本研究の目的は,運動習慣と食事制限による脂肪肝予防メカニズムを検討することであった.成長期 Zucker Fatty ラット(遺伝性肥満ラット)を用いて,「食餌制限単独」と「食餌制限と運動の併用」によって体重の増加を抑制した場合の肝脂肪蓄積予防効果を検討し,そのメカニズムについて,①肝への脂肪酸の取り込み②肝脂肪酸β酸化③デノボ脂肪酸合成④脂肪細胞サイズとの関連⑤脂肪組織の脂肪分解能に着目して検討を行った. 本研究の主な知見は,食餌制限単独あるいは食餌制限と運動の併用により体重の増加を同じ程度抑制した場合,その体重抑制方法の違いによって肝脂肪蓄積への影響が大きく異なった点である.本研究において,Zucker Fatty ラットに対する食餌制限単独の実施は,体重の増加を抑制するものの,肝脂肪蓄積の抑制効果が観察されなかった.一方,食餌制限と運動を併用することにより,体重の増加の抑制と共に,顕著な肝脂肪蓄積の抑制が観察された.さらに,そのメカニズムについて,食餌制限単独の実施により確認された肝脂肪蓄積の亢進は,脂肪組織の脂肪分解反応に依存して増加した血中遊離脂肪酸が,肝FAT/CD36の働きによって効率よく肝中に取り込まれた結果である可能性が示唆された.また,食餌制限と運動を併用したことで確認された肝脂肪蓄積の抑制は,運動による脂肪組織の脂肪分解反応の減弱と血中遊離脂肪酸の抑制,肝β酸化の亢進が寄与した可能性が示唆された. したがって,体重の増加を同様に抑制した場合でも,食餌制限単独と食餌制限と運動の併用した場合では,肝への遊離脂肪酸の取り込みや脂肪酸β酸化の違いを介して肝脂肪蓄積が異なることが明らかとなった.この結果は,脂肪肝の予防には,体重の増加を抑制するだけでなく,運動習慣を有することが重要であること示す科学的根拠となり得ると考えられた.
