Ce travail porte sur deux thèmes principaux. Le premier est relatif à la drépanocytose et comprend deux études. La première a examiné les connaissances qu’ont 178 hommes et femmes vivant au Bénin sur la maladie, ses causes, ses effets et sa prise en charge. Ils ont répondu à un questionnaire et, leurs réponses ont été comparées à celles d’un groupe d’expert. Il en est ressorti qu’il existait une méconnaissance globale de la maladie, et plus particulièrement de ses symptômes les plus graves et de son traitement. Les résultats révèlent que plus on a un niveau d’éducation élevé, moins on se fait de fausses idées concernant la maladie. En outre, cette méconnaissance était plus accrue chez les personnes se déclarant porteuses du trait génétique (4.83) ou drépanocytaire (5.19) que chez les personnes ayant déclaré ne pas être porteuses (3.72) ou saines (3.62). La deuxième étude a quant à elle cherché à savoir si les 137 personnes interrogées vivant au Bénin, avaient intégré de façon adéquate les règles de transmission génétique de la maladie, en se basant sur la théorie fonctionnelle de la cognition. L’analyse des données a permis de distinguer trois clusters distincts. Dans le premier groupe (n=46), les personnes jugeaient ainsi le risque de transmission de la maladie plus élevé en présence de l’un ou l’autre des deux parents atteints. Dans le deuxième groupe (n=51), le risque était jugé d’autant plus grand que chacun des parents était lui-même gravement atteint. Dans le troisième groupe (n=40), le risque n’avait été jugé élevé qu’en présence des deux parents atteints. Le deuxième thème a donné lieu à une étude, visant à examiner les motifs pouvant sous-tendre un refus d’amputation chez 224 personnes vivant au Bénin, par l’approche de la théorie du renversement. Les résultats montrent que la peur de la perte de l’identité personnelle, la peur des hôpitaux et du personnel médical, la conviction que la chirurgie était inutile, le manque de confiance dans la compétence et le dévouement des médecins, la peur de la perte de l’estime de l’autre et celle de la perte de la force spirituelle, font partie des facteurs principaux pouvant motiver un refus d’amputation.

This thesis focuses on two main themes. The first one is related to sickle cell disease and the second theme is related to limb amputations. For studying the first theme, this research proposes to examine the knowledge of 178 men and women living in Benin, on the disease, its causes, effects and care. They answered a questionnaire and their responses had been compared to those given by an expert group. It had been found that there was an overall lack of awareness on the disease, and especially its most severe symptoms and treatment. The results reveal that the higher level of education they have, the less misconceptions about the disease they have. ▼ More than 30 million people living in the US have diabetes. Over time, increased concentrations of glucose in the blood lead to cellular damage and cause a variety of serious complications in these individuals. Sodium-Glucose Co-Transporter 2 Inhibitors (SGLT2i) are the latest class of oral antidiabetic medications approved for use in the US. They work by blocking the resorption of glucose in the kidney thereby increasing urinary glucose excretion. Due to their distinct mechanism of action, these drugs demonstrate cardio- and reno-protective effects in addition to their antihyperglycemic effect. However, clinical trials report an increased risk for adverse events such as amputations, fractures, and mycotic infections raising concerns about their safety. The objective of this dissertation was to evaluate the comparative safety of these drugs in a real-world setting outside of clinical trials. Three observational studies were conducted using prescription and medical insurance data from Truven Health MarketScan Databases (2009-2015). The first study investigated the association between SGLT2i and amputations and concluded that the potential for harm cannot be ruled out. While there was a 38% increase in risk for amputations following initiation of treatment with SGLT2i, the 95% confidence interval included the null. The second study investigated the association between SGLT2i and fractures. There was an 82% increase in risk for fractures early in treatment with SGLT2i. Beyond the initial 2-week treatment period, there was no apparent effect on fracture risk. The third study investigated the association between SGLT2i and mycotic infections. There was a 53% increase in risk for mycotic infections within the first 90 days of treatment with SGLT2i. This dissertation evaluated the post-marketing safety of this new class of antidiabetic medications in a real-world setting. Evidence from such observational studies can supplement that from clinical trials to more thoroughly inform decision-making. Findings from this research can help clinicians understand patients at high-risk for experiencing these events and devise remedial measures in anticipation. O presente estudo tem por objetivo delinear um perfil da qualidade de vida em protetizados de membros inferiores, por meio de questionário validado, atendidos no ambulatório do Hospital da Vila São José Bento Cottolengo em um período retrospectivo de 02 (dois) anos. A pontuação dos escores da aplicação do questionário foi realizada com o programa estatístico SPSS (Statistical Package os the Social Sciences). Trata-se de estudo com características do tipo coorte retrospectivo qualitativo. Uma vez contatados, foram submetidos a um questionário específico para pesquisa de qualidade de vida, o WHOQOL bref. Em um contexto geral, os paciente pesquisados apresentam uma Qualidade de Vida de mediana a boa, pois se consideram amparados por situação civil estável, relações interpessoais seguras, possuem renda, advinda de aposentadoria ou trabalho, apreciam seu domicilio e locomovem-se bem. Como pontos negativos, temos a não aceitação de seu aspecto físico, pouca satisfação com seu desempenho profissional, com serviço de saúde público, transportes e presença freqüente de sentimentos negativos. Com o estudo destes pacientes, em todas as suas variáveis, temos subsídios para implementar programas educativos em saúde pública e privada, utilizando centros de difusão de idéias como Universidades, Escolas, Hospitais, Postos de Saúde, Creches, Instituições Asilares objetivando a prevenção de agravos importantes, que poderão ultimar em seqüelas irreversíveis como as amputações.

The studying present main objective is to evaluate the quality of life of lower limb amputees, through specified interviews (with questionnaires) among patients under treatment at Vila São José Bento Cottolengo hospital, in Trindade/Go (Brazil), in a retrospective time of 02 (two) years. The study type is the qualitative and retrospective cohort. Once contacted, the patients were submitted to a questionnaire to survey the quality of life, in a general context, the people surveyed have a Quality of Life from median to good. They consider themselves backed by stable civil situation, secure interpersonal relationships, income from retirement or job, they enjoy their home and move around without difficulties. As negative aspects, we may mention the frustration and rejection of their physical appearance, poor satisfaction with their professional performance, with the public health service and transport, as well as frequent negative feelings. By studying these patients, in all its variations, we achier way to implement education programs on public and private health, using ideological institutions - such as universities, schools, hospitals, health centers, kindergartens, nursing homes, etc. The focus of this action is the prevention of most hazardous injuries, which may ultimately lead to irreversible damage, as amputations.

▼ Occupational health surveillance projects are essential in providing information about the trends and health outcomes at both the state and national levels. In this study, data linkages on multiple datasets by zip code were performed to identify geographical clusters of work-related amputations, asbestosis, and coal workers’ pneumoconiosis (CWP) that target communities for possible interventions with high counts and rates for these health outcomes in Illinois between the years of 2000 and 2010. The Illinois hospital discharge dataset was used to conduct this study. A combined inpatient and outpatient data in 2010 determined 1,665 cases of occupational amputations, while a total of 3,172 asbestosis cases and 1,598 CWP cases were identified solely by inpatient data between the years of 2000 and 2010. Our evaluation of health predictors included employment data, demographic data, and population-size measures. High-risk communities were determined by looking at the upper quintile of rates and counts of cases. For all three health outcomes, a 20/80 split, was determined to indicate that the top 20% of zip codes with the most cases represented 80% of all the cases (amputations 75.9%, asbestosis 77.3%, and CWP 85.7%) in the same category. However, elevated rates did not show the same results due to the population divide between major metropolitan areas and the smaller rural towns. Spatial analysis identified geographical clusters in Illinois with elevated rates and high counts that can result in further targeted interventions. Our findings showed that high counts of amputations were located in the Chicago metropolitan area. Our spatial analysis identified clusters of middle-sized communities with elevated rates. Asbestosis cases and spatial clusters were predominantly located in middle and smaller communities. High counts and rates of CWP were found near residential areas surrounding rural coal mining towns. Our research has found that identifying spatial clusters for these health outcomes provides the necessary steps to further our purpose of community advocacy to provide training, education, and research knowledge about the hazards of nationally recognized occupational health indicators and accessibility and cost of workers’ health resources. ▼ The aim of the present study was: a) to record the clinical course and the outcome of patients with Diabetic Foot (DF) during a follow-up period of six years; b) to identify risk factors for ipsilateral re-amputation. In the first part of our study, 256 patients (171 men, 85 women; mean age 65.31±10.25 years) with diabetic foot were included. These were divided into 3 groups according to the aetiology of the lesion: Group I: 87 patients with neuropathic ulcers Group II: 34 patients with purely ischaemic ulcers and Group III: 120 patients with neuroischaemic ulcers In all 3 groups, lesion location was recorded, while lesion severity was graded according to the Meggit-Wagner system. Neuropathy was diagnosed by the Neuropathy Disability Score (NDS), and presence of painful neuropathy was assessed by means of Neuropathy Symptom Score (NSS). Moreover, patients were examined for the presence of peripheral arterial disease. The latter was diagnosed by measuring Ankle-Brachial Index with a Doppler device, as well by typical symptoms (intermittent claudication), palpation of peripheral pulses and, where possible, DSA. We also recorded past history of ulceration or amputation, sex, age, DM type, metabolic control (HbA1c), DM duration, antidiabetic treatment, smoking habits, BMI, classical micro- and macrovascular complications (nephropathy, retinopathy, coronary disease, stroke) and other risk factors, such as hypertension and dislipidaemia. We sought to identify the causal pathway of ulceration, the duration of lesions, the frequency and duration of hospitalisation, the outcome of healing or not, as well as the likelihood of subsequent amputation or vascular intervention. We also addressed the management of diabetic foot, the recurrence of lesion or appearance of new ulceration and, finally, patient mortality. The second part of the study focused on the appearance or otherwise of ipsilateral re-amputation during a 6-year follow-up among patients who had previously sustained a minor or major amputation. Included were 121 patients with prior amputation, who were divided into 2 groups: group A (95 patients without re-amputation) and group B (26 patients with re-amputation). We aimed to identify risk factors leading to ipsilateral re-amputation. Examined potential risk factors included HbA1c, smoking, nephropathy, age, sex, BMI, severity of neuropathy, severity of ischaemia, aetiology and severity of lesion, anatomic location of lesion, patient compliance, vascular intervention, and angiographic findings). .......................................................................

81 CASES WITH OCCLUSIVE DISEASES OF THE LOWER EXTREMITIES WERE STUDIED. THE ULTRASOUND FLOW DETECTION TECHNIQUE AND THE PRESSURE INDEX OF THE ANKLE WERE USED IN ORDER TO DETERMINE THE LEVEL OF AMPUTATION WHERE THE LESIONS WERE IRREVERSIBLE. WHERE AMPUTATION WAS UNAVOIDABLE THE OBJECTIVE WAS TO PERFORM, IF POSSIBLE BELOW-KNEE AMPUTATION. AMONG THE 81 CASES THERE HAVE BEEN 31 CASES OF BELOW KNEE AMPUTATION I.E. 38% AND 50 CASES OF ABOVE KNEE AMPUTATION I.E. 61%. WITH THE USE OF THAT METHOD NONE OF THE PATIENTS HAD TO UNDERGO OPERATION AGAIN.

▼ Dynamic stability is commonly defined as the ability to maintain balance through center of mass control during locomotion. Patients with locomotor impairments are especially challenged when walking over uneven surfaces (Richardson 2004). We studied dynamic stability margins in young healthy adults and in adults with unilateral traumatic trans-tibial amputations (TTA). To date, studies have not controlled for walking speed over an uneven surface in a patient population. We hypothesized that: 1) DSMs would increase over the uneven rocky surface (URS) for both groups, 2) DSMs would be greater on the involved side at faster walking speeds for subjects with TTA and, 3) DSMs would increase more for the involved limb when on the URS. 17 (4 females, 13 males) young healthy military service members (22.8 ± 6.4 years) and 12 (1 female, 11 males) service members (27.2 ± 4.7 years) with traumatic unilateral trans-tibial amputations participated in two study designs. A 15-segment model was used to estimate whole body COM motions. All subjects walked at 5 dimensionless speeds over a flat level surface (FLS) and an URS. Subjects completed 6-10 trials over each surface at each speed. Minimum frontal plane DSM values were extracted for each stride for statistical analyses. For young healthy subjects a two factor (speed x surface) ANOVA was used to test significance (p<.05). The DSMs were not greater over the URS (p=.307), but a main effect due to speed was found (p<.001) for young healthy subjects. In contrast, DSMs were significantly larger when subjects with TTA walked on the URS compared to the FLS (p = 0.011). For subjects with unilateral TTA, a three-factor ANCOVA ((amputation) side x speed x surface) with residual limb length (p=.029) and time in prosthesis (p=.741) as covariates was used for hypothesis testing. When limb length and time in prosthesis were accounted for there was no significant within subjects effect due to speed (p=.656). There were no significant differences between involved and uninvolved limbs (p = 0.211). There were no significant interaction effects. In conclusion, there was a difference in DSMs due to speed in unimpaired subjects and due to surface and residual limb length in subjects with unilateral TTAs. In subjects with unilateral TTA side-to-side symmetry was found for DSM measures, which was in contrast to an earlier study of subjects with unilateral trans-femoral amputations (Hof 2006). ▼ The safety of the American workplace has improved dramatically over the past 30 years. This improvement is directly correlated with the adoption and enforcement of OSHA regulations (OSHA, “OSHA Facts”). However, despite the great strides that have been achieved, some industry sectors continue to produce unnecessarily high numbers of serious and preventable injuries. Machine-related injuries are responsible for nearly half of the thousands of amputation injuries that occur each year. Most machine injuries are preventable through known methods that are well documented. For most machines, OSHA provides guarding and operational requirements that are very general and broadly applicable. However, in the case of mechanical power presses the codes are quite specific and intended to address the specific hazards associated with such presses. This study proposes that the OSHA codes related to mechanical power presses are adequate and address most of the guarding concerns, but employers often fail to comply with the codes, apparently out of a lack of understanding of their implementation. It is hypothesized that an effective tool to help guide personnel through the evaluation of press safety hazards will improve the likelihood of an individual in accurately identifying press hazards. Based on the perceived need, a software tool was developed to assist in the hazard identification process. This tool was tested experimentally to determine its effectiveness. The hazard evaluation performance of a software-assisted group of novices was compared with the performances of a peer group and a group of press professionals, both comparison groups using traditional evaluation methods (specifically ANSI B11.TR3). Each of the experimental groups evaluated three different mechnical power presses. The hazards identified by each experimental group were to address the specific requirements of the applicable OSHA codes for guarding of mechanical power presses (29CFR1910.212 and 29CFR1910.217). The accuracy and completeness of the experimental data were evaluated against those specific codes. The experimental results strongly indicate that the use of the SafetyNET software can allow an individual with no specialized background with mechanical power presses to perform more effective hazard assessments on mechanical power presses than might be achieved with traditional assessment techniques. The SafetyNET users performed assessments that were demonstrably more complete and accurate than those performed by either comparison group using traditional methods. ▼ This study is the first attempt in Greece to identify the prevalence and the incidence rates of amputations in diabetic patients performed in a certain area (Hmathia prefecture), during a decade (1990-99). Amputation in diabetic patients is a macrovascular complication of diabetes, although, in addition to peripheral vascular disease, diabetic neuropathy, a microvascular complication, is involved in the aetiopathogenesis of diabetic ulcer. Insulin, hyperglycaemia, impaired hemostatis and lipids are correlated with the aetiopathogenesis of macrovascular complications of diabetes. Individuals with diabetes are predisposed to a diabetic ulcer primarily because of the presence of peripheral neuropathy and peripheral vascular disease, but other factors contribute too. Programs aiming to reduce amputations among people with diabetes should provide patient education and foot-care programs. Summarizing our findings, the 84 diabetic amputees in Hmathia region during 1990-99, have undergone 101 amputations totally, (approximately 70% of all amputations). The prevalence of diabetic amputees in prefecture of Hmathia in 1999 was found 12,5/1000 diabetic patients (1,25%) and 2,48/10000 population. The mean incidence of diabetic amputations during 1990-99 was found 3,7/1000 diabetic patients/year and 0,59/10000 total population/year. Most of the amputees were men and they had a high mortality rate (56%), almost 3-fold higher comparing with the rest diabetic population. High amputations of tibia and thigh were found in higher percentage than the usual reported in international reports. The survivors were, in their majority, people of low educational state, mainly farmers, smokers at the time of examination or in the past. The mean duration of diabetes was 15 years, their glycaemic control was not good, while their lipid profile was not bad and 1/3 of them reported a history of angina or heart infraction. Micro- or macroalbuminuria was found in most of the examined patients and impaired renal functure in half of them. In spite of this, only 18% of patients reported albuminuria, and this fact possibly reflects the absence of frequent medical attendance. A small percentage of them reported Laser therapy, obviously because they rarely visited an occulist. Symptoms of peripheral vascular disease from the other limb, were present in 56% of the patients, and in most of them an impaired ankle-brachial index ( ABI) was observed. Peripheral sensory neuropathy was detected in 41% of the patients. Despite the fact that prevalence and incidence of diabetic amputations in the certain population is not higher than the prevalence and incidence rates in intenational reports, the younger age of first amputation, the higher percentage of mortality, the smoking habits, the high percentage of above knee amputations and the high percentage of microvascular and macrovascular complications in the population of amputees, who were at low educational and social level, should alarm government and primary health services to pay attention on the matter of…

Background: Severe preamputation pain is associated with phantom limb pain (PLP) development in limb amputees. We investigated whether optimized perioperative analgesia reduces PLP at 6-month follow-up. Methods: A total of 65 patients underwent lower-limb amputation and were assigned to five analgesic regimens: (1) Epi/Epi/Epi patients received perioperative epidural analgesia and epidural anesthesia; (2) PCA/Epi/Epi patients received preoperative intravenous patient-controlled analgesia (PCA), postoperative epidural analgesia, and epidural anesthesia; (3) PCA/Epi/PCA patients received perioperative intravenous PCA and epidural anesthesia; (4) PCA/GA/PCA patients received perioperative intravenous PCA and general anesthesia (GA); (5) controls received conventional analgesia and GA. Epidural analgesia or intravenous PCA started 48 h preoperatively and continued 48 h postoperatively. The results of the visual analog scale and the McGill Pain Questionnaire were recorded perioperatively and at 1 and 6 months. Results: At 6 months, median (minimum–maximum) PLP and P values (intervention groups vs. control group) for the visual analog scale were as follows: 0 (0–20) for Epi/Epi/Epi (P _ 0.001), 0 (0–42) for PCA/Epi/Epi (P = 0.014), 20 (0–40) for PCA/Epi/PCA (P = 0.532), 0 (0–30) for PCA/GA/ PCA (P = 0.008), and 20 (0–58) for controls. The values for the McGill Pain Questionnaire were as follows: 0 (0–7) for Epi/Epi/Epi (P = 0.001), 0 (0–9) for PCA/Epi/Epi (P = 0.003), 6 (0–11) for PCA/Epi/PCA (P = 0.208), 0 (0–9) for PCA/GA/PCA (P = 0.003), and 7 (0–15) for controls. At 6 months, PLP was present in 1 of 13 Epi/Epi/Epi, 4 of 13 PCA/Epi/Epi, and 3 of 13 PCA/GA/PCA patients versus 9 of 12 control patients (P=0.001, P=0.027, and P=0.009, respectively). Residual limb pain at 6 months was insignificant. Conclusions: Optimized epidural analgesia or intravenous PCA, starting 48 h preoperatively and continuing for 48 h postoperatively, decreases PLP at 6 months. ▼ Cette recherche se situe à la jonction des études féministes, trans et sur la production du handicap. Elle s’intéresse aux limites des analyses féministes intersectionnelles et leurs difficultés à considérer certains axes identitaires, telles l’identité de genre et les capacités, à travers l’analyse de deux cas de figure de modifications corporelles, soit la transsexualité et la transcapacité (transformation visant à acquérir un handicap). Cette thèse s’articule autour de la question suivante : quels sont les angles morts des analyses intersectionnelles et comment pouvons-nous y remédier? Pour répondre à cette question, cette thèse propose trois objectifs : 1) Participer aux questionnements critiques entourant certaines limites des analyses féministes intersectionnelles pour en diversifier les axes à partir de l’identité de genre et des capacités; 2) Remettre en question les présupposés conceptuels et les privilèges cisgenristes et (cis)capacitistes qui sous-tendent les discours négatifs entourant ces modifications corporelles et mettre en valeur les points de convergence entre les mouvements féministes, transactivistes et anticapacitistes pour instaurer des dialogues et des solidarités entre eux; 3) Fournir un modèle enrichi de l’intersectionnalité pouvant être déployé en études féministes, trans et sur la production du (trans)handicap, apte à répondre aux nouvelles revendications de justice de groupes opprimés, comme celles des personnes transsexuelles et transcapacitaires. À l’aide d’une méthodologie alliant l’analyse critique de discours, l’analyse archéologique et généalogique, les discours négatifs de certaines féministes et d’autres scientifiques sur ces transformations corporelles sont cartographiés et critiqués. Les réflexions présentées ici, s’inscrivant dans une approche poststructuraliste et queer, ne proposent pas une recherche des « origines » et des motivations de la transsexualité et de la transcapacité, mais plutôt une étude des discours négatifs vis-à-vis ces phénomènes et des systèmes normatifs qui sous-tendent ces discours. C’est sous cet angle que cette recherche constitue un apport important aux champs des savoirs, tant sur le plan théorique, normatif que politique : en se penchant sur deux cas de figure « extrêmes », elle met en lumière les normativités corporelles des régimes dominants, notamment cisgenriste et (cis)capacitiste, occultées tant dans nos sociétés que dans nos mouvements sociaux.

AT THE ORTHOPAEDIC DEPARTMENT OF THE UNIVERSITY OF IOANNINA, WE TREATED OPERATIVELY 1174 PATIENTS WITH HAND INJURIES FRON JUNE 1992 TO MARCH 1997. NINE HUNDRED TWENTY-FOUR OF THEM WERE MALE (78,7%) AND 250 FEMALE (21,3%). ISOLATED INJURIES OCCURED IN 50,4% AND COMBINED IN 49,6%. THE PATIENTS' AGE FLUCTUATED BETWEEN 8 MONTHS AND 88 YEARS WITH AVERAGE 34,8 YEARS. THE COMBINED INJURIES WERE CLASSIFIED ACCORDING TO THE LOCATION (DIGITS, METACARPAT, WRIST AND MIXED). PALMAR AND DOCSAL INJURIES WERE SUBDIVIDED ACCORDING TO THE ZONES OF FLEXOR AND EXTENSOR TENDONS, RESPECTIVELY. FOLLOWING POSTOPERATIVE REHABILITATION PROGRAMME, DORSAL INJURIES WERE SEPERATED INTO TXO MAIN GROUPS. EACH ONE WAS SUBDIVIDED INTO TWO SUBGROUPS ACCORDING TO DIFFERENT METHOD OF MOBILIZATION.GROUP A FOLLOWED A CONVENTIONAL METHOD AND GROUP B A METHOD OF PRIMARY MOBILI- ZATION. THE SUBGROUP A1 INCLUDED 76 PATIENTS WITH 91 INJURED FINGERS HAD ISOLATED INJURY AND THE SUBGROUP A2 INCLUDED 82 PATIENTS WITH 98 INJURED FINGERS, HAD COMBINED INJURY. THE SUBGROUP B1 CONSISTS OF 23 PATIENTS WITH ISOLATED INJURY AND SUBGROUP B2, 23 PATIENTS WITH COMBINED INJURY RESPECTIVELY. THEUSE OF PRIMARY MOBILIZATION PROGRAMME GAVE BETTER RESULTS COMPARED WITH CONVENTIONAL METHOD IN ALL SUBGROUPS. IN A SECOND PROSPECTIVE STUDY, 38 PATIENTSWITH 45 FLEXOR TENDON LACERATIONS IN ZONE II UNDERWENT PRIMARY MOBILIZATION USING KLEINERT BANDS. ▼ At the Ottawa Hospital in Ontario, Canada, clinicians use full body immersion virtual reality to treat a variety of health conditions, including: traumatic brain injuries, post- traumatic stress disorder, acquired brain injuries, complex regional pain syndrome, spinal cord injuries, Guillain-Barré syndrome, and lower limb amputations. The system is shared between military and civilian patient populations. Viewed by clinicians and the system’s designers as a value neutral medical technology, clinical virtual reality’s sights, sounds, movements, and smells reveal cultural assumptions about universal patient experiences. In this dissertation I draw from reflexive feminist research methodologies, visual anthropology and sensory ethnography in a hospital to centre the body in current debates about digital accessibility in the 21st Century. 40 in-depth interviews with practitioners and patients, 210 clinical observations, and film and photography ground research participant experiences in day-to-day understandings of virtual reality at the hospital. In this dissertation I address an ongoing absence of the body as a site of analytical attention in anthropological studies of virtual reality. While much literature in the social sciences situates virtual reality as a ‘post-human’ technology, I argue that virtual reality treatments are always experienced, resisted and interpreted through diverse body schemata. Furthermore, virtual reality cannot be decoupled from the sensitivities, socialities and politics of particular bodies in particular places and times. The Ottawa Hospital’s Computer Assisted Rehabilitation Environment (CAREN) system features a digitally enhanced walk-in chamber, treadmills on hydraulic pistons, surround sound audio, advanced graphics and user feedback utilizing force plates and a dynamic infrared motion capture system. The CAREN system utilizes hardware and software reliant on specific assumptions about human bodies. For example, these assumptions are echoed in depictions of race, gender, class, and indigeneity. Patients using virtual reality technologies can experience more than one disability or health condition at a time, further disrupting the idea of universal user experiences. As clinicians and patients confront the limitations of body normativity in the CAREN system’s interface design, they improvise, resist, and experience virtual reality in ways that defy design agendas, ultimately shaping patient treatments and unique paths to healing and health.
