▼ Introdução: A disfunção temporomandibular é uma designação que abrange vários problemas clínicos envolvendo a musculatura mastigatória, a ATM e as estruturas associadas a ambos. Este trabalho pretende realizar uma abordagem histórica, definindo e clarificando definições, terminologias e índices mais utilizados, em investigação na Oclusão. Por fim, realizar uma revisão bibliográfica apoiada num grupo de casos sobre a investigação em si, ou seja, limitações dos estudos e dificuldades da investigação em Oclusão.
Metodologia: Realizámos um estudo observacional longitudinal em que avaliámos uma amostra de 87 pacientes consultados na Clínica Universitária da UCP-Viseu e na Clínica Médica Corpus Dental, com idade entre os 20 e 70 anos. Cerca de 43,7% dos indivíduos pertenciam ao género masculino e 56,3% ao feminino. Estes foram sujeitos ao inquérito Índice RDC/TMD (“Research Diagnostic Criteria for Temporomandibular disorders”) eixo I. No final analisámos as dificuldades e limitações do estudo, produzindo uma lista que pode auxiliar futuros estudos.
Resultados: A dor orofacial foi referida por 43,7% da amostra, sendo 32,2% indivíduos do género feminino. Dos pacientes com dor orofacial 73,7% apresentava dor á palpação muscular, e 42,1% foi diagnosticada com mialgia dos músculos da mastigação. Destes 81,3% foram sujeitos a tratamento com placas de relaxamento muscular, dos quais 68,8% afirmaram no final deste estudo, estar satisfeitos com o tratamento.
Conclusões: De modo a minimizar a controvérsia dos estudos sobre DTM, dificuldades e limitações, é importante que este obedeça a um conjunto de características como existência de um grupo controle e homogeneidade da amostra, que deve ser grande e significativa. Apesar de serem necessários mais estudos com uma amostra maior, concluímos que uma parte significativa da população sofre de sintomas dolorosos na face, e que as placas de relaxamento muscular contribuem para a remissão do sintoma.
Introduction: The temporomandibular joint dysfunction is a term that encompasses many clínical problems involving the masticatory muscles, the TMJ and associated structures. This work intends to conduct a historical approach, defining and clarifying definitions, terminologies and indexes used in occlusion’s research. Finally, conduct a literature review supported by a group of cases about the investigation itself, limitations of the studies and specific difficulties of research in Occlusion.
Methods: We conducted a longitudinal study in which we evaluated a sample of 87 patients consulted in the University Clinic of UCP-Viseu and in the Medical Clinic Corpus Dental, aged between 20 and 70 years. Approximately 43,7% of the individuals were males and 56,3% were females. These were both under investigation Index RDC/TMC (“Research Diagnostic Criteria for Temporomandibular Disorders”) Axis I. ▼ This study evaluated the effects of video modeling on new staff implementation of discrete trial instruction. Three new direct care staff were evaluated on target skills needed to correctly implement discrete trial instruction. A modified pretest/ posttest design was used, and percent accuracy was calculated using a 10 skill checklist. During baseline participants performance averaged 60%, 40%, and 50% respectively. After video modeling, there was an immediate increase in percent accuracy for two of the participants. Performance feedback was provided to the third participant until stable responding was achieved. Results from the two week maintenance probe confirm the skills were maintained in the absence of video modeling. This data coincides with Catania et al., 2009 study in which video modeling can be an effective tool in training new staff on discrete trial instruction.

▼ Trial-based functional analysis (TBFA) is an efficient strategy for assessing challenging behavior in the classroom and a necessary step in developing effective functional assessment- based interventions (FABI). However, researchers have identified several barriers limiting the applicability of this practice in schools, including difficulty in (a) training teachers to mastery; (b) assessing whether teachers can implement this practice independently, with fidelity, in natural settings; and (c) determining whether teachers can use functional analysis (FA) to identify functions of problem behavior and develop effective interventions. This study will address these gaps in the literature by using a novel approach for training teachers to implement TBFA and develop subsequent behavior plans. Video modeling is an evidence-based practice for helping children in school-based settings acquire new skills, and has some evidence supporting its use with teachers. This study assessed the effects of using video self-modeling in training teachers to conduct TBFA in classrooms. The primary dependent variable was treatment fidelity of teacher implementation of TBFA. Additional analyses included teachers’ ability to use student data to identify the function of problem behavior and develop appropriate FABI.

▼ Three spatial cuing experiments assessed whether the nature of the prior trial affects performance in a two choice target identification task. In Experiment 1 current trial RT was strongly affected by whether prior trial cue validity, prior trial target identity, and prior trial target location were the same as on the current trial when an exogenous spatial cue was 50% valid. Experiment 2 demonstrates that, with an endogenous cue, current trial RT was also affected by whether target identity or target location changed from the prior trial. Finally, in Experiment 3 when the cue was exogenous and cue validity was 75%, current trial RT was only affected by the target’s identity on the prior trial. It is concluded that (a) the effects of prior trial history reflect yet another context in which the effects of exogenous and endogenous spatial cuing differ, and (b) the difference in RT to a target preceded by a valid or invalid cue does not reflect a pure measure of spatial attention. Instead, participants appear to weigh information from all dimensions on the previous trial when identifying the target on the current trial. An analysis of the RT distributions yielded significant differences with respect to where in the distribution each factor affected performance. Overall, these data demonstrate that both prior trial effects and analyses of RT distributions of such effects are a rich source of systematic variance that merits further investigation and theoretical consideration.

▼ <b>Background:</b> Defined by their mechanisms, social and psychological interventions are those interventions that work through mental processes and social phenomena. They are often complex and challenging to evaluate, so understanding randomised controlled trials (RCTs) of these interventions requires detailed reports of the interventions tested and the methods used to assess them. However, reports of these RCTs often omit important information. Poor reporting hinders critical appraisal and synthesis of RCTs in systematic reviews, thereby impeding the effective transfer of research evidence to policy and practice. The Consolidated Standards for Reporting Trials (CONSORT) Statement is a reporting guideline that has contributed to improvements in the quality of RCT manuscripts in journals publishing medical research. However, studies have shown persistent deficiencies in the reporting quality of social and psychological intervention trials. A new CONSORT extension for these interventions may be needed given their distinct and complex features. This DPhil thesis reports on a project to develop and disseminate an official CONSORT Extension for Social and Psychological Interventions: CONSORT-SPI. <b>Structure:</b> Following a preface, this DPhil thesis includes eight chapters. Chapter 1 provides an overview of the conceptual rationale that prompted the CONSORT-SPI project. Chapter 2 details the project protocol, which consists of a five-phase methodology that follows current best practices for reporting guideline development and dissemination. Chapter 3 discusses systematic literature reviews to assess reporting guidelines for and the reporting quality of publications of social and psychological intervention RCTs. Chapter 4 discusses an online, international Delphi process to generate a prioritised list of possible items to include in the CONSORT-SPI extension. Chapter 5 discusses a formal consensus meeting to select reporting items to add to or modify for the CONSORT-SPI Extension checklist. Chapter 6 involves drafts of the CONSORT-SPI checklist as well as a template for the Explanation and Elaboration (E&E) document providing detailed advice and examples of good reporting for each checklist item. These drafts have not yet been circulated to co-authors or other members of the project team; their purpose in this thesis is to give an indication of how previous project phases have led into initial prototypes of the checklist and E&E, which will undergo further development and revision by the project team before publication. Chapter 7 proposes a coordinated dissemination and implementation strategy informed by theoretical frameworks and tools used to guide the implementation of clinical guidelines and empirically-supported interventions. This thesis explores aspects of the design and analysis of stepped wedge cluster randomised trials from a statistical viewpoint. It contains a review of current research practices when the design is used in the field and also explores the existing methodological research into the design. It was found that stepped wedge trials often have few clusters (45% < 10 clusters) and a binary outcome (62%). Following this there are three simulation studies presented that aim to explore the use of binary outcome measures in stepped wedge trials with few clusters. The first simulation study examines the different ways in which data from a stepped wedge cluster randomised trial with repeated cross-sections might be analysed. This study also explores the minimum number of clusters needed for consistent and reliable inference under ideal circumstances: such as equal cluster sizes, a time trend that is truly linear, and an intervention effect that is identical for every cluster. For a stepped wedge cluster randomised with 3 steps we found that randomising less than 6 clusters led to estimation problems for all methods of analysis. The second simulation study compares the existing power and sample size determination method for stepped wedge cluster randomised trials to the statistical power of simulated data using the same assumptions. The aim of this study was to see how well the use of Normal approximations in the case of a binary outcome worked for formula based approaches when few clusters were available for analysis. As these approximations became less appropriate, formula based approaches consistently overestimated stepped wedge trial power. The final simulation study is about stepped wedge cluster randomised trials in which cohorts of participants are repeatedly measured. Both closed cohorts, where the same participants are followed throughout the trial, and open cohorts, where participants may enter into the trial at any point, are considered. The effect on study power of different values for the level two and level three variation for both designs is explored. Given the same number of participants per cluster per time, there was generally little difference between the open cohort and closed cohort design for the correlation values we used.

For many philosophical representations of the evolution of theories, as well as representations of the meaning of the language used to express these theories, it has been essential that there exists some kind of convergence. This thesis introduces and collects four papers in philosophical logic pertaining to two different aspects of this basic tenet. On one hand, we have theories, their axioms and their rules of inference. We often have reason to revise a theory over time, to delete some axioms, add some new ones, or perhaps even revise our modes of reasoning. A simple model of such activity, providing a definition of what it may mean that something is provable in the long run in such a dynamic setting, is here investigated, and its relevance for the philosophical discussion about mechanism and knowable self-consistency is evaluated. On the other hand, the notion of a convergent concept, a term which, for whatever reason, has a certain tendency to its application over time, gets a precise explication in terms of trial-and-error classifiers. Formal languages, based on these classifiers, are introduced with semantics and proof systems, and are explored using standard logical methods.

▼ Abstract: The interest in microbial lipase production increased significantly in the last decades, because of the large potential in industrials applications such as: additives in foods (flavor modification), fine chemicals (synthesis of esters), detergents (hydrolysis of fats), waste water treatment (decomposition and removal of oily substances), leather (removal of lipids of animal skins), pharmaceutical and medical areas (remedies, digestives and enzymes for diagnostics). The lipase production can be influenced by different variables such as the microorganism, the carbon sources, nitrogen and lipid, the aeration and agitation conditions, the impeller type, and also including the geometry of the reactor. In the first stage of this work tests was carried out with different inductors (soy oil, com oil, sunflower oil, canola oil and olive oil) for lipase production by Geotrichum candidum NRRL- Y552, getting as the best inductor the soy oil. After that, a study for standardization of inoculum was carried out, making possible the beginning of the optimization of the culture medium, in shaker-flasks, getting as half optimized 3.58% of peptona and 0.64% of soy oil, with initial pH of 7,0, 30°C and 250 rpm that conditions allow reaching 16 U/mL in 48 hours of fermentation. Using the optimized conditions of production, the enzyme was characterized concerning to optimum pH, optimum temperature and stability temperature, the influence of salts in the enzymatic activity and the determination the kinetic parameters KM and Vmáx, Sequentially, with the optimized medium culture, the influence of the impellers it was verified for Rushton turbine, helix naval and pitched blade up in the production of lipase in fermenter, reaching around 25 g/L of biomass for all impellers and 19 U/mL in 30 hours, 12 U/mlL in 30 hours and 22 U/mlL in 54 hours, respectively. Using the impeller type pitched blade up the effect of the agitation and aeration in the of lipase production was investigated, being the optimum conditions 300 rpm and 1 vvm at 30°C for enzyme production, reaching approximately 21 U/mL in 54 hours of fermentation. The effect of different aeration rates in a not conventional reactor air lift was also investigated, resulting in about 20 U/mL of lipolytic activity ín 30 hours of fermentation, making possible to obtain with this geometry of reactor a larger productivity (40% greater ín comparison to the conventional reactors). These results for the process of lipase production are larger than the reported ones in the literature for the same microorganism

trial, the clinical
trial has remarkably increased during the past years. The composition of volunteers is not only included patients but also healthy subjects, which catered for the differences of research design of the
trial. Thus, many ethical issues related to clinical trials have gained attention, particularly, the motivations of participating in clinical trials from healthy volunteers are crucial issue. The intention of the study is not only to understand the mainly motivation, perception and personality trait of healthy volunteers, but also to discuss the influence of demographic variables and personality trait on the motivation of volunteers participating clinical trials in Taiwan.
This study was a cross-sectional survey research. The study sample included 196 healthy volunteers from three different biomedical clinical
trial companies. The self-administered questionnaire contained 15 questions. The internal consistency reliability (α) for the BFI scale was .789. One-way ANOVA and post-hoc analysis were used to examine the differences of the motivation in different demographic variables. In addition, hierarchical regression analysis was employed to investigate the determinants of different motivation.
The results of the study showed most of the healthy volunteers were single male undergraduate students with age 20-24. The types of personality, opennessãextraversion and agreeableness, tended to be high tendentiousness. On the other hands, the subjects in neuroticism personality tended to be low tendentiousness. Moreover, the results showed the main motivation of the healthy volunteers is financial rewards, followed by free physical examination and the last is invitation by friends, respectively.
According to the results of the study, the relatedness between the motivation of participating clinical trials and different variables showed that the elder subjects considered the free physical examination and promoting scientific and medical development as the mainly motivation, and showed less concern of companion invitation. Also, the subjects with higher education concerned the invitation by companion as less priority. Comparing with students, the waged volunteers showed less consideration of promoting scientific and medical development as motivation. Moreover, the healthy volunteer with average income between 10,000 to 19,999 NT considered mostly free physical examination, promotion of scientific and medical development and curiosity as the main motivation for the
trial. The more involved in clinical
trial the more consideration of other motivation and less concern of the invitation by companion. Furthermore, the subjects with more agreeableness personality trait tend to be more consideration of promoting scientific and medical development as their mainly motivation.

▼ News coverage of high profile criminal matters has increased in South Africa. Such matters are
of public concern, as every citizen has a right to receive and impart information and to debate
openly and frankly matters which are of public concern, including matters before the courts.
The legitimacy of the courts is dependent on robust media reportage and public scrutiny of
judicial matters which such reportage stimulates. However, criminal trials of high profile
accused persons such as Oscar Pistorius, Shrien Dewani and J Arthur Brown, turn easily into a
show with strong entertainment value, giving the media strong profitmaking reasons to cover
it. In their pursuit of profit and in seeking to satisfy the curiosity of their readers, listeners or
viewers, the media regularly resort to
trial by media or adverse pre-
trial publicity.
Trial by
media is nothing more than commercially motivated expression which does not warrant
constitutional protection.
At the receiving end of such coverage are accused persons. Public censure of crime and
of accused persons which follows
trial by media should not be imposed on the innocent. The
right to a fair
trial requires that an accused be treated fairly from the inception of the criminal
process, from which point the person suspected of committing the crime in question is
considered innocent. Any pre-
trial process which implies that the accused is guilty, including
any such process influenced by media reports surrounding criminal offences, violates the
presumption of innocence.
Despite the availability of remedies, the media in South Africa usually are not held to
account for their actions and persist with adverse, biased and irresponsible pre-
trial reporting.
Courts have shown a tendency to protect the media in these cases, despite the effect of such
reporting on the judicial process, the administration of justice and the fair
trial rights of
accused persons. The reason for this is usually the hesitation on the part of judges to
recognise their susceptibility to extraneous matters. Judges should not be placed in a position
where their independence and impartiality are questioned as a result of media sensationalism.
Where the media create mistrust in the integrity of the judiciary, the rule of law is in peril.

▼ Empirically tracking when defendants fail to appear (FTA) for their court date is important for virtually all court systems. Factors related to FTA can inform court decision-making. Developing a typology of FTAs may provide added accuracy in pretrial detention decisions and using pretrial risk assessments. The current exploratory study expands on current knowledge of factors associated with FTA by identifying profiles of those most likely to fail to appear, and comparing with profiles of those who do show up for their court date. Seven cluster profiles were established for FTAs, and eight cluster profiles were established for non-FTAs. While there was some overlap between profiles, there were a few profiles that were particularly distinct. The profiles and their policy implications are discussed.

▼ O objetivo do estudo foi avaliar a efetividade de dois procedimentos, Tentativa Discreta (TD) e Tentativa Discreta com Intervalo entre as Respostas (TD/IRI), na produção da variabilidade comportamental em diferentes condições (diretamente reforçada e induzida) e averiguar que condição, em cada procedimento, mais facilitaria a seleção de uma sequência de baixa probabilidade inicial. Selecionou-se 18 universitários com um desempenho na linha de base, medido pelo índice U, igual ou inferior a 0,5. Estes foram divididos nos dois procedimentos e subdivididos em três grupos com três participantes (VAR, ACO e CON). A tarefa consistia em formar figuras na tela do computador pressionando duas teclas (direita e esquerda) de teclados laterais. A unidade comportamental era quatro respostas às teclas. Nos dois procedimentos apresentava-se um pedaço da figura, um tom e 0,5 segundo, caso a sequência fosse passível de reforço, e um timeout de 1,0 segundo, caso não fosse. Somente no TD/IRI havia um IRI de 0,5 segundo após as três primeiras respostas da sequência. O experimento continha três fases. Na Fase 1, linha de base, vigorava o reforçamento contínuo (CRF) das 16 sequências. Para dois grupos (TD-VAR e TD/IRI-VAR) na Fase 2 havia o reforçamento direto da variabilidade, para 15 sequências, no qual a sequência menos frequente e menos recente teria maior probabilidade de ser reforçada, e CRF de uma sequência alvo (sequência menos frequente na linha de base). Na Fase 3, havia a distribuição acoplada dos reforços da fase anterior para as 15 sequências e CRF de outra sequência alvo. Para os grupos TD-ACO e TD/IRI-ACO a ordem de exposição às fases foi inversa, visto que na Fase 2 a distribuição de reforços foi acoplada ao desempenho dos participantes dos grupos VAR. Nos grupos TD-CON e TD/IRI-CON havia somente o CRF da sequência alvo nas duas fases. Os resultados mostraram que a variabilidade aumentou na primeira sessão da Fase 2 para todos os participantes. O grupo TD-ACO foi o que apresentou maior índices de variabilidade dentro deste procedimento. Já no TD/IRI, isto foi verdadeiro para o grupo TD/IRI-VAR. Com relação à seleção da sequência alvo, os grupos que apresentaram maiores porcentagens de seleção em todas as sessões foram TD-CON e TD/IRI-VAR, respectivamente dentro de cada procedimento. diagnozavimas. Darbo tikslas – sukurti įmonių finansinių sunkumų
diagnozavimo modelį ir jį patikrinti pasirinktų įmonių pavyzdžiu.
Tyrimo uždaviniai: išanalizuoti ir susisteminti anksčiau sukurtus
bankroto prognozavimo modelius ir jų testavimo rezultatus;
sudaryti įmonių finansinių sunkumų diagnozavimo modelį;
patikrinti sukurto modelio tinkamumą pasirinktų įmonių tarpe.
Tyrimo metodai. Analizuojant bankroto prognozavimo modelius ir jų
testavimo rezultatus atlikta mokslinės ir metodinės literatūros
loginė ir lyginamoji analizė bei sintezė. Įmonių finansinių sunkumų
diagnozavimo modeliui parengti skaičiuoti finansiniai santykiniai
rodikliai bei atlikta statistinė analizė, rezultatai pateikti
taikant monografinį metodą. Remiantis indukcijos metodu ir logine
analize tikrintas sukurto finansinių sunkumų diagnozavimo modelio
tinkamumas analizuotų įmonių tarpe. Tyrimo rezultatai. Pirmojoje
darbo dalyje išanalizuota finansinių sunkumų esmė bei juos
sąlygojančios priežastys, nustatytas finansinių sunkumų
diagnozavimo būtinumas, išnagrinėti ir apibendrinti bankroto
prognozavimo modeliai bei išskirti dažniausiai juose naudojami
finansiniai santykiniai rodikliai. Antrojoje darbo dalyje sukurtas
logistine analize paremtas finansinių sunkumų diagnozavimo modelis
bei iškelta hipotezė, kad jis leidžia patikimai apskaičiuoti įmonių

Research object – diagnosis of company
financial difficulties. Research aim - to create diagnostic model
for company financial difficulties and to test it using cases of
selected companies. Objectives: to analyze and systematize
previously developed bankruptcy prediction models and their test
results; to create a diagnostic model for company financial
difficulties; to analyze the relevance of the model with the
selected companies. Research methods: logical and comparative
analysis of scientific and methodical literature, statistical
analysis, monographic method, induction method, logical analysis.
Research results. The first part analyzes the financial
difficulties and their underlying causes, establishes the necessity
of the financial stress test model, analyzes and summarizes
bankruptcy prediction models, and distinguishes the most frequently
used financial ratios. The second part creates a logical analysis
based diagnostic model for financial difficulties and presents
hypothesis, that this model ensures reliable calculation of
probability of company financial difficulties among the tested
corporations. The third part uses randomly selected companies to
test the created diagnostic model for financial difficulties. The
developed model is suitable for the manufacturing companies,
because it separates financially well standing companies from the

▼ Many ethnic groups, who migrated to the United States, experience psychological predictors such as depression, psychological distress, social and linguistic problems that hinders acculturation. The purpose of this study was to examine these stressors and its effects on Hmong Americans??? acculturation process. The sample represented an n = 50 out of 52 participants who volunteered to participate in the study. The findings tested the two hypotheses that Hmong men experience less acculturation
difficulties than Hmong women because of cultural practices and Hmong Americans who came to the United States following the end of the Vietnam War have made very little progress towards acculturation. Cross-tabulation between selected independent and dependent variables were used to analyze data. In conclusion, the results show that there was no significance association between gender and acculturation
difficulties. The results also show that Hmong Americans who came to the United States following the end of the Vietnam War have made some progress towards acculturation.

▼ Research has indicated the importance of calculus knowledge for undergraduate programs in science and technology fields. Unfortunately, one of the main challenges faced by students who join science and technology fields is their knowledge of calculus concepts. The main purpose of the study is to overcome students‟
difficulties in learning calculus concepts by developing a literature informed intervention model. A design-based research approach of three phases was conducted. Grade 12 natural science stream students in one administrative zone in Ethiopia were used as the study population.
Triangulated themes of students‟
difficulties and common conceptual issues that are causes of these synthesized
difficulties in calculus were used as a foundation to propose an intervention model. Based on the proposed model, an intervention was prepared and administered. A pre post-test aimed to asses students‟ conceptual knowledge in calculus was used to examine the effect of the model. Quantitative analysis of the test revealed that the intervention has a positive effect. The
experimental group score is better than the controlled group score with independent t-statistics, t = 4.195 with alpha =.05. In addition, qualitative analysis of the test revealed that students in the
experimental group are able to overcome many of the
difficulties. In particular, many students demonstrated process level conception, conceptual reasoning, qualitative justification, a consistency in reasoning, less algebraic error, and a proficiency in symbolic manipulation.
The study concludes with Implications for practice that includes the use of students‟ errors and misconceptions as an opportunity for progression. Besides, students should be assisted to make sense of concepts through real-life problems, including training teachers in problem-solving approaches and mathematical thinking practice.

▼ Aims. The aim of this thesis is to bring forth Aalto university School of Engineering associate professors' conceptions of good teaching, their construction of the conceptions and experiences about resources and difficulties in developing teaching. These issues are partly elaborated in the light of teaching competence assessment which is part of the tenure track career path for professors. In this thesis the following research questions were addressed: 1) What are the associate professors' conceptions of good teaching? 2) Through which elements the associate professors construct their conceptions? 3) What kinds of experiences the associate professors have about the resources in developing teaching? 4) What kinds of experiences the associate professors have about the difficulties in developing teaching?
Methods. Seven associate professors of the School of Engineering were interviewed and they had all participated in teaching competence assessment before being admitted as associate professor. Seven thematic interviews were conducted in the Winter 2015-2016. The qualitative method of analysis was an abductive thematic analysis. The theoretical framework is based on Boyer's concept of the scholarship of teaching and Kreber and Cranton's (2000) perception of three knowledge domains (instructional, pedagogical and curricular knowledge) that are based on three reflections (content, process and premise reflection) which together construct the scholarship of teaching. Boyer's (1990) concept and Kreber and Cranton's (2000) three types of reflections and knowledge domains were utilized in analyzing the conceptions of good teaching as well as resources and difficulties in developing teaching.
Results and conclusions. Three different conceptions about good teaching emerged. The conceptions were 1) teaching as providing multifunctional tools for working life, 2) teaching as activation of students and 3) teaching as emphasizing students. The interviewees constructed their conceptions through eight elements of which pedagogical training and discussions with pedagogical experts were described as central in changing own conception. Activating students was emphasized in all of the conceptions, but in different ways. The most fundamental resources for developing teaching were pedagogical training, discussions with pedagogical experts and proactivity. The most central difficulties for developing teaching were lack of pedagogical skills, time and the distance in between pedagogical experts and teachers. Based on the interviewees reports, the conceptions of good teaching, resources and difficulties in developing teaching could be partly explained with the financial situation of the society as well as university teachers' workload. Additionally, the difficulties in developing teaching could be eased by making the assessment process of teaching competence more transparent and reaching pedagogical experts easier.

(2004) and it suggests that changes that are expected or "probable" are detected more easily than changes that are unexpected or "improbable". This research study investigates the change probability effect and the differences between observers who are looking for changes and those who are unaware that changes may exist. The results suggest that observers who are not expecting a change to occur are more likely to detect an improbable change than a probable change. The findings from this study have real world applications and also have implications for change blindness research studies.

▼ Moving to the groove of the music is a phenomenal and universal human behavior. Common characteristics to most dance music include a salient beat, rhythmic complexity, and a dynamic musical structure. What is unknown, however, is why music has the power to promote physical movement. Musical rhythm and motor processing are tightly linked activating brain areas important for motor preparation and execution. While studies demonstrate that the motor system is active during beat processing, it is still unclear how musical groove, a beat-influenced quality to music that makes us want to move, can engage these motor areas. Here, I performed a novel investigation to understand how high-groove music can enhance sensorimotor processing. In contrast to previous work that utilized transcranial magnetic stimulation, I used electroencephalography to measure motor-related neural activity on a fine temporal scale during continuous music listening. Specifically, I measured the lateralized readiness potential, an event-related potential related to motor activation and response execution. I predicted that music with high amounts of groove would promote sustained motor area engagement. Particularly, I hypothesized that high-groove music, as defined by Janata et al. (2012), would promote greater motor system priming than low-groove music, noise, or silence, resulting in faster neural activation during both response preparation and execution. ▼ Physical appearance has a far-reaching influence on a variety of human interactions, yet the effects on one domain has been largely ignored. This dissertation examines the potential effect of physical attractiveness on health care quality. Theoretical explanations for the development of both prejudice and stereotypes are presented, with a focus on the development and effects of attractiveness-based stereotypes. Several studies have found that workers in the medical field are susceptible to the same biases as the general public and that these are associated with lower health care quality for those more marginalized by society. Therefore, two studies were conducted to establish first, that health care workers possess attractiveness-based stereotypes similar to other segments of society and second, that these stereotypes affect the quality of health care received by individual patients. Based on these findings, there is a dire need for further research into this area. Automatic stereotype activation is extremely difficult to prevent, yet even more so when individuals are unaware of these stereotypes. Although programs exist to help reduce race and gender based stereotypic behavior in fields such as medicine or law enforcement, attractiveness-based stereotypes are largely ignored. Attempts cannot be undertaken for education and corrective action until we have a larger body of substantial basic research supporting the power that attractiveness-based stereotypes have over health-related decision-making.

▼ Questionnaires are often used as measures of inner experience. This study questions the adequacy of such measures, using inner speech as an example. We compared two questionnaire measures of inner speaking to each other and to inner experience as apprehended by Descriptive Experience Sampling (DES, a naturalistic, high fidelity method of exploring inner experience). Undergraduate volunteers (N = 260) took two questionnaires designed to measure inner speaking: the Self-Talk Scale (STS) and the Nevada Inner Experience Questionnaire (NIEQ). A subset of these (N = 16) participated in DES to investigate their inner speaking with fidelity. Scores on the NIEQ and STS were strongly correlated. However, the correlations between either questionnaire and DES-apprehended inner speaking were near zero—questionnaire ratings of inner speaking grossly overestimated the frequency of inner speaking as found by DES. These results suggest that questionnaire responses may be based more on presuppositions about experience than on actually occurring experience, and suggest caution in using questionnaires to study inner experience.

▼ When listening to music and language sounds, it is unclear whether adults recruit domain-specific or domain-general mechanisms to make sense of incoming sounds. Unique acoustic characteristics such as a greater reliance on rapid temporal transitions in speech relative to song may introduce misleading interpretations concerning shared and overlapping processes in the brain. By using a stimulus that is both ecologically valid and can be perceived as speech or song depending on context, the contribution of low- and high-level mechanisms may be teased apart. The stimuli employed in all experiments are auditory illusions from speech to song reported by Deutsch et al. (2003, 2011) and Tierney et al. (2012). The current experiments found that 1) non-musicians also perceive the speech-to-song illusion and experience a similar disruption of the transformation as a result of pitch transpositions. 2) The contribution of rhythmic regularity to the perceptual transformation from speech to song is unclear using several different examples of the auditory illusion, and clear order effects occur because of the within-subjects design. And finally, 3) when comparing pitch change sensitivity in a speech mode of listening and, after several repetitions, a song mode of listening, only a song mode indicated the recruitment of music-specific representations. Together these studies indicate the potential for using the auditory illusion from speech to song in future research. Also, the final experiment tentatively demonstrates a behavioral dissociation between the recruitment of mechanisms unique to musical knowledge and mechanisms unique to the processing acoustic characteristics predominant in speech or song because acoustic characteristics were held constant.

▼ Synchronization to rhythmic stimuli is an everyday experience, whether it is exercising to the beat of music, dancing salsa, or rocking a baby to sleep. Commonly, humans synchronize their movements with the frequency of the beat (a quasi-isochronous pattern of prominent time points). Previous research has shown that the intended beat periodicity of a rhythmic stimulus can be observed in periodic neural activity; however, the extent to which this reflects robust perception of musical rhythm versus purely stimulus-driven activity is unknown. In Experiment 1 and 2, I investigated how long listeners can maintain a percept of the beat once the stimulus evidence becomes beat-ambiguous. In Experiment 3, I used electroencephalography (EEG) to investigate whether steady state-evoked potentials (SS-EPs, the electrocortical activity from a population of neurons resonating at the frequency of a periodic stimulus) arising from auditory cortex reflect beat perception when the physical information in the stimulus is ambiguous and supports two possible beat patterns. In both experiments, participants listened to a musical excerpt that strongly supported a particular beat pattern (context), followed by an ambiguous rhythm consistent with either beat pattern (ambiguous phase). During the final probe phase, listeners indicated whether a superimposed drum matched the beat. We found that participants perceived probes that matched the beat of the context as better fitting the ambiguous rhythm, compared to probes that did not match the beat of the context. We also found that SS-EPs during the ambiguous phase had higher amplitudes at frequencies corresponding to the beat of the preceding context. These findings support the idea that SS-EPs arising from auditory cortex reflect perception of musical rhythm and not just stimulus encoding of temporal features.

▼ Competitive athletes regularly use a range of nutritional supplementation products in an effort to improve their sporting performance. These supplements are often used despite any knowledge on whether they actually have any effect on performance. Recently the use of beetroot juice has shown promise as an effective supplementation strategy for endurance athletes, and in particular cyclists of moderate fitness. However, there is little knowledge on the effectiveness of beetroot juice on the performance of highly trained cyclists. Therefore, the purpose of this thesis was to determine the effectiveness of nitrate supplementation on cycling and physiological responses in highly-trained cyclists. A series of studies were undertaken during which the performance and physiological responses of highly-trained cyclists were assessed before, and after a range of beetroot juice supplementation strategies. Collectively, the studies in this thesis reveal that the effectiveness of NO3-supplementation on physiological and performance measures appears to be sensitive to the training status of athletes, and the environmental conditions that the event is conducted in. In particular, despite previous positive findings in cyclists of lesser aerobic ability, any potential improvement in sporting performance or enhanced physiological responses appear to diminish in cyclists of greater aerobic ability. As such, the findings of this thesis suggest that effects of NO3- supplementation appear not to influence performance or a range of physiological measures to the same extent as lesser-trained populations.

▼ Stroke is the primary cause of permanent disabilities worldwide. Hemiparesis and hemiplegia (the most common consequences of stroke) are the decreases in motor-functionality of the brain on one side of the body which will affect the daily life activities of the patient. There are several challenges with the current state of delivering rehabilitation services such as limitations on the number of the clinics, financial resources needed for providing rehabilitation, associated costs of transportation, and human resources. To overcome the issues related to conventional ways of delivering therapy, different robotic systems have been developed to benefit healthcare systems and patient with disabilities. Although various devices have been developed for rehabilitation on either upper or lower extremity, there is no robotic system in the market for physical rehabilitation on both upper and lower limbs. Developing such devices is useful particularly in the case of hemiparesis as the rehabilitation process can be performed on both upper and lower limbs using the same device. This thesis represents the design process, manufacturing, and control of a novel robotic rehabilitation device for delivering post-stroke therapy on both lower and upper extremities. This portable, lightweight, inherently safe robot has five degrees of freedom, and its mechanical characteristics are modifiable to suit different modes of therapy. An ethics application was submitted and approved by the Western University Ethics Board to validate the functionality of the device by performing experiments on healthy participants. The proposed robot has been manufactured, verified through the experimental tests and been tested on the healthy subjects.

▼ This thesis examines the mode of trial concerns in the U.S.A., New York State, California, England and Wales and Canada – specifically the ability of the jury to comprehend complex cases and the perception/reality that bench trials may not be as fair as jury trials. Defining complex cases as those involving serious fraud indictments, capital murder trials, and lawsuits or indictments against corporations and their managers, the thesis examines problems associated with jury trials in such cases. It evaluates the comparative law and customs and practices regarding the use of juries, emphasizing problems with jury selection, deficits in jury deliberation and post trial problems associated with jury verdicts. The thesis also evaluates the judge only trial, attempting to determine whether a state imposed non jury trial in a criminal case as is presently proposed in the England and Wales Parliament creates an unfairness to the defendant because bench trials significantly differ from jury trials in the application of the rules of evidence and in the role of the judge. The thesis reports on the results of a survey of New York State trial judges, a like survey of New York State lawyers, and the opinions of nine England and Wales judges authorized to try serious fraud cases who were interviewed regarding these issues. The surveys and interviews finds that there is a high degree of support for jury verdicts expressed by the judges, examines evidentiary and pretrial practices in both modes of trial and attempts to evaluate whether claims of procedural flaws and prejudice in bench trials by respected academics are accurate. The thesis concludes by affirming the competence of juries to try complex cases, proposing modifications to post jury verdict procedures to evaluate jury misconduct and advocating that the bench trial evidentiary rules and conduct rules become comparable to the jury trial. The thesis recommends that mode of trial choices be given to the defendant, advocates that when a bench trial is selected that peremptory challenges of the trial judge be permitted and postulates that these reforms will make the bench trial a more attractive alternative to the jury trial in complex cases.

▼ Chronic obstructive pulmonary disease (COPD) is characterised by shortness of breath, cough and excess sputum production. The most common conditions included in COPD are chronic bronchitis (inflammation in the airway) and emphysema (destruction of lung tissue) resulting in irreversible airflow restriction. COPD is a major cause of morbidity and mortality and predicted to be the third leading cause of death by 2030. The causes of COPD are the long-term exposure to toxic substances and gases such as cigarette smoke, which induce an amplified inflammatory response in the lung, particularly in the small airway. Due to the irreversible nature of the condition, available treatments only provide symptomatic relief and management of exacerbations. These treatments are limited in their effectiveness and are associated with a range of adverse events. Patients with COPD have increasingly used complementary and alternative medicines (CAM). In Australia, 17.3% or nearly one in five COPD sufferers have used some form of CAM therapy. Panax ginseng is one herb, which shows promise for a number of health conditions including COPD. However, existing Panax ginseng studies have produced inconsistent evidence on efficacy due to a range of methodological issues. The research presented in the thesis achieved the objectives, which include: 1) search and analysis of the classical literature on Chinese herbal medicine for symptoms associated with COPD; 2) review extant systematic reviews of Panax ginseng; 3) systematically review the English and Chinese literature of Panax ginseng in randomised controlled trials (RCTs) ; 4) review the potential mechanisms of Panax ginseng on reducing inflammation and oxidative stress; and 5) design and implement an RCT that addresses the methodological short falls identified in the literature and evaluate the efficacy and safety of Panax ginseng for patients with moderate COPD. A stepwise approach was developed to identify candidate herbal medicines for COPD through systematic reviews on classical and modern literature, and then prioritise the key herb for further investigation with respect to its efficacy, potential mechanisms of action and evaluation of its efficacy and safety through an RCT. Although there is extensive literature, methodological flaws often limit the acceptance and applicability of results. This highlights the importance of improving standards and reporting in future research. The clinical trial presented here will provide critical clinical data and build on previous RCTs and systematic reviews. Given the increasing prevalence and burden associated with COPD, particularly in the elderly, this research is timely to explore alternatives that may help patients to improve their quality of life and lung function. ▼ Migraine affects 8 -16% of population in different countries resulting in significant economic and social impacts. Current pharmacological treatment provides symptomatic relief, but not without side effects. Hence, an increasing proportion of patients prefer complementary therapies including acupuncture for migraine relief. Randomised controlled trials (RCTs) of acupuncture treatment for migraine have produced conflicting evidence due to methodological and reporting deficiencies, including small sample sizes and inappropriate outcome measures. Furthermore, systematic reviews on acupuncture for headache failed to adequately represent non-English studies such as those conducted in China. This project aimed to: (1) systematically review studies of acupuncture for migraine; and (2) conduct a RCT addressing key deficiencies identified from the systematic reviews (SRs). Two SRs were conducted based onfollowing Cochrane review protocols. Major English, Chinese, Japanese and Korean databases were searched. The first SR included 15 English studies and the second SR had 17 Chinese studies. No Japanese or Korean RCTs were identified. Overall, those two SRs supported the value of acupuncture in the treatment and prevention of migraine when compared with western medications. However, conflicting results were found when real acupuncture treatments were compared with sham/placebo procedures. When compared with the studies published in English, Chinese studies had a higher frequency of acupuncture treatment, displayed poor methodological quality, and commonly used pharmacotherapy as a comparator. The SRs revealed that there was limited evaluation of acupuncture for frequent migraine. In the RCT, fifty participants with a minimum of eight migraine days per four weeks were randomly allocated to receive real (RA, n = 26) and sham (SA, n = 24) acupuncture for a total of 16 sessions over 20 weeks in a gradually decreasing treatment frequency. Fixed and supplementary acupoints were used. Participants were followed up for further assessment at three months and one year. Results showed that at the end of the treatment period, the mean (SD) migraine attack days per four weeks decreased from 11.81 (5.76) to 5.17 (5.02) in the RA group and from 12.41 (6.4) to 10.08 (7.11) in SA (group difference: p = .008). Intensity of migraine assessed using a Six-point Likert scale was lower in the RA (2.18 ±1.05) than that in the SA group (2.93 ± 0.61, p = 0.004). The percentile changes of pressure pain threshold (PPTs) detected at the bilateral points of TaiYang (Ex-HN5) were significantly higher in the RA group (RA 228.48% vs. SA -0.66 % on the left and 92.69% vs. -2.52% on the right). However, there was no s tatistically significant difference between the two groups with respect to Migraine Specific Quality of Life (MSQOL). ▼ Background: N-of-1 trials are prospectively planned,
multiple crossover evaluations, conducted in individual patients.
Evidence shows that a range of designs and statistical methods have
been applied to N-of-1 trials. This thesis helps to provide a
comprehensive understanding about the methodology and reporting of
N-of-1 trials by synthesizing all published evidence. Furthermore,
while the primary objective of N-of-1 trials is to assess treatment
response in individual patients, this thesis explores whether any
secondary benefits can be derived from N-of-1 trials and the data
they generate. Given the number of N-of-1 trials conducted in the
area of attention deficit/hyperactivity disorder, this condition
was chosen as the clinical model explored in the thesis.
Objectives: i) To provide a systematic overview of published N-of-1
trials; ii) To assess how N-of-1 trials that have been conducted to
assess the same interventions for the same conditions, using
identical outcome measures can be aggregated in order to yield
group estimates of treatment effect; and iii) To assess how N-of-1
trials can be combined with RCT data into a single meta-analysis.
Methods: A series of systematic reviews were conducted in which
each review consisted of a thorough search strategy, an assessment
of inclusion of primary studies, a risk of bias assessment and
either a qualitative or quantitative synthesis of data. A second
reviewer was involved in all reviews. Results: This thesis found
that N-of-1 trials have been conducted in over 50 conditions, and
that the majority of published N-of-1 trials are published as a
series. Our findings also indicate that N-of-1 trials can be
meta-analyzed across participants in order to yield population
treatment effect estimates. Furthermore, we found that combining
N-of-1 trials with RCT data into a single meta-analysis, impacts
both the magnitude and precision of overall treatment effect
estimates. Conclusions: This thesis examined the potential for
N-of-1 trials beyond their primary purpose of providing estimates
of individual treatment effectiveness and demonstrates a method of
aggregating N-of-1 trials across participants as well as with RCT
evidence. Clinical and research recommendations on how to move this
field forward have been provided.

processes which are proposed to maintain distress across diagnostic
categories might have advantages over disorder-specific approaches
for people experiencing first-episode psychosis (FEP). This thesis
aimed to evaluate the feasibility and acceptability of a
transdiagnostic cognitive therapy called Method of Levels (MOL) for
people experiencing FEP, and to consider whether adaptations are
needed to support its implementation into routine practice. MOL is
designed to ameliorate distress, rather than reduce symptoms. A
related aim, therefore, was to increase our understanding of the
sources of distress for this population. The results of the thesis
will be used to inform decision making about the suitability of MOL
for further testing in a larger clinical
trial. The thesis
comprises three studies described over four papers: (i) a
qualitative metasynthesis exploring sources of distress in FEP
(Chapter 3); (ii) a feasibility randomised controlled
trial (RCT)
of MOL for FEP (Chapters 4 and 5); and (iii) a qualitative study
exploring experiences of receiving MOL amongst people with FEP
(Chapter 6). The metasynthesis is the first review to address the
question of what causes distress in people experiencing FEP from a
first-person perspective. Following a systematic review, findings
from qualitative studies (n = 33) were synthesised using thematic
analysis. Sources of distress for this population were found to be
diverse and multifaceted. The results suggest that practitioners
who pay attention to wider sources of distress will be experienced
as more helpful by people experiencing FEP. Participants (n = 36)
were successfully recruited to the RCT and randomised to one of two
conditions: treatment as usual (TAU) or TAU plus MOL. Retention at
final follow up was 97%, substantially higher than the 80%
threshold pre-specified as a successful feasibility outcome. This
was the first study of its kind and the results provide evidence
that it is feasible to recruit and retain participants experiencing
FEP in a
trial of MOL, suggesting that it is suitable for further
testing in a larger
trial. It also provides preliminary evidence of
MOL’s acceptability for this population. A proportion of
participants from the RCT (n = 12) were interviewed about their
experience of receiving MOL. Results were analysed using thematic
analysis. Four superordinate themes were identified: ‘The
therapist’s approach’; ‘I was in control’; ‘Thinking and
talking’; and ‘Looking at problems from a different
perspective’. Engaging with MOL was predominantly perceived to be
a helpful experience. The study provides additional evidence of the
acceptability of MOL for people experiencing FEP. This thesis makes
a novel contribution to the literature on psychotherapeutic
interventions for people experiencing first-episode psychosis and
increases our general understanding of the lived experiences of
this population.

Uvod: Osteoartritis kuka najčešće dovodi uz ostale
pridružene bolesti do generalno lošeg zdravstvenog statusa osoba
gerijatrijske populacije. Krajnji stadijum osteoartritisa kuka se
uspešno tretira operativno, totalnom artroplastikom. Nakon totalne
artroplastike kuka sledstvena primena rehabilitacionih programa je
jedini izbor za obnovu mobilnosti i fizičke funkcionalnosti. Cilj:
Da se odredi efektivnost dodatog rehabilitacionog programa za
vežbanje ruku i gornjih partija tela na ishode rehabilitacije nakon
artroplastike kuka. Metod: Studija je dizajnirana kao prospektivna,
paralelna, randomizovana, kontrolna studija, sprovedena na
odeljenju ortopedije i rehabilitacije. U istraživanje je bilo
uključeno ukupno 70 pacijenata, starijih od 60 godina od ukupno
regrutovanih 98 nakon primenjenih kriterijuma isključivanja u
postoperativnoj rehabilitaciji nakon ugradnje totalne artroplastike
kuka. Učesnici interventne grupe su uzeli učešće u suplementarni
program vežbanja ruku i gornjih partija tela uz standardni
rehabilitcioni program, dok su učesnici kontrolne grupe imali samo
standardni rehabilitacioni program. Primarni ishod rehabilitacije
je procenjivan Harisovim upitnikom kojim su se merile promene u
fizičkim sposobnostima i u funkcionalnosti afektiranog kuka, a
sekundarnim ishodima su merene mišićna snaga preko snage stiska
šaka korišćenjem dinamometra kao i kvalitet života korišćenjem
opšeg upitnika o zdravlju SF-36. Rezultati: Interventna grupa je
pokazala signifikantno poboljšanje u odnosu na kontrolnu grupu u
fizičkim sposobnostima merenim Harisovim skorom nakon dve nedelje
(prosečna razlika = 4.7 boda) i nakon 12 nedelja (prosečna razlika
= 5.85 bodova). Takođe su učesnici interventne grupe pokazali
statistički veće poboljšanje nakon 12 nedelja: u boljoj mišićnoj
snazi merenoj stiskom obe šake (prosečna razlika dominantne šake
=4.16 i druge strane=2.8) i u domenu Fizička uloga SF-36 upitnika
(prosečna razlika=6.42 boda). Metodom linearne regresione analize
nakon prilagođavanja na sociodemografske i kliničke karakteristike
dodatni program vežbanja koji je praktikovala interventna grupa se
pokazao kao nezavisni prediktor za bolju fizičku fukcionalnost
(Harisov skor) nakon dve (b=4.593; p=0.018) i nakon 12 (b=3.324;
p=0.005) nedelja; bolju mišićnu snagu (stisak šaka) nakon 12
nedelja za dominantnu (b=1.258; p=0.007) i za nedominantnu šaku
(b=1.176; p=0.001), kao i za bolji kvalitet života povezanog sa
zdravljem SF 36 u sledećim domenima: Fizičko fukcionisanje nakon 2
nedelje (b=5.999; p=0.004), Fizička uloga nakon 12 nedelja
(b=6.909; p=0.040), Telesni bol nakon 12 nedelja (b=10.677;
p=0.002), i Mentalno zdravlje nakon 2 nedelje (b=9.562;
