8. Tillock, Katrina. Dementia and families: relational closeness, psychological wellbeing and navigating the medical model of care.

Degree: wives, adult daughters and adult grandchildren) of four men and one woman with dementia. Age of primary caregivers ranged from 53 to 80, persons with dementia age ranged from 70 to 84 and the average length of time caring was 5.3 years. This study sought subjective interpretations of a unique and poorly explored phenomenon, drawing from Interpretative Phenomenological Analysis (IPA) of semi-structured interviews. Methodologically, IPA is philosophically underpinned by phenomenology, hermeneutics and symbolic interactionism and provides the platform for an object or event, to be subjectively explored through a first person account and to be iteratively interpreted by the researcher and participant. IPA allows for psychological positioning that aims to understand the conscious experience of a person’s interaction with their world. Inter-subjective analysis occurs through acknowledgment of the interplay between the existence of a phenomena outside of our mind (objective) and the individual cognitions and concepts of reality (subjective). Furthermore symbolic interactionism epistemologically unites with IPA as it acknowledges that to understand a person’s lived experience, an inquiry into their cultural position, is necessary. With IPA the participant is the expert assisting the researcher-practitioner to acquire insightful knowledge of the participant’s experience. Specifically, in this case it is the experience of the medical model of dementia care from a family member’s perspective. Results: <i>Navigating ‘system’ control</i> was discovered as the superordinate theme with three overarching sub-themes: <i>Connecting/Disconnecting, Windows Closing</i> and <i>Capacity for Sensation</i>. Interpretative exploration finds the transition of families on the dementia journey as initial primary caretakers who become passengers embarking on a system perceived as oscillating between service orientation and a ‘<i>warden</i>’ mentality. Themes represent the confusion and emotional challenges with which would-be advocates and often guilty family members struggle to make sense of a large systemic medical environment. Many parallel paths challenge participants’ ability to adapt to the speeding ‘<i>roller coaster ride</i>’ of dementia. On one hand changing and often convoluted communication patterns part of dementia, tests their relational connectedness. On the other hand, remaining connected necessitates a commitment to this system, despite distress of acquiescence hierarchy. Unexpectedly there are opportunities for growth.
