▼ Aims: This PhD project was designed under two studies, the genetic association study to investigate a genetic component or a genetic pathway that might be associated with both schizophrenia and type-2 diabetes (T2D). The gene functional study to explore whether clozapine could affect expression of the genes associated with obesity and T2D. Methods: In genetic association study, a total of 17 single nucleotide polymorphisms (SNPs) were genotyped in the PPARG, PLA2G4A, PTGS2 and AKT1 genes in 221 British nuclear families. In the gene functional study, U937 cells were treated with clozapine (1g/ml and 2g/ml) for 48 hours and 96 hours. Quantitative real-time PCR analysis was used to measure the mRNA expression levels of the genes of interest in clozapine-treated and untreated cells. Results: Eight SNPs tested across the PPARG gene did not show allelic association with schizophrenia. An association was detected at rs2745557 in the PTGS2 locus (2=4.19, p= 0.041) and rs10798059 in the PLA2G4A locus (2=4.28, p=0.039), but these associations did not survive after 10,000 permutations (global p=0.246). Allelic association for the AKT1 gene was detected at rs1130214 (2=6.28, p=0.012) and at rs11847866 (2=4.64, p=0.031) only although the global p-value of overall associations for the AKT1 was 0.059 after 10,000 permutations. Haplotype analysis showed a disease association for the rs1130214-rs2494746-rs11847866 haplotypes (2= 10.18, df= 4, p=0.037), of which the T-G-A haplotype was excessively transmitted (2=6.93, p=0.008) and this haplotypic association survived the Bonferroni correction (p=0.04). The expression of the MTCH2 gene showed a significant decrease in mRNA expression (combined p=0.001) and that of the PPARG gene showed a significant increase (combined p=0.005) in the cells treated with 1g/ml clozapine for 96 hours. Conclusions: The present results support the AKT1 gene association with schizophrenia as reported in previous studies; both the MTCH2 and PPARG genes may be involved in the development of clozapine-induced obesity and in an increased risk of T2D.

▼ Gestational diabetes mellitus (GDM) is one of the most common pregnancy complications. It has been shown that a history of GDM is associated with an increased risk of incident type 2 diabetes in women. In this project, we aim to investigate 1) the trend of GDM incidence in Louisiana State University Health Care Services Division (LSUHCSD) hospital system during 1997 to 2009; 2) the race-specific association between a history of GDM and the risk of incident type 2 diabetes and how the risk changes over years after the index pregnancy. We conducted a retrospective study among women aged 13-50 years. Pregnancies, GDM cases and type 2 diabetes cases were identified by using the International Classification of Disease (ICD) -9 code from the Louisiana State University Hospital-Based Longitudinal Study (LSUHLS) database. The annual incidence of GDM and it standard error (SE) were calculated. Cox proportional hazards regression models were used to estimate the association of a history of GDM with the risk of incident type 2 diabetes. The association between previous GDM and the risk of type 2 diabetes in different postpartum periods was examined using logistic regression. The incidence of GDM increased in most years from 1997 to 2009 and reached a peak in 2002. The incidence of GDM increased with age and reached the peak at 35-39 years of age. Among the three study races, Asians had a significantly higher incidence of GDM than Whites and African Americans. Between 1990 and 2009, 1,142 GDM women and 18,856 non-GDM women presented their first record of pregnancy in the LSUHLS database. During a mean follow-up of 8.6 years, 1,067 women without a history of GDM and 327 women with a history of GDM developed type 2 diabetes. The multivariable-adjusted (age, smoking, income, postpartum body mass index (BMI), postpartum systolic blood pressure, and race) hazard ratio of type 2 diabetes suggested that a history of GDM is a strong predictor of subsequent type 2 diabetes among Louisiana women, especially among African American women. In addition, risk of type 2 diabetes was decreased by the time after the index delivery.

▼ Type 2 Diabetes Mellitus (T2DM) is a widespread chronic disease that negatively impacts an individual's health and well-being, particularly when uncontrolled. Due to the nature of T2DM, individuals are responsible for the challenge of self-managing the disease. Several factors act as barriers and facilitators to self-management, but the literature has failed to establish consensus about how these factors interact with one another. The present study utilized a correlational design to examine the relationships among
diabetes distress, social support, self-efficacy, and performance of
diabetes self-care activities. A total of 33 adults with T2DM participated in the study by completing a battery of surveys regarding performance of
diabetes self-care activities and psychosocial factors. Self-efficacy was associated with
diabetes distress (ρ = -.419). Support satisfaction was related to both self-efficacy (ρ = .495) and
diabetes distress (ρ = -.431), although relationships were not found with other aspects of social support. We did not find any significant relationships among the psychosocial variables and performance of
diabetes self-care activities, though both psychosocial factors and performance of
diabetes self-care activities were linked to key health indicators like A1C and BMI. Our findings suggest that these psychosocial factors should be areas of interest for healthcare practitioners, researchers, and individuals with T2DM.
Diabetes distress, self-efficacy, and social support should be assessed and monitored, in addition to performance of
diabetes self-care activities. Future research should continue to explore relationships among psychosocial and contextual factors and their potential impact on ability to successfully self-manage T2DM.

Resumo: Introdução: Hipertensão Resistente (HAR) é uma condição multifatorial caracterizada por um fenótipo de maior risco cardiovascular. Frequentemente a HAR esta associada a Diabetes Tipo 2 (DMT2) e obesidade, que juntas podem impactar o controle da pressão arterial, aumentando as lesões em órgãos alvo (LOA). Dessa forma, novas abordagens para o tratamento dessa condição estão em estudo com objetivo de diminuir níveis pressóricos e consequentemente melhorar prognóstico cardiovascular (CV). Os antidiabéticos da classe inibidores de SGLT2 mostraram resultados promissores em parâmetros CV,
independente do controle de glicose. Vários mecanismos parecem estar envolvidos nestes resultados, a maioria deles podendo impactar favoravelmente sob os mecanismos de resistência à terapia anti-hipertensiva. O objetivo deste estudo foi avaliar os efeitos da Dapagliflozina, um inibidor de SGLT2, sobre os parâmetros pressóricos em pacientes com HAR e DM2. Métodos: Estudo clínico cruzado, cego para o avaliador com 16 pacientes HAR com DM2 (HbA1c >6,5%) em uso de cloridrato de metformina. Os pacientes foram randomizados e receberam Glibenclamida (5mg/dia) ou Dapagliflozina (10mg/dia) durante 12 semanas, adicionados à terapia anti-hipertensiva usual. Avaliações foram realizadas antes e após cada intervenção, foi adotado um período de 30 dias de washout entre os tratamentos. Ao final ainda foi verificada a adesão farmacológica, e reações adversas. Os parâmetros foram: PA de consultório, monitorização ambulatorial da pressão arterial (MAPA), PA central avaliado por onda de pulso
(PWA) parâmetros hemodinâmicos pelo sistema Finometer. Resultados: Não encontramos diferença nos níveis de pressão arterial (PA) de consultório após 12 semanas de intervenção com glibenclamida e dapagliflozina (PAS: -2 ± 28 vs -8 ± 18 p=0,61); PAD : -1,5 ± 14 e-1,6 ± 14 p=0,82 ) assim como para a PA de 24hrs (PAS:0 ± 16 vs -5 ± 15 p=0,50; PAD: 0,7 ± 11e -2 ±10 p=0,55),PA central (PAS:-1,7 ± 28 vs -4,5 ± 19 p=0,81; PAD: -1 ± 13,9 vs -0,4 ± 11,5 p=0,88) PA Finometer (PAS: -5,8 ± 31,4 vs -3 ± 33,6 p=0,88; PAD: -3 ± 16,3 vs -2 ± 17,7 p=0,94) respectivamente. Conclusão: Embora o uso da dapagliflozina tenha demonstrado uma tendência em reduzir os parâmetros pressóricos, nenhuma das intervenções administradas durante 12 semanas nos pacientes HAR diabéticos alteraram os parâmetros pressóricos avaliados. Exceto para diminuição de pelo menos 10 mmHg de PAS pela analise do aparelho finometer.Estudos com uma população maior de HAR podem demonstrar resultados mais promissores para essas

▼ Worldwide, 1% of all live born children are born with a congenital heart disease (CHD) and currently >95% reach adulthood due to better diagnostics and medical care. At the same time, Diabetes Mellitus (DM), type 1 (T1DM) and type 2 (T2DM), is increasing worldwide. The incidence of the endocrine disease T2DM, which makes up more than 90% of all diabetes increases in particular and is part of the metabolic syndrome. T2DM is due to a decrease in insulin sensitivity and insulin production depending on genetic factors as well as obesity and a sedentary lifestyle. T1DM is an autoimmune disease that can develop due to i.e. genetic factors, exposure to infections and stress-strain leading to an autoimmune response.
The incidence of T1DM in patients with CHD is unknown and the incidence of T2DM in patients with CHD is previously not extensively studied. Also, the effect of T1DM and T2DM in the CHD population on mortality is unknown.
The aim of this thesis was to in large reliable registers and cohorts investigate the prevalence and incidence of T1DM and T2DM in a CHD population, and how this influences the mortality and morbidity in patients with CHD and T1DM and T2DM.
Paper I, a retrospective comparative cohort study, investigated the risk of concurrent CHD in patients with T2DM, regarding T2DM onset, mortality and morbidity compared with patients with T2DM without CHD. The study combined data from the National Diabetes Register (NDR), National Patient Register (NPR) and the Cause of Death Register (CDR).
Out of patients with T2DM, 833 patients with CHD were matched with 5 controls without CHD, matched by sex, year of birth and year of entry in to the NDR.
CHD patients had significantly lower body mass index (BMI), higher creatinine and were more sedentary as compared to patients with T2DM but without CHD. The overall mortality was 26.2% for CHD patients as compared with 19.9% (P<0.001) for the control group, and five-year mortality rates were 5.2% for patients with CHD and T2DM com- pared to 3.4% (P=0.014) in the controls.
In conclusion, CHD and secondary risk factors for cardiovascular disease frequently coexist and the development of T2DM in the adult CHD population is not uncommon with an estimated prevalence of between 4 and 8%. Treatment of conventional cardiovascular risk factors in patients with CHD could be considered important given the relatively high morbidity and high risk for mortality observed in patients with the com- bination of CHD and T2DM.
Paper II, a retrospective comparative cohort study investigated the risk of concurrent CHD in patients with T1DM, regarding T1DM onset, mortality and morbidity compared to patients with T1DM without CHD. Diabetes Mellitus (DM) is a heterogeneous group of metabolic disorders that have in common hyperglycemia. Diabetics under treatment with insulin therapy must embrace responsibility of the control and management of their condition on a daily basis; in this context the congnitive theory of self efficacy (SE) proposed by Bandura can be considered a decisive factor in achieving behavioral goals of diabtes, resulting in improvements to self-care and glycemic control. The research here presented aimed to analyse the self efficacy to the treatment of patients with diabetes mellitus type 1
and 2 in insulin therapy and its correlaton with sociodemographic and clinical variables. To that end, we conducted and analytical study of cross-sectional, with quantitative approach and sample consisting on 134 patients with DM type 1 and 2 in insulin therapy at University Hospital's Endocrinology Clinic - Unit President Dutra (HUUPD) of São Luís-MA. For data collection, we used two instruments: identification data with open and closed questions related to sociodemographic and clinical variables and the Insulin Management Diabetes Self Efficacy (IMDSES), Brazilian version. The search resulted 3.07 global media, on the scale of self efficacy, the areas that stood out with the highest and lowest average respectively were the general management domain (3.38) and the diet field (2.90). We have found the following significant correlations: marital status (widowed had more SE in the diet), education (the higher the education, the better the SE in the management, control and correction of
glycemia), treatment with diet and insulin (higher SE in the diet), absence of dyslipidemia (increased SE to the overall management of diabtes) and age (higher SE for older in the diet). It was concluded that there are factors relates to socioeconomic and clinical features that interfare in self efficacy of patients with Diabetes mellitus type 1 e 2 in insulin therapy, against the desease.

▼ About 180 million people have been estimated to suffer from
type 2 diabetes (T2DM) in 2006 and the annual death rate due to this disease was 3 million by that time. More than
400 medicinal plants used for the treatment of
diabetes mellitus have been recorded, but only a small number of these plants have received scientific and medical evaluation to assess their efficacy. The most common plant used to treat
diabetes mellitus is Hypoxis hemerocallidea (HH). The present study was undertaken to investigate the effects of Hypoxis hemerocallidea (HH) on T2DM in rats. Male Wistar rats weighing 200-250 g were used in this experiment. Hypoxis hemerocallidea (HH) corm was used as plant material in the experiment. The study was based on three parts, an acute
diabetes study, chronic
diabetes study and insulin secretion study. In the acute study, the rats were randomly divided into
2 groups (control and
diabetes). The saline solution was added to different concentrations of HH corm to produce concentration of (50, 200, 400, 800 mg/ml).
Diabetes was induced by intraperitoneal injections of STZ (65mg/kg). Two weeks after the injection (STZ 65 mg/kg), different concentrations of HHS was administered
intraperitoneally after an overnight fast. The blood glucose levels were monitored in the diabetic and control rats at, 30, 60, 120, 180 and 240 minutes post injection. In the chronic study, the rats were randomly divided into 6 different groups (control, HFD, DM,
DM-HH, DM-PTHH, and HH).
Diabetes mellitus was then induced in the groups of diabetic rats by intraperitoneal injections of STZ (40 mg/kg) and rats were fed a high fat diet (HFD). The body weight of the rats were measured weekly for 7 weeks. An intraperitoneal glucose tolerance test (IPGTT) was performed at the end of week 6. At the end of week 7, rats were killed and serum sample were collected for determination of fatty acid and insulin. Liver and pancreatic tissue was collected for histological evaluation. In the insulin secretion study, Hypoxis hemerocallidea was tested for its effects on insulin secretion by
pancreatic islet cells exposed to low (3mM) and high (20mM) glucose medium. Results of the acute study indicated that HHS at a dose 800 mg/ml decreased blood glucose levels fastest in both normal and diabetic rats reaching significance after 30 minutes and 60 minutes respectively and remained below the baseline value until 240 minutes. In the chronic study, it was illustrated that HH had no effect in normal rats on any of the parameters evaluated. Animals in the DM group gained weight the first two weeks, but thereafter began to lose weight. At the end of seven weeks the animals gained significantly less weight than the rest. Animals fed a HFD have more visceral fat compared to the control group. The visceral fat gain occurred in the absence of a significant increase in body weight. ▼ Adiposity has been shown to secrete bioactive cytokines and growth factor known as adipocytokines, they can contribute to obesity,
diabetes and complications of
diabetes. Visfatin is a novel adipocytokine, and it was shown to exert insulin-mimetic effects in stimulating glucose transport and induced triglyceride accumulation in preadipocytes and triglyceride synthesis from gluvose. Visfatin plasma levels are increased in morbid obesity and
type 2 diabetes mellitus. These finding indicate that visfatin may play a role in the association between visceral obesity and increased metabolic risk, visfatin gene suggested that genetic variation in the visfatin gene may, indeed, have a minor effect on visceral and subcutaneous visfatin messenger RNA expression profiles and parameters of glucose and insulin metabolism.
In this study, we explored the relationships between the plasma level of visfatin and genetic single nucleotide polymorphisms (SNPs) of visfatin gene in
type 2 diabetes mellitus (T2DM) with and without macrovascular disease. Plasma visfatin was found to be elevated significantly in T2DM with macrovascular disease patients. Moreover, waist to hip ratio was independently associated with plasma visfatin level. There were statistically significant differences in visfatin -948 G/T genetic variants distribution between T2DM with macrovascular disease and the T2DM control group. The visfatin -948 G/T heterozygotes showed higher mean high-density lipoprotein cholesterol than the carriers of the G allele.
The results of the current study indicated that plasma visfatin levels were associated with macrovascular complications in
type 2 diabetes. However, the definite roles of visfatin in the pathogenesis of insulin resistance, glucose and lipid metabolism are unclear. The observation of changes in the plasma concentrations of visfatin seen in T2DM and T2DM with macrovascular diseases may exert beneficial effects in understanding roles of visfatin in physiologic activity and metabolic disorder. Further studies are needed to elucidate the mechanisms behind visfatin overexpression in humans.

Background:
Diabetes mellitus (DM) is a metabolic problem involving chronically high blood
glucose levels or hyperglycemia, which is thought to arise from insulin deficiency. This insulin
deficiency can be decrease in amount or a complete absence in body. In the long run high blood
glucose level could lead to various complications. Beside altered glucose metabolism carbohydrate,
protein, and lipids metabolisms are also affected in
diabetes mellitus.
Objective: To assess appropriateness of
Type 2 diabetes mellitus drug therapy (T2DM) and
investigate association between fasting blood glucose (FBG) and the patients’ clinical and
demographic characteristics at the
diabetes clinic of Tikur Anbessa specialized hospital (TASH).
Methods: The study was a cross sectional hospital based survey and used both quantitative and
qualitative methods. T2DM patients who came for their follow up treatment at the
diabetes clinic of
TASH over a one month period (April 2013) were used as research participants. Plus key informant
interview with the help of semi-structured open ended questioner was used focusing on the
experience and practice physicians within the
diabetes clinic regarding medication choice and
guidelines they use.
Results: A total of 103 patients enrolled in the survey and the proportion of females (59.
2%) was
higher than males (40.8%). The mean age was 52.
2 years and most of them live in Addis Ababa.
When it comes to clinical characteristics mean FBG was 155.99 ± 44.32 mg/dl, the difference in
FBG between male and female was insignificant (P=0.325). Body mass index (BMI) had a mean
value of 26.4± 3.05 kg/m2 and 51.45% of the patients were overweight. Medication wise NPH
(56.3%), metformin plus glibenclamide (19.4%) and metformin (10.7%) were the most prescribed
drug therapies. And 3.88% of patients were only on glibenclamide. The association of gender, age,
BMI and medication used with the FBG levels of patients was not significant with a p value of 0.6,
0.803, 0.97 and 0.081 respectively. The key informants point out that patient load, physician
rotation, lack of guide-line and devices as a reason for inadequacy of treatment.
Conclusions: The treatment of T2DM is appropriate with existing problems. Metformin was the
first choice oral medication, while most patients were on NPH. And no single factor was associated
with influencing the FBG of patients.

Background: Type 2 Diabetes is becoming increasingly prevalent worldwide at an alarming rate. Physical
activity and adequate dietary patterns are cornerstones for the treatment and prevention of diabetes.
Lifestyle intervention programs can be effective, but their application to the elderly needs further study.
Methods: overall, 43 individuals with diagnosed type 2 diabetes were assigned to control (12 men, 12
women; age 68.9 ± 11.3, range 46- 84 years) or intervention (6 men 13 women; age 69.5 ± 8.9, range 52-
81 years) groups based on their own choice. The intervention group received a lifestyle program
including physical activity classes and dietary counseling for a 9 month period. The control group
received the usual care. Improvement in physical activity and dietary patterns was assessed as well as
changes in biological (weight, body composition, blood pressure, lipid profile, HbA1c, glycemia), fitness
(strength, aerobic endurance, flexibility, agility), psychological (profile of mood states) and economic
variables (individual and state cost of medications).
Results: participants in the intervention group tended to increase their overall physical activity
(p=0.020) and to adopt better dietary patterns (p=0.050). However changes in lifestyle were mild:
changes in overall physical activity outside the program were non-significant and no significant changes
were found in the percentage of compliers with the dietary recommendations for the Portuguese
population. Significant changes were found in lower limb strength (p=0.014), upper limb strength
(p=0.001), aerobic endurance (p=0.001) and upper limb flexibility (p=0.040). Conversely no significant
changes were found for body composition, blood pressure, lipid profile or glycemic control. The profile
of mood states showed a significant decrease in the Fatigue-Inertia factor (p=0.002). The cost of
medication increased substantially for the State (p=0.028) but not for individuals.
Conclusions: The program achieved moderate success in relation to behaviour change, but not enough
to have a significant impact on most clinical and biological variables. Fitness was improved and
perceived fatigue reduced. Our results suggest that in order to be more effective, intervention should
begin as early as possible, as its implementation among elderly subjects is difficult. This would lead to a

▼ Heat Shock Proteins (HSPs) are a class of ubiquitously expressed and functionally related proteins found in all living organisms from humans to bacteria. Their expression is increased in response to various cellular stressors in what is referred to as the heat shock response. The induction of one particular HSP, HSP60, has been found to be correlated with mitochondria specific cell stress. Recently HSP60 has been found to be secreted and expressed extracellularly, after first being thought to be strictly intracellular. However, the mechanisms of these mechanisms of translocation and secretion have not been clearly identified.
The aim of this study was to develop a model in vitro system for HSP60 expression in human cells so that treatments which resulted in mitochondrial impairment could be investigated.
This study looked at several aspects of high glucose, hydrogen peroxide (H₂O₂), and sodium azide treatment on human HeLa cells. At low levels, each treatment had a hormetic effect on HeLa cell growth; however at high concentrations growth was significantly inhibited. Additionally, high treatment concentrations resulted in increased cell lysis as determined by LDH assays. Treatment with concentrations of 100mM glucose, 200μM H₂O₂, and 50μM sodium azide were the only treatment concentrations that did not result in significantly different levels of cell lysis when compared to a control sample. Mitochondrial dehydrogenase activity was also found to be significantly decreased at high treatment concentrations as determined by MTT assays. Thus, 100mM glucose, 200μM H₂O₂, and 50μM sodium azide treatments were identified as optimal conditions for mitochondrial targeted cell stress, as each of these treatments impaired cell growth and inhibited mitochondrial activity while having no significant effect on the degree of cell lysis.
The 100mM glucose, 200μM H₂O₂, and 50μM sodium azide treatments were then investigated for the intracellular reactive oxygen species (ROS) activity they induced over treatment periods of 24 hours, 3 and 7 days. After 7 days of treatment, the 200μM H₂O₂ and 50μM sodium azide treatments resulted in an approximate three-fold increase in ROS activity, while the 100mM glucose treatment resulted in almost twice as much ROS compared to a control sample.
Finally, the effects of 100mM glucose, 200μM H₂O₂, and 50μM sodium azide treatments on HSP60 and HSP70 expression was also examined over 3 and 7 day time periods. After the 7 day treatment period, the 100mM glucose treatment had induced a
2.43 fold increase in HSP60 expression and a
2.75 fold increase in HSP70 expression, the 200μM H₂O₂ induced a 3.48 fold increase in HSP60 expression and a 3.98 fold increase in HSP70 expression, and finally the 50μM sodium azide induced a 4.74 fold increase in HSP60 expression and a 5.08 fold increase in HSP70 expression.
It can therefore be concluded that 100mM glucose, 200μM H₂O₂, and 50μM sodium azide results in the upregulation of HSP60 and HSP70 in human HeLa cells. Therefore this model may be used to…

▼ Numerous studies in a variety of species indicate that caloric restriction (CR) elicits beneficial metabolic effects including reduced adiposity and improved glucose homeostasis, as well as enhanced insulin and leptin sensitivity. Conversely, chronic feeding on high caloric diets brings about opposite outcomes and can progress to
type-II-
diabetes and obesity. An intense effort by many researchers has begun to elucidate the homeostatic mechanisms accounting for these beneficial metabolic effects of CR in order to facilitate development of CR-mimetic drugs to combat the alarming increase of these diseases. Mounting experimental evidence suggests that Sirtuins may be principal mediators of the metabolic effects of CR (1). Sirtuins are believed to sense and respond to cellular energy deficit via their (NAD+)-dependent enzymatic activities including lysine deacetylation of a variety of cytosolic, mitochondrial and nuclear proteins (
2). Initial studies involving SIRT6, one of three nuclear sirtuins, suggest that it may be an attractive drug target for obesity and
type-II-
diabetes therapy. Knockout studies indicate that SIRT6 is required for normal growth, adiposity, and glucose homeostasis (3). Yet, contrasting these Sirt6-null phenotypes with those from opposing SIRT6 gain-of-function animal models lead to incongruous and seemingly contradictory conclusions regarding the stress-responsive homeostatic functions of SIRT6, casting doubt as to whether SIRT6 agonist or antagonist drugs should be sought after. To address these issues, I generated genetically engineered mice (Sirt6BAC mice) designed to eutopically overexpress SIRT6 and mimic its moderate eutopic upregulation observed during CR. This was achieved via BAC- mediated genomic insertion of an isogenic 187kb DNA region from chromosome 10 of mus musculus encompassing Sirt6. These Sirt6BAC mutants fed a high caloric diet exhibit improved glucose homeostasis as indicated via intraperitoneal glucose tolerance tests and intraperitoneal pyruvate tolerance tests. Hyperinsulinemic/euglycemic clamp indicate that these mutants exhibit enhanced insulin-sensitive inhibition of endogenous glucose production as well as enhanced blood glucose disposal and uptake into gastrocnemius and soleus muscle. Importantly, these data suggest that SIRT6 agonist drugs may be worthy of translational research for the treatment of
type-II
diabetes in humans.

▼ The use of complementary and alternative medicine for the management of type 2 diabetes mellitus stems from the experiences, beliefs, and its efficacy. This study was intended to explore the use of complementary and alternative medicine among patients with type 2 diabetes mellitus and to find ways of improving patient -centered care in managing type 2 diabetes successfully. The study used an exploratory descriptive research design and purposive sampling was used to recruit 13 respondents from all the patients diagnosed with type 2 diabetes mellitus who seek health service at the facility. The study was conducted at the Ledzokuku-Krowor Municipal Assembly (LEKMA) Hospital situated at Teshie Nungua in the Greater Accra Region. Semi-structured interview guide was used to conduct in-depth interviews. All interviews were audio recorded and transcribed word for word. Data were analyzed using thematic content analysis. The key findings were; people diagnosed with type 2 diabetes use complementary and alternative medicine due to convincing information from family and friends, the belief in alternative medicine and media influence. Negative attitudes of health professionals in the mainstream health care was found to be a driving force to alternative medicine use. Marketing of herbal medicine, overdose of herbal medicine and possible kidney failure were identified as some risk associated with herbal medicine use. It is recommended that curriculum developers include customer care in the nursing curriculum to enhance clients care. Again, the Ministry of Health need to collaborate with the National Insurance Authority to integrate the cost of complementary and alternative medicine.

type 2 diabetes (T2DM), increases the risk for heart and kidney disease, blindness, stroke, and death. Efforts to prevent T2DM have centered primarily around behavioral interventions, which include increased physical activity and decreased caloric intake. Importantly, the interventions are most effective when implemented early on in disease progression. In this study, we sought to examine the effects of a high fat diet on the epigenetic profile of PGC-1α, a gene responsible for maintaining mitochondrial biogenesis. The mitochondria, the powerhouse of the cell, is responsible for maintaining the energy systems in the body. Therefore, we examined how increasing in caloric intake resulted in changes in the epigenetic profile of the PGC-1α promoter, and how these changes impacted mitochondrial number. Further, we sought to examine how hypermethylation of PGC-1α led to changes in gene and protein expression in the mitochondria. Results from our study indicate that DNA methylation changes preceded disease onset, as characterized by the homeostatic model assessment for insulin resistance (HOMA-IR), the homeostatic model assessment for β-cell dysfunction (HOMA- β), and the quantitative insulin-sensitivity check index (QUICKI). Our data indicate that methylation analysis may serve as diagnostic and risk assessment tool for pre-
diabetes and T2DM in conjunction with physiological measures.

Type 2 Diabetes Mellitus (DM) as they transition from pediatric endocrinology providers to adult DM providers. A secondary purpose of this study was to identify if there was a relationship between attendance rate, age, gender, years diagnosed with DM, ethnicity, insurance
type,
Type of DM, and current county of residence with control of
diabetes, measured by glycosylated hemoglobin (HgbA1c). A quantitative study through chart audits was conducted at Valley Children’s Healthcare, VCH, pediatric endocrinology practice, in Madera, California. Patients with the diagnosis of
Type 1 or
Type 2 DM born been 1997-1999, aged eighteen to twenty-one years old seen for at least one provider visit in 2017, were audited. The data was analyzed for Chi-Square Test of Independence to identify if attendance rate to appointments between 2014-2017 has a relationship to patients year of birth and to determine if the average HgbA1c in 2017 has a relationship to patient demographics.
The research instrument was an excel work sheet used to analyze collected data. Data was collected by manual chart audits and analyzed with SPSS soft wear.
Current literature lacks conclusive data of when transition to adult care skills should be taught to AYAs with DM and how it impacts diabetic control in adolescents and young adults with DM. In addition, unique factors that have not been previously studied in the population seen at VCH are that eighty percent of patients have Medi-Cal California Children Services insurance indicating the majority of the population lives below the federal poverty level; the practice is a regional center for over twelve counties in California; and sixty-five percent of children in this region are from Hispanic origin.

▼ In the general population, fractures are a significant healthcare issue. However, fractures also complicate several endocrine disorders. The overall aim of my study was to investigate the changes in bone structure and material composition underlying bone fragility in a number of endocrine diseases such as type 1 and type 2 diabetes mellitus as well as patients with PTH excess and PTH deficiency.
High-resolution peripheral quantitative computed tomography was used to acquire images of the distal radius and tibia in cases and controls. Bone structure was assessed by the standard analysis protocol from the manufacturers that uses a threshold base algorithm in the type 1 and type 2 diabetes studies. A non-threshold base algorithm was used to analyze bone structure and matrix mineral density in the PTH excess and PTH deficiency study.
The main findings were that patients with type 1 diabetes had no significant deficits in cortical and trabecular bone relative to controls. Patients with T1DM had a tendency towards lower bone formation and bone resorption markers. Patients with type 2 diabetes had similar cortical morphology whereas trabecular bone was better maintained than controls. These results are consistent with higher BMD in the spine and hip as well as lower bone remodelling markers observed in diabetic patients.
PTH excess is deleterious to both cortical and trabecular bone. In patients with untreated primary hyperparathyroidism (PHPT), the normal or high trabecular density reported in several studies is likely to be the result of inclusion of cortical remnants in the medullary compartment. In patients with untreated PHPT, there was a left shift of the void-matrix mineralization density distribution curve relative to controls due to increased cortical porosity. Cortical and trabecular deficits in untreated PHPT may not be completely reversible with surgical treatment. Patients with PTH deficiency due to secondary rather than primary hypoparathyroidism have better maintained cortical and trabecular bone relative to controls as well as higher matrix mineral density. Similar results were found in women and in men. There was a rightward shift of the void bone matrix mineralization distribution curve for patients with hypoparathyroidism relative to controls.
In conclusion, patients with type 1 and type 2 diabetes may have underlying bone fragility predisposing to fractures, but my study failed to identify microstructural abnormalities. Further work is needed to determine whether abnormalities in the material properties of bone may account for the increased fracture risk seen in diabetes. Patients with PTH excess have deficits to both cortical and trabecular bone. ▼ Type 2 Diabetes Mellitus is a chronic disease. Chronic
disease self- management,
diabetes self-management education,
diabetes self-management education and support, and other chronic
disease self-management programs help to decrease complications
from chronic diseases and improve the quality of life among those
experiencing these diseases in the general population. The
incidence of chronic disease is high among the homeless population;
furthermore, they experience an increase in morbidity and mortality
rates from these diseases. The literature related to homeless
individuals with
diabetes and the management of their
diabetes in
the homeless environment is limited. The purpose of this study was
to interpret the lived experience of homeless individuals with
Type
2 Diabetes Mellitus. The study findings provide an increased
understanding of how people with
diabetes can self- manage their
diabetes in a homeless environment. This study was a qualitative
study using the principles of interpretive phenomenology based on
Heideggerian philosophy. Using semi-structured interviews, the
participants were asked to describe their lived experience being
homeless with
diabetes. There were 32 homeless individuals
recruited to the study. The interviews were digitally recorded and
transcribed. The data were analyzed in a disciplined and systematic
manner according to the modified seven step process outlined by
Diekelmann, Allen, and Tanner (1989). The overarching theme
identified from the study was “Being homeless with
diabetes is
difficult” and the subthemes were “It stresses me out” and “It’s
hard to take care of myself.”The findings from the study
demonstrate that
diabetes self-management education and
diabetes
self-management education and support can effectively be
implemented in the homeless environment tailored to homeless
individuals with
diabetes with ongoing
diabetes
support.

diabetes literacy in the elderly
The increase in life expectancy is a prevalent evidence, but efforts are necessary to promote health and prevent diseases, because it appears that elderly people are increasingly at high risk of developing pathologies has they age.
Diabetes is a disease whose incidence and prevalence has increased in the last years, particularly seniors.
This internship report features the work developed in the area of nursing care needs in a specific population, having as general objective to promote the knowledge to the elderly people of the Concelho da Vila do Bispo, about
type 2 DM. The methodology of health planning was used, and a descriptive study of quantitative nature and education for health was carried out.
After the identification of these results, it was performed several health educational actions that counted with the involvement of seniors in order to promote information on
diabetes. The work developed evidence the necessity to continue to develop strategies that include the involvement of diabetic people, so that they can obtain gains in health.

Due to population growth, aging, urbanisation, increasing prevalence of obesity and physical
inactivity, diabetes mellitus (DM) has become one of the most important and prevalent chronic
diseases. Glycated haemoglobin A1c (HbA1c) assessment is currently being used all over to
monitor glycaemic control as a cornerstone of diabetes care. It might also be a useful screening tool
for non–insulin dependent DM, also known as type 2 DM (T2DM). Elevated HbA1c can be linked
with long–term risk of cardiovascular complications.
Aim
The aim of the study was to determine whether HbA1c can be used as reliable screening tool for
early detection of T2DM in an African population.
Methods
This study was a cross–sectional study and was part of the South African, North–West Province (SANWP)
leg of the 12–year Prospective Urban and Rural Epidemiological (PURE) study. Baseline
data was collected from March to December 2005. A total of 2010 volunteers were recruited from
randomly selected households. Data was collected on socio–demographic characteristics, physical
activity, dietary intakes, blood pressure and anthropometry. HbA1c, fasting plasma glucose (FPG),
liver enzymes and HIV status were determined. Ethical approval for the PURE study was obtained
in July 2004. Oral glucose tolerance tests (OGTT) were also done for a sub–group of 465 subjects.
The Statistical Consultation Services of the North–West University were consulted to analyse data
with SPSS 17.0 and STATISTICA 9.0.
Results
The HbA1c values within the diabetic FPG groups were 7.46% for men and 8.08% for women.
HbA1c values increased significantly progressively from the normal FPG groups to the groups with
impaired FPG and the diabetic FPG groups for both men and women. No significant increases were
found in HbA1c between the OGTT groups (normal 2 hour plasma glucose (PG), impaired 2–hour
PG and diabetic 2–hour PG). Total cholesterol, triglycerides, body mass index and FPG increased
significantly and high–density lipoprotein cholesterol decreased significantly with an increase in
HbA1c values in men and women. In addition, systolic blood pressure increased significantly in
women with increased HbA1c. Thus, with an increase in HbA1c, an increase in the number of risk
factors was observed. When using HbA1c and FPG in combination, 43 subjects of the whole population were detected with having a risk of developing T2DM. However, when considering the
commonality of subjects identified to be diabetic or at risk by the OGTT, FPG and HbA1c
individually, only one subject was identified by all the methods as having diabetes or being at risk to
develop diabetes.
Discussion and conclusions
An increase in HbA1c and FPG was associated with an increase in risk factors and therefore with
metabolic syndrome (MS). MS is associated with an increased risk of developing T2DM and
therefore it can be concluded that HbA1c was useful for detecting in this population individuals at

Material y métodos: Se realizó una búsqueda de artículos en español e inglés en las bases de datos PubMed y Scopus, estableciendo como límites los últimos 10 años y que abordasen a la población de 60 o más años.
Resultados: La diabetes mellitus tipo 2 constituye un factor de riesgo para padecer cualquier tipo de demencia aunque con una asociación más fuerte con la demencia vascular. Sin embargo, los mecanismos que vinculan ambas patologías no están del todo claros.
Conclusiones: La diabetes mellitus tipo 2 se puede considerar un factor de riesgo de demencia, quedando la duda de si la fisiopatología principal que conduce al deterioro cognitivo es puramente degenerativa, exclusivamente vascular o una combinación de estas dos. Este proceso requerirá de estudios longitudinales que incluyan una evaluación detallada de la cognición.

▼ Background: Diabetes Mellitus type II is a chronic disease that is characterized by impaired insulin secretion an insulin-resistant tissue. This disease may, if insufficiently threated cause multiple symptoms like fatigue, difficulty sleeping and dry skin. Complications caused by the disease include arteriosclerosis where cardiovascular disease is the most common cause of death in diabetics. Self-care is relevant to decrease these symptoms and complications where Orem’s self-care model can provide insight. Aim: To explore the factors that may be of importance for the patient’s adherence to self-care and health promoting actions at Diabetes Mellitus type II. In addition, the included studies selection groups are described. Method: This study was conducted as a descriptive literature study with 11 articles with qualitative, quantitative and mixed approaches. Main Result: The adherence of self-care at Diabetes Mellitus type II may be affected by factors as if the patient has enough knowledge about the disease and self-care. Additional factors are if the patient is motivated or not, the effect of self-care on the daily life, complications, social support and if the patient is accepting the illness or not. The participants were recruited from different countries and included nurses specialized in diabetes, patients with diabetes and patients with hypertonia. In addition, the participants were recruited from mother studies, health centers and patient records. Conclusion: The nurse should work to promote health and to do that the nurse needs to see the whole patient as an individual and his or her needs. It is therefore important to have sufficient knowledge about the disease, as well as all the factors that may affect the patient's adherence to self-care in diabetes mellitus type II.
Bakgrund: Diabetes Mellitus typ II är en kronisk sjukdom som kännetecknas av nedsatt insulinsekretion och insulinresistent vävnad. Denna sjukdom kan om otillräckligt behandlad resultera i flera symtom som trötthet, sömnsvårigheter och torr hud. Komplikationer av sjukdomen är bland annat åderförkalkning där hjärt- och kärlsjukdomar är den vanligaste dödsorsaken hos personer med diabetes. För att minska risken för dessa symtom och komplikationer är egenvård relevant där Orems egenvårdsmodell kan bidra med insikt. Syfte: Var att undersöka vilka faktorer som kan ha betydelse för patientens följsamhet av egenvård och hälsofrämjande åtgärder vid Diabetes Mellitus typ II. Därtill beskrivs de ingående studiernas urvalsgrupper. Metod: Studien genomfördes som en litteraturstudie med 11 artiklar med kvalitativ, kvantitativ och mixad ansats. ▼ Bakgrund: Typ 2-diabetes ökar med hög omfattande över hela världen och är vanligast hos äldre men drabbar personer i allt yngre åldrar. Egenvård utgör en stor del av behandlingen av typ 2 diabetes och är därför en viktig faktor för att upprätthålla en god livskvalité. Bristande egenvård kan resultera i livshotande komplikationer. Syfte: Syftet med studien var att undersöka vilka svårigheter och möjligheter patienter med typ 2-diabetes upplever att de har för att kunna utföra egenvården själva. Vidare var syftet att redovisa hur data samlats in i de artiklar som används till litteraturstudien. Metod: En beskrivande litteraturstudie som har baserats på elva vetenskapliga artiklar mellan år 2010-2015. Samtliga artiklar fanns att finna i databaserna Cinahl, Pubmed och Scopus. Artiklarna var av kvalitativ ansats och granskades utefter syfte och frågeställningar. Resultat: Patienterna hade bristande kunskap om egenvården, vilket kunde bero på att informationen från sjuksköterskan inte var fullständig eller tillräcklig. Livsstilsförändringar ansåg patienterna som viktigt, ändå fanns en negativ inställning till förändrade vanor. Slutsats: Information och kunskap är avgörande för att bedriva en god egenvård då diabetes typ 2 är en krävande sjukdom för både patient och sjuksköterska. Patienterna såg livsstilsförändringar som en svår uppgift att utföra på egen hand, därför var sjuksköterskan en betydelsefull resurs för att uppnå behandlingsmålen.
Background: Type 2 diabetes has increased significantly worldwide and is most common with older people but affects people at younger and younger ages as well. Self-care constitutes a large part of the treatment of type 2 diabetes and is an important factor in maintaining a good quality of life. Lack of self-care can result in fatal complications. Aim: The purpose of this study was to investigate the difficulties and possibilities of patients with type 2 diabetes and how they feel about managing their self-care. A further aim was to show how the data have been collected in the articles used in the literature study. Method: A descriptive literature study is based on eleven scientific articles published between the years 2010-2015. All the articles were found in databases Cinahl, Pubmed and Scopus. Articles of qualitative approach were examined along the purpose and research questions. Results: The patients had insufficient knowledge about self-care which could be due to the information from the nurse was incomplete or insufficient. ▼ Bakgrund: Diabetes är en sjukdom som ökar kraftigt över hela världen, 80-90 procent har diabetes mellitus typ 2 som är den vanligaste sorten. Egenvård är en viktig del av behandlingen som kan förebygga och fördröja att följdsjukdomar och komplikationer uppkommer. Syfte: Att beskriva upplevelser av sjukdom och egenvård hos personer med diabetes mellitus typ 2, samt att beskriva hur datainsamlingsmetoden gått till i de granskade artiklarna. Metod: En litteraturstudie med beskrivande design. Resultat: Personer upplevde det ofta svårt att leva med nya rutiner men med strategier blev det lättare att behålla följsamheten vid egenvården. Personerna upplevde rädsla och ångest över risken att drabbas av följdsjukdomar och komplikationer. Med stöd och information kunde motivationen till livsstilsförändringar öka. En del kände stort stöd av familj och vänner medan andra tyckte att det var jobbigt med pekpinnar eller kände stor besvikelse då hjälpen inte fanns. Sjukvården hade en stor påverkan på egenvården. Majoriteten av de granskade artiklarnas datainsamlingsmetod använde sig av individuella intervjuer. Slutsats: Med goda kunskaper om diabetes mellitus typ 2 och vikten av egenvård hos sjukvårdspersonal, kan det bidra till att på ett bra sätt ge information till personer med sjukdomen. Individanpassad vård är avgörande för att personer själv ska kunna påverka sin egenvård vilket gör att risken för komplikationer och följdsjukdomar minskar.
Background: Diabetes is a disease that is increasing strongly worldwide, 80-90 percent have type 2 diabetes mellitus which is the most common type. Self-care is an important part of treatment that can prevent and delay the occurrence of sequelae and complications. Aim: To describe experiences of the disease and self-care in people with diabetes mellitus type 2, as well as describe how the data collection method was used in the reviewed articles. Method: A literature study with descriptive design. Results: People often found it difficult to live with new routines, but with strategies it became easier to maintain compliance with self-care. The individuals experienced fear and anxiety over the risk to develop complications and sequelae. With support and information, the motivation for lifestyle changes could increase. Some felt great support from family and friends, while others felt it was difficult with pointers or felt disappointing when the help was not available. The health care had a major impact on self-care. The majority of the collected articles' data collection method used individual interviews. Conclusion: With good knowledge of diabetes mellitus type 2 and the importance of self-care in healthcare professionals, it can help to provide information to people with the disease. Individualized care is crucial for individuals to be able to influence their self-care, which reduces the risk for complications and sequelae.

▼ Estudo descritivo transversal, cujo objetivo foi analisar a relação das variáveis sociodemográficas, de tratamento, hábitos de vida, clínicas e laboratoriais com as variáveis sintomas e fases do estresse, em pessoas com
diabetes mellitus tipo
2, em unidade ambulatorial de hospital de nível terciário de atenção à saúde. A amostra foi constituída por 222 pessoas e os dados foram coletados no período de junho de 2011 a agosto de 2012. O instrumento utilizado foi o Inventário de Sintomas de Stress de Lipp, composto por três quadros de sintomas de estresse, que fornecem porcentagens para classifica-los em quatro fases: alerta, resistência, quase exaustão e exaustão. Entre as características sociodemográficas, de tratamento, hábitos de vida, dados clínicos e laboratoriais, destacam-se que 53,15% das pessoas eram do sexo feminino, 43,85% do masculino, com média de idade de 60,68 anos (DP=8,40), tempo médio de escolaridade de 5,07 anos (DP=4,15), 71,17%
viviam com o companheiro, 57,66% procediam da região de Ribeirão Preto (SP), 50,9% eram aposentados/pensionistas, 87,38% referiu fazer uso de insulina, 78,82% antidiabético oral, 68,91% seguir a dieta recomendada e 46,85% não praticava exercícios físicos regulares. O tempo médio de diagnóstico foi de 15,15 (DP=8,03) anos, índice de massa corporal 32,10 (DP=5,75) Kg/m2 , pressão arterial sistólica 180,10 (DP=14,03) mmHg e diastólica 143,7 (DP=23,63) mmHg, taxa de hemoglobina glicada 9,446 (DP=
2) %, colesterol total 74,9 (DP=41,95) mg/dl, lipoproteína de alta densidade 35,67 (DP=8,58) mg/dl, lipoproteína de baixa densidade 101,55 (34,52) mg/dl e triglicerídeos 203,60 (DP=147,72) mg/dl. Quanto às variáveis do estresse, destaca-se que a maior média das porcentagens obtidas ocorreu para os sintomas de estresse do quadro
2 (20,16%-DP=20,165). Para fins de análises estatísticas, excluiu-se a fase de alerta e aglutinou-se as de exaustão e quase exaustão. A frequência dos sintomas de estresse
referidos pelos participantes nos três quadros distintos permitiu verificar a presença do estresse em 63,51% da amostra, bem como a frequência de 53,60% na fase de resistência ao estresse, e predomínio de sintomas físicos (61,53%). As fases do estresse associaram-se com a idade, procedência, uso de insulina e lipoproteína de baixa densidade; os sintomas do estresse com o uso de antidiabéticos orais, tabagismo, consumo de bebida alcoólica e lipoproteína de baixa densidade. Quanto à porcentagem média obtida nos três quadros, observou-se que houve associação dos sintomas do primeiro quadro com a ocupação; do segundo quadro com o seguimento da dieta, glicemia plasmática de jejum, o colesterol total e lipoproteína de baixa densidade; correlação do terceiro quadro com o índice de massa corporal, de modo positivo e com a renda familiar, de modo negativo. A idade e a hemoglobina glicada correlacionaram-se com o segundo e terceiro quadros, porém de forma negativa e positiva, respectivamente.

Resumo: Estudos epidemiológicos evidenciaram um aumento da prevalência de periodontite crônica (PC) em pessoas idosas com diabetes mellitus tipo 2 (DM2) e sugeriram que a terapia periodontal (TP) pode melhorar o seu controle glicêmico. O presente trabalho tem o objetivo de relatar os resultados de uma revisão sistemática com metanálise e responder a seguinte pergunta: o tratamento periodontal é efetivo para diminuir os índices glicêmicos de idosos portadores de diabetes tipo 2 e peridontite? Para a obtenção dos dados, foi realizada uma revisão sistemática da literatura, com metodologia baseada nos critérios da Cochrane.
Todos os estudos clínicos randomizados, envolvendo a população de idosos com 60 anos ou mais os quais relatassem o efeito da doença periodontal sobre o diabetes mellitus tipo 2 foram considerados elegíveis para a presente revisão. Após o processo de seleção dos 5.856 artigos, 22 estudos foram incluídos na revisão sistemática e 13 na metaanálise. Os resultados mostraram um efeito favorável para o tratamento periodontal em comparação com não-tratamento. A metaanálise indicou redução média de -0,21% HbA1c (IC -0.35, -0.07) após a terapia periodontal em comparação com nenhum tratamento (p = 0,003). O teste do qui-quadrado de heterogeneidade entre os estudos foi 19,20 e o teste para o efeito global (Z) foi de 3,01. Não foi possível determinar implicações clínicas apenas para idosos, uma vez que os estudos não mostraram resultados separados por faixa etária. Os resultados da metanálise apoiam a eficácia da TP na melhoria do controle glicêmico em pacientes com DM2 e PC; no entanto, estudos
futuros são necessários para confirmar estes resultados em amostras específicas de idosos
Epidemiologic studies have reported increased prevalence of chronic periodontitis (CP) in older people with type 2 diabetes mellitus (T2DM) and some have suggested that periodontal therapy (PT) may improve their glycemic control. The aim of this study was to answer to the following question: ``Does PT improve glycemic control in older people with both T2DM and CP?¿¿. This meta-analysis was conducted according to the Cochrane Collaboration. A search of electronic databases was performed for randomized controlled trials involving older people samples and reporting the effects of PT on T2DM. Resumo: A obesidade e suas complicações são um dos grandes problemas de saúde pública da atualidade, trazendo prejuízos para o indivíduo e acarretando grande despesa ao sistema de saúde público. A casca de maracujá amarelo (Passiflora edulis) é um produto rico em compostos bioativos que podem resultar em benefícios para a prevenção ou serem adjuvantes no tratamento das complicações da obesidade. O objetivo deste estudo foi avaliar o efeito da ingestão da farinha da casca de maracujá (Passiflora edulis) (FCM) em parâmetros metabólicos associados à obesidade e metabolismo da glicose em ratos Sprague-Dawley em longo
prazo e o seu efeito agudo nas repostas pós-prandiais em humanos. A FCM foi avaliada quanto à sua composição centesimal, minerais, ácido cianídrico, fitato, tanino, polifenóis, flavonóides e carotenóides totais e sua atividade antioxidante. Para avaliar seu efeito in vivo, ratos machos da linhagem Sprague-Dawley (21 dias) foram divididos em 4 grupos: C (controle), CPF (controle com casca de maracujá), HF (hiperlipídico); HFPF (hiperlipídico com casca de maracujá). Os animais foram alimentados por 4 semanas com dietas normo ou hiperlipídica sem adição de FCM. Após este período, os grupos CPF e HFPF receberam as dietas contendo 2,5% de FCM, por seis semanas adicionais. Foram mensurados os ácidos graxos de cadeia curta (AGCC) e alguns micro-organismos no conteúdo cecal; citocinas, incretinas, lipopolissacarídeos, e perfil lipídico no soro; tolerância à glicose e sensibilidade à insulina; e modulação da expressão gênica de neuropeptídeos hipotalâmicos. Em adultos saudáveis foram
avaliadas as respostas pós-prandias ao consumo de 30 g de FCM em um ensaio agudo. Quanto à sua caracterização, verificou-se que a FCM é composta por grande quantidade de fibra alimentar, especialmente pectina, além de minerais, destacando-se o ferro e o manganês. A FCM também é fonte de polifenóis e carotenóides. Nos animais do grupo CPF, a ingestão energética foi diminuída, seguida pela redução do peso e mudanças em parâmetros metabólicos responsivos à baixa ingestão alimentar. Entretanto, mesmo com menor ingestão da FCM, observou-se uma modulação positiva de micro-organismos patógenos e ácido butírico no conteúdo cecal, acompanhado por uma redução nos níveis de LPS e aumento de GLP-1. Nos animais que ingeriram dieta hiperlipídica, embora a FCM não tenha modificado a contagem de micro-organismos intestinais, ela foi capaz de aumentar os ácidos graxos de cadeia curta e reduzir a concentração de LPS sérico. A Doença Periodontal (DP) e o Diabetes Mellitus tipo 2 (DM2) apresentam a mesma etiopatogênese inflamatória e demonstram uma relação bidirecional, pois DM2 afeta a severidade da DP, e esta pode contribuir para a carga inflamatória total do indivíduo, influenciando a evolução
do DM2. O objetivo do presente estudo foi avaliar os fatores de risco para o desenvolvimento de aterosclerose em pacientes portadores de DM2, com e sem periodontite crônica. Foram analisados 48 pacientes, os quais foram divididos em 2 grupos: Teste (pacientes diabéticos com periodontite crônica) e Controle (pacientes diabéticos sem periodontite crônica). O grupo teste foi tratado com debridamento periodontal e o grupo controle recebeu profilaxia supragengival. Ambos os grupos receberam controle de placa a cada 3 meses. No baseline e 6 meses após o tratamento, foi realizada tomada dos parâmetros clínicos periodontais (IP, IG, PS, RG e NIC) e coleta de sangue para avaliação dos marcadores séricos inflamatórios (oxLDL, CT, TG, LDL, HDL, glicose, HbA1c, PCRus, leucócitos e neutrófilos). Os parâmetros periodontais mostraram melhora significativa (p<0,05) no grupo teste, exceto RG. IP e IG também diminuíram significativamente no grupo controle após 6 meses. CT e HbA1c apresentaram
taxas significativamente maiores no grupo teste, em comparação com o grupo controle. As taxas de PCR-us e HbA1c diminuíram significativamente no grupo teste após a terapia periodontal. A contagem de leucócitos mostrou uma diminuição relevante no grupo controle durante o tempo do estudo. A terapia periodontal promove melhoras nos parâmetros clínicos periodontais, nos parâmetros inflamatórios e nas taxas de HbA1c em pacientes portadores de DM2 e periodontite, mas não interfere nos níveis séricos de oxLDL.

▼ Background: Diabetes mellitus is associated with increased cardiovascular prevalence and mortality. However, this relationship is different between type 1 and type 2 diabetes mellitus. While classical and diabetes-related cardiovascular risk factors are associated with cardiovascular events in both diabetes type, it is unclear how diabetes type plays a role in the prevalence of these cardiovascular risk factors.Objective: The objective of this study was to compare cardiovascular risk factors between type 1 and type 2 diabetes mellitus, in adults, performing a novel method for diabetes type classification.Methods: This is a cross-sectional analysis of participants aged 20 years or older in the National Health and Nutrition Examination Survey (NHANES) 1999-2016 that completed the medical examinations, laboratory tests, and the diabetes questionnaire.Results: A total of 5347 adult participants (age ≥ 20) with diagnosed diabetes were included. Our algorithm classified 230 as type 1 diabetes and 4677 as type 2 diabetes mellitus cases (remaining 440 were excluded). Higher age, triglycerides level, Body Mass Index, and being from races other than White were indicative of type 2 diabetes mellitus, while longer diabetes duration, higher HbA1c rate, more prevalent of chronic kidney disease and having higher education were more indicative of type 1 diabetes mellitus. Lipid panel disturbances, Chronic Kidney Disease, and insulin resistance appeared more prevalent in type 1 diabetes mellitus cohort as previously described.Conclusion: The prevalence of cardiovascular risk factors differs by diabetes type and have also attested temporal trends. Diabetes type should be considered in cardiovascular disease risk assessments and preventions.
