diabetes is an epidemic in the United States and many parts of the world, especially in the Latino community who are 2.5 times more likely than white, non-Hispanics to develop the disease. Research has shown that risk factors for the development of type 2
diabetes are obesity, heredity, and sedentary lifestyle along with others. However, limited research has been conducted studying whether or not a person???s general
knowledge of
diabetes has an effect on the development of
diabetes in their life. In this study 338 college students from CSU Chico completed a questionnaire assessing their
knowledge of
diabetes, its prevalence in their families, and BMI (Body Mass Index). It was found that more Latino college students had relatives with
diabetes. Latino college students knew significantly less about
diabetes than white, non-Hispanic students and Latino college students had significantly higher BMI???s than white, non-Hispanics students. Implications for the results indicate that Latino college students are at high risk

for developing type 2
diabetes and that immediate action in needed to prevent this. Possible explanations of these results are discussed as well as suggestions for future research.

▼ In the United States (U.S.), Type 2 Diabetes Mellitus (T2DM) has reached epidemic portions with a prevalence of approximately 29.1 million people and is the seventh-leading cause of death (Centers for Disease Control and Prevention [CDC], 2014; Healthy People 2020, n.d.). The U.S. spends an estimated $245 billion in direct and indirect medical costs. The indirect costs include disability and lost productivity (CDC, 2014).
Previous studies focusing on diabetes education acknowledge the significance of self-management activities in managing T2DM. Extant studies examining self-management and treatment outcomes for persons with T2DM have paid limited attention to the relationship between patients' disease perceptions, diabetes knowledge, self-efficacy, self-management practices, and glycemic control. The purpose of this investigation was to identify factors and outcomes associated with healthy diabetes self-management practices among a cohort of individuals with T2DM residing in southern California. Nola Pender's Health Promotion Model (Pender et al., 2015) guided the study. Specific aims include:
1) Provide conceptual clarity for the phenomenon of perception through a concept analysis using Walker and Avant (2011) methodology,
2) Identify factors and outcomes associated with diabetes self-management through a systematic review of the literature, and
3) Examine relationships among perceived self-efficacy, specific patient characteristics, diabetic knowledge, self-management practices, and HbA1C values among a sample of individuals with T2DM living in southern California. The research aims provided the structure to generate new scientific data and evidence towards the improvement of self-management practices and those characteristics leading to improved HbA1C outcomes.

▼ Diabetes is a permanent disease in which the human body‟s cells either do not respond properly to insulin or insulin production is insufficient. If the disease is not treated well and on time, it can lead to severe health problems like heart disease, blindness, failure of kidney, and amputations of the lower extremity. Therefore, this chronic disease needs dietary control, physical exercise and insulin management.
However, among people in the developing countries like Ethiopia, permanent diseases are growing to be causes of death. These problems are becoming worse due to the scarcity of specialists, practitioners and health facilities. In Ethiopia, there has been observed a threat of increased
diabetes prevalence and the number of death rates imputed to
diabetes reached above 21,000 in 2007.
In an effort to address such problem, this study attempts to design and develop a prototype self-learning
knowledge-based system that can provide advice for physicians and patients to facilitate the diagnosis and treatment of diabetic patients. To this end,
knowledge is acquired using both structured and unstructured interviews from domain experts which are selected using purposive sampling technique from Black Lion Hospital
Diabetes Center. Relevant documents analysis method is also followed to capture explicit
knowledge. Then, the acquired
knowledge is modeled using decision tree that represents concepts and procedures involved in diagnosis and treatment of
diabetes and production rules are used to represent the domain
knowledge and
knowledge-based system is developed using SWI Prolog editor tool. It uses backward chaining which begins with possible solutions or goals and tries to gather information that verifies the solution.
Moreover, in testing and evaluating the prototype system eighteen patients‟ history are selected in order to test the accuracy of the prototype system and also for ensuring whether the prototype system satisfies the requirements of its end-users or not. Thus, the overall total performance of the prototype system is 84.2%. The prototype system achieves a good performance and meets the objectives of the study. However, in order to make the system applicable in the domain area for diagnosis and treatment of
diabetes additional study is needed like updating the rules in the
knowledge base of the system automatically, incorporating a well designed user interface and a mechanism of NLP facilities.

▼ ABSTRACT Background: The prevalence of diabetes is expected to rise all over the world, which puts an increases demand and strain on the current health care systems. Self-care is an important part of caring for patients with diabetes. Objective: The purpose of this study was to describe nurses’ experiences and knowledge regarding assisted self-care of diabetic inpatients within medical care. Method: This study used a qualitative approach with individual interviews. Ten nurses were involved in the interview process and an interview guide with open questions was used. Content analysis of the transcribed material was performed. Results: The nurses experienced their knowledge and understanding related to diabetic selfcare of patients to be varied. Newly diagnosed patients, patients with bad compliance and elderly patients were perceived as more difficult by the nurses involved. Participation in selfcare varied depending on patient compliance, patient ability and if the patient was invited to take part. Nurses knowledge related to diabetes and medicines varied, difficulties were due to lack of knowledge related to abnormal blood sugar levels and insulin doses, as well as being unable to communicate the appropriate information to the patient due to patient inability to comprehend the information. Another difficulty was the lack of time to keep up to date with the latest knowledge regarding diabetic medicines. Nurses wanted to give the patient the correct information and considered this crucial, as well as it being appreciated by the patients. Conclusion: The nurses’ perception of their knowledge and experiences varied. An increased requirement of knowledge within diabetes and self-care was wanted. Nurses should feel confident when caring for patients with assisted self-care and be able to inform, support and encourage the patient to be involved in self-care. Continued training and guidance should therefore be provided to nurses working in the medical ward

▼ African Americans are disproportionately represented in the United States’ Type 2
Diabetes Mellitus (T2DM) statistics. This disparity has long been investigated, but no marked improvement has been made. This study examines
knowledge gaps, perceptions of risk and conditions contributing to the persistence of this disparity, utilizing a mixed-methods approach.
This study examines T2DM
knowledge and perceptions of risk in middle-class, African Americans living in New Orleans, Louisiana. Specifically, this study: 1) examines the T2DM prevention literature; 2) assesses
knowledge levels, and perceptions of severity and susceptibility to T2DM in non-diabetic, or asymptomatic, middle-class African Americans; 3) explores the role of built environments on health behaviors; and 4) examines attitudes and feelings of self-efficacy in relation to preventative health strategies that may deter the development of T2MD.
This study found that the majority of the T2DM literature focused on the treatment of T2DM complications (n=177), not prevention of the disease (n=101). Study designs for T2DM prevention varied widely, from pilot and feasibility studies (n=9) to randomized control trials (n=10); the largest study type, however, was observational (n=19). None of the studies reviewed focused on middle-class African Americans. Income levels or socioeconomic status were largely unspecified (n=62).
The study did not find a correlation between participants’ perceptions of risk and actual risk, based on the ADA’s risk assessment (p =.110). There was a statistically significant, negative correlation (p = -.748) found between ADA risk totals and age, with significance set at the 0.01 level. There was also a statistically significant, negative correlation (p = -.214) found between T2DM risks and participant education levels, with significance set at the .05 level.
At large, participants expressed a perceived severity of T2DM. Participants noted, however, that the condition has become somewhat normalized in the African American community. Respondents generally felt sufficiently informed about the condition, which contributed to feelings of high self-efficacy and low levels of concern for developing T2DM. There were
knowledge gaps and misinformation that provide grounds for re-evaluating those feelings, however.

▼ Estudo transversal que teve como objetivo avaliar o conhecimento sobre
diabetes mellitus de alunos do último ano de Cursos de Bacharelado em Educação Física, no interior do estado de São Paulo, em 2012. A amostra aleatória foi constituída por 69 alunos de três instituições de ensino particulares em Educação Física. Para a coleta de dados foi construído um questionário dividido em duas partes. Na primeira parte foram avaliadas as variáveis sociodemográficas, e na segunda, com 27 questões, sendo 19 de múltipla escolha e oito de alternativas verdadeiras ou falsas, as variáveis relacionadas ao conhecimento sobre conceito, sinais e sintomas, e exercício físico. Os dados foram analisados por meio de estatística descritiva. Os resultados mostraram que em relação à caracterização dos alunos encontrou-se que a maioria era do sexo masculino, na faixa etária dos 20 aos 24 anos e solteiros. Quanto à renda familiar 55,0% percebiam de um a seis salários mínimos.
Em relação às questões relacionadas à categoria conceitos obteve-se que o maior índice de acertos, esteve relacionado às características do DM tipo 2, 37 (53,6%), e os menores foram referentes à classificação do DM e ao valor de glicemia capilar ao acaso que reflete a hipoglicemia, 7 (10,1%), respectivamente. Quanto às questões relacionadas à categoria sinais e sintomas obteve-se que o maior índice de acertos foi relacionado aos sinais e sintomas clássicos de hipoglicemia, 27 (39,1%) e os menores índices de acerto foram referentes aos sintomas clássicos do DM, e os sinais e sintomas clássicos de hiperglicemia, 11 (15,9%), respectivamente. No que se refere às questões relacionadas à categoria exercício físico obteve-se que o maior índice de acertos foi relacionado às recomendações para prescrição do exercício físico para indivíduos com DM, ao exercício físico para indivíduos com neuropatia periférica, e ao exercício físico para indivíduos com retinopatia e nefropatia diabética, 55
(79,7%), respectivamente. Já os menores índices de acerto foram referentes à quantidade de carboidratos recomendada para indivíduos com DM em terapia medicamentosa durante exercícios prolongados e intensos, 4 (5,8%). Em relação ao número de respostas corretas e incorretas do questionário obteve-se que 12 (17,4%) alunos acertaram 12 e 13 questões respectivamente. Cabe destacar que 2 (2,9%) alunos não mostraram conhecimento das 27 questões do questionário, mostrando desta forma uma média de 11,1 e um desvio padrão de 3,4. Já em relação ao número de respostas incorretas obteve-se que 13 (18,8%) alunos erraram 15 questões e que um aluno (1,4%) errou 23 questões, constatando assim uma média de 15,0 e um desvio padrão de 2,8. Conclui-se que os alunos apresentaram índice satisfatório de acertos na categoria exercício físico, e insatisfatório na categoria conceitos e sinais e sintomas. ▼ Estudo quantitativo observacional, de intervenção do tipo antes e depois, com objetivo de analisar o efeito do apoio telefônico no conhecimento e atividades de autocuidado de pessoas com
diabetes mellitus tipo 2. Foi realizado no Centro de Saúde Escola de uma cidade do interior paulista. A amostra foi constituída por 48 pessoas que participaram do programa Apoio telefônico para o monitoramento em
Diabetes mellitus - ATEMDIMEL/EERP/USP, em 2013. Para avaliação, foi utilizado um questionário para avaliação das variáveis sociodemográficas, clínicas e antropométricas, e os instrumentos
Diabetes Knowledge Questionnaire e Questionário de Atividades de Autocuidado com o
Diabetes. Para análise, utilizou-se estatística descritiva, os testes Qui-Quadrado de Pearson e Exato de Fisher. A idade dos participantes variou de 47 a 87 anos, com predomínio do sexo feminino (52,1%). A maioria era casado (64,6%) e aposentados (60,4%). A média de anos de estudo foi de 7
anos. O tempo de diagnóstico foi de 11 a 20 anos (37,5%), sendo o tratamento medicamentoso com insulina o mais utilizado (97,9%). A hipertensão arterial foi a comorbidade predominante (75,0%). A maior parte apresentava obesidade grau I (41,7%) antes e após a intervenção. Pela circunferência abdominal, a maioria apresentou alto risco cardiovascular (83,3%) antes da intervenção e após houve aumento para 87,5% dos participantes. A maioria antes da intervenção apresentou hiperglicemia (54,2%), com HbA1c alterada (77,1%), LDL-C classificado com ótimo (64,6%) e HDL-C alterado (95,8%). Para os triglicerídeos e colesterol, a maior parte foi classificado como ótimo. Após a intervenção, a maioria apresentou glicemia de jejum dentro da normalidade (52,1%) e HbA1c também alterada (85,4%). Os níveis de LDL-C permaneceram classificados com ótimo (66,7%) e os de HDL-C alterados (95,8%). Triglicerídeos e colesterol total mantiveram a classificação como ótimo. A maioria das pessoas apresentaram
conhecimento maior ou igual a 8 (87,4%) antes da intervenção e 83,3% após. Sobre o autocuidado, antes da intervenção, os itens que obtiveram seguimento em cinco ou mais dias na semana foram relacionados ao \"consumo de doces\", ävaliar o açúcar no sangue\", \"secar entre os dedos dos pés\", \"tomar os medicamentos do
diabetes\", ütilizar a insulina conforme o recomendadoë \"tomar os comprimidos do
diabetes\". A prevalência da diabetes tipo 2 está a aumentar em todo o mundo. A diabetes tipo 2 é uma doença crónica, associada a graves complicações, que origina elevadas taxas de morbilidade e mortalidade. Verifica-se a persistência do controlo metabólico inadequado, podendo ser reflexo tanto do fracasso do autocontrolo pelas pessoas diabéticas, bem como das estratégias de intervenção inadequadas. O conhecimento e o autocontrolo tornam-se cada vez mais importante no tratamento da diabetes tipo 2, promovendo assim a qualidade de vida das pessoas diabéticas e a diminuição dos custos económicos do processo de cuidados.
Tomando como referência os dados epidemiológicos da diabetes e a evolução dos índices demográficos, como o índice de envelhecimento, na região do Alto Minho, verificamos um enorme problema de saúde pública com perspetivas muito negativas, por isso torna-se de extrema importância o envolvimento dos cidadãos e principalmente das pessoas diabéticas, promovendo a sua literacia em saúde e empoderamento.
Com o objetivo de avaliar a capacidade de autocontrolo, o nível de conhecimento e a qualidade de vida em saúde das pessoas diabéticas tipo 2 seguidas no Hospital Santa Luzia – ULSAM, EPE, face à sua patologia, foi realizado um estudo de natureza exploratória, descritivo-correlacional. A recolha de informação decorreu no período de Maio a Agosto de 2014, através de um instrumento de caraterização sociodemográfica e clínica criado para o efeito, do Diabetes Empowerment Scale - Short Form (DES-SF), do Diabetes Knowledge Test (DKT) e do EuroQol-5 Dimensions (EQ-5D). Estes instrumentos foram aplicados numa amostra aleatória sistemática (n=260) de pessoas diabéticas tipo 2, que se deslocaram nesse período à consulta de diabetes do Hospital Santa Luzia - ULSAM, EPE. Definiu-se o nível de significância de 5%.
O perfil sociodemográfico dos inquiridos carateriza-se por ser, de forma dominante, constituído por pessoas do sexo feminino (53,5%), possuir o ensino básico (75,9%) como habilitações académicas e uma idade média ± dp de 62,7 ± 12,8 anos. Do ponto de vista clínico, os inquiridos apresentaram valores médios ± dp de duração de diagnóstico, de hemoglobina glicosilada e de índice de massa corporal de 14,65 ± 9,468 anos; 7,9 ± 1,5 % e 30,8 ± 5,73 kg/m2 respetivamente, sendo a hipertensão arterial a patologia associada com maior relevo (52,9%). O índice DES-SF apresentou uma pontuação média ± dp de 3,677 ± 0,69, o DKT apresenta valores de respostas corretas, avaliando o conhecimento global médio ± dp em 65,74 ± 12,42%. O índice do EQ-5D apresentou a média ± dp 0,65 ± 0,27. Detetamos uma associação positiva significativa entre o DES e o DKT (rsp=.17, p<.01) e entre o DES e o EQ-5D (rs p=.30, p<.001). diabetes têm aumentado, sendo que a literacia em saúde melhora a capacidade de viver melhor com esta patologia. Este relatório apresenta o projeto desenvolvido na área da promoção da literacia em saúde, durante o Estágio de Mestrado em Enfermagem, tendo como objetivo promover o conhecimento das pessoas idosas do Concelho da Vila do Bispo sobre a DM tipo 2. Foi utilizada a metodologia de planeamento em saúde. Os resultados obtidos mostram que os idosos têm dúvidas acerca da
diabetes relativamente à importância da vigilância, controle, sua evolução e cronicidade, tendo sido realizadas ações de educação para a saúde no sentido de contribuir para o aumento da sua literacia. diabetes literacy in the elderly
The increase in life expectancy is a prevalent evidence, but efforts are necessary to promote health and prevent diseases, because it appears that elderly people are increasingly at high risk of developing pathologies has they age.
Diabetes is a disease whose incidence and prevalence has increased in the last years, particularly seniors.
This internship report features the work developed in the area of nursing care needs in a specific population, having as general objective to promote the
knowledge to the elderly people of the Concelho da Vila do Bispo, about type 2 DM. The methodology of health planning was used, and a descriptive study of quantitative nature and education for health was carried out.
After the identification of these results, it was performed several health educational actions that counted with the involvement of seniors in order to promote information on
diabetes. The work developed evidence the necessity to continue to develop strategies that include the involvement of diabetic people, so that they can obtain gains in health.

Este estudo se trata de uma pesquisa exploratÃria documental transversal de abordagem quantitativa realizada no municÃpio de Sobral Ce Foram analisadas as condiÃÃes de saÃde bucal sistÃmica e sociodemogrÃficos dos pacientes portadores de Diabetes Mellitus Tipo II cadastrados nas Unidades BÃsicas de SaÃde deste municÃpio bem como o nÃvel de conhecimento dos pacientes sobre a doenÃa e sua relaÃÃo com a saÃde bucal A amostra constou de 422 portadores de DM Tipo II que participaram voluntariamente do estudo Os pacientes foram avaliados atravÃs da aplicaÃÃo de um instrumento que abordou dados sociodemogrÃficos; a avaliaÃÃo sistÃmica bucal questionÃrio do conhecimento em diabetes DKN â A e sobre saÃde bucal/diabetes Os fatores sociodemogrÃficos estudados foram: sexo idade estado civil grau de instruÃÃo empregabilidade e renda A condiÃÃo de saÃde bucal avaliou o edentulismo presenÃa de lesÃo bucal e o Ãndice CPOD. anÃlise do IMC glicemia capilar pressÃo arterial tempo de evoluÃÃo da doenÃa circunferÃncia abdominal e terapia medicamentosa Os resultados mostraram que 77,3% dos avaliados eram mulheres 76,1% tinham idade mÃnima de 51 anos 61,85% eram casados e apresentavam baixa escolaridade Cerca de 72,28% dos indivÃduos estavam com sobrepeso e obesos 69,18% dos avaliados tinham hipertensÃo associada ao DM Tipo II Quanto à glicemia capilar 54,9% dos entrevistados possuÃam glicemia capilar acima de 201mg/dl Ao avaliar as condiÃÃes de saÃde bucal 91% dos entrevistados apresentavam-se desdentados parciais ou totais e apenas 2,37% eram portadores de lesÃes bucais Para os Ãndices de CPOD a mÃdia encontrada foi de 27,51 na idade de 60 a 70 anos No que tange à avaliaÃÃo do conhecimento sobre diabetes 61,61% da amostra obteve escores iguais ou inferiores a 6 indicando resultados insatisfatÃrios para conhecimento e compreensÃo do gerenciamento da doenÃa Sobre conhecimentos de saÃde bucal e diabetes

▼ The purpose of this study was to examine the perceived threat of type 2
diabetes in young adults (<30 years of age) with a family history of the disease in comparison to those without a family history; and how the potential perceived threat affects participation in protective health behaviors. 120 participants completed the survey, with the majority being women.
Knowledge of risk factors for type 2
diabetes was also measured. Though no significance between groups was found, the majority of participants did acknowledge the severity of type 2
diabetes but it did affect their participation in healthy eating or exercise. As for
knowledge, the majority of all participants were knowledgeable of the risk factors. The only slight difference was in the older age risk factor. Within the protective health behaviors category, it was found that the majority of participants are non-smoking, and do not participate in enough exercise or fruit and vegetable consumption. This research shows that collecting data via social networking sites is fast and efficient though it does hold potential biases. It is important to acknowledge social networking for opportunities to education yound adults in disease prevention. Based on the data collected, though participants are knowledgeable, they do not parcipate in healthy eating or exercise enough. Continuous awareness and education via social networking sites may be an excellent opportunity for health educators, in hopes of increasing healthy eating and exercise and decreasing
diabetes in the future.

▼ This dissertation presents three separate studies developed to provide structure and evidence-based insight into the characteristics associated with short term and long term foot self-care investments of African Americans living with type 2
diabetes. First, a systematic literature review of thirty-four empirical studies on foot care
knowledge and foot self-care interventions in people living with type 2
diabetes will be presented to determine where further interventions and research are needed in foot care.
Secondly, a qualitative examination of common sense associations of lower extremity disease will be presented. Employing an emergent design, semi-structured interviews were conducted with African Americans with type 2
diabetes. The final sample size comprised 12 individuals. The Self-Regulatory Model of Illness Representations was proposed to assist in interpreting the qualitative findings and to theorize factors associated with making common sense assumptions about type 2
diabetes risks and disease progression.
Lastly, a quantitative examination of foot care
knowledge using a previously validated foot care questionnaire will be discussed. Qualtrics (Provo, Utah) served as the host site for both pilot- and final testing phases of the questionnaire, but hard copies of the questionnaire were also distributed to participants. The final sample comprised a convenience and snowball sample of African Americans living with type 2
diabetes. Principal components analysis identified six subscales with satisfactory internal consistency (alpha = 0.77-0.91).
Prior to this study, very few interventions were available addressing foot care
knowledge and self-care skills within African Americans with type 2
diabetes, very few studies were available that attempted to understand common sense associations of illness representations in African Americans with type 2
diabetes, and there was no standardized instrument for measuring foot care
knowledge and foot self-care among people with type 2
diabetes, despite the devastating effects lower extremity complications have on quality of life. Thus, this study attempts to address the limitations associated with foot care
knowledge and foot self-care skills research and interventions.

▼ Background: Namibia is one of the sub-Saharan African countries where
diabetes mellitus ranks among the top ten health conditions contributing to the disease burden and among the top 15 in-patient causes of death. An understanding of the population's
knowledge, attitudes and perceptions is required to inform health education and interventions targeting
diabetes mellitus. Aim: The aim was to assess the level of
knowledge, attitudes and perceptions about
diabetes mellitus among an adult population living in Windhoek, Namibia. Methodology: A quantitative cross-sectional descriptive survey design was used. Data was collected from 300 adult respondents using a structured questionnaire administered by research assistants. Data was analysed using Epi-Info version 7. Descriptive statistics were used primarily to summarise and describe levels of
knowledge, perceptions and attitudes. A scoring framework was developed to categorize responses. Analytical statistics was used to describe association between
knowledge, attitudes and perceptions scores and demographic and socio-economic variables. A P-value < 0.05 was regarded as statistically significant. Results: Of the 300 respondents interviewed, 50.3% were males, 49.3% were females and 0.4 were missing. The majority of the respondents were between 26 – 30 years. With regards to employment, majority (62%) were employed full time. About 10.7% of the respondents had post-graduate degree, while 3.3% had no schooling.
Knowledge about
diabetes mellitus was higher amongst females (51.2%) compared to males (48.2%) and was associated with age. Only 34.7% of respondents had poor perceptions about
diabetes (i.e. diet, curability and distribution). About 49.7% respondents had good
knowledge about risk factors for
diabetes mellitus while 50.3% had poor
knowledge thereof. Conclusion: This study reveals that the general
knowledge of respondents regarding
diabetes mellitus was poor in Windhoek. Despite the respondents having good
knowledge there were still misconceptions about
diabetes related complications, risk factors and its treatment. In addition, attitudes and perceptions of respondents about
diabetes were favourable and thus suggesting some level of understanding about
diabetes in the different communities in Windhoek. This study shows that there is
knowledge about
diabetes, however lack of
diabetes knowledge among some respondents suggests a need for a systematic education programme for
diabetes. This study highlighted the areas that
diabetes education programmes should focus on aspects or issues such as life style and healthy food intake.

Syftet Var att ta reda på om lärare anser att de har tillräcklig kun- skap om diabetes för att kunna vara delaktig i dess egenvård samt hur de får kunskap och informat- ion om diabetes. Jag ville även ta reda på om de upplever oro kopplat till elevens sjukdom och oro kring ansvarstagandet i samband med elevens egenvård. Följande frågeställningar har använts: Hur upplever lärare det medicinska ansvaret i mötet med elever med diabetes? Vad orsakar oro respek- tive skapar trygghet hos lärare till elev med diabetes? Hur ser lärarna på sin egen kunskap om diabe- tes, anser de sig ha tillräcklig kunskap? Hur har lärarna fått information och kunskap om diabetes? Insamling av data har skett genom skriftliga kvalitativa intervjuer via e-post och litteraturstudie. Re- sultatet visar att kunskapen om diabetes är viktig. När Lärarna har den kunskap som de anser sig be- höva i arbetet med eleven med diabetes leder det till ett tryggare arbete och först då uppfattar de sin kunskap som tillräcklig. Okunskap tillsammans med rädsla för akutsituation är de faktorer som skapar oro hos lärarna. Undersökningen visar på tre källor som lärarna anger som informatörer: från föräldrar, eget initiativ och genom erfarenhet eller bekantskap.

▼ Glycaemic index (GI) categorizes carbohydrate-containing food according to postprandial glycaemic effect. The Canadian Diabetes Association supports integration of GI-education into standard care for type 2 diabetes mellitus (T2DM). Notwithstanding, most Canadian RDs identify as “non-users”; citing a need for studies examining GI-mechanism and GI-efficacy and a lack of reliable educational materials as barriers to GI-utility. These barriers highlight practice-based research opportunities spanning bench-to-bedside, such as: The examination of novel metabolic pathways and evaluation of low GI-education/ diet effectiveness using established markers of utility. The overall purpose of this dissertation was to address, experimentally, gaps in the literature, highlighted by educators as barriers to GI-utility.
METHODS: Three studies were completed. Study one examined the effect of slowing carbohydrate absorption on postprandial oxidative stress (novel GI-mechanism) in overweight/obese participants (n=18). Study two involved the development and evaluation of a questionnaire designed to evaluate an evidence-based GI-education platform for DM (n = 29), using a one-armed pre/post-intervention pre-test design. Study three (RCT; n=99) was performed to evaluate the effect of the low GI-education platform on glycaemic control in women with GDM using the questionnaire pre-tested in study two. RESULTS: Sipping 75g dextrose solution slowly over 3.5h reduced postprandial oxidative stress compared to consuming it over 5min. ▼ Glycaemic index (GI) categorizes carbohydrate-containing food according to postprandial glycaemic effect. The Canadian Diabetes Association supports integration of GI-education into standard care for type 2 diabetes mellitus (T2DM). Notwithstanding, most Canadian RDs identify as “non-users”; citing a need for studies examining GI-mechanism and GI-efficacy and a lack of reliable educational materials as barriers to GI-utility. These barriers highlight practice-based research opportunities spanning bench-to-bedside, such as: The examination of novel metabolic pathways and evaluation of low GI-education/ diet effectiveness using established markers of utility. The overall purpose of this dissertation was to address, experimentally, gaps in the literature, highlighted by educators as barriers to GI-utility.
METHODS: Three studies were completed. Study one examined the effect of slowing carbohydrate absorption on postprandial oxidative stress (novel GI-mechanism) in overweight/obese participants (n=18). Study two involved the development and evaluation of a questionnaire designed to evaluate an evidence-based GI-education platform for DM (n = 29), using a one-armed pre/post-intervention pre-test design. Study three (RCT; n=99) was performed to evaluate the effect of the low GI-education platform on glycaemic control in women with GDM using the questionnaire pre-tested in study two. RESULTS: Sipping 75g dextrose solution slowly over 3.5h reduced postprandial oxidative stress compared to consuming it over 5min. ▼ American Indians and Alaska Natives are at an increased risk of developing type 2
diabetes and are more likely than the general population to suffer from
diabetes-related complications. This study attempts to clarify the relationships between indigenous
knowledge, land, local history/historical trauma, and
diabetes on the White Mountain Apache Indian Reservation, using place' as an anchoring concept. The concept of place is largely absent in sociological literature, and a growing number of researchers argue that place should be central to sociology. Further, many researchers argue that place and context matter for health and are necessary for a deeper understanding of societal inequalities. I conducted an explanatory, single-case study research design of the White Mountain Apache Indian Reservation supplemented by in-depth interviews. This study is the first step for future work and will serve to develop a working methodology and establish preliminary findings. Quantitative and qualitative data, including interviews with community members, were collected to examine the relationship between indigenous
knowledge, land, the experiences of local history/historical trauma, and
diabetes. Findings suggested that historical experiences and losses associated with those experiences disrupted the Apache way of life, the effects of which are seen today. Historical experiences created changes that affected indigenous
knowledge, including how people interacted with the land, how they prepared and consumed food, and their consequent activity levels. This research contributes to theory by highlighting the role of place, especially the role of place-based history to
diabetes.

▼ A diabetes mellitus é uma doença crónica cujo tratamento exige um papel ativo por
parte do paciente no controlo da doença. A presente investigação teve como principal
objetivo caracterizar uma amostra de utentes com diabetes tipo II, ao nível das
representações da doença, conhecimentos e autocuidados, explorando as diferenças
entre os diabéticos controlados e não controlados. Trata-se de um estudo observacional,
descritivo transversal com análise comparativa dos dados. A amostra foi constituída por
140 utentes de um centro de saúde da região Norte. Os instrumentos utilizados foram
Ilness Perception Questionnaire- Brief (IPQ-B), Questionário dos Conhecimentos da
Diabetes (QCD), Questionário de Avaliação das Actividades de Auto-Cuidado com a
Diabetes (SDSCA). Os resultados da comparação entre diabéticos controlados e não
controlados evidenciaram diferenças significativas na maioria das dimensões do IPQ,
nos conhecimentos acerca da doença e nos autocuidados. Estes dados apontam no
sentido de intervenções diferenciadas por diabéticos controlados e diabéticos não
controlados.
Diabetes mellitus is a chronic disease the treatment of which requires an active role by
the patient to control the disease. This research has as main objective to characterize a
sample of clients with Type II diabetes, the level of illness representations, knowledge
and self-care, exploring the differences between the controlled and uncontrolled
diabetic. This is an observational, cross-sectional comparative analysis. The sample
consisted of 140 users of a health center ARS North. The results obtained in the
comparison between controlled and uncontrolled diabetic there are significant
associations between most subscales of the IPQ with knowledge about the disease. It
was also found significant differences in the dimensions of the most IPQ knowledge and
self-care. These data point towards different interventions for diabetes controlled and
uncontrolled diabetics.

diabetes mellitus is neglected in South Africa. The research on
which this thesis is based assessed factors contributing to
glycaemic control as well as evaluated an intervention aimed at
improving of such control in diabetic inpatients. A survey of
doctors and nurses measuring their perceptions,
knowledge and
attitudes regarding care of diabetic inpatients was done. This
indicated a need for special training in inpatient
diabetes care,
where 90.5% of respondents realised that
diabetes is a serious
condition and 92.2% valued the importance of tight glycaemic
control. Despite these perceptions, the
knowledge of doctors and
nurses caring for diabetic inpatients was suboptimal. A before and
after study regarding an intervention to improve glycaemic control
of diabetic inpatients consisted of a training programme and the
introduction of an inpatient management protocol. The mean blood
glucose on day one of admission after the intervention was
significantly higher than before the intervention (p < 0.001). A
significant improvement in mean blood glucose from day 1 to day 7
of hospitalisation was seen after the intervention (p < 0.001),
which was not significant before (p = 0.33). The proportion of
patients achieving glycaemic control did not significantly differ
before and after the intervention (43.0% versus 43.7%, p = 0.97). A
double blind randomised controlled trial to assess superiority of
Ringer’s lactate solution compared to 0.9% Sodium chloride solution
in the normalisation of pH in patients with diabetic ketoacidosis
was done. The outcome of this study indicated that the time to
normalisation of venous pH (pH > 7.32) (HR: 1.863, CI: 0.937 to
3.705, p = 0.758) was not significantly different between the two
resuscitation fluid groups The time to reach a blood glucose of 14
mmol/L was significantly longer in the Ringer’s lactate group (p =
0.044) and patients needed significantly more insulin (p = 0.02).
The overall conclusion of this study is that there is no
significant benefit in using Ringer’s lactate solution as initial
resuscitation fluid compared to the currently advised 0.9% Sodium
chloride solution.

▼ RESUMO: O aumento da longevidade humana é um facto prevalente na actualidade mas, a requerer grandes esforços na promoção da saúde e prevenção de doenças, estando, ainda, as pessoas idosas sob riscos elevados de desenvolver doenças à medida que envelhecem. Assim, é importante o contributo dos próprios indivíduos no sentido minorar estes riscos e retardar a evolução e agudizações das doenças uma vez manifestadas. Nesse sentido, o objectivo geral deste estudo é descrever de que modo os conhecimentos do diabético, podem estar relacionados com a presença de complicações da doença.
Realizámos um estudo exploratório descritivo, no Hospital Nossa Senhora do Rosário E.P.E, em pessoas idosas residentes no concelho do Barreiro que frequentaram o Hospital Dia de Diabetologia, após as consultas médicas e de enfermagem.
O método utilizado para a selecção da amostra foi a amostragem não probabilística por conveniência (n=50). O instrumento utilizado na colheita de dados foi um questionário, aplicado sob a forma de inquérito, para colheita de dados sócio-demográficos, saúde em geral, estado mental, dados sobre a
diabetes e comorbilidades. Os dados foram tratados informaticamente, recorrendo ao programa estatístico S.P.S.S. e a análise de conteúdo.
Os resultados obtidos demonstraram que os participantes possuíam conhecimentos sobre a sua doença. Verificámos que os inquiridos que já tiveram complicações devido à
diabetes responderam correctamente à questão acerca da medicação ser mais importante que a dieta e o exercício físico para controlar a
diabetes, do que os que nunca tiveram complicações devido à
diabetes, que responderam incorrectamente ou desconheciam a resposta à questão. – – – – ABSTRACT: The increase in human longevity is a fact prevalent nowadays, but to require great efforts on health promotion and disease prevention and is also in the elderly high risk of developing illnesses as they age.
It is therefore important contribution of the individuals themselves towards mitigating these risks and delaying the development and exacerbations of disease once manifested. Thus, the general purpose of this study is to describe how the
knowledge of
diabetes, may be related to complications of the disease.
We conducted an exploratory study in the Hospital Nossa Senhora do Rosario EPE, in elderly people living in the county of Barry, who attended the hospital
Diabetes Day, after the medical and nursing care.
The method used for the sampling was not probabilistic by convenience sampling (n = 50). The instrument used for data collection was a questionnaire applied in the form of survey to collect socio-demographic data, general health, mental state, data on
diabetes and comorbidities. The data were processed by computer, using the SPSS and content analysis.
The results showed that participants had

▼ Introdução: Dada a crescente incidência da Diabetes tipo 2 e considerando que parte do seu controlo metabólico se deve à atitude do paciente face à doença, torna-se importante munir os profissionais de saúde com ferramentas capazes de determinar as carências educacionais dos utentes, permitindo uma intervenção mais personalizada na correção de hábitos nocivos.
Objetivos: Verificação da fiabilidade da “Escala de Conhecimentos da Diabetes” em Portugal. Análise e correlação das diferentes variáveis sociodemográficas e patológicas com o número de respostas corretas.
Metodologia: Aplicação da escala a uma amostra de conveniência de diabéticos tipo 2 do Centro de Saúde da Covilhã. Foi realizada análise descritiva, teste “Alfa de Chronbach”, teste t-Student, teste “one way ANOVA”, teste U de Mann-Whitney e correlações com o teste de Pearson, tendo sido considerados significativos os valores de p <0,05.
Resultados: Na generalidade, as questões obtiveram um valor de alfa de Cronbach>0,800. Diabéticos com melhores resultados demonstraram melhor valor de hemoglobina glicosilada, estabelecendo uma relação positiva entre o conhecimento e o controlo metabólico da diabetes. Apenas 9,2% dos diabéticos apresentou bom conhecimento sobre a doença, sendo que a maioria (65,8%) demonstrou um conhecimento mediano. Os insulinotratados exibiram os piores resultados (77,6% com baixo conhecimento). As variáveis “Via de tratamento” e “Complicações” demonstraram ter impacto no desempenho obtido no questionário (valores p iguais a 0,00 e 0,048, respetivamente). Os diabéticos residentes em áreas rurais, os casados, os sem complicações e os não insulinotratados revelaram-se os mais cumpridores e conhecedores.
Conclusão: Demonstrou-se a fidelidade psicométrica moderada a elevada do teste, assim como uma correlação positiva entre o controlo da diabetes e o desempenho no inquérito. Espera-se que a sua futura aplicação possibilite identificar as razões do descontrolo dos diabéticos, permitindo uma atuação mais individualizada pelos profissionais de saúde.
Introduction: Given the rising incidence of type 2 diabetes and taking into account that much of its control depends on the patient's attitude towards his illness, loomed the need to equip health professionals with capable tools to determine the educational needs of their patients, allowing a more personalized intervention to correct habits that could be harmful to the diabetics’ health.

Atualmente a prática de regular de atividade física é mencionada na literatura como
uma estratégia fundamental no tratamento da diabetes tipo 2, com influencia positiva
na redução das comorbilidades associadas a esta doença. (Sigal et al., 2006; Praet &
van Loon, 2009). No entanto, e apesar deste reconhecimento, a maioria da população
com diabetes tipo 2, apresenta baixos níveis de atividade física que na literatura têm
sido relacionados com conhecimento deficitário ou inadequado acerca dos benefícios
e das recomendações para a mesma (Madden, et. al., 2009). Este estudo foi realizado com o objetivo de determinar o nível de atividade física e de avaliar a sua associação com o conhecimento acerca dos benefícios da atividade física
e recomendações específicas para a sua prática. Trata-se de um estudo observacional,
de natureza descritiva e do tipo Survey (estudo de levantamento) realizado com uma
amostra de 50 indivíduos recrutados a partir da consulta de diabetes de uma Unidade
de Saúde Familiar da região de Setúbal. A recolha de dados foi feita através da
aplicação conjunta, e de uma só vez, de três questionários (Questionário de
caracterização sócio-demográfica; versão portuguesa do International Physical
Activity Questionnaire - IPAQ; Questionário de Avaliação de Conhecimentos acerca
dos benefícios e recomendações específicas da prática regular de atividade física),
adaptados ao método de entrevista telefónica.
Os resultados revelaram que a maioria dos participantes tinham baixos níveis de
atividade física (60%), apesar de demonstrarem bons conhecimentos acerca dos
benefícios da sua prática regular (67%). Nas analises exploratórias verificou-se uma
associação estatisticamente significativa entre as variáveis, “género” (p= 0,045) e
“existência de recomendação para a prática do exercício por parte de um profissional
de saúde” (p=0,017), com os conhecimentos acerca dos benefícios da prática regular
de atividade física. São os indivíduos do género feminino e com a existência de
recomendação para o exercício por parte dos profissionais de saúde, que tendem a
demonstrar um nível mais elevado de conhecimento acerca dos benefícios da
atividade física.
The practice of physical activity has been referred in the research literature as a key
strategy in the management of type 2 diabetes mellitus (T2DM), with positive

▼ Trata-se de uma pesquisa de avaliação, realizada por análise de resultados, do tipo äntes e depois\", cujo objetivo geral foi avaliar a contribuição de intervenções educativas em diabetes mellitus, para o conhecimento sobre a doença, adesão ao tratamento medicamentoso e controle glicêmico das pessoas com diabetes mellitus tipo 2. O estudo foi desenvolvido em unidade ambulatorial de um hospital universitário, nível terciário de atenção à saúde do interior paulista, no período de junho de 2011 a maio de 2013. As intervenções educativas desenvolveram-se em quatro sessões, em grupo aberto, por meio dos \"Mapas de Conversação em Diabetes\", fundamentadas nos pressupostos teóricos da Teoria Social Cognitiva, cujos temas foram respectivamente: \"como o corpo e o diabetes funcionam\", älimentação saudável e atividade física\", \"tratamento medicamentoso e monitoramento da glicose no sangueë ätingindo as metas com a insulina\", com dois momentos
adicionais para a coleta dos dados, antes e após as intervenções. Os dados foram coletados por meio dos instrumentos Diabetes Knowledge Scale (versão brasileira) e Medida de Adesão aos Tratamentos. Mediante os critérios de inclusão e exclusão, a amostra ficou constituída por 82 pessoas, das quais 48 (58,5%) eram do sexo feminino e 34 (41,5%), do masculino, com média de idade de 60,43 (DP= 8,38) anos e escolaridade de 4,86 (DP= 3,86) anos de estudo; 59 (72%) eram casados, 44 (53,7%), procedentes da região de Ribeirão Preto e 44 (53,7%), aposentados ou pensionistas. Entre os outros resultados, destacam-se o tempo médio de diagnóstico da doença de 15,38 (DP= 8,22) anos; 56 (68,3%) possuíam pressão arterial sistólica alterada, 19 (23,2%), pressão arterial diastólica alterada e 43 (52,4%) eram obesos. O uso de antidiabéticos orais foi referido por 71 (86,6%) dos participantes, e o uso de insulina, por 68 (82,9%); 60 (73,2%) relataram o seguimento regular da dieta e 38 (46,3%), a prática
regular de exercício físico. Para o valor de p<0,05, observou-se que, após o término das intervenções educativas, houve melhora do escore médio total das variáveis referentes ao conhecimento, à adesão ao tratamento medicamentoso (antidiabéticos orais) e à taxa de hemoglobina glicada. Na análise dessas variáveis, em suas respectivas categorias, houve um maior número de pessoas nas categorias: \"bom conhecimento(̈p<0,05), ädesão ao tratamento medicamentoso(̈antidiabéticos orais) e \"normalp̈ara a taxa de hemoglobina glicada. The Diabetes mellitus (DM) is a heterogeneous group of metabolic disorders has in common hyperglycemia, result from defects in insulin action or insulin secretion or both. This is one of the health problems of greater magnitude in Brazil. So the selfcare activities are essential in carriers of diabetes mellitus type two to maintain and improve their health. However represent challenge for carries and health professionals. The research aimed to analyze the factors associated to knowledge about diabetes and
to self-care activities of carries of Diabetes mellitus type two. For that was conducted a cross-sectional analytical study with a quantitative approach with 360 carries of diabetes mellitus type two attended at Ambulatory Endocrinology of Federal University of Maranhão. The data collect was conducted in the period from february to july 2015. For data collection was used three instruments, one containing identification data with open and closed questions concerning sociodemographic and clinical variables, self-care activities questionnaire with Diabetes (QAD) and questionnaire of scale knowledge of Diabetes (DKN). We used Spearman coefficient in the correlation between self-care activities with age and time of disease, the MannWhitney test for association of self-care of scores with the sociodemographic and clinical variables and the Chi Square test to association between knowledge and sociodemographic and clinical variables. For interpretation of statistic has adapted results the
significance level of 5% (p < 0.05). The research has shown that carries of diabetes mellitus type 2 have generally low self-care for items general food, physical activity and monitory glycemia and the best self-care activities are related the specific food, medication and care of the feet. Observed that most participants had higher scores to 8, indicates a high degree of knowledge about disease. If found associations between the scores of knowledge and self-care activities with the socioeconomic and clinical variables. It was concluded that there are factors related to socioeconomic and clinical features that interfere with the self-care activities and the knowledge of carries with diabetes mellitus type 2 against disease.

▼ Type 2 diabetes is spiraling upward around the world, ensnaring adults and youth from societies in the throes of industrialization. Over time, uncontrolled diabetes leaves in its wake people facing renal failure, blindness, or heart disease,
and communities with little hope for preventing this "new," chaotic phenomena. Westernized lifestyles (e.g., physical inactivity, processed foods), in concert with human genomes yet being mapped, are recognized explanations for the escalating prevalence.
The web of causation, however, is woven by complex interactions with environmental, sociological, and historical roots. Reservoirs of adaptive capability reside in the knowledge systems of indigenous elders who have, in their lifetimes, known the rarity
of diabetes and the epidemic’s impact on their people. The purpose of this ethnographic study was to document, understand, and support Lakota and Dakota elder women’s beliefs and knowledge about health protection and diabetes prevention. In-depth
interviews among 9 elders served as the primary method to answer these research questions: 1) To what connected factors do Lakota and Dakota elders attribute diabetes? 2) What narratives do they offer that reveal health protection and diabetes prevention
as culturally constituted practices, both in the past and the present? and 3) How do these elders express their concern and share their wisdom? Secondary methods included exploratory fieldwork, observation, and documentary photography. This study
revealed diabetes as an “outside,” or externalizing, disorder for which attributions were both externalizing (e.g., environmental damage, sadness, boarding school eating habits) and internalizing, involving personal imbalance (e.g., inactivity,
indiscriminate eating). Chaos narratives were balanced with narratives of restitution, testimony, quests for cures and meaning, and health wisdom. Emergent themes included: the “place” of place, local knowledge, storytelling, and memory in emotional,
mental, spiritual, and physical health; the goodness of the land; water as a gift of life; respect for all things, including food; gratitude to God; generosity and sharing. Health messages need to be rooted in understanding and cultural values, including
hope.

diabetes (T2DM) has reached epidemic proportions within Māori communities, yet there are major gaps in
knowledge. This impacts on prevention strategies. The purpose of this research was two-fold: to explore the prevention of type 2
diabetes among Māori and to raise critical questions about Māori health research and practice. It was in part a ‘cultural audit’ to illustrate how health research and practice ‘masks’ whiteness, or the majority culture, being a consequence of the dominance of Western science-based theoretical ideas and policy models. Te Tiriti o Waitangi is introduced focussing on the three principles of participation, partnership and protection, and the implications of applying Te Tiriti to nutrition research and practice is discussed.
The first aim of the research was to conceptualise a framework to provide a structural understanding of Māori wellbeing for Māori health research and health-related indices and the thesis begins with a description of the researcher’s own story, her conceptualisation of kaupapa Māori models of
knowledge sharing and a discussion on Māori health models and the significance of kai (food) and physical activity.
The thesis then reports on research which embodies a kaupapa Māori paradigm to address two further aims:
• to compare different dietary approaches in insulin resistant individuals to determine the macronutrient composition most likely to reduce risk of T2DM among Māori;
• to relate body mass index (BMI) to body composition and measures of
diabetes and cardiovascular risk in Māori and non-Māori to determine the appropriateness of applying BMI cut offs based on European populations to Māori.
Two related issues were considered: the most appropriate macronutrient composition for Māori at high risk of T2DM; and whether different BMI or waist circumference cut-offs should be used to define overweight and obesity among Māori.
A ‘Kai Study” was undertaken because dietary advice for people with T2DM is largely based on research in European populations. It is a comparison of the standard
diabetes dietary prescription that emphasises the benefits of fiber rich carbohydrate and fat reduction (HCHF), with a diet compatible with the preferences of indigenous people, including more protein rich food and less carbohydrate (HP). Eighty-four Māori participants were randomised to HCHF, HP or a control group for 24 weeks. It was found that dietary advice compatible with the food preferences of Māori is associated with changes in body composition which might be expected to reduce risk of type 2
diabetes, whereas standard advice to increase fiber rich carbohydrate containing foods produces little improvement. This confirms the importance of basing nutritional recommendations on research with relevant populations.

▼ A literacia em diabetes parece ser uma das ferramentas fundamentais para combater o aumento de prevalência desta patologia crónica que tem crescido exponencialmente ao longo dos anos quer a nível mundial, quer a nível nacional. Com este estudo pretendeu-se avaliar o nível de literacia em diabetes nos estudantes do ensino superior público do concelho de Mirandela, para, em consequência, fornecer algumas diretrizes para a proposta de um projeto de intervenção nesta comunidade. Teve como objetivos específicos: validar o questionário dos conhecimentos da Diabetes (QCD); relacionar os níveis de conhecimento sobre diabetes com as variáveis sociodemográficas e clinicas. A amostra estudada compreende 432 alunos do ensino superior público do concelho de Mirandela que reponderam ao questionário no período de 15 de fevereiro a 18 de março de 2016. Verificou-se que a aplicação do questionário dos conhecimentos da diabetes (QCD; Sousa & McIntyre, 2003) que avalia os conhecimentos das pessoas acerca da diabetes e seu tratamento, parece ser aceitável na população em geral e o modelo bidimensional obtido com a análise fatorial confirmatória revelou bons índices de ajustamento. Os resultados indicaram um reduzido nível de literacia em diabetes na amostra de participantes e o seu desconhecimento está associado a mitos e falsos conceitos mais do que a incerteza; que o conhecimento na dimensão duração da doença era maior que na dimensão conhecimento global da diabetes (causas, controlo, tratamento e complicações); os alunos mais novos possuem mais conhecimento sobre a diabetes, o género feminino parece revelar maior conhecimento do que o masculino, que os alunos que residem no concelho de Mirandela apresentam menor conhecimento do que os restantes e que os alunos que possuem e residem com familiares com diabetes possuem mais conhecimento do que os outros, embora apresentem igualmente elevado desconhecimento por mitos e falsos conceitos. Conclui-se que existe uma necessidade de aposta em projetos educacionais comunitários para aumentar a literacia em diabetes.
Alfabetización en la diabetes parece ser una de las herramientas clave para combatir la creciente prevalencia de las enfermedades crónicas que ha crecido exponencialmente en los últimos años y en el mundo a nivel nacional. ▼ Printed educational materials (PEMs) have long been used as a tool for knowledge translation despite the conflicting evidence surrounding their effectiveness. A pragmatic, 2x2 factorial, cluster-randomized controlled trial was designed to ascertain the effectiveness of two distinct formats of a PEM (insert and outsert) at improving adherence to guideline recommendations for the management of diabetes complications among Ontario family and general practitioners. Administrative databases were used to compare patient’s treatment regimens at baseline and one year following PEM mailout to determine whether prescription rates intensified in response to the PEMs. A total of 4,118 practices (4,957 physicians) and 185,454 patients were included. Intensification rates in the four groups were similar and approximately equal to 46%. In intention-to-treat analyses, no treatment effect was found with the insert (OR 0.99, 95% CI 0.96 to 1.02), nor with the outsert (OR 1.01, 95% CI 0.98 to 1.04). Thus, PEMs were not effective at improving physician’s adherence to guidelines for diabetes care.
