▼ This study was conducted to confirm the presence of
bovine tuberculosis (BTB) and determine its prevalence, based on
skin test reactivity, in cattle reared under extensive farming
conditions in the Govuro district, Inhambane province, Mozambique.
The study was comprised of a primary screening test using the
single intradermal test (SIT) in randomly selected animals from
Colonato and Sede dip tanks in Govuro. Positive reactors to the SIT
were tested again with bovine and avian
tuberculin using the single
intradermal comparative test (SICTT) 7 weeks after the SIT. The
sample size was calculated using Win Episcope 2.0 based on 95%
confidence to detect a 2% expected prevalence using the SIT, with a
1% accepted error and accounting for a total population size of
7208. The calculated sample size was 682 animals. To compensate for
the probability of 20% default in reading, the sample size was
increased to 853. During the testing process (SIT), it was evident
from the first 3 reading days that the apparent prevalence (61,
94%) was higher than expected (2%), hence we decided to stop when
the total number of cattle was 530. During the testing process

This was due to the fact that, at such a high prevalence, it would
not be necessary to achieve as high a precision as 1% accepted
error. A sample size of 530 would be sufficient to achieve a
precision of 4% accepted error, which was regarded as more than
adequate. The 530 cattle, 3 or more years of age, were selected
using systematic random sampling from the two dip tanks (Colonato
371 and Sede 159 animals). All animals were identified by numbers
painted, dorsally on the sacral region. Out of 530 tested cattle by
SIT, 268 were read, and 166/268 (61.94% with 95% confidence
interval [CI]: 55.8 – 67.8%) were found positive, with visible
swallow at the injection site. Apparent prevalence (AP) was found
to be 61.94% while the true prevalence (TP) was 75.92%. The
predictive value of a positive result (PV+) was found to be 87.9%.
No significant difference in apparent prevalence between the two
areas was detected by Fisher’s exact test (P = 0.11). By SICTT, out
of 28 animals positive reactors to SIT, 21 were possible to read,
and 13/21 (61.9%; 95% CI: 55.1 – 89.3%) were found positive. A
three year old bull, positive reactor to the SIT, was slaughtered,
and a detailed post mortem was carried out and organs with visible
lesions were collected for further laboratory testing
(histopathology, culture and isolation of M. Bovis and PCR). Later
on, 30 more positive reactors to the SIT test were slaughtered:
25/30 (83.3%) showed visible lesions compatible with BTB, and total
condemnation of carcass was made in 3/25 (12%) due to generalized

▼ O crescimento da caprinocultura no Brasil e a necessidade de controle sanitário dessas criações justificam o estudo de avaliação do teste tuberculinico na espécie caprina. Utilizando os parâmetros de Silva et al. (2006) foram avaliados 600 caprinos procedentes dos Estados de Minas Gerais, Paraná, Pernambuco, Paraíba e São Paulo. Baseados nos valores do Teste Cervical Comparativo (TCC) 60 animais foram selecionados e submetidos a exame clínico, eutanásia e colheita de material para exames microbiológicos, histopatológicos e moleculares. Dos 36 caprinos positivos, 27 (72%) resultaram em isolamento de micobactérias tipificadas como sendo do complexo M. tuberculosis. Foram identificados focos de tuberculose em caprinos nos Estados de São Paulo, Minas Gerais e Paraíba. Além do isolamento de micobactérias do complexo M. tuberculosis, foram identificados em alguns animais outras bactérias como M. kansasii, M. flavescens, M. avium, complexo M. florentinum M. lentiflavum M. simiae, Corynebacterium pseudotuberculosis e C. bovis. Em um caso houve o isolamento de C. bovis concomitante com micobactéria do complexo M. tuberculosis. As lesões macroscópicas e histopatológicas não diferenciaram a infecção provocada por C. bovis. ou por micobactéria do complexo M. tuberculosis. Os resultados bacteriológicos, histopatológicos e de identificação genética validam a utilização do padrão de interpretação de Silva et al. (2006) no TCC para o diagnóstico da tuberculose em caprinos.
The development of farming goats in Brazil, and the need of sanitary control of these flocks, justify the study of tuberculin test evaluation in goats. Using cut-off points established by Silva et al. (2006), 600 goats from Minas Gerais, Paraná, Pernambuco, Paraíba and São Paulo states were evaluated. According to Comparative Cervical Test values, 60 goats were selected, and submitted to clinical exams, euthanized and materials collected for microbiologycal, histopathologycal and genetic typing. Out of the 36 positive goats, 27 (72%) had isolation of micobacteria classified like Mycobaterium tuberculosis complex. Focus of tuberculosis were identified in São Paulo, Minas Gerais and Paraíba states. Besides isolation of the M. tuberculosis complex micobacteria, other bacterias like M. kansasii, M. flavescens, M. avium, M. florentinum lentiflavum simiae complex, Corynebacterium pseudotuberculosis and C. bovis were identified in some animals. In one case, isolation of C. bovis occurred together with micobacteria of the M. tuberculosis complex. The macroscopic and histopathological lesions did not discriminate infection for C. bovis or of the M. tuberculosis complex micobateria. The bacteriological, histopathological and genetic typing results validate the use of cut-off points established by Silva et al. (2006), in the Cervical Comparative Test for diagnosis of tuberculosis in goats.

▼ ENGLISH ABSTRACT: Cattle and African buffaloes are major maintenance hosts of Mycobacterium bovis in South Africa and therefore serve as a potential source of infection for other animals and humans. Mycobacterium bovis is the causative agent of bovine tuberculosis (BTB). Identification and removal of M. bovis-infected animals from the herd at early stages of infection is important for effective disease management and relies on the strategic application of ante-mortem diagnostic tests. Ante-mortem diagnostic tests rely on the detection of specific immunological responses in the animal. However, these responses vary over the course of infection and therefore the accuracy of the diagnostic test may change, depending on the stage of infection. We therefore aimed to investigate the temporal changes in antigen-specific cytokine production and humoral responses to BTB in buffaloes. Interferon-gamma (IFN-ɣ) and IFN-ɣ-induced protein 10 (IP-10) production in response to mycobacterial antigens ESAT-6/CFP-10, and serum antibodies to bovine purified protein derivative (bPPD) were measured in a group of chronically test-positive, newly converted test-positive and test-negative buffaloes over one year. No consistent trends in immune responses over time were observed in any of the groups.
Cytokine release assays (CRAs) are often used in conjunction with the
tuberculin skin test (TST) to improve diagnostic performance. However, many studies have shown that the TST may influence IFN-ɣ production and thereby the outcome of immunodiagnostic tests in cattle. However, these results were conflicting. A better understanding of the influence of the TST on cytokine production and the duration of this effect in a South African situation is required. We therefore aimed to investigate if the TST has an influence on IFN-ɣ and IP-10 production and test outcome in cattle and African buffaloes. IFN-ɣ and IP-10 release were measured in response to bPPD, avian PPD, ESAT-6 and CFP-10 in groups of TST-positive cattle 6, 7, 21, 41 and 78 days post-TST and in a group of TST-negative cattle 7, 21 and 78 days post-TST. IFN-ɣ and IP-10 levels in response to PPD were elevated 1-3 weeks post-TST, followed by a decrease by 41 days, suggesting immune boosting by the TST. ESAT-6/CFP-10-specific cytokine release showed conflicting results, with a group of animals showing decreased cytokine production by 41 days, whereas another group showed no change in cytokine release over time. Our findings suggest that cattle should not be tested with CRAs between 6 and 41 days post-TST to avoid boosting of cytokine levels and inaccurate test results.

▼ O diagnóstico e o tratamento da infecção latente pelo Mycobacterium tuberculosis (ILTB) são indicados para grupos nos quais a prevalência da infecção latente é alta, em contactantes de casos novos de tuberculose (TB) e quando o risco de reativação é alto como em pessoas vivendo com HIV/AIDS (PVHA). Tanto o vírus da imunodeficiência humana (HIV) facilita a reativação da ILTB quanto o Mycobacterium tuberculosis contribui para a progressão da doença pelo HIV. O conhecimento acerca do diagnóstico e do tratamento da ILTB em PVHA torna-se fundamental visto que o Relatório Global de Controle da Tuberculose da Organização Mundial da Saúde (OMS) indica que as PVHA estão 26 a 31 vezes mais propensas a desenvolver TB ativa quando comparadas à população geral. Além disso, a taxa de letalidade da TB em PVHA é 3 vezes maior do que a observada na população geral. Apesar de suas limitações, o teste tuberculínico (TT) continua sendo a principal ferramenta de diagnóstico da ILTB, entretanto, isso não parece refletir no número de TT solicitados e realizados e, consequentemente, no número de tratamentos prescritos para ILTB. No Brasil, e em particular na Paraíba, os dados sobre a solicitação e realização do TT e acerca da prescrição do tratamento para ILTB são pouco conhecidos. Esse estudo objetivou verificar a frequência de solicitação e de realização (inoculação do derivado protéico purificado (PPD) e leitura) do TT, a frequência de TT reator e a frequência da prescrição do tratamento para ILTB e caracterizar as PVHA atendidas em serviço de referência em HIV/AIDS e TB no estado da Paraíba quanto a aspectos sociodemográficos e laboratoriais, no período de janeiro de 2009 a dezembro de 2013. Para obtenção dos dados, utilizou-se formulário padronizado, preenchido, retrospectivamente, a partir das informações contidas na primeira consulta registrada nos prontuários dos pacientes atendidos no período do estudo. Dos 3.191 pacientes incluídos na pesquisa, 2.303 (72,2%) tiveram o TT solicitado. Destes, 2.047 (89,0%) foram submetidos a realização do TT que compreendeu a inoculação do PPD e a leitura da induração. Dos 2.047 pacientes que tiveram o PPD inoculado e submetidos a leitura da induração, 90 (4,4%) pacientes tiveram o TT reator sendo o tratamento para ILTB prescrito para todos. Os resultados da pesquisa sugerem que há uma excelente adesão à solicitação do TT e à prescrição do tratamento para ILTB entre os profissionais médicos e baixa prevalência de ILTB no local do estudo. Outrossim, acessibilidade adequada para realização e boa compreensão por parte dos pacientes quanto a sua importância no contexto da atenção à saúde das PVHA garantiram a frequência elevada de realização do TT.

▼ Natural and synthetic self-assembling polymers and proteins could be bioengineered to
display and/or encapsulate antigens to serve as innovative antigen carrier systems for
the induction of desirable immunities. Polyhydroxyalkanoates (PHAs) are naturally
occurring polyesters synthesized as cytoplasmic polyester inclusions (polyester
particles) by various bacteria. The particles have been used as an antigen delivery
platform by translationally fusing antigens to the particle surface-associated protein,
PHA synthase. Furthermore, it has been found that protein inclusion bodies contain a
large amount of correctly folded and biologically active proteins and could be
engineered to perform as an antigen carrier system. Tuberculosis (TB) is a global health
issue for both humans and animals. Inaccurate diagnosis and inefficacious vaccination
make TB control problematic. The Mantoux tuberculin skin test gives false positive
results if humans or animals are vaccinated with the Bacille Calmette-Guérin (BCG)
strain or exposed to environmental mycobacteria. BCG cannot provide effective
protection against TB. Subunit vaccines have great promise to protect against infectious
diseases, but they are often weak immunogenically. A strategy to circumvent this
problem is the use of self-assembly particulate vaccines, which could present multiple
copies of antigens and serve as a depot for prolonged multivalent antigen display to
induce enhanced immunogenicity. In this thesis, four specific TB diagnostic antigens —
CFP10, Rv3615c, ESAT6, and Rv3020c — were displayed on polyester particles. The
results showed that polyester particles displaying TB antigens specifically distinguished
TB-infected from non-infected cattle. Antigen immunogenicity was dramatically
enhanced after the display on polyester particles, which lowered the antigen
concentration (0.1 to 3 μg dose/inoculum) required for skin tests. Mycobacterial
vaccines H4 (Ag85B-TB10.4) or H28 (Ag85B-TB10.4-Rv2660c) were bioengineered to
display H4/H28 on polyester particles and/or self-assemble H4/H28 into protein
inclusion bodies. The results demonstrated that polyester particle-/protein inclusion
body-based particulate TB vaccines increased overall immunogenicity by enhancing
humoral (for example, IgG1 and IgG2c) and cellular (for example, IFNγ and IL17A)
immune responses when compared to respective soluble antigens.

▼ Os profissionais que prestam cuidados assistenciais em saÃde constituem populaÃÃo de risco para tuberculose (TB) infecÃÃo. O teste tuberculÃnico (TT) desempenha um importante papel no controle epidemiolÃgico da doenÃa, identificando indivÃduos com maior risco de desenvolvÃ-la. Estudo transversal que objetivou investigar a ocorrÃncia de TB infecÃÃo entre profissionais que trabalhavam no Instituto Doutor Josà Frota (IJF) entre os meses de outubro e dezembro de 2014 e o nÃvel de conhecimento que apresentavam sobre tuberculose. Os setores da instituiÃÃo hospitalar classificados pelo serviÃo de medicina do trabalho quanto à presenÃa ou ausÃncia do risco biolÃgico foram agrupados quanto à presenÃa ou ausÃncia do risco de exposiÃÃo profissional a M. tuberculosis e logo apÃs, sorteados aleatoriamente dentre de cada grupo de risco. Aplicou-se um questionÃrio semi-estruturado e o TT. Foram avaliados 220 profissionais, sendo 151 (68,6%) de setores assistenciais e 69 (31,4%) de administrativos. Houve predominÃncia do sexo feminino (80,9%), com mediana de idade de 45 anos, moravam na capital (54,1%), com familiares (90,5%), possuÃam cÃnjuge (72,7%), nÃvel de escolaridade mÃdio (46,9%) e que nunca fumaram (77,7%) ou ingeriram bebidas alcoÃlicas (56,8%). A prevalÃncia de infecÃÃo por TB, foi de 47,3% e a presenÃa do efeito booster foi identificada em 8% dos profissionais de saÃde que trabalhavam em setores de risco, sendo 3 (1,4%) enfermeiras, 3 (1,4%) tÃcnicas de enfermagem e 1 (0,5%) psicÃloga. Os setores classificados como risco de exposiÃÃo profissional a M. tuberculosis, onde foi localizada a maior quantidade de profissionais com resultados positivos foram Centro CirÃrgico (n=18; 8,2%), EmergÃncia (n=12; 5,5%) e Traumatologia (n=11; 5%). Jà dentre aqueles classificados como sem risco, foram encontrados profissionais com teste positivo na Biblioteca e AuditÃrios (n=3; 1,4%), seguido por Almoxarifado Central, Procuradoria JurÃdica e NÃcleo de Pessoal, onde cada setor apresentou dois indivÃduos, totalizando seis. A Ãrea de enfermagem foi a de maior positividade: 28 (12,7%) tÃcnicos de enfermagem, 23 (10,4%) auxiliares de enfermagem e 8 (3,6%) enfermeiras, obtiveram diÃmetros de teste que variaram entre 10mm e 25mm. A presenÃa de TB infecÃÃo esteve estatisticamente associada com a escolaridade (p=0,017), renda individual (p=0,028) e nÃvel da categoria profissional (p=0,028). A avaliaÃÃo do conhecimento sobre TB revelou que somente 3,2% dos profissionais que trabalhavam na EmergÃncia possuÃam conhecimento suficiente sobre o assunto. Os tÃpicos onde houve o maior nÃmero de acertos foram os que abordavam sobre caracterÃsticas gerais da infecÃÃo e transmissÃo por TB. Pode-se concluir que a prevalÃncia de infecÃÃo por TB nos profissionais que trabalhavam no IJF foi considerada alta e o conhecimento que os mesmos tÃm sobre tuberculose mostrou-se insatisfatÃrio

▼ A tuberculose, causada por Mycobacterium bovis, permanece como uma das principais doenças infecciosas de bovinos, em virtude dos prejuízos causados com a redução na produção de carne e leite, condenação de carcaças em abatedouros e embargos na comercialização de animais e produtos de origem animal, bem como pelo potencial zoonótico do patógeno, particularmente pela eliminação pelo leite. Ademais, micobactérias não tuberculosas ou atípicas são causa emergente de doenças em humanos e em animais. Apesar de a tuberculinização representar o principal teste diagnóstico “in vivo” da doença em rebanhos, poucos estudos têm avaliado a eliminação de micobactérias pelo leite em vacas positivas nos testes intradérmicos. O objetivo do presente estudo foi identificar espécies de Mycobacterium em amostras de leite de vacas positivas no teste de tuberculinização cervical comparativo. Um “pool” do leite dos quartos de 142 (2,97%) vacas positivas - proveniente de 4.766 animais tuberculinizados - foi coletado assepticamente e cultivado em condições de aerobiose a 37 °C nos meios de Lowestein-Jensen e Stonebrink, mantidos por até 90 dias. Colônias compatíveis com micobactérias foram submetidas à coloração de Ziehl-Neelsen para o diagnóstico de bacilos álcool-ácido resistentes (BAAR) e detecção molecular. Catorze estirpes isoladas em 12 amostras (n=12/142, 8.4%) foram classificadas pela técnica de reação em cadeia da polimerase com enzimas de restrição (PCR-PRA) e sequenciamento como segue: M. mucogenicum tipo 2 (n=4), M. nonchromogenicum tipo 1 (n=3), M. hodleri type 1 (n=2), M. terrae tipo 3 (n=3), M. cookii tipo 1 (n=1), e M. szulgai tipo 1 (n=1). O sequenciamento do gene hsp65 revelou >98% de similaridade com as espécies de micobactérias depositadas no GeneBank, e detectou: M. engbaekii (n=5), M. arupense (n=4), M. nonchromogenicum (n=3) e M. heraklionense (n=2). Houve concordância na detecção das espécies de micobactérias entre PCR-PRA e sequenciamento em três dos isolados, exclusivamente da espécie M. nonchromogenicum (n=3/14; 21.4%). Árvore filogenética e dendograma (baseado no gene hsp65) dos 14 isolados revelou 4 clusters e 3 subclusters (79,0% de similaridade), respectivamente, e classificou os isolados como micobactérias atípicas agrupados no complexo M. terrae. Apesar da ausência de espécies do complexo M. tuberculosis nas amostras cultivadas, a presença de micobactérias atípicas, alerta para o risco de transmissão do patógeno pelo leite ou derivados para os humanos, visto que muitas dessas espécies de micobactérias já foram descritas causando infecções pulmonares e extrapulmonares em humanos imunocompetentes e imunossuprimidos

▼ O objetivo deste trabalho foi avaliar a ocorrência da tuberculose em um plantel bovino pelo teste cervical comparativo (TCC), e analisar a eficiência de exames complementares na confirmação da infecção. O estudo foi realizado em um rebanho de exploração leiteira no município de Perdizes MG. Realizou-se a tuberculinização em 164 bovinos, sendo que destes 40 foram encaminhados ao abate sanitário. A inspeção macroscópica post mortem das carcaças foi acompanhada de colheita de amostras de muco nasal, sangue e tecido (fígado, pulmão e linfonodo mediastínico) para os exames de PCR, ELISA e histopatológico com colorações de Hematoxilina-Eosina (HE) e Ziehl-Neelsen (ZN) respectivamente. Dos 164 bovinos avaliados pelo TCC, 41 (25%) foram positivos, 29 (17,68%) inconclusivos e 94 (57,32%) negativos. Dos 40 bovinos encaminhados para abate, 22 (55%) carcaças apresentaram algum tipo de lesão macroscópica sugestiva de tuberculose, sendo 14 (35%) nos linfonodos mediastínicos, sete (17,5%) no fígado e seis (15%) no pulmão. Nos achados histopatológicos visualizados em HE 13 (32,5%) carcaças apresentaram alterações histológicas, sendo seis (15%) nos linfonodos mediastínicos, cinco (12,5%) no fígado e três (7,5%) no pulmão. Não foi observada a presença de bacilos álcool-ácido resistentes em nenhuma das amostras avaliadas. O ensaio sorológico de ELISA/IDEXX identificou um (2,5%) animal reagente, e o teste de PCR detectou DNA de M. bovis em uma (2,5%) amostra. Por ser um rebanho leiteiro, a ocorrência da tuberculose pode representar um risco zoonótico. O TCC já deveria ser um exame de rotina obrigatório em toda propriedade que comercializa leite ou carne, pois reconhece infecções recentes, muitas vezes não detectadas em exames complementares.
The aim of this study was to evaluate the incidence of tuberculosis in a cattle herd by the cervical comparative intradermal tuberculin test (ITT), and analyze the efficiency of complementary diagnostic methods in confirming the infection. The study was conducted on a breeding dairy farm in the city of Perdizes MG. Tuberculin test was performed in 164 cattle, of which 40 were positive and slaughtered. The macroscopic post mortem inspection of carcasses was followed by the collection of nasal swabs, blood and tissue samples (liver, lung and mediastinal lymph node) for PCR tests, ELISA and histopatology with Hematoxylin-Eosin (HE) and Ziehl-Neelsen (ZN) respectively. Of the 164 cattle analysed by ITT, 41 (25%) were positive, 29 (17.68%) inconclusive and 94 (57.32%) negative. Of the 40 cattle slaughtered, 22 (55%) carcasses had macroscopic lesions suggestive of tuberculosis, 14 (35%) in the mediastinal lymph nodes, seven (17.5%) in the liver and six (15%) in the lung. The histopathology HE identified 13 (32.5%) carcasses with histological changes, six (15%) in the mediastinal lymph nodes, five (12.5%) in the liver and three (7.5%) in the lung. ▼ ENGLISH ABSTRACT: Mycobacterium bovis, a member of the Mycobacterium tuberculosis complex and the cause
of bovine tuberculosis (bTB), has an extensive host range that includes livestock and wildlife.
While warthogs are considered spill-over hosts for bTB, they could potentially become
reservoir hosts, if conditions are favourable, i.e. increased population densities. With limited
knowledge on the infection status of warthogs in South Africa and their epidemiological
significance for other species, it is imperative to have readily available diagnostic tests for
warthogs.
Therefore, this study aimed to (i) establish reference cohorts of M. bovis-infected and
uninfected warthogs; (ii) utilize these for the development and evaluation of diagnostic tools
that can distinguish between infected and uninfected individuals; (iii) determine the
seroprevalence of bTB in warthogs using the newly developed diagnostic tools; and (iv)
characterize the genetic diversity of M. bovis isolates from warthogs.
Three serological assays, i.e. the indirect PPD ELISA, the TB ELISA-VK® and the DPP®
VetTB Assay, could distinguish between M. bovis-infected and uninfected warthogs with
high sensitivity (75-88%) and specificity (79-89%). The overall seroprevalence from four M.
bovis-endemic locations was high, i.e. 38%. Furthermore, three tests measuring the cellmediated
immune responses of warthogs were developed. A cytokine release assay
measuring interferon gamma induced protein 10 was able to distinguish between M. bovisinfected
and uninfected warthog with a sensitivity of 68% and a specificity of 84%. The
comparative intradermal
tuberculin test classified 100% of culture-negative warthogs as test
negative and 81% of culture-positive warthogs as test positive. Lastly, CXCL9, 10, 11, IFNG and TNFA gene expression were significantly increased in whole blood from M. bovis-infected warthogs in response to antigen stimulation, with CXCL10 showing the greatest mean fold increase. High genetic diversity of M. bovis isolates from warthogs was confirmed through spoligotyping and whole genome sequencing. Two distinct clades of M. bovis were identified by WGS, even though they shared the same spoligotype patterns This study has demonstrated that warthogs develop measurable and specific immune responses to M. bovis infection, which can be used to identify infected individuals ante-mortem. Furthermore, these tests will facilitate epidemiological studies of bTB in warthogs. With a high culture prevalence in warthogs from bTB endemic areas such as uMhkuze Nature Reserve and the Greater Kruger National Park, and high seroprevalence, warthogs seem to be highly susceptible to M. bovis infection. BCG randelis, ištirti specifiniai antikūnai kraujo serume,
įvertintos alerginės būklės. Disertacijos išvadose konstatuojama,
kad 3 mėn. amžiaus BCG vakcinuotų vaikų tuberkulino reakcijos yra
stipresnės bei susidariusi vietinė BCG žymė dažnesnė, nei įprasta
BCG vakcinos doze naujagimio amžiaus skiepytųjų, tiriant juos 3
mėn. po BCG vakcinacijos ar 1 m. amžiaus. Specifinių antikūnų
dėsningumų nepavyko aptikti. 3 mėn. amžiaus kūdikių BCG vakcinacija
sumažino su maistu siejamų alerginių bėrimų riziką mergaitėms
ikimokykliniu laikotarpiu.
Object of dissertation: The influence of
different BCG vaccination schedules on the tuberculin reactivity,
BCG scarring, specific serology and allergy. 509 children were
included in this research, all of them received BCG vaccination
with standard or half dosage as newborns or 3-months old. 3 months
after BCG vaccination and at one, two, and six years of age they
were tested with tuberculin, and examined for BCG scarring.
Anti-tuberculosis antibodies were detected in sera and children
were evaluated for allergy. In conclusions of the dissertation it
is declared, that 3 months after BCG vaccination and at 1 year of
age children, vaccinated as 3 months - old had stronger tuberculin
reactions and better BCG scarring, in comparison to neonatal
vaccination. Clear consistent patterns in anti-tuberculosis
antibody levels were not established. BCG vaccination at 3 months
of age reduced the risk of food –related allergic rash for girls in
preschool period.

▼ Background: Streamlining onboarding processes for new hires to maximize efficiency and reduce costs while meeting regulatory requirements is a constant challenge for healthcare systems’ Employee Health staff. Health screening is a required step and includes obtaining a detailed health history, tuberculosis screening, drug screens, immunizations, fit for duty examinations, obtaining medical records, clarification of disability accommodations, pre-work screens, and other tests which are time consuming and result in delays in hire dates. Faced with a high volume of potential new employee hires a major southeast healthcare system was concerned about delays in new hire start dates. The two-step
tuberculin skin test administration and follow-up process was identified as a potential area for concern to improve onboarding efficiency.
Method: A quality improvement study was designed and implemented to compare baseline testing for new employees with an Interferon-Gamma Release Assay (IGRA) known as QuantiFERON®-TB Gold In-Tube Test (QFT®-GIT) to the two step PPD
Tuberculin Skin Test (TST) for tuberculosis screening time, overall onboarding time, compliance with screening within 10 days of hire date, and associated costs. A retrospective electronic record review included a sample of 484 new hire employees.
Results: Results showed that the QFT®-GIT for tuberculosis screening in comparison to the TST testing significantly reduced tuberculosis screening time for new hire employees (TST = 8.03 days, QFT®-GIT = 4.11 days; p<.0001) and overall onboarding time (TST = 7.92 days, QFT®-GIT = 5.07 days; p<.0001) while improving compliance with tuberculosis screening within 10 days of hire date (TST = 92.92%, QFT®-GIT =100%; p<.0001).
Conclusions: The utilization of QFT®-GIT for tuberculosis screening of new employees significantly reduced screening and onboarding time while improving compliance with screening within 10 days of the hire date. Anecdotal feedback from hiring managers and senior management indicated improved satisfaction with the Employee Health hiring process. Implications: Healthcare systems should consider implementation of an IGRA to streamline processes for onboarding new employees. New processes require negotiations between healthcare systems and lab vendors, changes in policies and procedures, and employee health and laboratory staff development. Future research should focus on cost analyses, as well as, IGRA use for annual screenings.

▼ Aim of this study was to estimate the incidence of Tuberculin Skin Test (TST) conversion in health care professionals and to assess the risk factors for the transmission of mycobacterium tuberculosis to health care professionals.
Material and Methods: The sample consisted of 115 persons from the nursing and support staff of three large hospitals in Athens. Tuberculin Skin Test (TST) was done using the intradermal injection of tuberculin (Mantoux technique). Along with the implementation of the Tuberculin Skin Test (Mantoux) a questionnaire was completed by the participants that provided the possible source of contamination in case of a positive Mantoux or illness from tuberculosis.
Results: Of the 115 participants, 4.3% resulted positive to the Mantoux and did not further participate in the study. At the end of the first year 22% of the participants who were negative in the initial Mantoux became positive. People who worked in Respiratory Medicine Department showed a significantly higher percentage (30.5%) of positive Mantoux compared to those working in lnternal Medicine Department (12%). Greater impact on the occurrence of a positive Mantoux at 1 year was found among those aged over 42 years compared with those below 32 years. (p=0,046%)
Conclusions: 1) The department, in which a health care professional works, plays an important role in the conversion of Mantoux.
2) It appears that age is the major risk factor for the infection with mycobacterium tuberculosis.
Σκοπός της παρούσης μελέτης ήταν ο υπολογισμός του δείκτη διαμόλυνσης από φυματίωση (θετική Mantoux) και η εκτίμηση των παραγόντων κινδύνου της μετάδοσης του Μυκοβακτηριδίου της Φυματίωσης στους επαγγελματίες υγείας.
Υλικό και Μέθοδος: Το δείγμα της μελέτης αποτέλεσαν 115 άτομα από το νοσηλευτικό και βοηθητικό προσωπικό τριών μεγάλων Νοσοκομείων της Αττικής. Ο έλεγχος της δερμοαντίδρασης στη φυματίνη έγινε με την ενδοδερμική ένεση φυματίνης κατά Mantoux (Tuberculin Skin Test-TST). Παράλληλα με την πραγματοποίηση της δερμοαντίδρασης φυματίνης (Mantoux) συμπληρώθηκε από τους συμμετέχοντες ειδικά σχεδιασμένο ερωτηματολόγιο, που θα προσανατολίζει την πιθανή πηγή μόλυνσης σε περίπτωση θετικής Mantoux ή νόσησης από φυματίωση.
Αποτελέσματα: Από τους 115 συμμετέχοντες, το 4,3% βρέθηκε θετικό στη δερμοαντίδραση Mantoux κατά την 1η εξέταση και δε συμμετείχε περαιτέρω στη μελέτη. Μέσα στο έτος θετικοποιήθηκε το 22% των συμμετεχόντων που ήταν αρνητικοί στην αρχική Mantoux. Τα άτομα που εργάζονταν σε πνευμονολογική κλινική παρουσίαζαν σημαντικά υψηλότερο ποσοστό (30,5%) θετικών Mantoux σε σύγκριση με τα άτομα που εργάζονταν σε παθολογική κλινική (12%). Μεγαλύτερη επίπτωση για την εμφάνιση θετικής Mantoux στο 1 έτος βρέθηκε στα άτομα ηλικίας άνω των 42 ετών σε σύγκριση με τα άτομα κάτω των 32 ετών. (p=0,046%)
Συμπεράσματα: 1) Το τμήμα, στο οποίο εργάζεται το προσωπικό υγείας παίζει σημαντικό ρόλο στη θετικοποίηση της Mantoux.
2) Φαίνεται ότι η ηλικία είναι ο βασικός παράγοντας κινδύνου λοίμωξης από το Μυκοβακτηρίδιο της Φυματίωσης.

▼ Objectives: The determination of the rate of tuberculosis (TB) screening test conversion during anti-tumour necrosis factor (TNF) therapy in rheumatic patients with negative baseline screening.Methods: Prospective study of rheumatic patients with negative baseline TB screening (tuberculin skin test (TST): <5 mm, and negative T-SPOT.TB, QuantiFERON-TB Gold In Tube (QFT-GIT) and chest X-ray) treated with anti-TNF agents. All patients underwent re-screening for TB with all assays 1 year later. Factors associated with TB test conversion were analysed and compared between 'converters' and 'non-converters'. Results: Seventy patients (mean age 50.6±15.5 years) with rheumatic disease (33 with rheumatoid arthritis, 33 with spondyloarthropathies and 4 with other conditions) were enrolled. Patients were treated with different anti-TNFs (27 with adalimumab, 14 etanercept, 16 infliximab, 8 golimumab, 5 certolizumab pegol) for 1 year. Twenty patients (29%) displayed conversion of at least one screening assay 12 months after anti-TNF therapy: conversion of TST occurred in 9 (13%), T-SPOT.TB in 7 (10%) and QFT-GIT in 5 (7%). Only one patient had concomitant conversion of more than one screening test. Univariate and multivariate analysis revealed that only infliximab was associated with a decreased rate of TB screening assay conversion (OR 0.048, 95% CI 0.004 to 0.606, p=0.017). No patient (40% received isoniazid therapy) developed active TB during follow-up (27±12 months). Conclusions: Approximately one third of patients with negative baseline TB screening develop conversion of at least one screening test during anti-TNF treatment. These findings should be considered when designing rescreening strategies and contemplating latent TB therapy.
Σκοπός της παρούσας μελέτης ήταν να μελετηθεί η συχνότητα θετικοποίησης των δοκιμασιών για φυματίωση (ΤΒ) (TST, IGRAs) κατά τη διάρκεια αγωγής με anti-TNF παράγοντες σε ασθενείς με αρνητικό έλεγχο προ της έναρξης αγωγής. Μέθοδοι. Πρόκειται για μια προοπτική μελέτη ασθενών με ρευματικές παθήσεις με αρνητικό αρχικό έλεγχο (δερματική φυματινοαντίδραση (TST <5 mm) και αρνητικές μέθοδοι T-SPOT.TB, Quantiferon-TB Gold In Tube(QFT-GIT) και ακτινογραφία θώρακα) και οι οποιοι υποβλήθηκαν σε αγωγή με anti-TNF παράγοντες. Όλοι οι ασθενείς υποβλήθηκαν σε επαναληπτικό έλεγχο για ΤΒ ένα έτος μετά. Παράγοντες που σχετίζονται με τη μετατροπή των δοκιμασιών της ΤΒ αναλύθηκαν και συγκρίθηκαν μεταξύ εκείνων που «θετικοποίησαν» και εκείνων που «δεν θετικοποίησαν». Αποτελέσματα: 70 ασθενείς (μέση τιμή ηλικίας 50±15,5 έτη) με ρευματολογικές παθήσεις (33 με ρευματοειδή αρθρίτιδα, 33 με σπονδυλοαρθροπάθειες και 4 με άλλες νόσους) συμπεριλήφθηκαν στη μελέτη. Οι ασθενείς έλαβαν αγωγή με διάφορους anti-TNF παράγοντες (27 έλαβαν adalimumab, 14 etanercept, 16 infliximab, 8 golimumab, 5 certolizumab pegol) για ένα έτος. 12 μήνες μετά παρατηρήθηκε σε είκοσι ασθενείς (29%) τροποποίηση (θετικοποίηση) σε μία τουλάχιστον μέθοδο ελέγχου : θετικοποίηση στην TST παρατηρήθηκε σε 9 (13%), T-SPOT.TB σε 7 (10%) και QFT-GIT σε 5 (7%)…

▼ The use of diagnostic tests is central to the practice of modern medicine, but knowing which test to use, and when, can be problematic. To make evidence-based diagnoses, clinicians need efficient ways of accessing diagnostic studies, interpreting the results of several studies, and checking the applicability of studies to their own setting. The aim of this thesis was to explore solutions to these problems by addressing a specific clinical question; What is the best screening test for latent tuberculosis in patients undergoing transplantation? In a study of diagnostic filter performance in MEDLINE, we found that current ‘specific’ clinical queries limit for diagnosis (used in PubMed and Ovid SP) missed up to 80% of studies in nephrology journals. Other filters (Deville 2000 Broad, Deville 2000 Balanced, Haynes 2004 Balanced, and Vincent 2003 Narrow) had similar specificity to the ‘specific’ clinical queries limit, but identified a greater proportion of the total evidence. Using systematic review methodology, we found that current available data was inadequate to determine whether interferon gamma release assays performed better, worse or the same as the tuberculin skin test for diagnosing tuberculosis in candidates for solid organ transplant. Current international guidelines recommend using either the tuberculin skin test or an interferon gamma release assay, or both in combination. Our findings support these guidelines. We conducted a cross-sectional descriptive of candidates for kidney transplantation and found that despite a high prevalence of risk factors among the group, less than a quarter of candidates were screened for latent tuberculosis before transplant, and only 36% of the 101 patients with risk factors for tuberculosis were tested. This study demonstrates that candidates for kidney transplant are at increased risk of tuberculosis and highlights the need for a nation-wide tuberculosis screening protocol in work-up for transplant.

▼ A infecÃÃo por Mycobacterium tuberculosis atinge um terÃo da populaÃÃo da terra, sendo considerada uma emergÃncia mundial. Pesquisas apontam para maior risco entre profissionais e estudantes da Ãrea da saÃde. O estudo buscou identificar a prevalÃncia da infecÃÃo tuberculosa e o conhecimento dos alunos da Ãrea da saÃde sobre o tema. Foi realizada pesquisa do tipo transversal, de natureza quantitativa, com alunos de Medicina e Enfermagem da Universidade Federal do CearÃ, estratificados por ciclo do curso. Os participantes responderam a um questionÃrio sociodemogrÃfico e de conhecimentos sobre tuberculose e foi aplicada prova tuberculÃnica (PT). Foram considerados dois pontos de corte diferentes para PT positiva, ou seja, ≥ 5 mm (utilizado para populaÃÃo geral) e ≥ 10 mm (para profissionais de saÃde). Foram realizados os testes χÂ, χ de tendÃncia e razÃo de verossimilhanÃa para as associaÃÃes entre o resultado da PT, o conhecimento dos alunos e as variÃveis categÃricas. Compararam-se as mÃdias com os testes t de Student e Kruskal-Wallis. Foi considerado estatisticamente significante se p < 0,05. Dos 311 participantes do estudo, 69,5% eram do sexo feminino, 67,5% estavam na faixa etÃria de 20 a 24 anos e 90,7% apresentavam cicatriz de BCG. As caracterÃsticas socioeconÃmicas foram diferentes entre os dois cursos. Apenas 17,4% dos alunos relataram ter participado de treinamento sobre tuberculose. Cursavam o ciclo bÃsico 30,5% dos alunos, o ciclo clÃnico 38,3% e o internato ou estÃgio curricular supervisionado 31,2%. Dos estudantes de Medicina, 38 (21,6%) e 30 (17,0%) apresentaram PT de 5 a 9 mm e ≥ 10 mm, respectivamente. Na Enfermagem, a positividade foi de 5 a 9 mm em 21 (22,8%) e ≥ 10 mm em 26 (28,3%) dos alunos. A anÃlise do resultado da PT por ciclo do curso para a Medicina demonstrou mÃdia e desvio-padrÃo de 3,5 ( 4,2) mm no ciclo bÃsico e de 5,3 ( 4,5) mm no internato (p=0,013). No curso de Enfermagem, a mÃdia e o desvio-padrÃo da PT foram de 5,1 ( 4,8) mm no inÃcio do curso e 5,2 ( 5,4) mm ao fim do curso (p=0,464). O conhecimento dos alunos sobre os temas bÃsicos da tuberculose foi em geral de 56,05%, tendo crescido de 34,13% no ciclo bÃsico para 69,75% no internato/estÃgio curricular supervisionado. O estudo reforÃa a importÃncia da realizaÃÃo de inquÃritos tuberculÃnicos periÃdicos entre estudantes da Ãrea da saÃde, reforÃando a necessidade de implementaÃÃo de medidas de biosseguranÃa e educaÃÃo permanente sobre o tema para futuros profissionais de saÃde.

▼ A inexistência de padrões nacionais para a realização e interpretação da prova de tuberculina em ovinos motivou o presente trabalho, pois apesar da tuberculose não estar incluída em Plano Nacional de Controle e Erradicação em pequenos ruminantes, estabelece a legislação vigente que é obrigatório o sacrifício de animais com essa zoonose, sendo o diagnóstico firmado nesses animais principalmente através do alergoteste tuberculínico. Assim, esta pesquisa visou além de avaliar reações clínicas provocadas pela resposta imuno-alérgica intradérmica à tuberculina, estabelecer valores de referência para a interpretação da reação em ovinos experimentalmente inoculados com antígenos de cepas padrões de Mycobacterium bovis e Mycobacterium avium. Utilizou-se 30 ovinos, clinicamente sadios, negativos à prova de tuberculina cervical comparativa (TCC), segundo critérios estabelecidos pelo Ministério da Agricultura, Pecuária e Abastecimento (MAPA) através do Plano Nacional de Controle e Erradicação da Brucelose e Tuberculose (PNCEBT) em bovinos e bubalinos, distribuídos por três grupos de 10 animais cada, a saber: A - sensibilizados com M. avium; B - sensibilizados com M. bovis e C (Controle) - que receberam injeção de solução fisiológica. Os ovinos foram monitorados através de exames físicos e complementares semanais. Após 45 dias da sensibilização, realizou-se a prova de TCC com reação medida pela variação da espessura da pele com cutímetro de mola, nos seguintes momentos: antes da aplicação das tuberculinas (PPDs); 12h, 24h, 48h, 72h e 96h após a aplicação das mesmas. Em relação à reação ao PPD bovino, no teste cervical simples, lido às 72 horas (± 6 horas) pós-tuberculinização (p. t.), considerou-se uma reação como positiva, quando o aumento da espessura da pele foi igual ou maior que 2,49 mm; como inconclusiva, com as medidas de aumento entre 1,00 e 2,48 mm, e como reação negativa, aumentos da espessura da pele inferiores a 1,00 mm, sendo estes valores propostos como padrão para o teste intradérmico simples na região cervical. A análise dos resultados da leitura do teste cervical comparativo, realizada às 72 horas (± 6 horas) p. t., permitiu concluir-se que o animal poderá ser considerado com resposta: positiva, quando a reação ao PPD bovino superar aquela ao aviário em pelo menos 2,00 mm; inconclusiva, quando a reação ao PPD bovino for maior que aquela ao aviário, com diferença entre 1,00 e 1,99 mm, e negativa quando a reação bovina for menor que a aviária ou > em até 0,99 mm. Na avaliação histológica das respostas tuberculínicas homólogas, 96 horas após a injeção do PPD, constatou-se a presença de moderado a intenso infiltrado inflamatória, constituído, preferencialmente, por células mononucleares.

▼ Foi investigado o valor diagnóstico da resposta alérgica cutânea à tuberculina e do ELISA indireto, com antígeno recombinante MPB 70, em leitões experimentalmente sensibilizados, pela via intramuscular, com suspensões oleosas de M. bovis ou M. avium inativados. Foram utilizados 91 animais divididos em quatro grupos. Os grupos A e B, cada um com 25 indivíduos, grupos C e D com 21 e 20 indivíduos respectivamente, balanceando-se as características de raça, linhagem, faixa etária e sexo. Aos 21 dias de idade, todos os animais foram submetidos a uma triagem com a aplicação de tuberculina PPD de M. bovis, pela via intradérmica na base da orelha e não houve qualquer tipo de reação. Decorridos 60 dias do teste tuberculínico de triagem, o grupo A, recebeu injeção intramuscular de 0,5 mL de uma suspensão oleosa de M. avium estirpe D4; o grupo B, recebeu 0,5 mL de uma suspensão oleosa de M. bovis estirpe AN5; o grupo C (controle I), recebeu 0,5 mL do adjuvante oleoso e o grupo D (controle II), recebeu 0,5 mL de solução fisiológica. Foi realizado o exame histopatológico de biopsias das reações cutâneas e a colheita de sangue para o teste de ELISA de captura. Após 30 dias da sensibilização, foi efetuada a prova de tuberculinização comparativa com reação medida pela variação da espessura da pele com paquímetro às 0h, 24h, 48h e 72h, após a aplicação das tuberculinas. No teste comparativo, lido às 72 horas, a reação foi considerada negativa quando a diferença das reações entre o PPD bovino e o PPD aviário foi menor que 6,7 mm; suspeito ou inconclusivo quando a diferença se situou na faixa de 6,7 a 7,5 mm; e positiva para o tipo de PPD, considerando-se tuberculose para PPD M. bovis e micobacteriose para PPD M. avium, quando a diferença da reação foi superior a 7,5 mm. Nos exames histopatológicos, foi observado intenso infiltrado inflamatório linfocitário no local das reações intradérmicas dos animais testados com o PPD homologo ao tipo de micobactéria utilizada na suspensão oleosa sensibilizante. O ensaio de ELISA com antígeno, MPB 70 recombinante, foi capaz de revelar a presença de anticorpos contra o M. bovis, porém não revelou anticorpos para M. avium.

▼ Introdução- A busca ativa de indivíduos infectados pelo Mycobacterium tuberculosis, com o auxílio do teste tuberculínico, principalmente em populações de detentos, moradores de rua, infectados pelo vírus do HIV entre outras, tem como objetivo prevenir a evolução da infecção para a doença tuberculose e auxiliar o conhecimento de novos casos da doença. O teste tuberculínico é uma importante ferramenta, para identificar os indivíduos reatores (infectados) e não reatores (não infectados). Ele pode ser aplicado a partir de várias técnicas, mas a que permite a padronização e a comparação dos resultados é a técnica intradérmica de Mantoux, a mais usada atualmente. No Brasil, a leitura do resultado do teste tuberculínico é realizada, por recomendação do Ministério de Saúde, com régua milimetrada. Para seu uso é necessário um treinamento rigoroso, o que tem contribuído para a diminuição de leitores do teste tuberculínico na rotina dos serviços de saúde. Como alternativa temos outro instrumento, paquímetro, ainda não padronizado no Brasil. Outros países o utilizam nas rotinas dos serviços de saúde para busca de indivíduos reatores ou não ao teste tuberculínico, ele não cabe para aferição ou inquéritos epidemiológicos. Objetivo- Comparar os instrumentos de leitura dos resultados do teste tuberculínico obtidos através da leitura padronizada, feita por régua milimetrada, e a leitura, não padronizada, feita por paquímetro. Métodos- Estudo descritivo realizado numa população prisional de uma Penitenciária e de um Centro de Detenção Provisória, da cidade de Guarulhos, Grande São Paulo, entre os meses de março a junho de 2008. Foi realizada a leitura do teste tuberculínico de forma dupla cega, por dois leitores, com instrumentos diferentes, paquímetro e régua milimetrada. Resultados- Foram administrados testes tuberculínicos em 1954 indivíduos; destes 111 foram excluídos por não terem comparecido à leitura do teste, resultando em uma amostra de 1843 indivíduos divididos em 1059 (57,5 por cento ) na Penitenciária e 784 (42,5 por cento ) no Centro de Detenção Provisória. Os resultados comparativos entre os dois instrumentos, quando avaliados os reatores com os não reatores, foram altos e equivalentes, pois a sensibilidade (93 por cento ) e a especificidade (97 por cento ) foram praticamente as mesmas. A média da concordância foi alta entre os dois instrumentos (95,5 por cento ) e a discordância foi baixa (4,5 por cento ). O Ministério da Saúde preconiza uma concordância acima de 80 por cento para aferição de novos leitores. Conclusões- Com este estudo observamos que o paquímetro pode ser considerado equivalente à régua milimetrada e poderá ser uma alternativa à régua para a leitura do teste tuberculínico no Brasil, principalmente para a busca de casos de tuberculose latente na rotina dos serviços de saúde, por ser um bom instrumento de separação dos indivíduos e não é suficiente para aferir um novo leitor. ▼ Background: HIV-infected women have increased risk of reactivation of latent TB infection, making screening crucial. There are limited guidelines for latent TB screening in India. There is need to study the two available tests, which are known to be compromised in this population - Tuberculin Skin test (TST) and Quantiferon Gold-in-Tube assay (QGIT). Thus, our aim was to evaluate the performance of these two tests in two facilities – Antenatal Clinic (ANC) and Antiretroviral Therapy Center (ARTC). Methodology: We conducted a cross-sectional study of HIV-infected women seeking care at the ANC and ARTC within a government hospital in Pune, India. Trained staff administered sociodemographic/medical history questionnaires before TST and QGIT testing. The percent positivity, percent agreement between tests results and kappa statistic, were calculated. Multivariate logistic regression was used to study risk factors for TST and QGIT positivity. Finally for each test, the
number needed to screen (NNS) inorder to avert one TB case, was estimated in each facility. Results: Among ANC participants, 15 (11%) were TST+ and 40 (29 %) were QGIT+, with 73.2 % agreement (95% CI = 63.2-81.4, k = 0.214 (0.04 - 0.39)). Among ARTC participants, 22 (15.1 %) were TST+ and 69 (47.2 %) were QGIT+, with 62.1 % agreement (95% CI = 52-71, k=0.22 (0.09-0.34)). TST-/QGIT+ discordance was common in both groups (21% ANC, 34% ARTC). It was observed in the ANC that NNS estimate is 200 for TST alone and 73 for QGIT alone. Similarly in ARTC TST alone has an NNS of 147 compared to 47 for QGIT alone. Conclusions: The QGIT estimated a higher prevalence of LTBI as compared to TST in both facilities, more comparable to the Revised National TB Control Programme estimate of 30-40 %. Further advantage of using QGIT is the less than half of NNS estimate of TST alone in both settings. Thus there is need for development and evaluation of an algorithm with QGIT alone or two test strategy
with both TST and QGIT in these settings, considering the issues with both the tests in this population.

▼ Dados referentes à tuberculose caprina no Brasil e sua importância em saúde pública são escassos. Diante disso, este trabalho teve por objetivos investigar a ocorrência e pesquisar aspectos epidemiológicos da tuberculose em rebanhos leiteiros caprinos na mesorregião do Triângulo Mineiro e Alto Paranaíba, MG. Foram tuberculinizados 233 caprinos provenientes de sete propriedades leiteiras, utilizando-se o teste cervical comparativo, tomando-se como referência os procedimentos e critérios de interpretação de resultados padronizados para esta espécie. Dos animais submetidos ao teste, 1,29% (3/233) reagiram positivamente, 2,14% (5/233) apresentaram reações inconclusivas e 96,57% (225/233) não reagiram. Todos os caprinos positivos e um com resposta inconclusiva foram submetidos à necropsia. Amostras de linfonodos, fígado, pulmão e rim que apresentaram alterações foram colhidos para exame histopatológico (Hematoxilina-Eosina). Material proveniente de abscessos e fragmentos de linfonodos foi submetido à cultura em meio de Stonebrink e Petragnani. Um inquérito epidemiológico foi procedido para análise dos fatores relacionados à ocorrência de tuberculose nas propriedades. Não foram encontradas lesões macroscópicas sugestivas de tuberculose nos animais necropsiados, nem alterações histopatológicas. Também não houve crescimento de micobactérias na cultura. A taxa de reatividade à tuberculina dos caprinos foi de 1,29 %. Por se tratar de rebanhos leiteiros, a ocorrência de tuberculose pode representar risco zoonótico na mesorregião do Triângulo Mineiro e Alto Paranaíba, MG. A interpretação dos registros epidemiológicos, associada aos resultados da tuberculinização constatou reagentes positivos somente na propriedade onde se utilizou leite bovino in natura para alimentar cabritos recém nascidos, porém a ocorrência da tuberculose não pode ser confirmada nesses caprinos.

Therefore, the purpose of this study was to investigate the occurrence and examine epidemiological aspects of tuberculosis in dairy goat herds in the Triângulo Mineiro and Alto Paranaíba mesoregion in the state of Minas Gerais, Brazil. Two hundred and thirty-three goats from seven dairy farms were tuberculinized using the comparative cervical tuberculin test, taking as reference the procedures and interpretation criteria of standard results for this species. Of the animals subjected to the test, 1.29% (3/233) reacted positively, 2.14% (5/233) showed inconclusive reactions, and 96.57% (225/233) showed no reaction. All the goats that tested positive and one showing an inconclusive response were necropsied. Samples of lymph nodes, liver, lung and kidney presenting alterations were collected for a histopathological examination (hematoxylin-eosin). ▼ This study was conducted to confirm the presence of
bovine tuberculosis (BTB) and determine its prevalence, based on
skin test reactivity, in cattle reared under extensive farming
conditions in the Govuro district, Inhambane province, Mozambique.
The study was comprised of a primary screening test using the
single intradermal test (SIT) in randomly selected animals from
Colonato and Sede dip tanks in Govuro. Positive reactors to the SIT
were tested again with bovine and avian
tuberculin using the single
intradermal comparative test (SICTT) 7 weeks after the SIT. The
sample size was calculated using Win Episcope 2.0 based on 95%
confidence to detect a 2% expected prevalence using the SIT, with a
1% accepted error and accounting for a total population size of
7208. The calculated sample size was 682 animals. To compensate for
the probability of 20% default in reading, the sample size was
increased to 853. During the testing process (SIT), it was evident
from the first 3 reading days that the apparent prevalence (61,
94%) was higher than expected (2%), hence we decided to stop when
the total number of cattle was 530. During the testing process

This was due to the fact that, at such a high prevalence, it would
not be necessary to achieve as high a precision as 1% accepted
error. A sample size of 530 would be sufficient to achieve a
precision of 4% accepted error, which was regarded as more than
adequate. The 530 cattle, 3 or more years of age, were selected
using systematic random sampling from the two dip tanks (Colonato
371 and Sede 159 animals). All animals were identified by numbers
painted, dorsally on the sacral region. Out of 530 tested cattle by
SIT, 268 were read, and 166/268 (61.94% with 95% confidence
interval [CI]: 55.8 – 67.8%) were found positive, with visible
swallow at the injection site. Apparent prevalence (AP) was found
to be 61.94% while the true prevalence (TP) was 75.92%. The
predictive value of a positive result (PV+) was found to be 87.9%.
No significant difference in apparent prevalence between the two
areas was detected by Fisher’s exact test (P = 0.11). By SICTT, out
of 28 animals positive reactors to SIT, 21 were possible to read,
and 13/21 (61.9%; 95% CI: 55.1 – 89.3%) were found positive. A
three year old bull, positive reactor to the SIT, was slaughtered,
and a detailed post mortem was carried out and organs with visible
lesions were collected for further laboratory testing
(histopathology, culture and isolation of M. Bovis and PCR). Later
on, 30 more positive reactors to the SIT test were slaughtered:
25/30 (83.3%) showed visible lesions compatible with BTB, and total
condemnation of carcass was made in 3/25 (12%) due to generalized

▼ Tuberculosis is still amongst the most important causes of human morbidity and mortality. The tuberculin skin test is widely used to define the tuberculous infection in the community. Serial representative tuberculin surveys demonstrate the trend of the risk of tuberculin infection. The annual risk of Tuberculosis infection (ARTI) is a key indicator in epidemiology, of the extent of transmission in a community. There have been several suggested methods in order to evaluate the prevalence of Tuberculosis infection using tuberculin skin data. This survey estimates the annual risk of Tuberculosis infection (ARTI) in young hellenic men. The effect of BCG vaccination has also been investigated. Materials and Methods: During the period November 2006-November 2007 tuberculin skin tests were conducted to estimate the prevalence of mycobacterium tuberculosis infection and also to determine the ARTI. We used the Mantoux technique with tuberculin PPD–RT 23 with Tween 80, dose 2 IU was used in 7.492 Greek air force military recruits with an average age of 23.57 years. All recruits were examined for previous bacill Calmette-Guérin vaccination through BCG scar. A vast number of personal, epidemiological significance, data of the participants was collected through a specially designed demographic form which was filled in and concerned information regarding their age, geographical birth place and residence, smoking habits, parental occupation, parental and partecipants’ level of education, recent contact with immigrants from high-incidence countries. The ARTI was calculated based on the equation R=1-(1-P)1/a.(R= annual risk of infection, P= prevalence, a= average age.) Results: The ARTI was 0.2%, in those who were not previously BCG vaccinated, and this was derived from a tuberculin skin test cut-off point of 10 mm . There were not any statistically significant differences, neither between urban and rural population concerning the positivity of the tuberculin skin test, nor among the population in recent contact with immigrants from high–incidence countries. Conclusion: The estimated ARTI among young Greek men is 0.2%. This survey demonstrates further decline in the prevalence of TST positivity in young Hellenic enlistees compared to the outcomes of former studies on this population.

▼ Cell mediated immunity and delayed type hypersensitivity play key roles in the pathogenesis of tubersulosis. Infection with M. Tuberculosis results in delayed type hypersensitivity reaction to antigens derived from the organism, which is the basis of the tuberculin skin test. Tuberculin reaction is a localized skin reaction with typical histological picture, which is the result of the mobilization of delayed hypersensitivity reaction, whenerver mycobacterial products have been injected intradermally. Two types of tuberculin preparation have been in use, old tuberculin and purified protein derivative. Several studies have indicated a considerable variability on the biological results between different tuberculin preparations in different geographical areas or even within a single region. The purpose of this study was to evaluate and compare the biological potency of two different tuberculins, RT23 and Merieux. We performed simultaneous testing with both tuberculins to compare these reagents during a tuberculin survey of 546 Police Academy Students in Athens. The students were separated in three groups and the participants of each group were tested using the mantoux method simultaneously with the two tuberculins in different concentrations. Using a 10mm cut-off point for a positive skin test, the positive reactions with the tuberculins RT23 and Merieux were highly correlated. No statistically significant difference was established. The results indicated significant superiority of the size of the reactions with RT23 tuberculin (p=0,014). It appears that both tuberculins have equivalent potency, with RT23, producing more frequently reactions of larger sizes.
H κυτταρική ανοσία καθώς και η υπερευαισθησία επιβραδυνόμενου τύπου, που αναπτύσσονται μετά την έκθεση στο μυκοβακτηρίδιο της φυματίωσης, έχουν βασικό ρόλο στη παθογένεια της νόσου. Η φυματίνη αποτελεί βιολογικό προϊόν που περιέχει μυκοβακτηριδιακά συστατικά ικανά να διεγείρουν την υπερευαισθησία επιβραδυνόμενου τύπου προς το μυκοβακτηρίδιο ενός ήδη ευαισθητοποιημένου ξενιστή. Οι πρώτες φυματίνες ήταν μη καθαρά προϊόντα που οδηγούσαν σε ψευδώς θετικά αποτελέσματα. Από την περαιτέρω όμως επεξεργασία τους προέκυψαν κεκαθαρμένα παράγωγα απαλλαγμένα από προσμίξεις. Σύμφωνα με διάφορες εργασίες η ίδια φυματίνη μπορεί να έχει διαφορετική αντιγονική διέγερση όταν εφαρμόζεται σε ανόμοιους πληθυσμούς. Για το λόγο αυτό είναι διεθνής πρακτική η χρήση του ιδίου προϊόντος σε ένα πληθυσμό επί μακρόν για πληρέστερη εξοικείωση του ιατρικού προσωπικού. Σκοπός της μελέτης μας ήταν η σύγκριση της βιολογικής ισχύος δύο διαφορετικών φυματινών, των RT23 και Merieux. O rastreio para infecção tuberculosa latente (ILTB) é fortemente recomendado antes de iniciar a terapia com agentes anti-TNF. Os objetivos deste estudo foram identificar a prevalência de ILTB e TB entre pacientes com doenças reumáticas em uso dos medicamentos anti-TNF. Métodos: Estudo transversal. Foram revisados os registros médicos eletrónicos de todos os doentes adultos (≥ 18 anos) em uso da terapia anti-TNF. Todos os pacientes foram submetidos ao teste tuberculínico (TT) antes de iniciar o tratamento com os medicamentos anti-TNF. Resultados: No total, 176 pacientes foram incluídos no estudo. A idade média de todos os pacientes foi de 51,9 ± 12,4 anos, 34,7% eram do sexo masculino e 90,9% eram brancos. As doenças subjacentes mais comuns foram: Artite Reumatóide (AR) em 89 pacientes (50,6%), Espondilite Anquilosante (EA) em 49 (27,8%) e Artrite Psoriática (AP) em 31 (17,6%). A prevalência de TT positivo foi de 29,5%. O contato domiciliar com TB foi significativamente associado com TT positivo (p = 0,020). Os pacientes com AR apresentaram reações TT menores do que os pacientes com EA (p = 0,022). Houve seis casos de TB (3,4%) diagnosticados durante a terapia anti-TNF. Conclusões: Demonstrou-se alta prevalência de TT positivo (29,5%) em pacientes com doenças reumáticas em uma região com alta prevalência de TB. Nossos dados corroboram a recomendação do Colégio Americano de Reumatologia (ACR) de que os pacientes que vivem em configurações de alta incidência de TB devem ser testados anualmente para ILTB.
Background: The introduction of biological agents, especially the blockers of tumor necrosis factor (anti-TNF), for the treatment of rheumatic diseases increased the risk of developing tuberculosis (TB). Screening for latent TB infection (LTBI|) is strongly recommended before starting therapy with anti-TNF agents. The objectives of this study were to identify the prevalence of LTBI and TB among patients with rheumatic diseases on anti-TNF drugs. Methods: Cross-sectional study. The electronic medical records of all adult patients (≥ 18 years old) undergoing anti-TNF treatment were reviewed. Every patient underwent TST test before starting anti-TNF treatment. Results: In total, 176 patients were included in the study. The mean age of all patients was 51.9 ± 12.4 years, 34.7% were males, and 90.9% were white. The most common underlying diseases were: RA in 89 patients (50.6%), AS in 49 (27.8%), and PA in 31 (17.6%). The prevalence of positive TST was 29.5%. Household contact with TB was significantly associated with a positive TST (p=0.020). RA patients had lower TST reactions than AS patients (p=0.022).
