Doxycycline (DC) are used
worldwide as locally applied adjuncts in the treatment of
periodontal diseases. As a group the tetracyclines are well known
for their advantageous properties. There is however possible
cytotoxicity towards cells in the area of application. This study
determined the minimum cytocidal concentration of MC and DC on the
growth and proliferation of Human Periodontal Ligament Fibroblasts
(HPLF) in vitro. This was facilitated by growing cells (PDL1 and
PDL2) in the presence of MC and DC in media in 96 tissue wells
starting at a concentration of 1400 µg.ml-1 (100%). Serial
dilutions of the MC and DC at 10% increments were investigated in
order to detect significant HPLF cell growth inhibition. The
significant LD50 was further determined at one percent increments
in order to arrive at a specific percentage value. The results were
read as LD50 values from growth concentration curves. The LD50 of
MC on PDL1 and PDL2 after one hour exposure was 686 µg.ml-1 and 896
µg.ml-1 respectively while for DC it was 252 µg.ml-1 and 546
µg.ml-1. The LD50 of MC on PDL1 and PDL2 after 24 hour exposure was
196 µg.ml-1 and 266 µg.ml-1 respectively while for DC it was 252
µg.ml-1 for both. The LD50 of MC on PDL1 and PDL2 after 48 hour
exposure was 252 µg.ml-1 and 182 µg.ml-1 respectively while for DC
it was 154 µg.ml-1 and 168 µg.ml-1. Based upon the LD50 values this
study found that DC is more cytotoxic than MC and linked to this,
the two cell lines reacted slightly differently. The concentrations
MC and DC tested in this study did however not influence growth of
HPLF significantly.

Doxycycline (DC) are used worldwide
as locally applied adjuncts in the treatment of periodontal
diseases. As a group the tetracyclines are well known for their
advantageous properties. There is however possible cytotoxicity
towards cells in the area of application. This study determined the
minimum cytocidal concentration of MC and DC on the growth and

▼ During a ten week period the clinical picture caused by an Ehrlichia canis infection in fifty dogs was investigated and the clinical signs and current ideas about diagnostics and therapies were described on the island of Curaçao.
All the dogs were examined, a blood sample was taken, as many ticks as possible were removed and the dog owners filled in a questionnaire. The clinical signs had a wide variety and were often non-specific. Most common clinical signs were anorexia and lethargy, followed by a little more specific signs like pale mucous membranes, lymphadenopathy, fever and tick infestation.
Thirty-two of the fifty dogs were tested for hematological abnormalities of which 84% had thrombocytopenia, 69% had anemia and 3% had leucopenia.
The Snap ® 3Dx ® test was performed on 42 dogs, with an positive test result for 30 dogs and a negative test result for 12 dogs. Not all dogs diagnosed with ehrlichiosis showed antibody titers against E. canis.
Treatment for the infected dogs was started with
doxycycline hyclate (5-10- mg/kg/day) for a period of one to three weeks. Additional treatment with Imidocarb dipropionate , 6.6 mg/kg (Imizol®) in combination with atropine-sulfate 0,03-0,1 mg/kg (Atropine®) against possible side effects, repeated after two weeks for a total of two treatments was given to two dogs and prednisolone-acetate 0,5-4 mg/kg/day (Prednisolon®) was given to one dog.
A clinical and hematological check-up was performed on ten out of the fifty dogs (22%), whereas all owners were requested to come back after treatment had finished.
Three out of the ten dogs needed prolonged treatment, because they were still suffering from anemia and/or severe thrombocytopenia.

▼ Les militaires français sont engagés depuis 2002 dans des opérations de lutte contre l’orpaillage illégal en Guyane. Malgré un programme de prévention du paludisme dimensionné et organisé, ces opérations se sont accompagnées d’une augmentation de l’incidence du paludisme. Cette thèse a pour objectif d’identifier les déterminants humains, vectoriels et parasitaires du paludisme en Guyane. La première partie de cette thèse permet de mieux décrire le faciès épidémiologique du paludisme en forêt guyanaise. Nous avons clairement établi le lien entre paludisme et orpaillage illégal. Une étude, menée en milieu orpailleur a démontré l’existence de foyers de paludisme hyperendémiques, avec une prédominance de P. falciparum et un risque d’émergence de résistance aux traitements combinés à base d’artémisinine. Les investigations entomologiques ont permis de confirmer le rôle d’Anopheles darlingi comme vecteur majeur du paludisme en forêt mais aussi l’existence d’une activité de piqûre diurne sous la canopée. Elles ont permis en outre de mieux identifier les autres vecteurs secondaires en forêt : An. nuneztovari et An. triannulatus, mais surtout An. ininii et An. marajoara. En seconde partie, une étude de cohorte en population militaire a permis d’estimer l’exposition au paludisme par l’utilisation d’outils sérologiques. La distribution des preuves sérologiques d’une infection palustre était en faveur d’une exposition plus fréquente à P. falciparum. Les taux d’incidence des infections étaient de 40 pour 100 personnes années que ce soit pour P. falciparum ou P. vivax. Le seul facteur protecteur était l’observance parfaite de la chimioprophylaxie par doxycycline.
Since 2002, French armed forces are involved in operations against illegal gold mining in French Guiana. Despite a properly dimensioned prevention program against malaria conducted, a drastic increase of malaria incidence has been observed among military personnel. The first part of this thesis aimed to describe the determinants of malaria in French Guiana forest, including human, vector and parasite features. We have established the link between malaria among military personnel and illegal gold mining sites. A study conducted among gold miners’ population has shown hyperendemic malaria foci in the rain forest, mainly due to P. falciparum, and highlighted a real risk for emergence of artemisinin resistance. Entomologic investigations supported that Anopheles darlingi is the main vector for malaria in rain forest. In addition, we have demonstrated the existence of daytime biting activity by An. darlingi in the forest, which might play a key role in malaria epidemic outbreaks among military personnel. Other sylvatic vectors were identified, as An. nuneztovari and An. triannulatus, but especially An. ininii and An. marajoara. ▼ La doxycycline est l’une des molécules recommandées par l’OMS en prophylaxie pour les voyageurs dans les zones d’endémie palustre, en particulier dans les zones de multirésistance. Une étude récente avait suggéré que les isolats de P. falciparum présentaient différents niveaux de sensibilité à la doxycycline et que l’augmentation du nombre de copies de deux gènes, pfmdt ou pftetQ, pouvait être associée à une baisse de sensibilité.Le premier objectif de ce travail a consisté à valider ce modèle à partir d’un nouvel échantillonnage d’isolats africains. Le second objectif était d’évaluer le nombre de copies de ces deux gènes sur des isolats originaires de Thaïlande. Le troisième objectif a consisté à rechercher d’autres sources de résistance en investiguant le polymorphisme des gènes codant l’ARN ribosomal plasmodial potentiellement impliqués dans la résistance in vitro à la doxycycline.Les résultats nous ont permis de confirmer que le nombre de copies des gènes pfmdt ou pftetQ pouvait être impliqué dans la résistance in vitro à la doxycycline en Afrique. Les résultats concernant les isolats Thaï n’ont pas permis de corréler le nombre de copies des gènes pfmdt et pftetQ au phénotype CI50. Ces éléments montrent que ce mécanisme de résistance seul est insuffisant pour expliquer la résistance à la doxycycline ; les résultats sont en faveur d’une résistance médiée par plusieurs gènes.La recherche de points de mutation sur le gène pfssrRNA codant pour la petite sous-unité ribosomale de l’ADN plasmodial n’a pas abouti. D’autres cibles moléculaires sont en cours d’étude pour expliquer les mécanismes de résistance de P. falciparum à la doxycycline.
Doxycycline is currently one of the recommended chemoprophylactic regimens for travellers visiting malaria-endemic, particularly in countries with a high prevalence of resistance to chloroquine and multiresistance. A previous study suggested that increased pfmdt or pftetQ copy number could be associated with a lower susceptibility to doxycycline.The first aim of this study was to validate the pre-established model involving these two molecular markers with other African isolates. The second was to evaluate these markers in P. falciparum isolates coming from a multiresistance area in Thaïland. The third was to investigate the eventual association between the polymorphism in genes encoding ribosomal rRNA and in vitro resistance to doxycycline.The results confirm that pfmdt or pftetQ copy numbers should be involved in in vitro susceptibility to doxycycline in African P. falciparum isolates. The results concerning the Thai isolates indicate that there is no correlation between the pfmdt and pftetQ genes copy numbers and the belonging to the high doxycycline IC50 phenotype; this implies that this mechanism of resistance is not enough by itself to explain resistance to doxycycline; it augurs that the resistance to doxycycline should be controlled by multiple genes, and that these genetic markers could be continent-dependent. Para avaliar a capacidade da doxiciclina em prevenir ou reverter o comportamento tipo-depressivo induzido pela administraÃÃo sistÃmica de LPS, esta foi administrada nas doses de 25 ou 50 mg/kg, i.p. 30 min antes de LPS (prÃ-LPS) ou 1,5 e 23,5 horas apÃs a LPS (pÃs-LPS). A imipramina foi utilizada como antidepressivo padrÃo nas mesmas condiÃÃes de tempo. Em ambas as situaÃÃes, prevenÃÃo (prÃ-LPS) e tratamento (pÃs-LPS), o comportamento dos animais foi avaliado 24 horas apÃs a administraÃÃo de LPS, um perÃodo conhecido pela ocorrÃncia de um comportamento tipo-depressivo. Os nÃveis de citocinas (IL-1β e TNF-α) e nitrito foram avaliados no sangue (plasma) e as Ãreas cerebrais: cÃrtex prÃ-frontal (PFC), hipocampo (HC) e corpo estriado (ST). A administraÃÃo de LPS, 0,5 mg/kg aumentou significativamente o tempo de imobilidade em comparaÃÃo com os animais controle, enquanto que a doxiciclina, nas doses de 25 e 50 mg/kg e imipramina (10 mg/kg) foram capazes de prevenir e reverter a imobilidade induzida pelo LPS. A doxiciclina e imipramina, quando administrados prà e pÃs-LPS reduziram significativamente o tempo de imobilidade, mostrando um efeito antidepressivo. Em relaÃÃo a citocina IL-1β, seus nÃveis foram diminuÃdos, enquanto os nÃveis de TNF-α nÃo foram alterados significativamente. A doxiciclina e imipramina, preveniram e reverteram a diminuiÃÃo dos nÃveis de nitrito induzido por LPS. Com base nos resultados do presente estudo, avaliando o uso da doxiciclina, sugere-se que este antimicrobiano possa atuar como um antidepressivo.

▼ A Pilocarpina à um agonista colinÃrgico caracterÃstico por induzir convulsÃes que evoluem para status epilÃpticus, similar à epilepsia do lobo temporal humana. Neste presente trabalho, nÃs avaliamos a possÃvel aÃÃo neuroprotetora da doxiciclina, uma tetraciclina de segunda geraÃÃo, nas convulsÃes induzidas pela pilocarpina em ratos Wistar machos, que receberam pilocarpina (300mg/kg i.p) presenÃa ou na ausÃncia de doxiciclina (25 à 100 mg/kg) administrada intraperitonealmente uma vez ao dia durante sete dias. ApÃs a injeÃÃo de pilocarpina, foram observados os sinais colinÃrgicos perifÃricos, as latÃncias de 1 convulsÃo e de morte. Foram determinadas as concentraÃÃes de aminoÃcidos no cÃrtex temporal atravÃs de cromatografia lÃquida de alta eficiÃncia HPLC, e a atividade do sistema antioxidante, catalase e as dosagens dos nÃveis de TBARS e nitrito. Os resultados mostraram que a doxiciclina nÃo alterou os sinais colinÃrgicos perifÃricos, contudo aumentou a latÃncia decorrida para a primeira convulsÃo (1.6 a 5 vezes), quando comparada ao grupo (P300). Resultados semelhantes foram demonstrados com a latÃncia de morte, que foi aumentada de 1.9 a 9.9 vezes. Observou - se que o prÃ-tratamento com doxiciclina 50 mg/kg foi capaz de reduzir em 25% os nÃveis de MDA, 64% os nÃveis de nitrito e 67.7% a atividade da catalase no cÃrtex temporal desses animais, demonstrando com clareza seu potencial antioxidante. Interessantemente, a doxiciclina diminuiu as concentraÃÃes de glutamato de 28 a 33%, e aumentou GABA em 112 e 91%, nas dose de 50 e 100mg/kg respectivamente nos animais administrados com P300, Na maior dose, a droga alterou os nÃveis de aspartato e taurina, diminuindo em 61% aspartato enquanto elevou os nÃveis de taurina em cerca de 34%. Surpreendentemente, somente a menor dose alterou os nÃveis de glicina, aumentendo a concentraÃÃo deste aminoÃcido em 132%. Em conclusÃo, mostramos que o inÃcio e a intensidade das convulsÃes induzidas pela pilocarpina foram significativamente reduzidos pela doxiciclina. Portanto, pelo menos em parte, este mecanismo de aÃÃo parece estar relacionado a uma diminuiÃÃo nos nÃveis de aminoÃcidos excitatÃrios e a um aumento nas concentraÃÃes de aminoÃcidos inibitÃrios no cÃrtex temporal desses animais.
Pilocarpine is known to induce convulsions leading to status epilepticus, similar to the temporal lobe epilepsy in humans. In the present work, we evaluated the possible protection affored by doxycyvline, a 2nd generation tetracycline, agaist pilocarpine- induced convulsions in male Wistar rats (P300mg/kg, i.p) in the absence and in the presence of doxicycline (25 to 100 mg/kg i.p.) daily for 7 days.After the pilocarpine injection, all groups were observed for cholinergic signs, latency to the first convulsion and latency to death. ▼ O aumento no consumo de antibióticos por seres humanos e animais tem elevado a sua concentração na sua forma inalterada ou de seus metabólitos, que chegam às estações de tratamento de efluentes, onde os tratamentos convencionais são incapazes de degradar estas moléculas, sendo liberadas diretamente nos corpos d\'água receptores. A liberação destas moléculas no meio ambiente tem proporcionado a seleção de organismos patogênicos resistentes, capazes de transmitir geneticamente esta característica a seus descendentes. Este trabalho tem como objetivo o estudo da degradação da molécula de cloridrato de doxiciclina, por ser um antibiótico de largo espectro da família das tetraciclinas, através do processo Fenton. Foram determinados neste trabalho as influências da temperatura, concentração inicial de peróxido de hidrogênio, concentração inicial de íon ferroso e do pH sobre a concentração residual de cloridrato de doxiciclina, concentração residual de peróxido de hidrogênio, concentração de íon ferroso e de carbono orgânico total (COT) em função do tempo de reação. Os métodos analíticos empregados foram baseados em espectrofotometria, análise instrumental para a determinação do carbono orgânico total (COT), titulometria e cromatografia líquida (CLAE) com detectores de UV e de massa. Testes preliminares mostraram que as melhores condições operacionais de temperatura, concentrações de íon ferroso e de peróxido de hidrogênio estariam ao redor de CFe2+ = 62,5 mg/L, CH2O2 = 500 mg/L e T = 20ºC. Estas condições foram utilizadas como ponto central do planejamento fatorial do tipo Delineamento Composto Central Rotacional (DCCR). Os resultados do planejamento tratados com o software Statistica® mostraram que as condições operacionais para a menor concentração residual de fármaco (0 mg/L) e redução de 40,9% da carga orgânica em solução deveriam estar ao redor de concentração de íon ferroso, CFe2+ = 25 mg/L, concentração de peróxido de hidrogênio, CH2O2 = 611 mg/L e temperatura = 35ºC, das quais a variável CH2O2 apresentou a maior importância estatística. Ao redor destas condições foi feita uma análise paramétrica para se verificar as influências individuais da temperatura, CFe2+, CH2O2 e da relação CFe2+/ CH2O2. Também foi testada a citotoxicidade dos resíduos gerados. Este estudo mostrou que a variável de maior importância sobre o processo foi a concentração de peróxido de hidrogênio, que tem relação direta com a razão CFe2+/ CH2O2 apresentado melhores resultados quando esta é mantida igual a 0,16. Houve melhora acentuada na mineralização da matéria orgânica com a inserção de microbolhas de ar, obtendo assim redução de até 44% da matéria orgânica em relação à concentração inicial de carbono orgânico de 55 mg/L. ▼ Asphyxia is a significant cause of newborn morbidity
and mortality. Multiorgan injury and dysfunction is a common
finding, mediated in part through an increase in matrix
metalloproteinase (MMP) activity. We hypothesized administration of
doxycycline, a known MMP inhibitor, would improve systemic and
regional hemodynamics as well as attenuate myocardial, renal and
intestinal injury and dysfunction in a clinically translatable
newborn swine model of hypoxia-reoxygenation. Newborn piglets were
subjected to hypoxia-reoxygenation and received normal saline
(control) or one of three doses of doxycycline five minutes into
resuscitation. Doxycycline improved recovery of cardiac index,
stroke volume index, systemic arterial pressure (SAP), systemic
oxygen delivery and consumption with no effect on heart rate.
Pulmonary artery pressure (PAP) and PAP/SAP were reduced, while
renal artery flow index and oxygen delivery were improved. Markers
of myocardial, renal and intestinal injury were attenuated with
doxycycline and associated with a reduction in tissue oxidative
stress and total MMP-2 activity. We conclude that in a clinically
translatable newborn swine model of hypoxia-reoxygenation, early
administration of doxycycline during resuscitation improves
systemic and regional hemodynamics, and attenuates myocardial,
renal and intestinal injury.

▼ Transthyretin (TTR)-associated amyloidoses are diseases wherein wild-type or mutant TTR forms amyloid fibrils that infiltrate multiple organs. Wild-type TTR amyloidosis, ATTRwt, is a sporadic disease characterized by deposits that occur mainly in the heart. Alternatively, >100 TTR mutants cause inherited forms, ATTRm, frequently featuring cardiac amyloid deposits.
The goals of this research were to create a cell-based model of ATTR amyloidosis, to define the mechanism of cardiac TTR-associated amyloid at the cellular level, and to study several agents that could interrupt the amyloid process. We hypothesized that TTR oligomers were cardiotoxic and played a role in the mechanism of ATTR amyloidosis, and that cytotoxicity could be inhibited by diflunisal, doxycycline, and Kiacta®. Focusing on TTR proteins associated with cardiac amyloidosis (wild-type, L55P, V30A, and V122), we developed a thermal denaturation method for creating TTR oligomers that allowed us to study the direct effect of oligomers on cells. Congo red and thioflavin T analyses confirmed that the oligomers were on pathway to amyloid fibril formation. We tested the effect of TTR oligomers on rat and human cardiac cells by measuring cell viability and stress response (through live protease activity and qPCR). TTR-L55P oligomers elicited a cytotoxic effect; fluorescent microscopy indicated cellular uptake of the oligomers and continued intra-cellular aggregation. Cytotoxicity was blocked when TTR was heated in the presence of doxycycline; the drug appeared to dissociate TTR aggregates or stabilize the monomeric forms. We also investigated retinol-binding protein (RBP), a natural binding partner of TTR. By immuno-histochemistry, RBP was demonstrated in ATTRwt and ATTRm `non-amyloid' transplant heart tissues, localized to areas containing amyloid or in the case of the transplant tissue, regions that appeared to display ischemic damage. Serum RBP levels were significantly different in ATTR vs. age-matched controls (p = 0.03), and in ATTRwt vs. ATTRm (p <0.0001) by ELISA.
These data provide evidence that TTR oligomers are cardiotoxic, possibly due to cellular internalization and progressive intracellular aggregation. Furthermore, our results support the use of doxycycline as a therapeutic in ATTR to target these amyloidogenic oligomers, and suggest that RBP may have potential as a disease biomarker.

▼ The evolution of pest resistance to Cry proteins threatens the long-term use of Bt crops.
Busseola fusca developed resistance to Bt maize in South Africa but the mechanism of
resistance is not well understood. According to the gut microbiota theory, extensive cell lysis
caused by Cry proteins provide gut microbes access to the more favourable environment of
the hemocoel where they germinate and reproduce, causing septicemia and death of the
host. This theory brought on questions about the role of gut microbes in the efficacy of Bt
maize against target lepidopteran pests. The aim of this study was to determine whether
microbes present in the mid-gut of B. fusca influence the efficacy of Cry 1Ab proteins.
Larvae were collected from 30 different geographical locations, dissected to excise the midgut
and mid-gut content which was separated according to morphological types. The
morphological types were used to test the antibiotic susceptibility of the bacteria and proved
that ciprofloxacin, ampicillin and doxycycline were the most effective bacteriostatic and
bactericidal antibiotics. These three antibiotics were exposed to the morphological types at
different concentrations to visualise the possible deleterious effects of the antibiotics on the
bacteria. This visualisation was performed by observing the growth curve of the bacteria in
the presence of the combination of antibiotics. The antibiotics concentration of 500 μg/ml
showed the highest efficacy compared to the other concentrations tested. An antibiotic
concentration of 500 μg/ml of ciprofloxacin, ampicillin and doxycycline was incorporated into
an artificial diet for the larvae to feed on for 7 days. This method was used to rid the larvae of
gut microbes before allowing them to feed on Bt maize (MON810) plant material expressing
Cry proteins. The results suggests that by placing antibiotic reared larvae on a Bt plant, the
absence of the mid-gut microbes contributed to larvae survival on Bt maize. This observation
will contribute to understanding the role of gut microbes on the efficacy of Cry proteins.

▼ Substandard antibiotics are thought to be a major threat to public health in developing countries and a cause of antimicrobial resistance. However, assessing quality outside of a laboratory setting, using simple equipment, is challenging. The aim of this study was to validate the use of a portable Fourier transform infrared (FT-IR) spectrometer for the identification of substandard antibiotics. Results are presented for amoxicillin, ciprofloxacin, and doxycycline packages from Haiti, Ghana, Sierra Leone, Democratic Republic of Congo, India, Papua New Guinea, and Ethiopia collected over the course of 6 months in 2017, including two field trips with the FT-IR to Ghana and Sierra Leone. Canadian samples were used as a control. Regarding drug quality, of 290, 280, and 42 individual capsules and tablets of amoxicillin, ciprofloxacin, and doxycycline analyzed, 13, 14, and 27 respectively were found to be substandard with total active pharmaceutical ingredients (API) lying outside the acceptable range of 90–110%. The FT-IR reliably identified the outliers of amoxicillin and ciprofloxacin and was found to yield results in good agreement with the established pharmacopeia liquid chromatography protocol. Results for doxycycline were less promising, but LC-UV analysis was also found to be problematic. We conclude that the portable FT-IR may be suitable to intercept substandard antibiotics in developing countries where more sophisticated techniques are not readily available.

concern. WHO estimated that 10 % of the medicines in the market are of poor quality. One
major way to circumvent such problem is development and deployment of rapid,
economical and efficient analytical methods to check the quality of drugs. Hence the aim of
this research is to develop HPTLC method for the determination of
Doxycycline hyclate
pharmaceutical formulations.
A rapid and simple high-performance thin layer chromatography (HPTLC) method with
densitometry at 360 nm was developed and validated for determination of
doxycycline
hyclate in capsule formulation. Chromatography was performed on glass plates coated with
C18 Reverse phase silica gel 60 F 254 and pretreated with 0.27 M EDTA solution; the
mobile phase was dichloromethane: methanol: acetonitrile: 1% aqueous ammonia in the
ratio of 10:22:53:15 (v/v). The linearity range lies between 200 and 1000 ng/spot with
correlation coefficient of 0.997. The R
f
value is 0.5 ± 0.02 %. Recoveries were in the range of
94.50 % – 100.5 %. Limit of detection and imit of quantitation values for
doxycycline
hyclate were 40 ng/spot and 160 ng/spot respectively. The developed method was
validated as per ICH guidelines. In forced degradation study
doxycycline was found to
degrade in acidic, alkaline, and oxidative stress conditions and the drug was found to be
relatively stable in heat and photo degradation (5 hr durations), the maximum degradation
occurred under alkaline stress condition. The result in forced degradation study shows the
specificity of the developed method. The proposed method was successfully applied for the
routine quantitative analysis of dosage forms containing
doxycycline hyclate. Five brands
of tablets and capsules tested were in compliance with the USP requirements with respect
to assay. The Developed method offered comparable results (as confirmed by F-test) with
that of the HPLC pharmacopoeial analysis method. The method also offered efficiencies in
terms of time and cost.

▼ Rotator cuff tendon and muscle can beneficially adapt to exercise, but it is unknown how acute responses following a single bout of exercise lead to chronic, beneficial adaptations. To develop new prevention and treatment strategies for overuse injuries, we must first characterize how tissues adapt to non-injurious loading. One feature that may distinguish beneficial and detrimental responses are pathways associated with inflammation and tissue healing, including activation of the arachidonic acid cascade and matrix turnover facilitated by matrix metalloproteinases (MMPs). The objective of this dissertation was to characterize temporal, acute and chronic responses of the rat supraspinatus to non-injurious exercise and examine the roles of cyclooxygenases-1 and -2 (arachidonic acid cascade components) and MMPs in governing these responses by using pharmacologic inhibition. Using a rat model of non-injurious exercise, we showed that tendon adapted chronically by enhancing its mechanical properties. Following a single bout of exercise, tendon showed mild trends toward reduced mechanical properties, which may initiate the beneficial adaptations detected chronically. Exercised tendon also had decreased MMP activity. We administered ibuprofen, an inhibitor of cyclooxygenases-1 and -2, to understand how this commonly used over-the-counter drug affects muscle and tendon responses to exercise. Ibuprofen did not detrimentally affect tendon mechanical properties but did result in decreased muscle fiber cross-sectional area. Taken together, these findings imply that the arachidonic acid cascade is not a primary modulator of tendon adaptations to non-injurious exercise but is important for muscle growth. Finally, we studied the effects of doxycycline, a broad-spectrum MMP inhibitor, on supraspinatus muscle and tendon adaptations to exercise. Doxycycline enhanced tendon mechanical properties and collagen organization from sedentary animals but not in exercised animals, which supports the increased MMP activity detected in sedentary tendons. Doxycycline reduced muscle fiber cross-sectional area specific to exercised animals. We conclude that the pathways responsible for muscle and tendon adaptations to exercise may be tissue-dependent, and drugs such as ibuprofen and doxycycline also have activity-dependent effects. These studies provide a foundation for future work to develop more efficient physical training regimens and new prevention, treatment, and rehabilitation modalities for overuse injuries.

▼ La fièvre boutonneuse Méditerranéenne (FBM) est due à Rickettsia conorii subsp. conorii, bactérie intracellulaire stricte. Cette maladie, initialement décrite comme bénigne, présente actuellement un taux de sévérité de l’ordre de 10% avec une augmentation de cette sévérité décrite dernièrement. Au cours d’une étude clinique rétrospective portant sur 161 cas de FBM, nous avons observé que le traitement par fluoroquinolones était associé à une évolution défavorable, ce qui à ce jour n’avait jamais été rapporté, alors que la doxycycline semble être protectrice. Nous avons également observé cet effet délétère des fluoroquinolones sur un modèle in vitro d’infection cellulaire à R. conorii, effet qui n’a pas été observé avec la doxycycline. Une des hypothèses pouvant expliquer cet effet est l’induction du module toxine-antitoxine par les fluoroquinolones. Ainsi nous avons montré que la ciprofloxacine modulait l’expression des gènes du couple toxine-antitoxine. Nous avons également montré que les statines pouvaient avoir un effet prophylactique au cours de l’infection par R. conorii. Enfin nous avons étudié la réponse de l’hôte au sein de l’escarre d’inoculation par une approche transcriptomique.Cette thèse a mis en évidence que les traitements reçus au cours de la FBM peuvent modifier le pronostic de cette maladie. Le choix des antibiotiques est donc crucial et doit faire l’objet d’études complémentaires.
Mediterranean spotted fever (MSF) is caused by Rickettsia conorii subsp. conorii, a strict intracellular bacterium. The disease initially described as benign presents currently rates of severity around 10% with an increase described recently. In a retrospective clinical study of 161 cases of MSF, we observed that treatment with fluoroquinolones was associated with an unfavourable outcome whereas doxycycline appeared to be protective. We also observed this deleterious effect of fluoroquinolones in vitro in a cellular model of R. conorii infection, which was not observed with doxycycline. One hypothesis that could explain this effect is the induction of toxin-antitoxin module by fluoroquinolones. Thus we have shown that ciprofloxacin modulates the gene expression of toxin-antitoxin module. We have also shown that statins may have a prophylactic effect during infection by R. conorii. Finally, we have studied the host response within the inoculation eschar by a transcriptomic approach. This thesis has shown that the treatment received during the MSF can change the prognosis of this disease. The choice of antibiotics is crucial, and should be subject to further studies.

▼ HIV-Nef is established as an important factor for pathogenicity during HIV infection. The exact role and underlying mechanisms however are still unclear. Several studies have shown that Nef expression in T cells results in increased T cell activation. This Nef-mediated T cell activation has been suggested to result in higher viral replication and increased pathogenicity. Studies performed in cell lines have shown that Nef disturbs TCR signaling, resulting in increased T cell activation. This Nef-mediated T cell activation has been suggested to result in higher viral replication and increased pathogenicity. To examine Nef-mediated T cell activation, we used constitutive Nef transgenic mice, which had been earlier described to be activated. During our studies in these mice we came to the conclusion that the observed activation in this model was caused by homeostatic mechanisms, resulting from Nef-mediated thymic atrophy, rather than by direct Nef-mediated effects on T cells. This suggested that Nef expression does not lead to spontaneous T cell activation upon expression in primary T cells in vivo. This finding was substantiated in conditional Nef transgenic mice (chapter 3) with tightly regulated Nef protein induction in the T cell compartment. We generated these mice in order to circumvent Nef-mediated effects on thymic development and secondary effects on peripheral T cell compartment. When Nef expression was induced, a rapid thymic atrophy occurred, substantiating the Nef-mediated atrophy observed in constitutive Nef transgenic mice. To study effects of Nef on T cell activation after TCR stimulation, T cells of inducible Nef transgenic mice were stimulated by infection of the mice with influenza virus. We showed that Nef expression in T cells leads to a reduced anti-viral T cell response, suggesting a role for Nef in the disturbance of T cell function and possibly TCR signaling. The infection of antigen presenting cells (APCs) by HIV is important for viral spread and replication. Since several studies reported a Nef-mediated stimulation of HIV replication in APCs, we generated mice with inducible Nef expression in antigen presenting cells (APCs) and studied Nef effects in APCs during influenza infections. We showed that Nef expression in APCs results in increased T cell responses. This suggests a role for Nef in the stimulation of T cell activation via APCs, in this way increasing the likelihood of HIV infection, as activated T cells are more prone to HIV infection than naﶥ T cells. With the development of the models we present in this thesis, we made a step forward in Nef biology. ▼ The alveolar bone repair model, through dental excision in rats, allows the longitudinal study of local and systemic parameters. This allows learning the pathophysiology in inflammatory bone remodeling process. The objective of the research was to investigate the effect of doxycycline associated or not with dexamethasone on the events that follow the bone reabsorption process in alveolar bone repair in rats. The animals (190-200 g) were treated daily (per os) with saline solution Control (SSC), doxycycline (DOX 10 and 25 mg/kg), Dexamethasone (DEXA 1 mg/kg) (IP), and sacrificed according to experimental group (7, 14 and 21 days) to remove their jaws, which were used in the qualitative analysis of microstructural parameters, histomorphometry, and immunohistochemistry for Wnt10b and Dkk-1. It was observed that DOX (10 and 25 mg / kg ) improve some parameters such as the number of osteoblasts increase, reduction in the number of osteoblasts and % increase in bone tissue compared to the CTL group. The results also showed an increase in the expression of Wnt 10b, and Dkk-1 reduction, during the process of bone repair in animals that received DOX (10 and 25 mg/kg), compared to groups CTL. These findings suggest that DOX offers osteoinductive and osteoprotector effects on bone tissue during the alveolar bone remodeling process. It is possible that, in addition to anti-inflammatory effects recognized in DOX, this Tetracycline can influence the expression of Wnt 10b, favoring bone neoformation, and modulate the expression of Dkk-1, inhibiting tissue resorption in the alveolus of the animals.
O modelo de reparo Ãsseo alveolar atravÃs de exÃrese dentÃria em ratos permite que parÃmetros locais e sistÃmicos possam ser estudados longitudinalmente possibilitando o conhecimento da fisiopatogÃnese no processo de reabsorÃÃo Ãssea inflamatÃria. O objetivo da pesquisa foi investigar o efeito de doxiciclina associada ou nÃo à dexametasona sobre os eventos que sucedem o processo de remodelamento Ãsseo em modelo de reparo Ãsseo alveolar em ratos. Os animais (190-200 g) foram tratados diariamente (v.o.) com soluÃÃo salina Controle (CTL), doxiciclina (DOX 10mg/kg e 25 mg/kg), Dexametasona (DEXA 1 mg/kg)(i.p.) e sacrificados de acordo com grupo experimental (7, 14 e 21 dias) para remoÃÃo de suas maxilas utilizadas na anÃlise qualitativa de parÃmetros microestruturais, histomorfometria e imunohistoquÃmica para Wnt 10b e Dkk-1. ▼ Embora a terapia mecânica promova resultados satisfatórios no controle da doença periodontal, a completa remoção do biofilme bacteriano é rara. Assim, antimicrobianos de uso local, como a doxiciclina e a tetraciclina, têm sido propostos como coadjuvantes da terapia convencional. O objetivo deste trabalho foi avaliar, in vitro, a cinética de desorção de doxiciclina e tetraciclina impregnadas a dois dispositivos de liberação local (DLLs) de colágeno: DOX e TET. Para tanto, tais dispositivos foram imersos em 25ml de solução tampão de fosfato, permanecendo em banho termostatizado a 36,5 °C durante 15 dias. As soluções foram analisadas no espectrofotômetro (Shimadzu, modelo UV 2501 PC) em comprimento de onda 275nm, nos tempos de 1, 2, 4, 8, 12, 24, 48, 96, 168, 240 e 360 horas após o início do experimento. Em seguida, foram construídas curvas de calibração para a doxiciclina e tetraciclina em solução tampão de fosfato. Para o estudo da cinética de desorção, foram aplicados modelos de primeira ordem, através de gráficos do logaritmo neperiano (ln) da massa dos fármacos liberada versus tempo. Os resultados demostraram que os fármacos foram liberados por 96 horas, sendo observada maior velocidade de desorção nas primeiras doze horas. TET liberou maior quantidade de antimicrobiano quando comparado a DOX em todos os períodos analisados.
Although mechanical treatment promotes satisfactory results in the control of periodontal disease, complete removal of the bacterial biofilm is rare. Local antimicrobials like doxycycline and tetracycline have therefore been proposed as coadjuvants of conventional treatment. The objective of this work is to evaluate, in vitro, the desorption kinetic of doxycycline and tetracycline incorporated into two controlled release devices (CRDs) of collagen: DOX and TET. For this, the devices were introduced to 25ml of phosphate buffer and maintained in a thermostated bath at 36,5 °C for 15 days. The solutions were analyzed in a spectrophotometer (Shimadzu, model UV 2501 PC) at wavelength 275nm, at intervals of 1, 2, 4, 8, 12, 24, 48, 96, 168, 240 and 360 hours after the start of the experiment. Next, calibration curves were constructed for the doxycycline and tetracycline in phosphate buffer. For the desorption kinetic study, first order models were applied, through neperian logarithm graphs (In) of drug mass released versus time. The results demonstrate that the drugs were released for 48 hours, with higher speed of desorption observed in the first four hours. TET released a higher quantity of antimicrobial than DOX in all the periods analyzed.

▼ This study evaluated as overall purpose the influence of doxycycline used as a matrix metalloproteinase (MMPs) inhibitor on the microtensile bond strength (μTBS) of a total etching adhesive system to dentin, considering two formulations (different pHs) and two substrates (source and demineralized) of primary and permanent teeth. The study is divided into two articles, the aim of the first article was to evaluate the applications effect of acidic or neutral doxycycline on the μTBS to dentin. Twenty extracted noncarious third molars were divided into 4 groups (doxycycline acidic pH, doxycycline neutral pH, chlorhexidine and control - no treatment). A flat dentin surface was exposed, wet grinding and submitted to: acid etch, application of the substance corresponding treatment group, application of adhesive system and resin composite. The specimens were stored (24 h in distilled water) and sectioned to obtain sticks (cross-sectional area of approximately 0.9 mm2). The sticks were submitted to tensile test. The mean μTBS were submitted to a one-way ANOVA and Tukeys test (α = 0.05). No significant difference of immediate μTBS (MPa) was observed between the neutral and acidic doxycycline, furthermore the solutions of different pH values were statistically similar to control and chlorhexidine groups. For the second study 30 permanent molars and 30 second primary molars were divided into 12 experimental groups (n = 5) according to the surface treatment after etching: doxycycline, chlorhexidine and control, and the condition substrate: sound and demineralized (pH cycling). The specimens preparation, restorative procedures and tensile test were performed in the same way as in the previous study. The μTBS were subjected to a two-way ANOVA and Tukey s test (α = 0.05), considering separately primary and permanent teeth. For both teeth types the application of doxycycline did not affect the immediate μTBS, while the demineralized substrate values were statistically lower than the sound group. The doxycycline solution, even at acidic pH, does not seem to affect the immediate bond strength to dentin, while demineralized substrate needs attention by presenting μTBS immediately reduced compared to sound.

▼ Lyme neuroborreliosis, the infection of the nervous system by the tick-borne bacterium Borrelia burgdorferi, is a common infection in the temperate parts of the Northern hemisphere. Manifestations of the disease include facial palsy, radicular pain, sensory disturbances, and occasionally CNS symptoms such as confusion and paraparesis. The diagnosis of Lyme neuroborreliosis is based on medical history, clinical examination and cerebrospinal fluid (CSF) analysis. Recommended antibiotic treatment is oral doxycycline or intravenous ceftriaxone. The overall aims of this thesis were to improve the diagnosis and treatment of Lyme neuroborreliosis.
In paper I, 102 patients with peripheral facial palsy were studied. Onset of symptoms in July to October, additional neurological symptoms and CSF pleocytosis were factors that discriminate patients with peripheral facial palsy caused by Lyme neuroborreliosis from patients with Bell’s palsy.
In paper II, it was shown that CSF levels of the chemokine CXCL13 are highly elevated in Lyme neuroborreliosis and that levels decline after treatment, but high and overlapping CXCL13 levels were also seen in patients with asymptomatic HIV-infection and the decrease in CXCL13 is correlated to the decrease in CSF cell count. The additional diagnostic value of CXCL13 analysis is therefore limited.
In paper III, new reference ranges for CSF cell counts when analyzed with automatic cell counters were determined, based on CSF sampling of 80 healthy volunteers. The differentiation of mononuclear cells into lymphocytes and monocytes was shown to be of limited value in the discrimination between Lyme neuroborreliosis and viral CNS infections.
In paper IV, it was shown that treatment with oral doxycycline resulted in a similar decrease in CSF mononuclear cell counts in patients with Lyme neuroborreliosis with CNS symptoms compared with patients with peripheral nervous systems symptoms, and that all patients with CNS symptoms improved on treatment with no need for retreatment. Oral doxycycline can therefore be considered an effective treatment for Lyme neuroborreliosis, irrespective of the severity of symptoms.

▼ Type 2 diabetes mellitus, caused by the malfunction of insulin-depended glucose and lipid metabolism. Complications of diabetes include retinopathy, nephropathy, neuropathy and cardiovascular disease. Periodontal diseases are infections affecting the periodontium and resulting in the loss of tooth support. There is an association between type 2 diabetes mellitus and periodontitis. Both of these diseases have a relatively high incidence in the general population and common pathways in their pathogenesis. They are polygenic disorders with some degree of immunoregulatory dysfunction. Current studies support a higher incidence and severity of periodontitis in patients with type 2 diabetes mellitus. And other studies show that the prevalence of diabetes in patients with periodontitis is higher that seen in the non-periodontitis patients.
Periodontal therapy using scaling and root planning (SRP) intent to control bacterial infection. Although mechanical periodontal therapy is generally effective, in diabetic patients the reduction of bleeding on probing, probing depth and attachment loss is not satisfactory. Treatment studies provide evidence to support periodontal infection has an effect on glycemic control and treating periodontitis can be influential in contributing to glycemic management.
In response to endotoxins derived from periodontal pathogens, MMPs and particularly MMP-8 target the destruction of alveolar bone and supporting connective tissues, as osteoclast-related mediator. Furthermore, therapeutic approach that target MMP-8, such as doxycycline, maybe is effective strategy to periodontal management and treatment of high risk patients such as type 2 diabetic patients.
Material and Methods
A total of 54 chronic periodontitis patients were included in the present study (29 with type 2 diabetes with HbA1c<7 and 25 systemically healthy). Scaling and root planning (SRP) was performed in all subjects and doxycycline (100mg/daily for 3 weeks) was administered to 15 diabetic patients at 3 months after SRP. Pocket depth, clinical attachment level, recession, bleeding and plaque indices were recorded at baseline and at 3, 6 and 12 months. At the same time points, subgingival plaque samples from 2 sites of each patient were analyzed by "checkerboard" DNA-DNA hybridization for P. gingivalis, T. forsythia, T. denticola, A. actinomycetemcomitans, C. rectus, F. nucleatum, P. nigrescens, P. intermedia and V. parvula and gingival crevicular fluid (GCF) samples from the same sites, as well as blood samples from cephalic vein, were analyzed with ELISA for MMP-8 levels.
Results
Clinical and microbiological parameters were homogenous at baseline (Levene’s test, p>0.05), but MMP-8 levels were statistically significantly higher in diabetic patients. ▼ Methodology
In the present study we created resistant strains of Brucella abortus were created in vitro for the antibiotics doxycycline, rifampicin and streptomycin. Possible instances of cross-resistance to other antibiotics was also tested. Genes known to be involved in these antibiotics resistance were amplified and analysed. The proteomics study was performed using a gel-free shotgun comparative proteomics approach. The proteomics study results were analysed using bioinformatics methods with online software.
After completion of the experiments all resistant strains were destroyed.
Results - Discussion
Specific primers were designed for the amplification of the target genes of this study. The genes examined by PCR-Sequencing are rpsL, rpoB, bepC θαη 16SrRNA. There was only one mutation detected in the rpoB gene.
The rifampicin resistant strain had an MIC of 128κg/ml, the doxycycline resistant revealed an MIC of 2,4κg/ml and the MIC for the streptomycin resistant strain was 4.096κg/ml.
Cross-resistance testing showed that rifampicin resistance elevates the MIC levels of trimethoprim/sulfamethoxazole. Doxycycline resistance affects almost all other groups of antibiotics and streptomycin resistance affected mostly the MIC of rifampicin.
In brucella doxycycline resistance is related to the blocking of the drug entry within the cell and the restriction of all of the cells metabolic activities
Resistance in rifampicin is not only a result of mutations in the rpoB gene. Reduced susceptibility in this antibiotic is the end result of many altered metabolic processes.
The comparison of the differentially expressed proteins of the two datasets revealed 10 common proteins that alter their expression levels in both instances of resistance (Doxycycline, Rifampicin).

Finally a further 180 proteins were added to the list of the experimentally detected proteins and 109 proteins have been described as worthy to investigate for possible use in diagnosis, vaccines and/or as drug targets

▼ O advento do uso de cateteres venosos centrais na prática médica trouxe muitos benefícios aos pacientes, porém está relacionado a um aumento na incidência de infecções por microrganismos multirresistentes. Além disso, freqüentemente ocorre colonização por bactérias produtoras de biofilme. Estes microrganismos se aderem ao material abiótico desses dispositivos intravenosos, ficando protegidos sob a matriz exopolissacarídica do biofilme. Isso faz com que sistema imunológico e antimicrobianos sejam incapazes de ter sua ação plena e, muitas vezes, não atingem os microrganismos mais internos. O motivo deste insucesso é porque muitos desses agentes biológicos e farmacológicos apresentam propriedades físico-químicas incompatíveis com a penetração nesta matriz. Com o objetivo de determinar quais antimicrobianos são mais adequados para uso quando o microrganismo é produtor de biofilme e quais as propriedades físico-químicas que estão diretamente relacionadas à penetração do antimicrobiano na matriz polissacarídica, utilizou-se método colorimétrico com cristal violeta em microplacas modificado para obtenção de concentração inibitória mínima em biofilme (MBIC) e método já padronizado para concentração inibitória mínima (MIC). Para isso foram testados 10 antimicrobianos em Staphylococcus spp.: rifampicina, azitromicina, claritromicina, eritromicina, levofloxacino, gentamicina, doxiciclina, cloranfenicol, clindamicina e vancomicina. Para Pseudomonas aeruginosa foram testados os mesmos, exceto rifampicina e vancomicina. A discrepância entre MIC e MBIC foi muito grande para vários fármacos, mostrando a necessidade de se avaliar estes parâmetros antes do início da farmacoterapia para uma escolha correta, especialmente em hospitais. Os fármacos que apresentaram melhores resultados foram a rifampicina e os macrolídeos, enquanto que os menos efetivos foram vancomicina e clindamicina. Isso foi atribuído ao perfil lipofílico, porém com alguma solubilidade em água das melhores moléculas. Em contra ponto, a elevada área polar, complexidade e massa molar foram características negativas para a penetração em biofilme, resultando numa ineficácia para essas moléculas. Além disso, também foi avaliado o tratamento de polímeros plásticos com EDTA, obtendo-se redução significativa da produção de biofilme nas placas tratadas com o agente químico.

▼ A malária, causada por protozoários intracelulares do gênero Plasmodium, é uma das doenças tropicais mais devastadoras existentes. Mais de 3 bilhões de pessoas vivem em regiões endêmicas para a malária. Cinco espécies de Plasmodium (falciparum, vivax, ovale, malariae e knowlesi) causam doenças em humanos e a infecção com P. falciparum, o mais letal desses parasitas, resulta em mais de 1 milhão de mortes anualmente. O desenvolvimento de resistência aos fármacos antimaláricos tradicionais, leva ao uso de combinações de fármacos como a quinina (QN) e a doxiciclina (DOX). Nesse contexto, os objetivos deste trabalho foram desenvolver e caracterizar formulação de nanopartículas lipídicas sólidas (NLS) contendo a associação de QN/DOX, avaliar sua eficácia em um modelo in vivo de malária berghei, determinar a sua farmacocinética e o coeficiente de partição nos eritrócitos dos fármacos livres e nanoencapsulados. A formulação de NLS contendo QN/DOX (2,0/0,2 mg/mL) foi preparada pela técnica de homogeneização a alta pressão, utilizando polissorbato 80 e Lipoid® como emulsionantes e palmitato de cetila como matriz lipídica. No estudo preliminar de estabilidade, a formulação de NLS contendo QN/DOX apresentou tamanho de partícula adequado (152,8 ± 5,26 nm), índice de polidispersão (0,173 ± 0,006), potencial zeta (-38,6 ± 1,82 mV), alto conteúdo dos fármacos (95,9% ± 0,70/ 94,1% ± 2,41) e adequada eficiência de encapsulação (94,2% ± 1,14/83,0% ± 2,52), após 21 dias de armazenamento em temperatura ambiente. Para a análise do teor um método rápido e específico de cromatografia líquida-acoplada a espectrometria de massa (LC-MS/MS) foi desenvolvido e validado para a determinação simultânea de QN e DOX nas formulações. O método por LC-MS/MS utilizou coluna Waters Sun Fire C18 (50 mm x 3,0 mm de diâmetro) e a fase móvel foi composta de acetonitrila:ácido fórmico 0,1% (75:25, v/v), no fluxo de 0,45 mL/min (split 1:3). O volume de injeção foi de 10 μL. Ratos Wistar infectados por P. berghei foram utilizados para avaliar a eficácia da formulação de NLS contendo QN/DOX utilizando diferentes regimes de dose. As doses efetivas da formulação, i.v. (75/7,5 mg/kg/dia) e oral (105/10,5 mg/kg/dia), representam uma redução de quase 30% em comparação com os fármacos livres utilizados em associação. A farmacocinética foi avaliada após a administração dos fármacos livres ou nanoencapsulados pela vias i.v. (10/1 mg/ kg) e oral (25/2,5 mg/kg) em ratos Wistar infectados. Para a quantificação das amostras de plasma dos ratos, método por LC-MS/MS foi desenvolvido e validado. A QN, a DOX e a cimetidina (padrão interno, PI) foram extraídos do plasma através de precipitação de proteínas e a fase móvel consistiu de metanol/ácido fórmico 0,1% (70:30, v/v), no fluxo de 0,5 mL / min (split 1:3). ▼ Objetivos: Este Estudo randomizado duplo cego placebo controlado comparou, clínica, sitemica e imunologicamente um protocolo de Terapia fotodinâmica antimicrobiana (TFA) de aplicações múltiplas com um protocolo já consagrada com o uso da Doxiciclina sistêmica no tratamento da doença periodontal em pacientes diabéticos tipo 2 descontrolados. Materiais e Métodos: Vinte seis pacientes com HbA1c> 7% foram selecionados e randomicamente alocados em dois grupos que receberam raspagem e alisamento radicular. Um dos grupos recebeu a aplicação adjunta de aplicações múltiplas de TFA (n=12) e o outro utilizando a doxiciclina sistêmica na dose de 100mg (n=14). Os parâmetros monitorados foram índice de placa, Sangramento à sondagem, Profundidade de sondagem, Supuração, Recessão gingival e nível clinico de inserção relativo, o parâmetro sistêmico avaliado foi a HbA1c, medida antes e 3 meses pós tratamento. Os níveis de IL1-β, TNF-α e TGF-β foram medidos antes, 1 e 3 meses pós tratamento através da coleta de fluido crevicular gengival. Resultados: Não houveram diferenças significantes em nenhum dos parâmetros clínicos avaliados e nos níveis de HbA1c. O uso do antibiótico sistêmico demonstrou ser superior na redução de IL1-β até o período de 1 mês pós tratamento, porém, em 3 meses a TFA se mostrou superior. Não houve diferença na redução dos níveis de TNF-α e TGF-β entre os grupos. Conclusões: Ambos tratamentos foram eficientes nas melhoras dos parâmetros clínicos e sistêmicos. A TFA parece possuir maior estabilidade na redução dos níveis de citocinas inflamatórias.
Aim: This randomized, double-blind, placebo-controlled, clinical study compared a multiple application Antimicrobial Photodynamic Therapy (aPDT) treatment protocol with systemic doxycycline as adjuvant to scaling and root planning, to treat chronic periodontitis on type 2 diabetic patients on clinical, systemic and immune-inflammatory outcomes. Materials and Methods: Twenty six patients with Hba1c >7% were randomically allocated in two groups, SRP+Doxy (n=14) using systemic doxycycline 100 mg and SRP+aPDT (n=12) with multiple applications (0, 3, 7 and 14 days). Monitored parameters of plaque score (PS), bleeding on probe (BOP), probing depth (PD), suppuration (S), gingival recession, and relative clinical attachment level (RCAL), glycated haemoglobin (HbA1c) were measured at baseline and 3 months after therapy, the cytokine profile was assessed at 0, 1 and 3 month to measure IL1-β, TNF-α and TGF-β on Gingival Crevicular Fluid. Results: There were no statistically significant differences on intergroup on clinical parameters and HbA1c levels. Systemic doxycycline shoed difference in reduction of IL1-β at 1 month, but aPDT better results at 3 months IL1-β levels. ▼ Abstract: The aim of this study was to assess the effect of the association of local administration of nanospheres loading
doxycycline (DOX) with the periodontal debridement treating advanced chronic periodontitis. Nanospheres were made by double-emulsion method (W/O/W) and characterized regarding morphology, by scaning eléctron microscopy (SEM); and drug/polymer interaction, by Fourier transform infrared spectroscopy (FTIR). The DOX controlled release was assessed by high performance liquid chromatography (HPLC) of gingival crevicular fluid (GCF) samples collected 2, 5, 7, 10, 15 and 20 days after treatment. It was performed randomized double-blinded clinical trial, with thirty patients diagnosed with chronic periodontitis presenting at least seven sites with bleeding on probing and probing depth (PD) ? 5 mm, and 2 sites with PD ? 7 mm. The patients were randomly allocated in two groups receiving interventions as followed: 45 minutes periodontal debridement + subgengival nanospheres loading DOX (DB+DOX); and 45 minutes periodontal debridement + subgengival void nanospheres (DB). Plaque index (IP), bleeding on probing (BP), probing depth (PD) and clinical attachment level (CAL) were evaluated on baseline, 3, and 6 months after therapy. Real-time polymerase chain reaction (real-time PCR) was used to quantify Porphyromonas gingivalis, Tannerella forsythia, Treponema denticola, Aggregatibacter actinomycetemcomitans and Prevotella intermedia from subgingival biofilm samples collected on baseline, 1, 3 and 6 months after treatment. The results were compared by variance analyses test for repeated measures, with significance of 5%. Nanospheres varying from 700 nm to 4 um were obtained. There was a good interaction between DOX and PLGA, with no chemical properties alterations. After local administration of the nanospheres, it was observed constant DOX release in the GCF until 20th day post-treatment, with concentration above the minimum inhibitory concentration. DB+DOX group showed significant PD reduction and CAL gain in deep and moderates pockets comparing to baseline and between groups. It was observed significant reduction of bacteria levels along follow up period, and DB+DOX group was more eficiente in keeping lower levels of bacteria after 6 months from treatment. Within limitations of this study, the results can suggest that PLGA nanospheres are effective carriers for controlled release of DOX and when used adjunctively to periodontal debridement, improved results can be achieved compared to mechanical therapy alone

▼ Abstract: In Duchenne Muscular Dystrophy (DMD) and in mdx mice, absence of dystrophin in skeletal and cardiac muscle disrupts the sarcolemma and leads to increased calcium influx and consequent myonecrosis. The inflammatory reaction elicited to remove muscle fiber debris results in production of pro-inflammatory cytokines which, in turn, increases oxidative stress, and ultimately causes interstitial fibrosis. The gold standard therapy used in the treatment of DMD consists in the use of corticosteroids such as prednisone and deflazacort (DFZ). However, these drugs have limited therapeutic value and their combination with drugs already approved for the treatment of other human diseases such as
doxycycline (DOX) could be beneficial in the clinical management of DMD patients. In this study, we investigated the benefits of the association between DFZ and DOX over therapy with DFZ (gold standard), in mitigating the evolution of dystrophinopathy in skeletal and heart muscle in mdx mice. The treatments with the single drug (DOX 6 mg/mL), (DFZ 1,2 mg/Kg) or DOX (6 mg/mL) / DFZ (1,2 mg/Kg) combination were administered in drinking water to mothers and newborns, starting on the day of birth in the short-term treatment group for a period of 36 days. In the long-term treatment group, which was composed of mice 17 months of age, the same treatments were administered in drinking water to mdx mice for 9 months, starting on their 8 month of birth. Histopathological, functional and biochemical parameters were evaluated in skeletal (biceps brachii and diaphragm) and cardiac muscle. DOX/DFZ combination was found to be advantageous over monotherapy with DFZ either by promoting an initial protection against myonecrosis or by slowing the progression of dystrophinopathy of skeletal muscles. It was also advantageous for cardiac protection. The combination of DOX with DFZ, which has already been used in the treatment of some diseases, has emerged as a potential useful therapy for DMD

▼ Abstract: Experimental and clinical evidence indicate the involvement of matrix metalloproteinases (MMPs) in the pathogenesis of many disease conditions, including cardiovascular diseases. Particularly, imbalanced MMP-2 activity apparently plays a critical role in hypertension, heart failure and in other alterations of the cardiovascular system. Various studies show many targets unrelated to the extracellular matrix for MMP-2, including intracellular proteins and vasoactive mediators. Additionally, recent studies indicate the involvement of this enzyme in proteolytic cleavage of 'beta'1- and 'beta'2-adrenoreceptors. Although some studies have suggested that MMP-2 may affect the vascular tone and impair 'beta'-adrenoreceptor function, no previous study has examined the acute hemodynamic effects of this enzyme. We examined the effects of recombinant human MMP-2 (rhMMP-2) administered intravenously (i.v.) to anesthetized lambs at baseline conditions and during ?-adrenergic stimulation with dobutamine. Twenty-six anesthetized male lambs were used in two study protocols. Firstly, rhMMP-2 (220 ng.kg-1.min-1 over 60 min) or vehicle was infused and no significant hemodynamic changes were found. In the second protocol, we infused dobutamine (5 ug.kg-1.min-1, i.v., over 180 min) or saline in lambs that had received the same rhMMP-2 infusion preceded by treatment with
doxycycline (10 mg.kg-1, i.v., during 15 min) or saline. Plasma and cardiac MMP-2 levels were assessed by gelatin zymography and gelatinolytic activity was assessed by spectrofluorimetry. We found that, while the infusion of rhMMP-2 did not increase plasma and cardiac MMP-2 levels, it increased cardiac gelatinolytic activity, and
doxycycline blunted this effect. Dobutamine decreased systemic vascular resistance index (SVRI) and increased the cardiac index (CI) and left ventricular dP/dtmax. However, co-infusion of rhMMP-2 and dobutamine was associated with lower dobutamine-induced decrease in SVRI and with lower dobutamine-induced increase in CI and dP/dtmax. Pre-treatment with doxyxycline blunted rhMMP-2-induced changes in dobutamine responses. Additionally, we found that rhMMP-2 decreased cyclic AMP levels in cardiomyocytes and that tne inhibitors of MMPs doxyxycline and ONO-4817 prevented this reduction. Our findings show that rhMMP-2, at baseline conditions, exerts no major hemodynamic effects in lambs. However, rhMMP-2, during cardiac stimulation, impairs the responses elicited by activation of 'beta'-adrenoreceptors
