Purpose: Many diseases are very complex and this is especially true with temporomandibular disorders (TMDs). The lack of consensus on the aetiology, signs and symptoms and treatments available makes it extremely difficult for health care practitioners to treat TMDs. A clear definition of the condition itself has not been agreed on between many disciplines involved in the diagnoses and treatment of TMDs. Two competing definitions stand out in current literature. The physical medicine and dental professions define TMDs as various musculoskeletal disorders of the masticatory system. Some authors also stated that most patients with TMDs suffer primarily from a muscular disorder caused by masticatory myofascial trigger points (TrPs). The chiropractic profession defines TMDs as a group of somatic disorders that occur around the structurally integrated craniomandibular system but primarily affects the temporomandibular joint (TMJ) system. The term TMDs recognises that this craniomandibular pathology is a result of dysfunction in any structural area of the head or neck, not only the TMJ. This purpose of this study was to compare an adjustment treatment approach versus a soft tissue approach for the treatment of TMDs. Method: This study consisted of two groups; both the experimental and control groups consisted of 15 participants with TMDs. The participants were between the ages of 18 and 40 years. Potential participants were examined and accepted based on the inclusion and exclusion criteria. The experimental group received adjustments delivered to the TMJ and upper cervical spine, the control group received therapeutic ultrasound (U/S) to the xiv muscles of mastication. Objective and subjective findings were based on the treatments. Procedure: Each participant agreed to 6 treatment sessions and a seventh follow up session over a 4 week period. The participant completed a TMDs Disability Questionnaire. Algometer readings were taken over the masseter and temporalis TrPs bilaterally. Calliper readings measured the range of motion (ROM) of the TMJ, i.e. mandibular depression, protrusion and right and left deviation. The experimental group received adjustments to restricted segments of the TMJ and upper cervical spine as determined by motion palpation. The control group received therapeutic U/S to the masseter and temporalis muscles bilaterally. The same treatment procedures were administered over the study; subjective and objective readings were only taken again at visit 4 and 7. Results: In terms of subjective measurements based on the TMDs Disability Questionnaire scores, a statistically significant difference in the disability, severity and frequency of symptoms within the experimental and control groups individually, as well as the usual symptom intensity level within the control group was revealed. ▼ Background: Therapeutic exercises have been used by Physiotherapists to manage musculoskeletal
disorders, specifically
Temporomandibular Disorders (TMD) for some time but they are less commonly used in dental practice. The current evidence about home exercise programs to treat TMD is weak despite some generally recognised benefits such as low cost, reversibility and reinforcement of the patient’s coping skill, essential for management of chronic conditions like TMDs. Objective: To evaluate the effectiveness of home exercise programs used as part of dental management for TMDs. Methods: A search of the Cochrane Central Register of Controlled Trials (Cochrane Library Issue 7, 2012), Medline, Web of Knowledge and Scopus databases (January 1966 to February 2012) and reference lists of articles. Only randomized and quasi-randomised controlled trials published in English that compare exercise therapy to treatments without exercise therapy for TMDs were included. Two authors independently assessed the suitability of trials for final inclusion and also contacted study authors for additional information as required. Results: Eleven trials involving 688 people were included. Two trials compared exercise programs to no-treatment control subjects for Anterior Disc-Displacement with Reduction (ADDwR). A significant risk ratio of 0.44 (0.29-0.59) (44% improvement rate) for improvement in clicking after at least four weeks of exercise therapy was revealed. Three trials compared exercise programs for Anterior Disc-Displacement without Reduction (ADDwoR) with no-treatment control subjects. An overall improvement in the pain-free range of maximal mouth opening with exercise intervention was observed although the differences were not statistically significant in one of three trials. Six trials compared exercise programs for Myofascial Pain patients with no-treatment controls. A standardised mean difference in jaw pain score of 0.73 (95% CI -0.63 – 2.10) indicates no effect from exercise therapy for this TMD subgroup. Conclusion: Exercise therapy appears to be effective for treatment of some TMD subcategories and unlikely to present any adverse outcomes.

▼ Aim: To compare osseous changes in the mandibular condyles in patients presenting to the Oral Health Center, Tygerburg Campus, with and without clicking of the
temporomandibular joint. Background: Clicking of the
temporomandibular joint (TMJ) is not a normal occurrence in its form or function. A chronic click may lead to the development of osteoarthritis. A clinical finding of clicking of the joint can reflect osseous changes of the bony structures and form part of the early signs of degenerative joint diseases. These osseous changes can be detected on Cone Beam Computed Tomography (CBCT) images. The purpose of this study is to confirm the presence osseous changes of the joint and institute the early management of these patients. Failure to intervene in the early stages could result in disease progression to possible osteoarthritis. CBCT imaging will be used to assess osseous changes in the
temporomandibular joints with reference to erosions, flattening, lipping, sclerosis and osteophyte formation. Materials and methods: 25 patient records were selected for a control group and 25 patients that attended the Oral Health center were screened for asymptomatic clicking of the
temporomandibular joints. Osseous changes of the 100 condyles were examined by the author and a senior member of the department. Changes were recorded when consensus was reached on the presence of any changes. Cone Beam Computed Tomography was used to assess the joints in both groups. Results: Age and gender showed no statistical significance between the 2 groups. The proportion of ‘yes’ for the variables showed that sclerosis (right) was statistically significant when comparing case versus control groups (P = 0.002). A chi-squared test applied to the data resulted in observed chi-square = 15.68, df = 1, p-value = 7.501e-05, (<0.0001) confirming that the discrimination is statistically significant. Conclusion: Osseous changes were found in both the control and case group. The case group exhibited equal or greater prevalence of osseous changes. Patients with asymptomatic click of the
temporomandibular joints demonstrate osseous changes.

▼ The epidemiology of
temporomandibular disorders varies widely in the literature. The aim of this study was to determine the prevalence of TMD in dental students of the Federal University of Rio Grande do Norte assessed by different indexes. The sample consisted of 101 individuals selected by a randomized process, whose general outline was systematic sampling. For evaluation of the signs and symptoms of TMD, an anamnestic index, Fonseca s protocol, and two clinical indexes, the RDC/TMD (Research Diagnostic Criteria for
Temporomandibular Disorders), or standard index, and the Helkimo s Clinical Dysfunction Index were applied. Data were analyzed using the chi-square test and kappa, besides verifying the sensitivity and specificity (5% significance). The diagnosis of TMD by different indexes showed a variation in the prevalence between 72.3% (Helkimo s Clinical index), 64.4% (Fonseca s anamnestic index) and 35.6% (RDC/TMD). There was no statistical difference between the sexes for the RDC/TMD, although this difference was found for Fonseca s and Helkimo s indexes (p<0.05). The most frequent type of TMD were joint
disorders (Groups II and III), and the subtypes disc displacement with reduction (17.8%) and arthralgia (15.8%). Most individuals showed a mild TMD (45.5%) for both indexes, Fonseca and Helkimo. When comparing the types of diagnoses, RDC/TMD with Fonseca and Helkimo, low agreement was found (k=0.17 and k= 0.35, respectively). A moderate correlation between the severity of TMD was obtained (kw= 0.53) for Fonseca s protocol and Helkimo s index. High sensitivity and low specificity were seen for both diagnoses compared to standard, resulting in excessive false positives. Within the limitations of the study, it was concluded that the prevalence of TMD can vary widely, depending on the index used for its diagnosis

▼ A síndrome de Moebius (SM) é pouco freqüente e caracterizada por paralisia total ou parcial de certos pares cranianos, obrigatoriamente VI e VII, podendo associar-se a outras anormalidades. Clinicamente ocorre falta de expressão facial, hipoplasia da língua, micrognatia, lábio curto e maloclusão. Disfunção temporomandibular (DTM) não tem sido estudada em pacientes com SM. O objetivo deste estudo é conhecer a prevalência de DTM em pacientes com SM e comparar a incidência e tipo de DTM com um grupo controle. Para tanto, foi realizado um estudo clínico caso controle observacional randomizado, no Centro de Atendimento a Pacientes Especiais (CAPE) da Faculdade de Odontologia da Universidade de São Paulo (FOUSP), entre julho de 2007 e dezembro de 2009, que avaliou 101 pacientes divididos em dois grupos: Grupo de estudo (GE) composto por 36 pacientes com SM, sendo 16 do gênero masculino e 20 do gênero feminino, com idade média de 10,2 anos, variando de 3 a 18 anos de idade; e grupo controle (GC) composto por 65 pacientes sem SM e sem nenhuma deformidade crânio facial, sendo 36 do gênero masculino e 29 do gênero feminino, com idade média de 10,7 anos, variando entre 3 e 18 anos. Nossos resultados mostraram que dos 36 pacientes do GE, 20 tinham DTM ou sinal de DTM (bruxismo). Desses 20 pacientes com DTM, 15 realizaram tomografias computadorizadas de ATM (TCs). Dos 65 pacientes do GC, 8 tinham DTM ou sinal de DTM (bruxismo) e 6 concordaram em realizar as TCs. Foram obtidas imagens volumétricas multiplanares e 3D. As alterações morfológicas indicando processo degenerativo encontrado nos exames de imagem encontradas nos dois grupos foram semelhantes. Concluimos que DTM é mais freqüente em pacientes com SM e que o diagnóstico em idade tenra pode representar uma oportunidade de prevenção e tratamento precoce da doença evitando seqüelas futuras. Por esta razão a avaliação da ATM deve ser incluída no exame clínico odontológico rotineiro de pacientes com a SM.
Moebius syndrome (MS) is rare and characterized by partial or total paralysis of some cranial nerves, unavoidably VI and VII, and may be associated with other abnormalities. The clinical aspects include lack of facial expression, hypoplasia of the tongue, micrognathia, short lip and malocclusion. Temporomandibular Joint disorders (TMJ) has not been studied in patients with MS. The aim of this study is to assess the prevalence of TJD in patients with MS and to compare the incidence and type of TMJ with a control group. There was no statistical difference between the sexes for the RDC/TMD, although this difference was found for Fonseca s and Helkimo s indexes (p<0.05). The most frequent type of TMD were joint
disorders (Groups II and III), and the subtypes disc displacement with reduction (17.8%) and arthralgia (15.8%). Most individuals showed a mild TMD (45.5%) for both indexes, Fonseca and Helkimo. When comparing the types of diagnoses, RDC/TMD with Fonseca and Helkimo, low agreement was found (k=0.17 and k= 0.35, respectively). A moderate correlation between the severity of TMD was obtained (kw= 0.53) for Fonseca s protocol and Helkimo s index. High sensitivity and low specificity were seen for both diagnoses compared to standard, resulting in excessive false positives. Within the limitations of the study, it was concluded that the prevalence of TMD can vary widely, depending on the index used for its diagnosis

Na literatura é demonstrada a existência de uma relação entre qualidade do sono e a presença de disfunções temporomandibulares (DTMs), sendo que o aumento do grau de disfunção implica um decréscimo da qualidade de sono ou vice-versa. As condições musculares são as mais comuns em pacientes com má qualidade de sono.
O objectivo deste estudo foi avaliar a qualidade do sono em estudantes universitários e a sua relação com DTMs musculares.
Foi realizada uma revisão bibliográfica narrativa do tema com as palavras-chave: “sleep medicine”; “sleep mecanisms”; “neurophysiology of sleep”; “sleep disorders”; “sleep disorders AND students”; “temporomandibular disorders”; “myogenous temporomandibular disorders”; “temporomandibular disorders AND students”; “sleep disorders AND temporomandibular disorders”. Realizou-se, posteriormente, uma investigação científica, numa amostra de 100 estudantes da Universidade Fernando Pessoa, utilizando-se um questionário, o Pittsburgh Sleep Quality Index (PSQI), e realizando-se o exame clínico, segundo a versão portuguesa dos Research Diagnostic Criteria for Temporomandibular Disorders (Critérios de Diagnóstico para a Pesquisa de Disfunções Temporomandibulares-CDP/DTM) para a realização do diagnóstico das DTMs musculares.
No estudo realizado, verificou-se que 29% dos estudantes universitários apresentaram má qualidade de sono, sendo o género feminino o mais afectado. Do total de estudantes com má qualidade de sono, apenas 27,6% classificaram a qualidade do seu sono como moderadamente má ou muito má. Verificou-se uma relação estatísticamente significativa entre a má qualidade de sono e a presença de contracções musculares durante o sono. 25% destes casos manifestaram dor miofascial com limitação da abertura. A DTM muscular esteve presente em 19% da amostra, sendo que o subtipo mais comum foi a dor miofascial com limitação da abertura. 73,2% dos estudantes que não relataram dor facial apresentaram boa qualidade de sono, assim como 70,8% daqueles sem DTM muscular apresentaram, também, boa qualidade de sono. Dos estudantes com DTM muscular, 20% tinham má qualidade de sono. A associação da qualidade do sono com DTM muscular não foi verificada neste estudo, pelo que é fundamental investigações futuras sobre o tema com a utilização de critérios mais adequados quer para a avaliação da qualidade do sono quer das DTMs. In the literature it is shown that there is a relationship between quality of sleep and the presence of temporomandibular disorders (TMD), and the increasing degree of impairment involves a decline in sleep quality or vice versa. ▼ O objetivo deste estudo foi determinar prevalência de Disfunção Temporomandibular (DTM) entre alunos da Universidade Federal de Uberlândia, e ainda correlacionar tal prevalência com variáveis oclusais, hábitos parafuncionais, e tratamento ortodôntico prévio. Obteve-se amostra de 201 universitários após aplicação dos critérios de inclusão e exclusão. Foi realizado preenchimento do critério diagnóstico denominado Research Diagnostic Criteria (RDC), que possibilitou classificar os universitários em dois grupos: portadores de dor miofascial (G-DM) e/ou portadores de deslocamento de disco (G-DD). Durante análise oclusal, considerou-se alterações morfológicas: overjet e overbite acentuados, mordida cruzada e mordida aberta, enquanto discrepância entre posição de relação cêntrica (RC) e máxima intercuspidação habitual (MIH), interferências latero e mesiotrusivas, interferência durante protrusão e limitação de abertura bucal, foram considerados distúrbios funcionais. Bruxismo, apertamento dental, e realização de tratamento ortodôntico prévio foram considerados apenas pelo auto-relato de cada participante. Testes X2 e análise de regressão logística foram utilizados para tratamento estatístico dos dados. Dor miofascial estava presente em 18,4% dos universitários, e 12,4% foram classificados para o grupo G-DD. Apertamento dental foi o único hábito parafuncional estatisticamente associado com DTM, por ambos os testes estatísticos (p=,000; OR= 4,236). Dentre as variáveis oclusais, somente overjet demonstrou correlação com o Grupo G-DM. Nenhuma associação entre alteração funcional e DTM foi observada. Análise de regressão desenvolvida para o grupo G-DD, possibilitou encontrar associação entre tratamento ortodôntico e deslocamento de disco com redução (p=,008; OR= 15,477). Concluiu-se que prevalência de DTM na população universitária foi baixa, e que Overjet e apertamento dental estiveram correlacionados com G-DM. Tratamento ortodôntico prévio demonstrou associação significativa apenas com o grupo G-DD.
The aim of this study was to evaluate the prevalence of Temporomandibular Disorders (TMD) between students of Federal University of Uberlandia, and correlate this prevalence with occlusal variables, functional habits, and previous orthodontic treatment. After application of the inclusion and exclusion criteria, 201 undergraduates were selected. ▼ O diagnóstico clínico de desordens temporomandibulares é bastante complexo e, muitas vezes são necessários exames de imagem para complementar a investigação. A Imagem por Ressonância Magnética (IRM) é descrita como padrão-ouro na avaliação dos tecidos moles da articulação temporomandibular (ATM), entretanto a literatura mostra resultados conflitantes quanto à reprodutibilidade do método. Isso se deve a fatores relacionados à qualidade da imagem, como a força do campo magnético, e às condições do examinador. O objetivo do estudo foi avaliar a reprodutibilidade de diagnósticos da ATM por IRM em unidades de 0,5 e 1,5T. Exames de 212 pacientes (134 de 0,5T e 78 de 1,5T) foram avaliados por um examinador treinado e cegado. Nove condições foram avaliadas quanto à presença ou ausência de alteração. A concordância dos diagnósticos foi maior que 80% para ambas as unidades e não houve diferença significativa entre elas (P > 0,05). Alta reprodutibilidade foi obtida para deslocamento anterior de disco sem redução (κ = 0,82 para 0,5T e 0,80 para 1,5T), hipermobilidade (κ = 0,84 e 0,90) e hipomobilidade (κ = 0,80 e 0,95), enquanto valores apenas regulares foram alcançados para deslocamento anterior de disco com redução (κ = 0,48 e 0,42) e alteração de forma do disco (κ = 0,45 e 0,37). De maneira geral, os diagnósticos por IRM em 0,5 e 1,5T apresentaram alta concordância. Entretanto, a menor reprodutibilidade para deslocamento anterior de disco com redução e alteração de forma do disco evidencia a dificuldade do diagnóstico dessas condições.
Clinical diagnosis of temporomandibular disorders is complex making the professional often refer to imaging for complementary research. Magnetic Resonance Imaging (MRI) is described as the gold standard in the evaluation of the temporomandibular joint (TMJ) soft tissues, however the literature shows conflicting results regarding the reproducibility of the method. This is due to factors related to image quality, as the field strength, and the conditions of the rater. The aim this study was to assess the reproducibility of TMJ diagnoses at 0.5 and 1.5T MRI. Examinations of 212 patients (134 of 0.5T and 78 of 1.5T) were evaluated by a trained and blinded rater. Nine conditions were evaluated for the presence or absence of change. The diagnostics agreement was greater than 80% for both units and there was no significant difference between them (P > 0.05). High reproducibility was obtained for anterior disc displacement without reduction (κ = 0.82 at 0.5T and 0.80 at 1.5T), hypermobility (κ = 0.84 and 0.90) and hypomobility (κ = 0.80 and 0.95), while only regular values were achieved for anterior disc displacement with reduction (κ = 0.48 and 0.42) and disc deformity (κ = 0.45 and 0.37). MRI at 0.5 and 1.5T showed high agreement. However, the lower reproducibility for anterior disc displacement with reduction and disc deformity highlights the difficulty in diagnosing these conditions.

▼ The aim of this controlled trial was to evaluate the effectiveness of counseling in pain, function and well-fare outcomes on the management of patients with
temporomandibular disorder (TMD). Therefore, 51 consecutive patients were allocated to one of the research groups. In Group I, was instituted counseling therapy for Group II was conducted treatment as usual with occlusal splint. Patients were followed for returns at 7, 15, 30 and 60 days after baseline. At baseline, all patients were examined and assessed RDC/TMD form, which was administered by a single trained and calibrated examiner, in addition, the patients were referred for specific treatment according to the group to which belonged. The clinical and functional impairment was assessed at each visit through the
Temporomandibular Index (TMI). In each session, the patients were also surveyed about pain intensity using a Visual Analogue Scale (VAS). To analyze the impact of pain on quality of life, OHIP-14 questionnaire was used. The results showed 26 patients in Group I with a mean age of 35.15 ± 10.79 years. 25 patients were allocated to Group II. The mean age was 27.36 ± 10.34 years. The counseling was effective in reducing the intensity of pain (VAS), with significant improvement observed at 7 day follow-up (p <0.001). The functional impairment (TMI) showed significant results at 15 days follow-up (p = 0.002). Counseling was also responsible for significant improvement in the impact of TMD on quality of life (OHIP-14) at all times of the analysis (p <0.001). When comparing research groups, no significant difference was observed for any of the analyzed indices (p> 0.05) nor in the short term (7 days) neither in long term (60 days). It was concluded therefore that, for the studied sample, counseling consisted in an effective treatment option for the control of signs and symptoms of TMD, with results in the short and long term similar to the usual treatment group.

▼ This study had as its aim evaluate the incidence of signs and symptoms of temporomandibular disorders (TMD) in children and adolescents during the growth, with Angle Class II division 1 malocclusion. The sample comprised 56 subjects (males and females), randomly divided into treated group (28 patients) and control group (28 patients), being the first one submitted to an orthopedic treatment for mandibular advancement (for 18 months) with the Fränkel functional regulator 2 (RF-2). The patients from both groups answered to an anamnestic questionnaire and were examined to detect TMD signs, according to discipline of Occlusion(UMESP). Both procedures were done at the beginning of the research and at the end of the 18 months. The data were analyzed by he chi-square proportion test, and the results showed that, in the treated group, there was an increase of the proportions finded at the beginning to headaches, temporomandibular joint tenderness, joint sounds and dental pressing, pointing higher frequency of patients with answers no to TMD signs in final evaluation. In the control group, there was a higher proportion of individuals without headaches by the end of the afternoon, TMJ tenderness and clicks from TMJ (in 2 situations).In treated group there was not increase of the initial proportions any sign or symptom evaluated; however, in control group there was worst of the initial situation to 3 of the symptoms evaluated. The most frequent signs/symptoms of TMD were headaches, headaches by the end of the afternoon, joint sounds and limitation of jaw opening. There was no statistical difference between genders (p>0,05), when the 19 variables (signs/symptoms) were analyzed. This investigation has shown that signs and symptoms of TMD can occur in children and adolescents, and Fränkel therapy, at the same time, corrected the occlusal Classe II relationship in the treated patients and promoted an improvement in signs/symptoms of TMD, although there were no significant differences.
Este estudo teve como objetivo avaliar a incidência de sinais e sintomas de disfunção temporomandibular (DTM) em pacientes jovens em crescimento, com maloclusão de Classe II divisão 1 de Angle. A amostra consistiu de 56 pacientes de ambos os gêneros, dividida aleatoriamente em grupo tratado (28 pacientes) e grupo controle (28 pacientes), sendo o primeiro submetido a tratamento ortopédico de avanço mandibular (por 18 meses) com o regulador de função de Fränkel 2 (RF -2 ). Os pacientes dos dois grupos responderam a um questionário com perguntas sobre disfunção e foram examinados para se detectarem indícios de DTM, segundo metodologia utilizada na disciplina de Oclusão da Universidade Metodista de São Paulo, sendo esses dois procedimentos realizados no período inicial da pesquisa e ao término dos 18 meses. ▼ Este estudo teve como objetivo avaliar a incidência de sinais e sintomas de disfunção temporomandibular (DTM) em pacientes jovens em crescimento, com maloclusão de Classe II divisão 1 de Angle. A amostra consistiu de 56 pacientes de ambos os gêneros, dividida aleatoriamente em grupo tratado (28 pacientes) e grupo controle (28 pacientes), sendo o primeiro submetido a tratamento ortopédico de avanço mandibular (por 18 meses) com o regulador de função de Fränkel 2 (RF -2 ). Os pacientes dos dois grupos responderam a um questionário com perguntas sobre disfunção e foram examinados para se detectarem indícios de DTM, segundo metodologia utilizada na disciplina de Oclusão da Universidade Metodista de São Paulo, sendo esses dois procedimentos realizados no período inicial da pesquisa e ao término dos 18 meses. Os dados foram analisados pelo teste do qui-quadrado de proporções, e os resultados mostraram que, no grupo tratado, houve um aumento das proporções encontradas inicialmente para as dores de cabeça, dores na articulação temporomandibular (ATM), estalos na ATM e apertamento dentário, indicando uma freqüência maior de pacientes com respostas do tipo não para indícios de DTM no tempo final. No grupo controle, houve uma proporção maior de indivíduos sem dores de cabeça no final da tarde, dores na ATM e estalos na ATM (em 2 situações). No grupo tratado não houve aumento das proporções iniciais para nenhum sinal ou sintoma estudado; porém, no grupo controle houve uma piora do quadro clínico inicial para 3 dos sintomas valiados.Os sinais/sintomas mais freqüentes de DTM foram dor de cabeça, dor de cabeça no final da tarde, estalos na ATM e restrição na abertura mandibular. Não houve diferenças significativas para o gênero, quando avaliadas as 19 variáveis (sinais/sintomas de disfunção) com p>0,05. Esta pesquisa permitiu concluir que sinais e sintomas de DTM podem estar presentes em crianças e adolescentes, e o aparelho de Fränkel, ao mesmo tempo em que corrigiu a maloclusão de Classe II nos pacientes tratados, levou a uma tendência na melhora dos sinais e sintomas de DTM, mas sem significância estatística.

▼ O objetivo deste estudo foi avaliar a atividade elétrica dos músculos da mímica de pacientes com dor muscular mastigatória utilizando eletromiografia de superfície. Foram avaliadas 15 mulheres com dor muscular mastigatória (idade média de 33.4, variando de 22-44), grupo de estudo (GMM) e 21 mulheres (idade média de 28.5, variando de 21-45), do grupo controle (GC) Foram realizadas avaliação clínica fonoaudiológica e eletromiográfica (eletromiógrafo ADS 1200, da Lynx Tecnologia Eletrônica Ltda, eletrodos de disco de prata, descartáveis, gel condutor, auto-adesivos com 10mm de diâmetro por HAL Ind. e Com. LTDA) dos músculos: orbicular dos lábios, bucinador, supra-hióideos e masseter em repouso e nos movimentos: protrusão dos lábios, apertamento dos lábios, inflar bochechas, sorriso fechado e sorriso aberto. Houve diferença significativa entre os grupos: os valores da eletromiografia de repouso foram inferiores para o músculo masseter direito (p=0.012) e esquerdo (p=0.019); nos movimentos, os valores eletromiográficos foram superiores no grupo controle durante os movimentos: protrusão dos lábios bucinador esquerdo (p=0.005); suprahióideo esquerdo (p=0.011); apertamento dos lábios bucinador esquerdo (p=0.005); bucinador direito (p=0.007); supra-hióideo esquerdo (p=0.046); supra-hióideo direito (p=0.039); orbicular superior (p=0.042); inflar as bochechas masseter esquerdo (p=0.021); bucinador esquerdo (p=0.007); bucinador direito (p=0.002); orbicular superior (p=0.039); sorriso fechado masseter esquerdo (p=0.004); masseter direito (p=0.019); bucinador esquerdo (p=0.013); supra-hióideo esquerdo (p=0.046) e no sorriso aberto masseter esquerdo (p=0.030). Desta forma, o músculo de maior atividade eletromiográfica foi o orbicular da boca. The objective this study is assess the electric activity of mimic muscles in patients with masticatory muscle pain using surface electromyography (EMG). Was evaluated 15 women with masticatory muscle pain (mean age 33.4, ranging between 22 44 years old), study group (GMM) and 21 women (mean age 28.5, ranging between 21 45 years old), control group (CG). Clinical phonoaudiological and electromyographic assessments were performed (electromyograph ADS 1200 of Lynx Tecnologia Eletrônica Ltda, 10mm disposable pregelled self-adhesive silver disk electrodes of HAL Ind. e Com. LTDA) in the muscles: orbicularis oris, buccinator, suprahyoid and masseter, both at rest and in movement: lip protrusion, lip contraction, cheek inflation, close smile and open smile. ▼ A disfunção temporomandibular (DTM) engloba as alterações relacionadas com as articulações temporomandibulares (ATMs), a musculatura mastigatória e demais estruturas do sistema estomatognático apresentando-se na forma de dores nos músculos da mastigação, ATMs, músculos cervicais, reduções e desvios nos movimentos da mandíbula, sons articulares e padrões de oclusão inadequados. É de relevante importância estudar a musculatura mastigatória por causa da grande influência que a mesma tem no sistema estomatognático. O objetivo deste estudo foi analisar a espessura do músculo masseter e temporal, bilateralmente, por meio da ultrassonografia em indivíduos portadores de DTM muscular, com a finalidade de definir o padrão de espessura muscular nesta disfunção. Os resultados deste estudo permitiram a avaliação do sistema estomatognático do ponto de vista funcional em indivíduos portadores de DTM muscular e poderão nortear o cirurgião dentista na elaboração de diagnósticos, prognósticos e planos de tratamento, com ganho de qualidade de vida para seus pacientes. Portanto conclui-se que na condição de repouso como no apertamento em máxima intercuspidação habitual houve uma diminuição da espessura do músculo masseter esquerdo, temporal direito e esquerdo para o grupo com DTM muscular. E com relação ao gênero houve uma diminuição da espessura muscular, tanto para o músculo masseter quanto para o músculo temporal, nas mulheres.
The temporomandibular disorders (TMD) includes changes related to the temporomandibular joints (TMJ), masticatory muscles and other structures of the stomatognathic system presents itself as pain in the masticatory muscles, joints, cervical muscles, cuts and deviations in the movements of jaw joint sounds and patterns of inadequate occlusion. It is of major importance to study the masticatory muscles because of the great influence which it has in the stomatognathic system. The objective of this study was to analyze the thickness of the masseter and temporal bilaterally by ultrasonography in patients with TMD muscle, in order to set the standard for this thick muscle dysfunction. The results of this study will allow assessment of the stomatognathic system\'s functional point of view in individuals with TMD muscle and can guide the dentist in making diagnoses, prognoses and treatment plans, with gains in quality of life for their patients. Therefore it is concluded that at rest as in clenching in maximum intercuspation was a thinning of the left masseter, temporalis left and right for the group with muscular TMD. And with respect to gender there was a decrease in muscle thickness for both the masseter and temporalis muscle for in women.

In Group I, was instituted counseling therapy for Group II was conducted treatment as usual with occlusal splint. Patients were followed for returns at 7, 15, 30 and 60 days after baseline. At baseline, all patients were examined and assessed RDC/TMD form, which was administered by a single trained and calibrated examiner, in addition, the patients were referred for specific treatment according to the group to which belonged. The clinical and functional impairment was assessed at each visit through the
Temporomandibular Index (TMI). In each session, the patients were also surveyed about pain intensity using a Visual Analogue Scale (VAS). To analyze the impact of pain on quality of life, OHIP-14 questionnaire was used. The results showed 26 patients in Group I with a mean age of 35.15 ± 10.79 years. 25 patients were allocated to Group II. The mean age was 27.36 ± 10.34 years. The counseling was effective in reducing the intensity of pain (VAS), with significant improvement observed at 7 day follow-up (p <0.001). The functional impairment (TMI) showed significant results at 15 days follow-up (p = 0.002). Counseling was also responsible for significant improvement in the impact of TMD on quality of life (OHIP-14) at all times of the analysis (p <0.001). When comparing research groups, no significant difference was observed for any of the analyzed indices (p> 0.05) nor in the short term (7 days) neither in long term (60 days). It was concluded therefore that, for the studied sample, counseling consisted in an effective treatment option for the control of signs and symptoms of TMD, with results in the short and long term similar to the usual treatment group.

Temporomandibular disorders (TMD) and fibromyalgia (FM) are characterized by chronic pain. Individual pain variability in these syndromes can be associated with individual changes in the complex system of reception of nociceptive stimuli, such as reduced ability in recruiting controls endogenous opioid pain inhibitors. The ?-opioid receptor (MOR) is encoded by the OPRM1 gene and it is the main target of exogenous and endogenous opioids able to mediate basal nociception. Mutation in OPRM1 such as single nucleotide polymorphism (SNP) 118A>G (rs1799971), promotes amino acid changes in the receptor protein, and can alter their expression. In the first paper we reviewed the literature in order to investigate the comorbidity between TMD and FM. In the following chapters, the SNP 118A>G in the ?-opioid receptor gene (OPRM1) was identified in symptomatic and asymptomatic subjects presenting
temporomandibular joint internal disarrangement (TMJDI), fibromyalgia patients and healthy subjects, and was investigated whether the presence of the G allele is associated or not with chronic pain phenotype. In chapter 2 we performed a case-control study with 529 women who were divided into 3 groups: control (healthy women) (n=190); TMJ-ID without pain (n=164); TMJ-ID with chronic pain (n=175). In chapter 3, we selected 66 healthy women (control group) and 41 women with fibromyalgia. For polymorphism analysis, genomic DNA was collected from each volunteer from mouthwash solution of 3% glucose. The fragment of interest was amplified by polymerase chain reaction (PCR) and alleles were identified using specific restriction enzyme DpnII for 1 hour at 37 ° C. Based on the evidences by these works, we concluded that TMD is often present in patients with FM; and that the polymorphism 118A>G is not associated with the phenotype of chronic pain in patients with internal derangements of the
temporomandibular joint and in patients with fibromyalgia

temporomandibular disorder (TMD) is a change multifactorial and difficult to diagnose, their well-founded assessment is important for the proper and efficient treatment planning. However, there is no consensus among researchers regarding the diagnosis of TMD. The aim of this study was to evaluate the correlation between accuracy and assessment tools TMD. We evaluated 224 women with (TMD group) and without TMD (control group) through the Research Diagnostic Criteria for
Temporomandibular Disorders (RDC / TMD), for the diagnosis of myofascial dysfunction and then underwent evaluation of anamnestic index of Fonseca (AIF) and electromyography (EMG) of the masseter, temporal and supra-hiódeos, both at rest and isometry of the jaw elevator muscles. The data between the DTM and control group was performed using the Mann-Whitney test. For the correlation we used the Spearman correlation coefficient and to evaluate the accuracy, we used the receiver operating characteristic curve, to obtain the values of accuracy, sensitivity, and specificity. The results show that the AIF is an accurate instrument (0.94) for the diagnosis of TMD and a sensitivity of 86.3%, specificity of 91.19% and a cutoff of 47.5. The EMG correlated (correlation coefficient between 0.61 - 0.83) with a diagnosis of muscle
disorders and presents accuracy (0.65 - 0.84) for the diagnosis of TMD held for pickup at rest all the muscles tested, and the supra-hyods muscles in isometrics, however the data of the masseter and temporal collected during isometric showed no acceptable accuracy. It is concluded that the AIF is an instrument for accurate diagnosis of TMD muscular, EMG correlates with a diagnosis of muscle dysfunction and moderate accuracy when collected at rest. It is suggested that the AIF can be used as a complementary tool and prior to the RDC / TMD and EMG can be used in assessing the DTM

temporomandibular disorders (TMD), define them as a set of joint and muscle
disorders in the orofacial region, mainly characterized by pain, noise in the
temporomandibular joints (TMJ) and altered mandibular function, and may be associated with cervical changes arising from Cranio Cervical
Disorders. Electromyographic studies have shown that changes in head position can change the pattern of the mastication muscles activities and the jaw position. Based on these aspects, the aim of this study was to evaluate the electromyographic activity of the supra-?hyoid muscles, sternocleidomastoid, trapezius, anterior temporal and masseter in 15 edentulous volunteers with low vertical dimension of occlusion (VDO), TMD treated with plan occlusal splints according to the clinical protocol of CETASE (Center for Studies and Treatment of Functional Changes of the Stomatognathic System) FOP -? Unicamp. The electromyographic evaluations were performed simultaneously and bilaterally, before the beginning of the treatment and at 30, 60 and 90 days of treatment, with the jaw at rest position and isometric closure with resistance of the splint. The results showed in the jaw rest position a significant increase in electromyographic activity of the right sternocleidomastoid muscle, when comparing the initial period and after 60 and 90 days (p = 0.03), and right medium trapezius muscles after 60 days (p = 0.03), and left medium trapezius muscles, after 90 days of treatment (p=0.04). Related to isometric closure with resistance significant results were found for the electromyographic activity of the right suprahyoid muscles (p=0.04) and right (p=0.04) and left middle trapezius (p=0.02) at 90 days after treatment. A significant correlation was observed when comparing bilaterally all muscles in both conditions (p<0.05). After the plan occlusal splint therapy, there was a significant functional equalization of the electromyographic activity of neck and suprahyoid muscles, suggesting that changes in mandible position can influence the electrical activity of cervical muscles

▼ O objetivo deste estudo foi avaliar a presença e o nível de ansiedade e depressão em indivíduos portadores de desordens temporomandibulares (DTMs) que procuraram voluntariamente o Centro de Oclusão e Articulação Temporomandibular (COAT) da Faculdade de Odontologia da Universidade Federal da Bahia (FOUFBA). Foram selecionados cem pacientes de ambos os gêneros, sendo que metade (cinqüenta indivíduos apresentavam DTM (grupo teste) e os outros cinqüenta não apresentavam DTM (grupo controle). O Índice Anamnésico de Helkimo, o Índice de Disfunção Clínica de Helkimo e questionários específicos classificaram o indivíduo como portador ou não de DTM. Os indivíduos de ambos os grupos foram ainda pareados por gênero e idade. Para avaliação dos níveis de ansiedade e depressão da amostra foi utilizada a Escala Hospitalar de Ansiedade e Depressão Hospital Anxiety and Depression Sacale - HADS e, para avaliação da severidade global das DTMs, foi utilizado o Índice Craniomandibular (ICM), o qual apresenta dois subíndices, o Índice de Palpação (IP) e o Índice de Disfunção (ID), capazes de identificar o grau de comprometimento muscular e articular da disfunção, respectivamente. Foi verificada uma correlação positiva entre os sinais e sintomas das DTMs tanto com a ansiedade quanto com a depressão. Foi encontrada também uma correlação positiva entre a severidade das DTMs e os níveis de ansiedade e depressão apresentados pela amostra, e ainda, que existe uma correlação entre a ansiedade e a depressão com o grau de envolvimento muscular.
The aim of this study was to analyze the presence and the level of anxiety and depression on patients with temporomandibular disorders TMD who voluntarily consulted COAT Temporomandibular and Occlusion Center of Dentistry College of Federal University of Bahia. There were selected one hundred patients of both sexes which fifty patients have TMD (test group) and other fifty patients did not have TMD (control group). The patients were submitted to Helkimos Anamnestic Index added to COATs anamnestic questionnaire, based in these criteria the patients were divided in two groups. The selection of the control group was based on the test group regarding age and gender. The data were collected from: a questionnaire containing a spontaneous report from the patient and general health information; a specific questionnaire about TJD, masticatory muscles and unspecific reports. The Craniomandibular Index (CMI) was used to evaluate the global severity of the TMD, thats composed by two sub-indexes: Palpation Index (PI) and Dysfunction Index (DI), witches are able to identify the level of muscular and articular damage of the dysfunction, respectively. For measuring the level of anxiety and depression of the sample, was used the Hospital Anxiety and Depression Scale HADS. A positive match was found between symptoms and signs of TMD disorders and anxiety as well as depression. temporomandibular, músculos mastigatórios e estruturas associadas. A hipermobilidade articular generalizada (HAG) é uma condição sistêmica que tem sido considerada como fator contribuinte para o desenvolvimento de sinais e sintomas da DTM, porém, não existe um consenso sobre essa relação. Este estudo teve como objetivo verificar a prevalência de HAG em indivíduos com DTM e assintomáticos e sua relação com aspectos clínicos/psicossociais e a atividade elétrica dos músculos mastigatórios. Foram avaliados 61 voluntários do gênero feminino (34 indivíduos com DTM e 27 assintomáticos), com idades entre 18 e 35 anos, diagnosticados com e sem DTM pelo Inventário Critérios de Diagnóstico para Pesquisa de Desordens Temporomandibulares (RDC/TMD). Os participantes foram avaliados quanto à presença HAG pelo Escore de Beighton. O exame eletromiográfico dos músculos masseter e temporal anterior foi realizado, bilateralmente, em situação de repouso mandibular, máxima intercuspidação e mastigação. Verificou-se presença de HAG em 64,71% indivíduos com DTM e 40,74% no grupo de assintomáticos. Foi observada correlação positiva moderada entre os escores de HAG e a amplitude de abertura bucal passiva (p=0,0034), com dor (p= 0,0029) e sem dor (p=0,0081) no grupo com DTM. Neste grupo, os indivíduos com HAG apresentaram maiores valores de amplitude de movimento mandibular, exceto para a protrusão. Houve amplitude de abertura com dor (p=0,0279) significativamente maior no grupo com DTM e HAG. Múltiplos diagnósticos estiveram presentes na maioria dos indivíduos com DTM, sendo que todos apresentaram dor miofascial e 91,12% algum comprometimento articular, especialmente artralgia (79.41%). Desordens discais estiveram presentes em 41% dos sujeitos. Elevados percentuais de depressão e sintomas físicos não específicos foram observados em indivíduos com DTM, independente da presença de HAG. Na avaliação eletromiográfica, observaram-se maiores níveis de atividade elétrica de repouso dos músculos temporais em relação aos masseteres nos indivíduos com DTM, com significância estatística para o músculo temporal esquerdo (p=0,0352). Os voluntários com DTM e HAG apresentaram maiores valores de RMS dos músculos mastigatórios em relação aos indivíduos sem hipermobilidade, com valores em níveis de hiperatividade para os músculos temporais e significância estatística para os músculos masseter direito (p=0,0232) e esquerdo (p=0,0129). Na mastigação, maior atividade EMG foi registrada no grupo controle, com diferença significativa para o músculo temporal direito (p=0,0286), porém não houve diferença significativa quanto à presença de HAG. A partir destes resultados, conclui-se que indivíduos com DTM associada ou não à HAG não diferem quanto aos aspectos clínicos e psicossociais avaliados, exceto quanto à amplitude de movimento de abertura mandibular. ▼ As disfunções temporomandibulares são cada vez mais frequentes e uma vez que afetam a qualidade de vida, são necessários tratamentos que possam atenuar sinais e sintomas. A viscosuplementação com ácido hialurónico é um tratamento usado com o intuito de tratar estas limitações.
Objetivos: compreender de que maneira a infiltração com ácido hialurónico atua nos distúrbios temporomandibulares, eliminando ou atenuando os sinais e sintomas. E, em seguida, se existe alguma diferença nas suas características moleculares e nas técnicas de aplicação.
Metodologia: pesquisa realizada na base de dados da biblioteca da FMDUP, PubMed e repositório ResearchGate, com recurso às palavras-chave “hyaluronan”, “TMJ”, “
Temporomandibular disorders”, “Hyaluronic acid”, “viscosupplementation”, “arthrocentesis”, “
temporomandibular joint”. Os artigos selecionados foram publicados entre 1974 e 2015.
Discussão: o ácido hialurónico é uma substância natural constituinte dos tecidos articulares, componente base do fluido sinovial. Quando existe uma patologia articular geralmente ocorre uma limitação dos movimentos mandibulares, ruídos articulares e dor. Foram realizados vários estudos em que o ácido hialurónico teve um efeito benéfico tanto na dor, nos movimentos mandibulares e na redução de ruídos articulares. Quanto ao tamanho molecular não existiu diferença significativa entre baixo e alto peso molecular. Relativamente ao número de sessões, um menor número de sessões poderia ser benéfico devido aos efeitos notados logo após a segunda sessão.
Conclusão: a viscosuplementação com ácido hialurónico acompanhada de lavagem articular, promove a diminuição da inflamação e do atrito nas superfícies articulares, aumenta a lubrificação, permite a diminuição da dor e a melhoria dos movimentos e ruídos articulares. No entanto, é necessário realizar mais estudos sobre os seus efeitos a longo prazo e estabelecer protocolos específicos a usar em cada tipo de patologia.

▼ Purpose: The main objective of this study was to
compare the masticatory and cervical muscle tenderness and general
pain sensitivity between patients with TMD and healthy controls.
Methods: Eligible patients completed the Neck Disability Index and
Jaw Dysfunction Index. Tenderness of the masticatory and cervical
muscles and general pain sensitivity in the hypothenar region of
the left hand were measured using an algometer. Results: The PPTs
of the masticatory as well as cervical muscles of subjects with TMD
were significantly lower statistically at almost all sites tested
when compared with the healthy controls. Effect sizes were moderate
to high indicating a clinically relevant difference between groups.
General pain sensitivity of patients with TMD was significantly
higher than the healthy control group. The correlation between jaw
disability and neck disability was significantly high. Conclusion:
The results of this study suggest a relationship between neck
muscle tenderness and TMD. These findings emphasize the importance
of including the neck when evaluating and treating patients with
TMD.

▼ Masseter muscle (MM) has been found to undergo morphological changes in temporomandibular disorders (TMDs). These changes are poorly understood even though muscle architecture is an important determinant of function. The purpose was to model and quantify the musculo-aponeurotic architecture of MM in 3D. In total, 9300 FBs were digitized and modeled from 8 formalin-embalmed specimens (mean age 74.9±15.9 years). Each MM consisted of multi-laminar superficial (SH) and deep heads (DH), with FBs spanning between superior and inferior aponeuroses. Superficial head had FBs that were on average 12mm longer than DH, 3 times the physiological cross-sectional area and 4 times the volume. On average SH had 3 laminae with 5 aponeuroses, while DH had 2 laminae with 2 aponeuroses. The results of this study can be used to develop in vivo ultrasound protocols for studying normal and pathologic muscle architecture and construct finite element models for simulating functional biomechanics in the muscle.

▼ The current study is a continuation of studies by Gatchel and colleagues. Data were collected from 101 patients at several community dental clinics. Based on the patients' initial evaluations, they were randomly assigned to one of three treatment groups: Low Risk/Non-intervention Group; High Risk/Biobehavioral Group; or High Risk/Self-Care Group.
This study attempted to better understand and objectively quantify meaningful symptom relief by determining the minimal clinically important difference (MCID) for
temporomandibular joint disorder (TMD). Despite limitations and controversy with determining the most appropriate method, this information will play an important role in determining treatment effectiveness for not only TMD, but for other pain conditions as well. The most commonly referenced methods for determining meaningful change are the distribution- and anchor-based approaches. Distribution-based minimal detectable change (MDC) values were calculated using the formula 95% CI=1.96 x Square Root(2) x SEM, while the anchor-based approach minimal clinically important change (MCID) values were calculated using a Receiver Operating Curve (ROC). Both mean particle size and broadness of distribution served as two separate functional anchors, and normal range and .5 SD as two separate cutoff methods.
Despite some variability, the MCID values were relatively consistent with the MDC values regardless of method, anchor, or cutoff for both the Physical Component Scale (PCS) and Mental Component Scale (MCS) of the SF-36. The Characteristic Pain Inventory and Graded Chronic Pain Scale showed a narrow range of variation within the MCID values; however, the MCID values calculated were significantly higher than the MDC values reported for the same measures.
Findings indicated that the PCS component of the SF-36 provided stronger evidence of clinically meaningful change. The PCS resulted in asymptotic values closer to .1 (at the 90% confidence interval) with areas under the curve that better fit the model compared to the other subjective measures (considered fair at .701 when using the normal range and .740 when using .5SD for the Biobehavioral Group). Additionally, broadness of distribution resulted in more clinically meaningful changes as a result of better metric values when comparing the biobehavioral versus the self-care groups.

Temporomandibular Joint Disorder Subtypes Oral Medicine OBJECTIVE: The purpose of this study was to assess the prevalence of tinnitus within a TMD population and to determine an association between the presence of tinnitus and type of TMD diagnoses. METHODS: A secondary data analysis was performed using data from ‘Research Diagnostic Criteria for
Temporomandibular Disorders (RDC/TMD) baseline (Validation project) study and follow up (Impact project) study. Self-reported questionnaires for reporting tinnitus and medical history and gold standard diagnoses after clinical examination were used. Log-binomial regression was used to compute risk ratios for tinnitus by TMD subtype and adjusted for patient characteristics. All statistical analysis was performed using SAS 9.3 software (SAS Institute), and a two-sided significance level of 0.05 to determined statistical significance (p<0.05). RESULTS: At baseline, 614 subjects met required criteria for TMD diagnosis. Prevalence of tinnitus within sample was 41% (253 of 614). Approximately 80% of TMD subjects received a MPD diagnosis. Tinnitus frequency in the MPD group was 48% (238/495) while subjects without MPD diagnosis the rate of tinnitus was 13% (15 of 119). Using log-binomial regression analysis, the risk ratio for tinnitus was calculated. The relative risk for tinnitus by number of sites painful to palpation by TMD diagnosis for MPD, DD and DJD groups was 1.03 (95% CI: 0.97, 1.10; p=0.28), 1.24 (95% CI: 1.05, 1.46; p = .0086) and 1.20 (95% CI: 1.01, 1.43; p = .033), respectively, when adjusted for age, gender, study site and somatization. Among the population, 207 subjects received a TMD diagnosis also reported headaches. The adjusted risk ratio for tinnitus among subjects with TMD diagnosis and headache was 4.52 (95% CI: 1.67, 12.19; p = .0002) higher than in subjects with only TMD diagnosis (RR=3.8) or only headaches (RR=1.4) Similarly, the adjusted risk ratio for tinnitus among subjects with an MPD diagnosis and headaches was 3.59 (95%CI: 1.82, 7.06) higher than in subjects with only MPD diagnosis (RR=3.02) and only headaches (RR= 1.75) CONCLUSION: These findings suggest higher rate of tinnitus among subjects with MPD than other forms of TMD. Moreover, the risk for tinnitus is six times higher if
subject has TMD diagnosis and headache and three times higher in myofascial group with headaches.

▼ Aims: Many patients with temporomandibular disorders (TMD) seem to go undetected within primary dental health care. Previous studies have concluded that further efforts are needed to safeguard the clinical decision process in dentistry for patients with probable TMD. Primarily we evaluated if the implemented intervention in 2015 that was based on the 3Q/TMD increased the clinical decision-making for TMD patients compared to clinics that did not receive the intervention; secondarily we evaluated if other factors could be identified that predict performed or recommended TMD treatment; thirdly we evaluated treatment frequencies between 3Q-positives and 3Q-negatives at the intervention clinics. Methods: This case-control study was carried out within the Public dental health service in Västerbotten county, Sweden. An intervention based on a decision-tree with three screening questions for TMD (3Q/TMD) was implemented at four PDH clinics,intervention clinics. A total of 400 individuals were selected – 200 of 3Q-positives and 200 of 3Q-negatives. Clinics that were not included in the intervention programme was used as reference. The 3Q/TMD answers were analysed in relation to any TMD related decision that was collected from the digital dental records. Results: The intervention did not increase the frequencies of traceable clinical decisions among 3Q-positive patients at the interventions clinics when compared to the reference clinics. Conclusions: Despite the tailored intervention aimed at optimal use of the 3Q/TMD, the indicated under-treatment of patients with TMD remains. Future studies are still needed to gain a deeper understanding of the clinical decision-making process for TMD patients in general practice dentistry.

▼ Nesta pesquisa, realizou-se o estudo dos movimentos de abertura e fechamento da mandíbula por meio de recursos da optoeletrônica para a obtenção e avaliação dos seus centros instantâneos de rotação. Os dados foram obtidos por duas câmeras filmadoras digitais e o processamento das imagens foi realizado por um software de computador desenvolvido junto ao Departamento de Engenharia Mecatrônica da Escola Politécnica da Universidade de São Paulo. Foram realizadas filmagens de dois pacientes, um sem sinais e sintomas de disfunção temporomandibular (DTM) e outro com deslocamento anterior unilateral do disco articular. Foi desenvolvida uma estrutura removível fixada aos dentes dos pacientes, que por meio de hastes metálicas, permitiu projetar para a porção extra-oral, o movimento da mandíbula, com pontos de referência (alvos) fixados à mesma. Foram realizadas duas filmagens do movimento de abertura e fechamento de cada paciente, uma a partir da posição de máxima intercuspidação (MIC) e outra com a mandíbula sob retrusão manual. As sequências de imagens obtidas geraram arquivos de dados em forma de coordenadas dos pontos correspondentes à variação do movimento dos alvos, bem como de suas trajetórias. Pelo processamento das imagens, foram calculados os centros instantâneos de rotação mandibular (CIRs), resultando em subsídios para análise clínica. Os resultados demonstraram que: o método desenvolvido foi capaz de capturar e processar o movimento mandibular de abertura e fechamento em um plano tridimensional de forma dinâmica, produzindo dados palpáveis para análise clínica; que foi possível, com nível de grandeza na ordem de pixels, analisar o comportamento dos CIRs da mandíbula e de toda a face dos pacientes; os CIRs se encontraram em regiões diferentes durante o movimento de abertura e fechamento com ou sem retrusão manual e com ou sem DTM; a localização dos CIRs esteve próxima à região de ATM quando se executou o movimento de abertura e fechamento com retrusão mandibular, e próximos ao forame mandibular, quando se executou o movimento de abertura e fechamento natural a partir da posição de MIC.
This study assessed the mandibular instantaneous center of rotation (ICR) of mandibular opening and closing movement using the optoelectronic method. Two digital camcorders provided the images and the data were evaluated by a software developed by the Department of Mechatronic Engineering, Polytechnic School- USP. The images were obtained from two patients, one without signs and symptoms with concerns on temporomandibular disorders and the other with unilateral disc displacement. A removable structure attached to patients teeth allowed recording references points outside of the mouth. Two movies of mandibular opening and closing movement were made of each patient, one starting from intercuspal position and the other starting from a forced retrusive mandible position. ▼ Abstract: ABSTRACT The purpose of this study was to observe three different aspects related to
temporomandibular disorders (TMD): The use of self-medication, the correlation between the instruments for assessing the TMD, and functional outcomes with the use of arthroscopy. In the first study, we prospectively selected 34 patients who responded to Fonseca Anamnesic Index (FAI) and a questionnaire containing questions regarding the sought professional and the use of self-medication. In the second study, 38 medical records of patients were included. The data used were from follow evaluation tools: FAI; Jaw Symptom & Oral Habit Questionnaire (JSOHQ), Helkimo index (HI) and American Association of Orofacial Pain Questionnaire (AAOPQ). In the third study, a review of 42 medical records of patients that underwent arthroscopy of the
temporomandibular joint [operative (OA) or lysis and lavage (ALL)] was performed. The presence of pain and maximum interincisal opening were recorded for the follow-up periods of one month and two months. Data were analyzed using SPSS version 18.0. The prevalence of females were identified in all studies (91.2%; 84.6%; 85.7%) and an average age of half of the fourth decade of life. In the first study, 17 patients reported self-medication habit, especially painkillers, more specifically dipyrone. The dentist was the most sought professional (55.5%). There was no association between self-medication and the severity of TMD using the FAI. In the second study, patients classified having a severe TMD in FAI had more positive responses in AAOPQ, with a statistically significant difference when compared to mild TMD patients (p <0.01). A positive correlation was identified between the number of positive responses in AAOPQ and the sum of JSOHQ scores (p <0.01 r = 0.78). Patients classified having a severe TMD in FAI had higher scores in JSOHQ, with a statistically significant difference when compared to patients with moderate TMD (p <0.01) and mild TMD (p <0.01). In the third study, 17 patients underwent to ALL and 25 to OA. There was no statistically significant difference in the improvement of maximal mouth opening regarding the artroscopic procedure performed (OA = 2.24 mm; ALL = 1.92 mm). For both treatments, the preoperative maximal mouth opening is smaller than the postoperative in the one and two months of follow-up. Two patients that received ALL remained with pain after two months of follow-up. From the studies carried, it is possible conclude that: self-medication seems to have a high prevalence in patients with TMD, however this practice does not seem to change the severity of the disease at initial consultation; there was a congruence between the results of the assessment instruments used for DTM, then the choice should be based on the clinic and research purposes; Arthroscopy appears to alleviate the pain of patients unresponsive to conservative treatment, however the improvement of maximum mouth opening was small regardless of the technique used (AO and ALL)

Temporomandibular (DTM) é um termo coletivo que abrange um largo espectro de problemas clínicos da articulação e dos músculos na área orofacial. O sistema estomatognático integra o sistema postural, assim sendo, alterações que ocorrem em um sistema podem interferir no funcionamento do outro. A manutenção da relação maxilo-mandibular é muito comentada, porém, pouco explorada na literatura odontológica. Esta pesquisa teve o objetivo de verificar, através da telerradiografia lateral e análise de traçados cefalométricos, se há alteração da posição da mandíbula, e por meio da fotogrametria computadorizada, se há alteração da cabeça, quanto a anteriorização, antes e após o tratamento de disfunção
temporomandibular e, verificar o posicionamento da mandíbula. Foram selecionados 27 voluntários, com idade acima de 18 anos, que buscaram atendimento na Faodo/UFMS. O exame clínico foi baseado no índice diagnóstico RDC – Research Diagnostic Criteria. Os voluntários usaram um dispositivo intrabucal anterior e receberam as orientações necessárias para o tratamento. Foram realizadas as tomada radiográficas (telerradiografia) e a avaliação postural, em Relação Cêntrica (RC) fisiológica e com o dispositivo em posição, antes e após 8 semanas de tratamento. A análise estatística foi realizada por meio do teste t student, com nível de significância de 5%. A percepção à dor, avaliada pela Escala Visual Analógica, para o grupo tratamento, foi de 6,43 ± 2,84 e 2,17 ± 2,39, respectivamente, antes e após tratamento (p<0,05). Quanto ao alinhamento vertical da cabeça, no grupo tratamento, nas situações iniciais e finais, obteve-se resultado de 21,84 ± 17,49⁰ e 11,38 ± 14,61⁰ (p<0,05). Quanto ao posicionamento da mandíbula, para o grupo tratamento, sem o uso do dispositivo, no momento da tomada radiográfica, em posição de RC fisiológica, obteve-se resultado: A-NB (inicial): 4,95 ± 2,52 mm; e A-NB (final): 4,64 ± 2,52 mm (p<0,05). ▼ O objetivo desse estudo foi avaliar a posição e a intensidade dos contatos oclusais, em portadores de Disfunção
temporomandibular (DTM) de ordem muscular após o uso de uma placa intra oral de recobrimento parcial dos dentes anteriores, placa essa confeccionada em resina autopolimerizavel, incolor e classificada como placa estabilizadora. A amostra contou com 40 pacientes obedecendo aos critérios de inclusão e exclusão, sendo alocados em dois grupos: grupo I, denominado de tratamento e composto com 20 pacientes portadores de DTM muscular, e o grupo II, denominado de controle e compostos com 20 pacientes com ausência de sinais e sintomas da DTM muscular. Toda a amostra foi acompanhada durante oito semanas quando, durante esse período, os pacientes receberam a orientação do uso do dispositivo intra oral apenas para dormir. A avaliação foi realizada através de duas tomadas fotográficas dos toques oclusais, uma no inicio da pesquisa na primeira avaliação e outra no final da pesquisa, na ultima avaliação, após oito semanas. As imagens obtidas foram avaliadas através de uma análise descritiva comparativa. Os resultados demonstraram, através do Teste “t” de Student bicaudal para estudos pareados, sobre a efetividade do tratamento, tanto no quesito da distribuição da localização dos contatos oclusais (controle vs tratamento): p=0,527, quanto no quesito da intensidade do toque (controle vs tratamento) p=0,874. Onde não houve diferença significativa entre controle e tratamento p(>0,5). Baseado no exposto acima conclui-se que, mesmo com melhora na sintomatologia dolorosa da DTM, com aumento significante da distribuição da localização dos contatos oclusais e menor intensidade dos toques oclusais, não se pode afirmar sobre a efetividade do tratamento quando feita a comparação intra grupos tratamento vs controle.
