▼ BACKGROUND Hydrotherapy plays an important role in the management of
surgical patients, and especially so in those that have sustained burns to their skin.
Different centers practice hydrotherapy differently. At the University Teaching
Hospital in Lusaka, Zambia, patients with burns in surgical wards use a common
bathtub for cleaning their wounds. This breaches patient isolation and increases risk
of cross-infection. Audit records from the department of surgery show that burn
wound colonization and infection is an important source of morbidity and mortality.
However, there is no evidence yet that the hydrotherapy as practiced at our institution
does lead to cross infection among patients with burns.
OBJECTIVE The general objective was to determine if the hydrotherapy practice
plays a role in cross-infection. We also wanted to characterize these organisms being
spread by cross-infection.
METHODS This was a prospective analytical study. Patients meeting the admission
criteria were recruited. Swabs from the burn wounds were collected on admission
(day 0), day 4 and day 7. Weekly swabs of the bathtub were also collected, after the
tub had been cleaned and declared ready for the next patient. Weekly water samples
were also collected. Selected results, for Staphylococcus aureus and Klebsiella
pneumoniae, were subjected to further analysis and PCR. Results were analyzed using
statistics software, SPSS version 23.
RESULTS In this study, there were 96 participants of which 51 (53.1%) were males
and 45 (46.9%) were females. Age distribution ranged from 5months old to 91 years
old. The modal age range was 1 to 2 years old. The modal burn percentage was 6% to
10%, followed by 11 to 15%. Hot water was the cause of burns in 65.6%.
Staphylococcus aureus and Klebsiella pneumoniae were the commonest organisms
isolated. These came from wounds that looked clinically clean. Others were enteric
organisms. In terms of readily available antibiotics, there was more sensitivity to
Amikacin and Chloramphenicol than Ciprofloxacin (our commonly used antibiotic).
The bathtub also had Staphylococcus aureus and Klebsiella pneumoniae, besides
enteric organisms. Sixty five point four percent (65.4%) of the Klebsiella were ESBL
v
producers. The tub had samples that were both ESBL producers as well as widely
resistant Klebsiella by other means. Of the ESBLs, 29.4% had the SHV gene, 23.5%
had the TEM gene and 47.1% had both SHV and TEM. There was no CTX gene
identified. MRSA accounted for 30.6% of all the Staphylococcus in this study. The
PVL gene was detected in 11.8%, SPA gene in 35.3%, while 5.9% of the
Staphylococcus had both PVL and SPA genes. No growth was obtained from the
water samples. Seventy-two point nine percent (72.9%) of the patients were
discharged, 19.8% died, while 7.3% left against medical advice.
CONCLUSION Hydrotherapy as currently practiced at the University Teaching
Hospital does contribute significantly to cross-infection among burns patients. The

▼ Problem: Poor balance is considered a major issue that children with Cerebral Palsy (CP) encounter. Since balance skills constitute a crucial part of gross motor ability, compromised balance control in these children with CP have a varied impact on their everyday life.
Hydrotherapy has been used in the rehabilitation of children with CP, but little is known about the effects of this rehabilitative modality on both static and dynamic balance of children with CP especially in Sri Lanka where no
hydrotherapy study has been conducted. Therefore, the purpose of this present study is to examine the feasibility of a 6-week aerobic exercise-based
hydrotherapy program and its effects on static and dynamic balance in children with CP in the Sri Lankan context.
Methods: A quasi-experimental pretest-post-test design without a control group was used. Five children with mild to moderate CP between the ages 7 and 11 years participated in 45-minute
hydrotherapy sessions twice per week for 6 weeks. Training intensity was monitored and expressed as a percentage of Heart Rate Reserve (HRR). The primary outcome measures were static and dynamic balance as measured by the Pediatric Reach Test (PRT) and modified Timed-Up and Go (mTUG) test, respectively. As the secondary outcome measure, Health-Related Quality of Life (HRQOL) of children was assessed by the primary caregiver using 5-item Visual Analogue Scale (VAS).
Results: All participants were able to achieve and maintain an intensity level of 30% to 60% of HRR with an average intensity level of 43% during the intervention period. Except forward reach distance when sitting, all other components of the PRT showed improvement with statistical significance at p < 0.004 with Bonferroni correction. The mTUG test did not show statistically significant improvement even though there was a positive trend towards improvement. All the participants showed improvement in HRQOL, but the findings were not statistically significant.
Conclusion: This 6-week aerobic exercise-based
hydrotherapy program was feasible and demonstrated potential for improving static and dynamic balance abilities in children with CP.

▼ The purpose of this study was to identify the effect of
hydrotherapy on length of
first stage of labor and analgesia use for the nulliparous woman in labor. A quasi-experimental
design was used for this quantitative study. The researcher and labor and
delivery nurses collected data throughout or after the labor experience of the convenience
sample who met the following criteria: nulliparous, 37 to 42 weeks gestation, in
established active labor, vertex presentation, singleton fetus, absence of major medical
problems, uncomplicated pregnancy, and not requiring continuous fetal monitoring. There
were forty-eight participants in the study. Thirty-three were in the experimental group and
fifteen were in the control group.
The women in the experimental group were those who chose to use
hydrotherapy
as a means of pain relief/relaxation during the first stage of labor. The control group
included those who chose not to use
hydrotherapy as an intervention for pain
relief/relaxation during the first stage of labor. Evaluation of cervical dilation was checked
periodically according to physician and agency protocols for both groups. Evaluation of
cervical dilation was also measured prior to and after use of
hydrotherapy for those in the control group. Length of labor and analgesia use: type, amount and route was
documented and statistically evaluated.
The data were compiled and statistical consultation was sought. The statistical
significance was set at p = <.05. Two x two factorial analysis of variance designs were
used to evaluate the difference in length of labor for
hydrotherapy and epidural groups in
labor and the difference in analgesia use for
hydrotherapy and epidural groups in labor.
There was a significant difference between length of labor for those who received an
epidural and for the interaction of epidural therapy and
hydrotherapy on length of labor.
The women using neither
hydrotherapy nor an epidural had the shortest labors, with the
women who used an epidural and no
hydrotherapy had the longest labors. There was a
significant difference noted between analgesia use and epidural therapy. The women who
selected epidural therapy had the most amount of analgesia used in labor.
This study had a small sample size. It would be beneficial to conduct further
research to provide further information on the use of
hydrotherapy in labor.

▼ A reabilitação do joelho utilizando exercícios terapêuticos em meio aquático tem sido
utilizada com o intuito de iniciar, de forma imediata os movimentos articulares, pois reduz
a dor e permite a carga de peso mais precoce. Apesar destas vantagens serem reconhecidas,
poucos estudos têm sido realizados no sentido de determinar a efectividade dos exercícios
terapêuticos em meio aquático na reabilitação pós-cirúrgica do ligamento cruzado anterior
(LCA) Objectivo: Descrever os efeitos de um programa de exercícios terapêuticos em
meio aquático num caso clínico de ligamentoplastia do cruzado anterior. Metodologia:
Um jogador de futebol de 27 anos de idade, sujeito a uma ligamentoplastia do LCA,
completou um programa de exercícios terapêuticos em meio aquático que constou de um
total de 24 sessões (4 por semana, e 60 minutos por sessão). O atleta foi avaliado em três
momentos distintos (T0, no início da intervenção, T1, três semanas após o início da
intervenção e T2, seis semanas após o início da intervenção. Como parâmetros de avaliação
foram utilizadas as amplitudes articulares (goniometria), o teste muscular manual, o
volume muscular (perimetria), o equilíbrio estável (Y Balance Test (YBT)) e a
funcionalidade (Knee Injury and Osteoarthritis Outcome Score (KOOS)). Resultados:
Após as 24 sessões verificou-se uma melhoria nos diversos parâmetros avaliados (flexão
do joelho: 75º vs 134º; extensão do joelho: -7º vs 0º; força dos flexores e extensores do
joelho: 3 vs 4+; perimetria da coxa: 46,2 cm vs 51,5 cm; YBT: equilíbrio anterior
(resultados correspondentes aos momentos T1 e T2): 53 cm vs 72 cm; posterolateral: 46
cm vs 61 cm; posteromedial: 39 cm vs 57 cm; KOOS: dor/sintomas: 17 vs 92, outros
sintomas: 25 vs 75, actividades da vida diária: 12 vs 90, actividades desportivas e de lazer:
0 vs 55, qualidade de vida relacionada com o joelho: 0 vs 50. Conclusão: O programa de
exercícios terapêuticos em meio aquático utilizado neste estudo parece ter efeitos benéficos
num caso clínico de ligamentoplastia do cruzado anterior.
Exercises in water could expedite knee rehabilitation because of the decreased stress on the
joint, allowing early weight-bearing and reduction of pain. In spite of its advantages, there
are few studies of the effectiveness of aquatic therapeutic exercises following anterior
cruciate ligament (ACL) reconstruction.
Aim: This study aims to describe the effects of an aquatic therapeutic exercises program
following ACL reconstruction (case report). Method: A 27 seven years old soccer player
had 24 sessions of aquatic therapeutic exercises (4 times a week, 60 minutes each session)

▼ In this article a lot of aspects of water will be discussed and the points who are important for
hydrotherapy are highlighted. Also I discussed a lot of articles in which there is a form of training in water. At last I did a small research about the outcome of patients with injury of the tendons of the lower extremities which have been in an aquatrainer in comparison with patients which have a standard protocol of walking. I have taken a group patients who have had an injury of the tendons of the lower extremities and have been in the aquatrainer from Horse Aquatherapy in Olst. This group did I have compared with a group of patients who have had an injury of the tendons of the lower extremities and have been in Utrecht at the Facultary Veterinary Medicine and had a rehabilitation in the form of a standard protocol of walking. Of the group which was at the Horse Aquatherapy in Olst 9 horses have been come to the level of sports which the horses had before the injury. 2 horses are sound enough to ride with, but they did not have come to the level of sport which the horse has before the injury. 1 horse did not become sound enough to ride with. The other group who have been to Utrecht at the Faculty Veterinary Medicine and had an upgoing protocol of walking 2 horses have been come to the level of sports which the horse has before the injury. 6 horses did not become to the level at which the horse was before the injury but are sound enough to ride with. 7 horses did not become sound enough to ride with. There are a lot of points of discussion the way my comparison is been done, but there is a lot of difference in the outcome of the two groups which are compared. Further research is necessary about the best way a horse with an injury of the tendons of the lower extremities can be treated.

▼ Introduction: Maternity care providers around the world offer warm water immersion for pain relief in labor and birth, but there are scant data regarding waterbirth availability, utilization, and outcomes in the United States. A 2014 joint committee opinion by the American Academy of Pediatrics and American College of Obstetricians and Gynecologists called for restricting waterbirth to experimental settings; the impact of this statement was explored in this survey.Methods: 7,374 members of the American College of Nurse-Midwives were invited to take an online survey about experiences with waterbirth, including barriers to and concerns about waterbirth, perceived colleague support for intrapartum immersion, and practice changes resulting from the 2014 AAP-ACOG committee opinion. Surveys were administered over 4 weeks, with weekly reminder emails to the membership; items varied according to respondents’ answers to questions about their current clinical practice.Results: 1,827 individuals accepted the study invitation and 1,565 met inclusion criteria, for a net response rate of 20.9%. Respondents reported high levels of exposure to waterbirth (81% have seen at least one) and low levels of concern about it (medians 1-2 on a 5-point Likert scale). They also reported moderate to significant barriers to waterbirth, mainly in lack of support from medical and administrative colleagues, and commonly linked to the AAP-ACOG statement. Midwives who currently attend waterbirths perceived lower barriers, higher colleague support, and lower concerns related to waterbirth than providers who do not currently attend waterbirths.Discussion: Survey results are consistent with the social amplification of risk framework, in that respondents’ perceptions of barriers to, support for, and concerns about waterbirth vary with their current practices. The AAP-ACOG statement reinforced some existing barriers to waterbirth, whereas day-to-day experience of clinicians currently providing waterbirth reinforces their perception of it as a reasonable practice. Respondents would like ACNM to engage actively in waterbirth research, education, and development of practice guidelines, generating an authoritative source of pro-hydrotherapy knowledge.

▼ Aims: To explore and inform current water immersion recovery practice of high performance athletes; and to compare recovery interventions of 5 minutes cold water immersion, warm water immersion and passive rest, in trained subjects, following intense exercise replicating the demands of game-sports. Methods: Study 1: In a repeated measures design, a measurement approach for use in the evaluation of water immersion efficacy was piloted. The within-day and between-day reliability of surface electromyelography (sEMG), particularly functional wavelet analysis, was evaluated in human lower limb muscles. Functional wavelet analysis provides the opportunity to measure neuromuscular function at the greatest level of detail by differentiating the relative intensity of low and high frequency motor unit recruitment. On 2 consecutive days (Trial 1 & Trial 2), 12 participants performed 3x5 second isometric 80% maximal voluntary contractions (MVC) on a Biodex® dynamometer in each of 150 ankle plantarflexion, 200 knee extension and 200 knee flexion. sEMG was obtained from the medial gastrocnemius (MG), vastis medialis (VM), vastis lateralis (VL) and biceps femoris (BF) muscles. Joint position and force production were controlled. Electrodes remained in situ during each trial. Electrodes were removed upon completion of Trial 1 and replaced in the same position the next day for Trial 2. Simultaneous sEMG metrics for intervals of consistent force production were compared between contractions in Trial 1 and Trial 2 (between-day) and contractions within Trial 2 (within-day). Study 2: 11 trained participants completed the 90 minute Loughborough Intermittent Shuttle Test (LIST). Five minutes of COLD water immersion (8.8 ± 0.30C), WARM water immersion (35.1 ± 1.80C) and REST were compared in a repeated measures randomised cross over design. Recovery was evaluated at 2, 4 and 24 hours post exercise using circulating markers of muscle damage, muscle dynamometry, drop jump and repeated single leg hop performance tests and perceived recovery. Study 3: Current water immersion practice of high performance athletes, practice implications stemming from this study’s findings, and the rationale were explored. In a purposive, theoretical sampling approach of expert consultation, 8 professionals advising internationally competing athletes on water immersion recovery practice were provided with a research brief of this project in advance of a scribed, semi-structured interview. Participants were of Sports Coach, Strength & Conditioning Coach and Sports Physiotherapist professions with a minimum of 5 years’ experience working with internationally competing athletes; and differed in international location and sporting disciplines.

▼ The purpose of this study was to evaluate the relative effectiveness of two methods of administration of the cooling bath for reducing fever due to infection. Fifteen infants and children age six months to three years with rectal temperatures of 101 degrees Fahrenheit or over were included in the study.
The experimental method with a retroactive comparison of a parallel group was the research approach used. For a previous study by Kueffner, seven subjects had been immersed in tubs of water 95 degrees Fahrenheit which was gradually reduced to 75 degrees Fahrenheit, each
subject remaining in the tub a total of twenty minutes. These subjects were the Kueffner Group for this study. The eight subjects in the Experimental Group were immersed in tubs of water 100 degrees Fahrenheit which was gradually reduced to 95 degrees Fahrenheit, each
subject remaining in the tub for a total of fifteen minutes.
Immediately following each procedure, each
subject was gently dried with a cotton bath blanket. A rectal temperature was taken immediately before the administration of each cooling bath and at fifteen, thirty, forty-five, and sixty minute intervals following the procedure. The relative effectiveness of the method of administration of the cooling bath was evaluated by the measured amount of drop in rectal temperature following the bath.
Analyses were made of the diagnosis, age, body surface area, and initial temperature of the subjects. An analysis of the mean drop in temperature indicated that the mean temperature of subjects given the cool immersion bath was lower for all time intervals than was the mean temperature of subjects given the warm immersion bath. The mean temperature drop for subjects in the Kueffner Group who were given the cool immersion bath was a maximum 3.2 degrees thirty minutes following the treatment and was 1.94 degrees Fahrenheit sixty minutes following the treatment. The mean temperature drop for subjects in the Experimental Group who were given the warm immersion bath showed a gradual decline following the treatment and was 1.22 degrees Fahrenheit at the end of one hour.
Because of the small number of subjects it was impossible to draw unqualified conclusions, but it would appear that immersion in a tub of water 95 degrees Fahrenheit which was gradually reduced to 75 degrees was more effective than the warm immersion bath as used in this study for causing a drop in rectal temperatures due to infection in children.
While no criteria were set up to evaluate the reaction of subjects to each treatment, the subjects in the Kueffner Group cried vigorously during the cool bath while the subjects in the Experimental Group were able to relax and enjoy the warm bath. Following the warm bath three of the subjects went to sleep which was in contrast to their generally restless and irritable state before the bath.

Com o aumento da ocorrência de doenças articulares em cães tem-se vindo a recorrer
cada vez mais a fisioterapia como forma de tratamento e prevenção destes casos.
Embora esta não seja uma disciplina normalmente integrada no plano de recuperação
dos animais, os seus benefícios podem ser muito relevantes. Com exercícios de
diferentes exigências, desde caminhadas em ambientes controlados e exercícios de
obstáculos a sessões de massagem, alongamentos e hidroterapia, é possível realizar
um protocolo de reabilitação física adequado às capacidades dos animais.
Paralelamente, também é criado um ambiente de trabalho conjunto com o proprietário
do animal, que se sente como parte integrante do protocolo de reabilitação.
Na presente dissertação são apresentados seis casos clínicos de doenças articulares,
com sede no membro torácico, tendo cada animal beneficiado de um protocolo de
reabilitação individual e específico. Simultaneamente, a evolução dos sinais clínicos é
apresentada e discutida individualmente.
Embora todas as modalidades da fisioterapia sejam importantes e tenham objetivos
diferentes, a hidroterapia mostrou ser uma das técnicas de reabilitação física com maior
potencial na recuperação de cães com lesões articulares no membro torácico, por
oferecer uma panóplia de variáveis que permitem encontrar as condições ideais à
recuperação de um número elevado de animais.
ABSTRACT - Physical rehabilitation of dogs with forelimb joint diseases - With the increase of joint diseases in dogs the demand for physiotherapy both as
treatment and prevention of such cases has been increasing. Despite not usually being
included in the recovery plan, its benefits may be utterly relevant. The use of exercises
with different demands, from walks in controlled environments and obstacle courses to
massage sessions, stretches and hydrotherapy, makes it possible to adjust the physical
rehabilitation protocol to the animal´s abilities. Concurrently, it is also possible to create
a work environment with the animal’s owner, who feels included as part of the
rehabilitation protocol.
In this study 6 clinical cases of joint disease, exclusively of the forelimb are presented.
Each animal having benefited from an individual and specific rehabilitation protocol.
Simultaneously, the progression of clinical signs is presented and individually discussed.
Although all physiotherapy modalities are important and inspire different goals,
hydrotherapy was shown to be the rehabilitation technic with the biggest potential in the
recovery of joint diseases of the forelimb in dogs, since it offers a display of possibilities
which allow to find the ideal conditions to recover a large number of animals.

▼ A Fibromialgia (FM) é uma síndrome reumática crônica, caracterizada por dor músculo esquelética difusa, onde os exercícios aeróbicos representam uma parcela fundamental na sua abordagem terapêutica. O objetivo deste estudo foi avaliar a eficácia de exercícios aeróbicos realizados na água do mar (talassoterapia) para mulheres com FM e comparar com exercícios realizados na piscina, envolvendo uma equipe multidisciplinar, composta por reumatologistas, fisioterapeutas e estudantes de fisioterapia e educação física. Quarenta e seis (46) mulheres com idade entre 18 e 60 anos com FM foram randomizadas em 2 grupos: grupo da piscina (23 pacientes) e grupo do mar (23 pacientes). Ambos os grupos foram treinados com o mesmo programa de condicionamento aeróbico: três vezes por semana, durante 60 minutos, por 12 semanas. Todas as pacientes foram avaliadas, antes e imediatamente após o tratamento, pela Escala Visual Analógica (EV A) para dor e fadiga, contagem do número de tender points, Fibromyalgia Impact Questionnaire (FIQ), Short Form 36 Health Survey (SF-36), Pittsburgh Sleep Quality Index (PSQI) e Beck Oepression Inventory (BDI). Na análise estatística, foram utilizados o teste t pareado para análise intragrupo e o teste t não-pareado para análise intrergrupos, sendo considerado um nível de significância p <0,05. Quatro pacientes de cada grupo não completaram o programa de treinamento. Os grupos foram homogêneos e comparáveis na avaliação inicial, com exceção do BOI (p <0,05). Ambos os grupos apresentaram melhora estatisticamente significante em todos os parâmetros avaliados no pós-tratamento, comparado com a avaliação inicial, com redução da intensidade da dor e fadiga, do número de tend,er points, melhora da capacidade funcional (FIQ), qualidade de vida (SF-36), qualidade do sono (PSQI) e dos índices e depressão (BOI). Entretanto, na comparação entre os grupos, o grupo do mar (talassoterapia) apresentou melhores resultados em todos os parâmetros, porém com diferença estatisticamente significante apenas nos índices de depressão (BOI). Ao final, observamos que a realização de exercícios aeróbicos na água do mar ou da piscina se mostrou efetiva como parte do tratamento de pacientes com FM. Entretanto, o programa de exercícios associado à talassoterapia parece trazer mais benefícios, principalmente relacionados a aspectos emocionais, podendo ser uma opção terapêutica de baixo custo para pacientes com FM em nossa região

▼ Fibromyalgia (FM) is a chronic rheumatic syndrome characterized by diffuse muscle-skeletal pain, and aerobic exercises represent a fundamental portion in therapeutic approach. Objective of this study was to evaluate the effectiveness of aerobic exercises accomplished in the water of the sea (thalassotherapy) for women with FM and to compare with exercises accomplished in the swimming pool, involving a multidisciplinary team, composed by rheumatologists, physical therapists, students of physical therapy and students of physical education. Forty six (46) women with age between 18 and 60 years with FM were randomized in 2 groups: a swimming pool group (23 patients) and a sea group (23 patients). 80th groups trained a week with the same program of aerobic conditioning 3 times (60 minutes each) for 12 weeks. Ali the patients were evaluated, before and immediately after treatment, with Visual Analogical Scale (VAS) for pain and fatigue, number of tending points, Fibromyalgia Impact Ouestionnaire (FIO), Short Form 36 Health Survey (SF-36), Pittsburgh Sleep Ouality Index (PSOI) and Beck Oepression Inventory (BOI). For statistical analysis, it was used paired-t test for analysis intra-group and non-paired test for inter-groups analysis, significance levei of p <0,05. Four patients, of each group, didn't complete the training programo Groups were homogeneous and they were compared in initial evaluation, except for BOI (p <0,05). Both groups presented statistically significant improvement for ali appraised parameters in the post-treatment compared with initial evaluation, there were reduction of intensity of pain and fatigue, number of tending points, better functional capacity (FIO), life quality (SF-36), quality of sleep (PSQI) and depression indexes (BOI). However, in comparison among the groups, group of sea (thalassotherapy) presented better results for ali parameters, however with statistically significant difference just only for depression indexes (BOI). At the end, it was observed that accomplishment of aerobic exercises in sea water or swimming pool was effective as part of treatment for patients with FM. However, exercise programs with thalassotherapy seems to bring more benefits, mainly related to emotional aspects, could be a therapeutic option of low cost for patients with FM in our area

▼ Background: Increased physical activity has a modest effect on the spatial distribution of bone mineral. The risks and benefits of exercise for women with osteoporosis at high risk to fracture are not known. The decompression of aquatic exercise is an option to increase exercise compliance. The dual energy absorptiometry (DXA) vertebral fracture assessment (VFA) images 2D bone geometry however measurement properties have not been established for this outcome. Purpose: To determine relative and absolute intra-rater reliability of DXA-based vertebral height (VH) measures and to assess the feasibility of investigating bone outcomes in women with established osteoporosis before and after a 6 month aquatic exercise program. Methods: DXA VFA scans of 32 women over 60 years old were analyzed on 2 occasions, 4 weeks apart. Relative reliability, intraclass correlation coefficient (95% CI) and absolute reliability, standard error of measurement (95% CI) were estimated for visible anterior, middle and posterior VHs from T4 to L4. Women with osteoporosis at high risk to fracture (clinical practice guidelines used) were recruited from two osteoporosis clinics. Feasibility of recruitment, adherence to the exercise, adherence to the assessment protocols (DXA, pQCT, physical performance measures), safety and retention were assessed. Results: Intra-rater reliability ICC >0.87 (0.74, 0.94) and SEM <1.17 from T10 to L4. Recruitment of 9 women in 10 weeks; average adherence 68%; pQCT data loss 46%; 1 fracture after class; retention 89% at 12 months. Conclusion: Further study of measurement properties of VH and protocols for recruitment, data management and safety are required.

É constituído por três secções: casuística, monografia e casos clínicos. Na casuística são expostos de forma breve os casos clínicos e procedimentos acompanhados durante o período de estágio. Já a monografia, intitulada “Fisioterapia no paciente neurológico” faz uma descrição das principais afeções neurológicas que necessitam reabilitação no paciente canídeo e descreve as modalidades e métodos aplicados, fazendo uma correlação entre a neuroanatomia e as técnicas de reabilitação. Finalmente, nos casos clínicos são apresentadas de uma forma prática diferentes abordagens adotadas nos casos neurológicos em reabilitação durante o período de estágio; ABSTRACT: PHYSIOTHERAPY IN THE NEUROLOGIC PATIENT
This report this based on the internship carried out at Hospital Veterinário da Arrábida (HVA), under orientation of the clinical director Dr. Ângela Martins.
It is divided in three sections: casuistry, monography and case reports. The casuistry section briefly states the clinical cases and proceedings followed during the internship period. On the other hand, the monography section entitled “Physical medicine and rehabilitation in the neurologic patient” describes the main neurological diseases that require rehabilitation in dogs and the modalities and methods apllied, relating neuroanatomy and rehabilitation techiniques. Lastly, in the case reports section, different approaches for the neurologic cases in rehabilitation during the internship period are exposed in a practical way.

▼ L'étude du paysage balnéaire de Rome dans l'Antiquité (Ier siècle av. J.-C.- Ve siècle ap. J.-C.) a pour but de combler certaines lacunes concernant la recherche sur les bains dits mineurs dans la capitale de l'Empire. La thèse a suivi essentiellement cinq grands axes : nous avons au préalable défini un cadre historique et social à notre étude en insistant sur les problèmes relatifs à l'origine du bain romain et à sa diffusion en péninsule italique. Nous nous sommes ensuite penchés sur les problèmes méthodologiques rencontrés lors de l'établissement de notre corpus. Afin d'appréhender de manière concrète le paysage balnéaire romain, nous avons en effet établi une base de données dont chaque entrée correspond à un complexe balnéaire connu par les sources (archéologiques, épigraphiques et littéraires). Ce catalogue a été abordé sous forme descriptive dans notre troisième grande partie. Notre quatrième axe concerne particulièrement la législation relative à ces balnea, la définition de leur statut et l'identification de leurs propriétaires. Enfin, une étude typologique et technique des bains de Rome s'est révélée nécessaire pour terminer cette étude de façon exhaustive.
The study of the baths landscape of Rome in the Antiquity(Ist century B.C.- Vth century A.D.) aims at filling certain gaps concerning the research on the minor baths in the capital of the Empire. The thesis followed essentially five main chapters: we beforehand defined a historic and social frame in our study by insisting on the relative problems at the origin of the Roman bath and in its distribution in italic peninsula. We then bent over the methodological problems met during the establishment of our corpus. To conceive in a concrete way the Roman bath landscape, we indeed established a database every evidence of which corresponds to a bath complex known by sources(archaeological, epigraphic and literary). This catalog was approached under descriptive shape on our third chapter. Our fourth axis concerns particularly the legislation relative to these balnea, the definition of their status and the identification of their owners. Finally, a typological and technical study of the baths of Rome showed itself necessary to end this study in a exhaustive way.

Hydrotherapy according to Kneipp is recommended to relieve
symptoms in the treatment of polyneuropathy. The cold stimulus could influence
the functioning of neuronal nets, and stimulation of blood flow could improve
neuronal functions. To date, efficacy of serially applied
hydrotherapy in the
case of polyneuropathy, has only been examined in a preliminary pilot study,
the results of which led to this randomized interventional study. Material and
method: 64 patients (23 women, 41 men, 63 ± 7,7y, mean age ± 1 standard
deviation) suffering from slight to medium intensity cases of polyneuro-pathy
were included. Participants were randomly assigned to immediate treatment or a
waiting list group. Symptomatic effects of home-based self treatment with
hydrotherapy according to Kneipp (local cold stimulus) over a period of four
weeks were compared to the waiting group. Both groups were followed-up during
an observational period of eight weeks. Primary end points were analyzed with
validated questionnaires assessing patients’ complaints. To compare the
therapeutic effects with and without Kneipp therapy, mean intraindividual
differences of the sum scores of complaints after 4 weeks of therapy were
compared with corresponding differences of the waiting group. Results:
Comparison of mean intra-individual differences between both groups' sum
scores of complaints did not result in any significant group differences of
immediate treatment vs. the waiting list. Secondary and explorative analysis
suggested a possibly delayed efficacy of Kneipp therapy. After eight weeks of
treatment, both groups showed a significant relative decrease of the
polyneuropathic examination score. Furthermore, there was a decrease over
eight weeks in the sum score of complaints for both groups. In the within
group analysis the decrease was only significant. Conclusion: Within a period
of four weeks of self-care treatment with Kneipp
hydrotherapy did not show any
significant benefits compared to the waiting group. This clinical study does
not provide obvious evidence for the symptomatic efficacy of outpatient
hydrotherapy according to Kneipp in the treatment of polyneuropathic
complaints. However, Kneipp self-care is easily applicable and was well
accepted by the patients. Further studies with larger populations, should
evaluate, whether a longer application period could induce more pronounced
effects.

▼ Achieving a balance between training and competition stresses and recovery is important in maximizing the performance of athletes. Specifically, following contact sport where the degree of muscular damage and trauma caused by numerous collisions is substantial, the rate and quality of recovery is crucial for each player to optimally recover prior to the first training session of the training week following a game. The physiological response of specific recovery modalities and their effect on performance have been commonly investigated following exercise. However the investigation of recovery modalities following contact sport and their effect on important recovery markers leading into the first training session following a game is limited. The use of active recovery and various
hydrotherapy procedures are common modalities that are implemented into a recovery regime following a game and are thought to enhance the recovery process. The objective of this thesis was to analyze whether these recovery modalities provided beneficial recovery effects following professional rugby league matches. Literature surrounding all recovery strategies (excluding nutritional strategies) and its possible influence on physiological recovery following both delayed onset of muscle soreness (DOMS) inducing exercise protocols and team contact sport matches was then appraised and discussed. Following these reviews an experimental study was conducted to investigate the relative efficacy of post game recovery modalities on jump height performance, perceived ratings of muscle soreness, and muscle damage 1, 18, and 42 hours following professional rugby league competition games. Twenty-one professional rugby league players performed three different recovery modalities (cold water immersion (Bleakley & Dawson, 2010), contrast water therapy (Vaile, Halson, Gill, & Dawson, 2008) and active recovery (Abraham, 1977)) following three competition games at 1, 18, and 42 hours post game. The effects of the recovery treatments were analyzed with mixed modeling with a covariate included (fatigue score) to adjust for changes in the intensity of each match on the post-match values of dependent variables. Standardization of effects was used to make magnitude-based inferences, presented as mean; ±90% confidence intervals. CWI and CWT clearly improved jump height performance (CWI 2.3; ±3.7%, CWT 3.5%; ±4.1%), reduced muscle soreness (CWI -0.95; ±0.37, CWT -0.55; ±0.37), and decreased creatine kinase (CWI -11.0; ±15.1%, CWT 18.2; ±20.1% ) by 42 hours post game compared to ACT. CWT was however clearly more effective compared to CWI on the recovery of muscle soreness and creatine kinase by 42 hours post game. Therefore CWT recovery following team contact sport is recommended.

Introducción: El Trastorno del Espectro Autista (TEA) es considerado por
la Clasificación Internacional de Enfermedades (CIE 11) (1) y el Manual
Diagnóstico y Estadístico de los trastornos mentales (DSM V); y según
datos oficiales de la OMS 1 de cada 160 niños presenta TEA. La Terapia
Acuática (TA), es un método terapéutico que usa el agua como método
rehabilitador. Desde hace unos años este ha empezado a mostrar un
amplio potencial en el tratamiento de las personas con TEA, siendo su
evidencia científica actual prometedora pero insuficiente para poder
demostrar su eficacia.
Objetivos: Se pretende comprobar la efectividad de la terapia acuática en
el tratamiento de las personas con Trastorno del Espectro Autista (TEA), y
ver que repercusiones tiene este método de tratamiento sobre los aspectos
social, motor (movilidad funcional) y ocupacional de las personas con TEA.
Metodología: Se realizará un ensayo clínico aleatorizado-controlado, el
cual contará con una muestra total de 150 personas con TEA (75 Grupo
control, 75 Grupo intervención) pertenecientes a la Asociación ASPANAES.
Como herramienta de medida se empleará la escala Pedi-CAT, y la
intervención se basará en sesiones individualizadas en piscina en las que
se trabajará con el método Halliwick y la técnica del aprendizaje orientado
a tareas.

▼ Sammanfattning: Bassängträning är en vanlig träningsmetod som många patienter efterfrågar. Fördelar med att träna i varmt vatten är bland annat vattnets tyngdavlastande effekt samt värmens smärtlindrande och muskelavslappnande effekt. Efterfrågan på bassängträning är ibland större än tillgången, vilket kan innebära att sjukgymnasten behöver prioritera vilka patienter som ska få tillgång till bassäng. Tydliga och jämlika rutiner och riktlinjer vid bassängträning behöver finnas. I samband med arbete för att ta fram riktlinjer vid bassängträning är det också viktigt att ha kunskap om patienternas egna upplevelser och behov för att kunna förbättra rehabiliteringen. En enkätundersökning genomfördes med syfte att beskriva hur patienterna upplever bassängträningen, vilka mål de har och hur målet med träningen bestäms samt uppnås. Syftet var också att undersöka vilka faktorer patienterna anser som viktiga för bassängträningens kvalitet. Resultaten visar att de vanligaste målen vid bassängträning är ökad rörlighet och minskad smärta. Nästan alla patienter har varit delaktiga att sätta målen och en övervägande del av patienterna anser att de till stor del uppnår målen. Positiva effekter som upplevs med bassängträningen är fysiska förbättringar och psykiskt välbefinnande. Patienterna anser att det som är viktigast för bassängträningens kvalitet är att ha bra och utbildad personal samt att få gå kontinuerligt på bassängträning.

▼ Syfte: Att kartlägga förutsättningarna för gravida kvinnor att använda dusch och bad som smärtlindring på Sveriges förlossningsavdelningar. Metod: Deskriptiv tvärsnittsstudie genomfördes med telefonintervjuer utifrån ett frågeformulär. Deltagare: Alla 48 Sveriges förlossningsavdelningar deltog i studien. Respondenter var chef för förlossningsavdelning alternativt barnmorska som arbetar på förlossningsavdelning. Resultat: Förutsättningarna för att nyttja dusch och bad som smärtlindring varierar i Sverige. Många undantag finns för vem som får bada. Dusch och bad används tämligen frekvent. Information om denna smärtlindringsmetod ges mestadels muntligt. Detta har ändå inträffat senaste året. Riktlinjer för dusch och bad som smärtlindring finns på färre än hälften av förlossningsavdelningarna och enbart fem av dessa riktlinjer har referenser till forskning. Ingen förlossningsavdelning bedriver vård med vattenförlossningar men detta har ändå skett på tio förlossningsavdelningar senaste året. Ingen avdelning har riktlinjer för hur vattenförlossning ska handläggas. Slutsats: Möjligheterna till dusch och bad som smärtlindring är mycket olika i landet. Eftersom vården ska vara lika oavsett hemort bör detta ses över. Riktlinjer bör utarbetas för ett gemensamt arbetssätt baserad på aktuell forskning.
Objective: To chart the possibilities for hydrotherapy, i.e. bathing or showering, during labour in Swedish labour wards. Design: Descriptive, cross-sectional interview study involving short phone interviews based on an interview schedule. Participants: All 48 labour wards in Sweden participated. Head of labour ward or midwife working in labour ward was interviewed. Findings: The possibilities for hydrotherapy varied. Many women are for various reasons not allowed to bathe. Hydrotherapy is relatively commonly used in Sweden. Information about hydrotherapy is mostly given verbally. Less than half of the labour wards have guidelines for hydrotherapy, only five have references to current research. Water births are not allowed but have despite this occurred in ten labour wards during the last twelve months. None of the labour wards have guidelines for management of water birth. Key conclusions and implications for practice: The Swedish government has decided that all individuals should receive equal care. There is much to be done to nationally achieve equal possibilities for hydrotherapy during childbirth. Guidelines based on current research for handling hydrotherapy and water birth are needed.

▼ The purpose of this study was to investigate the impact of Halliwick Hydrotherapy on the gross mobility, balance, speed and daily skills of children with cerebral palsy as well as the translation and weighting of specific water assessment trials in the Greek population. The study involved 85 children, including 80 with GMFCS. The duration of the intervention was 9 months. All children were evaluated through the GMFM, PEDI, TUG, PBS assessment trials, while the water intervention team was further evaluated through the WOTA 1 & 2, SWIM, HAAR special tests. A significant improvement was noted in the final evaluations at the end of nine months' intervention. In gross mobility, the water intervention group showed an improvement from an average of 56.64 (SD: 23.21) to 62.74 (SD: 24.54), while the land intervention group averaged 58.64 (SD: 22.76), at 61.42 (SD: 22.68). In the daily life skills, the water intervention team averaged 52.09 (SD: 20.14), at 59.17 (SD: 21.21), while the land intervention group averaged 52.43 (SD: 19.25) before intervention, at 55.20 (SD: 19.72) after intervention. In the balance, the water intervention team averaged 23.75 (SD: 16.30) before intervention, at 27.55 (SD: 17.18), while the land intervention group average 23.09 (SD: 15.00) before intervention, at 25.06 (SD: 15.19) At the speed, the water intervention team showed a decrease in TUG execution time in sec from an average of 16.25 (SD: 10.78) before intervention, at 14.96 (SD: 11.01), while the land intervention team averaged 17.85 (SD: 10.26), at 16.96 (SD: 10.69). In the adaptation to the aquatic environment, WOTA 1, 22.65 (SD: 6.68) to 32.10 (SD: 7.03), WOTA 2, 25.70 (SD: 10.68) to 44.60 (SD: 9.75), SWIM, 30.45 (SD: 7.86) to 41.12 (SD:5.94), and HAAR from 18.07 (SD: 3.34) to 23.40 (SD: 3.61). Water Assessment tools have been found to be highly reliable and appropriate to investigate changes over time. Inter-rater reliability (ICCs), WOTA 1: 0.997, WOTA 2: 0.999, SWIM: 0.989, HAAR: 0.948, while test-retest reliability (ICCs), WOTA 1: 0.989, WOTA 2: 0.997, SWIM: 0.981, HAAR: 0.951 and validity WOTA1, rP = 0.63, WOTA2, rP = 0.58, SWIM, rP = 0.59, GMFM and HAAR, rP = 0.54, (p <0.05).
Σκοπός αυτής της μελέτης ήταν η διερεύνηση της επίδρασης της υδροθεραπείας Halliwick στην αδρή κινητικότητα, την ισορροπία, την ταχύτητα και τις καθημερινές δεξιότητες των παιδιών με εγκεφαλική παράλυση, καθώς και η μετάφραση και η στάθμιση ειδικών δοκιμασιών αξιολόγησης στο νερό στον ελληνικό πληθυσμό. Στη μελέτη συμμετείχαν 85 παιδιά, εκ των οποίων 80 με εγκεφαλική παράλυση διαφόρων επιπέδων GMFCS. Η διάρκεια της παρέμβασης ήταν 9 μήνες. ▼ Introdução: Tem se afirmado que não há a possibilidade de haver melhora da estabilidade dinâmica postural utilizando exercícios na água, isto é, onde a gravidade apresenta-se diminuída. Neste estudo randomizado e controlado avaliamos e comparamos a estabilidade dinâmica postural em indivíduos saudáveis que realizaram exercícios sensório-motor no solo ou na água. Métodos: Através do Biodex Balance System®, foram avaliados os limites de estabilidade postural, antes e após um programa de exercícios, de 60 indivíduos saudáveis do sexo masculino divididos em 3 grupos (solo, piscina e controle). Os indivíduos dos grupos solo e piscina realizaram um treinamento sensório-motor por 2 meses no solo e na água respectivamente; o grupo controle não realizou nenhum tipo de exercício. Resultados: 1) Foram encontradas diferenças significativas na estabilidade dinâmica entre o grupo solo e controle; 2) Foram encontradas diferenças significativas na estabilidade dinâmica entreo grupo piscina e controle; 3) Não foram encontradas diferenças significativas entre o grupo solo e piscina. Conclusões: realizar exercícios sensório-motor melhora a estabilidade postural em indivíduos saudáveis do sexo masculino, sem diferenças significativas entre os ambientes de treino (solo e água) comparados
Introduction: It has been stated that there is no way to improve dynamic postural stability using exercises in water, i.e. where there is reduced gravity. In this controlled, randomized study, we evaluate and compare postural dynamic stability in healthy individuals who performed sensory-motor exercises on the ground or in water. Methods: Through the Biodex Balance System®, the limits of postural stability were evaluated before and after a program of exercises, in 60 healthy males, divided into three groups (ground, swimming pool and control). The individuals in the ground and swimming pool groups carried out sensorial-motor training for two months, on the ground and in the water, respectively; the control group did not perform any kind of exercises. Results: 1) Significant differences were found in dynamic stability between the ground and control groups; 2) Significant differences were found in dynamic stability between the swimming pool and control groups; 3) No significant differences were found between the ground and swimming pool groups. Conclusions: performing sensory-motor exercises improves postural stability in healthy males, without significant differences between the training environments (ground and water) compared in this study

▼ A lombalgia é um dos mais comuns e desafiadores problemas de cuidados primários de saúde com alta prevalência e incidência. Apresenta etiologia multifatorial e as principais manifestações clínicas são: dor e alterações biomecânicas. Promove alterações na realização de atividades de vida diária e prática, com conseqüente repercussão sobre a qualidade de vida. No que concerne o tratamento fisioterapêutico, deve-se considerar que os efeitos fisiológicos ocasionados pela imersão em meio líquido associados à realização de exercícios específicos assim como a possibilidade de realização do tratamento em grupo justificam a abordagem ao paciente com lombalgia crônica. Objetivos: avaliar a eficácia de uma proposta de exercícios aquáticos, realizados em grupo, na qualidade de vida e na dor de pessoas com dor lombar crônica. Método: participaram deste estudo 28 indivíduos, com média de idade de 58,4 anos, que concordaram em receber intervenção em piscina por meio de exercícios aquáticos padronizados, realizados em grupo executados 3 vezes por semana por um período de 6 meses. Foi realizada a avaliação da dor por meio da escala numérica de dor no início e final de cada intervenção, e da qualidade de vida pela aplicação do questionário SF-36 e Roland-Morris (RM) em 3 momentos (início das intervenções, após 3 meses e após 6 meses). Resultados: o teste t-pareado revelou redução significativa da dor (p<0,000045) do início da terapia para o final da terapia, entretanto não foi observada mudança significativa no seguimento longitudinal desta variável. Em relação à qualidade de vida o teste de Wilcoxon evidenciou melhora significativa para os domínios dor (p<0,028), aspectos sociais (p<0,046) e saúde mental (p<0,002). O teste de correlação de Pearson evidenciou correlação moderada negativa entre os instrumentos SF-36 e RM. Conclusão: os exercícios aquáticos propostos neste estudo foram eficazes no sentido de proporcionar uma diminuição dos níveis de dor, assim como de alguns domínios da avaliação da qualidade de vida. Os instrumentos SF-36 (de caráter genérico) e RM (de caráter específico) mostraram-se sensíveis a avaliação da qualidade de vida em portadores de lombalgia. Dada a sua eficácia os exercícios realizados no presente estudo podem ser sugeridos como um protocolo a ser normatizado para esta população.
Low back pain is one of the most common and challenging problems in primary health care with high prevalence and incidence. ▼ Até a década de 70, acreditava-se que os recém-nascidos não sentiam dor e nem tinham memória dolorosa. Nas últimas duas décadas, esforços vêm sendo realizados no que tange à introdução de conceitos relacionados à humanização e aos cuidados centrados no desenvolvimento neuropsicomotor. À atenção especial a dor e às técnicas que visem diminuí-las pode ser o início para a melhoria da qualidade de vida dos recém-nascidos internados em unidades neonatais. A hidroterapia tem sido utilizada como ferramenta terapêutica há milhares de anos. Ela tem como objetivos promover o relaxamento, melhorar a circulação, restaurar a mobilidade, alongar os músculos, reabilitar, melhorar a coordenação e função motora, além de promover recreação. Este estudo quase experimental teve como objetivo avaliar a influência da hidroterapia em recém-nascidos hospitalizados. Os dados foram coletados em um período de quatro meses consecutivos, por meio de instrumento que abordou os aspectos relacionados ao nascimento, à internação e à frequência cardíaca, frequência respiratória, saturação de oxigênio, cortisol salivar e escala de dor Neonatal Infant Pain Scale. A população foi composta de todos os recém-nascidos que preenchiam os critérios de inclusão, totalizando dez recémnascidos. Posteriormente, foram submetidos à análise estatística; utilizou-se medidas de tendência central e variabilidade para variáveis quantitativas; na análise inferencial para as medidas de frequência cardíaca, frequência respiratória e saturação de oxigênio, incluiu o teste T-pareado para dois grupos e análise de variância com medidas repetidas para três ou mais grupos. Já na análise inferencial da escala de dor Neonatal Infant Pain Scale e para o cortisol, utilizou-se o teste dos sinais e ANOVA-Friedman para três grupos ou mais. Para múltiplas comparações utilizou o teste de Dunn. A média da saturação dos recém-nascidos antes da hidroterapia era de 94,3% e após a hidroterapia subiu para 97,5%, a frequência cardíaca variou de 144,8 batimentos por minuto para 129,7 batimentos por minuto, a frequência respiratória teve variações de 46,8 incursões respiratórias por minuto para 38,9 incursões respiratórias por minuto, e os valores da escala de dor se mantiveram abaixo de três após a hidroterapia, indicando que logo depois do procedimento, os recém-nascidos não sentiam dor e estavam mais relaxados. Os dados evidenciaram que os níveis de cortisol salivar para a avaliação sem a hidroterapia, aumentaram em média, de 2,9 nmol/L às 7h30min para 4,4 nmol/L às 8 horas. Após os recém-nascidos serem submetidos à hidroterapia, os níveis de cortisol salivar tenderam a diminuir de 4,5 nmol/L para 3,4 nmol/L. Conclui-se que a hidroterapia pode ser sugerida como um método seguro no tratamento da dor em recém-nascidos, e pode ser incorporada como tratamento rotineiro para minimizar a dor aguda em unidades de terapia intensiva e semi-intensiva neonatais.

▼ Os exercícios em ambiente aquático e principalmente a marcha, têm sido amplamente utilizados em programas de reabilitação. Por este motivo, as características biomecânicas como os ângulos articulares, momentos, força de reação do solo e eletromiografia têm sido investigados atualmente. Apesar disso, pouco se sabe sobre as estratégias de controle motor empreendidas pelo sistema nervoso central (SNC) durante a marcha em ambiente aquático (AA). Sabe-se que a trajetória do Centro de Massa (CM) durante a marcha está diretamente relacionada à postura e ao equilíbrio. Sendo assim, estudar a trajetória do CM forneceria importantes informações sobre como o SNC adota estratégias para controlar a estabilidade dinâmica durante a marcha. Portanto, o objetivo deste estudo é investigar a trajetória do CM em ambiente aquático (AA) e compará-la com aquela em ambiente terrestre (AT). Para coleta de dados, 19 sujeitos foram filmados, caminhando em velocidade confortável em AT e AA (com a água na altura processo xifóide). Para a obtenção desses dados marcadores foram posicionados em pontos anatômicos, suas coordenadas foram recuperadas e a trajetória do CM foi calculada através do método de análise segmentar. Além de trajetória do CM, características como a excursão vertical, velocidade e distância horizontal entre o CM e o ponto de apoio foram calculados. Durante a fase de apoio, esta distância permite inferir a estabilidade do corpo naquele meio. Diante dos resultados, foi possível constatar diferenças na trajetória do CM em AA e AT, isto pode indicar que a troca de energia potencial em cinética é diferente em AA. Outro achado é que o CM está mais afastado do ponto de apoio em AA do que em AT, o que remeteria a uma situação de instabilidade. Porém, a manutenção desta postura, sem que haja uma queda demonstra que o arrasto e a força de empuxo atuam auxiliando na estabilização do corpo em AA.
Water based exercises have been largely adopted in rehabilitation programs. Among them, walking in water is one of the most popular. For this reason, biomechanical characterization of walking in water have been done by investigating joint angles, joint moments, ground reaction forces and electromyographic activation patterns. Despite of this, very few is known about motor control strategies employed by the central nervous system (CNS) during walking in water. It is well known that control of body center of mass (BCOM) during gait is strongly related to posture and balance. In this sense, examination of the BCOM displacement during gait provides information about dynamic stability control and CNS strategies to achieve it. Therefore, the aim of this work was to investigate the behavior of BCOM during walking in water, comparing its characteristics with those on dry land. Numerous amounts ofliterature has confirmed the positive correlation that exists between exercise and psychological well-being. With the increased interest in the associated psychological factors ofpain, the present study was undertaken to investigate the effects of chronic pain on rheumatoid arthritic and osteoarthritic patients, with the overall aim of reducing, and alleviating these factors. The psychological factors studied were depression, effects ofthe impact ofarthritis, for example, on the independent, physical and psychosocial aspects oftheir lives, as well as coping. It was hypothesized that by engaging in an aquatic exercise program there would be a reduction/alleviation ofpain, which in tum would demonstrate a reduction in the psychological components ofpain. Evidence was found to support the hypothesis, due to the fact that, after exposure to the aquatic therapy program, subjects demonstrated a reduction in pain followed by a reduction in depression, slight improvement in coping with their arthritis, and the impact of arthritis was partially alleviated. The need for a larger sample group, as well a longer period of investigation will be needed for future research.

atrativa e estimulante, diferente do habitual, o terrestre. Sendo assim, as
atividades aquáticas proporcionam às pessoas, vivências e experiências
diferenciadas das realizadas em terra, pois o meio líquido oferece alguma
sensualidade e bem-estar físico.
As atividades aquáticas são fundamentais para um desenvolvimento
harmonioso das qualidades físicas, psicológicas e sociais de todas as pessoas,
independentemente das suas capacidades e limitações. A partir do momento
em que o portador de necessidades especiais descobre as suas
potencialidades, apesar das suas limitações, descobrindo a capacidade de se
movimentar na água sem auxílio, inicia o prazer de desfrutar da água, o que
aumenta a sua autoestima, autoconfiança e consequentemente a sua
independência.
Assim, a oportunidade de movimento que as atividades aquáticas
propiciam às pessoas com hidrocefalia, é extremamente frutífera e benéfica no
seu desenvolvimento, na sua aprendizagem e na sua integração social.
Tendo em conta esta problemática, o presente trabalho pretende
verificar a influência da Hidroterapia na qualidade de vida do portador de
Hidrocefalia, nomeadamente no que se refere ao valor social, terapêutico e
recreativo. Para isso, o participante deste estudo (J.A.) é um rapaz de 12 anos
de idade, apresentando diagnóstico clínico como sendo portador de sequelas
de Hidrocefalia congénita, com hemiparésia direita, lesão sequelar cerebral
parietal à esquerda, angioma cavernoso da pele do centro interno do olho
esquerdo, deformidade em equino do pé direito e polidipsia compulsiva.
Os resultados obtidos através da avaliação da qualidade de vida
demonstraram que o J.A. evoluiu em seis dos oito domínios avaliados. Os dois
domínios que não apresentaram percentagem de melhoria do pré para o pós
programa foram os que apresentavam valores mais elevados mesmo antes da
aplicação do programa.
The power of Hydrotherapy is, consensually, believed to be an attractive
and stimulating technique, different from the usual, the terrestrial. Thus, the
aquatic activities provide people with differentiated experiences from those
made on land, because the liquid medium offers some sensuality and physical
well-being.
Aquatic activities are fundamental to the harmonious development of
physical, psychological and social qualities of all people, regardless of their
capabilities and limitations. From the moment that the bearer of special needs
discovers his/her potential, despite his/her limitations, discovering the ability to
move in the water without assistance, he/she starts the pleasure of enjoying the
water, which increases his/her self-esteem, self-confidence and, consequently,
his/her independence.
Thus, the opportunity to move in the water which aquatic activities
provide to people with hydrocephalus, is extremely fruitful and beneficial in their
development, in their learning and in their social integration.

▼ Partagé entre d'anciennes traditions et une pratique médicale moderne, le thermalisme contemporain s'est construit comme une culture particulière de la santé qui confronte, sans pour autant les opposer, ces deux champs de savoir. Un regard ethnographique, fondé sur plusieurs terrains effectués en France (Aix-les-Bains) et en Italie (Salsomaggiore), permet de préciser plusieurs particularités de la pratique thermale européenne. En premier, l'expérience du corps et de la maladie relève autant de l'expérience individuelle que de l'expérience sociale. Dans la pluralité et la diversité des recours médicaux disponibles, le thermalisme a aussi cette particularité de mettre en avant la volonté du curiste qui est devenu un acteur de la « mécanique » physique et psychique du traitement. Non seulement il interagit activement avec ses thérapeutes mais il contribue à la construction et à la diffusion de la mythologie et de l'imaginaire thermal. Un imaginaire qui est d'ailleurs véhiculé par la ville thermale. Cette-dernière n'est pas qu'un lieu de cure mais plutôt un théâtre ou se met en scène et se légitime une certaine vision de la santé. Enfin, un dernier élément participe à l'épanouissement de la pratique thermale, le soin lui-même qui se compose de l'efficacité de l'eau et de la qualité des prestations des soignants. Ces derniers ont développé, au fil de leurs pratiques un rapport particulier au corps fait de proximité et de distance. Ils jouent ainsi le rôle du « passeur » et mettent en contact le curiste et la source.
Shared between ancient traditions and modern health practice, contemporary hydrotherapy has become a peculiar health culture encompassing, without mutually excluding, both of these knowledges. This ethnographic work based on field studies in France (Aix-les-Bains) and in Italy (Salsomaggiore) shed light on several aspects of the use of hydrotherapy in Europe. First, the perception of one's own body and of the disease rises from both personal and social experiences. Among all available health practices, hydrotherapy is peculiar in giving a central role to its user, at the same time subject and object of both the physical and psychological aspects of the treatment. Not only he actively interacts with his therapists, but he plays a pivotal role in building and spreading the mythology and the imagination of thermal baths, which are in turn conveyed by the spa town. This becomes not just the geographical place where the therapy is held, but also and most importantly a stage to represent and legitimate a certain view of health. A final important aspect contributing to the flourishing of hydrotherapy is the treatment itself, the effectiveness of the therapeutic water, the quality of the services provided by the therapists who developed a particular relationship with illness. ▼ Objective Aim of this study was to evaluate changes of quality of life (QoL)
and complaints of patients with post-polio-syndrome (PPS) under a self-
administered cold water
hydrotherapy over a period of 8 weeks. Method This is
a randomised, controlled, clinical pilot-study in a waiting group design over
4 weeks with a follow-up of 8 weeks. Complaints and QoL were examined at the
time of inclusion into the study, at the end of the waiting period and after 4
and 8 weeks of
hydrotherapy. After completion of the individual study period
patients evaluated tolerability and practicability of the different types of
cold water administrations. After inclusion into the study, patients were
randomised either to an immediate treatment group (SBG) or a waiting group
(WG). The waiting group began with
hydrotherapy after a waiting period of 4
weeks. Immediately before starting the individual study period all patients
were trained by a qualified hydrotherapist in performing the
hydrotherapeutical measures, enabling them to treat themselves independently
at home . Primary outcome parameters were the changes of the psychic and
physical sum-scores of SF-36 after a 4-week
hydrotherapy treatment compared to
the waiting group. Secondary outcome parameters were the subscales of SF-36,
MFI-20 and the visual analogue scale (VAS) for pain. Statistical evaluation
was done after 4 and 8 weeks in both groups. Changes of temperature
sensitivity was descriptively documented along the results of the rating
scale, the physical examination at the end of study and the free notes. Among
the secondary outcome parameters the VAS-pain score for back pain and tender
joints in the SBG showed a tendency of improvement, in the WG pain became
worse (back pain SBG: from 55±23 to 46±23 mm (p=0,063), WG: from 41±28 to
48±29 mm; tender joints of originally affected extremities SBG: from 40±28 to
32±26mm (p=0,064), WG: from 34±30 to 39±29 mm). The subscales of MFI-20 did
not show any difference over time in both groups. After 8 weeks of
hydrotherapy changes were more accentuated in both groups (SBG: n=17, WG:
n=10). The SBG showed a still non-significant improvement of the physical sum
score of the SF-36 (from 25,1±9,3 to 27,7±8,9; p=0,069), the waiting group a
significant improvement of the psychic sum score of SF-36 (from 52,4±7,9 to
58,7±6,5; p=0,038). The subscale of SF-36 „physical pain“ improved
significantly in the SBG from 37,4±11,4 to 45,1±14,8 (p=0,016). The VAS-scores
for back pain improved significantly in both groups (WG: p=0,02, SBG: p=0,03).
The results concerning blood circulation of the extremities and sensitivity to
cold temperature were not conclusive. The temperature rating scale showed no
improvement of intolerance to cold temperature or for the presence of cold
extremities. The patients rated the tolerability and practicability of the
hydrotherapy treatment mostly as good or very good. Results: Out of 42
patients, who were enrolled into the study (male n=12, female n=30, average
age 59,8 +/-7,2), 39 fulfilled either four weeks of
hydrotherapy…
