▼ Background. Lamellar keratoplasty is a relatively new surgical procedure for replacing the diseased layer of the cornea with a healthy layer. There are only few data regarding graft rejection in lamellar keratoplasty. The objective of this paper was to compare lamellar keratoplasty with penetrating keratoplasty for the incidence and risk of graft rejection. Methods. This retrospective study included a total of 216 consecutive patients who underwent penetrating keratoplasty or lamellar keratoplasty at the Department of Ophthalmology, Charité CVK, between 1999 and 2000 and between 2007 and 2009. Complete data were obtained in 172 cases. Statistical analysis was performed us¬ing SPSS (Statistical Package for the Social Sciences), version 12.0. Descriptive statistics, χ2-test and mean values were used to describe and assess the data. Results. Out of 172 eyes 41 were treated with lamellar keratoplasty and 131 eyes were treated with penetrating keratoplasty. The incidence of graft rejection was 2.4% in the lamellar group and 29.8% in the penetrating keratoplasty group (χ2 = 13.07, p < 0.001). Of 44 patients with Fuchs’ endothelial dystrophy (FED) 28 underwent posterior lamellar keratoplasty and 16 patients were treated with penetrating keratoplasty. The incidence of graft rejection in this group was 3.6% after posterior lamellar keratoplasty and 18.75% after penetrating keratoplasty. Conclusions. In this retrospective study with univariate analysis of risk factors, graft rejection was less frequent after lamellar keratoplasty than after penetrating keratoplasty. A comparison of both groups was limited because of different indications for surgery. Posterior lamellar keratoplasty decreased the risk of graft rejection under conditions with the same indications (Fuchs’ endothelial dystrophy). Advisors/Committee Members: m (gender), Priv.-Doz. ▼ Introduction: Acquired corneal blindness is the third leading cause of visual loss globally. The greatest proportion of the blindness is either preventable or treatable; most of those due to corneal diseases are amenable to successful prevention or treatment. Penetrating keratoplasty can alleviate selected cases of corneal blindness and is one of the most successful and most frequently performed solid organ transplants world-wide. However, not many centres offer penetrating keratoplasty in Africa; those that do depend on modest amounts of donor corneas from overseas. Aim: To document the indications for and outcomes of penetrating keratoplasty in Kenya over a 2-year period. Methods: A retrospective case series of 174 eligible penetrating keratoplasties performed in 6 different facilities in Kenya from January 2001 to December 2011 was undertaken. Univariate and multivariate analyses were done. Corneal graft survival probabilities were determined using the Kaplan-Meier method. Results: Keratoconus was the commonest indication accounting for 48.8% followed by bullous keratopathy at 18.4%.Pre-operatively, 66.7% of the eyes were blind with visual acuities <3/60.Corneal epithelial defect (43.5%) was the commonest early complication followed by corneal oedema (25.2%); persistent corneal oedema was the commonest late postoperative complication. At 24 months, 82.2% of the grafts remained clear while 17.8% (n=31) were not clear; only 8.24% of keratoconus grafts failed while 26.97% from other indications failed (relative risk=3.2745, [95% CI 1.4895-7.1986], p<0.0012).Postoperatively, 71.82% of all grafts had final uncorrected vision of 6/60 or better, while 16.67% of grafts remained blind. Thirty-one (17.82%) grafts failed, of which 25.8% were due to primary graft failure and 74.2% were due to secondary graft failure. Survival probabilities at 24 months showed 90% of keratoconus grafts survived while 68% for the other indications survived with p< 0.0068. Poor predictors of corneal graft outcome were: bullous keratopathy (p<0.0019), postoperative glaucoma (p<0.0023), infection (p<0.0001), and persistent corneal oedema (p<0.0001). Conclusions: Keratoconus remains the leading indication for corneal grafts in Kenya and grafts provide meaningful vision for the commonest indications. Corneal graft survival has improved, especially for corneal dystrophies and keratoconus. These findings are similar to results in industrialised countries. Poor predictors of graft outcome were corneal ulcers, herpes simplex keratitis, corneal graft glaucoma, and corneal graft oedema. Contemporary causes of corneal blindness in Africa such as vitamin A deficiency and trachomatous corneal blindness were not encountered and may be receding in importance. Our findings suggest most corneal grafts in this part of Africa could attain longer graft survivals as well as better visual outcomes.

▼ Die "Descemet Membrane Endothelial Keratoplasty" (DMEK), als Verfahren der posterioren lamellären Keratoplastik, hat sich seit der Erstbeschreibung 2006 für die Behandlung endothelialer Keratopathien fest etabliert. Die wichtigsten Indikationen stellen hierbei die Fuchs Endotheldystrophie, die bullöse Keratopathie und die Transplantatdekompensation nach Keratopastik dar. Im Vergleich der klinischen Ergebnisse nach DMEK konnten wir für diese wichtigsten Indikationen Unterschiede im postoperativen Verlauf herausarbeiten. Hierbei zeigten sich vergleichbare Visusergebnisse zwischen Fuchs Endotheldystrophie und bullöser Keratopathie, während der Visus bei Transplantatdekompensation nach DSEK, auch 12 Monate postoperativ, reduziert war. In diesem Zusammenhang konnten wir mittels Immunhistochemie fibrotische Veränderungen im Stroma-Interface dekompensierter Hornhäuten nach Keratoplastik, die als Ursache des reduzierten Visus angesehen werden können, nachweisen. Desweiteren zeigten sich beim Vergleich zwischen Fuchs Endotheldystrophie und bullöser Keratopathie unterschiedliche Muster im Auftreten postoperativer Transplantat-Ablösungen. Bei der Fuchs Endotheldystrophie kam es häufiger zu wiederholten Ablösungen. Diesbezüglich konnten wir immunhistochemisch eine Rarefizierung von Matrixproteinen innerhalb der Descemet-Membran bei der Fuchs Endotheldystrophie beobachten. In weitergehenden Untersuchungen zu den Pathomechanismen der Transplantat-Ablösungen nach DMEK konnten wir zeigen, dass der Verlust von Matrixproteinen mit einer Auflockerung der Verbindung zwischen den Schichten der Descemet-Membran (Anterior Banded Layer (ABL) und Posterior Non-Banded Layer (PNBL)) assoziiert war und in Fällen mit Transplantat-Ablösung häufiger Risse innerhalb der Descemet-Membran beobachtet wurden. Somit können Reste der Descemet-Membran als Hindernis für die adäquate Transplantat-Adhärenz und damit als eine Ursache für Transplantat-Ablösungen angesehen werden. Insgesamt konnten wir sehr gute funktionelle Ergebnisse nach DMEK aufzeigen, sodass eine vollständige Visuserholung in Augen ohne okuläre Begleiterkrankungen möglich ist. Zur Beuteilung prognostischer Faktoren der klinischen Ergebnisse analysierten wir Daten nach primärer DMEK bei Fuchs Endotheldystrophie. Als wesentlichen Prädiktor für den Visus, 12 Monate nach DMEK, konnten wir die präoperative zentrale Hornhautdicke identifizieren. Auge mit einer präoperativen Hornhautdicke < 625 µm zeigten dabei bessere Visusergebnisse, als Augen mit einer präoperativen Hornhautdicke ≥ 625 µm. Außerdem konnte ein früherer Visusanstieg in Augen beobachtet werden, deren zuvor operiertes Partnernauge bereits 6 Monate nach DMEK eine Sehschärfe von ≥ 0,8 Dezimalvisus erreichte. Hinsichtlich der Prognose der zu erwartenden postoperativen Endothelzelldichte zeigte sich die Ausgangs-Endothelzelldichte des Transplantats als prognostisch relevant. ▼ Cornea transplant surgery, also known as Keratoplasty, is a procedure in which a part of a patient’s cornea is replaced with corneal tissue from a healthy donor eye obtained from an eye bank. The cornea being replaced is referred to as the host while the donor cornea is referred to as the graft. Presently, the procedure utilizes 16 to 24 interrupted sutures in a radial pattern to ensure the graft-host wound closure post operation. Suture placement needs to be performed with a high level of skill, ensuring all sutures are near identical in their pass length, depth, tension, distance from the optical center of the cornea and radiality. Problems with any of the mentioned factors could result in the induction of astigmatism, requiring multiple post-operative visits – usually over the course of a year – to assess and mitigate it. The procedure in itself is time intensive, taking between 45 and 75 minutes for a full transplant by an experienced surgeon. Therefore, the cornea transplant surgeon needs a method of graft-host wound closure that changes the way current sutures are used or the way their forces are distributed, minimizes the number of sutures used in the current manner, and/or abandons sutures in lieu of another technology because sutures as used in the current manner create astigmatism that is difficult to manage (for the reasons listed above, and probably others). Finite Element Analysis was used to predict the behavior of the cornea under different loading conditions. One of the solutions proposed in this work attempt to limit the number of sutures required to a maximum of 12 by utilizing stopping forces created due to the geometry of the graft-host wound interface that minimize the separation of the graft from the host under the influence of intra-ocular pressure. The other solution is the introduction of an intra-stromal corneal ring segment which enables suturing force to be taken by the ring rather than the central cornea leading to less central cornea distortion.

Objetivou-se avaliar e comparar as repercussões de duas etapas do transplante lamelar posterior sobre o endotélio da córnea de suínos utilizando a microscopia eletrônica de varredura. Utilizaram-se 30 bulbos oculares de 20 suínos, mestiços (1/2 Large White ½ Landrace), machos, com seis meses de idades e com peso médio de 100 kg. Foram designados dois grupos experimentais. No G1 constituído por 10 bulbos oculares, foi avaliado o dano ao endotélio da córnea após a confecção do botão doador. No G2, formado por 20 bulbos oculares, foi avaliado o dano ao endotélio da córnea após a inserção do botão doador no leito receptor utilizando introdutor de Busin. Perdas celulares ocorreram e variaram de acordo com o grupo estudado. A perda endotelial média do G1 foi 8,41% e no G2 17,31%. As diferenças foram estatisticamente significativas entre os grupos estudados. No presente estudo e nas condições experimentais realizadas foi possível concluir que a inserção do botão doador induziu maiores danos ao endotélio corneano de suínos, comparativamente à sua criação.

Endothelial keratoplasty has been adopted as an alternative to penetrating keratoplasty in the treatment of corneal endothelial disorders. However, references identifying at what time of posterior lamellar transplant surgery occurs greater damage to the endothelial cells of the donor button were not found, when consulting the literature. The aim of this study was to assess and compare the effects of two stages of posterior lamellar keratoplasty on the corneal endothelium of swine using scanning electron microscopy. A total of 30 eyes were evaluated in this study. The eyes were divided in two groups of ten each eye: G1 (evaluated after delamination and preparation of the donor button) and G2 (evaluated after delamination and preparation and insertion of the donor button using Busin glide). Cell loss occurred and varied according to the study group. The average endothelial cell loss was 8.41% in G1 and 17.31% in G2. The differences were statistically significant between the groups studied. Scanning electron microscopy demonstrated damage in either group. The average endothelial cell loss was 8.41% in G1 and 17.31% in G2. The differences were statistically significant between the groups studied. It was concluded in the present study and also in the experimental conditions that the insertion of the donor button has induced greater damage to the corneal endothelium of swine compared to its creation.

Zielsetzung: Wir untersuchten die Effekte eines selektiven Glucocorticoidrezeptoragonisten (SEGRA) sowie von Spironolakton und Everolimus hinsichtlich der Prävention einer Transplantatabstoßung in einem Ratten-Hornhauttransplantatmodell mit diskrepanter MHC Klasse I/II. Methoden: Es wurde an insgesamt 133 weiblichen Lewis-Ratten eine perforierende Keratoplastik durchgeführt. Syngene Hornhauttransplantationen an 20 Tieren dienten als Kontrolle für technische Probleme. 113 Lewis-Raten erhielten allogene Transplantate von Dark Agouti-Spendertieren. Die Empfänger wurden randomisiert den Therapiegruppen zugeteilt, erhielten das Wirkstoffvehikel oder blieben unbehandelt. ME-CD-SEGRA (0,25% SEGRA-haltige Mikroemulsion) oder Everolimus (als 0,05% bzw. 0,025% Mikroemulsion) wurden fünf Mal täglich topisch angewandt, während Spironolakton als Suspension 100 mg/kg pro Tag oral gegeben wurde. Die Therapie wurde bei der SEGRA Gruppe über 35 Tage postoperativ appliziert. Die Behandlung mit Everolimus oder Spironolakton wurde bis zum Zeitpunkt der Transplantatabstoßung durchgeführt. Zum Monitoring immunologischer Parameter wurde die Genexpression von Zytokinen (IL-4, IL-10, IFN-γ, TNF-α) und T-Zell Markern (CD3, CD25) im Transplantat zu vorbestimmten Zeitpunkten vorgenommen. Ergebnisse: Durch topische Anwendung von ME-CD-SEGRA konnte die mittlere Überlebenszeit der Hornhauttransplantate (42,2 ± 4,0 Tage) gegenüber der unbehandelten Kontrollgruppe (11,7 ± 1,2 Tage, p = 0.00003) oder Tieren, die nur die Wirkstoffvehikel erhielten (15,0 ± 1,5 Tage) signifikant verlängert werden. ME-CD-SEGRA verminderte die mRNA-Spiegel aller untersuchten Zytokine und T-Zell-Marker, insbesondere für IL-4 gegenüber unbehandelten Tieren (p < 0.05). Die lokale Anwendung von 0,05 oder 0,025 % Everolimus verlängerte die mittlere Überlebenszeit der Transplantate (21 ± 6.57 bzw. 16,4 ± 2,3 Tage) im Vergleich zur Wirkstoffvehikel-Gruppe (13,3 ± 1,7 Tage, für beide p < 0.001). Gleichzeitig wurde eine erhöhte IL-10 mRNA Expression (p = 0,015) im Transplantat nachweisbar. Spironolakton verlängerte ebenfalls signifikant die Überlebenszeit (14,9 ± 2,0 Tage) im Vergleich zur Wirkstoffvehikel Gruppe (12,3 ± 1,2 Tage, p = 0,007) und der unbehandelten Kontrollgruppe (13,0 ± 1,0 Tage, p = 0,01). Spironolakton beeinflusste sowohl die systemische (Senkung der CD25 mRNA Expression in der Milz; p < 0,01) als auch die lokale Immunregulation (Erhöhung der IL-10 mRNA Expression in der Hornhaut; p < 0,01). Schlussfolgerung: Alle getesteten Medikamente haben das Überleben des Hornhauttransplantats nach der Keratoplastik statistisch signifikant verlängert. ▼ The incidences of intraocular pressure (IOP) elevation and post-keratoplasty glaucoma in corneal endothelial disorders cannot be ignored, despite that the incidences after DMEK appear to be lower than after DSAEK and after penetrating keratoplasty, because complications are possible, especially irreversible visual field defects. For the early detection of an IOP elevation the IOP measurement is crucial. So far, there is no gold standard for IOP measurement. A direct exchange of the different measurement methods is not recommended because of a broad distribution of values and follow-ups should always be done with the same measurement method. The most common cause of IOP elevation in all three surgical techniques is the steroid-induced IOP elevation. The rate of steroid-induced IOP elevation may be lower in the future due to changes in the postoperative therapy regime after lamellar techniques to less potent steroids. Another important risk factor for IOP elevation and post-keratoplasty glaucoma represents the pre-existing glaucoma. Patients with pre-existing glaucoma should therefore be monitored postoperatively more often and over a longer period. Especially because these patients often require not only a drug therapy. After posterior lamellar keratoplasty, the risk of IOP elevation in the first 48 hours after surgery is significantly increased because of a mechanical angle-closure glaucoma, but so far with no detectable consequences. Peripheral anterior synechiae appear less often after DMEK than after DSAEK and penetrating keratoplasty as a cause of IOP elevation and post-keratoplasty glaucoma. For all surgical procedures, patients with FED have a low risk of IOP elevation and post-keratoplasty glaucoma. In future, the expansion of the spectrum of indications for the lamellar techniques, in particular the DMEK, possibly increases the risk of IOP elevation and the development of a post-keratoplasty glaucoma. Especially when it is necessary to use higher potent steroids again due to the more complicated indications. Overall, a successful management of IOP elevation and the post-keratoplasty glaucoma is possible by an effective anti-glaucomatous therapy and results in a good visual acuity, a good graft survival and no increased endothelial cell loss. ▼ Keratoconus is a disease of the cornea that usually has its onset during puberty or early adulthood. The annual incidence of keratoconus is 1 per 7500 and the estimated prevalence in the general population is 1 per 375. These values are five-fold to ten-fold higher than previously reported values in population studies. In advanced keratoconus cases a corneal transplantation is often warranted to restore visual function. Receiving a corneal transplant has a substantial impact on quality of life, especially when visual acuity deteriorates or if a corneal transplantation must be performed to restore visual acuity. Preventing the need for corneal transplantation is one of the priorities in keratoconus care. In the Netherlands, the annual number of corneal transplantations decreased after the introduction of corneal crosslinking, suggesting that crosslinking can reduce the need for corneal transplantation. Lamellar transplantation techniques were increasingly performed over the last decade, accounting for approximately 35% of keratoplasties in 2010 and thereafter. The classic penetrating keratoplasty is still common, with a stable frequency form 2010 onwards. This thesis provided evidence for the stabilizing effect of crosslinking in children up to five years. An interesting finding was that the chance of progression despite crosslinking treatment is much higher in children (22%) then in adults (2-10%). The prognosis after treatment is related to the location of the cornea that is most affected by keratoconus; patients with more central cones benefitting more from treatment in terms of cone flattening and patients with lower pre-treatment visual acuity are more likely to benefit from crosslinking with respect to visual acuity. Although the transepithelial crosslinking technique was initially lauded by many for its potential beneficial effects, head-to-head comparison between the transepithelial technique and the (original) epithelium-off technique demonstrated that transepithelial crosslinking was less effective in halting keratoconus progression than epithelium-off crosslinking. Furthermore, there was no clinically relevant difference between the two techniques on higher-order aberration, which are optical disturbances of the cornea that are not correctable by glasses and that are much more frequent in keratoconic eyes than in normal eyes. The cost of the crosslinking treatment, including pre-treatment screening and one year follow-up are €1759.06 ($1929.47). In order to investigate the rational of investing money in crosslinking treatments in order to prevent future corneal transplantations and loss of visual acuity, we performed a cost-effectiveness analysis. ▼ textabstractThe first section deals with the mechanisms by which sutures, incisions and intracorneal contact lenses produce a change in corneal curvature. To clarify the mechanisms by which incisions and sutures produce astigmatism, we made incisions and placed sutures in the corneoscleral limbus of human eye bank eyes (chapter 2). The changes in corneal curvature were studied by central keratometry and by shadowgraph photography of the corneal contour. A radial corneal suture induces astigmatism by tissue compression. A radial anterior suture comprises more tissue on the outside than on the inside of the cornea. This moves the epithelial side of the cornea posteriorly towards the center of the globe. This posterior displacement decreased the sagittal depth of the cornea, so that the depth of the anterior chamber decreased slightly. The center of the cornea steepened in the meridian of the suture. This can be illustrated by holding a card between the thumb and first two fingers and applying pressure to make the card arch over the palm. When the forefinger of the other hand presses down near the arched card, this area moves toward the palm like limbal tissue near a suture and the arch of the central area of the card increases; resembling the steepening of the central cornea. With this theory we can understand the mechanism by which sutures produce corneal astigmatism after cataract surgery. Incisions in the anterior layers of the cornea weaken the mechanical stability. The intraocular pressure then causes the wound to gape arid outward bulging of the cornea. The central corneal curvature flattens. Corneal incisions are clinically used with radial keratotomy, an operation for the correction of myopia. It is one of the techniques of refractive corneal surgery. Refractive corneal surgery (chapter 3) constitutes of surgical techniques to change the corneal power so that the patient can see better without contact lenses or glasses. All these techniques are irreversible which led us to try and find a method that would be reversible. We found it to be possible to flatten the central corneal curvature of human eye bank eyes by radial interrupted nylon sutures (chapter 4).

▼ Purpose: Intraocular pressure (IOP) elevation and post-keratoplasty glaucoma occur commonly after penetrating keratoplasty (PK), but also after Descemet stripping endothelial keratoplasty (DSEK). In this study, we evaluated the risk after Descemet membrane endothelial keratoplasty (DMEK) to develop IOP elevation and post-DMEK glaucoma. Methods: The 12-months incidence of IOP elevation and post-DMEK glaucoma was analyzed retrospectively in the first 117 consecutive eyes that underwent DMEK between September 2011 and December 2012 at the Universitätsmedizin – Charité, Berlin. IOP elevation was defined as IOP ≥22 mmHg, or ≥10 mmHg from preoperative baseline. The assessment included the pre-operative history of corneal disease and glaucoma. Furthermore, the response to anti-glaucoma treatment, the graft failure, the IOP, the visual acuity, and the endothelial cell count were evaluated. Results: The 12 month- incidence of IOP elevation was 12.10% [95% confidence interval (CI): 0.94%, 18.37%], post-DMEK glaucoma 2.7% (95% CI: -0.44%, 5.85%). The most frequent cause remained steroid-induced IOP elevation, with an 12-month incidence of 8.0% (95% CI: 7.95%, 8.05%). In all cases, IOP elevation was treated effectively by tapering down steroid medication and initiating or increasing anti-glaucoma medication. The incidence of postoperative pupillary block IOP elevation was 15.40% (95% CI: 8.93%, 21.87%). The number of eyes with iridocorneal contacts after surgery was low (4.2%). Only the preoperative increased IOP is a significant risk factor for IOP elevation (p=0.005). Visual acuity improved significantly after surgery (p<0.001), and clear grafts were achieved in all eyes. Mean endothelial cell count did not differ between patients with and without IOP elevation. Conclusion: Incidence of IOP elevation and post-keratoplasty glaucoma after DMEK were low, but regular IOP measurements, especially in eyes with pre-existing glaucoma and bullous keratopathy are necessary. Steroid-induced IOP elevation was the most frequent reason, and could be treated effectivelyby tapering down steroid medication or changing the steroid drug. Development of peripheral anterior synechiae after DMEK occured rarely. Therefore, the risk for IOP elevation and especially post-DMEK glaucoma was reduced compared to PK and DSEK. The surgical correction of hyperopia and hyperopic astigmatism has always been a challenge for refractive surgeons. The efficacy, quality of vision and most im-portantly safety, provided by the existing laser surgical techniques are often not satis-factory. On the other hand, thermokeratoplasty procedures are minimally invasive, do not require corneal tissue removal, and operate outside the central clear zone of the cornea. The goal of conductive keratoplasty, which is a thermokeratoplasty technique, is to produce structural changes in the three-dimensional grating of collagen in the stroma of the cornea. These changes are achieved through the elevation of the tissue temperature. The increased tissue temperature is induced by electric impedance in the flow of energy through the collagen fibrils. Controlled-release radiofrequency energy (350 KHz) is delivered to the depth of 500 μm with the help of a Keratoplast tip ViewPoint™ CK (Refractec, Inc., Irvine, CA, USA). With the increase of the tissue temperature the collagen fibrils shrink by 30% of their original length. The application of the energy takes place in the periphery of the cornea where the increase of tempera-ture causes “corneal shrinkage” and flattening of the corneal surface, the fact that leads to a comparative increase of curvature in the central optically active part of the cornea. This new technology is currently used for the correction of low to moderate hyperopia and most recently presbyopia in patients of 45 years and older. In the course of the current study conductive keratoplasty was used to primarily treat hyper-opic astigmatism through the application of an original astigmatic nomogram. The current study reports on the results of conductive keratoplasty treatment of primary and secondary astigmatism trough the application of the original astigmatic nomogram, the safety profile of the new technique obtained trough human and animal histological studies, as well as the expansion of the existing nomogram for the spheri-cal corrections to higher degrees of hyperopia. The purpose of the current thesis is to study the shrinkage of the corneal tissue with the means of conductive keratoplasty in order to produce safe, predictable and effective treatment of low to moderate hyperopia as well as hyperopic astigmatism. In order to achieve this goal the technique was applied for the treatment of low to moderate hyperopia in individuals over 45 years of age. The safety of the treatment was evaluated through the histological studies of CK-treated human and rabbit cor-neas. The technique was applied for the treatment of higher levels of hyperopia due to the modification of the surgical technique as well as for the treatment of primary and secondary astigmatism.

▼ Descemet’s membrane endothelial keratoplasty (DMEK) is the current gold standard treatment for irreversible corneal oedema. After Melles developed this technique in 2006, Muraine proposed in 2013 an alternative technique for the dissection and implantation of the graft. Its main contributions were: hidrodissecting the graft from a partially trephined, inverted donor tissue, and folding the graft over the endothelial side, which favoured the protection of endothelial cells and the graft’s natural tendency to unfold in the receptor’s anterior chamber. The purpose of this doctoral thesis is to compare Muraine’s technique to the Standard through analysis of the postoperative endothelial cell density (ECD) and visual acuity (VA), surgical time, and intraoperative and postoperative complications. An observational, multicentric, prospective, cohorts trial was carried out in Hospital Universitari MútuaTerrassa and Institut de Microcirurgia Ocular in a daily praxis basis. There were follow-up controls over the six months following the surgery, at least at day one, first week and first, third and sixth months. Twenty-seven eyes from 20 patients were included in the Standard technique group. Forty-two eyes from 40 patients were included in the Muraine’s technique group. The ECD at six months was 1488 (1337-1679) cells/mm2 for the Standard group and 1170 (734-1614) cells/mm2 for Muraine’s group. The mean VA at six months was 0.89 for the Standard group and 0.79 for Muraine’s group, in the decimal scale (P=0.19). Around 80% of the eyes reached a VA of 0.5 or higher and 50-70%, 0.8 or higher. The ECD and the percentage of ECD loss with Muraine’s technique at the first month after surgery were equivalent to the Standard technique’s. The percentage of ECD loss at six months was higher with Muraine’s technique, although the ECD was clinically comparable. The VA achieved at six months was equivalent. Muraine’s technique was as safe as the Standard technique for the graft dissection. The incidence of intraoperative complications among the eyes with uncomplicated phacoemulsification was not statistically higher with Muraine’s technique. The graft dissection with Muraine’s technique was slower. Conversely, the unfolding was slightly faster. Both techniques had a high graft survival rate. The most frequent postoperative complication in both groups was cystoid macular oedema. The grafts dissected with Muraine’s technique had a higher incidence of need for rebubbling. ▼ Purpose: To evaluate the refractive and biomicroscopic changes in a series of patients in which a penetrating keratoplasty had been previously performed to treat advanced keratoconus. To demonstrate the recurrence of the disease in the long-term follow up. Hypotesis: Keratoconus is a multifactorial corneal disease in which genetical, biochemical and environmental factors have been implicated, the hypotesis of a possible long-term recurrence of the disease inducing refractive, slit lamp and histopathological findings is proposed. Foundations: The classical symptoms and signs of keratoconus, diagnostic examinations, etiopathogenic factors, historical evolution of the disease in the ophthalmological community, the medical and surgical therapies used throughout history and at present are presented. A detailed explanation of the technique of penetrating keratoplasty used in the analyzed series of patients is fully described, as well as its results and the methods to analyze the evolution of astigmatism during the long post-operative follow up period. Patient and methods: The refractive and keratometric changes thru time of corneal astigmatism, the anatomical slit lamp findings and histopathological findings in a series of 80 patients previously operated with penetrating keratoplasty for keratoconus are analyzed. The follow-up time was between 15 and 25 years after the surgical procedure. Results: Significant refractive changes in astigmatism were observed starting 10 years after the surgery in 70% of the patients, together with slit lamp and histopathological findings consistent with the diagnosis of recurrence of keratoconus in the studied recipient corneas. Conclusions: Keratoconus is a corneal disease that can recur in a significant percentage of cases in the recipient cornea and in the donor-host junction long after a penetrating keratoplasty. ▼ In the last ten years, posterior lamellar keratoplasty methods have become increasingly important. Lamellar methods currently used to treat corneal endothelial diseases are DSAEK and DMEK. The aim of this work is to investigate factors that influence the results of different posterior lamellar transplantation techniques. We demonstrate a significantly better postoperative visual acuity for DMEK than for penetrating keratoplasty. In addition, DMEK was associated with less induced astigmatism and fewer higher- order aberrations. This resulted in a higher patient satisfaction after DMEK in our patient survey. Compared to DSAEK, DMEK patients had a better uncorrected and best corrected visual acuity and higher contrast vision. The limited visual acuity after DSAEK could be related to a higher refractive interface and differences in graft thickness. We evaluated the influence of intra- and postoperatively measured graft thickness on functional results. We found that the postoperatively measured graft thickness correlated significantly with the resulting visual acuity. A postoperative graft thickness of  120 μm was associated with a better visual acuity without negatively influencing the postoperative endothelial cell count. In a prospective study, we could show that a temporal incision induces significantly less astigmatism and, in addition, causes fewer higher order aberrations. However, a temporal access seems to result in a higher rate of graft detachment. Despite the better postoperative results, DMEK is establishing itself only slowly. This is mainly due to the more sophisticated surgical technique. We could show that corneal donor characteristics have no influence on the difficulty of the DMEK procedure. A more difficult surgical procedure, however, meant an increase in endothelial cell loss and a higher rate of graft detachment. Postoperative graft detachment is the most common complication after DMEK surgery. In histological and immunohistochemical investigations we found that an incomplete removal of the recipient Descemet´s membrane, with separation of the anterior banded layer, may be a risk factor for graft detachment after DMEK surgery. ▼ This is a study on the prevalence of refractive errors and patients requiring refractive services at 15 eye clinics in the Amathole, Chris Hani, Joe Gqabi and OR Tambo District Municipalities of the Eastern Cape. This is an area characterised by extreme poverty where the cost of an eye examination and prescription spectacles remains financially unobtainable for most. Optometry services are provided mainly by private optometrists who service the small proportion of the population that can afford them. Adults and children remain house bound or are labelled as dull and unproductive simply because they don’t have access to an eye examination and a pair of spectacles. Purpose The purpose of the study is to identify patients with refractive errors and those requiring refractive services at the 15 eye clinics in the Eastern Cape. Another purpose is to describe the refractive services that are available to patients attending health facilities, where the eye clinics are conducted. Lastly, the purpose is to explore the possibility of nurses providing refractive services independently or under the supervision of optometrists to supplement the lack to refracting and dispensing services. Study Method A quantitative and qualitative non-experimental descriptive design was used. Research involved the analysis of Vision Care’s eye clinic records collected from 15 eye clinics from January 2010-June 2010. Semi-structured interviews were conducted with 30 nurses working at the 15 health facilities where the eye clinics were conducted using purposive sampling. The quantitative data was analysed using excel spreadsheets and graphs and qualitative data was analysed using coding and categorizing methods. Conclusion According to Vision Care’s data of the patients assessed, 19.2 percent had a refractive error and 54 percent of the patients required refractive services. It is estimated that 71.41 percent of the patients had a refractive error according to the optometrist. Although there were some organisations active in the eradication of cataracts, there was little healthcare available in the form of refraction services. xiv Patients needed to travel an average of 63.8kms to access refraction services against the backdrop of poor roads, poverty and unemployment. 28 out of 30 nurses either ‘strongly agreed’ or ‘agreed’ that nurses could be trained to perform refractions and dispense spectacles. 29 out of 30 nurses thought that this would have a positive impact on eye care. Further research is necessary to assess the feasibility of implementing a nurse operated refractive program and whether it should be within their scope of practice to refract and dispense spectacles. ▼ This work analysed new strategies for the prophylaxis of corneal graft rejection. In the first section clinical data of patients of our clinic were analysed. It could be demonstrated that glaucoma is a main risk factor for chronic endothelial cell loss. Moreover our data could show for the first time that a chronic subclinical immune reaction contributes to endothelial cell loss of homologous grafts. Broad experimental investigations on topical application of immunomodulatory substances are described in the second section. Topical formulations of mycophenolate mofetil (MMF) and sirolimus / everolimus are demonstrated for the first time. Our examinations showed that the best preparation of MMF is a cyclodextrin formulation (MMF-CD) whereas sirolimus / everolimus is best prepared as a microemulsion. Application of MMF-CD formulation can obtain high concentration in the aqueous humour whereas topical sirolimus permeates the cornea only after removal of the epithelium. Topical MMF and sirolimus do not prolong experimental corneal graft survival in a rat model whereas grafts survive longer under topical therapy with everolimus. Moreover, topical application of MMF and sirolimus / everolimus remain interesting therapeutic options for other immune mediated diseases of the ocular surface like immune mediated corneal ulcers or atopic keratoconjunctivitis. In the third section therapeutic options of gene therapeutic strategies for prophylaxis of corneal graft rejection were analysed. Own investigations on this topic showed that liposome mediated cytokine gene transfer (interleukin 10) to corneal endothelial cells in culture is possible. For transfection of organ cultured corneas transfection efficiency of adenoviral gene transfer is by far higher. Nevertheless on the way to clinical application of gene therapy for the prophylaxis of corneal graft rejection numerous further problems have to be solved. ▼ Mammalian corneal endothelial cells are considered to be non-proliferative due to the arrest of cells at the G1 phase of the cell cycle. The purpose of this study was to determine whether the down regulation of cyclin dependant kinase inhibitors (p21cip1 and p27kip1) levels by Endothelin-1 (ET-1), would overcome the G1 phase arrest and promote cell cycle progression and proliferation in cultured BCECs (Bovine corneal endothelial cells). BCECs were isolated from bovine corneas and cultured in DMEM supplemented with 10% serum. 5-Bromo 2-deoxy Uridine (BrdU) incorporation was determined in serum starved cultures in 24-well plates as a measure of cell proliferation. Confluent serum starved cells grown in T-25 flasks were treated with 100nM Endothelin-1 in DMEM. The control cells were left untreated in serum free medium. Total cellular protein was isolated using RIPA buffer and quantified according to the Peterson modification of the Lowry method. The level of expression of p21cip1 and p27kip1 proteins relative to β-actin was determined by western blotting technique. Immuno fluorescent localization of p27kip1 was performed using polyclonal anti-p27kip1 and anti-p21cip1 antibodies in confluent and growing cells. An increase in cell proliferation was observed in sub-confluent cultures with Endothelin-1 treatment. This evidence was supported by an increase (~18%) in BrdU incorporation in response to Endothelin-1. Densitometry analysis of immunoblots revealed an increase in the expression of p27kip1 in confluent cell cultures when compared to sub-confluent, dividing cells. p21cip1 was almost undetectable in sub-confluent, actively dividing cultures. Immuno fluorescent analysis revealed that the nuclear staining of p27kip1 was apparently decreased with ET-1 treatment. In conclusion, Endothelin-1 treatment resulted in decrease in p27kip1 and p21cip1 expression in confluent cultures that was greatest at 30 hr of post incubation with Endothelin-1. Endothelin-1 appears to promote cell proliferation. Expression of p27kip1 and p21cip1 was greatly reduced in actively dividing BCECs. Endothelin-1 treatment down-regulated these cyclin dependent kinase inhibitors and may promote cell cycle progression via this mechanism.
