▼ OBJECTIVE: To develop an animal model for the implementation of human telomerase enzyme reverse transcriptase (hTERT)-immortalized human corneal epithelial cell line (hTCEpi) transduced with the hygromycin-thymidine kinase gene (HyTK) as a viable cell source for the reconstruction of the corneal surface. METHODS: HyTK cells were cultured in KGM-2 serum-free culture media under hygromycin B selection. Limbal stem cell deficiency (LSCD) was established in athymic nude mice (n=75) using ethylenediaminetetraacetic acid (EDTA) and mitomycin C treatment followed by mechanical debridement of corneal and limbal epithelium. Immunofluorescence (IF) using Anti-Laminin and Propidium iodide (PI) was used to assess presence of basement membrane after epithelial removal. Cultured HyTK cells were stained with Cell Tracker Green CMFDA (5-chloromethylfluorescein diacetate) and transplanted onto the right eye after epithelial removal; the left eye served as a control. Transplanted cells were evaluated at 4 hours, 1 day, and 7 days post-transplantation using laser scanning confocal microscopy (LSCM). At 4 hours and 1 day, corneas were imaged for the presence of CMFDA. At day 7, corneas were stained using antibodies to Keratin 3, Ki-67, and the fluorescent probes – PI, and Phalloidin. Cytotoxicity of ganciclovir was assessed at concentrations of 0.1, 0.5, 1.0, 5.0, and 10.0 μM using Live-Dead Assay. RESULTS: IF confirmed an intact corneal basement membrane following epithelial removal. At 4 hours and 1 day post-transplantation, CMFDA staining demonstrated that transplanted cells were dispersed throughout the corneal surface. After 7 days, HyTK cells showed stratification and IF confirmed a differentiated corneal epithelial phenotype. Incubation in ganciclovir induced a cytotoxic effect on HyTK cells in vitro and had no significant effect on the hTCEpi control cell line. This effect was significant at 0.5 µM (p < 0.05). CONCLUSIONS: These studies demonstrate the first reconstitution of a multi-layered, differentiated corneal epithelium by HyTK cells in the nude mouse model; further, proliferating HyTK cells can be killed with ganciclovir treatment in vitro, which may reduce the potential for risk of oncogenic transformation in vivo. Introduction: It is common to note that corneal transplantation has evolved rapidly in recent years. The development of new lamellar techniques of corneal transplantation has yielded results equivalent or superior to those of penetrating keratoplasty, with regard to postoperative visual acuity, even with reduced risk of complications.Purpose: Aim of the present study is to describe the evolution of indications and techniques of keratoplasty procedures in the largest transplantation center in Greece over a period of 16 years.Methods: All patients who underwent corneal transplantation from 2000 to 2015 in the two ophthalmology clinics of General Hospital of Athens, G. Gennimatas were retrospectively reviewed. The main data analyzed was the annual incidence of various keratoplasty indications and techniques, as well as their respective trends over time.Results: A total of 1256 keratoplasty procedures were performed by three surgeons. Mean recipient age was 62.5 ± 4.7 years. The most common indications were bullous keratopathy (38.4%), followed by regraft (18.6%), corneal scar (10.3%), keratoconus (9.6%), Fuchs’ dystrophy (9.6%). In addition, there was a decreasing tendency of keratoconus (P=0.009), whereas bullous keratopathy (P=0.003) and Fuchs' dystrophy (P=0.001) showed an increasing trend.In terms of keratoplasty techniques, penetrating keratoplasty was the only technique from 2000 to 2009 (when the first lamellar keratoplasty was performed in our hospital) and was limited to 21.4% in the last year of our study. In the same year, DSAEK was the most commonly performed keratoplasty technique (59.8%) and the corresponding percentage of DMEK was 18.8%.Conclusion: Leading indication at our center was bullous keratopathy with increasing trend over the observation period. A tremendous shift from penetrating keratoplasty to more selective techniques such as DSAEK and DMEK was observed, with both techniques having become procedures of choice in the treatment of corneal endothelial disease.

▼ The human cornea, specifically the endothelial cell layer, has little to no regenerative potential. Accordingly, the eye has evolved multifactorial mechanisms that limit local immunogenic inflammation and trauma to the corneal endothelium. This local immune deviation is achieved through multiple anatomical, physiological, and immunoregulatory features intrinsic to the ocular environment that allow the cornea to enjoy an immune privileged status. Corneal immune privilege allows first time recipients of corneal allografts to enjoy a 90% success rate in the absence of systemic immunosuppressive drugs and without prior tissue typing. The myriad of immunoregulatory components that conspire to maintain the immune privilege of the cornea can be assigned to one of two extensively studied subsets. They include: (a) immunosuppressive molecules and membrane molecules present in the aqueous humor and on the surface of corneal cells layers, and (b) cell-mediated regulatory mechanisms such as anterior chamber-associated immune deviation (ACAID) that suppress systemic immune responses. This dissertation proposal investigates the role of a newly identified T cell subset, the Th17 CD4+ T cell lineage and its production of the cytokine, IL-17A in modulating corneal immune privilege and the outcome of keratoplasty. Previously, the Th17 T cell subset and its pro-inflammatory cytokine, IL-17A had been associated with the pathogenesis of several autoimmune diseases and had been implicated in cardiac, lung, and renal allograft rejections. Interestingly, in contrast to the classical paradigms that suggest that CD4+ T cells mediate graft rejection, the IL-17A produced by CD4+ T cells appears to be necessary for corneal allograft survival. My investigations led me to two distinct mechanisms via which IL-17A promotes corneal immune privilege. The results indicate that IL-17A is required for inhibiting the generation of an allospecific Th2 CD4+T cell subset that can independently exacerbate corneal allograft rejection. Simultaneously, IL-17A is also required for the functioning of CD4+CD25+ T regulatory cells (Tregs) that suppress alloimmune responses directed against the corneal transplant. Finally, by comparing and contrasting the cellular and cytokine requirements of corneal immune privilege and ACAID, these investigations unveil the independent mechanisms operating in the establishment of these respective phenomena. These findings redefine the role of IL-17A as a cytokine essential for the maintenance of corneal immune privilege and establish a new paradigm whereby interplay between IL-17A and CD4+CD25+ Tregs is necessary for survival of corneal allografts. ▼ Corneal allografts are the most commonly transplanted solid organs in humans and have a success rate of over 90%. This low incidence of graft rejection is largely due to the unique properties of the eye that decrease the likelihood of mounting an immune response, a phenomenon called "immune privilege". Despite this characteristic, immune rejection remains the leading cause of corneal graft failure, indicating that immune privilege can be abolished. We used a model of corneal transplantation in which C57BL/6 corneal allografts transplanted to BALB/c mice experienced a 50% survival rate. This is in sharp contrast to the 100% rejection that occurs with other organ grafts exchanged between these two mouse strains. The 50% survival rate decreased to 0% in mice with ongoing allergic conjunctivitis. We set out to delineate the mechanisms by which allergic conjunctivitis abolishes immune privilege and results in corneal allograft rejection. Anterior chamber-associated immune deviation (ACAID) is initiated when foreign antigens, such as histocompatibility antigens sloughed from the cornea, enter the anterior chamber of the eye and induce a systemic down-regulation of immune responses. Studies have shown that abolishing ACAID increases the incidence of corneal allograft rejection. The hypothesis that allergic conjunctivitis abolished the induction of ACAID was tested and results showed that allergic conjunctivitis did not. Moreover, allergic conjunctivitis did not cause a qualitative or quantitative difference in cell-mediated immune responses to the donor’s alloantigens. Regulatory T cells (Tregs) have also been shown to be important in corneal allograft survival. Our results indicated that allergic conjunctivitis altered the Tregs that support corneal allograft survival. Th2 cytokines, namely IL-4, generated during allergic conjunctivitis, render effector cells resistant to the suppressive ability of Tregs. Furthermore, allergic conjunctivitis did not exacerbate corneal allograft rejection unless the host’s effector T cells were able to respond to IL-4. Moreover, corneal immune privilege was restored in short ragweed (SRW) pollen-sensitized, allergic mice if they were isolated from SRW pollen for 14 days after receiving a corneal allograft. These results suggest that the exacerbation of corneal allografts in allergic hosts is due to the production of IL-4, which renders effector T cells resistant to Treg suppression. ▼ Modulation of corneal transplant rejection using gene therapy shows promise in experimental models but the most appropriate vector for gene transfer is yet to be determined. The overarching aim of the thesis was to evaluate the potential of a lentiviral vector for use in human corneal transplantation. Specific aims were: (i) to assess the ability of an HIV-1-based lentiviral vector to mediate expression of the enhanced yellow fluorescent protein (eYFP), and a model secreted protein interleukin-10 (IL10), in ovine and human corneal endothelium; and (ii) to examine the influence of lentivirus-mediated IL10 expression on the survival of ovine corneal allografts. Four lentiviral vectors expressing eYFP under the control of different promoters, were tested: the simian virus type-40 (SV40) early promoter, the phosphoglycerate kinase (PGK) promoter, the elongation factor-1alpha (EF) promoter, and the cytomegalovirus (CMV) promoter. Two lentiviral vectors expressing IL10 were tested: one containing the SV40 promoter and another containing a steroid-inducible promoter (GRE5). Lentivirus-mediated expression in transduced ovine and human corneal endothelium was assessed by fluorescence microscopy, real-time quantitative RT-PCR and ELISA, following alterations of transduction period duration (2–24 hr) and vector dose, as well as in the presence or absence of polybrene or dexamethasone (GRE5 vector). It was also compared to expression mediated by adenoviral vectors. Orthotopic transplantation of ex vivo transduced donor corneas was performed in outbred sheep. Allografts were reviewed daily for vascularisation and signs of immunological rejection. Lentivirus-mediated eYFP expression was delayed in ovine corneal endothelium compared to human. However, in both species the final transduction rate was greater than 80% and expression was stable for at least 14 d in vitro. Lentivirus-mediated expression in ovine and human corneal endothelium was higher with the viral promoters in comparison to the mammalian promoters. A 24 h transduction of ovine corneal endothelium with the lentiviral vector encoding IL10 resulted in expression levels which were increasing after 15 d of organ culture but logarithmically lower than those achieved by adenovirus. Shortening the lentiviral transduction period to 2 h led to a reduction in expression, but the addition of polybrene (40 micrograms / ml) to the transduction mixture restored expression to levels comparable to those attained after a 24 h transduction period. Lentivirus-mediated IL10 expression was higher and more rapid in human corneal endothelium compared to ovine corneas. Dexamethasone-responsive transgene expression was observed in both ovine and human corneal endothelium using the lentiviral vector containing the GRE5 promoter. Lentivirus-mediated expression in ovine corneal endothelium was stable for 28 d in vivo. ▼ The human corneal endothelium has very little regenerative capacities and cannot fully heal in response to infection or trauma. Evolutionarily, the eye developed a mechanism to retain visual acuity by protecting the eye from inflammatory damage, referred to as immune privilege. The mechanisms that protect the eye from inflammation-induced injury are 1.) the presence of immunosuppressive molecules within the aqueous humor; 2.) the expression of pro-apoptotic factors on corneal cells ; and 3.) the induction of a form of immune tolerance called anterior chamber-associated immune deviation (ACAID). Immune privilege contributes to the 90% success rate of corneal allografts without the need for histocompatibility matching and use of systemic immunosuppressive therapy. However, when one or more parameters that contribute to immune privilege are broken, the cornel allograft becomes vulnerable to the immune system, resulting in corneal allograft failure. Patients that elect to have a corneal allograft replaced are the population at the highest risk of immune rejection and have only a 70% success rate with the second corneal allograft, and the rate of rejection continues to increase with each successive graft. Why subsequent corneal allografts have an increased incidence of rejection is unknown. Due to the high success rate of corneal allografts, the donor tissues are not tissue matched to the recipients. With the limited documentation on the histocompatibility gene loci expressed by the corneal tissue donors, it is unknown if the rejection of the initial corneal allograft sensitizes the corneal allograft recipient. This study provides evidence that the sensitization of the corneal transplant recipient towards alloantigens also expressed on the subsequent corneal allografts is not a requisite for the high incidence of graft rejection. Furthermore, the enhanced incidence of graft rejection is an immune response directed towards alloantigens expressed on the subsequent corneal transplant. The aim of this study is to determine why the corneal transplantation procedure enhances the rejection of subsequent corneal allografts in both eyes. Experimental evidence demonstrates that the destruction of the corneal nerves in one eye fundamentally alters the expression of the immunoregulatory neuropeptides in the contralateral eye. The altered expression of these neuropeptides inhibits both the induction and maintenance of immune privilege. The alteration in the microenvironment mediated a quick and prolonged loss of immune privilege, which could be reversed by blocking the activity of the pro-inflammatory neuropeptide substance P (SP). The survival of the corneal allograft requires the participation of antigen-specific T regulatory cells. Neuropeptides within the ocular environment are important for immune privilege through the induction of tolerance. ▼ It has previously been demonstrated by our lab that allogeneic MSCs prolong corneal allograft survival however syngeneic MSCs fail to do so. The aim of this study was to investigate if pre-treating syngeneic MSCs with pro-inflammatory cytokines (licensing) can enhance their immunomodulatory properties and enable them to prolong corneal allograft survival. The first part of this work focused on the optimization of an in vitro licensing strategy to most potently enhance syngeneic MSCs immunomodulatory properties. It was determined that licensing MSCs with a combination of TNF-α and IL-1β (MSCTNF-α/IL-1β) was optimum to stimulate their immunomodulatory properties. MSCTNF-α/IL-1β potently suppressed CD4+ and CD8+ T cell proliferation in addition to significantly inhibiting the MHC II expression of CD11b/c+ antigen presenting cells. The immunomodulatory capacity was primarily mediated by NO and also included significant up-regulation of other immune modulatory molecules (IL-10, PD-L1 and IL-6). Further characterisation of TNF-α/IL-1β licensed MSCs demonstrated an increase in surface expression of migration and adhesion associated molecules (ICAM-1, VCAM-1, CD47, CD44 and CD73) and their enhanced migratory potential was confirmed by transwell experiments. Moreover, microarray data indicated an increase in chemokine expression on licensed MSCs identifying an enhanced ability to recruit immune cell populations to their microenvironment. The second part of this work investigated the ability of in vitro licensed syngeneic MSCs to prolong corneal allograft survival. Alterations to the timing strategy demonstrated untreated syngeneic MSCs could increase the allograft survival rate to 50%, an effect that was further enhanced to 70% by TNF-α/IL-1β licensing of the MSCs. Prolongation of graft survival was associated with an initial increase in B220+ regulatory macrophages and Foxp3+ regulatory T cells in the lung and spleen post-administration (day 9), culminating in an increased proportion of Foxp3+ regulatory T cells in the spleen, lung and crucially in the draining lymph node at the average time point of rejection (day 17-19). Finally, we confirmed that NO plays a critical role in mediating licensed MSCs ability to prolong allograft survival as TNF-α/IL-1β licensed MSCs transduced with Nos2-shRNA lentiviral particles lost the ability to significantly prolong allograft survival. In summary, this work demonstrates that TNF-α/IL-1β potently enhances the immunomodulatory properties of syngeneic MSCs both in vitro and in vivo, mediated primarily by the production of NO. In vivo, MSCTNF-α/IL-1β therapy was associated with increased proportions of regulatory cells which resulted in a significant prolongation of corneal allograft survival. Therefore, this project identifies pro-inflammatory cytokine licensing as a strategy to enhance syngeneic MSC therapy for the promotion of allogeneic tissue transplant survival. Organ transplantation is the most preferred treatment modality for patients with end-stage organ disease. At present the supply of cadaver organs is so inadequate as not to be commensurate with need. In India, there are very few Organ Procurement Organizations (OPO s) to supervise the activities of organ donation and transplantation at a national level. Our country seems to lack an active organ sharing network both at the state and national level. The present study focuses on integrating applications of systems and industrial engineering techniques to increase the number of organ transplants in India, particularly at the state level. i.e. in Tamil Nadu (Case study region). The different issues considered for the study include: A retrospective study of the renal failure patients was done to understand the association of the lifestyle factors and the Chronic Kidney Disease (CKD); A system dynamics simulation study was carried out to study the various dynamics involved in organ transplants, in particular corneal transplants.; A website has been designed, developed and launched to promote organ donation awareness among the community, which is most critical to increase the number of transplants in our country; Networking of approved Transplant Centres (TC s) and optimal location for setting up of OPO s were proposed using facility location P-median model. This will facilitate optimal distribution and allocation of deceased organs to recipients in the waiting list without organ wastage due to lapse of the Cold Ischemic Time (CIT); A simulation study was carried out on the world renowned different transplantation programmes and compared with the Cadaver Transplant Program (CTP) which is the only legalized transplantation program in India. ▼ Descemet’s membrane endothelial keratoplasty (DMEK) is the current gold standard treatment for irreversible corneal oedema. After Melles developed this technique in 2006, Muraine proposed in 2013 an alternative technique for the dissection and implantation of the graft. Its main contributions were: hidrodissecting the graft from a partially trephined, inverted donor tissue, and folding the graft over the endothelial side, which favoured the protection of endothelial cells and the graft’s natural tendency to unfold in the receptor’s anterior chamber. The purpose of this doctoral thesis is to compare Muraine’s technique to the Standard through analysis of the postoperative endothelial cell density (ECD) and visual acuity (VA), surgical time, and intraoperative and postoperative complications. An observational, multicentric, prospective, cohorts trial was carried out in Hospital Universitari MútuaTerrassa and Institut de Microcirurgia Ocular in a daily praxis basis. There were follow-up controls over the six months following the surgery, at least at day one, first week and first, third and sixth months. Twenty-seven eyes from 20 patients were included in the Standard technique group. Forty-two eyes from 40 patients were included in the Muraine’s technique group. The ECD at six months was 1488 (1337-1679) cells/mm2 for the Standard group and 1170 (734-1614) cells/mm2 for Muraine’s group. The mean VA at six months was 0.89 for the Standard group and 0.79 for Muraine’s group, in the decimal scale (P=0.19). Around 80% of the eyes reached a VA of 0.5 or higher and 50-70%, 0.8 or higher. The ECD and the percentage of ECD loss with Muraine’s technique at the first month after surgery were equivalent to the Standard technique’s. The percentage of ECD loss at six months was higher with Muraine’s technique, although the ECD was clinically comparable. The VA achieved at six months was equivalent. Muraine’s technique was as safe as the Standard technique for the graft dissection. The incidence of intraoperative complications among the eyes with uncomplicated phacoemulsification was not statistically higher with Muraine’s technique. The graft dissection with Muraine’s technique was slower. Conversely, the unfolding was slightly faster. Both techniques had a high graft survival rate. The most frequent postoperative complication in both groups was cystoid macular oedema. The grafts dissected with Muraine’s technique had a higher incidence of need for rebubbling. Les agressions graves de la surface oculaire peuvent se compliquer d'ulcérations cornéennes épithéliales, de néovascularisation, d'opacification et d'inflammation chroniques, échappant à tout traitement médical. Les progrès dans la compréhension de la physiologie du renouvèlement de l'épithélium et du stroma cornéen ont permis d'introduire une approche thérapeutique, la greffe de cellules souches. L'objectif de notre étude est d'apporter une source de cellules souches cornéennes par thérapie cellulaire afin de restituer une fonction épithéliale et stromale permettant l'obtention d'une cornée claire chez l'animal. Les cellules souches épithéliales limbiques (LSC) et les cellules stromales limbiques (SSC) ont été isolées dans un milieu exempt de produits animaux tout en gardant leur caractère indifférencié. L'effet régénérateur des SSC a été étudié in vivo dans un modèle de fibrose cornéenne chez la souris. Les SSC ont été injectées dans le stroma cornéen et ont permis la restitution d'un stroma organisé et d'une cornée transparente. Le projet a aussi consisté à élaborer et mettre en forme une matrice à base de collagène I afin de favoriser la culture concomitante de tous les types cellulaires présents dans la cornée tout en préservant organisation, transparence et propriétés mécaniques. La connaissance des conditions physico-chimique du collagène a permis de produire des matrices transparentes qui ont pu être colonisées par les SSC et les LSC. Notre étude ouvre de nouvelles perspectives dans le traitement des pathologies provocant des opacités cornéennes afin de faire face au manque de dons de cornée.

Cornea, the outer part of the eye, features high transparency crucial for good vision. The maintenance of a healthy cornea is linked to the presence of corneal stem cells in the limbus (peripheral region of the cornea). Several diseases can lead to its opacification requiring transplantation of donor tissue to restore vision. Although corneal transplantation has achieved clinical success, there is a shortage of donor corneas worldwide. To solve this problem, cell therapy and tissue-engineered artificial cornea are promising approaches that could eventually outperform current treatment. Corneal epithelial stem cells (LSC) and stromal stem cells (SSC) were isolated in feeder free and xeno free medium. The therapeutic effect of SSC was investigated in vivo by their injection in mouse cornea treated with liquid nitrogen. SSC have the ability to restore the extracellular matrix organization and corneal transparency. Transparent collagen I fibrillated matrices have been synthesized and were evaluated for fibril organization, transparency, mechanical properties and their ability to be repopulated by corneal stromal stem cells and to support limbal epithelial stem cell growth. We show that the matrices were organized and transparent. SSC and LSC were able to repopulate and to grow into the collagen matrix. These cells present a potential for stem cell-based treatment of corneal blindness.

▼ INTRODUCTIE: Patiënten na een hoornvliestransplantatie oogdruppelen postoperatief één jaar om afstoting van het hoornvlies te voorkomen. Therapietrouw ten aanzien van oogmedicatie wordt als belangrijke factor gezien voor een succesvolle transplantatie van het hoornvlies. Ondanks de huidige verpleegkundige zorg – in een Nederlands academisch medisch centrum – gericht op het bevorderen van therapietrouw ten aanzien van oogmedicatie, blijft de praktijkervaring van cornea specialisten dat deze patiëntengroep therapieontrouw zijn. DOEL: In dit onderzoek worden problemen en behoeften geïnventariseerd van patiënten ten aanzien van therapietrouw bij oogmedicatie na een hoornvliestransplantatie. Dit dient als basis voor het ontwikkelen van een verpleegkundige interventie gericht op het bevorderen van therapietrouw. METHODE: Een multimethod component design werd gehanteerd. De problemen en behoeften van patiënten werden middels vragenlijsten en semi-gestructureerde interviews geïnventariseerd. Het ASE-model werd als leidraad gebruikt. Vijfenvijftig patiënten – getransplanteerd tussen februari 2007 tot februari 2008 – werden uitgenodigd voor deelname aan het onderzoek. Van deze groep hadden zes patiënten een afspraak in de periode april 2008 binnen de onderzoekssetting en werden gevraagd voor deelname aan de behoefte analyse. Data-analyse gebeurde middels beschrijvende statistiek. RESULTATEN: De respons op de probleemanalyse was 84% (n=46). Achtentwintig procent van de respondenten druppelden niet zoals voorgeschreven. Respondenten hadden problemen op het gebied van de ASE determinanten attitude en barrières, als lage motivatie en vergeten van oogdruppelen. Behoefte aan verpleegkundige ondersteuning bleek voor te komen op het gebied van de ASE determinanten attitude, vaardigheden en barrières. CONCLUSIE: Een verpleegkundige interventie dient gebaseerd te worden op de ASE determinanten attitude, vaardigheden en barrières. Aanbevolen wordt om aanvullend kwalitatief onderzoek te verrichten naar mogelijk meer problemen en behoeften van patiënten in verschillende onderzoekssettings. ▼ Although corneal transplantation success rates are high within the first year, the prognosis is much less favourable 5 years post-transplantation with immune-mediated rejection the primary cause of allograft failure. For this reason, on-going preventive and therapeutic measures are needed. Mesenchymal stem cells (MSCs) are being studied extensively in a wide range of diseases/disorders and more recently in solid organ transplantation, due to their ability to suppress or dampen host immune responses. However, reports on the therapeutic efficacy of MSC therapy have been mixed and the genetic modification of MSCs using viral vectors is one option to improve their therapeutic potential. Consequently, the focus of this research work was twofold. Firstly, to investigate if the genetic modification of MSCs using recombinant adenovirus alters the syngeneic host immune response and secondly, to investigate the therapeutic efficacy of MSCs in the promotion of corneal allograft survival. The results from the first part of this work showed that Ad-transduction of MSCs does not lead to up-regulation of major histocompatibility complex class I and II and co-stimulatory molecule expression. Moreover, Ad-transduction caused no significant changes in terms of pro-inflammatory cytokine expression, chemokine and chemokine receptor and Toll-like receptor expression. Ad-modification of MSCs also had no effect on their ability to suppress T cell proliferation in vitro. In vivo injection of Ad-transduced MSCs did not change the frequency of various immune cell populations either in the blood or in tissues. Overall, the results indicate that Ad-modification has no major influence on the immunological properties of MSCs and therefore can be considered as a suitable gene vector for therapeutic applications of MSCs. In the second part of this work the results showed that, in contrast to syngeneic MSC treatment which failed to significantly prolong survival, corneal allograft survival was significantly prolonged in approximately 90% of allogeneic MSC and 80% of 3rd party MSC treated recipients. Flow cytometric analysis showed lower percentages of infiltrating natural killer (NK) T cells in corneas of both allogeneic and 3rd party MSC treated animals, coupled with a significantly higher frequency of splenic regulatory T cells, compared to controls. Thus, allogeneic and 3rd party MSC treatment prolongs corneal allograft survival by suppressing peripheral immune responses and promoting an intragraft immunoregulatory milieu. THE PURPOSE OF THIS STUDY IS TO PRESENT THE RESULTS OF PENETRATING KERATOPLASTY TO ALL PATIENTS (31 EYES) WITH P.C.E. WHO WERE OPERATED IN UNIVERSITY EYE CLINIC OF THESSALONIKI FROM 1985 TO 1992. THERE WERE 31 PATIENTS WITH THE FOLLOWING INTRAOCULAR LENSES: 1 IRIS - FIXATED, 26 ANTERIOR CHAMBER (22 WITH OPEN LOOPS AND 2 WITH CLOSED LOOPS), AND 4 POSTERIOR CHAMBER. IN 9 PATIENTS THE INTRAOCULAR LENS WAS RETAINED (1 IRIS FIXATED, 6 ANTERIOR CHAMBER, 2 POSTERIOR CHAMBER), IN 12 PATIENTS IT WAS REMOVED (10 ANTERIOR CHAMBER, 2 POSTERIOR CHAMBER) AND IN 10 PATIENTS (ALL WITH ANTERIOR CHAMBER INTRAOCULAR LENSES) WASEXCHANGED WITH ANOTHER ONE PIECE OPEN-LOOP ANTERIOR CHAMBER INTRAOCULAR LENS. IT WAS FOUND THAT 25 PATIENTS (80,64%) STILL HAD CLEAR TRANSPLANT WITH AN AVERAGE OF 31,6 MONTHS AFTER KERATOPLASTY. THE PATIENTS VISUAL AQUITY WAS IMPROVED AFTER PENETRATING KERATOPLASTY. IN SEVEN PATIENTS THE IMPROVEMENT OF VISUAL AQUITY WAS DETERIORATED BECAUSE OF COMPLICATIONS. PENETRATING KERATOPLASTY FOR PSEUDOPHAKIC CORNEAL EDEMA RELIEFS THE PATIENT FROM THE PAIN AND IMPROVES THE VISION.

CORNEAL GRAFTING REMAINS ONE OF THE MOST CHALLENGING OCULAR INTERVENTIONS. SPECIFICALLY, THE IMMUNOLOGICAL PARAMETERS INVOLVED IN THE COMPLEX PROCESS THATDETERMINES THE ULTIMATE FATE OF THE CORNEAL GRAFT REMAIN UNKNOWN. THE PRESENT STUDY IS AN ATTEMPT TO PROVIDE MORE COMPREHENSIVE INFORMATION ON THIS PROBLEM. FIFTY-NINE PATIENTS WERE DIVIDED CLINICALLY INTO 3 GROUPS (LOW, MEDIUM AND HIGH RISK). TISSUE TYPING (HLA-A, HLA-B, HLA-DR AND ABO ANTIGENS) WAS CARRIED OUT IN 10 PAIRS OF DONOR AND RECIPIENT CORNEAS TO DETERMINE THE CLINICAL UTILITY OF CORNEAL EPITHELIAL CELL CULTURE. CONTROL NORMAL CORNEAL TISSUE WAS ALSO CULTURED IN ORDER TO ENHANCE THE RELIABILITY OF THE IN VITRO ASSAY. IN THE FIRST STUDY THERE WAS NO OBVIOUS CORRELATION BETWEEN HLA/ABO TYPING AND CORNEAL GRAFT REJECTION. THE RATE OF GRAFT SURVIVAL IN THE 0-1 HLA ANTIGENS MATCHING GROUP (90%) WAS NOT SIGNIFICANTLY DIFFERENT FROM THAT IN THE 2-4 HLA MATCHING GROUP (93%). ON THE CONTRARY, THE CORNEAL CULTURE STUDY WAS AN ACCURATE PREDICTOR OF THE FATE OF THE CORNEAL GRAFT. THUS, IN 3 CASES DIVERSE MORPHOLOGY OF THE CORNEAL EPITHELIAL CULTURE (PRESENCE OF BLAST CELLS) WAS ASSOCIATED WITH SUBSEQUENT REJECTION OF THE GRAFT. FURTHER RESEARCH IS REQUIRED TODETERMINE THE PRECISE ROLE OF HISTOCOMPATIBILITY IN HIGH RISK PATIENTS UNDERGOING CORNEAL TRANSPLANTATION.

Objetivo: Analisar a influência da espessura corneana central na acuidade visual (AV) corrigida após transplante de córnea endotelial lamelar profundo (TCELP). Métodos: Foram estudados de forma prospectiva 155 olhos de 127 pacientes portadores de ceratopatia bolhosa ou distrofia endotelial de Fuchs no sexto mês de pós-operatório do TCELP, entre março de 2000 e março de 2005. Foram excluídos pacientes com outras alterações oculares que justificassem baixa AV. Todos os pacientes foram submetidos à avaliação oftálmica, quando foram determinadas AV corrigida, por meio de exame refratométrico, e espessura corneana central, através da paquimetria ultra-sônica. As técnicas usada foram previamente descritas. Os olhos foram agrupados de acordo com as medidas de AV: grupo I (20/20 - 20/30), grupo II (20/40 - 20/50), grupo III (20/60 - 20/80), grupo IV (20/100 - 20/400). Para correlação com paquimetria e análise estatística, as medidas de AV foram convertidas da tabela de Snellen para a tabela logarítmica (logMAR). Foram criadas variáveis categóricas para expressar status de faixa de normalidade de espessura corneana (entre 495 e 651 ?m), usando como pontos de corte valores encontrados na literatura. Resultados: A média, o desvio padrão e a variação da paquimetria foi: grupo I (n=38) 571 ±80 um, 408 a 784 um; grupo II (n=79) 598 ±80 um, 437 a 816 um; grupo III (n=30) 605 ±99 um, 454 a 945 um e grupo IV (n=8) 607 ±120 ?m, 410 a 781 ?m. Analisando o resultado da AV e a porcentagem de casos com espessura corneana acima de 651 um, foi observada associação linear significativa (P=0,037; ?2 de tendência linear) entre o aumento da paquimetria e a piora da AV. Analisando a associação entre os grupos de AV e a porcentagem de casos com espessura corneana abaixo da faixa de normalidade (<495 um), não foi encontrada significância estatística (P=0,92; x2 de Pearson). Quando analisado o resultado visual do grupo I em relação ao resultado dos grupos II+III+IV em conjunto, observou-se que somente 13% dos casos do grupo I e 30% dos casos dos demais grupos apresentaram espessura corneana maior do que 651 ?m. Essa correlação demonstrou significância estatística limítrofe (P=0,066; x2 de Pearson com correção de Yates). Conclusão: Observou-se associação linear significativa entre piora da AV corrigida e aumento da espessura corneana central. Quando analisados somente casos com paquimetria abaixo da faixa de normalidade, não foi observada associação significativa entre piora da AV corrigida e espessura corneana central

▼ Aim. The aim of this study was to present different aspects of the outcome after corneal transplantation based on data from the Swedish Cornea Register. Papers. The first paper describes the register and gives descriptive statistics and analysis of data from a two-year follow up, while the last paper presents data from a ten-year follow up. Papers two and three deal with two specific problems in corneal transplantation, astigmatism and corneal oedema after cataract surgery (bullous keratopathy, BK). Paper four and five compare the Swedish patients with a cohort from the Middle East. Results. The major indications were keratoconus (29%), BK (21%) and a mixed group of other diagnoses (32%), including regraft. The overall incidence of rejection at two years was 15%, and regrafting, which occurred in 10% of cases was related to rejection and other complications. Visual acuity (VA) after two years improved most in keratoconus and this was still the case after ten years. Most changes in visual outcome after PK in all indications occurred during the first two postoperative years. Graft survival and VA at ten years depended mainly on complications occurring before two years postoperative. The mean value of astigmatism at two years was 4.6 D (95% CI 4.4-4.7), independent of indication and preoperative astigmatism. In a group with high astigmatism (mean value 8.4 D) relaxing incisions reduced the astigmatism by 50%. At ten years there was a small increase in astigmatism in all indications. Bullous keratopathy was one of the indications with poorest outcome. The risk of developing BK at the time of cataract surgery was influenced by pre-existing endothelial disease and cataract surgery done by phaco-emulsification. In the Palestinian Territories the preponderance of keratoconus was higher than in Sweden. The patients came to surgery with a more advanced disease and more risk factors. They also developed more postoperative complication and the outcome was poorer, even though most gained some visual acuity. Conclusion. Through the data analysed from the register our knowledge of the outcome after corneal transplantation has increased. The register will also allow evaluation of new techniques of corneal transplantation.

▼ Corneal wound healing is a complex, and highly coordinated process of cell-cell signaling, cell spreading, migration, proliferation, and cell-extracellular matrix interaction. The main limitation in natural wound healing is that cells can only fill in the wound void from the outside wound margin. We hypothesize that epithelial cells applied to the interior of a wound will attach to the exposed extracellular matrix of the wound, and accelerate the healing process. In order to deliver epithelial cells to wounds, we have developed a cell transfer contact lens. Our objective was to produce and evaluate transfer contact lens designs that allow for primary epithelial cell attachment and in vitro culture growth, and in addition after application result in the transfer of a portion of these cells to wound areas to aid in re-epithelialization of corneal injuries. In the process of meeting these design criteria, we explored the use of surface topographies and surface coatings to enhance cell transfer. We found that cell transfer from Polydimethylsiloxane contact lenses to extracellular matrix was a function of both cell motility and adhesion. In order to enhance cell transfer through the modulation cell motility and adhesion, we explored some of the underlying biological mechanisms of these two inter-related cellular behaviors. Though there were many potential molecular targets to augment cellular adhesion and motility, we specifically explored the regulation of cell-cell contacts, which have a profound impact on cell phenotype and migration. Normal corneal epithelial cells are motile cells that move slowly as a sheet. In contrast, fibroblasts move more quickly as individual cells. When epithelial cells exist as individual cells at very low cell density, they are more motile and more closely resemble fibroblast cells. Individual epithelial cells do not exhibit cell junction proteins. In this investigation we altered Bves, a protein we have shown is associated with tight junctions, in an effort to make epithelial cells become more like fibroblasts regardless of cell density. This transition from an epithelial cell type to a fibroblastic cell type is called epithelial-mesenchymal transition, and has broad reaching implications for other fields such as metastasis in cancer biology.
