ENGLISH ABSTRACT: Point of care (POC) haemoglobin (Hb) concentration estimations guide acute decisions on red blood cell transfusion. We studied the accuracy of three POC devices when compared to central laboratory Hb testing and how between-method Hb disagreements could affect the decision to transfuse. The Health Research Ethics Committee of the University of Stellenbosch approved a method comparison study of perioperative Hb concentration measurement in arterial blood sampled from 58 adult cardiothoracic surgery patients. The Hb concentration was measured by using two Ilex GEM PremierTM 3500 blood gas analysers (Blood gas A and Blood gas B), a HemoCue® Hb 201+ System (HemoCue), and our central laboratory’s Siemens Advia® 2120 flow cytometry system (Laboratory haemoglobin). We regarded that a between-method Hb difference exceeding 10% (1 g/dL at an Hb value of 10 g/dL), would likely erroneously influence the transfusion decision. Furthermore, one unit of packed red blood cells will increase the Hb by ±1 g/dL and clinically represents the smallest transfusion associated risk that a patient will be exposed to. From the 58 patients included in the study, 70 central laboratory Hb concentration (Laboratory haemoglobin) measurements and 58, 72, and 71 measurements by the HemoCue, blood gas analyser A (Blood gas A) and blood gas analyser B (Blood gas B), respectively were taken. We found that the mean POC (HemoCue, Blood gas A, and Blood gas B) Hb levels underestimated the mean central laboratory Hb level by 0.79 g/dL, 0.81 g/dL, and 0.67 g/dL, respectively. The 95% confidence interval (CI) of the between-method difference revealed that unlike the Blood gas A (0.43 to 1.15 g/dL) and Blood gas B (0.46 to 1.16 g/dL) values, the HemoCue (0.47 to 0.87 g/dL) values did not breach the predetermined 1 g/dL limit. A Bland-Altman analysis revealed similar between-method mean Hb differences. However, the HemoCue upper and lower limits of agreement (LOA) were narrower, and the 95% CI of the LOAs do not overlap with those of Blood gas A and Blood gas B. Overall the HemoCue is more accurate. Also, the 95% CI of the HemoCue’s lower LOA was below 1 g/dL. This device does not give readings that clinically overestimate the Laboratory haemoglobin. Similarly, the mountain plot demonstrates the greater agreement and precision of the HemoCue device, as compared to Blood gas A and Blood gas B. An error grid analysis focused on the Hb 6 to 10 g/dL clinical decision-making range where only a 10% error was permitted. The error grid analysis revealed that the HemoCue and blood gas analysers had potential transfusion errors of less than 5% and more than 20%, respectively. However, none of the evaluated devices produced values in the major therapeutic error zone, where a decision to transfuse or not transfuse blood will be made erroneously.

▼ The primary aims of the thesis were to investigate the respiratory effects of supplemental oxygen (O2) in people with obesity hypoventilation syndrome (OHS), and to investigate the effects of positive airway pressure (PAP) on responses to O2 in people with OHS. In a double-blinded randomised crossover study, 14 participants with stable untreated OHS and 14 age-and gender-matched controls breathed O2 concentrations 28% and 50% each for 20 minutes in random order, separated by a 45-minute washout. OHS participants were then commenced on nocturnal PAP and re-tested after 3 months. Among untreated OHS participants, O2 induced hyperoxia and hypoventilation, leading to rises in arterialised-venous carbon dioxide (PavCO2) of 2.0±1.7 mmHg (28% O2) and 3.7±3.2 mmHg (50% O2) (both p<0.05), and mild acidaemia during 50% O2. In controls, PavCO2 and pH did not change with either O2 concentration. After PAP, OHS participants experienced smaller O2-induced PavCO2 rises of 1.3±2.3 mmHg (28% O2) and 0.9±1.7 mmHg (50% O2), changes which were initially significantly different to responses before PAP, but were non-significant after statistical adjustment. It was concluded that moderate concentration O2 worsens carbon dioxide and pH in people with OHS, and that these responses may be attenuated by PAP. The validity of arterialised-venous blood gas (AVBG) sampling and LifeShirt™ respiratory inductive plethysmography (RIPLS) were also investigated. In the study validating AVBG, 42 paired AVBG and arterial blood samples from OHS and control participants were compared using Bland Altman analysis. It was concluded that AVBG-arterial agreement is acceptable for PavCO2, pH and bicarbonate, allowing this specifically validated tool to be used in the clinical studies. In the study validating RIPLS, it was concluded that RIPLS produces valid measures of respiratory frequency among controls but not OHS participants, and is not valid for quantifying respiratory volumes among either population.

The analysis of gases and metabolites in arterial and venous blood reflects the condition of homeostasis of the organism and which instantaneous energy demand required, indicating how biological systems behave in front of some physical stress, including the variation of intensity and volume of physical activity performed. Reactions of these compounds maintenance of constant blood pH by neutralization and / or elimination of organic acids derived from the metabolism momentary. It was an objective method, minimally invasive, to evaluate the acid-base resulting from the progressive increase of physical exercise from 100μL of blood capillary. The methodology used was the Balke protocol for cycle ergometer adapted to the operational needs of the equipment Gas Analyzer Radiometer ABL 800. The progressive increase of the load was 50 by 50 Watts until voluntary exhaustion with collecting and analyzing samples of capillary blood. Volunteers are 13 active students (5 women and 8 men) and three amateur athletes UFSCar aged between 20 and 30 years 25 2, and body mass index 24 4. The results are presented in the acid base nomogram Siggaard-Andersen, developed by the pH values between 7.43 0.02 and 7.35 0.04, ABE (Excess Base Real) between 4.16 1 44 and -7.75 3.17 mmol / L, pCO2 between 35.48 2.61 and 37.20 4.67 mmHg and [HCO3-] between 24 and 17 0.6 2.2 mmol / L with analysis of statistical tools ANOVA and Student s t test. The concentration of gases and metabolites present in blood capillary, presents the characteristic behavior expected during graded exercise as the exponential increase of lactate concentration and pH of the linear decay.

A análise de gases e metabólitos, de sangue arterial e venoso, reflete a condição de homeostase instantânea do organismo e qual a demanda energética exigida, indicando como os sistemas biológicos se comportam frente a algum stress físico, entre eles a variação da intensidade e volume da atividade física executada. Reações destes analitos garantem a manutenção do pH sanguíneo através da constante neutralização e/ou eliminação dos ácidos orgânicos derivados do metabolismo momentâneo. Foi objetivo um método, minimamente invasivo, para avaliar o equilíbrio ácido base conseqüente do aumento progressivo do exercício físico a partir de 100μL de sangue capilar. A metodologia utilizada contempla o protocolo de Balke para ciclo ergômetro adaptado as necessidades operacionais do equipamento Analisador de Gases ABL 800 Radiometer. ▼ Improvement of chemical capture is an important part of wildlife conservation and animal welfare to minimise distress for the animals and the risk of morbidity and mortality. The aim of this thesis was to improve wildlife immobilisation and anaesthesia by studying physiological variables and clinically evaluate different drug combinations and methods of capture in selected species. Reversible anaesthetic protocols were developed for use in free-ranging lions and four species of South- East Asian primates. Capture and anaesthesia of free-ranging wolverines, brown bears, black and white rhinoceros were physiologically evaluated. The effect of intranasal oxygen supplementation on arterial oxygenation was assessed in brown bears and rhinoceros. Partial reversal of the opioid effect and different body positions were evaluated in rhinoceros. Capture methods used included darting from a helicopter or the ground, and physical restraint followed by drug injection. Medetomidine-ketamine was used in wolverines and medetomidine-zolazepam-tiletamine in primates, lions and brown bears. Rhinoceros were immobilised with a combination of an opioid, azaperone and an alpha2-agonist. Body temperature and cardiorespiratory variables were monitored in all animals. Arterial blood samples were analysed to interpret pulmonary gas exchange and acid-base status in carnivores and rhinoceros. Low doses of medetomidine-zolazepam-tiletamine rapidly anaesthetised primates and lions, and reversal with atipamezole resulted in a smooth and calm recovery. Physiological alterations varied with different protocols and species and included changes in body temperature, respiratory and heart rates, gas exchange and acid-base balance. Hypoxaemia was recorded in all rhinoceros and most carnivore species. The major contributor to hypoxaemia was likely ventilation-perfusion mismatch including shunt. In rhinoceros, hypoventilation contributed to an impaired gas exchange and the animals remained hypoxaemic despite partial reversal. However, in black rhinoceros arterial oxygenation was higher during sternal compared to lateral recumbency. Capture-induced lactic acidaemia was recorded in carnivores and rhinoceros. Intranasal oxygen supplementation improved arterial oxygenation. In conclusion, this thesis increases the understanding of the effects of capture and anaesthesia in several wildlife species. Physiological derangements were identified, potential causative factors were investigated and methods for improvement were evaluated.

Perante a suspeita de intoxicação aguda, a abordagem célere e sistemática ao doente, permite uma rápida prestação de cuidados específicos. A triagem, recolha da história clínica, avaliação e intervenção médica de urgência, o diagnóstico e a terapêutica são processos chave nessa abordagem. O diagnóstico, por norma, é suportado por um painel analítico inicial. O recurso a testes imediatos (“point of care”), permite avaliar não só a condição geral dos doentes em tempo real, mas também os efeitos que os xenobióticos possam ter a nível orgânico e que se traduzam, nomeadamente, em alterações do equilíbrio hidroeletrólitico e estado ácido base. O presente estudo pretendeu avaliar as alterações analíticas, detetadas através da ferramenta ePOC (“Enterprise Point of Care”), em animais suspeitos de intoxicação por inseticidas anticolinesterásicos e a sua eventual influência no diagnóstico e implementação da terapêutica. A amostra estudada consistiu em 14 canídeos, que na triagem médica apresentavam sintomas inespecíficos e outros compatíveis com intoxicação aguda por insecticidas anticolinesterásicos: tremores musculares (93%), hipersiália (64%), hipertermia (57%), cianose (50%), diarreia (57%) e vómito (36%). Foram colhidas amostras de sangue para a realização do teste ePOC e para ulterior análise toxicológica.Os resultados obtidos através do ePOC mostraram diminuição da pressão parcial de dióxido de carbono (pCO2) (64%), aumento da concentração de lactato (36%), aumento dos valores de hemoglobina e hematócrito (50%), hiperglicémia (36%) e aumento dos valores de creatinina (50%), não havendo, contudo, relação estatisticamente significativa entre as variáveis testadas e o diagnóstico toxicológico (p>0.05). Porém, em todos os animais considerados acidémicos houve detecção de compostos do grupo dos inseticidas anticolinesterásicos na análise toxicológica. O estudo clínico individualizado de cada caso permitiu verificar que, apesar da sintomatologia similar, os animais apresentavam distúrbios ácido-base diferentes, sendo por isso também necessária uma abordagem terapêutica diferenciada, fulcral especialmente em condições críticas. Estudos futuros, com uma amostra de maior dimensão e preferencialmente com exposição a diferentes xenobióticos, poderão permitir uma avaliação mais exata e abrangente da relação entre a etiologia das intoxicações e as alterações hidroeletrolíticas e de gases sanguíneas verificadas.

▼ The incidence of post-operative pulmonary complications is large, in consequence of the manipulation of abdominal cavity with changes on pulmonary volumes and capacities, resulting in atelectasis and hypoxemy. Alteration of breath pattern in association with postoperative pain can interfere in gaseous exchanges with larger repercussions in the presence of risk factors. Respiratory rate and arterial blood gases have an important role on the assessment of lung function, specifically on pulmonary ventilation and gaseous exchanges, monitoring carbonic gas arterial pressure and oxygen arterial pressure. The objectives of this research were: analyze the influence of respiratory rate on arterial blood gases at immediate postoperative of exploratory laparotomy for trauma, observe arterial gases concentrations, respiratory rate and oxygenation index in the first three post-operative days. Moreover, analyze if there is a correlation between respiratory rate on arterial gases, and carbonic gas arterial pressure on oxygen arterial pressure in the first three post-operative days. It was a non-experimental cohort study of analytic and descriptive character from April to September 2005 at Governador João Alves Filho Hospital, in Aracaju, during the first three postoperative days. 55 patients of both gender with ages between 18 to 62 were assessed, submitted to exploratory laparotomy for trauma in urgent character. As criteria for inclusion, there were urgency surgical procedures, with xipho-pubic incision, general anesthesia with time larger or equal to 120 minutes, where the respiratory rate and arterial blood collected were measured according to American Association for Respiratory Care guidelines to assess arterial blood gases partial pressures. After collecting the blood the PaCO2 and PaO2 values were achieved and calculated the oxygenation index through PaO2/FiO2 formula. The data were analyzed by SPSS 10.0 software through median, and standard deviation, ANOVA test to analyze the variation between the average median, Tukey test to analyze the difference between significant median and simple linear correlation to observe the behavior of tendencies among the variables, all in significance level of 95% or p<0,05. the study was approved by Ethics Committees of Human Search of Federal University of Sergipe. The outcomes showed median for respiratory rate from 27,49 5,31 bpm (24 h), 25,35 5,32 bpm (48 h), 24,15 3,94 bpm (72 h). For PaCO2 median of 32,84 4,49 mmHg (24 h), 33,65 3,36 mmHg (48 h), 34,04 3,73 mmHg (72 h). For PaO2 median of 85,29 18,30 mmHg (24 h), 87,53 17,56 mmHg (48 h), 89,31 16,57 mmHg (72 h). The analysis of median in the first three post-operative days by ANOVA presented statistic significance only for respiratory rate (p<0,05), while for PaCO2 and PaO2 presented p>0,05. Comparing the variables, it was observed correlation between RR x PaCO2 regular in 24 h (r= - 0,42604), weak in 48 and 72 h (r= - 0,23857 and -0,02807), between RR x PaO2 weak correlations at 24, 48 and 72 h (r= 0,02339, 0,02305 and -0,0505).… Advisors/Committee Members: Valdinaldo Aragão de Melo.

The objectives of this research were: analyze the influence of respiratory rate on arterial blood gases at immediate postoperative of exploratory laparotomy for trauma, observe arterial gases concentrations, respiratory rate and oxygenation index in the first three post-operative days. Moreover, analyze if there is a correlation between respiratory rate on arterial gases, and carbonic gas arterial pressure on oxygen arterial pressure in the first three post-operative days. It was a non-experimental cohort study of analytic and descriptive character from April to September 2005 at Governador João Alves Filho Hospital, in Aracaju, during the first three postoperative days. 55 patients of both gender with ages between 18 to 62 were assessed, submitted to exploratory laparotomy for trauma in urgent character. As criteria for inclusion, there were urgency surgical procedures, with xipho-pubic incision, general anesthesia with time larger or equal to 120 minutes, where the respiratory rate and arterial blood collected were measured according to American Association for Respiratory Care guidelines to assess arterial blood gases partial pressures. After collecting the blood the PaCO2 and PaO2 values were achieved and calculated the oxygenation index through PaO2/FiO2 formula. The data were analyzed by SPSS 10.0 software through median, and standard deviation, ANOVA test to analyze the variation between the average median, Tukey test to analyze the difference between significant median and simple linear correlation to observe the behavior of tendencies among the variables, all in significance level of 95% or p<0,05. the study was approved by Ethics Committees of Human Search of Federal University of Sergipe. The outcomes showed median for respiratory rate from 27,49 5,31 bpm (24 h), 25,35 5,32 bpm (48 h), 24,15 3,94 bpm (72 h). For PaCO2 median of 32,84 4,49 mmHg (24 h), 33,65 3,36 mmHg (48 h), 34,04 3,73 mmHg (72 h). For PaO2 median of 85,29 18,30 mmHg (24 h), 87,53 17,56 mmHg (48 h), 89,31 16,57 mmHg (72 h). The analysis of median in the first three post-operative days by ANOVA presented statistic significance only for respiratory rate (p<0,05), while for PaCO2 and PaO2 presented p>0,05. Comparing the variables, it was observed correlation between RR x PaCO2 regular in 24 h (r= - 0,42604), weak in 48 and 72 h (r= - 0,23857 and -0,02807), between RR x PaO2 weak correlations at 24, 48 and 72 h (r= 0,02339, 0,02305 and -0,0505).… Advisors/Committee Members: Valdinaldo Aragão de Melo.

▼ This work was designed to investigate several aspects of a new type of mechanical ventilation known as high frequency oscillatory ventilation (HFOV) which employs frequencies of 5-40 Hz. The subject has been the focus of a great deal of recent interest and research. There have been two main reasons for this. First, it was shown with HFOV that normal gas exchange could be achieved using tidal volumes that were less than the anatomical dead space, indicating that the mechanisms of gas transport must be quite different from those at physiologic breathing frequencies and tidal volumes. Second, it was suggested that HFOV may offer a useful alternative to artificial ventilation by conventional positive pressure devices. It was considered that, because of lower distal airway pressures, HFOV might lead to less barotrauma, and because of its different gas exchange properties, might be a more effective means of artificial ventilation in the presence of severe lung disease. These hopes for HFOV, however, rested to some extent on its actions on other physiologic systems in the lung. The first part of the study reported herein was undertaken in attempt to elucidate mechanisms of gas exchange. The latter two parts were designed to test the effects of HFOV on lung mucociliary transport and respiratory mechanics. The results can be summarized as follows: 1. Gas Exchange. HFOV was found to markedly alter the pattern of elimination from the lung of 6 simultaneously infused trace gases. The result raised the possibility of a marked change in lung blood flow with altered ventilation-perfusion ratios. However, further studies that were undertaken to investigate this possibility revealed a normal lung blood flow distribution. It was found instead that the conducting airways were participating actively in the elimination of higher solubility gases. ' Additional studies in an airway model suggested that this property of HFOV was dependent on the presence of a wet internal airway surface. This observation led to the development of new theory for gas transport during HFOV which involves a reciprocating exchange of gas with the surface of conducting airways. The mechanism would seem to be most important for very high solubility gases but it might also facilitate C02 removal from the lung. A similar pattern of inert gas elimination was observed in dogs ventilated with a conventional positive pressure ventilator at frequencies 3 to 4 times normal. In this case, however, it appeared to be the result of an increase in alveolar dead space and not enhanced high-solubility gas transport. 2. Mucociliary Transport. A radiolabel surface marker was used to test the effects of HFOV on mucociliary function in the dog lung. HFOV was found to significantly depress the clearance of label from most lung regions over 4 hours, and also appeared to result in the formation of excessive central airway secretions. ENGLISH SUMMARY : Intro: Arterial blood gas (ABG) analysis is a useful tool in point-of‐care testing for patients presenting to an emergency center (EC). Emergency Medicine (EM) doctors need to be equipped with sufficient skills to interpret ABGs in order to effectively manage patients. This prospective descriptive cross-sectional study assessed ABG theoretical knowledge, interpretation skills and confidence in analysing ABGs amongst EM registrars (trainees) and consultants currently enrolled in the Division of Emergency Medicine in the Western Cape. Methods: Thirty EM registrars and twenty-three EM consultants responded to the ABG interpretation questionnaire. Scores were compared to validated expert scores. Confidence with ABG interpretation and satisfaction with current registrar teaching methods was analysed using a 10-point visual analogue scale. Results: The average ABG questionnaire score for the group was 63%. No candidates achieved expert scores. Senior registrars (3rd and 4th years of training) scored highest, followed by consultants and junior registrars (1st and 2nd years of training). There was no significant difference between registrar and consultants scores (21.1 vs. 22 respectively; p=0.72). There was no significant difference in overall and individual test scores between consultants and registrars at different levels of training (h=10.85; p=0.28). Registrars’ self-rated ABG accuracy improved with increasing level of training, although satisfaction with ABG training did not. Registrars preferred future methods of ABG learning were focused EM teaching sessions, ‘on the floor’ ABG teaching in ECs, and access to online resources and case-based tutorials. Conclusion: Mediocre levels of theoretical knowledge and interpretation skills in ABG analysis were evident. Registrars reported dissatisfaction with current registrar ABG teaching. There is scope to improve the current EM curriculum with regards to ABG training, with the potential to reinforce existing registrar teaching sessions, enhance ‘on the floor’ ABG training in ECs, and to investigate and incorporate social media platforms and computer-assisted learning (CAL) techniques into existing teaching modalities. Consultant continuing education (CME) should focus on reinforcing existing ABG knowledge and interpretation skills.

▼ Acid-base status in umbilical cord blood is an objective measure of the fetus’ exposure to and ability to handle hypoxia. The objective of this thesis was to clarify some of the methodological pitfalls in interpreting umbilical cord blood gases and lactate values at birth. Study I pinpoints the methodological confounding in calculating base deficit (BD) with algorithms used in different brands of blood gas analyzers and reports the consequences for diagnosing metabolic acidosis (MA) at birth. Neonatal MA rates cannot be compared between maternity units or between scientific articles where different fetal compartments (blood or extracellular fluid) and different algorithms for calculating BD have been used. Study II addresses the issue of possible diagnostic discrepancies when acid-base parameter value decimals are rounded off. A drift of a dichotomy parameter value cut-off due to decimal rounding will result in a shift in distribution of negative and positive cases in a population sample. The findings warrant a discussion on standardization of round-off rule and the number of decimals for a specific analyte result. Study III demonstrates that delayed cord blood sampling with intact pulsations affects umbilical acid-base values and hematological parameters in both vaginal and cesarean deliveries. The changes were more marked after vaginal delivery. A change towards acidemia and lactemia can be explained by the hidden acidosis phenomenon, i.e. a surge into the central circulation of peripherally trapped acid metabolites when the newborn starts to breathe. Study IV shows that clinical characteristics have a significant influence on the distribution of veno-arterial and arterio-venous gradients (Δ values) in umbilical cord blood. Validation criteria based on fixed ΔpH and ΔpCO2 values may then exclude correct samples of clinical outliers. Lactate cannot be used for validation of umbilical cord blood samples. A negative ΔpO2 value indicates delayed cord blood sampling or mix-up of samples and is the only certain validation criterion.

▼ Pneumoperitoneum leads to multiple changes in the mechanics of respiration, in the heart function, causes subcutaneous emphysema, pneumothorax, pneumomediastinum and carbon dioxide embolism. The aim of this project is to study the arterial blood gases’ and haemodynamic changes due to pneumoperitoneum in laparoscopic cholecystectomy, given the fact that all other parameters are standardized. Two hundred patients with cholecystopathy (100 men and 100 women) who underwent laparoscopic cholecystectomy under general anesthesia with endotracheal intubation were studied. The hemodynamic changes and blood gas changes were studied preoperatively the day before surgery (10.00-11.00 a.m.), intraoperatively (15 to 20 minutes after establishment of pneumoperitoneum) and on the 1st postoperative day (8.00-9.00 a.m.) and were standardized for all patients. Patients with respiratory system diseases, severely ill patients from cardiovascular diseases as well as patients under treatment with b-blockers were excluded, so as to study the changes that are attributed solely to pneumoperitoneum. All patients participating in this study fall under ASA1 and ASA2 categories. All patients were with normal temperature during sampling, and the use of analgesics and fluids was standardized. Pain was scored in the scale of 0-10 (0=no pain, 10=severe pain), yet no patient suffered and their score was 0-3. 59 patients (26 men and 33 women) were operated urgently due to acute cholecystitis, while 141 patients (74 men and 67 women) were operated electively due to cholelithiasis. There was minimal blood loss and no patient was in need of transfusion during the surgery or postoperatively. Anaesthesia parameters were uniformly applied to all patients. Arterial blood sampling was performed from the radial artery with a 26G needle in a 2.5ml heparinized syringe and analysis was done in a Roche OMNI-S blood gas analyzer. Blood pressure and pulse measurements were performed with an electronic blood pressure measuring device and a pulse oxymetry monitor NIHON – KOHDEN, LIFE SKOPE, BSN 5105K. Variables recorded in our database were: gender, age, emergency status of the operation, blood pressure systolic and diastolic, heart rate, hematocrit, hemoglobin and arterial blood gases (pO2, pCO2, pH, BE, SO2, HCO3). Statistical analysis used for measurements of blood gas alterations and hemodynamic changes was paired samples t-test, and the normality of the distributions was checked with Kolmogorof – Smirnov test.In our study, we observed that 15-20 minutes after the induction of the pneumoperitoneum, blood pressure decreased (systolic 6,3% and diastolic 2%), heart rate decreased (18,5%), pCO2 increased (5,76%), pO2 increased (81,2%), pH had no noticeable change, base excess decreased (36,6%), SO2 increased (1,8%), HCO3 had no noticeable changes, hematocrit decreased (5,4%) and hemoglobine also decreased (5,15%). ▼ Abdominal wall hernia repair surgery is one of the most common procedures performed worldwide. Apart from the aesthetic effect that has on human body, it also leads to an increase of the intra-abdominal pressure in the immediate postoperative period, which tends to get normalized in the later postoperative period. The aim of our project is to study the arterial blood gas changes that occur due to the intra-abdominal pressure difference and its consequent effects on the respiratory mechanics, given the fact that all other parameters are standardized.Fifty patients with inguinal hernia were studied (37 men and 13 women), as well as 36 patients with umbilical hernia (15 men and 21 women), and finally 30 patients with postoperative hernia (10 men and 20 women). Among them, 31 patients underwent to emergency operation (13 men and 18 women), and 85 patients were operated electively (49 men and 36 women). Hernia sac content was omentum and intestine. All patients were operated under general anesthesia with endotracheal intubation, for abdominal wall hernia of medium or large size (containing omentum and part of intestine), were with normal temperature during sampling, and the use of analgesics and fluids was standardized. Pain was scored in the scale of 0-10 (0=no pain, 10=severe pain), yet no patient suffered and their score was 0-3. There was minimal blood loss and no patient was in need of transfusion during the surgery or postoperatively. Anaesthesia parameters were uniformly applied to all patients . Arterial blood sampling was performed, with the patients being in the open air, from the radial artery with a 26G needle in a 2.5ml heparinized syringe and analysis was done in a Roche OMNI-S blood gas analyzer. Timing of arterial blood gas sampling was performed preoperatively the day before surgery (11.00-12.00 a.m.), the 1st postoperative day (8.00-9.00 a.m.) and the 8th postoperative day when patients came for the follow up and the removal of the sutures (10.00-12.00 a.m.), and it was standardized for all patients. Patients suffering from severe respiratory diseases were excluded, as well as patients with compromised cardiovascular function (under treatment for heart failure, arrhythmias and coronary syndromes), in order to avoid false negative or false positive results. As a result, the patients participating in this study fall under ASA1 and ASA2 categories. Variables recorded in our database were: arterial blood gases (pH, pO2, pCO2, HCO3, SO2, BE), gender, age, ex operations, diabetes, hypertension, alcohol abuse, hernia size and the emergency status of the operation. ▼ Arterial desaturation in fit athletes, during exercise at an intensity greater than or equal to 90% of VO₂ max has been reported by a number of authors yet the etiology of these changes remain obscure. Inadequate pulmonary ventilation due to a blunted respiratory drive, or lung mechanics has been implicated as a factor in the etiology of this phenomenon. It was the purpose of this experiment to investigate the relationship between arterial desaturation and ventilatory response to hypoxia (HVR). Twelve healthy male subjects ( age = 23.8 ± 3.6 yrs., height = 181.6 ±₋₁ 5.6 cms., Weight = 73.7 ± 6.2 kg., VO₂ max = 63.2 ± 2.2 ml .kg . -1 2 .min⁻¹) performed a five minute exercise test on a treadmill at 100% of VO₂ max. Arterial samples for pH, PCO₂, PO₂, and SaO₂ were withdrawn via an indwelling arterial cannula at rest and every 15s throughout the exercise test. The blood gas samples were analyzed with an Instrument Laboratories 1306 blood gas analyzer. Ventilation and VO₂ were measured by a Beckman metabolic measurement cart. On a separate occasion the ventilatory response to hypoxia (HVR) was determined by recording VE as progressive hypoxia was induced by adding N₂ to a mixing chamber. SaO₂ was measured using a Hewlett-Packard ear oximeter; to maintain isocapnia small ammounts of CO₂ were added to the open circuit system. ANOVA for repeated measured was used to evaluate changes in blood gases, ventilation, and VO₂. Simple linear regression and multiple linear regression was used to evaluate the relationship between the changes in SaO₂ and HVR and the descriptive variables. Subjects showed a significant decline in arterial saturation and PO₂ over the course of the test (p < 0.01,and p < 0.01). Four subjects (Mild) exhibited modest decreases in SaO₂ to (94.6 ± 1.9%), three (Moderate) showed an intermediate response (SaO₂ 91.6 ± 0.1%) and five (Marked) demonstrated a marked decrease in arterial saturation (SaO₂ = 90.0 + 1.2%). The differences in PO₂ and SaO₂ between Mild and Marked groups were significant ( p < 0.05, and p < 0.01); there were no significant differences between groups in VE, VO₂, pH or PCO . There was no significant correlation between the lowest SaO₂ reached and HVR, or any of the descriptive variables. Nine subjects did not reach maximal VE (as determined by the VO₂ max test) on the exercise test, two subjects 2 exhibited similar ventilation, and the remaining subject exceeded maximal VE, but fell into the Mild group with respect to desaturation. Oxygen uptake exceeded that recorded for the VO₂ max determination for four of the five subjects in the Marked group; the remaining subjects demonstrated lower or similar values. It was concluded that arterial desaturation was not related to blunted hypoxic drive.

▼ To investigate the relationship between pulmonary diffusion limitation, ventilation-perfusion (VA/Q) mismatch, pulmonary transit times (PTT) and pulmonary gas exchange during exercise, 10 highly trained male athletes (age=26.4±4.4 years, Height=185.5±5.3cms, Weight=78.2±8.6 kg, V 02max=5.15±0.521-min-1) under went exercise testing at rest (R) and 150W, 300W and maximal exercise (372±22W), corresponding to an oxygen consumption (V0₂) of 0.41±0.09, 2.16±0.17, 4.32±0.35 and 5.13±0.50 1-min-1respectively, while trace amounts of six inert gases were infused via a peripheral vein. Arterial blood samples, mixed expired gas samples and metabolic data were obtained. Observed alveolar arterial difference ([A-a]D0₂(0)was calculated according to the alveolar gas equation. Indices of VA/Q mismatch: LogSDi and Log SDa and predicted [A-a]D0₂([A-a]DO₂(p)) were derived from 50 compartment model analysis of retentions and excretions of the inert gases. Additional indices of '/A/I,) mismatch: DISPR*, DISPE and DISPR*_E and inert gas alveolar difference ([A-a]D, R(A-a)D and E(A-a)D) were obtained directly from the inert gas data. One to two weeks later, the subjects underwent first pass radionuclide angiography using a Siemens ZLC wide field of view gamma camera. Following in vitro labeling with 99mTechnecium, 5-10 ml of the subject's blood, containing 10-20 mCi of activity, were injected at rest. First pass and post-static data were obtained on an ADAC 3003 computer and cardiac output was calculated using the Stewart Hamilton equation. PTT was determined using deconvolution and centroid methods. Gated radionuclide angiography was then performed at rest, 150, and 300W. On a separate occasion, first pass cardiac outputs and pulmonary transit times were obtained at maximal exercise. Mean arterial partial pressure of 0₂ (Pa0₂) decreased significantly from rest to 150W , and from 150 to 300W to a low value of 86±9 torn, before increasing to near resting values at maximal exercise. [A-a]D0₂(3) increased across each exercise levels however only the increase from 150 to 300 W was significant. The overall and perfusion-related indices of VA/Q mismatch showed a significant increase with exercise, mainly as a result of increasing perfusion of areas of high VA/Q [A-a]D0₂(0 was greater than predicted, becoming significant during heavy exercise, indicating diffusion limitation. Cardiac output increased from 6.9±0.9 1-min-1 (R) to 25.2±2.5 1-min-1 at 300W and 33.3±3.7 1-min-1 at maximal exercise. End diastolic volume increased from R to heavy exercise (p < 0.001), accompanied by a decrease in end systolic volume (p =0.05). Stroke volume and ejection fraction also increased significantly from R to 300W (p <0.001). ▼ CO2 as a stressor has been used in earlier studies with chicken embryos. De Smit et al. (2008) stated that higher CO2 levels might even have beneficial effects in preventing ascites-syndrome in ascites-sensitive broilers. For a better understanding about the mechanisms triggered by CO2 two broiler-strains (Ross and Isa), differing in ascites-susceptibility, were compared to each other. The presented results show which influence higher CO2-levels in combination with normoxia or hypoxia and a higher relative humidity in the first ten days of incubation have on embryonic fluids (blood and amniotic fluid). We determined exclusive gas parameters (pH, pO2, pCO2, HCO3- ,Hb, Hkt), electrolyte contents (K+, Na+, Ca2+, Cl-) and metabolites (glucose, lactate) in blood and amniotic fluid and morphological parameters (heart- and bodyweights). Significant differences between the two strains in the gas parameters were achieved by NV (higher amounts of CO2 and a higher relative humidity) until D10. Ross showed lower pH- and pO2-levels in blood as well as significant higher pCO2- and HCO3 – concentrations in blood and amniotic fluid than Isa. However an appropriate oxygen supply in Ross was accomplished by higher amounts of Hb and Hkt compared with Isa. NV stimulated Ross’s metabolism. Thus the carbonic acid- bicarbonate- buffer system has already been trained prenatal (higher HCO3 – level). Due to the embryonic training the efficiency of the buffer system can be improved in the adult animal. Moreover the gas parameters showed a close relation to the embryos´ age. The different, age dependent concentrations between the compartments (blood and amniotic fluid) were maintained by the blood-amnion-barrier. The electrolyte concentrations also showed significant differences between the two strains. NV-Ross showed higher amounts of Na+ and K+ compared to the control group. Isa on the other hand did not reveal any differences between NV- and control group. It is assumed that NV conditions Ross’s hormone system which regulates water movement and excretion. So the positive effect to ascites-susceptibility can be explained. NV-Ross showed higher Ca2+ concentrations in blood than NV- Isa. This is due to eggshell resorption for generating HCO3- in order to buffer the lower pH in blood of NV-Ross. NV led to higher correlations in Na+ and Cl- between blood and amniotic fluid. This confirms the cooperation of these two compartments. The amniotic fluid serves as a reservoir for blood. By the examination of the metabolites it stood out that NV led to differences between the two strains. NV-Ross showed lower glucose levels in blood than Isa. Hypoglycaemia develops due to Ross’s higher metabolism and the higher demand for glucose. Lactate did not show any differences. Even the morphological parameters demonstrated differences between the two strains under NV. The heart- and bodyweights, shown by NV-Ross, were higher than the ones shown by NV-Isa. L’objectif de cette thèse est de détailler les effets respiratoires induits par les associations de l’éthanol au THC et de l’éthanol au GHB. Les études ont été menées chez l’animal non anesthésié par pléthysmographie corps entier pendant les quatre heures suivant l’administration intrapéritonéale. Dans une première étape, les effets respiratoires de la prise isolée d’éthanol et de GHB ont été étudiés. Ces deux substances modifient le mode respiratoire : l’éthanol provoquant une tachypnée dès 3 g.kg-1, le GHB une respiration apneustique dès 600 mg.kg-1, sans insuffisance respiratoire (PaO2 normale). Les modifications des gaz du sang observées : acidémie pour l’éthanol et alcalose pour le GHB sont d’origine métabolique. A ces doses, ces deux substances perturbent aussi la conscience des animaux et la thermorégulation : l’éthanol induit une hypothermie et le GHB une évolution triphasique de la température : hypothermie/hyperthermie/ hypothermie. Les dosages sanguins et les études cinétiques menés lors de ces études confirment la vraisemblance de notre modèle et sa pertinence clinique et médicolégale. L’étude des associations à l’éthanol montre que les effets respiratoires du THC et du GHB sont conservés, seule leur association à la dose de 3 g.kg-1 d’éthanol a provoqué une baisse de la ventilation minute avec réduction du débit inspiratoire mais selon des mécanismes différents : baisse du volume courant pour l’association THC-éthanol et augmentation de la durée des apnées expiratoires pour celle du GHB à l’éthanol. Pour cette dernière, l’interaction cinétique observée après administration intrapéritonéale n’explique pas l’intensité du phénomène, une potentialisation est probable.

The objective of this thesis was to study the time-course of the respiratory effects of THC/ethanol and GHB/ethanol associations. Respiration was studied using whole body plethysmography in non-anesthetized rats during the four hours following intraperitoneal injection. In a first step, both GHB and ethanol affected the respiratory pattern. Ethanol caused bradypnea at doses greater than 3 g.kg-1 and GHB induced apneusis above 600 mg.kg-1. Arterial blood gases modifications were metabolic: acidemia with ethanol and alkalosis with GHB. At those doses, both substances altered consciousness and body temperature of animals. Ethanol induced hypothermia and GHB a temperature triphasic change: hypothermia/ hyperthermia/hypothermia. Blood determinations and kinetic studies reinforced the likelihood, and clinical and forensic relevancies of our animal model. Association studies showed that the respiratory effects of THC and GHB were unchanged in presence of ethanol. Association with 3 g.kg-1 ethanol dose only provoked a reduction in minute volume and in mean inspiratory flow, but mechanisms were different. Ethanol and THC induced a decrease in tidal volume; ethanol and GHB an increase in expiratory apnea duration. The observed kinetic interaction between ethanol and GHB did not explain phenomenon intensity; a potentialization could occur.

▼ Recurrent airway obstruction (RAO) is a chronic disease of the lower respiratory tract in horses with multifactorial aetiology. As RAO is a widespread disease in the horse population, there is a great interest in finding new treatment options in addition to the established therapies, which mainly aim at controlling the symptoms of the disease instead of treating the causes. A therapy option that can modulate immunodeficiency is the inhalation of unmethylated cytosine-phosphate-guanine oligodeoxynucleotides (CpG-ODN or CPG for short). These are short DNA strands which resemble the DNA of bacteria or viruses due to the frequency of the motifs and the degree of methylation and are thus recognized as foreign by the body. These CpG could redirect the immune response from an allergic dominant Th2- to a specific Th1-imprinted immune response. In order to improve the uptake of the CpGs into the cells and to protect them from premature degradation of the DNA by nucleases, gelatin nanoparticles (GNP) are used as the carrier system. In the current double-blinded study, RAO horses were divided into three different treatment groups by a stratified random distribution. The first group was treated with a simple CpG dose (CpGsd) of 187.5 μg CpG/inhalation, the second group with the double dose (CpGdd) of 375 μg CpG/inhalation, and the third group with an established beclometason therapy which is an inhalative corticosteroid. All horses received a total of 10 inhalations. The aim of the study was to demonstrate the efficacy of CpG therapy by inhalation, to evaluate the dose-response effect and the efficacy of CpG therapy in direct comparison with a proven anti-inflammatory inhalative therapy with beclometasone. In this blinded field study, a total of 29 RAO horses were treated in their home stables. The therapeutic effect was measured using the clinical parameters of respiratory rate, breathing type, nasal discharge, auscultation, arterial oxygen pressure, arterioalveolar oxygen difference, interpleural pressure, tracheal mucus, neutrophils in BAL (bronchoalveolar lavage), clinical scoring and HOARSI (Horse Owner Assessed Respiratory Signs Index). For this, all horses were examined three times. The first examination was performed before the start of the treatment (US1), the second on the day after the last treatment (US2). The third examination (US3) took place eight weeks after the last inhalation in order to assess the long-term effect. Directly after treatment (US2), the CpGsd group, which achieved a significant improvement in 90% of the clinical parameters, as well as the CpGdd group, which led to significant improvements in 81% of the measured values, had an advantage over cortisone therapy which only showed significant improvement in 72% of the parameters. The results obtained regarding the long-term evaluation are even clearer. ▼ Background: Tube resistance and apparatus dead space are essential parameters of an endotracheal tube (ETT). They are inversely correlated and may result in impaired gas exchange in the ventilation of premature infants. In 2003, Kolobow and his research team developed an ultrathin-wall, two-stage, twin endotracheal tube (UTTS-T-ETT). This tube’s increased internal diameter and integrated Y piece separating the inspiratory and expiratory limb of the ventilator circuit reduce both tube resistance and dead space. However, by inserting the Y piece use of a conventional in-line flow sensor for volume monitoring is no longer possible. The aim of this doctoral thesis was - based on an in-vitro and an in-vivo study - to investigate the superiority of the UTTS-T-ETT compared with standard ETT with connected flow sensor in terms of tube resistance and dead space. Methods: Tube resistances of the UTTS-T-ETT and of the standard ETT (Portex, Vygon and Mallinckrodt) were investigated by ventilating a lung model (50 ml bellows; compliance 0.6 ml/kPa) at a respirator flow of 6 l/min and 8 l/min. The tubes including ETT connector had a standardized length of 12.6 cm. The impact of the UTTS-T-ETT’s dead space reduction on gas exchange, compared with a conventional ventilation system, was analyzed in a crossover trial in 14 tracheotomized surfactant depleted piglets (aged <12 hours; body weight 705 g to 1200 g). The animals were ventilated alternately for 15 minutes by either a standard ventilation system or the UTTS-T-ETT. In both systems the internal diameter of the ETT was 3.6 mm. Compared with the dead space of the UTTS-T-ETT (1.34 ml), the dead space of the standard ventilation system (3.0 ml) was more than twice the size. Results: Within the in-vitro study the UTTS-T-ETT showed the lowest tube resistance at 98.25±0.43 cmH2O/l/s and 102.63±0.86 cmH2O/l/s at a respirator flow of 6 l/min and 8 l/min, respectively. Contrary to previous publications the tube resistances of the standard ETT were hardly higher than those of the UTTS-T-ETT. The deviations were <7% at 6 l/min and <6% at 8 l/min. Within the in-vivo study a statistically significant improvement in gas exchange was only evident in the smaller animals (body weight <900 g) when using the UTTS-T-ETT. In this weight group paCO2 was decreased by 11% (p<0.01) whereas the decline in paCO2 showed a positive correlation with the paCO2 baseline (p<0.05). Furthermore, paO2 increased by 8.7% (p<0.05) and SaO2 by 2.2% (p<0.05). Conclusion: The investigations showed that compared with the standard ETT with connected flow sensor the UTTS-T-ETT allows reducing both tube resistance and apparatus dead space. However, in the animal model studied the impact on gas exchange was low and therefore the current concomitant loss of volume monitoring seems not to be justified. ΕΠΙΣΗΣ ΟΤΙ Η ΠΙΕΣΗ ΑΠΟΚΛΕΙΣΜΟΥ ΕΙΝΑΙ ΜΙΑ ΑΞΙΟΠΙΣΤΗ, ΑΠΛΗ ΚΑΙ ΧΩΡΙΣ ΚΟΣΤΟΣ ΚΑΙ ΕΠΙΠΛΟΚΕΣ ΓΙΑ ΤΟΝ ΑΡΡΩΣΤΟΜΕΘΟΔΟΣ ΕΛΕΓΧΟΥ ΤΗΣ ΕΠΑΡΚΕΙΑΣ ΤΗΣ ΠΑΡΑΠΛΕΥΡΗΣ ΚΥΚΛΟΦΟΡΙΑΣ, ΓΙΑ ΝΑ ΓΝΩΡΙΖΕΙ Ο ΧΕΙΡΟΥΡΓΟΣ ΠΟΤΕ ΚΑΙ ΕΑΝ ΘΑ ΤΟΠΟΘΕΤΗΣΕΙ ΕΝΔΑΡΤΗΡΙΑΚΗ ΠΑΡΑΚΑΜΨΗ (SHUNT). Η ΠΙΕΣΗΑΠΟΚΛΕΙΣΜΟΥ ΚΑΙ ΤΟ ΗΕΓ ΑΠΟΤΕΛΟΥΝ ΣΗΜΕΡΑ ΤΙΣ ΠΙΟ ΑΞΙΟΠΙΣΤΕΣ ΜΕΘΟΔΟΥΣ ΔΙΕΓΧΕΙΡΗΤΙΚΗΣ ΠΑΡΑΚΟΛΟΥΘΗΣΗΣ ΚΑΤΑ ΤΗΝ ΕΝΔΑΡΤΗΡΕΚΤΟΜΗ ΤΩΝ ΚΑΡΩΤΙΔΩΝ.

THIS STUDY WAS UNDERTAKEN TO ASSESS BRAIN PERFUSION DURING CAROTID ENDARTERECTOMY. FIFTEEN PATIENTS WITH INTERNAL CAROTID ARTERY STENOSIS WERE THE SUBJECTS OF THIS STUDY. THE STUDY INCLUDED THREE PHASES: A) PHASE I BEFORE COMMON AND EXTERNAL CAROTID ARTERY CLAMPING. B) PHASE II WHEN BOTH COMMON AND EXTERNAL CAROTID ARTERY ARE OCCLUDED. C) PHASE III FOLLOWING TWO AND FIVE MINUTES CAROTID CLAMPING. ARTERIAL AND INTERNAL JUGULAR VENOUS BLOOD GASES WERE MEASURED IN ALLPHASES OF THIS STUDY, AS WELL AS THE MEAN COMMON AND INTERNAL CAROTID ARTERYPRESSURE AND STUMP PRESSURE WERE DETERMINED. THIS STUDY SHOWED THAT JUGULAR VENOUS SATURATION AND PO2 DO NOT CORRELATE WELL WITH RCBF OR ICA STUMP PRESSURE. THIS IS IN AGREEMENT WITH MANY OTHER REPORTS. STUMP PRESSURE IN PARTICULAR CORRELATES WELL WITH MEAN ARTERIAL PRESSURE AND CONTINUES TO BE QUITE RELIABLEFOR INDICATING THE ADEQUACY OF COLLATERAL PERFUSION OR FOR IDENTIFYING THE PATIENTS WHO NEED TEMPORARY SHUNTING.
