▼ This research investigated underlying issues that were critical to the success of the bifocal trial and comprised of three studies. The first study evaluated if Chinese-Canadian children were suitable subjects for the bifocal trial. The high prevalence of myopia in Chinese children suggests that genetic input plays a role in myopia development, but the rapid increase in prevalence over the last few decades indicates environmental factors are also important. Since this bifocal trial was conducted in Canada, this work aimed to determine whether Chinese children who had migrated to Canada would still have high myopia prevalence and a high rate of myopia progression. The second study determined the optimal bifocal lens power for myopia treatment and the effect of incorporating base-in prism into the bifocal. In the majority of published myopia control studies, the power of the prescribed near addition was usually predetermined in the belief that the near addition would always help to improve the near focus. In fact, the effect of near addition on the accommodative error might be quite different even for individuals in which the same magnitude of accommodation lag had been measured. Therefore, this work was necessary to guide the selection of bifocal and prism powers most suitable for the subsequent bifocal trial. The third study, the ultimate goal of this research, was to conduct a longitudinal clinical trial to determine if bifocals and prismatic bifocals could control myopia progression in children.

exploring the function of tiny larval eyes? And one might answer
that an exploration of the structure of such eyes and of the
mechanical properties that evolved under their taxing size
constraints has already served to inspire engineers in devising
novel methods of navigation and visual task performance in robots.
Moreover, an exploration of their specializations has also served
to facilitate the study of very particular aspects of vision that
can be found across many other species, including vertebrate
animals and humans. Thus far, attention has been largely focused on
adult compound eyes, but invertebrates also have chambered eyes,
which should probably merit no less attention. Contrary to what is
generally thought about these eyes, some of these visual systems
are highly developed and complex. They too may result in the
discovery of novel visual mechanisms, which may also serve to
inspire engineers or to provide us with a more complete
understanding of vision as well as of the evolution and development
of eyes in general. One might ask what one could possibly gain from
exploring the function of tiny larval eyes? And one might answer

Asian soybean rust caused by Phakopsora pachyrhizi, is one of the main phytosanitary problems of soybean in Brazil. Aiming to reduce the losses caused by the pathogen, the treatment of soybean seeds with fungicide fluquinconazole has motivated researchers to conduct studies and simulate their performance. The compilation of these various studies may be conducted by the meta-analysis, which is a probabilistic technique that produces a result which summarizes studies. In agricultural context, meta-analysis is used to perform empirical estimates of technical efficiency for the development of productivity and economic research of agriculture, such as the use of the fungicide fluquinconazole. Meta-analysis can be applied through existing software such
as R, run through commands, and WMA, which operates through a graphical user interface for data entry made by the user. The results generated by both software are shown in graphical form without providing interactivity with the user, neither reproducing a friendly software and easy to understand interface. This study applied meta-analysis using the WMA Software, which uses R to calculate the statistical results, to determine the efficiency of fungicide fluquinconazole in soybean seed, used to minimize the impact of the asian rust disease in the crop productivity. To provide a dynamic display of obtained results by meta-analysis, this study created a method to identify the best information visualization technique to represent hierarchical data. This method was applied to identify which better visualization technique represented the graphic forest plot, namely, bifocal tree. Changes were made at the WMA, such as: incorporation of a new effect measure, change in the help system and font
files and the implementation bifocal tree technique using Gephi as a support tool. The implementation allowed to create a more interactive environment for analyzing the results of the R and provides two display options for the user: basic and advanced. The advantages of the visualization are that the user can explore more the data and results. In this study it was observed that when fluquinconazole was used to minimize the effects caused by rust soybean productivity (yield) was improved.

However, left ventricular lead implantation is technically unsuccessful in up to13% of cases.The primary aim of this study was to evaluate bifocal right ventricular pacingas a modality of cardiac resynchronization therapy in patients with severe congestiveheart failure and significant intraventricular conduction delay and to compare itsbenefits with biventricular pacing.We also compared the acute hemodynamic changes associated with pacing theright ventricular apex or outflow tract alone, the left ventricle alone, or biventricularpacing of the RVA and LV together.The optimal RV pacing site for cardiac resynchronization remains unclear.The present study compared the acute hemodynamic response to biventricular pacing,at two different right ventricular stimulation sites: right ventricular apex and outflowtract.METHODS AND RESULTS: Acute hemodynamic findings were measuredin 15 patients with severe heart failure and intraventricular conduction defect.Steerable pacing catheters were placed in the high right atrium, right ventricular apex,right ventricular outflow tract and the lateral wall of the left ventricle. Using apulmonary artery flotation catheter (Opti-Q) diastolic pulmonary artery pressure(DPPA) and cardiac index (CI) were measured in different pacing configurations: Comparison between Baseline and left ventricle-based pacingPacing of either the left ventricle alone or in combination with rightventricular pacing resulted in a significant improvement in a number of hemodynamicvariables compared with those observed under basal conditions. Both biventricularpacing modes induced a significant increase in cardiac index (AAI: 2.79±0.46,Apex+LV: 3.08±0.53, Out+LV: 3.02±0.57, p<0.01) and a significant decrease in QRSduration (AAI: 152±16, Apex+LV: 131±21, Out+LV: 133±18, p<0.01). There was nosignificant difference in mean arterial blood pressure between these two pacingprotocols. DPAP decreased significantly during Apex+LV pacing compared withAAI while Out+LV pacing resulted in a nonsignificant decrease in DPAP (AAI:23.45±5,61, Apex+LV: 20,45±7,12, p<0.01 Out+LV: 21.09±6,36, NS).The mean CI increased during pacing of the left ventricle alone compared withthe index obtained during AAI pacing, however, the difference trended towardsignificance (p value 0.06). Compared with AAI pacing, LV pacing was associatedwith a decrease in DPAP (23,45±5,61 vs 19,91±7,88, p<0.01). QRS duration, andmean arterial pressure were not significantly modified between these two pacing sites.Comparison between baseline and right ventricle-based pacingWhen mean CI values under baseline conditions and after right ventricle-basedpacing were compared, there were no significant differences, however, with bifocalpacing (Out+Apex) there was a non significant increase in CI while pacing from theapex resulted in a non significant decrease in CI. ▼ Appropriate glycosylation on biological macromolecules such as proteins and lipids is vital for the normal growth and development of organisms. However, the elaborate set of biological events ensuring proper glycosylation has not been
completely elucidated. Drosophila melanogaster embryonic nervous system synthesizes a family of related glycan species that are structurally similar and recognized by antibodies against the plant glycoprotein, Horseradish peroxidase (HRP). The specific
presentation of HRP epitopes in the developing insect nervous system provides a platform for examining the genetic and molecular pathways controlling tissue-specific glycan expression. Drosophila mutations such as neurally altered carbohydrate (nac),
sugar free frosting (sff) and tollo have been shown to specifically affect the neural expression of HRP epitopes. Transgenic overexpression of a Golgi GDP-fucose transporter or of a Drosophila homolog of the vertebrate SAD kinases rescues HRP-epitope
expression in nac1 and sff mutants, respectively. By in-situ hybridization, sff mRNA is detected at reduced levels in tollo mutants, indicating that sff expression is regulated through Tollo signaling. To further characterize the signaling pathways
regulating neural specific glycan complexity in Drosophila embryos, we undertook differential phosphoproteomic analysis to identify molecular targets of Sff/SAD kinase. Bifocal (Bif), Rasputin (Rin) and Liprin-alpha (Lip-α) were detected among a set of
proteins phosphorylated in wild-type but not in sff mutant embryos. Rin and lip-α genetically interact with sff to affect glycoprotein glycosylation. Confocal analysis of Stage 14 embryos reveals an increase in the colocalization of Rin with Golgi
compartments in sff mutants compared to wild-type. Western analysis detected a decrease in the steady state concentration of Rin protein in sff mutants, consistent with the genetic interaction detected between these two genes. In lip-α/lip-α mutants,
compartmentation of the Golgi apparatus is altered in a manner similar to disruptions previously detected in sff mutants, consistent with a role for phosphorylation of this multi-domain protein in modulating glycoprotein glycan processing. Taken
together, our results provide mechanistic insights into the signaling pathways through which Sff/SAD kinase influences cellular glycosylation in response to intra and extracellular cues.

▼ The limited field of view caused by high magnification in minimally invasive surgery incurs safety concerns for instrument insertion tasks. The Bifocal Display has been designed to address this problem by compressing a wider context area while retaining a magnified focus zone. However, the distortion will still hinder instrument insertion when a large focus zone is required.
This thesis proposes a Dynamic Multifocal Display to improve steering performance in the context area of the visual field. By tracking the instrument tip in real-time, the Dynamic Multifocal Display introduces a local zone of magnification surrounding the instrument tip. Two parameters, the magnification and width of the magnified area around the tip, are investigated in a contrived experimental environment. The results show that both parameters have significant effects on instrument insertion task performance. This finding has important implications for not only minimally invasive surgery but also other relevant teleoperation scenarios.

assentam na hipótese de que o desfocado hipermetrópico retiniano acelera o crescimento axial do
olho, enquanto o desfocado miópico abranda essa progressão. Desta hipótese surgiram duas
teorias para o mecanismo de atuação das lentes de contacto: a capacidade alterar o desfocado
periférico no sentido miópico; ou a correção do elevado atraso acomodativo em jovens com
endoforia em visão de perto (a correção do atraso acomodativo permitiria diminuir o desfocado
hipermetrópico, enquanto manteria a visão binocular equilibrada).
O principal objetivo deste trabalho consistiu em avaliar a influência da diminuição do
diâmetro de zona interna em lentes de contacto bifocais de centro-longe, na resposta acomodativa.
Uma das lentes estudadas apresentava um diâmetro de zona interna de visão de longe de 2,10
mm, e a outra 4,00 mm. A adição presente em ambas as lentes era de +2,00D.
No estudo participaram 17 jovens adultos, com idade média de 26,15±6,06 anos, e esfera
equivalente -2,50±1,75D. A resposta acomodativa foi medida monocularmente com um autorefratómetro
de campo aberto adaptado com um sistema de Badal. Os dois desenhos de lentes
de contacto bifocais foram simultaneamente adaptados a cada sujeito, ficando uma lente no olho
direito e a outra no olho esquerdo, segundo tabela de aleatorização. A resposta acomodativa foi
medida para as seguintes vergências: 1D (1m), 2D (50 cm), 3D (33cm), 4D (25cm) e 5D (20cm).
Como medida de controlo utilizou-se a resposta acomodativa sem lente de contacto. Registou-se
ainda a variação da acuidade visual de alto e baixo contraste com e sem a utilização das lentes de
contacto bifocais.
Os resultados obtidos sugerem que sujeitos jovens adaptados com uma lente de contacto
bifocal com um diâmetro de zona interna de visão de longe mais estreito, reduzem a resposta
acomodativa de forma clinicamente significativa. As lentes bifocais parecem não afetar a acuidade
visual de alto contraste, alterando apenas de forma significativa a acuidade visual de baixo
contraste.

▼ Presbyopia affects individuals from the age of 45 years onwards, resulting in difficulty in accurately focusing on near objects. There are many optical corrections available including spectacles or contact lenses that are designed to enable presbyopes to see clearly at both far and near distances. However, presbyopic vision corrections also disturb aspects of visual function under certain circumstances. The impact of these changes on activities of daily living such as driving are, however, poorly understood. Therefore, the aim of this study was to determine which aspects of driving performance might be affected by wearing different types of presbyopic vision corrections. In order to achieve this aim, three experiments were undertaken. The first experiment involved administration of a questionnaire to compare the subjective driving difficulties experienced when wearing a range of common presbyopic contact lens and spectacle corrections. The questionnaire was developed and piloted, and included a series of items regarding difficulties experienced while driving under day and night-time conditions. Two hundred and fifty five presbyopic patients responded to the questionnaire and were categorised into five groups, including those wearing no vision correction for driving (n = 50), bifocal spectacles (BIF, n = 54), progressive addition lenses spectacles (PAL, n = 50), monovision (MV, n = 53) and multifocal contact lenses (MTF CL, n = 48). Overall, ratings of satisfaction during daytime driving were relatively high for all correction types. However, MV and MTF CL wearers were significantly less satisfied with aspects of their vision during night-time than daytime driving, particularly with regard to disturbances from glare and haloes. Progressive addition lens wearers noticed more distortion of peripheral vision, while BIF wearers reported more difficulties with tasks requiring changes in focus and those who wore no vision correction for driving reported problems with intermediate and near tasks. Overall, the mean level of satisfaction for daytime driving was quite high for all of the groups (over 80%), with the BIF wearers being the least satisfied with their vision for driving. Conversely, at night, MTF CL wearers expressed the least satisfaction. Research into eye and head movements has become increasingly of interest in driving research as it provides a means of understanding how the driver responds to visual stimuli in traffic. Previous studies have found that wearing PAL can affect eye and head movement performance resulting in slower eye movement velocities and longer times to stabilize the gaze for fixation. These changes in eye and head movement patterns may have implications for driving safety, given that the visual tasks for driving include a range of dynamic search tasks. ▼ Dans un contexte de vieillissement de la population, et donc d’augmentation du nombre de presbytes, l’amélioration des optiques destinées à la correction de ce défaut visuel est un enjeu crucial. Nous avons cherché à optimiser le design des optiques bifocales, qui ne présentent que deux puissances différentes: une pour la vision de loin et une pour la vision de près. Pour cela, nous avons simulé des images telles qu’elles seraient vues à travers nos designs, et les avons fait noter par des sujets. La première étude s’est penchée sur le ratio d’aire optimisant la vision au loin comme au près. Les meilleurs résultats ont été obtenus pour un ratio de 40% de vision de près au centre, même si des valeurs de 20 à 50% étaient satisfaisantes. Cependant, ces optiques à deux zones concentriques ont un défaut : elles sont très sensibles au changement de diamètre pupillaire, qui est très fréquent. Un changement de taille de pupille entrainant une modification du ratio d’aires, la solution consiste à utiliser plusieurs zones concentriques alternant entre correction de loin et de près. La seconde étude s’est focalisée sur l’optimisation du nombre de zones concentriques dans ces optiques bifocales par la même méthode. Lorsque le nombre de zones est augmenté suffisamment, des interférences permettent d’augmenter également la qualité de vision intermédiaire, nos profils étant disposés comme des réseaux de Fresnel. Les profils à 5 zones permettent une bonne vision de loin et de près en redonnant une légère vision intermédiaire, et les profils à 8 zones donnent une qualité de vision similaire au loin, en intermédiaire et au près. Ces profils ont ensuite été évalués dans des conditions plus réalistes, en présence d’aberrations, de décentrement, de différents diamètres pupillaires par deux populations d’âge différent. L’âge des sujets n’influence pas leur évaluation des optiques. Les aberrations ont plus d’impact sur la qualité visuelle que le décentrement. Le diamètre pupillaire a une influence plus forte sur les profils possédant moins de zones. Enfin, nous avons comparé nos profils optimisés avec des lentilles de contact disponibles sur le marché. Du point de vue de la qualité visuelle comme de la profondeur de champ, nos profils étaient meilleurs que les lentilles du marché. Il serait donc intéressant d’en créer des prototypes pour effectuer des tests au porté. Les designs développés dans cette thèse pourraient servir à effectuer une personnalisation de la correction optique des presbytes. Différents designs pourraient être proposés pour chaque oeil, permettant de combiner deux profils par sujets. De nombreuses variations inter-individuelles, également observées dans les études précédentes, restent toujours imprévisibles. Elles pourraient être dues à des différences de sensibilité au contraste ou de tolérance au flou. Il serait donc intéressant de développer un test optométrique, consistant à faire évaluer des images floues par le sujet, de façon à prédire s’il s’agit d’un bon candidat au port de lentilles bifocales.

▼ In minimally invasive surgical procedures, limited access to the surgical site, in combination with high magnification and a narrow field of view, create a challenging operating environment for the surgeons. In particular, it has been observed that surgical instruments are not in the surgeons’ field of view at all times, forcing them to manoeuvre the instruments without direct view of the instruments’ tip position and trajectory, which poses a risk of inadvertently causing injury to internal tissues or organs. This thesis investigates the effectiveness of Bifocal displays for addressing this problem, by displaying a wide field of view to facilitate instrument manoeuvring while concurrently providing the high magnification necessary for performing detailed surgical manipulation tasks. Furthermore, the two design parameters that define the nature of the Bifocal transformation function were explicitly defined and their impact on task performance was systematically investigated. These parameters are the maximum magnification level and the detail-to-total ratio – the latter being the proportion of the screen allocated to the magnified view. An experimental platform was developed to evaluate operator performance in both instrument insertion and detailed manipulation type tasks while using the Bifocal display in an environment analogous to minimally invasive surgery. The results show that the Bifocal display significantly improves performance for instrument insertion. Furthermore, it was found that varying the display detail-to-total ratio significantly impacts performance in both instrument insertion and detailed manipulation tasks, regardless of the magnification level, with performance improving in each task in an asymptotic manner. More importantly, it was confirmed that there exists a performance trade-off between these two types of task as a function of varying the detail-to-total ratio. These findings have important design implications for not only minimally invasive surgery but also analogous applications such as microsurgery or remotely operated space or underwater vehicles.

▼ Myopia has become a public health problem, affecting
nearly one-third of people in the United States. There have been
many proposed mechanisms of myopia control,including atropine,
undercorrection, corneal reshaping therapy, progressive
additionlenses, and soft
bifocal contact lenses. Soft
bifocal
contact lenses are thought to decrease the progression of myopia
through peripheral defocus created by the contact lenses. There is
currently only peer-reviewed literature on myopia control with
center distance soft
bifocal contact lenses, so it is unknown
whether center near soft
bifocal contact lenses slow the
progression of myopia. The purpose of this study was to determine
if center near multifocal contact lenses (Biofinity “N”) provide
comparable vision to center distance multifocal contact lenses
(Biofinity “D”), as well as to determine if they provide as much
peripheral myopic defocus as the center distance lenses in children
aged 8 to 14 years. Both subjective and objective vision was
measured while the
subject was wearing each type of lens.
Peripheral defocus in 4 positions of gaze (central, 10° nasal, 20°
nasal, 30° nasal) was measured post-dilation using an
autorefractor. It was found that center distance contact lenses
provide better overall subjective vision than center near (9.0
/10.0 vs. 6.5/10.0, Wilcoxon rank sum p<0.019), as well as
significantly better subjective near vision (9.0/10.0 vs. 6.5/10.0,
Wilcoxon rank sum p<0.009). Center distance contact lenses also
provided better logMAR high contrast distance acuity (+0.02 vs.
+0.14, Wilcoxon rank sum p<0.004), low contrast distance acuity
(+0.17 vs. +0.43, Wilcoxon iii rank sum p<0.001), and high
contrast near acuity (-0.09 vs. +0.05, Wilcoxon rank sum
p<0.023). There was no statistically significant difference
between subjective distance vision for center distance (9.0/10.0)
and center near contact lenses (6.5/10.0, Wilcoxon rank sum
p<0.063). Comfort of the center distance (10.0/10.0) and center
near (9.5/10.0) lenses was also not significantly different
(Wilcoxon rank sum p<0.579). The center distance contact lenses
provided more peripheral myopic defocus at 30° nasal gaze only
(-0.84 vs. 0.07, Wilcoxon rank sum p<0.004). With the exception
of 30° nasal gaze in the center near lenses, both types of contact
lenses provided peripheral myopic defocus. Although center near
multifocal contact lenses may provide enough peripheral defocus to
decrease the progression of myopia, they would not improve vision
or decrease myopia progression more than the center distance
contact lenses already found to be effective for myopia
control.

▼ Before the later nineteenth century, the principle governing nearly all earlier tonal composition was monotonality. Monotonality explains the structure of most tonal compositions, but there are numerous examples for which monotonal analysis is inadequate, especially in music of the later nineteenth century. Deformations of monotonality often arise from off-tonic openings, but some compositions may be understood in terms of two or more co-equal tonics, making monotonal models insufficient. Such compositions exhibit polyfocal tonality, also known as directional tonality. In order to adequately address polyfocal structures, I propose a new method of analysis that combines Schenkerian analysis with interpretive approaches derived from the work of Harald Krebs, Edward T. Cone, and Patrick McCreless. Cone believes that a third level of hearing should be attained in which one synthesizes the diachronic (naïve) and synchronic (retrospective) levels, appreciating each level separately and as a whole. Polyfocal analysis is my representation of Cone’s third level. Additionally, McCreless’s hermeneutical analysis is incorporated into polyfocal analysis because it highlights important chromatic events, showing their relationship to the overall tonal structure of a composition. Such chromatic tonicizations are common in polyfocal structures and will be referred to as tonal allusions. More specifically, proleptic allusion will refer to chromaticism that anticipates a tonic, and analeptic allusion to chromaticism that recalls a tonic. Many composers have written polyfocal compositions, but the first to write a significant number of them is Franz Schubert. Polyfocal tonality is found primarily in Schubert’s Lieder; poetry seems to be the original inspiration for his polyfocal tonality. Polyfocal analysis will be applied to a representative number of songs from this body of early polyfocal repertoire, including revisions he made to some of them. These revisions (some altering the structure from monotonal to polyfocal, or vice versa) may reflect his later reinterpretation of texts, or merely a maturing perspective on the poetry he set. By analyzing Schubert’s Lieder polyfocally, new insights will be gained that contribute to the scholarship on Schubert’s compositional strategies, as well as to the general understanding of polyfocal structures.

▼ Rapid advances in communications and computer technologies in recent years have provided users with greater access to large volumes of data from computer-based information systems. The issue of the relatively small window through which an information space can be viewed brings with it two associated problems: presentation and navigation. This research is based on an approach called the Bifocal Display proposed by Spence and Apperley to address these inherent difficulties common in large information spaces in modern computing environments. The essence of this presentation technique is to provide the user with detailed local content as well as a global context to facilitate navigation. In this research, the original one-dimensional Bifocal Display concept has been extended in two-dimensional form to deal with two fundamental types of large information spaces: those with a high information density, for example, large databases and spreadsheets, and those with inherent spatial relationships, such as topographic maps and networks. An experimental study has been carried out to study the usability of the Bifocal Display and other presentation techniques based on various implementations of the London Underground map. Results have shown that the Bifocal Display is a usable and effective approach for the presentation of large information spaces. Presentation techniques can be broadly classified into distortion-oriented and non-distortion-oriented; the former generally requires more computational resources than the latter. With the increasing processing power of personal computers, researchers have developed a variety of novel distortion-oriented presentation techniques. Unfortunately, the distorting appearance resulting from the application of these techniques, coupled with the growing number of new terminologies used by researchers, has caused some confusion to the graphical user interface designer. A taxonomy of distortion-oriented techniques based on their magnification functions has been proposed to facilitate the identification of the similarities and differences of these techniques. A conceptual model has also been put forward to unveil the underlying principles which govern their operations. Despite the variety of novel presentation techniques currently available, the choice of a technique in a particular application remains very subjective; there is a general lack of selection guidelines or methodologies. An evaluation framework E3 has been developed to provide a basis for the comparison of different presentation techniques, given the nature and characteristics of the data to be presented, and the interpretation required. E3 focuses on three aspects of graphical data presentation: expressiveness, efficiency and effectiveness. This framework lays the foundation for the development of a set of metrics to facilitate an objective assessment of presentation techniques. ▼ One method being studied to slow myopia
progression is the use of distance center
bifocal soft contact
lenses. Animal studies show that the peripheral retina plays more
of a role in regulating eye growth than was previously thought.
They also show that it is myopic defocus that slows this eye
growth. By using distance center
bifocal soft contact lenses, the
central portion of the lens provides good focus of light to the
central retina. The peripheral part of the lens provides near
vision with light focused in front ofthe peripheral retina thus
creating myopic blur. This study is designed to
determine if children can tolerate
bifocal soft contact lenses in
future treatments for myopia control. Typically,
bifocal soft
contact lenses tend toslightly compromise acuity in adult wearers
because they correct two different ranges of vision. We want to
find out if children are bothered by this and to make sure the
pediatric subjects can tolerate constantly wearing a near add.
Therefore, we are comparing the change in quality of life when
children switch from spectacles to soft spherical contact lenses or
bifocal soft contact lenses. Historical data for age-and
gender-matched soft spherical contact lens wearers will beused for
comparison between the two modalities. We fitted
27 pediatric subjects in
bifocal soft contact lenses and
administered the Pediatric Refractive Error Profile (PREP) for
Glasses at their baseline visit and the PREP Survey for Contact
Lenses at the 1 month visit. Previously, 27 controlsubjects
completed the same surveys in the same time period while wearing
soft spherical contact lenses instead. The PREP
survey consists of 26 statements based on vision-related
functioning and well-being in children with some level of
refractive error. Scores are scaled from zero(poor quality of life)
to 100 (good quality of life), and an overall PREP score is the
mean score of all of the statements. Results from this survey
allowed us to compare how overall composite scores and scale scores
differed between spectacle and contact lens wearers as well as
between soft contact lens and
bifocal soft contact lens
wearers. Our data suggest that children were
tolerable of
bifocal soft contact lenses. The overall composite
score for the PREP showed an increase after one month of
bifocal
soft contact lenses wear (Student’s t test, p <0.001) and after
soft contact lens wear (Student’s t test, p = 0.004). However,
there was not a significant difference in the change from baseline
to one month between soft
bifocal and soft spherical contact lens
wearers (Student’s t test, p 0.59). Mean scores for each scale of
the PREP survey rose after one month of
bifocal soft contact lens
wear, with the greatest improvements in the scales of Satisfaction
and Activities (Student’s t test, p < 0.005 for
both). The quality of life changes with soft
contact lenses were similar to the changes following
bifocal soft
contact lens wear, which shows that
bifocal soft contact lenses are

▼ A two-year, double-masked, randomized clinical trial
of myopic children 6 to 11 years old is being conducted using
progressive addition lenses (PALs) to evaluate two theories of
juvenile-onset myopia progression. Eligible children had a high
accommodative lag and either: (1) low myopia (less myopic than
–2.25 D spherical equivalent) or (2) high myopia (more myopic that
–2.25 D spherical equivalent) with esophoria at near. The
accommodative lag theory hypothesizes that hyperopic retinal blur
drives myopia progression. The mechanical tension theory
hypothesizes that ciliary-choroidal tension created by the ocular
components restricts equatorial expansion and causes axial
elongation in people with factors that produce a large globe. To
test between these theories, children were randomly assigned to
wear either PALs with a +2.00-D add or single vision lenses (SVLs)
for one year to achieve a reduction in myopia progression in the
PAL group relative to the SVL group. All children then wear SVLs
for the second year to evaluate the permanence of the treatment
effect; a maintained treatment effect supports the lag theory,
while a rebound supports mechanical tension. The primary outcome is
central cycloplegic autorefraction. Complete ocular biometric data
are being collected at six-month intervals. Over 17 months, 192
children were screened, and 85 (44%) were eligible and enrolled.
The mean age (± SD) was 9.3 ± 1.4 years. The mean accommodative lag
was 1.71 ± 0.37 D, and 54 children (64%) were esophoric at near.
The mean cycloplegic spherical equivalent refractive error and
axial length were –1.95 ± 0.78 D and 24.17 ± 0.80 mm, respectively.
Baseline characteristics of the children enrolled are described.
Because the clinical trial is ongoing, all findings are
confidential. Aberrometry-based relative peripheral refraction
measurements were validated against measurements made with an
autorefractor. A method of analyzing peripheral aberration data
collected from a dilated pupil was validated. A single-value metric
of image quality was calculated to describe retinal image quality
centrally and in four peripheral locations. When including only
higher-order aberrations, image quality was best centrally and
decreased in the periphery. When relative peripheral refractive
error was included with higher-order aberrations, more significant
reductions in peripheral retinal image quality were present, and
the greatest reductions were in the temporal and superior retina
where astigmatism was highest. Baseline and six-month accommodative
lag data from children in the study were analyzed. Children still
had a significant lag of accommodation for a 4-D stimulus when
tested wearing a +2.00 D
bifocal add. No evidence of an effect of
bifocal adaptation on accommodative lag was found. Children with
greater myopia progression over the previous six months exhibited
higher accommodative lags when tested with their full manifest
correction. Myopia progression had no effect on accommodative lag

▼ Cette recherche est issue d’une observation longitudinale de quatorze enfants suivis en psychothérapie ; reçus en consultations familiales avec des interprètes ou médiateurs ; soutenus dans leur environnement social et scolaire ; et pour certains, accueillis en hôpital de jour. Il s’agit essentiellement d’enfants d’origine africaine, simultanément engagés par leur famille dans des traitements traditionnels. Le caractère bifocal – individuel et environnemental – de ces suivis, ouvre à un questionnement épistémologique dans ces deux champs respectifs – psychologique et social – et dans celui de leur articulation. Il engage, pour éviter l’écueil du relativisme ou des confusions sémantiques, à un approfondissement des notions de complémentarisme et d’acculturation antagoniste telles qu’elles ont été formalisées par Devereux, et prolongées, pour cette dernière, par certains sociologues comme Bourdieu et Sayad, ou du point de vue des paradoxes qu’elle soulève, par les travaux de Winnicott, de Roussillon, de Bateson, ou de certains cognitivistes. Ces observations conjointes et longues, sur trois à dix ans, soulignent les nombreuses interfaces entre les problématiques des enfants suivis et le champ familial et social auquel ouvrent les traitements culturels de leur malaise. En cela, la réflexion sur ces psychothérapies interculturelles soutient celle déjà engagée par d’autres cliniciens sur l’articulation de l’intrasubjectivité et de l’intersubjectivité, et sur les médiations thérapeutiques. This work is based on the longitudinal observation of 14 children treated with psychotherapy, seen in family consultations with interpreters or mediators, supported in their social and school environment and in some cases, admitted to day hospitals. Most children are of African origin, with families that simultaneously looked for traditional treatments. The bifocal character of these follow-ups – individual and environmental – allows for epistemological questioning in both of the respective fields – psychological and social – and in their articulation. It involves, in order to avoid the pitfalls of relativism or semantic confusions, a more profound analysis of the notions of complementarism and antagonistic acculturation as they have been formalized by Devereux, and developed in the case of acculturation, by certain sociologists such as Bourdieu and Sayad, or from the perspective of paradoxes that result, of the work of Winnicott, Roussillon, Bateson and certain cognitivists. These conjoined and long clinical follow-ups lasting from three to ten years underline the numerous interfaces between the problematics of the treated children and the family and social fields implicit in the cultural treatment of their illness.
