorectomy after age 45 compared to no history of oophorectomy was a risk factor for poorer performance in verbal learning (p = .03) in the cross‐sectional design and with a decrease in visual memory (p < .01) performance over time. Oophorectomy status was associated with a decrease in visual memory (p < .05) over time. Neither study showed a significant interaction of oophorectomy and hormone treatment. However, in Study I, hormone treatment was protective against cognitive impairment (p < .02 for length of time on hormone treatment, p < .01 for history of any hormone treatment). In both studies, depressive symptoms were not found to be a mediator but were found to be associated with poorer cognitive function. The dissertation findings suggest that ovarian removal may be associated with poorer performance in specific cognitive domains but not with longer‐term cognitive impairment. ▼ Menopausal symptoms are a concern for peri- and postmenopausal women, and for women who have been treated for breast cancer. Changes in memory are a commonly reported menopausal symptom which can interfere with daily activities, occurring in women who experience natural menopause as well as in women who have an abrupt onset of menopause following treatment for breast cancer. Menopause hormone therapy may be contraindicated or unacceptable for many women, reinforcing the need for well-tolerated, non-hormonal treatments for menopausal symptoms. The purpose of this study was to examine the efficacy of acupuncture to improve the menopausal symptom of memory changes in breast cancer survivors, and was a secondary analysis of a placebo-controlled, randomized clinical trial. The Framework for Interactions between the Individual and the Environment was the conceptual framework that guided the study. Subjects in the parent study were randomized to either the experimental group (symptom-specific acupuncture), control group (non-symptom-specific acupuncture), or enhanced usual care group (instruction on health-related topics). Each group received twelve intervention sessions over an 8-week period. Sixty subjects indicating a positive response to memory items on instruments at baseline were selected for this secondary analysis. The Daily Symptom Diary, Kupperman Index, and Menopause-Specific Quality of Life Questionnaire (MENQOL) measured frequency or severity of perceived memory changes at baseline, midpoint, and end of treatment. Frequency distributions were used to summarize the data. There were no statistically significant results from the Daily Symptom Diary. Improvement in the enhanced usual care group compared to the acupuncture groups was seen on the Kupperman Index, and validity concerns for this instrument were reviewed. Analysis of the MENQOL data revealed more positive change in the acupuncture groups in improvement of memory than in the enhanced usual care group. These findings support the need for further exploration of targeted interventions such as acupuncture to improve memory difficulties for menopausal women and breast cancer survivors, particularly as advances are made in the understanding of the mechanisms of cognitive change. ▼ The menopause is a natural transition that occurs at midlife for a woman and is accompanied by a number of symptoms which can significantly affect quality of life, the most commonly reported being hot flushes. Impaired cognitive function has also been observed in menopausal women and natural alternatives to hormone therapy for the alleviation of menopausal symptoms are being sought. The aim of this thesis was to investigate the effects of soya isoflavones on cognitive function and menopausal symptoms in post-menopausal women. In an observational study, food diaries were completed by post-menopausal women (n=75). Principal component analysis was used to determine dietary patterns and associations with cognitive function, as assessed by CANTAB, were investigated. 5 final dietary patterns were identified. The 'meat and dairy' dietary pattern was associated with greater performance in the spatial working memory cognitive test. 'The social' dietary pattern was associated with negative effects on cognitive performance as was the 'potatoes and poultry' dietary pattern. A randomised controlled trial in post-menopausal women (n=101) was conducted to investigate the effects of soya isoflavones on cognitive function and menopausal symptoms. Participants consumed a volume of soya drink providing a low (10mg; control group), medium (35 mg) or high (60 mg) dose of soya isoflavones daily for a period of 12 weeks and cognitive function was assessed using CANTAB (spatial working memory, spatial span, pattern recognition memory, 5-choice reaction time and match to sample visual search). Menopausal symptoms were also assessed using a validated questionnaire. Isoflavone supplementation had no effect on the primary outcome measure, cognitive function, although consumption of 350ml of soya drink/day (providing 35mg isoflavones) significantly reduced hot flushes in post-menopausal women with more severe symptoms at baseline. An observational study was conducted using focus groups to determine overall perceptions towards soya in post-menopausal women and to partly evaluate the soya intervention. Post-menopausal women reported barriers and enablers of consumption as well as attitudes towards soya products in an Irish and Northern Irish population. The main findings from the focus groups include the awareness of soya in the diet and also the benefits that can be attained from consuming soya. Main barriers to soya consumption include price and lack of knowledge about soya products while enablers of consumption include Alpro® being considered a reliable and good brand. This thesis questions the potential benefits of soya isoflavones on cognitive function in healthy post-menopausal women within 7 years post-menopause. Future studies should investigate effects in earlier post-menopausal women and/or in those with mild cognitive impairment. Soya isoflavone consumption in post-menopausal women had no effect on cognitive function although may alleviate vasomotor symptoms in those with more severe symptoms at baseline. ▼ Background: Interest in alternatives to hormone therapy (HT) for menopausal symptoms increased following its association with serious health risks. In terms of phytotherapeutic interventions, while traditional use supports a range of herbs for treating menopausal symptoms, evidence from rigorous scientific trials is limited, and has largely focused on the phytoestrogenic plants. Because of some safety concerns over long-term use of isoflavones, the present study focused on two non-estrogenic herbs, Hypericum perforatum and Vitex agnus-castus, also employed in this context in the Anglo-American and European traditions. Both herbs have shown effectiveness for the alleviation of symptoms of premenstrual syndrome (PMS), which is reported to be more severe during the perimenopause, and may account for many of the so-called 'menopausal symptoms' at this time. Research on menopausal vasomotor symptoms is prone to substantial placebo responses. There has been much interest in increasing our understanding of the placebo response with a view to controlling it in clinical research and harnessing it in clinical practice. Methods & Results: A double-blind, randomised controlled trial (RCT), with a 16-week treatment phase, was conducted on 100 late-perimenopause and early postmenopause women. The herbal combination (Hypericum and Vitex) was not found to be superior to placebo for any of the endpoints - daily weighted flushing scores, overall menopausal symptoms (on the Greene Climacteric Scale) and depression (on the Hamilton Depression Inventory). However, significant improvements across the treatment phase were observed in both arms for all of these outcome measures. No significant change was found for either group on the Utian quality of life scale. The effects of the herbal combination were also examined on PMS-like symptoms in the small sub-population of late-perimenopausal women, and found to be superior to placebo for total PMS-like symptoms and the sub-clusters, PMS-D (depression) and PMS-C (cravings). The active treatment group also showed significant improvements on PMS-A (anxiety) and PMS-H (hydration), although these effects were not superior to placebo. Predictors of the placebo response were investigated and found to include study-entry anxiety for the outcome measures of flushing, depression and overall menopausal symptoms, and improvement during non-treatment run-in for depression and overall symptoms. Because no difference had been found between 'active' and placebo groups in the menopause RCT, it was hypothesised that the same predictors would predict the response to the study treatment. However, low anxiety was significantly associated with improvement in this group. None of the other variables that predicted the placebo response was relevant to the study treatment response. ▼ Background: In the United States, the aging population is rapidly growing. By 2060, it is expected that 12 million women will be ages 85 years and older. However, determinants of longevity and healthy aging in women are not fully understood. This dissertation had three objectives: 1) Determine whether ages at menarche and menopause and reproductive lifespan were associated with survival to age 90, termed “exceptional longevity;” 2) Determine whether genetic factors associated with longevity in prior studies among populations of European descent were associated with survival to ages 85 and 90 and healthy aging in white, African-American, and Hispanic women; and 3) Determine whether accelerometer-measured and self-reported sedentary time were associated with leukocyte telomere length (LTL), a purported biomarker of aging, among older women. Methods: Three studies were conducted among participants from the Women’s Health Initiative, a longitudinal study investigating major determinants of chronic diseases in postmenopausal women. Study one was a prospective study among 16,251 women who had potential to survive to age 90 as of August 29, 2014. Study two was a prospective study among 11,154 women who could survive to age 85 as of August 29, 2014 and used genetic data from multiple genome-wide association studies. Study three was a cross-sectional study among 1,481 women with information on either accelerometer-measured or self-reported sedentary time. All studies consisted of racially diverse samples.Results: In study one, the odds of exceptional longevity were elevated among women with later menarche, later menopause, and longer reproductive lifespan. In study two, three variants at APOE were associated with survival to age 90 and healthy aging in white women, and seven variants at a novel locus were associated with survival to age 85 in Hispanic women. In study three, among women at or below the median level of moderate-to-vigorous intensity physical activity (MVPA), higher accelerometer-measured sedentary time was associated with shorter LTL. Among women with higher MVPA levels, sedentary time was not associated with LTL. Conclusions: Findings suggest that reproductive and genetic factors may be associated with late-age survival and that a high level of inactivity may be associated with short LTL.

▼ Background: Cognitive problems such as forgetfulness, concentration difficulty, and inattention are commonly reported among women in or around the menopause transition. However, objective neurocognitive data do not consistently corroborate subjective cognitive complaints, nor do they effectively distinguish women in different reproductive stages. Further assessments must be conducted to determine the specific cognitive changes occurring around the menopause transition in comparison to other reproductive stages (e.g., late postmenopause). Furthermore, non-pharmacological methods for addressing reported cognitive difficulties should be explored. Aims: The primary study aim (Aim 1) was to evaluate cross-sectional differences in subjective and objective cognitive function among women in perimenopause (irregular menstruation to 1 year after the final menstrual period), early postmenopause (1-5 years after the final menstrual period) and late menopause (> 5 years after the final menstrual period). The secondary aim (Aim 2) was to compare changes in cognitive function and physical activity among women who completed a six-month physical activity intervention. Hypotheses: It was hypothesized that women in perimenopause would score lower on tests of attention and working memory than women in postmenopause. It was also hypothesized that increases in physical activity would be significantly associated with improvements on attention and working memory tasks. Procedure: Forty-five women age 40 to 63 (mean age = 53.6 years) were recruited for a six-month, group-based intervention designed to help midlife women increase sustained aerobic activity. Neurocognitive and physical activity assessments occurred at baseline and 6 months. Participants were given weekly exercise prescriptions, gradually working toward 250 minutes of MVPA per week. Measures: The following neurocognitive domains were evaluated using clinician-administered tests: Global cognition (MoCA) and intelligence (WTAR), attention and working memory (Letter N-Back computer test; Digit Span), verbal learning and memory (Selective Reminding Test), verbal fluency (COWA), and executive function (Trails A and B). Physical activity was quantified by seven-day ActiGraph GT3X accelerometer readings. Statistical Analyses: Regression and ANOVA tests were performed (using the GLM procedure in SPSS), in addition to Pearson’s r correlations. Results: At baseline, women in early postmenopause showed significantly worse verbal memory performance than women in perimenopause or late postmenopause (p < .05), controlling for age, mood, and sleep quality. At six months, women in early postmenopause showed significantly greater improvement in verbal memory than women in perimenopause (p = .048). Reproductive stage also significantly predicted changes in verbal fluency after controlling for covariates. Self-reported improvement in concentration was also significantly associated with improved verbal fluency scores at 6 months (p = .004).
