Opinnäytetyön tarkoitus oli löytää ratkaisuja ja toimenpiteitä toimittajalaadun kehittämiseksi yrityksessä. Kallionporauslaitteita valmistavan yrityksen takuukustannukset ovat huomattavat, mikä vaikuttaa liiketoiminnan kannattavuuteen. Kolmasosa takuukustannuksista on toimittajalaadusta johtuvia kustannuksia. Toimittajalaadun parantamiseksi aloitettiin toimittajalaadun kehittämisohjelma. Opinnäytetyö kokonaisuudessaan koostuu useammasta osasta. Ensimmäiseksi selvitettiin eniten reklamaatioita ja laatukustannuksia aiheuttaneet toimittajat. Toimittajien valinnassa toimittajalaadun kehittämisohjelmaan otettiin huomioon kriittisten reklamaatioiden määrät ja kehittämispotentiaali. Lisäksi käytiin läpi kriittisimmät reklamaatiot, joista tehtiin juurisyyanalyysi. Työssä löydettiin ratkaisuja ja toimenpiteitä toimittajalaadun kehittämiseksi. Kehittämisen tulee perustua jatkuvaan parantamiseen ja yrityksen henkilöstön on sitouduttava laadun parantamiseen. Yrityksen laadun kehittämiseksi tulee tehdä lyhyen ja pitkän jakson toimenpiteitä. Lyhyen jakson toimenpiteenä keskitytään eniten laatukustannuksia ja reklamaatioita aiheuttaneisiin toimittajiin, joille tehdään tarkempi kehityssuunnitelma. Laadun mittariston avulla valitaan lyhyen aikajakson kehittämiskohteet. Pitkän jakson toimenpiteinä kriittisten mittojen toleranssit on suunniteltava ottamalla huomioon valmistusmenetelmien kyvykkyys. Yhteistyötä on lisättävä komponentteja valmistavien toimittajien kanssa. Komponenttien laadunsuunnittelussa on otettava laajemmin käyttöön APQP. Toimittajavalintoihin tulee kiinnittää enemmän huomiota ja ajatella toimittajavalinnat pitkäaikaisina kumppanuussuhteina. Toimittajalaadun parantamiseksi yrityksen on autettava toimittajia parantamaan laadun tuottokykyä kehitysprojektien avulla. Toimittajiin liittyviä kokonaiskustannuksia saatiin tunnistettua ja laskettua. Tehdystä analyysistä selvisi, että laatukustannuksien osuus kokonaiskustannuksista oli merkittävä. Laatukustannuksien seuraamiseksi tulisi laatia seurantajärjestelmä ja aloitettava kehitysprojektit, joissa toiminnan laatua parantamalla saadaan kustannuksia laskettua. Toimittajalaadun parantaminen ja laatukustannuksien vähentäminen tulee olemaan jatkossakin tavoitteena, koska toimittajalaadun merkitys liiketoiminnan kannattavuuteen on huomattava. Toimittajalaadulla on vaikutusta valmistavan yrityksen lopputuotteen laatuun ja asiakkaiden tyytyväisyyteen.

The purpose of the thesis was to find solutions and actions to develop supplier quality in the company. Technology company’s warranty claim costs are significantly high harming the company’s profitability. Supplier quality problems cause one third of the costs. In order to improve supplier quality a supplier quality program was started. The thesis consist of several parts. O Lean Six Sigma é a metodologia originada a partir da integração das metodologias Lean e Six Sigma. O Lean Six Sigma procura eliminar as perdas e reduzir os defeitos, agregando os benefícios de cada uma das duas metodologias que a dão origem. O objetivo deste trabalho é aplicar a metodologia Lean Six Sigma para melhoria de um processo produtivo. Neste trabalho é apresentada uma revisão bibliográfica sobre as metodologias Lean e Six Sigma, buscando a compreensão das origens do Lean Six Sigma, e uma revisão sobre a metodologia Lean Six Sigma. Baseado na revisão bibliográfica é apresentada uma metodologia para aplicação do Lean Six Sigma, que tem por estrutura o DMAIC (Define, Measure, Analyze, Improve and Control). Na sequência esta metodologia é aplicada para melhoria de um processo produtivo. Os resultados da aplicação do Lean Six Sigma foram: aumento do nível de sigma e redução dos defeitos do processo, demonstrando que as metodologias Lean e Seis Sigma podem ser integradas, e como ocorre essa integração. Os fatores de sucesso da aplicação da metodologia Lean Six Sigma também foram evidenciados.

Lean Six Sigma is a methodology originated from the integration of Lean and Six Sigma. Lean Six Sigma seeks for eliminate waste and reduce defects, combining benefits from both methodologies. The objective of this dissertation is apply the Lean Six Sigma methodology to improve a production process. In this dissertation is presented a bibliography review about the Lean and Six Sigma methodology, looking for understanding of the Lean Six Sigma sources and a review about the Lean Six Sigma methodology. Based on the bibliography review is presented a methodology to apply Lean Six Sigma, that has the DMAIC as the structure (Define, Measure, Analyze, Improve and Control). This methodology is applied to improve a production process. The results of the Lean Six Sigma application were the increase of sigma level and reduction of process defects, showing that Lean and Six Sigma can be combined and how this combination happens. The success factors of the application of Lean Six Sigma methodology were evidenced.

▼ The increase in costs of hospital services, the limitations of public financing, and increased demand in the healthcare sector required new management models focused on process rearrangements. Process with goals of eliminating loss, reducing costs, optimizing time and improving the patient’s experience. The lean six sigma methodology (LSS), originally from manufacturing, was introduced in many areas, including healthcare, and it is based on three pillars: patient focus, time and value. The goal of the study was to apply the lean six sigma guide in the different areas of the EBSERH Network hospital environment, based on the implementation of these concepts in the chemotherapy clinic of the Hospital das Clínicas of the Federal University of Goiás / EBSERH between the period from January 2018 to April 2018. A qualitative, quantitative, applied and exploratory approach was used. As a technical procedure, it was used a bibliographic and action research. The work was conducted using the lean six sigma DMAIC methodology in its 5 steps: define, measure, analyze, implement and control. The current and future value stream map (VSM) was elaborated and the waste was analyzed in the process. Patient scheduling for chemotherapy was changed from manual to electronic, via network communication, based on the available spots in the sector and infusion time of the chemotherapy protocol. The average occupancy rate of the sector was 48.75%, with an increase of 9.32% among the analyzed periods. The total lead time was decreased from 15 days 1 hour and 24 minutes to 9 days and 24 minutes (reduction of 42.8%). A takt (labor rate to meet the daily demand of the sector) was obtained with an average value of 26.9 minutes for the activities performed during the infusion of chemotherapy. The average billing of oncological procedures between january and april/ 2018 was R$ 492,361.54. This value corresponded to the use of 48.75% of the chemotherapy outpatient capacity. A projection of 75% occupancy target for the sector would lead to a proportional billing in the amount of R$ 757,479.29. That is, the sector could bill more R$ 265.117,75 monthly, using the structure and human resources already available. A major advantage of lean methodology as an instrument for healthcare management and innovation lies in the ability to make major changes, optimize processes and eliminate loss without the need for hard technology or high financial investments. Changes can be provided using simple materials, or even without the need for any material, once the redesign of the process, reallocating human resources and activities, can be the solution to the problems under analysis. ▼ I denna rapport har avgångspunktligheten på SJ:s tågdepå i Hagalund undersökts, med som syfte att hitta bakomliggande mönster och orsaker till förseningar. Det teoretiska ramverk som använts är en kombination av Lean och Six Sigma, där managementverktyget Lean har sin grund i optimering av resursutnyttjande och minskande av slöseri. De Lean-begrepp som främst tagits fasta på är Visualisering och Standardisering, då SJ har haft stora problem inom just dessa områden. Det praktiska arbetet i rapporten har i sin tur följt Six Sigma-metoden DMAIC (Define, Measure, Analyze, Improve, Control), där stor tyngdpunkt har lagts vid Mätning och Analysering. Den kvantitativa data som använts kommer direkt från SJ:s egna förseningsrapporter, där tåg som avgår fem minuter eller mer efter planerad avgångstid anses försenade. Denna försening avläses maskinellt, och den planerade avgångstiden kallas Rättidighet (RT). Alla RT > 5 avgångar noteras automatiskt, och för dessa förseningar rapporteras även förseningsorsak. Förseningsorsakerna sätts manuellt av personal på Depå Hagalund utifrån en begränsad lista med valbara förseningstyper (JDE-koder). Arbetet i denna rapport gjordes under en sexmånadersperiod hösten/vintern 2012/2013. Arbetet tittade främst på data från perioden för Tidtabell 2012 (T12), vilken sträckte sig från den 11e december 2011 till den 8e december 2012. Extra punktmarkering lades på november 2012, under vilken ett experiment i punktlighetsrapportering utfördes. Stor vikt lades vid visualisering och standardisering av processer, där tidtabellen är en naturligt styrande process vilken resursfördelat ger arbetsprocesser. En dataminering av förseningsstatistiken för T12 visade under arbetets gång på stora brister i den manuella förseningsrapporteringen, där inkonsekvent användning av JDE-koderna gjorde det omöjligt att med säkerhet få fram bakomliggande förseningsorsaksmönster. För att komma runt bristerna i grunddatan gjordes därför ett ovan nämnt experiment i november 2012 där all förseningsorsaksrapportering under månaden övervakades så att JDE-kodsättningen blev korrekt gjord. Resultatet visade på ett stort mörkertal i inrapporterade förseningar av typen ”Handhavandefel”, förseningar som tidigare dolts i slaktratskoden ”Övrigt”. En visualisering av den maskinellt inrapporterade avgångspunktligheten, RT-datan, kunde i sin tur visa på brister vid skiftesbyten, samt fastställa 13:00-18:00 som en generellt tungt förseningsdrabbad tid på dygnet. Dygnsvisualiseringen av förseningar jämfördes även med en dygnsvisualisering av tågens tid på bangården, den så kallade vändtiden, där tåg som spenderar mindre än tre timmar på Depå Hagalund kunde påvisas påverka den allmänna punktligheten i högre grad än andra avgångar. Med hjälp av regressionsanalys kunde även dokumenteras att sent ankomande tåg med kort vändtid i högre utsträckning även avgår försenade, meden sent ankommande tåg med längre vändtid ej statistiskt…

▼ PURPOSE: The objective of this study was to identify opportunities for pharmacy technician workflow improvement using the principles of Lean Six Sigma. The impact of this study may streamline pharmacy technician workflow as well as eliminate/consolidate tasks resulting in increased efficiency while maintaining quality. METHODS: This study took place at St. Luke’s Episcopal Hospital (SLEH) in Houston, TX. The focus group was used to identify the problem. Two methods for data collection were utilized to establish baseline information; automated information and observational time and motion studies. Observational and automated data was presented to the focus group and the focus group was responsible for recommending and piloting changes. The focus group also identified pharmacy technician tasks that needed improvement as well as relate these tasks back to the voice of the customer and task purpose. Data was presented to the focus group and the focus group was responsible for recommending and piloting changes. The suggested changes were implemented in a pilot study. RESULTS: The focus group identified “ordered medications not loaded” (OMNL) as an area for workflow improvement. OMNL cycle time was defined as the time from when the user logs in the Acudose machine to the point the user completed the OMNL process. There were five parts to OMNL which were conducted at various times throughout the day: restocking, pulling, assigning, unassigning and loading. Time studies were conducted and technicians were shadowed on all three shifts. Observational and automated data findings were presented to focus group. These studies found technicians spent approximately 69.5 hours a week on OMNL for 1096 line items. On average, a patient had an active order for this medication for 2 days. The focus group recommended two changes to the OMNL process. The first change was to consolidate the three aspects of OMNL to one step. The second was to eliminate loading scheduled daily medications during OMNL. These changes were piloted and implemented which resulted in reducing OMNL by 39.1% and a net time savings of 33.6 hours a week. Although the cart fill volume did increase, there was no impact on missing doses. CONCLUSION: The application of Lean Six Sigma can improve operational workflow of pharmacy technicians as well as reduce waste as seen in this study. The SLEH should continue to utilize these concepts to identify further opportunities for pharmacy technician workflow improvement and reduction in waste. ▼ A continuous improvement project (CIP) is a structured improvement project using a team of people "typically representing different departments or units in the organization" working to improve a process or work area over a relatively short period of time, such as a few days or up to several months. A CIP may use different improvement methodologies and tools, and may thus be defined according to the improvement approach. For instance, an organization adopting Lean as an improvement approach is likely to have CIPs implementing Lean tools, such as 5S or value stream mapping. These projects may be referred to as Lean projects in general, although they may also represent accelerated improvement projects such as Kaizen events, Kaizen blitz, or rapid improvement projects. Alternatively, an organization utilizing Six Sigma as an improvement approach may have Six Sigma projects that use the Define-Measure-Analyze-Improve-Control (DMAIC) process and statistical tools. Some organizations adopt an integrated improvement approach, such as Lean Six Sigma, and therefore may have CIPs with an even broader set of tools from which to choose. Lastly, many organizations may have an improvement approach not characterized by any single set of improvement processes and tools, and thus, may be thought of generally as process improvement, or quality improvement, projects using a traditional methodology as plan-do-study/check-act (PDSA or PDCA). In this dissertation, all of these types of improvement projects are referred as CIPs. Since the 1980s, hospitals have been using CIPs to address some of the problems in hospitals, such as quality in healthcare delivery, internal process efficiency, communication and coordination, and the cost of services. Some hospitals have achieved significant improvements, such as reducing the turnaround time for clinical laboratory results by 60 percent and reducing instrumentation decontaminations and sterilization cycle time by 70 percent. However, as with many other companies, hospitals often experience difficulty achieving their desired level of improvements with CIPs. Therefore, the purpose of this dissertation is to identify the critical success factors (CSFs) related to CIP success. In order to achieve this goal, five objectives were achieved: creating a methodology to assess the maturity or evolution of a research field (manuscript1), identifying a comprehensive list of CSFs for CIPs (manuscript #2), assessing the maturity of the published literature on CIPs in hospitals (manuscript #3), identifying the most important factors related to CIPs in hospitals (manuscript #4) , and conducting an empirical investigation to define the CSFs for CIPs in hospital settings (manuscript #5 and #6). This investigation was conducted in three phases: research framing, variable reduction, and model development and testing. Numa época de grave crise económica com a maioria dos mercados a registarem recessões significativas impõem-se a mudança de paradigmas e das abordagens de gestão tradicionais. Tendo em conta o mercado cada vez mais concorrencial, questões relacionadas com a qualidade e o baixo custo adquirem vital importância para a subsistência da maioria das empresas. Assim, a garantia de prosperidade dos negócios das empresas poderá ser atingida através da análise e da reengenharia dos seus processos, visando a eliminação de tudo o que não acrescente valor para a empresa, isto é, a eliminação de desperdícios. A metodologia Lean Six Sigma surge da fusão da metodologia Lean (com origem no Toyota Production System) e da metodologia Six Sigma (com origem na Motorola). A sua aplicabilidade há muito que passou as barreiras dos processo de produção sendo, actualmente, aplicada aos serviços. Tem vindo a afirmar-se no contexto mundial, com diversas empresas a implementarem as suas técnicas e ferramentas e a obterem bons resultados na redução de custos, na melhoria de qualidade, na relação com o cliente e no relacionamento entre os intervenientes nos processos da empresa. A presente dissertação propõem a aplicação de ferramentas e técnicas Lean Six Sigma ao contexto real de uma empresa da indústria farmacêutica e que se encontra inserida num mercado fortemente concorrencial, com o objectivo de proporcionar a criação de valor e a eliminação de desperdícios. Para o desenvolvimento do caso de estudo recorreu-se a inúmeras ferramentas Lean Six Sigma, nomeadamente, 5S, Workflow Diagrams, Kaizen, Andon, Standard Work, Matriz de Prioridades (GUT), 5WHY‟s, Brainstorming, SIPOC, entre outras técnicas da engenharia industrial, tendo como metodologia de abordagem o ciclo DMAIC. O caso de estudo permite reconhecer a importância desta metodologia que é sustentada pelos ganhos obtidos e pelo investimento nulo na maioria das situações. São evidenciadas algumas das limitações metodologia e é realçado o aspecto de complementaridade com a ergonomia de postos de trabalho.

▼ This research deals with the optimization of preventive maintenance (PM) and process improvement for maintenance management in service sector. To stay competitive and sustain long-term profitability, process improvement methodologies have become strategically important for maintenance management in recent years. These include well-known approaches such as Total Quality Management (TQM), Business Process Reengineering (BPR), Six Sigma, Lean and Lean Six Sigma (LSS). The adoption of LSS and PM optimisation in the maintenance services sector, however, is still at an early stage. There has been very limited research in this topic reported in the literature. This research has explored the LSS and PM optimisation through case studies in vehicle fleet maintenance. This research has made contributions to knowledge in the quality management and, in particular, the process improvement methodology and service quality for the vehicle maintenance service sector, but potentially also in a broader context. The main contribution is the establishment and demonstration of a sound methodology and model to integrate LSS and PM optimisation in the vehicle fleet maintenance. The model also provides guidelines for further development of a practical process improvement framework. The proposed model is therefore considered as a basis for further empirical work relating to the process improvement in the services context. Further, this study has developed a total cost model to optimise the PM activities based on both the PM maintenance cost and the quality loss cost. There have been two parallel developments for determining the optimum PM interval, one based on the maintenance cost without considering the quality loss, and the other based on the quality loss without considering the maintenance cost. A novel approach combining the maintenance cost and quality loss has been developed. Moreover, the total productive maintenance (TPM) implementation in the service process and the integration with the LSS/PM optimisation has enhanced the theory and practice of continual improvement in maintenance. The implementation of the integrated model of LSS and PM optimisation through case studies in vehicle fleet maintenance has provided an impetus for establishing best practices within the organisation under study. The implementation of this model has also increased the future performance of the organisation. It has enabled the maintenance management based on a strong customer-supplier relationship by satisfying customer requirements. The proper utilisation of the resources and the application of LSS tools and techniques will upgrade the company procedures and reduce the maintenance non-conformities, with the key process parameters continually improved and ultimately optimized.

A l’image des grands groupes, les PME souhaitent, elles aussi, profiter des méthodes modernes de management pour améliorer leurs processus, leurs performances et leur compétitivité. L’intérêt du secteur des PME se porte sur les éléments du concept Lean Six Sigma qui leur semblent appropriés pour réaliser ces objectifs. Après avoir constaté une lacune dans les recherches sur ce sujet et une lacune méthodologique concernant l’implémentation du LSS dans les PME, nous proposons une méthodologie de conception, d’application et de contrôle du LSS pour ces PME, que nous avons appelée LSS Plutus. Elle est dédiée aux PME de la production et elle prend en compte leur spécificité. Dans ce but, nous avons restreint la boîte à outils traditionnelle du LSS utilisée dans les grands groupes. Nous avons également créé deux variantes pour mettre en œuvre notre méthodologie qui correspondent aux besoins, capacités et maturités différents des PME. Nous avons élaboré des indicateurs, des outils, ainsi qu’un guide d’application de la méthode LSS pour les PME de la production et sa version informatique en TIBCO. Nous avons décidé de renforcer l’efficience des améliorations, en déployant les actions de planification, de standardisation et de pérennisation des changements. Dans ce but, nous proposons de modifier les critères du Modèle d’Excellence E.F.Q.M. pour qu’elles puissent prendre en compte les conditions de fonctionnement de la méthode LSS dans les PME. La méthodologie proposée a été vérifiée dans deux PME de la production en Pologne et en France, ce qui a confirmé son utilité et son efficacité en termes d’amélioration des performances et de développement des organisations

As well as the large companies, also the small and medium-sized enterprises desire to take advantage of modern management methods to improve their process, their performances and competitiveness. The interest of the SMEs is focused on the elements of the Lean Six Sigma concept that seem to be appropriate to achieve these goals. As we have identified a gap in the research on this topic and a methodological gap about the LSS implementation in the small and medium companies, we propose a methodology that we called LSS Plutus to design, apply and control Lean Six Sigma in the SMEs. It is dedicated to the small and medium production enterprises and it takes into account their specificity. For this purpose, we have limited the traditional LSS toolbox used by the large companies. We have also created two variants to apply our methodology according to the different needs, capacities and maturities of the SMEs. We have developed indicators, tools, as well as a practical guide for applying the LSS for the production SMEs and its computerized version in TIBCO. To enhance the efficiency of the improvements, we have deployed the planning, standardization and sustainability of the changes. For this purpose, we propose to modify the criteria of the EFQM Excellence Model in order to adapt it to the operating conditions of the Lean Six Sigma method in the SMEs. The proposed…
