▼ With the large number of spatial queries for spatial data objects changing with time in many applications, e.g., the location based services and geographic information systems, spatio-temporal databases have been developed to manipulate them in spatial or temporal databases. We focus on queries for stationary and moving objects in the spatial database in the present. However, there is no total ordering for the large volume and complicated objects which may change their geometries with time. A spatial
access method based on the spatial index structure attempts to preserve the spatial proximity as much as possible. Then, the number of disk
access which takes the response time is reduced during the query processing. Therefore, in this dissertation, based on the NA-tree, first, we propose the NA-tree join
method over the stationary objects. Our NA-tree join simply uses the correlation table to directly obtain candidate leaf nodes based on two NA-trees which have non-empty overlaps. Moreover, our NA-tree join accesses objects once from those candidate leaf nodes and returns pairs of objects which have non-empty overlaps. Second, we propose the NABP
method for the continuous range queries over the moving objects. Our NABP
method uses the bit-patterns of regions in the NA-tree to check the relation between the range queries and moving objects. Our NABP
method searches only one path in the NA-tree for the range query, instead of more than one path in the R*-tree-based
method which has the overlapping problem. When the number of range queries increases with time, our NABP
method incrementally updates the affected range queries by bit-patterns checking, instead of rebuilding the index like the cell-based
method. From the experimental results, we have shown that our NABP
method needs less time than the cell-based
method for range queries update and less time than the R*-tree-based
method for moving objects update. Based on the Hilbert curve with the good clustering property, we propose the ANHC
method to answer the all-nearest-neighbors query by our ONHC
method. Our ONHC
method is used to answer the one-nearest-neighbor query over the stationary objects. We generate direction sequences to store the orientations of the query block in the Hilbert curve of different orders. By using quaternary numbers and direction sequences of the query block, we obtain the relative locations of the neighboring blocks and compute their quaternary numbers. Then, we directly
access the neighboring blocks by their sequence numbers which is the transformation of the quaternary numbers from base four to ten. The nearest neighbor can be obtained by distance comparisons in these blocks. From the experimental results, we have shown that our ONHC and ANHC methods need less time than CCSF
method for the one-nearest-neighbor query and the
method based on R*-trees for the all-nearest-neighbors query, respectively.

У дисертaцији се решава проблем моделирања накнада
за коришћење железничке инфраструктуре. Проблем се решава двофазним
приступом и представља нови приступ решавању проблема дефинисања
накнада, инструмента за регулисање железничког тржишта. Према
двофазном приступу, проблем моделирања накнада се посматра као
проблем избора принципа накнада и проблем дефинисања структуре
накнада. Моделирањем накнада дефинисаним оваквим приступом,
планерима и менаџерима је омогућено да сагледају зависности између
принципа накнада, елемената накнадa, њихов утицај на укупно покриће
трошкова и будуће планирање пословања управљача инфраструктуре.
Основни циљеви дисертације су дефинисање модела избора принципа
накнада и математичка формулација структуре накнада за коришћење
железничке инфраструктуре за Европске железничке мреже средње и
мале величине. Први циљ се остварује кроз: дефинисање и
систематизацију кључних елемената који утичу на избор принципа
накнада, дефинисање релацијa измећу њих и развој модела избора.
Проблем је разматран као вишекритеријумски проблем где релације
између критеријума нису независне, односно постоји међузависност и
повратна спрега. На основу прегледа литературе која се односи
директно или индиректно на примену принципа накнада, модела за
прорачун и структуру накнада, анализе зависности појединих
перформанси инфраструктуре и тржишта у односу на обим саобраћаја и
коришћење капацитета инфраструктуре систематизовано Моделирање
накнада за коришћење железничке инфраструктуре Мирјана Бугариновић
v је двадесет два критеријума. Применом Делфи процедуре од три
круга генерисано је седам критеријума. То су: комплексност мреже,
продуктивност управљача инфраструктуре и оператора, степен
искоришћења капацитета мреже, доминантна категорија транспорта,
стратегија државе у погледу давања субвенција железници,
инвестициона политика за инфраструктуру и хармонизација принципа
накнада са изабраним у региону. У дисертацији је развијен модел
избора принципа накнада применом Аналитичког мрежног процеса (АNP)
и омогућено је да појединачне, изоловане анализе различитих
принципа накнада, стања и рада на мрежи, окружења, интереса државе,
буду систематизоване у једну "мрежу" која је концептуално разумљива
и која највише одговара постављеним циљевима ефикасног коришћења
инфраструктуре и стратегије развоја тржишта. Посебна пажња у
дисертацији је посвећена проблему утицаја и интереса различитих
заинтересованих страна на избор принципа накнада. Развијен је и
проширени АNP модел избора принципа накнада увођењем кластера који
представља заинтересоване стране. Овим моделом су утицаји и

▼ Due to the rapid development of communication networks, people gradually increasingly rely on mobile communication devices in their daily life, resulting in increasing demand for network bandwidth and transmission rate.To satisfy serve requirement, mobile communications have developed toward the fourth generation systems. In the near future, the Fifth-generation mobile communications network which is expected to boost thousands times of capacity will be deployed in 2020. To deal with explosion amount of data transmission, 5G systems aim to enhance spectrum efficiency, network capacity, transmission rate, and reduce latency in the future. Non-orthogonal multiple
access (NOMA) technique is one of key candidates currently. Multi-accepting technologies mostly allocate orthogonal radio resourced multiple users in time-domain, frequency domain, or code domain. By contrast, NOMA technology allows users
access and share the same radio resource through multiple-
access in power domain in order to effectively improve spectrum efficiency. In this paper, we consider a multi-input multi-output (MIMO) down-link system where using non-orthogonal multiple
access technology is adopted.The base station uses superposition coding to superimpose signals regarding multiple users. We propose to optimize the transmission of a MIMO base station toward multiple users equipped with antenna based on perfect channel state information. The user can use successive interference cancellation to enhance the signal and interference plus noise ratio. We will design the beamforming vector of the base station as well as the optimal power allocation of downlink users, in order to maximize sum rate of the downlink system. In this paper, we consider two beamforming strategies, one is to match the beamforming vector to the stronger channel (MSC), and the other is to find the optimal beamforming vector in terms of maximum sum rate. Our results show that the proposed NOMA scheme achieves higher sum rate than conventional orthogonal multiple
access system. With optimal beamforming vector, the propose scheme can support higher rate even when the geometrical angle between two users is large.

▼ Observed protein structures usually represent energetically favorable conformations. While not all observed structures are necessarily functional, it is generally agreed that protein structure is closely related to protein function. Given a collection of proteins sharing a common global structure, variations in their local structures at specific, critical locations may result in different biological functions. Structural relationships among proteins are important in the study of the evolution of proteins as well as in drug design and development.
Analysis of geometrical 3D protein structure has been shown to be effective with respect to classifying proteins. Prior work has shown that the Double Centroid Reduced Representation (DCRR) model is a useful geometric representation for protein structure with respect to visual models, reducing the quantity of modeled information for each amino acid, yet retaining the most important geometrical and chemical features of each: the centroids of the backbone and of the side-chain. DCRR has not yet been applied in the calculation of geometric structural similarity.
Meanwhile, multi-dimensional indexing (MDI) of protein structure combines protein structural analysis with distance metrics to facilitate structural similarity queries and is also used for clustering protein structures into related groups. In this respect, the combination of geometric models with MDI has been shown to be effective. Prior work, notably Distance and Density-based Protein Indexing (DDPIn), applies MDI to protein models based on the geometry of the C-alpha backbone. DDPIn's distance metrics are based on radial and density functions that incorporate spherical-based metrics, and the indices are built from metric-tree (M-tree) structures.
This work combines DCRR with DDPIn for the development of new DCRR centroid-based metrics: spherical binning distance and inter-centroid spherical distance. The use of DCRR models will provide additional significant structural information via the inclusion of side-chain centroids. Additionally, the newly developed distance metric functions combined with DCRR and M-tree indexing attempt to improve upon the performance of prior work (DDPIn), given the same data set, with respect to both individual k-nearest neighbor (kNN) search queries as well as clustering all proteins in the index.

▼ In recent years, the optimization, statistics and machine learning communities have built momentum in bridging methodologies across domains by developing solutions to challenging optimization problems arising in advanced statistical modeling. While the field of optimization has contributed with general methodology and scalable algorithms to modern statistical modeling, fundamental statistics can also bring established statistical concepts to bear into optimization. In the operations research literature, sensitivity analysis is often used to study the sensitivity of the optimal decision to perturbations in the input parameters. Providing insights about how uncertain a given optimal decision might be is a concept at the core of statistical inference. Such inferences are essential in decision making because in some cases they may suggest that more data need to be acquired to provide stronger evidence for a decision; in others, they may prompt not making a decision at all because of the high uncertainty of the decision environment. Statistical inference can provide additional insights in decision making by quantifying how uncertainty in input data propagates into decision making. In this dissertation, we propose a methodological and computational framework for statistical inference on the decision solutions derived using optimization models, particularly, high-dimensional linear programming (LP). In Chapter 2, we explore the theoretical geometric properties of critical regions, an important concept from classical sensitivity analysis and parametric linear programming, and suggest a statistical tolerance approach to sensitivity analysis which considers simultaneous variation in the objective function and constraint parameters. Using the geometric properties of critical regions, in Chapter 3, we develop an algorithm that solves LPs in batches for sampled values right-hand-side parameters (i.e. b of Ax = b in the constraints). Moreover, we suggest a data-driven version of our algorithm that uses the distribution of the bs and empirically compare our approach to other methods on various problem instances. Finally, in Chapter 4, we suggest a unified framework for statistical inference on the decision solutions and propose the remaining work, including the implementation of the framework to making statistical inferences on spatial disparities in
access to dental care services.

▼ This work addresses the problem of analyzing the best possible systems for communicating over a multiple-
access channel (MAC) without feedback in the discrete memoryless setting. We consider the
error exponents for this channel model and obtain upper and lower bounds on the channel reliability function. In particular, a unified framework to obtain all known upper and lower bounds on the
reliability function of a point-to-point discrete memoryless channel is developed. By using a similar idea with a two-user discrete memoryless multiple-
access channel, three lower bounds and two upper bounds on the reliability function are derived. To obtain upper bounds on the reliability function for DM-MACs, we formally
characterize the typicality graph and look at some subgraph containment problems. Finally, a
method to derive new bounds on the
average/maximal error exponent by using known bounds on the maximal/average error exponent is obtained.

access and underserved areas in the U.S. In essay one, I document how in-store product availability varies nationally across different food retailing landscapes, and place these findings in context with existing food desert literature. In essay two, I adapt a demand model, featuring the Distance Metric
method, that explicitly incorporates in-store attributes, store type, and store location to infer household behavior around price sensitivity, store switching, and expenditure sensitivity. In essay three, I conduct an equilibrium analysis of store choice that relates both the demand side, from chapter three, and supply side to store attributes, including price-cost margins and product assortment. Using this framework, I simulate changes in the food retailing landscape, based on a hypothetical policy-informed scenario, to investigate resulting costs and benefits to consumers and food retailers. Understanding the interdependencies between firm behavior, consumer reaction, and geographic variation in consumer demographics provides a foundation to examine the impacts of limited food
access and policies that might address the problems associated with limited food
access.

▼ In this thesis we describe the problematic of QoS security for various services provided through IP network. These applications have high QoS parameter requirements such as delay, loss rate and variation of delay. We provide the required quality using different methods that are responsible for network monitoring and traffic management. One of the main QoS elements we deal with in this thesis are AC methods. These methods have the task of deciding whether they accept or reject a new connection based on its parameters without affecting the QoS of other connections. Furthermore, this thesis deals with the use of neural networks in AC methods. At the end two methods are simulated and compared: the Gauss
method and the neural network utilization
method for 100, 1 000 and 10 000 accesses.

▼ Dans cette thèse, nous explorons des aspects performances de LoRa, l’une des technologies prometteuses pour des équipments IoT. LoRa appartient à une nouvelle classede réseaux appelés “Low Power Wide Area Networks” (LPWAN) et définit une couchede radio spécifique basée sur la modulation “Chirp Spread Spectrum” et une méthodesimple d’accès appelée LoRaWAN. Le fonctionnement de LoRaWAN est similaire àALOHA: un appareil se réveille et envoie immédiatement un paquet à une passerelle.Ce choix de la méthode d’accès a un impact important sur la capacité de LoRa et sonévolutivité vers un grand nombre d’appareils. Il en résulte un niveau élevé de paquetsperdus suite aux collisions à mesure que le nombre d’appareils augmente.Motivés par ces défis, nous avons conçu et mis en œuvre deux méthodes d’accèsaméliorées pour LoRa. Tout d’abord, nous avons proposé d’appliquer la technique“Carrier Sense Multiple Access” (CSMA) à LoRa pour diminuer le nombre de collisions. Nous avons utilisé le simulateur NS-3 pour évaluer le schéma et les résultatsde la simulation montrent que CSMA réduit considérablement le taux de collisionstout en n’augmentant que légèrement la consommation d’énergie. Ensuite, nous avonsproposé Timemaps, une nouvelle méthode d’accès pour améliorer les performances deLoRa. L’idée est de construire une carte temporelle de toutes les transmissions dans lapasserelle et distribuer le planning de transmission lors de l’opération d’attachementd’un équipement au réseau. La carte temporelle permet un chevauchement des transmissions effectués avec des facteurs d’étalement différents. Comme des appareils allouent un facteur d’étalement en fonction de l’éloignement croissant de la passerelle,les transmissions deviennent orthogonales, ce qui conduit à une augmentation globalede la capacité du réseau.Nous avons utilisé le simulateur NS-3 pour évaluer la méthode de Timemaps dansdeux cas: des horloges parfaites et des horloges avec une dérive. La simulation prend encompte la quasi-orthogonalité des transmissions avec différents facteurs d’étalement etl’effet de capture. Les résultats montrent que Timemaps bénéficie d’un taux de livraison de paquets remarquablement plus élevé et du taux de collisions considérablementplus faible par rapport à LoRaWAN avec une consommation d’énergie modérémentaugmentée.Pour valider les deux schémas proposés, nous avons développé un module NS-3qui simule l’opération de LoRa à grain fin. Le module utilise un cadre énergétiqueimplémenté dans NS-3 pour estimer la consommation d’énergie par nœuds et dansl’ensemble du réseau.

▼ Atualmente, a Onion-tree [Carélo et al., 2009] é o método de acesso métrico baseado em memória primária mais eficiente para pesquisa por similaridade disponível na literatura. Ela indexa dados complexos por meio da divisão do espaço métrico em regiões (ou seja, subespaços) disjuntas, usando para isso dois pivôs por nó. Para prover uma boa divisão do espaço métrico, a Onion-tree introduz as seguintes características principais: (i) procedimento de expansão, o qual inclui um método de particionamento que controla o número de subespaços disjuntos gerados em cada nó; (ii) técnica de substituição, a qual pode alterar os pivôs de um nó durante operações de inserção baseado em uma política de substituição que garante uma melhor divisão do espaço métrico, independente da ordem de inserção dos elementos; e (iii) algoritmos para a execução de consultas por abrangência e aos k-vizinhos mais próximos, de forma que esses tipos de consulta possam explorar
eficientemente o método de particionamento da Onion-tree. Entretanto, a Onion-tree apenas oferece funcionalidades voltadas à inserção dos dados um-a-um em sua estrutura. Ela não oferece, portanto, uma operação de bulk-loading que construa o índice considerando todos os elementos do conjunto de dados de uma única vez. A principal vantagem dessa operação é analisar os dados antecipadamente para garantir melhor particionamento possível do espaço métrico. Com isto, a carga inicial de grandes volumes de dados pode ser melhor realizada usando a operação de bulk-loading. Este projeto de mestrado visa suprir a falta da operação de bulk-loading para a Onion-tree, por meio da proposta de algoritmos que exploram as características intrínsecas desse método de acesso métrico. No total, são propostos três algoritmos de bulk-loading, denominados GreedyBL, SampleBL e HeightBL, os quais utilizam respectivamente as seguintes abordagens: gulosa, amostragem e de estimativa da altura do índice. Testes
experimentais realizados sobre conjuntos de dados com volume variando de 2.536 a 102.240 imagens e com dimensionalidade variando de 32 a 117 dimensões mostraram que os algoritmos propostos introduziram vantagens em relação à estrutura criada pelo algoritmo de inserção um-a-um da Onion-tree. Comparado com a inserção um-a-um, o tamanho do índice foi reduzido de 9% até 88%. Em consultas por abrangência, houve redução de 16% até 99% no número de cálculos de distância e de 9% a 99% no tempo gasto em relação à inserção. Em consultas aos k-vizinhos mais próximos, houve redução de 13% a 86% em número de cálculos de distância e de 9% até 63% no tempo gasto

▼ Outpatient clinic appointment scheduling is an important topic in OR/IE studies. Open-
access policy shows its strength in improving patient
access and satisfaction, as well as reducing no-show rate. The traditional far-in-advance scheduling plays an important role in handling chronic and follow-up care. This dissertation discusses a hybrid policy under which a clinic deals with three types of patients. The first type of patients are those who request their appointments before the visit day. The second type of patients schedule their appointment on the visit day. The third type of patients are walk-in patients who go to the clinic without appointments and wait to see the physician in turn.
In this dissertation, the online scheduling policy is addressed for the Type 2 and Type 3 patients, and the offline scheduling policy is used for the Type 1 patients. For the online scheduling policy, two stochastic integer programming (SIP) models are built under two different sets of assumptions. The first set of assumptions ignores the endogenous uncertainty in the problem. An aggregate assigning
method is proposed with the deterministic equivalent problem (DEP) model. This
method is demonstrated to be better than the traditional one-at-a-time assignment through both overestimation and underestimation numerical examples. The DEP formulations are solved using the proposed bound-based sampling
method, which provides approximated solutions and reasonable sample size with the least gap between lower and upper bound of the original objective value.
On the basis of the first set of assumptions and the SIP model, the second set of assumptions considers patient no-shows, preference, cancellations and lateness, which introduce endogenous uncertainty into the SIP model. A modified L-shaped
method and aggregated multicut L-shaped
method are designed to handle the model with decision dependent distribution parameter. Distinctive optimality cut generation schemes are proposed for three types of distribution for linked random variables. Computational experiments are conducted to compare performance and outputs of different methods. An alternative formulation of the problem with simple recourse function is provided, based on which, a mixed integer programming model is established as a convenient complementary
method to evaluate results with expected value.
The offline scheduling aims at assigning a certain number of Type 1 patients with deterministic service time and individual preferences into a limited number of blocks, where the sum of patients’ service time in a block does not exceed the block length. This problem is associated with bin packing problem with restrictions. Heuristic and metaheuristic methods are designed to adapt the added restrictions to the bin packing problem. ▼ Children with attention-deficit/hyperactivity disorder (ADHD) use educational services and health care services for ameliorating its symptoms and problems. Both civil rights legislation and special education legislation support educational services for children with ADHD, but a change in special education legislative policy on ADHD occurred in 1997 that was expected to enable their
access to these services. This dissertation research used a legal
method to investigate
access to educational services surrounding this change. In addition, parents were interviewed in focus groups in order to learn about their perspectives on
access to both educational and health care services for children with ADHD.
Access to services was conceptualized broadly for the study, as common steps within pathways for entering into the ADHD service delivery system.
The substantive nature of 121 administrative case complaints is described, using problem categories and specific issues within categories. Decision makers consistently interpreted policy in order to resolve these disputes, and findings lend support to the 1997 policy development and its implementation enabling
access to special education services for children with ADHD. However, parents in focus groups yet reported problems in
access to educational services for their children with ADHD, particularly with school evaluations for determining children’s eligibility for services. Study implications are discussed, and strategies for stakeholders are aimed at improving service dissemination. Important focus group findings on
access to health care services also emerged, and recommendations are made for improved service
access in this pathway. Parents reported problems of stigma within both services, but good communication occurred both within and between services once children began using services.

ABSTRACT:
21st Century education now requires students to learn using digital technologies. This allows for learning to occur any place and at any time. This requirement, however, involves
access to the internet and digital devices in order for learning to continue beyond the classroom. This study investigates perspectives of Pasifika parents and caregivers from low-socio communities and the impact of digital technologies on children’s education, family and culture.
Using talanoa qualitative methods within the Teu Le Va construct, allowed participants to talanoa through semi-structured interviews. Ten parents and caregivers of Pasifika descent namely from the Cook Islands, Tonga, Samoa, Niue and Fiji took part in the study.
The data analysis revealed three key themes: (1) Disconnections of parents/caregivers within cyberspace from their children; digital exclusions because of
access and skill (2) Reconnections to disadvantaged traditions with traditional inequalities and (3) Interconnections of parents/caregivers with digital technologies and how they currently navigate the digital space.
Recommendations are made for equitable
access for families in low-socio communities around wifi and digital devices. The importance of upskilling parents and caregivers from these communities so that they remain connected with their children in cyberspace and the digital economy. The need for schools to review how parents and caregivers are involved in creating culturally responsive pathways in children’s digital learning.

▼ Increasing energy use has caused many environmental problems including global warming. Energy use is growing rapidly in developing countries and surprisingly a remarkable portion of it is associated with consumed energy to keep the temperature comfortable inside the buildings. Therefore, identifying renewable technologies for cooling and heating is essential. This study introduced applications of steel sheets integrated into the buildings to save energy based on existing technologies. In addition, the proposed application was found to have a considerable chance of market success. Also, satisfying energy needs for space heating and cooling in a single room by using one of the selected applications in different Köppen climate classes was investigated to estimate which climates have a proper potential for benefiting from the application. This study included three independent parts and the results related to each part have been used in the next part. The first part recognizes six different technologies through literature review including Cool Roof, Solar Chimney, Steel Cladding of Building, Night Radiative Cooling, Elastomer Metal Absorber, and Solar Distillation. The second part evaluated the application of different technologies by gathering the experts’ ideas via performing a Delphi method. The results showed that the Solar Chimney has a proper chance for the market. The third part simulated both a solar chimney and a solar chimney with evaporation which were connected to a single well insulated room with a considerable thermal mass. The combination was simulated as a system to estimate the possibility of satisfying cooling needs and heating needs in different climate classes. A Trombe-wall was selected as a sample design for the Solar Chimney and was simulated in different climates. The results implied that the solar chimney had the capability of reducing the cooling needs more than 25% in all of the studied locations and 100% in some locations with dry or temperate climate such as Mashhad, Madrid, and Istanbul. It was also observed that the heating needs were satisfied more than 50% in all of the studied locations, even for the continental climate such as Stockholm and 100% in most locations with a dry climate. Therefore, the Solar Chimney reduces energy use, saves environment resources, and it is a cost effective application. Furthermore, it saves the equipment costs in many locations. All the results mentioned above make the solar chimney a very practical and attractive tool for a wide range of climates.

▼ This thesis critically examines a national programme in mental health which has been driven by the implementation of National Institute of Health and Clinical Excellence (NICE) guidance. Assumptions which underpin research method, drawn from the natural sciences, are critiqued in terms of their adequacy in accounting for human relating and expert therapeutic practice. The work of Dreyfus and Dreyfus (1986) is problematized in how they account for proficiency and expertise as intuition and the leap that they make from calculative to deliberative rationality. An alternative source of understanding, based on non-linear causality and complex responsive processes, is developed, building on the work of Stacey (2001, 2005, 2007). The ineffability of expert practice (or clinical judgement) is contrasted with competence based, rule governed practice, which necessarily underpins the early stages of learning. It is argued that because research practices undertaken in randomised controlled trials (RCTs) must be describable, measurable and focussed on predictable outcomes, then these cannot account for expert practice, therefore the assertion that the Improving Access to Psychological Therapies programme (IAPT) is wholly based on research based, evidence based therapies, cannot be substantiated. The work explores professionalism and specifically considers the role of psychiatrists, psychologists and psychological therapists in mental health and in increasing access to psychological therapies. The role of managers and managerialism are explored, specifically how the NHS has sought to manage 3 professional staff and multi-disciplinary teams in adopting corporate and new ways of working (NWW). This includes the importance of and difficulty in countering professional identity using competence based approaches. The performance management processes in the NHS are recognised as an equally relevant source of evidence (to that of NICE), despite there being a poor (traditional) evidence base for it (Stacey, 2010; Seddon, 2008). Power relating in human relationships is identified as immanent, using the context of a management group, and it is argued that Foucault’s concept of disciplinary power (1994) can account for what is considered to be knowledge and truth, drawing on specialist expertise based on science and research, with a forceful potential for rendering others silent as well as pervasively self-silencing, in processes of inclusion and exclusion (Elias, 1978). It is argued that these on-going processes of relating influence policy decisions at national and local levels and how these policies are implemented in practice. The inevitability of unpredictable outcomes is highlighted, despite strong centralised programme management along with the provision of an explicit blueprint for implementation.

HOSPITALABSTRACTThis pilot study explored Registered Nurses (RNs)
perceptions of the impact of student nurses’ clinical on the
workload of medical-surgical RNs in a Critical
Access Hospital
(CAH). The real-life experience of students in the clinical
learning environment has been seen as a valuable part of nursing
student education. With the increase in the number and size of
nursing programs leading to a shortage of clinical sites,
maintaining a positive relationship with clinical sites is
extremely important to nursing programs. Previous research has
found that nurses often have ambivalent feelings regarding nursing
students on their units.Purpose: Explore RNs” perceptions of the
impact of student nurses’ clinical on the workload of
medical-surgical RNs in a Critical
Access Hospital.Background:
Nursing programs in rural areas often utilize rural hospitals, many
of which are Critical
Access Hospitals (CAHs). CAHs have features
and challenges that make their nurses’ work environment unique.
With a shortage of clinical sites and the challenging work
environment of RNs in CAHs, it is vital that nursing programs
maintain a good relationship with these nurses. No studies have
been reported specific to medical-surgical RNs and students in a
CAH.
Method: Mixed-
method, descriptive, pilot study. The
quantitative portion utilized a 38 item, electronic, adapted
version of the Nursing Students’ Contributions to Clinical Agencies
(NSCCA). The qualitative portion of the study utilized RN
interviews, guided by semi-structured open-ended
questions.Population: Regularly scheduled, day-shift RNs on a
medical-surgical unit of a Critical
Access Hospital.Results: RNs
perceived the impact of student nurses’ clinical as positive. RNs
with less than 10 years of experience viewed students more positive
than RNs with more than 10 years of experience.Conclusions: RNs in
the CAH perceived student nurses’ presence more favorably than
nurses in previous studies in other hospitals types. Although not
statistically significant due to the small sample size, the
perception of students’ contribution to the clinical agency based
on nurses’ years of experience supported previous
research.

▼ Os Sistemas de Gerenciamento de Bases de Dados (SGBD) baseados na Teoria Relacional foram desenvolvidos para atender às necessidades do armazenamento e recuperação de grandes volumes de dados. Esses dados são representados por valores numéricos, datas e/ou pequenas cadeias de caracteres, e são chamados genericamente dados escalares. Com a evolução da tecnologia da informação, torna-se cada vez mais necessário organizar, armazenar e recuperar também outros tipos de dados, a que neste trabalho chamamos de dados complexos, tais como imagens, vídeo, séries temporais e sequências genéticas. As comparações baseadas em Relações de Identidade (RI) ou em Relações de Ordem (RO) são úteis para consultas sobre dados escalares, porém não são adequadas para dados complexos. Para estes, as consultas por similaridade têm sido a opção mais estudada, embora a sua disponibilidade nos SGBDs disponíveis ainda seja limitada. Métodos de Acesso Métrico (MAMs) são
usualmente aplicados para a indexação de dados complexos, de modo a agilizar a execução de consultas por similaridade. O presente trabalho de mestrado visou incorporar recursos de um MAM a um SGBD Relacional. Isso foi feito por meio da proposta e implementação de uma técnica para estender um SGBD Relacional de grande utilização. Assim, implementou-se o MAM conhecido como Slim-Tree no PostgreSQL, que é um SGBD Relacional. A implementação da técnica resultou no RAFIKI, um protótipo capaz de superar a sua antecessora KIARA em termos de velocidade, quando usado para realizar consultas por similaridade. A análise experimental realizada mostrou que o RAFIKI é até 6 vezes mais rápido que a KIARA. Utilizando a técnica proposta, é possível a extensão do PostgreSQL para dar suporte a outros MAMs.
Database Management Systems (DBMS) based on the Relational Theory are designed to meet the needs of storing and retrieving large amounts of data. These data can be represented by numeric values,
dates, and/or small strings, and are generically called scalar data. With the evolution of information technology, it is increasingly necessary to organize, store, and retrieve other types of data. In this work, we call such data complex data, such as images, videos, time series, and genetic sequences. Comparisons based on Identity Relations or Order Relations are useful for querying scalar data but are not suitable for complex data. For complex data, similarity queries have been the most studied option, although their availability in existing DBMS is still limited. Metric Access Methods (MAMs) usually are applied for indexing complex data to speed-up similarity queries. This Masters project aimed at incorporating MAM resources to a Relational DBMS, by proposing and implementing a technique for extending a widely used Relational DBMS. Thus, we implemented the existing Slim-Tree MAM into PostgreSQL, which is a Relational DBMS. This implementation resulted in RAFIKI, a prototype

▼ Um ambiente de data warehousing oferece suporte ao processo de tomada de decisão. Ele consolida dados de fontes de informação distribuições, autônomas e heterogêneas em um único componente, o data warehouse, e realiza o processamento eficiente de consultas analíticas, denominadas OLAP (on-line analytical processing). Um data warehouse convencional armazena apenas dados alfanuméricos. Por outro lado, um data warehouse de imagens armazena, além desses dados convencionais, características intrínsecas de imagens, permitindo a realização de consultas analíticas estendidas com predicados de similaridade entre imagens. Esses ambientes demandam, portanto, a criação de estratégias que possibilitem o processamento eficiente dessas consultas complexas e custosas. Apesar de haver na literatura trabalhos voltados a índices bitmap para ambientes de data warehousing e métodos de acesso métricos para melhorar o desempenho de consultas por similaridade entre
imagens, no melhor do nosso conhecimento, não há uma técnica que investigue essas duas questões em um mesmo contexto. Esta dissertação visa preencher essa lacuna na literatura por meio das seguintes contribuições: (i) proposta do ImageDWindex, um mecanismo para a otimização de consultas analíticas estendidas com predicados de similaridade entre imagens; e (ii) definição de diferentes estratégias de processamento de consultas sobre data warehouses de imagens usando o ImageDW-index. Para validar as soluções propostas, foram desenvolvidas duas outras contribuições secundárias, que são: (iii) o ImageDW-Gen, um gerador de dados com o objetivo de povoar o data warehouse de imagens; e (iv) a proposta de quatro classes de consulta, as quais enfocam em diferentes custos de processamento dos predicados de similaridade entre imagens. Utilizando o ImageDW-Gen, foram realizados testes de desempenho para investigar as vantagens introduzidas pelas estratégias propostas, de acordo com as classes de
consultas definidas. Comparado com o trabalho mais correlato existente na literatura, o uso do ImageDWindex proveu uma melhora no desempenho do processamento de consultas IOLAP que variou em média de 55,57% até 82,16%, considerando uma das estratégias propostas.
A data warehousing environment offers support to the decision-making process. It consolidates data from distributed, autonomous and heterogeneous information sources into one of its main components, the data warehouse. Furthermore, it provides effcient processing of analytical queries (i.e. OLAP queries). A conventional data warehouse stores only alphanumeric data. ▼ Le travail de cette thèse s’inscrit dans le cadre du projet européen H2020 Clean Sky 2 intitulé ALC (« Aircraft Light Communication ») qui a pour objectif d’apporter une preuve de concept de l’intégration de la technologie optique sans fil dans le cockpit d’un avion en alternative des solutions radiofréquences et filaires qui présentent plusieurs limites telles que la sécurité et le confort. Les travaux menés concernent le développement d’un casque audio communiquant en infrarouge dans un réseau bidirectionnel constitué d’un point d’accès situé au plafond du cockpit et de quatre casques portés par les membres de l’équipage. Nous avons commencé par présenter les techniques de communication numérique applicables au contexte afin de motiver le choix pour les formats de modulations OOK (« on-off keying »), PPM (« pulse-position modulation ») et de la méthode d’accès au canal DCF (« distributed coordination function ») avec RTS/CTS (« request to send/clear to send »). Ensuite, nous avons décrit les émetteurs-récepteurs et modélisé le canal optique à partir du logiciel de simulation RaPSor, basé sur la méthode de lancer de rayon associé à une technique de Monte-Carlo, développé à XLIM/SYCOMOR. Cette modélisation réalisée à partir d’un modèle 3D du cockpit et des corps des membres de l’équipage, a permis de déterminer les angles à mi-puissance optimaux des sources situées sur le point d’accès et sur les casques en tenant compte de la robustesse face aux mouvements aléatoires que peuvent effectuer les membres d’équipage durant le vol. Les gains statiques issus de la modélisation du canal ont servi ensuite dans l’étude conjointe des performances des couches physiques et liaison de données. Cette étude a permis de mettre en lumière le compromis entre le niveau de puissance émis et la latence pour une qualité de service visée. Nous avons montré qu’il était possible de concevoir un réseau en optique sans fil dans un cockpit AIRBUS A350 compatible au standard IEEE802.11 avec : le format de modulation 4-PPM, la méthode d’accès au canal DCF avec RTS/CTS, un taux de perte de paquets de 10-4 et un délai de communication avec succès de 2,5 ms. Cela est possible à condition d’adapter la fenêtre de contention du standard IEEE802.11 en fonction de la probabilité de collision, de la taille des paquets et du débit binaire du réseau. Nous avons enfin conçu un banc de test modulaire en radio logicielle. Notre choix s’est porté sur l’environnement GNU Radio associé aux périphériques USRP (« universal software radio peripheral »). Des front-ends optiques ont été spécifiquement développés pour les USRP. Nous avons pu montrer expérimentalement l’influence des paramètres des sources sur le taux de perte des paquets. Le travail effectué dans cette thèse ouvre plusieurs perspectives sur le plan de la simulation et de l’expérimentation.

▼ The subject of this thesis is the issue of students’ access to Greek higher education as well as the birth of the Panhellenic University entrance exams from the first years of the University’ s operation (1837) until the 1964 educational reform. My research method is based on M. Foucault’s archaeological and genealogical method. I have studied data based on the Archived of the Senate Secretariat (1917 - 1930) of the National and Kapodistrian University of Athens (Pergamos digital Library) and the Archives of the Senate Secretariat (1928 - 1955) of Aristotle University of Thessaloniki. Additionally, archives of the Proceedings of the meetings of the Faculties of the National and Kapodistrian University of Athens were studied (meetings on the debate on the introduction of entrance exams were chosen). Also, laws and articles of the daily press were studied. It was found that the establishment of the University was connected with the practice of the free admission to higher education. The high school diploma was the typical criterion to get into higher education because of the necessity of the new institution and in order to find a sufficient number of students. The Constitutional Monarchy (1864), the beginning of industrialization and the rise of the bourgeoisie, may guarantee basic individual freedoms, but also reasons for an ‘entrance’ exam arise. Among the new indirect disciplinary measures is the examination: combining surveillance and regulatory certifications, it extracts the cognitive ‘truth’ from individuals. Furthermore, the introduction of the examination practice was instituted by the Ordinance of 2905 (1922) which states that it will be valid during the School year of 1926-27. This is due to the legitimacy of civil servants (1911) and the passage of laws which regulated public sector recruitment, following participation in scheduled public competitions. The introduction of the entrance exams in the University Chemistry Faculty (1924) is linked to the pressure by the “polytechnic” discourse, the overproduction of chemists in the country as well as the possibility of the merger of chemistry faculties of the University and the Polytechnic.In 1926, the practice of entrance examinations was instituted, and in 1930 the ‘numerus clausus’ was incorporated into a social risk argument due to the overproduction of scientists and the anticipated improvement of the provided education. Ιn the Greek educational reality the following formations of examination discourse took the form of official practice: a) The examination practice within the University campus (1926 to 1938 and 1945 to 1954), b) the entrance examination practice within the University campus (1954 to 1962), c) the examination practice integrated into the context of Secondary Education with benchmarks of multilevel educational institutions (1938 to 1939 and 1964 to 1965).

▼ This research discusses the knowledge about one of the processes of open innovation system, such as the technological access. In this research, we aim to identify a combination of factors with minor level of associated risk, based on the inferential observations of technological risks and corporative about the innovative performance of twenty eight companies with technological base located in Brazil. In synthesis, the proposition of the research is centered in the importance of these companies to manage risks that influence in the innovation processes. The objective of the research was to propose an efficient model for the mitigation of risks in the processes of access to external sources of technological innovation, as part of a management model of open innovation processes. Experimental, statistical and inferential method, was used to collect data in observational situation, once, none of the studied variables were under the control of the researcher. In order to fulfill the general goal, two models were presented: one empirical and another generic. Empiric evidences were found to testify the influence and significance of elements of open innovation to maximize the corporative results. Among these elements, are: the development of partnership with universities and other research centers, the risk of underinvestment in internal and external R&D and the risk of absence of intellectual property. The results also demonstrated that the companies could not maximize totally and simultaneously all the results individualized; so the need of a broad management under the conditionals factors of the best answers and expected results. Moreover, the empirical model allows to the managers to take decisions, from the compatibility of factors that integrate the group of better risk profiles to the diverse expected answers. Based on their feature of probabilistic and generalization, the proposed models contributed promptly, demonstrating to the technology-based companies, the level of risk that must be accepted as condition to the achievement of better results with innovation and, consequently, to access external sources of innovation with minor associated risk.
Esta pesquisa aprofunda o conhecimento sobre um dos processos do sistema da inovação aberta, ou seja, o acesso tecnológico. Uma das diretrizes da pesquisa foi a de identificar uma combinação de fatores com menor nível de risco associado, a partir da observação inferencial dos riscos tecnológicos e corporativos sobre o desempenho inovador de vinte e oito empresas de base tecnológica situadas no Brasil. De forma sintetizada, o problema de pesquisa está centrado na importância das empresas gerenciarem os riscos que influenciam nos processos da inovação. ▼ La sécurité des systèmes d'information devient un enjeu préoccupant pour les organisations tant publiques que privées, car de tels systèmes sont pour la plupart universellement accessibles à partir de navigateurs Web. Parmi tous les aspects liés à la sécurité des systèmes d'information, c'est celui de la sécurité fonctionnelle qui est étudié dans cette thèse sous l'angle de la mise en œuvre de politiques de contrôle d'accès dans une architecture orientée services. L'élément de base de la solution proposée est un modèle générique qui introduit les concepts essentiels pour la conception de gestionnaires d'exécution de politiques de contrôle d'accès et qui établit une séparation nette entre le système d'information et les mécanismes de contrôle d'accès. L'instanciation de ce modèle conduit à un cadre d'applications qui comporte, entre autres, un filtre de contrôle d'accès dynamique. Cette thèse présente également deux méthodes systématiques d'implémentation de ce filtre à partir de politiques écrites en ASTD, une notation graphique formelle basée sur les statecharts augmentés d'opérateurs d'une algèbre de processus. La notation ASTD est plus expressive que la norme RBAC et ses extensions, la solution actuellement privilégiée dans l'industrie. La première méthode repose sur une transformation de politiques de contrôle d'accès, instanciées à partir de patrons de base exprimés en ASTD, en des processus BPEL. La deuxième méthode est basée sur une interprétation de spécifications ASTD par des processus BPEL. Dans les deux cas, les processus BPEL s'exécutent dans un moteur d'exécution BPEL et interagissent avec le système d'information. Ces deux méthodes permettent une implémentation automatique d'un cadre d'applications à partir de la spécification de départ. Finalement, un prototype a été réalisé pour chacune des deux méthodes afin de montrer leur faisabilité au niveau fonctionnel et de comparer leurs performances au niveau système
Nowadays information systems are, more than ever, available through Web browsers. Therefore, they become vulnerable against attack, which has made security an important issue for public and private organizations. This thesis examines in depth one aspect of information system security, namely functional security through access-control policy enforcement, when such systems are implemented in a service-oriented architecture framework. The foundation of the proposed solution is a generic model that introduces essential concepts to design enforcement managers for access-control policies and clearly separates responsibilities between the information system and access control mechanisms. Instantiation of this model results in a general framework, which encompasses many components including a dynamic access-control filter. ▼ Around 20% of the US population reports not acquiring a prescription because of cost, also referred to as cost-related medication nonadherence (CRN) leading to obvious adverse health effects. However, given how those with relatively high incomes report CRN, and given research evidence that people often have reasons for being nonadherent even with acquired medications, and given that many other factors affect health behavior, it seems likely that other factors may be playing a substantial role in medication-
access decisions for those who report CRN. The main research objective was to gain better understanding of what people are experiencing in situations in which they would report CRN. A grounded theory approach was used to better understand what happens when people report CRN. The data from which the framework was based was gathered by interviewing 21 clients at a clinic serving the under-insured who reported CRN. Data coding, analysis, and framework development were performed by the primary investigator and a second independent, with the resulting framework representing the primary investigator s compiling of the two separate ones. A framework that describes what happens when people report CRN was developed. People were able to initially acquire their medication only if they were offered help in acquiring it, were willing to
access known sources of help, or if they were willing to pay the given cost. For those not able to initially acquire their medication, whether or not they sought help depended on their belief in what help was available, and in a re-evaluation of how much they needed the medication. The vast majority of those who sought help eventually received help and were able to acquire their medication. Those who did not seek out help either lived without or substituted with an alternative. Reporting of CRN is not really a one-time event, but a dynamic process in which some people are able to eventually acquire their desired medication, with the key factor being whether or not they receive help in doing do. Factors previously shown to affect drug utilization (such as worries about side effects, addiction, regimen complexity, etc.) had little to no effect on whether or not the drug was acquired. This current research improves our understanding of what happens when people report CRN, and will help in the design of programs to improve
access to needed medication. ( en )

▼ Les troubles liés à l’usage d’alcool (TUAL) représentent un problème majeur mais en France seulement 10% des personnes concernées bénéficient d’une prise en médicale.Les médecins généralistes (MG) occupent une place privilégiée que ce soit pour leur dépistage ou leur prise en charge, tandis que l'’éventail d’approches ciblant autrefois principalement l’abstinence comme objectif thérapeutique, s’est depuis peu élargi pour intégrer des approches visant la réduction de la consommation.Les objectifs de cette thèse sont (1) d’améliorer les connaissances sur les barrières et les leviers de l'accès aux soins pour les TUAL chez les personnes avec des TUAL et chez les MG et (2), d’explorer les enjeux d’une meilleure prise en charge des TUAL au sein de populations avec des comorbidités lourdes. Ce travail s’appuie sur les données de l’étude ASIA,celle de la cohorte ANRS CO13 HEPAVIH.Chez les personnes ayant des TUAL, nous avons identifié deux types de barrières - socio-environnementales et structurelles.Nous avons aussi identifié deux leviers : l'influence familiale et la codépendance au tabac.Chez les MG, les pratiques de dépistage systématique et de prise en charge des TUAL sont associées à une attitude favorable aux stratégies de réduction de la consommation. Les MG qui prennent en charge les TUAL sont également plus susceptibles de prendre en charge le sevrage tabagique et de prescrire des traitements de substitution aux opiacés. Alcohol Use Disorder (AUD) represents a major health and social issue. However, in France, only 10% of people with AUD (PWAUD) receive associated medical care. General Practitioners (GP) play a pivotal role in AUD screening, managing and treatment referring. Until very recently, the standard of care for AUD was based on stopping consumption, maintaining abstinence, preventing chronic complications and managing withdrawal symptoms. However, alcohol-related risk reduction is now becoming an acceptable therapeutic goal for caregivers and patients.The objectives of this thesis were (1) to improve knowledge on barriers and levers to PWAUD accessing care (among GP and people with AUD) for AUD and (2) to explore the challenges of better management of AUD among populations with severe comorbidities.This work is based on data from the ASIA study as well as data from the ANRS CO13 HEPAVIH cohort of HIV-HCV co-infected patients.Among PWAUD, we identified two types of barriers: socio-environmental and structural. We also identified two levers: family influence, whereby the PWAUD accesses care with the aim of improving family relationships, and being a smoker. However, being aware that AUD care no longer necessitates abstinence was not linked to receiving or seeking care.Among GP, systematic screening and AUD management were associated with a favourable attitude toward controlled/reduced drinking strategies.…
