Gingival crevicular fluid (GCF) has been widely investigated as a potential source of biomarkers for an individual’s oral and general health information. MicroRNAs (miRNAs) are non-coding RNAs that are involved in post-transcriptional gene regulation. In this study, we investigated potentials of certain miRNAs in GCF as biomarkers for detection of periodontal remodeling during OTM. The aim of the study was to confirm the presence of secretory miRNAs in GCF and investigate temporal expression profiles of specific secretory miRNAs during the course of orthodontic tooth movement.
Methods: A total of 70 subjects were recruited in the study from the patients receiving treatment at the University of Illinois at Chicago’s orthodontic clinic. The inclusion criteria included subjects requiring extraction of maxillary first premolars as part of their comprehensive treatment with edgewise fixed appliances. Ultimately 11 GCF samples were collected from
healthy subjects between the ages 10 and 18 (mean 14.5 years old), who maintained excellent oral hygiene throughout the study. GCF was collected using absorbent Periopaper strip at six timepoints.
T0: prior to bonding the fixed orthodontic appliances
T1: on the day of canine retraction
T2: 60 minutes after activating the power chain
T3: 1 day after the canine retraction visit
T4: 7 days after the canine retraction visit
T5: between 5 weeks after initiation of canine retraction
Human Let7d, g and i were used for normalization of secretory miRNA levels in GCF. Wilcoxon Sign Rank and Kruskal-Wallis and Mann-Whitney U statistical analyses were used in this study
Results: In all studied miRNAs, the change in expression of miRNA from T1 to T5 showed statistical significance consistently (p-value ranging from 0.005 to 0.047). Changes between T1 and T2 were only observed in miRNA-29b. Changes between T1 and T4 were observed in both miRNA-101 and miRNA-29b. Analyzing all the time points together, Kruskal-Wallis test showed no statistical significant difference between miRNA-29a, miRNA-29b and miRNA-29c, (p>0.05, p-value ranging from 0.201 to 0.802) indicating the similarity in profile change of miRNA-29 family.
Conclusions: We concluded that secretory miRNAs exist in GCF and the expression levels of miRNA-29 family change during orthodontic tooth movement.

▼ Background: Wnt signaling is fundamental to a myriad of biological processes including embryogenesis, organogenesis and tumorigenesis. It drives the expression of important genes for the proliferation, differentiation, maturation and activity of osteoblast-lineage cells. Pathways associated with Wnt signaling are regulated by their physiological antagonists such as sclerostin (SOST) as well as Wnt-5a that play important roles in the pathophysiology of several inflammatory conditions. Objectives: To compare the levels of SOST, Wnt-5a and TNF-α between chronic periodontitis and periodontally healthy sites as well as to determine the correlation between clinical periodontal parameters and protein levels. Material and methods: In a cross-sectional assessment, 25 chronic periodontitis cases and 25 periodontally healthy controls were measured for probing pocket depth (PPD), clinical attachment level (CAL), bleeding on probing (BOP), plaque index (PI) and percentage of radiographic bone loss. Gingival crevicular fluid (GCF) was collected cross sectionally from one periodontitis (deepest pocket) and one healthy (shallowest pocket) site in each case, and from one randomly selected site in each control subject. The protein levels of SOST, Wnt-5a and TNF-α in GCF were measured by ELISA. The Shapiro-Wilk test was utilized to assess the normality of the distribution and non-parametric comparisons were performed. Results: Protein levels of SOST and Wnt-5a were similar between healthy and chronic periodontitis sites as well as the SOST levels between periodontally affected and healthy sites in the chronic periodontitis group (p > 0.05). Periodontally involved sites exhibited statistically significantly higher total Wnt-5a levels than the healthy sites in the periodontitis cases (p = 0.017). When the data were expressed as concentration, significant differences were found among the study groups (p-values range from < 0.001 to 0.003). Significant positive correlations were found between SOST and 1) full-mouth PPD (p = 0.044), CAL (p = 0.010), BOP (p = 0.035); 2) PPD of the sampled sites (p = 0.037); and 3) sites with PPD 4-6 mm (p = 0.018). A trend of a negative correlation was found between SOST and sites with a CAL 0-2 mm (p = 0.052). Among all included subjects, males showed a statistically significantly higher Wnt-5a levels than females (p = 0.031). Conclusions: Within the limitations of this cross-sectional study, sites with chronic periodontitis exhibited significantly elevated levels of Wnt-5a when compared to periodontally healthy sites in chronic periodontitis patients. No statistically significant association was found with SOST and TNF-α levels.

fluid that escapes from between the tooth surface and the epithelial lining of the periodontal ligament (PDL). There are over 100 regulatory proteins that can be detected in GCF, and they can be categorized on the basis of their biological significance such as bone resorption or formation. OBJECTIVES: To investigate the effect of vibratory stimulation (AcceleDent(R) Aura) on the expression of biomarkers in GCF using a human tooth movement model with clear aligners. METHODS: Prospective, single-center, randomized controlled crossover study comparing an active vibration device to a sham device. Participants received 6 Zendura(R) plastic aligners to move a target tooth 1.98 mm over 12 weeks. Patients were crossed over to the opposite device at the 4th aligner. GCF was collected at T0 (baseline 1), T1 (baseline 2), T2 (week 1), T3 (week 7, crossover) and T4 (week 12). 11 biomarker levels quantified by multiplex bead immunoassay. A mixed model analysis was used to compare the effects of active versus sham device on GCF composition. RESULTS: 16 males and 23 females completed the protocol. No statistically significant differences were found between the biomarker levels of the Sham-Active (SA Group) and Active-Sham (AS Group). The results of each group were then combined and biomarker concentrations were compared to tooth movement. There was a negative correlation between the baseline level of IL-6 and the total amount of tooth movement. A negative correlation was seen between the baseline levels of MMP-9 and tooth movement between weeks 1-2 followed by a positive correlation between the concentration of MMP-9 at T4 and the amount of tooth movement over the course of the study. CONCLUSIONS: The results suggest that vibration does not influence the secretion of GCF biomarkers; nevertheless, tooth movement with clear aligners seems to alter the proteomic composition of GCF. ▼ Nitric oxide (NO) plays a role in regulating the rate of orthodontic tooth movement (OTM) in rat models; however, in humans this role remains less clear. In this study, samples of gingival crevicular fluid (GCF) were collected from each maxillary central incisor and first and second molar immediately before (T0), 1 hour after (T1), and 3-4 days after (T2) application of light orthodontic forces in thirteen male participants (ages 11-18 years) undergoing orthodontic therapy. NO levels were measured in each GCF sample, and significantly higher NO levels (p<0.05) were found at T1 at the buccal surfaces of the central incisors when compared to the posterior teeth. The results indicate a possible role for NO in OTM at the pressure sites of incisors at early time points. Further studies are required to determine whether NO levels in the PDL of human teeth are affected by the magnitude of an applied force.

▼ A periodontite é uma patologia inflamatória, causada pela interação entre o biofilme bacteriano e a resposta imune do hospedeiro e é caracterizada pela degradação de fibras colágenas do periodonto.
Os avanços científicos possibilitaram o desenvolvimento de novas técnicas que possibilitem o diagnóstico precoce da doença, ou seja, prevenir a perda de inserção e progressão da periodontite. O diagnóstico da doença periodontal baseia-se em parâmetros clínicos como profundidade de sondagem, perda de inserção, hemorragia a sondagem e exames radiográficos.
A análise dos biomarcadores presentes no fluido crevicular gengival (FCG) e saliva tem sido alvo de inúmeros artigos, uma vez que o fluido crevicular gengival (FCG) é composto de uma gama de biomarcadores celulares e moleculares.
O presente trabalho teve como objetivo fazer uma revisão narrativa da literatura publicada a cerca dos métodos utilizados para diagnóstico precoce e prognóstico dos processos patológicos que podem estar presentes no periodonto. Foi realizada uma pesquisa bibliográfica online entre Dezembro de 2015 e Maio de 2016, com o objetivo de fazer uma revisão bibliográfica acerca do tema. Não foi estabelecido restrição quanto a data de publicação.
Através da identificação da concentração de determinadas proteínas presentes no FCG é possível avaliar a atividade da doença periodontal e o prognóstico. Um importante número de marcadores como a elastase neutrofílica, prostaglandina E2, catepsinas, osteocalcina, -glucoronidase, colagenase, fosfatase alcalina, aspartato aminotransferase são citados (Genco et al., 1992).
De acordo com a literatura publicada pode-se concluir que há alguns marcadores promissores para o diagnóstico precoce e prognóstico da doença periodontal, entretanto, não há ainda um marcador que possa identificar a futura perda de inserção ou a suscetibilidade à doença periodontal. São necessários estudos longitudinais para melhor entendimento do papel dos biomarcadores do FCG.

▼ Periodontal health is maintained by various host immune mechanisms. Neutrophils are a crucial component of health representing the first line of defense against microbial challenge. The migration and activation of these key effector cells in the gingiva are orchestrated by complex networks of host mediators called chemokines and cytokines. Recently, it has been shown that bacteria have an influential role in the immunomodulation of host mediators and hence host immune response. Indeed, dysregulation of the host immune response, as well as a dysbiosis of the oral microbial community, has been attributed to the pathogenesis of periodontal disease. However, the few investigations into the interaction between the subgingival microbial community and the host immune response have provided limited information to understand this relationship. Therefore, a full assessment of the host-bacterial interaction in health and disease is necessary to further our understandings of the etiology of periodontal disease and identify novel biomarkers of this disease. This thesis investigates the interaction of subgingival microbiome and the host immune response for both health and disease with disease defined as induced gingivitis. A comprehensive evaluation of forty major chemokines and inflammatory mediators in the
gingival crevicular fluid (GCF) was achieved by multiplex immunoassay. In parallel, 16S rRNA next-generation sequencing was performed to characterize microbial community composition and structure. Lastly, neutrophil infiltration was assessed by myeloperoxidase (MPO) detection in the GCF to correlate neutrophil recruitment with chemokine expression patterns. The cross-sectional study in healthy adolescents showed inter-individual variability in the chemokine expression patterns and the subgingival microbial profile. Two different chemokine expression patterns were observed among study participants; however, the different groups did not share common microbiome profiles. Additionally, there was no significant concordance between the combined chemokines and microbial profiles in
gingival health. Altogether, this data provides a better understanding of the interaction between the immune response and microbiome in the state of clinical health. Findings from the experimental gingivitis study demonstrated that during the initial transition from clinical health to gingivitis, there was a significant shift in the microbial composition that paralleled an alteration in host homeostasis. Despite variable immune responses to plaque accumulation, this alteration was characterized by a decrease in neutrophil chemotactic factors within most patients. Furthermore, patients were able to be separated into three distinct categories with unique microbial and chemokine profiles based on their immune response. ▼ Azithromycin, a macrolide antibiotic is active against
several periodontal pathogens, is taken up and concentrated inside
gingival fibroblasts, oral epithelium and polymorphonuclear
leucocytes. Interactions with host cells in gingiva could influence
the pharmacokinetics of this agent in periodontal applications.
This issue was examined in two separate clinical studies. The first
study, described in Chapter 2, tested the hypothesis that
steady-state concentrations of azithromycin are higher and more
sustained in
gingival crevicular fluid (GCF) than in serum. Four
healthy subjects received an initial dose of 500 mg azithromycin
followed by 250 mg doses on each of the next 2 days. Serum and GCF
samples were obtained 2 hours after the last dose and on days 4 and
7. GCF samples were collected from maxillary posterior
interproximal sites with paper strips. The strips were pooled and
eluted with high purity water. After extraction with diethyl ether,
the azithromycin content of the serum samples and GCF eluates was
determined with an agar diffusion bioassay. On days 2, 4 and 7, the
concentrations of azithromycin in blood serum were 0.22 ± 0.02,
0.08 ± 0.02 and 0.04 ± 0.01 µg/ml, respectively. The concentrations
in GCF were 8.82 ± 1.25, 7.90 ± 1.72 and 7.38 ± 1.15 µg/ml,
respectively. Mean GCF levels were consistently higher than mean
serum levels. The findings demonstrate that the pharmacokinetic
profiles of azithromycin are different in GCF and serum. At steady
state, azithromycin concentrations in GCF were higher and more
sustained than those in serum. Based on previous studies, the
levels observed in GCF were above the Minimum Inhibitory
Concentration for Aggregatibacter actinomycetemcomitans,
Porphyromonas gingivalis, and Prevotella intermedia.Azithromycin
preferentially localizes in inflamed tissues, suggesting that its
concentration in
gingival crevicular fluid could be higher at
gingivitis sites than in health. The study described in chapter 3
tested this hypothesis. Experimental gingivitis was induced in one
maxillary posterior sextant in nine healthy subjects. Contralateral
healthy sextants served as controls. Subjects took 500 mg of
azithromycin followed by 250 mg 24 hours later. Four hours after
the second dose, plaque was removed from the gingivitis sites. GCF
was collected from 8 surfaces in both the gingivitis and control
sextants and pooled separately. GCF samples were subsequently
collected on days 1, 2, 3, 8 and 15 and their azithromycin content
was determined with the bioassay procedure described in Chapter 2.
On days 2 and 3, the pooled GCF volume at gingivitis sites was
significantly higher than at control sites and the mean
azithromycin content in 30 second GCF samples pooled from
gingivitis sites was significantly higher than at control sites.
However, there were no significant differences in GCF azithromycin
concentration between the gingivitis and control pools at any

▼ O objetivo desse estudo foi analisar a expressão do interferon-gamma (INF-) em biópsias gengivais de sítios rasos e profundos de pacientes com periodontite crônica severa. O objetivo secundário foi correlacionar a expressão do INF- no fluido gengival com os sítios onde foram coletadas as biópsias gengivais. Foram coletadas biópsias de 22 pacientes portadores de periodontite crônica generalizada ou localizada severas (idade média 45,5 DP 8,9 anos), sendo 22 sítios profundos e 18 sítios rasos. O grupo controle foi composto por 14 pacientes clinicamente saudáveis (idade média 39,35 DP 16,5 anos). No total, foram 54 biópsias coletadas de 36 pacientes. As amostras do fluido gengival foram coletadas de alguns dos mesmos sítios de onde foram realizadas as biópsias, totalizando 12 sítios profundos, 8 sítios rasos e 4 sítios controle. Foram utilizados os parâmetros clínicos de avaliação de profundidade de bolsa à sondagem (PB); nível de inserção clínica (NIC); índice de placa visível (IPV) e índice de sangramento gengival (ISG). O tecido foi removido com punch de 2 mm de diâmetro, na área cirúrgica (grupo controle) ou na consulta para raspagem subgengival com ou sem acesso cirúrgico (grupo teste) e armazenados em Eppendorffs com 1 ml de solução de formaldeído a 10% para posterior análise morfológica e imuno-histoquímica. A intensidade da marcação do INF- foi avaliada semiquantitativamente nas células epiteliais, plasmócitos, macrófagos, fibroblastos e células endoteliais, considerando-se marcação forte (escore 2), marcação fraca (escore 1) ou ausência de marcação (escore 0). O teste de Kruskal-Wallis foi utilizado para comparar a expressão do INF- nos tecidos epitelial e conjuntivo, entre os três grupos (sítios profundos e rasos da periodontite e sítios controle). Observamos uma tendência a um padrão de marcação similar nos sítios rasos e profundos, com predomínio de marcação fraca nos sítios profundos. Nos sítios controle a marcação do epitélio demonstrou ser predominante. Porém, não foi possível demonstrar diferenças estatisticamente significativas entre a expressão do INF- nos tecidos epitelial e conjuntivo nos grupos analisados. A análise da correlação de Spearman revelou uma forte correlação entre a expressão imuno-histoquímica do INF- no epitélio com macrófagos, fibroblastos e células endoteliais (r ≥ 0,6 e p ≤ 0,01). A expressão do INF- nos tecidos demostrou não ter correlação significante com os dados clínicos apresentados e com o fluido gengival. Concluímos que não foi possível observar diferenças na expressão do INF- em biópsias gengivais nos sítios rasos e profundos de pacientes com periodontite quando comparados à indivíduos saudáveis, o que pode ser atribuído ao caráter bifásico do INF-. A baixa detecção do INF- no fluido gengival, dentro das limitações do estudo, pode sugerir que este talvez não seja o método de eleição para a detecção do INF-Y.

▼ A doença periodontal tem sido frequentemente associada ao parto pretermo. A plausibilidade biológica para tal associação baseia-se na hipótese de uma inflamação sistêmica de baixa intensidade de origem periodontal. Entretanto, os estudos de intervenção que investigaram o efeito da terapia periodontal durante a gestação não observaram reduções significativas na incidência de prematuridade. Como diversas citocinas têm sido associadas tanto a doença periodontal quanto ao parto pretermo, cogita-se que elas desempenhem um papel importante na associação observada. Até o presente momento, pouco se sabe a respeito da correlação entre os níveis de citocinas no soro e no fluído
crevicular gengival (FCG), assim como do efeito da terapia periodontal sobre esses marcadores de inflamação em gestantes. O objetivo desta tese foi avaliar a relação entre níveis sistêmicos e periodontais de biomarcadores inflamatórios relacionados com a resposta imune periodontal e também com os mecanismos de parto. Também investigou-se o efeito da terapia periodontal sobre os níveis dessas citocinas durante a gestação e 30 dias após o parto. Esta tese é composta por dois estudos que utilizaram uma sub-amostra de mulheres que haviam sido recrutadas para um ensaio clínico randomizado maior que investigou o efeito da terapia periodontal sobre a incidência de prematuridade. Mulheres entre 18-35 anos e com até 20 semanas de gestação foram aleatoriamente alocadas para receber tratamento periodontal não cirúrgico completo até a 24a semana de gestação (grupo teste) ou apenas uma consulta de uma remoção de cálculo supragingival (grupo controle). Dados clínicos e amostras de sangue e FCG foram coletadas no início do estudo, entre 26-28 semanas de gestação e 30 dias após o parto. Quatro sítios periodontais por paciente foram aleatoriamente selecionados para coleta de FCG entre aqueles com maior profundidade de sondagem. Os níveis de IL-1β, IL-6, IL-8, IL-10, IL- 12p70 e FNT-α foram analisados por citometria de fluxo. No estudo 1, investigou-se a correlação e concordância entre os níveis periodontais e sistêmicos dessas citocinas em 100 pacientes, utilizando dados coletados até a 20a semana de gestação. As pacientes apresentaram extensa inflamação e limitada destruição periodontal. A correlação entre os níveis de citocinas no soro e no FCG foi baixa e não significativa, com exceção da IL-12p70 que mostrou uma correlação moderada, porém significativa, entre as duas fontes (r=0.32, p=0.001). Os níveis de citocinas observados no FCG explicaram menos de 10% dos respectivos níveis observados no soro, com exceção da IL-12p70 em que eles foram responsáveis por 23% dos níveis séricos (p=0.0001, r2=0.23). A profundidade de sondagem e o sangramento a sondagem estiveram significativamente associados com os níveis de IL-1β, IL-6 e IL-8 no FCG, mas tiveram efeitos limitados sobre os níveis sistêmicos de todas as citocinas. ▼ A qualidade de uma reconstrução protética depende, primordialmente, de sua integração biológica e da sua adaptação. O afastamento gengival serve para se obter espaço para o material de moldagem e também para a visualização e acesso aos limites gengivais dos preparos, que são essenciais para a confecção de próteses com uma justeza de adaptação clínica. No entanto, a inserção de um material (fio retrator ou casquete) pode romper o sistema de fibras gengivais que conectam a margem gengival à superfície do cemento causando recessão gengival. Ocasionalmente, ocorre significativa inflamação ou perda de inserção. O objetivo deste trabalho foi quantificar a resposta inflamatória decorrente das diferentes técnicas de moldagem para prótese fixa, em termos da quantidade de IL-2, IL-4, IL-5, IL-10, TNF-α, IFN-γ, presentes no fluido gengival dos dentes com necessidade protética. A amostra foi composta por 10 dentes moldados pela técnica de afastamento gengival com fio retrator e um grupo de 10 dentes moldados pela técnica do casquete. Os pacientes foram selecionados considerando-se a necessidade de prótese parcial fixa, higiene bucal satisfatória e não possuírem sinais clínicos de doença periodontal. Previamente à moldagem, uma tira de periopaper foi inserida no sulco gengival do paciente e foi mantida por 30 segundos. A coleta foi repetida 4 vezes para as faces mesiovestibular, distovestibular, mesiolingual e distolingual. Terminado o procedimento da moldagem, após 2 horas, 4 horas e 24 horas, respectivamente, foi repetido o procedimento de inserção do periopaper nos 4 sítios dos dentes. A amostra foi preparada para a citometria de fluxo (CBA - cytometric beads array). Os resultados foram analisados pelo teste de Tukey. Na análise de variância a 3 critérios, por meio da comparação entre técnica de moldagem, paciente e tempo para os valores de TNF-α, a relação tempo e paciente versus tempo foi estatisticamente significante (p?0,05). Na comparação entre a técnica de moldagem com casquete e o fio retrator, não houve superioridade entre uma técnica e outra(p?0,05). O teste Tukey para a comparação intragrupo tempo (4 tempos), independente de técnica e paciente, mostrou relevância estatística entre os períodos antes da moldagem e após 24 horas (p=0,016) e, no período entre 2 horas e 24 horas após a moldagem (p=0,06). Portanto, foi possível concluir que não existe evidência científica que demonstre alguma superioridade das técnicas estudadas. Com isso, a escolha pelo método de afastamento gengival depende da situação clínica e habilidade do profissional.

▼ Dois importantes fatores inter-relacionados estão envolvidos na patogênese da doença periodontal: ativação do sistema imune e produção de peroxidases. Os objetivos deste estudo foram comparar os níveis da atividade do sistema peroxidase presentes no fluido gengival (MPO/FG) e saliva da parótida (POS) em pacientes portadores de periodontite crônica em relação a pacientes periodontalmente saudáveis e avaliar o efeito do tratamento periodontal não cirúrgico nestes indivíduos nestes parâmetros enzimáticos. MATERIAL E MÉTODO: Foram coletados dados dos parâmetros clínicos periodontais e amostras de FG e saliva da glândula parótida de 17 pacientes clinicamente saudáveis (GC) e de 17 pacientes portadores de periodontite crônica generalizada (GP) antes e depois do tratamento periodontal pareados em idade e sexo. As atividades de MPO/FG e POS Parótida foram avaliadas por espectofotometria. RESULTADOS: O GP apresentou valores maiores nos parâmetros clínicos periodontais em relação ao GC, sendo que os mesmos foram reduzidos após tratamento periodontal. Além disso, o GP apresentou valores mais elevados de atividade de MPO no FG (p=0,04) do que o GC. Após tratamento periodontal os níveis de MPO/FG (p<0,001) e POS parótida (p=0,013) foram reduzidos significativamente. No GP houve correlação negativa e significativa entre POS da parótida e MPO no FG (r=-0,59, p=0,02). CONCLUSÕES: Na doença periodontal, as atividades de MPO no FG são maiores quando comparados ao grupo controle, e tanto de MPO/FG e POS parótida são reduzidas após tratamento periodontal.
Two important and interrelated factors are involved in the pathophysiology of periodontal diseases: the activation of immune system and the production of peroxidases. The objectives of this study were to examine the peroxidase activities present in Gingival Crevicular Fluid (MPO/GCF) and in Parotid Saliva (POS) in patients with generalized chronic periodontitis with healthy controls and to compare the effects of periodontal therapy on enzyme activities. MATERIAL AND METHOD: Periodontal clinical measurements, parotid saliva and GCF samples were obtained of 17 age-matched healthy controls (PC) and 17 patients with chronic periodontal disease (PD) before treatment and after periodontal treatment. The MPO/GCF and POS parotid activities were determined spectrophotometrically. RESULTS: In PD group, clinical parameters, MPO/GCF (p=0,04) activities presented higher values than controls. After periodontal treatment levels of MPO/GCF (p<0.001) and POS parotid (p=0,013) were significantly reduced. In PD there was significant negative correlation between POS and MPO in the parotid FG (r =-0,59, p=0,02). CONCLUSIONS: In periodontal disease MPO/GCF presented higher levels than controls and MPO/GCF and POS parotid are reduced after treatment.

▼ O objetivo deste estudo foi determinar os níveis de interleucina-1β (IL-1 β), IL-2, IL-4, IL-8, interferon-γ (IFN-γ) e a atividade de elastase no fluido gengival (FG) de pacientes com periodontite crônica generalizada (PCG) e periodontite agressiva generalizada (PAgG), e correlacionar com indivíduos de um grupo controle com gengivite apenas. Um objetivo secundário foi analisar o perfil microbiológico subgengival destes indivíduos. Dados clínicos transversais foram obtidos de 20 pacientes com PCG, 17 pacientes com PAgG e 10 indivíduos com gengivite. Amostras de FG foram coletadas com tiras de papel e os níveis de: IL-1β, IL-2, IL-4, IL-8 e IFN-γ foram medidos, utilizando um imunoensaio do tipo multiplex (Luminex). Atividade da elastase foi avaliada por um ensaio enzimático. Amostras de placa subgengival foram analisadas através do checkerboard DNA-DNA hybridization. As diferenças de significância entre os grupos para dados imunológicos e microbiológicos foram realizadas utilizando o teste Kruskal-Wallis, ajustando para múltiplas comparações. As médias dos parâmetros clínicos e os volumes de FG foram maiores nos pacientes com PCG e PAgG comparados ao grupo gengivite. Níveis mais elevados de IL-1β e atividade de elastase foram encontrados em sítios profundos quando comparado a sítios rasos em ambos os grupos com periodontite (p <0,05). Os dados microbiológicos apresentaram níveis significativamente mais elevados das espécies do complexo vermelho em pacientes com PCG e PAgG, quando comparados aos indivíduos com gengivite (p <0,05). Não houve diferença estatisticamente significante nos níveis de biomarcadores no FG e nos níveis de espécies bacterianas subgengivais entre pacientes com PCG e pacientes com PAgG. Sendo assim, concluímos que os dados do presente estudo não mostraram diferença estatisticamente significante nos parâmetros imunológicos e microbiológicos medidos entre indivíduos com PCG e PAgG.
The goal of this study was to determine the gingival crevicular fluid (GCF) levels of interleukin-1β (IL-1β), IL-2, IL-4, IL-8 and interferon-γ (IFN-γ) and elastase activity from patients with generalized chronic (GCP) and generalized aggressive periodontitis (GAgP) and to compare with a control group with gingivitis subjects. ▼ Durante o tratamento ortodôntico, a resposta inicial dos tecidos periodontais ao estímulo mecânico envolve várias alterações estruturais e bioquímicas que permitem a movimentação do dente. As metaloproteinases da matriz (MMPs) parecem desempenhar um papel importante na manutenção da integridade funcional da matriz extracelular periodontal. O objetivo do presente estudo foi avaliar, em diferentes intervalos de tempo, os níveis de metaloproteinases da matriz -1, -2, -3, -7, -8, -12 e -13 no fluido gengival (FG) de caninos superiores submetidos ao movimento de distalização e testar a hipótese de possíveis alterações nos níveis destas MMPs com o emprego de forças ortodônticas. Amostras de FG foram obtidas de dezesseis pacientes ortodônticos saudáveis (nove do sexo masculino e sete do sexo feminino, com idades entre 13 e 27 anos, média de idade 17,7 anos) que possuíam indicação de exodontias dos primeiros pré-molares superiores e tiveram os caninos distalizados como parte da terapia ortodôntica. Um dos caninos superiores foi distalizado ortodonticamente, sendo considerado dente teste. O canino contralateral não foi submetido a nenhuma força, no entanto foi incluído na aparatologia ortodôntica e utilizado como controle. A coleta de FG foi realizada nos sítios mesial (tensão) e distal (pressão) dos dentes testes e controles 7 dias antes da montagem da aparatologia ortodôntica, imediatamennte após a aplicação da força ortodôntica, e após 1 h, 24 h, e 7, 14 e 21 dias, respectivamente denominados -7d, 0h, 1h, 24h, 7d, 14d e 21d. A arcada superior de cada paciente foi dividida em um lado teste e um lado controle. Os resultados mostraram que foram encontradas diferenças significativas no volume do FG apenas nos intervalos de tempo entre -7d e 0h nos lados controle-pressão (CP), teste-tensão (TT) e teste-pressão (TP). Em TP foi observado ainda aumento do volume entre os tempos 0h e 14d. Foi possível detectar no FG as MMPs estudadas nos lados controle/teste e lados pressão/tensão, em todos os intervalos de tempo. As flutuações dos níveis das MMPs apresentaram poucas alterações significativas nos diferentes intervalos de tempo, nos lados controle/teste e lados pressão/ tensão. As diferenças intergrupos (TT, TP, CT e CP) em cada tempo não mostraram resultados significativos assim como as comparações entre os lados pressão e tensão para cada tempo individualmente. Os níveis de expressão da MMP-8 foram muito superiores aos das outras MMPs avaliadas, porém sem diferenças signficativas entre os lados teste e controle.

▼ O objetivo deste trabalho foi examinar o impacto da movimentação dentária de dentes periodontalmente comprometidos no volume do fluido gengival crevicular (FGC) e nos níveis de expressão das metaloproteinases de matriz (MMPs) -1, -2, -3, -7, -8, -12 e -13 no FGC. Dez pacientes periodontalmente controlados (8 do sexo feminino e 2 do sexo masculino, média de idade de 46,2 10,4 anos) com incisivos projetados labialmente foram submetidos a tratamento ortodôntico. Uma arcada dentária foi submetida a movimentação ortodôntica e a arcada oposta foi usada como controle. Amostras de FGC foram coletadas da face lingual de dois incisivos do lado movimento e dois do lado controle uma semana antes da ativação ortodôntica (-7d), imediatamente após a ativação ortodôntica, e após 1 h, 24 h, e 7, 14 e 21 dias. A coleta do FGC foi feita utilizando-se tiras de papel absorvente e o volume foi calculado através do uso do Periotron. Todos os pacientes receberam orientações de higiene bucal e um kit contendo escova de dente, pasta de dente e bochecho de Chlorexidina 0,12% para ser usado durante todo o experimento. A técnica da multianálise imunoenzimática com microesferas foi usada para medir as MMPs no FGC. Os dados foram analisados utilizando-se os testes estatísticos Friedman e Mann-Whitney. Não foram encontradas diferenças estatisticamente significativas no volume do FGC. Em relação aos níveis de MMPs, a única diferença estatisticamente significativa encontrada no decorrer do tempo foi nos níveis de MMP-1 no grupo movimento (p<0,05). Quando os dois grupos foram comparados após a ativação, a única diferença estatisticamente significativa encontrada foi nos níveis de MMP-12 24 horas após a ativação (p<0,05). Estes achados sugerem que o volume de FGC não sofre alteração relacionada ao movimento dentário ortodôntico e que o movimento ortodôntico de dentes periodontalmente comprometidos não resultou em mudanças significativas nos níveis de MMPs no FGC.
The aim of this study was to evaluate whether orthodontic tooth movement of periodontally compromised teeth would result in detectable changes in gingival crevicular fluid (GCF) volume and in the levels of matrix metalloproteinases (MMP) -1, -2, -3, -7, -8, -12 and -13 in the GCF. Ten controlled periodontitis subjects (8 females and 2 males, mean age of 46.2 10.4 years) with flared incisors were submitted to orthodontic treatment. One dental arch was subjected to orthodontic movement, and teeth in the opposite arch were used as controls. GCF samples were collected from the lingual sites of two test and two control incisors one week before orthodontic activation (-7d), immediately after orthodontic activation, and after 1 h, 24 h, and 7, 14 and 21 days. Filter paper strips were used in combination with a Periotron for GCF volume measurements. Uvod. Prevremeni porođaj se definiše kao onaj
porođaj koji se dogodi pre navršene 37. nedelje gestacije.
Prevremeni porođaj se smatra značajnim uzrokom perinatalnog
morbiditeta i važnim činiocem perinatalnog mortaliteta.
Prostaglandin E2 (PGE2), interleukin-1β (IL-1β), interleukin-6 i
faktor nekroze tumora alfa (TNF-α) su medijatori zapaljenja koji su
označeni i kao biohemijski markeri - prediktori porođaja. Nivoi
ovih medijatora zapaljenja se postepeno povećavaju tokom trudnoće i
postizanje njihovih kritičnih vrednosti dovodi do započinjanja
porođaja. Pretpostavka je da postojanje udaljene infekcije koja ima
elemente imunskog odgovora slične prevremenom porođaju, usled
sposobnosti da naruši lokalnu homeostazu medijatora porođaja, može
predstavljati faktor rizika za njegov nastanak. Parodontopatija je
hronično inflamatorno oboljenje prouzrokovano predominantno
anareobnim Gram negativnim bakterijama. Utvrđeno je da pacijenti
oboleli od parodontopatije imaju povišene nivoe IL-1β, IL-6, PGE2 i
TNF-α u gingivalnoj tečnosti u poređenju sa ispitanicima sa zdravim
parodontalnim tkivima. Takođe, usled povećane vaskularne
propustljivosti u uslovima inflamacije u parodoncijumu,
parodontopatogene bakterije i medijatori zapaljenja imaju
sposobnost da prodru u sistemsku cirkulaciju. Brojna istraživanja
su sprovedena sa ciljem da se istraži eventualna povezanost
inflamacije u parodoncijumu i prevremenog porođaja. Ustanovljeno je
da parodontopatija trudnica predstavlja visok faktor rizika za
nastanak prevremenog porođaja. Predložena su dva mehanizma na
osnovu kojih parodontopatija može uticati na prevremeni porođaj: 1)
direktni koji podrazumeva prodor parodontopatogenih bakterija u
feto-placentalnu jedinicu i sledstvenu stimulaciju lokalne
inflamacije ili 2) indirektni mehanizam prema kom medijatori
zapaljenja iz parodontalnog žarišta cirkulišu i sinergističkim
delovanjem povećaju lokalnu inflamaciju feto-placentalne jedinice.
U pogledu mogućeg indirektnog mehanizma povezanosti parodontopatije
i prevremenog porođaja, oskudni su podaci o uticaju inflamacije u
parodoncijumu na sistemske nivoe medijatora porođaja. Hipoteza ove
studije je da patogenetski mehanizmi parodontopatije i prevremenog
porođaja uključuju povećanu produkciju određenih istih
proinflamatornih medijatora, te je moguće postojanje udruženosti

a chronic condition. Molecular epidemiology and biopsychosocial unique approach systems are required to
advance towards a better assessment and understanding of the complex interactions between HIV and its
human host. Therefore, it was proposed to establish the biopsychosocial risk factors of oral pathologies in
HIV, determining the relationship between oral‐systemic health status and antiretroviral treatment.
Material and Methods
Participants were recruited from HIV/AIDS healthcare providers, non‐governmental and community‐based
organizations in Monterrey, México and adopted a non‐probability and snowball sampling. The research
involved 202 consenting adult men who have sex with men (MSM [epidemiology], defined by the World
Health Organization as one of the Key populations at higher risk of acquiring HIV). Step 1 (Psychosocial
research). MSM filled out structured, analytical, self‐administered, anonymous questionnaires. Besides the
sociodemographic variables, the perception regarding public and private dental services and related
professionals there was also evaluated the perceived stigma associated with HIV/AIDS and sexual
orientation. Likewise, their perception towards rapid HIV‐1/2 testing in the dental practice. All the
biopsychosocial aspects were explored by designing psychometric Likert‐type scales. Step 2 (Biological
research). Gingival crevicular fluid (GCF) and serum samples from 78 HIV‐positive adult male subjects (cases)
and 39 HIV‐negative male subjects (controls) were examined for 17 cytokines using multiplex ELISA.
Results
Psychosocial analyses revealed that most subjects reported omitting their HIV serodiagnosis and agreed that
dentists must be trained and qualified to treat patients with HIV/AIDS. The factorial analysis revealed two
elements in HIV/AIDS perception: experiences of stigma and discrimination in dental appointments and
feelings of concern regarding the attitudes of professionals or their teams concerning patients’ HIV
serodiagnosis. Most respondents expressed their willingness to take a rapid HIV‐1/2 screening test during
their dental visit. Nevertheless, most of them considered important that dental professionals must be well
trained before applying any rapid HIV‐1/2 tests. Factor analysis revealed two factors towards sexual
orientation: experiences of sexual orientation stigma and discrimination in dental settings, and feelings of
concern about the attitude of dentists and dental staff towards sexual orientation

▼ Fasting exhibits anti-inflammatory effects in the human body. The impact on the periodontal tissues, however, has yet not been investigated. To examine the effects of prolonged multimodal clinically controlled fasting on periodontal parameters, a non-interventional clinical trial was conducted in patients with metabolic syndrome (MetS).
All 47 in-patients diagnosed with MetS were enrolled between August and November 2016 after signing an informed consent. Each patient participated in a multimodal fasting program according to Buchinger with an energy intake of 1200 KJl/d. Outcomes were accessed at baseline (T1) and after fasting (T2), and the inflammatory status (c-reactive protein – CRP) and periodontal parameters (bleeding on probing – BOP,
gingival crevicular fluid rate – SFFR, Plaqueindex – PI) performed by two calibrated examiners, one single-blinded, using a pressure sensitive probe were documented. Furthermore, the metabolic parameters blood pressure (BD), waist circumference (TU), fasting glucose (NGLU), triglyceride (TRG), and HDL-level were assessed.
Summary statistics of baseline characteristics were separately calculated for both men and women using the Mann-Whitney-U test. Correlation analyses were performed using Spearmans rank correlation. Differences between T1 and T2 parameters were evaluated using the Wilcoxon test.
For the statistical analysis, data of 28 women were used. BOP and SFFR significantly decreased, while PI and CRP remained unchanged (BOP 39 % - 33,1 % (p<0,001), SFFR 73,9 PE - 59,3 PE (p<0,05), PI 69,2 % - 69,3 % (p=0,446), CRP 4,2 mg/l - 4,9 mg/l (p=0,178)). All metabolic parameters were significantly reduced after fasting (BD systolic 135 mmHg – 120 mmHg, diastolic 80 mmHg - 75 mmHg (<0,001), TU 111 cm - 107 cm (p<0,001), NGLU 99,1 mg/dl - 76,6 mg/dl (p<0,001), TRG 173,7 mg/dl - 135,2 mg/dl (p<0,001), HDL 50,3 mg/dl - 46,4 mg/dl (p=0,002)). The reduction in bleeding correlated with fasting glucose levels at baseline (r=0,37, p=0,049) as well as weight loss (r=0,4, p=0,04).
Prolonged fasting exhibits beneficial short-term effects on all MetS parameters and may facilitate the reduction of
gingival inflammation. CRP seems to be an inadequate marker to predict inflammatory processes in periodontal tissues while fasting. To evaluate the effect of fasting on periodontal disease further studies are required.

▼ Orthodontic tooth movement is mediated by interactions between PDL cells and those of the alveolus. One protein—the receptor activator nuclear factor kappa B ligand (RANKL)—is critical for osteoclastogenesis, and osteoprotegerin (OPG) is a decoy ligand that competitively inhibits RANKL. A higher RANKL/OPG ratio is associated with areas of bone resorption, while a lower ratio occurs in areas of bone deposition and homeostasis. There have been almost no clinical studies of RANKL and OPG expression in human subjects undergoing orthodontic tooth movement. The purpose of this study was to quantify changes in the levels of RANKL and of OPG expression in human
gingival crevicular fluid in growing (adolescent) and non-growing (adult) patients in response to orthodontic force. Untreated adolescents (under 18 years of age) and adults (over 18 years of age) had a calibrated force applied across a left-right pair of maxillary premolars with a transpalatal spring (TPS). RANKL and OPG were measured in
gingival crevicular fluid (GCF) sampled serially from the pressure and tension sides of maxillary premolars at 5 different time points: before placement of transpalatal spring, 1 day (24 hours) after TPS placement, 2 days (48 hours) after TPS placement, 5 days (120 hours) after TPS placement (TPS was then removed), and 3 days (72 hours) after TPS removal. RANKL and OPG expression was measured by the ELISA assay. Expectations were that (1) force would raise RANKL and diminish OPG, (2) force removal would reverse the RANKL-OPG levels, (3) strength and duration of force are associated with RANKL-OPG levels, (4) responses would exhibit considerable inter-individual variation, (5) the RANKL/OPG levels will be higher in children due to general growth and bone remodeling in response to orthodontic forces, and (6) the RANKL/OPG levels will be lower in adults because no growth is occurring and osteoclastogenesis is triggered solely by orthodontic forces.
Gingival crevicular fluid (GCF) volume increased significantly after applying force with the nickel-titanium coil spring. The volume remained elevated until the force was removed, and had not quite returned to baseline by 3 days (72 hours) following force removal. There were no significant differences found in the levels of GCF collected in regards to age, sex, or race. When evaluating the changes in
gingival crevicular fluid in response to mechanical force, it appears that time is the significant factor regardless of the person’s age, sex, or race. In general, both RANKL and OPG levels decreased significantly over the time while the spring was in place; however, the RANKL/OPG ratio increased over time. No significant differences were found in the levels of RANKL when comparing sex, age, and race. Higher levels of OPG were found in males. A linear decrease in OPG was seen in regards to age; therefore, adults have less OPG than their younger counterparts. ▼ O objetivo do presente estudo foi comparar a expressão de IL-1β, IL-4, IL-8, interferon-γ, atividade de elastase e a composição do perfil microbiano subgengival antes e depois do tratamento periodontal não cirúrgico em pacientes com doença peridontal crônica generalizada (PC) e agressiva generalizada (PA). Vinte pacientes com PC e quatorze com PA foram avaliados. Dados clínicos, fluido gengival e biofilme subgengival foram analisados na visita inicial (VI) e 3 meses (3M) após o tratamento periodontal não cirúrgico. Amostras de fluido gengival (FG) foram coletadas com tiras de papel e os níveis de: IL-1β, IL-4, IL-8 e INF-γ foram medidos, utilizando um tipo de imunoensaio multiplexado (Luminex). Atividade da elastase foi avaliada por um ensaio enzimático. Amostras de placa subgengival foram analisadas através do checkerboard DNA-DNA hybridization. Na avaliação de 3 meses após terapia periodontal foi encontrado melhora significativa para todos os parâmetros clínicos em ambos os grupos. Foram encontradas reduções significativas na atividade de elastase nos sítios rasos e profundos dos pacientes do grupo PA e nos sítios profundos do grupo PC, também foi achado um aumento significativo de INF-γ nos sítios rasos do grupo PA. Os dados microbiológicos, mostraram reduções significativas para os níveis dos membros do complexo vermelho (P. gingivalis, T. forsythia, T.denticola), e para as espécies E.nodatum e P.micra no grupo PC. No grupo PA, ocorreram reduções significativas no níveis de P. gingivalis, T. forsythia, Fusobacterium nucleatum ss polymorphum e Fusobacterium periodonticum. Quando as respostas clínica e imunológica 3M após terapia foram comparadas entre os grupos, apenas diferenças sutis foram observadas. Nenhuma diferença microbiológica foi encontrada entre os grupos após a terapia. Em conclusão, os achados suportam nossa hipótese de que as periodontites cronica e agressiva respondem de forma semelhante ao tratamento periodontal nao cirúrgico.
Our aim was to compare the expression of IL1β, IL-4, IL-8, INF-γ, elastase activity and the composition of the subgingival microbiota profile before and after non-surgical periodontal treatment in patients with untreated generalized chronic (GCP) and aggressive periodontitis (GAgP). Twenty patients with GCP and 14 with GAgP were evaluated. Clinical data, GCF and plaque were analyzed at baseline and 3 months after non-surgical periodontal treatment. IL-1β, IL-4, IL-8 and INFγ were analyzed in a luminex assay. Elastase activity was assessed by an enzymatic assay. Subgingival plaque samples were analyzed using checkerboard DNA-DNA hybridization. Three months after periodontal therapy significant improvement for all clinical parameters in both groups were found. We have found significant reductions in the elastase activity in shallow and deep sites from GAgP and in deep sites from GCP group. ▼ O objetivo deste estudo foi avaliar o impacto do tratamento periodontal não cirúrgico sobre a atividade de elastase e o volume de fluido gengival nos pacientes portadores de periodontite crônica e agressiva generalizadas. Foram avaliados 18 pacientes com periodontite crônica (idade média 48,6 DP 7,5 anos), e 11 com periodontite agressiva (idade média 27,9 DP 6,54 anos). Foram utilizados os parâmetros clínicos de avaliação de profundidade de bolsa à sondagem (PB) (mm), nível de inserção (NI) (mm) e sangramento à sondagem (SS). As medidas clínicas e as amostras de fluido gengival foram colhidas a partir dos cinco sítios mais profundos (P) e de cinco sítios rasos com gengivite (G) de cada paciente, antes e 90 dias após o término do tratamento. As etapas clínicas obedeceram ao seguinte cronograma: seleção e exame periodontal; coleta de fluido gengival; tratamento periodontal; reavaliação, compreendendo o exame periodontal e coleta de fluido gengival. O tratamento levou, em média, 4 sessões de 40 minutos cada por paciente, com intervalo de 1 semana entre elas. As consultas para reavaliação foram feitas 90 dias após o término do tratamento. O teste de Wilcoxon foi utilizado para comparar os dados antes e depois do tratamento e o teste não pareado de Mann-Whitney U-test foi utilizado para comparar os grupos de periodontite crônica e agressiva. A amostra analisada antes e após o tratamento não apresentou diferenças significativas entre o grupo com periodontite crônica e agressiva, que responderam de forma similar a todos os indicadores avaliados, exceto para a profundidade de bolsa à sondagem nos sítios rasos com gengivite (p = 0,039) e para o sangramento à sondagem (p = 0,021) nos sítios profundos, ambos mais reduzidos na periodontite crônica após o tratamento. A elastase apresentou, após o tratamento, redução significativa nos sítios profundos, para a periodontite crônica (p = 0,012) e agressiva (p = 0,02). Em relação ao volume de fluido gengival, houve significativa redução após o tratamento nos pacientes com periodontite crônica e agressiva, tanto nos sítios rasos (p = 0,03 e p = 0,03) como nos profundos (p ˂ 0,001 e p = 0,003), respectivamente. Concluindo, os grupos com periodontite crônica e agressiva generalizadas comportaram- se de maneira semelhante frente à terapia mecânica não cirúrgica. Ainda, a terapia periodontal mecânica não cirúrgica mostrou redução significativa do volume de fluido gengival em todos os sítios analisados, e da atividade neutrofílica, nos sítios profundos, associada a reduções significativas em todos os indicadores clínicos analisados após o tratamento.

▼ O objetivo do estudo foi mensurar as alterações na quantidade do volume de fluido gengival (FG) dos dentes submetidos à movimentação ortodôntica, em adultos com periodontite crônica na fase de manutenção periodontal. O FG foi coletado com uma tira de papel absorvente padrão das áreas de pressão dos dentes de 10 pacientes (sete homens e três mulheres) e medido em microlitro no Periotron em seis dias distintos (dia -7, dia 0, dia 1, dia 7, dia 14 e dia 21). Os pacientes foram submetidos a um rigoroso programa de controle de placa. O teste de Wilcoxon foi aplicado para comparar os dados obtidos. Os resultados mostraram que o controle de placa efetivo baixou a quantidade de FG do dia -7 para o dia 0, e em seguida houve uma elevação do FG até o dia 7 por conta do início da movimentação ortodôntica, e uma nova queda do dia 7 até o 21. O maior volume de FG foi encontrado no dia 7, uma semana após o início da movimentação. O menor volume no dia 0. Concluindo, a movimentação ortodôntica aumentou o volume de FG do dia 0 até o dia 7.
The aim of this study was to measure the alterations in the amount of gingival crevicular fluid (GCF) of teeth during orthodontic tooth movement in adults with chronic periodontitis under a periodontal maintenance program. The GCF was collected with paper stripes from pression sides of teeth in 10 patients (7 males and 3 females) and measured in microlitres on Periotron in six distinct periods of time (day -7, day 0, day 1, day 7, day 14, day 21). Patients were under supervised program of plaque control. Wilcoxon test was applied to compare the data. The results shown that the effective plaque control reduces the amount of GCF from day -7 to day 0, followed by an elevation until day 7 after the beginning of orthodontic tooth movement and a new reduce from day 7 to 21. The highest amount of GCF was found on day 7, one week after the beginning of tooth movement, while the smallest found on day 0.

▼ O objetivo deste estudo foi utilizar biópsias minimamente invasivas para avaliar a expressão de mediadores inflamatórios e sua correlação com o fluido gengival em pacientes com periodontite severa. O grupo teste compreendeu 22 pacientes com periodontite severa (idade média 45,5 DP 8,9 anos), analisados por sítios rasos e profundos, e o grupo controle por 14 pacientes periodontalmente saudáveis (idade média 39,35 DP 16,5 anos). As amostras do fluido gengival foram coletadas com papel absorvente, nos mesmos sítios de onde foram realizadas as biópsias, e quantificadas, por Luminex, para IFN- γ, IL 1-β, IL-6, IL-21, IL-22, IL-23, IL-25, IL-31, IL-33, IL-4, IL-6, IL-10, IL-17A, IL-17F, sCD40L e TNFα. Foram coletadas biópsias, com um punch de 2mm de diâmetro nos grupos teste e controle. A avaliação imuno-histoquímica foi semiquantitativa, nas células epiteliais, plasmócitos, macrófagos, fibroblastos e células endoteliais, para a IL1-β, IFN-γ, IL-6 e IL-17. Foram observadas marcações imuno-histoquímicas, em todos os grupos celulares analisados, tanto nos pacientes com periodontite como nos controles, sem diferenças significativas entre eles. O fluido gengival apresentou maiores quantidades para os marcadores IL-1β e IL-23 nos sítios profundos. Não foram encontradas correlações significativas entre as marcações imuno-histoquímicas e o fluido gengival para os subgrupos analisados. Na análise comparativa entre as biópsias e o fluido gengival, a IL1-β mostrou alta concordância nos sítios rasos e profundos. Concluindo, a utilização padronizada de um punch de 2mm de diâmetro para biópsias em tecido periodontal mostrou- se viável, para estudos com imuno-histoquímica. O fluido gengival pode não expressar todos os marcadores do tecido correspondente, com variações dependendo do marcador analisado e das condições inflamatórias locais.
The aim of this study was to use minimally invasive biopsies to evaluate the expression of inflammatory mediators and their correlation with gingival fluid in patients with severe periodontitis. The test group comprised 22 patients with severe periodontitis (mean age 45.5 8.9 years SD), analyzed by shallow and deep sites and the control group for 14 periodontally healthy patients (mean age 39.35 SD 16.5 years). Gingival fluid samples were collected with absorbent paper in the same sites where biopsies were performed, and quantified by Luminex to IFN-γ, IL-1 β, IL-6, IL-21, IL-22, IL-23, IL-25, IL-31, IL-33, IL-4, IL-6, IL-10, IL-17A, IL-17F, TNF and sCD40L. Biopsies were taken with a 2 mm diameter punch in both groups. ▼ The periodontitis is an inflammatory disease condition caused by a variety of
oral microorganisms. A hyper reactive host response regulation is triggering
the disease progression in a small part of the world wide population – around
10% at most – resulting either in a local or generalized aggressive form of
manifestation. Bleeding is a final act in a process of tissue and capillary
damage initiated by bacterial presence and modulated by pro-inflammatory
cytokines. Calibrated assignment of bleeding intensity as a parameter for the
severity of the disease is considered golden standard for diagnosing
gingivitis – an inflammatory condition restricted to the soft tissues.
Periodontal inflammation regularly causes attachment loss and is accompanied
by loss of alveolar bone and a phenomenon named “pocketing”. The severity of
this disease condition is currently characterized by assessing either the
probing pocket depths (PPD) or the attachment loss (AL). The evaluation of
bleeding tendency as Bleeding on Probing (BoP) is currently an additional
dichotomous finding. The aim of the present study was to assess the
suitability of a visual calibration of a Pocket Bleeding Index (PoB-Index) to
reflect the real inflammatory status of each single probed periodontal pocket.
By matching single grades of the index suggested – i.e. the single visually
assigned bleeding grades - with parameters such as bleeding duration (stop
watch) and amount of blood sampled by capillary tube correlation coefficients
were calculated. Correlations between PoB-Index and the probing depth and PoBI
and the amount of
gingival crevicular fluid (GCF) sampled as exudates that
follow a gradient were calculated also to objectify the visual calibration of
bleeding intensity. The capillary dilatation is initiated as first step in a
cascade of inflammatory reactions in front of all clinical signs and results
in a measurable increase of the GCF. Since periodontitis progresses
episodically, i.e. phases of activity are followed by phases of inactivity the
capillary dilatation follows the same pattern. The mean age of 17
participating patients recruited was 32 years; 10 of them were non-smokers
whereas 7 were smoking. All patients enrolled did not have any periodontitis
therapy or received any antibiotics prescribed for the last 12 months. All
patients underwent intensive instructions in oral hygiene repeatedly, thus the
allocated plaque index remained for the duration of the study < 25% (API <
25%). After completion of instrumentation (S/RP) all patients received
Amoxicillin (500 mg) and Metronidazol (250 mg) x 3 daily for 10 days per os
prescribed. The assessment of the study parameters was carried out at baseline
after completion of the initial therapy (before S/RP), at month 3, 6 and 9
past active therapy. In total 575 sites were evaluated and monitored. For
improving the reproducibility, all measurements were undertaken under equal

Dessa enzym härrör från värdcellerna och är en del av det immunologiska svaret på bakteriella virulensfaktorer. Grönt te har studerats för sina hälsofrämjande egenskaper, som omfattar bland annat anti-inflammatoriska effekter. Effekten beror delvis på enzyminhibering av tepolyfenoler. Syftet med denna studie var att ytterligare undersöka den inhiberande effekten av grönt te, med fokus på enzymatisk aktivitet i gingivalvätska från patienter med parodontal sjukdom. Patienter med kronisk parodontit valdes ut för att delta i studien. Gingivalvätska extraherades med mikropipetter från patienternas gingivala sulci. Proverna behandlades med grönt te och jämfördes med obehandlade prover från samma försöksperson. Fluorescens proteasanalys med kasein som substrat utfördes på fjorton prover för att detektera skillnader i kaseinolytisk aktivitet. Zymogramanalys med användning av gelatin som substrat utfördes på fyra prover, för att undersöka skillnader i gelatinolytisk aktivitet och analysera molekylvikter för de olika enzymerna. Den fluorometriska analysen visade en signifikant lägre enzymaktivitet i prover med tillsatt grönt te jämfört med obehandlade prover (p<0.001). Zymogramanalysen visade en skillnad i enzymaktivitet som var mest uttalad i banden för molekylär vikt runt 255 kDa, analogt med komplex av matrixmetalloproteinas-9. Sammanfattningsvis har det i denna studie påvisats att grönt te har en hämmande effekt på kaseinolytisk aktivitet och en mindre, mer specifik, hämmande effekt på gelatinasaktivitet.
Chronic periodontitis involves tissue destruction by matrix metalloproteinase, derived from the host cells, as part of the immunological response to bacterial virulence factors. Green tea has been studied for its health promoting properties, which includes anti-inflammatory effects. The effect is in part due to enzyme inhibition by tea polyphenols. The aim of this study was to further investigate the inhibitory effect of green tea, focusing on enzymatic activity in gingival crevicular fluid from patients with periodontal disease. Patients with chronic periodontitis were selected for participation in the study. Gingival crevicular fluid was extracted with micropipettes from the gingival sulci of the patients. Samples were treated with green tea and compared with untreated samples from the same subject. Fluorescence protease assay with casein as substrate was made using fourteen samples for detecting differences in caseinolytic activity. Zymogram assay using gelatin as substrate was done using four samples to test gelatinolytic activity and analyse molecular weights of the different enzymes. The fluorometric assay showed a significantly lower enzyme activity in samples mixed with green tea than untreated samples (p<0.001). The zymogram assay showed a difference in band strength which was most pronounced in the bands of molecular weight around 255 kDA, analogous to complexes of matrix metalloproteinase-9. have been shown toproduce antimicrobial and anti-inflammatory
effects when used to treatchronic inflammatory airway diseases.
Azithromycin produces potentinhibition of <i>Aggregatibacter
actinomycetemcomitans</i>,
<i>Porphyromonasgingivalis</i> and <i>Prevotella
intermedia</i>, and previous studies suggestthat azithromycin
improves non-surgical periodontal therapy outcomesin patients with
chronic and aggressive periodontitis or drug-inducedgingival
enlargement. Macrolides distribute to gingiva at
concentrationshigher than those found in blood serum. They are
taken up and accumulatedby
gingival fibroblasts, epithelial cells
and phagocytes, which serve asreservoirs that buffer and sustain
their therapeutic concentrations inthe gingiva. It is unclear
whether macrolides produce anti-inflammatoryeffects in the gingiva,
but it is feasible that azithromycin can decreasethe rate of
gingival crevicular fluid (GCF) flow by decreasing the levelof
inflammatory mediators in GCF. Methods: Ten
healthy volunteersin good periodontal health were administered an
initial 500 mg dose ofazithromycin followed by 250 mg doses 24 and
48 hours later. GCF sampleswere collected from twelve maxillary
posterior interproximal sites onday 0 (prior to the first dose of
azithromycin) and on days 2, 4, 7,14 after the first dose. GCF
samples were collected with paper stripsfor 30 seconds and measured
with a
gingival fluid meter. The paperstrips were pooled and stored
in sealed vials in liquid nitrogen. At the time
for cytokine analysis, GCF samples were eluted from paperstrips in
200 µl of Hanks balanced salts solution. A commerciallyavailable
multiplex cytokine immunoassay was used to measure the contentof
IL-1β, IL-8, TNF-α, VEGF, IL-6 and
IL-10. Results:Azithromycin had significant
treatment effects on pooled GCF volume aswell as the amount of
IL-1β, VEGF, TNF-α and IL-8 measuredin GCF over time (P <
0.005). Significant decreases in pooled GCFvolume were evident on
days 2, 4 and 7 (P < 0.05). GCF volume increasedafter day 7 and
was not significantly different from baseline on day14.
Azithromycin triggered a significant decrease in GCF IL-1βcontent
relative to baseline on days 2, 4, 7 and 14. In comparisonto
baseline levels, VEGF content was significantly lower on days 2,4
and 7, while TNF-α content was significantly lower on days 4and 7.
IL-8 content decreased from baseline levels on days 2, 4 and 7,but
the difference was statistically significant only on day 4. IL-6
andIL-10 were not detected in GCF samples obtained from healthy
gingiva inthis population. The median
Gingival Index and Plaque
Index values were0 throughout the study
period. Conclusions: Azithromycin
inducessignificant decreases in GCF volume and biological mediator
contentat healthy periodontal sites that were essentially free of
bacterialplaque. This suggests it has potent anti-inflammatory

▼ As metaloproteinases da matriz (MMPs) foram identificadas na saliva, na placa dental, na dentina e no cemento. Este trabalho teve como objetivos: Estudo I (I) - avaliar a expressão de MMP-2 e MMP-9 presentes na saliva total e parotidiana e no fluido gengival crevicular (FGC) de pacientes portadores e não portadores de lesões cervicais não cariosas (LCNC); Estudo II (II) - investigar se a presença de MMP-8 e -9/TIMPs poderia influenciar a remineralização de lesões artificialmente criadas na superfície radicular, com ou sem desgaste por abrasão. Os métodos utilizados foram: (I) Coleta de amostras de saliva e do FGC de 32 pacientes, com (n=16) e sem LCNC (n=16). A atividade gelatinolítica das MMPs foi avaliada através de análise zimográfica e Western Blot. (II): Espécimes de dentina humana radicular foram obtidos. O grupo controle G1(10) não sofreu nenhum tratamento. Os demais segmentos radiculares foram desmineralizados G2(60). O Grupo A não foi submetido à escovação e o Grupo B foi submetido à abrasão por escovação em uma máquina de escovação simulada. G2(10) foi apenas desmineralizado, G3(10) desmineralizado e remineralizado, e os Grupos G4(10), G5(10), G6(10), G7(10) foram desmineralizados e remineralizados em presença de tampão neutro, TIMP, MMP-8 e -9, MMP-8,-9 e TIMP, respectivamente. Para a análise elemental, as concentrações de Ca+2, P, Mg+2 assim como a relação molar Ca/P e Mg/Ca foram determinadas através de uma sonda eletrônica para microanálise (EPMA). A análise qualitativa por retrodispersão (BSE) foi realizada para demonstrar a distribuição global da densidade mineral. Os resultados (I) mostraram que a principal gelatinase presente, tanto na saliva total quanto no FGC, é a proMMP-9. Na saliva secretada pela glândula parótida, não foram detectadas bandas indicando a presença de gelatinases. Os resultados do estudo (II) indicaram que os espécimes escovados apresentaram maior conteúdo de Ca+2 a 20µm e maior conteúdo de Mg+2 a 30 e 50µm. Em presença de TIMPs, ocorreu uma redução do conteúdo de Ca+2 a 20µm. Para os espécimes não escovados, em todas as profundidades, as amostras incubadas com MMPs apresentaram maiores valores de Ca+2. Portanto, pode-se concluir que (I) a comparação entre pacientes com e sem LCNC mostrou não haver diferença estatisticamente significante quanto à atividade gelatinolítica; (II) quando não inibidas pelos TIMPs, as MMPs degradaram o colágeno completamente desmineralizado na superfície radicular, permitindo melhor recalcificação na superfície subjacente. Esse fenômeno foi também facilitado pela abrasão por escovação.

O reconhecimento da presença de recetores específicos para as hormonas sexuais,
estrogénio e progesterona, na gengiva humana, fez com que o tecido periodontal
passasse a ser considerado alvo potencial para as variações hormonais as quais, por sua
vez, passaram a constituir possíveis fatores modificadores da resposta inflamatória
inerente ao hospedeiro quando do estabelecimento da doença periodontal. Para tanto,
evidencia-se o papel das hormonas sexuais na vascularização tecidual, na resposta
imunitária, no perfil funcional das células próprias do periodonto, assim como, no
reparo tecidual e no metabolismo ósseo. Diante desses aspectos, em razão da vasta
utilização de contracetivos orais como método hormonal preferencial das mulheres no
planeamento familiar, realizou-se revista na literatura para que fossem coligidos os
dados mais relevantes acerca do impacto que a administração de produtos sintéticos, a
base de estrogénio e progesterona, podem ter sobre o tecido periodontal. Foi realizada,
assim, pesquisa na base de dados PUBMED, Sciene Direct, Bireme, Scielo, B-on e
Google Académico, sem que limite temporal fosse estabelecido. Na pesquisa também
foram utilizadas algumas obras literárias. Do referido trabalho, concluiu-se que as
mulheres que utilizam contraceptivos orais, apresentam um aumento de perda de
inserção clínica e rápida progressão da doença periodontal, decorrente do aumento da
concentrações hormonais nos tecidos subgengivais, da existência de placa bacteriana,
bem como, da sua utilização prolongada. The recognition of the existence of specific hormone receptors, estrogen and
progesterone, in the human gingiva, has placed the periodontal tissue as a potencial
target to hormonal variations, which became possible modifier factors inherent to the
host’s inflammatory response when periodontal disease sets. Accordingly, the role of
sex hormones in vascular tissue, the immune response, the functional profile of the
periodontum’s own cells as well as in tissue repair and bone metabolism, are
highlighted.
Considering these aspects and due to the widespread use of oral contraceptives as a
preferential method used by women in family planning, a literary review was held in
order to collect the most relevant data about the impact that the administration of
syntethic products, the basis of estrogen and progesterone may have on periodontal
tissue.
Hereby, a research was made in PUBMED, Science Direct, Bireme, Scielo, B-on and
Google Académico, without any time limits. In addition, some literary works were also
used in this research. In this study it has been concluded that, women who use oral
contraceptives have an increased loss attachment and quick progression of the
periodontal disease, due to the increase of hormonal concentrations in subgengival
tissues,, the existence of bacterial plaque as well as its prolonged use.

▼ It is well established that biofilm formed on tooth surfaces is the major culprit for caries and periodontal disease development. The critical early event of biofilm formation is the specific interaction of microorganisms with the acquired enamel pellicle. This pellicle is formed from adsorbing proteins from saliva, and perhaps also from gingival crevicular fluid. The purpose of this project was two fold: first, to investigate the extent of serum protein adsorption in a salivary protein environment, and second, to investigate the colonization of salivary pellicles by early microbial colonizers. Hydroxyapatite (HA) was incubated with saliva and serum and adsorbing proteins were identified by LC-ESI-MS/MS. To investigate competition among salivary and serum proteins for adsorption to HA proteins were labeled with specific CyDyes, mixed in various ratios and incubated with HA. The early phase of oral biofilm formation in vivo was studied on teeth exposed to the oral environment. The harvested biofilm samples were analyzed with the Human Oral Microbiome Identification Microarray containing 407 different microbial probes. In the pure saliva- and serum-derived pellicles eighty-two and eighty-four proteins were identified. Concomitant presence of salivary and serum proteins showed that salivary protein adsorbers effectively competed with serum proteins adsorbers for the HA surface. Specifically acidic proline-rich protein, cystatin, statherin and amylase proteins in saliva competed off apolipoprotein, C-reactive protein, peroxiredoxin-1 and albumin. In vivo evidence supported the replacement of serum proteins by salivary proteins. The studies with oral biofilm formed in vivo led to the identification of 92 species with streptococci being the most abundant early colonizers. High frequency detection was furthermore made with Haemophilus parainfluenzae, Gemella haemolysans, Slackia exigua and Rothia species. Eight uncultivated phylotypes were detected. While there is a significant amount of serum protein emanating from the gingival sulcus, their ability to participate in dental pellicle formation is likely reduced in the presence of strong salivary protein adsorbers. Furthermore, the early pellicle colonizers exhibit considerable bacterial diversity and include non-cultivable species. These findings will be helpful in designing target-specific approaches for the prevention of and/or intervention in diseases exhibiting an oral-biofilm-based etiology.

▼ Background: The role of cytokines in the pathogenesis of
periodontal disease and the effect of smoking on these mediators
has been reported. However, findings have been variable and
simultaneous measurement of multiple cytokines has been limited.
This study utilized a quantitative multiplex assay to measure a
comprehensive panel of Th1, Th2, and pro-inflammatory cytokines
and chemokines (including several novel cytokines) in
gingival
crevicular fluid (GCF) in chronic periodontitis subjects. The
impact of cigarette smoking on these GCF mediators was also
assessed.
Methods: Forty subjects (age 40-75 years) with generalized severe
chronic periodontitis (20 smokers and 20 non-smokers) and 12
periodontally healthy controls participated in the study. GCF
was collected from four sites in the diseased groups: 2 diseased
sites [(probing depth (PD) and clinical attachment level (CAL)
≥5mm with bleeding on probing (BOP)] and 2 healthy sites (PD and
CAL ≤3mm, no BOP); 2 healthy sites were sampled in the healthy
controls. The volumes of the GCF samples were measured and the
GCF mediators assessed in duplicate utilizing a multiplex
immunoassay (Luminex). Intragroup, intergroup and pooled
comparisons were performed using non-parametric tests including
the Mann-Whitney and the Wilcoxon matched-pairs signed-rank
test.
Results: GCF in diseased sites (vs. controls) contained
significantly (p<0.05) higher amounts of IL-2, IFN-γ,
IL-3, IL-4 (Th1/Th2 cytokines); IL-1α, IL-1β, IL-6, IL-12 (p40)
(pro-inflammatory cytokines); IL-8, MIP-1, RANTES (chemokines);
and IL-15 (regulator of T-cells and NK cells). IP-10 was the
only mediator exhibiting lower levels (p<0.0005) in
diseased sites compared to controls. Smoking had an inhibitory
effect (p<0.05) on pro-inflammatory cytokines (IL-1α,
IL-6, IL-12 (p40)); chemokines (IL-8, IP-10, MCP-1, MIP-1,
RANTES) and regulators of T-cells and NK cells (IL-7, IL-15) in
comparison to sites within non-smokers. Interestingly, smokers
had elevated GCF levels (p<0.05) of IL-1α, IL-1β and IL-3
relative to sites in healthy controls.
Conclusions: Similar to previous reports, periodontitis subjects
had significantly elevated cytokines and chemokines compared to
healthy controls. Smokers exhibited a decrease in several
pro-inflammatory cytokines, chemokines and regulators of T-cells
and NK-cells as compared to nonsmokers however; little influence
was observed on Th1/Th2 cytokines. Interestingly, smokers
exhibited decreased amounts of GCF IL-7, IL-12(p40), IL-15,
IP-10, MCP-1, MIP-1 and RANTES, which calls for future
investigation. The multiplex comprehensive assay used in this
study to assess cytokines in a single GCF sample is a
