School nurses are at the forefront of leading health services for the school aged population and Government policy documents have endorsed their leadership role. Effective leadership is necessary for quality health provision and ensures that better outcomes are achieved for children and young people. Current reforms within the healthcare economy have resulted in major transformational change for school nurses. Effective leadership during change is essential if the services for children and young people are to be responsive and needs led. The literature available examines leadership within nursing, with the addition of limited studies of public health nurse leadership in the community. There remains a paucity of empirical research that examined leadership within school nursing. This study, therefore, aims to examine how school nurses lead clinical practice. This study utilised a focused ethnographic approach to examine school nurse leadership. A sample of 10 qualified school nurses were recruited, drawn from one Trust within England. Semi structured interviews and limited informant observations were undertaken to collect data from the informants, consistent with focused ethnography. Data were analysed using Brewer’s (2000) data analysis framework. The study found that school nurses were leading clinical practice within a climate of change and complexity. The rise in safeguarding and complex medical concerns had resulted in school nurse resources deployed to manage this priority work, limiting the development of a public health focused service. School nurses did not necessarily see themselves as leaders. However, they have provided a variety of leadership examples within clinical practice. They are leading clinical practice using a transformational, ethically positioned leadership approach. The findings of this study supported the development of a model for school nurse leadership, that encompasses current thinking from quantum leadership, which recognises that change in healthcare is persistent and encompasses the need for an ethical, transformational approach, with addition skills that focus on understanding whole systems within organisations and the impact change has on these. There were a number of recommendations, not least of which were for organisations and commissioners of the school nursing service who needed to understand the fundamental leadership role of the school nurse to work collaboratively to develop services for the school aged population. School nurses required specific leadership support, to enable them to lead clinical practice effectively in a constantly changing healthcare economy. Professional leadership development must be situated within practice to aid continuous professional development. Specialist practice programmes need to reflect the impact that persistent change has upon the school nurse leadership role and equip qualified school nurses with the skills and ability to lead in this climate, negotiate the way and act as an expert resource.
