▼ Spirometry is used to diagnose respiratory disease, to monitor disease progression and response to treatment, and in epidemiological surveys. As a large burden of disease is caused by cigarette smoking, spirometry has been incorporated in smoking cessation counselling in an attempt to improve quit rates. The concept of lung age (LA) was developed in 1985 in an effort to make spirometry results more easily understood by the lay person. Research results using LA to aid quitting remain inconclusive. This thesis investigates the need to update LA equations, as predictive equations based on old data may not be relevant for today’s populations, and contemporary equations may result in a stronger message for smokers. New LA equations were firstly developed using contemporary Australian data and four further LA equations were derived from previously published FEV₁ predictive equations. A series of comparisons of LA equations in contemporary Australian datasets followed. The first project compared the original Morris LA equations with newly developed Australian LA equations in an independent workplace dataset (males only). The second project compared four extra LA equations derived from previously published FEV₁ equations from Europe, the United Kingdom, America and Australia with the Morris and the new Australian equations. An independent dataset of randomly-selected males and females was used to compare these equations with the Morris LA equations and contemporary Australian LA equations. Lastly, a different type of LA equation expressed as delta lung age (ΔLA), the difference between chronological age and lung age, based on the ratio of Forced Expiratory Volume in one second/Forced Vital Capacity (FEV₁/FVC), was compared with three other LA equations based on FEV₁ alone. This project used three independent datasets (urban, rural and a workplace) for added strength. All LA equations confirmed poorer lung function in smokers than in never smokers in all 3 independent datasets. LA estimates were approximately 20 years lower using the original Morris equations when compared with the newest LA equations. The differences seen between estimated LA using all six equations were consistent in each analysis. The ΔLA equation gave extreme LA estimates in both the community-based datasets compared with the LA equations based on FEV₁ alone. These results show that the Morris LA equations need to be updated. However, there appears to be no advantage in using the ΔLA equation. The differences between the older and the newer LA equations are most likely a result of cohort and period effects. This is also the case in the predictive equations themselves. Continuously updating predictive equations using recently acquired data will result in LA equations that are more relevant to contemporary populations. Asthma is a chronic health problem affecting persons of all ages and has become a major health issue in India. The present study has been conducted to assess anthropometric profile and pulmonary functions of bronchial asthma patients and is based on cross sectional data collected on 403 (216 males, 187 females) bronchial asthma patients and 347 (158 males, 189 females) normal healthy subjects, ranging in age from 20-70+ years from Hoshiarpur district of Punjab. Anthropometric profile of asthma patients was studied by recording 13 anthropometric measurements i.e. weight, linear measurements (stature, sitting height, subischial length), chest width, circumferences (upperarm, chest in males only, waist and hip) and skinfolds (biceps, triceps, subscapular, suprailiac). Body mass index and waist to hip ratio have been derived from these measurements. Body composition i.e. body fat and lean body mass have been calculated by applying equations given by Durnin and Womersley (1974). Pulmonary functions have been assessed using portable electronic Helios- 401 spirometer (Recorders and Medicare Systems, Chandigarh). Eight pulmonary function parameters viz. forced vital capacity, forced expiratory volume in one second, ratio of forced expiratory volume in one second to forced vital capacity, forced expiratory flow 25-75%, peak expiratory flow rate, forced expiratory volume in three seconds, ratio of forced expiratory volume in three seconds to forced vital capacity and lung age have been recorded on each subject. Asthma patients have been found to be significantly lighter in weight, with lesser values of circumferences and skinfolds. The body fat and lean body mass have also been found to be lesser in patients. All the pulmonary function variables have significantly lower values in patients. In patients, pulmonary functions have been found to improve with increase in BMI even in the category of obese. Increase in waist circumference has also shown positive effect on pulmonary functions in patients.

Med en åldrande befolkning ökar vikten av att finna faktorer som kan bidra till bättre hälsa och funktion hos äldre personer. Personer med god lungfunktion lever längre och hälsosammare. Syftet med denna studie var att undersöka lungfunktion i hög ålder och dess samband med fysisk aktivitet från medelåldern upp till hög ålder. För att öka förståelsen för sambandet mellan fysisk aktivitet och lungfunktion i hög ålder undersöks även sambandet mellan fysisk aktivitet i medelålder och fysisk aktivitet i hög ålder. Två nationellt representativa undersökningar har använts, levnadsnivåundersökningen (LNU) och undersökningen om äldre personers levnadsvillkor (SWEOLD). Deltagarna har intervjuats vid tre tillfällen; i medelåldern (ålder i genomsnitt 53 år) sen medelålder (i genomsnitt 61 år) och i hög ålder (i genomsnitt 81 år) med en genomsnittlig uppföljningstid på 24-30 år. Resultaten från den longitudinella studien visar att fysisk aktivitet i sen medelålder har ett samband med lungfunktion i hög ålder. Detta samband blir mindre starkt när även fysisk aktivitet i hög ålder inkluderas i analysmodellen, men består. Det finns ett starkt samband mellan fysisk aktivitet i hög ålder och lungfunktion i hög ålder. Resultaten visar även att fysisk aktivitet i sen medelålder har ett samband med fysisk aktivitet i hög ålder. Sammanfattningsvis visar denna studie vikten av fysisk aktivitet även högt upp i åldrarna för att bibehålla en god lungfunktion i hög ålder. Att satsa på preventiva insatser som främjar fysisk aktivitet, och då även inkludera de allra äldsta (76+), är av särskild vikt för att främja ett fortsatt hälsosamt åldrande.

Finding predictors of good health and function in older age has become even more important as populations are aging all over the world. Individuals with good lung function live longer and healthier lives. The aim of this study was to investigate the role of physical activity, from midlife to older age, in lung function in older age. In order to increase the understanding of the relationship between physical activity and lung function in old age, the relationship between physical activity in midlife and physical activity in old age was also studied. Two Swedish studies based on nationally representative samples, linked at the individual level, were used in this study, the Level of Living Survey (LNU) and the Swedish Panel Study of Living Conditions of the Oldest Old (SWEOLD). The participants were interviewed on three occasions; at the mean ages of 53 years, 61 years, and 81 years. The Average follow-up time was 24-30 years. The results from this longitudinal study show that physical activity in late midlife was associated with lung function in older age. This association was attenuated when physical activity in older age was adjusted for in the analyses. Les données ont été collectées par des fichiers médicaux et par des questionnaires standardisés.Deux Matrices Emplois-Expositions (MEEs) ont été utilisées pour évaluer rétrospectivement l'exposition professionnelle à l'amiante et à la silice cristalline.Résultats : Il n’y a pas de différence de prévalence de l'exposition professionnelle à l'amiante pour les types histologiques. Pour la silice cristalline, un excès limite de la prévalence de l'exposition a été observé pour le carcinome épidermoïde. La prévalence a été maximisée chez les cas diagnostiqués entre 50 et 59 ans pour l'amiante et moins de 50 ans pour la silice cristalline. Une exposition supplémentaire à l'amiante ou à la silice cristalline n'a pas modifié l'effet du tabagisme pour les types histologiques, la localisation de la tumeur ou l'âge au diagnostic.Conclusion : Le type histologique, la localisation de la tumeur, et l'âge au diagnostic ne peuvent pas être utilisés comme indicateurs de l'exposition professionnelle à l'amiante ou à la silice cristalline.

Introduction: The objective of this thesis was to study the association between tobacco smoking and occupational exposure to asbestos and crystalline silica with the phenotypes of lung cancer. The second objective was to assess the effect modification of the association between tobacco smoking and the phenotypes of lung cancer by occupational exposure to asbestos or to crystalline silica. Methods : The CaProMat study is a pooled retrospective case-only study consisted of9,623 French and Canadian lung cancer cases. All lung cancer cases were histologically confirmed. Data were collected from medical records and through standardized questionnaires. Two job-exposure matrices (JEMs) were used to assess the occupational exposure to asbestos and to crystalline silica. Results: We did not identify a difference of prevalence of occupational exposure to asbestos according to histological type. For crystalline silica, a borderline excess of prevalence of exposure was observed for squamous cell carcinoma. The prevalence of occupational exposure was maximized among lung cancer cases diagnosed between 50 and 59 years for asbestos and less than 50 years for crystalline silica. ▼ Receptor humanog epidermalnog faktora rasta 2 (HER2) pripada porodici receptora protein-tirozin kinaze čija je aktivacija povezana sa proliferacijom malignih ćelija, inhibicijom apoptoze, tumorskom angiogenezom i sposobnosti invazije i metastaziranja. Povećana proteinska ekspresija HER2 receptora može nastati kao posledica amplifikacije gena i/ili transkripcijskih promena. Ekspresija HER2 receptora u humanim tumorima povezuje se sa agresivnijim ponašanjem i lošijom prognozom. Učestalost povećane proteinske ekspresije HER2 receptora u nesitnoćelijskim karcinomima pluća (NSCLC) je najviše zastupljena u adenokarcinomu u odnosu na druge histološke tipove. Identifikacija HER2 pozitivnih NSCLC omogućava određivanje grupe pacijenata koji bi bili kandidati za specifičnu terapiju. Problem predstavlja izbor metode detekcije HER2 receptora i nepostojanje utvrđenog protokola za očitavanje rezultata kao što postoji kod karcinoma dojke i želuca. Osnovni ciljevi ove doktorske disertacije su bili: da se odredi učestalost povećane proteinske ekspresije HER2 receptora u adenokarcinomu pluća; da se uporede rezultati povećane proteinske ekspresije HER2 receptora dobijene korišćenjem HER2 antitela „Hercep Test Dako“ i „Ventana anti-HER2/neu (4B5)“ antitela; da se uporedi prisustvo amplifikacije HER2 gena pomoću in situ hibridizacije (ISH) (Dual IHC HER2 kit;Ventana Medical Systems) retestiranjem uzoraka kod kojih je povećana proteinska ekspresija HER2 receptora ocenjena sa 2+ i 3+ dobijena „Hercep Test Dako“ sa prisustnom amplifikacijom HER2 gena na uzorcima koji su pomoću „Ventana anti-HER2/neu (4B5)“ ocenjeni sa 2+ i 3+; da se uporedi učestalost povećane proteinske ekspresije HER2 receptora i prisustva HER2 genske amplifikacije kod različitih histoloških podtipova adenokarcinoma pluća; da se utvrdi da li je povećana proteinska ekspresija HER2 receptora u adenokarcinomu pluća i/ili prisustvo genske amplifikacije povezano sa demografskim (starost i pol pacijenta) parametrima, pušačkim statusom, pojavom metastaza u regionalnim limfnim čvorovima i udaljenim organima, infiltracijom pleure i okolnih struktura, odnosno stadijumom bolesti. Povećana proteinska ekspresija HER2 receptora u adenokarcinomu pluća iznosi 7,4% za Hercep Test Dako i 2,7% za Ventana anti-HER2/neu (4B5) antitelo. Kod pozitivne ekspresije slažu se u 2%, dok se kod negativne ekspresije slažu u 91,9% slučajeva, što je ukupno 93,9%. Učestalost amplifikacije HER2 gena kod adenokarcinoma pluća je 17,6%, od toga je kod 2,7% slučajeva prisutna high grade amplifikacija. Postoji statistički značajna povezanost između povećane proteinske ekspresije HER2 receptora dobijene upotrebom HercepTest Dako i Ventana anti-HER2/neu (4B5) antitela i amplifikacije HER2 gena.
