 1.目的現在, 本邦の高齢化率は27.3%であり上昇の一途をたどっている. 誤嚥性肺炎は高齢者における死因の1つであり, 日本人の死因第3位である肺炎の約9割は65歳以上の高齢者である. 高齢者の誤嚥性肺炎による入院費用は年間4450億円と推計されており, 誤嚥性肺炎の予防および早期治療は急務の課題である.  誤嚥性肺炎の原因の1つとして, 摂食嚥下障害が挙げられる. 高齢者では摂食嚥下機能の老化の一つである「老嚥」が起こるため, 若年者に比し, 摂食嚥下障害発症のリスクが高くなり,同時に誤嚥性肺炎発症のリスクも高くなる. また,高齢者では加齢性変化やサルコペニアが全身性に起き, 呼吸機能が低下する. 呼吸機能の低下は, 咳嗽, バルサルバ,嚥下などの非換気機能低下をもたらし, 咳嗽力も低下する.これにより, 誤嚥した物質を喀出することが困難となるため,誤嚥性肺炎は発症のリスクが高くなり,生命予後にも影響を与える可能性がある. 誤嚥性肺炎の評価としての呼吸機能検査は, 多くの場合, 指示理解を必要とする検査である. しかし, 高齢な誤嚥性肺炎患者の場合, 意識障害や認知症を抱える場合が多く, これらの評価を実施できないことが少なくない. そこで,本研究では,コンピューター断層撮影(Computed… (more)
