▼ This dissertation examined use of tumor necrosis factor-alpha inhibitors (TNFI) and risk of TNFI-associated serious infection compared to oral immunosuppressants in children and young adults with juvenile idiopathic arthritis (JIA)/rheumatoid arthritis (RA) and inflammatory bowel disease (IBD). Four retrospective cohort studies were conducted using the Truven Health MarketScan Research Database (2009-2013). TNFI treatment patterns, early TNFI therapy rates, and time to TNFI initiation were examined in incident JIA/RA and IBD cohorts. Serious infections were defined as those requiring hospitalization. Cox proportional hazard models were used to estimate the risk of serious infection associated with TNFIs in JIA and IBD cohorts. High-dimensional propensity score models were used to control for potential confounding. In two studies examining treatment patterns, 18.6% of the incident JIA/RA cohort initiated TNFIs, and etanercept was most commonly used, while in the incident IBD cohort, 27.6% initiated TNFIs, and infliximab was most commonly used. In both cohorts, time from new diagnosis to first TNFI prescription was shorter in more recent years. Specifically, early TNFI therapy increased over time in the IBD cohort. In a third study, 2,495 JIA patients were followed for 1,810 person-years. Serious infection rates were 2.7/100 person-years for TNFIs and 1.28/100 person-years for disease-modifying antirheumatic drugs (DMARD). TNFI monotherapy posed a 2.7-fold increase in risk of serious infection compared to DMARDs alone. In the fourth study, 10,838 IBD patients were followed for 9,849 person-years. Serious infection rates were 5.25/100 person-years for TNFIs and 3.59/100 person-years for immunomodulators (methotrexate and thiopurines). New use of TNFI monotherapy was associated with a 1.36-fold higher risk of serious infection compared to immunomodulator initiation. The risk of serious infection differed by individual TNFIs and route of administration. This dissertation characterized the utilization of TNFIs and indicated that a more aggressive treatment approach has emerged for children and young adults with JIA/RA and IBD despite an FDA warning about TNFI-associated serious infections. However, study findings support the warning for children with JIA and IBD. This dissertation provides insights into how TNFIs are being used and informs decision-making about use of these drugs–particularly regarding balancing the benefits and risks of TNFIs. ▼ O retovírus HTLV-I é o agente etiológico da Mielopatia Associada ao HTLV-I/ Paraparesia Espástica Tropical (HAM/TSP), Leucemia de Células T do Adulto (ATL) e outras doenças sistêmicas mediadas pela resposta imune. A infecção pelo HTLV-I induz uma elevada proliferação espontânea das células T do perfil de citocinas com predominância das pró-inflamatórias. Nos indivíduos com HAM/TSP o TNF-α encontra-se elevado e envolvido na lesão tecidual. O surgimento de drogas inibidoras da síntese de TNF-α traz a possibilidade de uma terapêutica, buscando reduzir a inflamação e lesão tecidual. O objetivo deste estudo foi avaliar o poder inibidor destas drogas na produção de TNF-α em PBMC de indivíduos infectados com HTLV-I. PBMC de 37 indivíduos foram avaliados: assintomáticos (n=11), subclínicos (n=7) e com mielopatia (n=19). Foram utilizadas quatro drogas inibidoras da síntese de TNF-α: Pentoxifilina, Forskolin, Rolipram e Talidomida, as quais agem em diferentes etapas da síntese desta citocina. As concentrações espontâneas de TNF-α e IFN-γ e com as drogas inibidoras foram avaliados nos sobrenadantes das culturas de PBMC através da técnica de ELISA e os resultados comparados entre os grupos usando o teste Mann-Whitney. A produção espontânea de TNF-α foi mais elevada no grupo com HAM/TSP quando comparado ao assintomático e a diferença estatisticamente significante (p = 0.001). A produção espontânea de IFN-γ também foi mais alta no grupo com HAM/TSP quando comparados aos assintomáticos e a diferença estatisticamente significante (p = 0.017). Para avaliação das drogas inibidoras de TNF-α, utilizamos PBMC de indivíduos com de TNF-α e IFN-γ espontâneos maiores que 50 pg/ml e os resultados comparados pelo teste estatístico Wilcoxon signed ranks. Pentoxifilina foi utilizada nas doses de 50 e 200 M. A inibição da produção de TNF-α com 50 M foi de 71 26% (p = 0.003) e de IFN-γ com 50 M foi de 46 24% (p = 0.001) . Forskolin foi utilizado nas doses de 50 e 200 M. A inibição de TNF-α com 50 M foi 77 25% (p = 0.003) e de IFN-γ com 50 M foi 61 22% (p = 0.001). Rolipram foi utilizado nas doses de 1, 0.1 e 0.01M. O TNF-α foi inibido com 1 M em 80 22% (p = 0.007) e o IFN-γ na dose mínima de 0.01 M em 47 20% (p = 0.002). Talidomida foi utilizada nas doses de 1e 50 M. O TNF-α foi inibido com 1M em 65 22% (p = 0.007). A inibição sobre IFN-γ foi menor que 50% na dose máxima utilizada. Os dados mostram que as drogas testadas são capazes de suprimir a produção de TNF-α e IFN-γ e que a supressão é dose dependente. Também foram avaliadas as concentrações séricas das citocinas TNF-α e IL-6 em plasmas de 38 indivíduos infectados pelo HTLV-I. Destes, 18 tinham HAM/TSP e 20 eram assintomáticos. No grupo com HAM/TSP a média de TNF-α foi 7 28 pg/ml; nos assintomáticos foi 6 18 pg/ml. Em ambos os grupos só um resultado foi positivo, os outros não tiveram concentrações séricas detectáveis. ▼ Le lymphocyte B (LB) joue un rôle important dans la polyarthrite rhumatoïde (PR), en produisant des auto-anticorps qui ont un rôle pathogène, en activant les lymphocytes T, en sécrétant des cytokines pro-inflammatoires et en permettant la formation de centres germinatifs. Plus récemment, il a été montré que le LB pouvait également produire de l'interleukine (IL) 10, une cytokine anti-inflammatoire qui lui procure des fonctions régulatrices. Ces B régulateurs ont notamment la capacité de différencier les lymphocytes T en T régulateurs. De nombreux traitements sont actuellement disponibles dans la PR, notamment les anti-TNF alpha et les inhibiteurs du récepteur de l'IL-6 (tocilizumab). Dans la première partie de cette thèse, nous avons comparé les LB circulants de patients atteints de PR et de contrôles. Nous avons étudié l'influence de l'activité de la maladie et des traitements sur les LB. Nous avons montré qu'il existait une lymphopénie B globale chez les patients atteints de PR avec une répartition des différents sous-types de LB superposable à celle des contrôles. Les patients ayant une maladie active avaient significativement plus de LB mémoires totaux, pré- et post-switch, CD24hiCD27+ et double négatifs que les patients ayant une maladie peu active. Les traitements anti-TNF et le tocilizumab ne modifiaient pas la répartition des sous-types de LB. Nous avons également montré qu'un taux de plus de 26% de LB mémoires CD27+ avant l'instauration d'un traitement par anti-TNF était associé à la réponse clinique à 3 mois. Les LB mémoires semblent produire plus de TNF que les LB naïfs et par ce biais pourraient induire une réponse Th1. Dans la seconde partie de cette thèse, nous avons tout d'abord cherché à mieux définir les LB régulateurs. Nous avons ensuite étudié leur présence et leur rôle dans la PR. Chez les sujets sains, les LB CD24hiCD27+ et CD24hiCD38hi semblent produire plus d'IL-10 que les autres LB, qui peuvent néanmoins en sécréter. Il semble donc plus adapter de définir les B régulateurs comme B producteurs d'IL-10 ou B10. Les patients atteints de PR avaient significativement moins de B10 que les contrôles en pourcentage et en valeur absolue. Chez les patients ayant un facteur rhumatoïde (FR) positif, il y avait une corrélation inverse entre le pourcentage de B10 et le taux de FR. Il y avait une corrélation inverse entre l'activité de la maladie (DAS28) et le pourcentage de B10, qui était particulièrement marquée pour les PR évoluant depuis moins de 5 ans. Chez ces patients, il y avait également une corrélation inverse entre les B10 et l'inflammation biologique (protéine C réactive). L'instauration d'anti-TNF ou de tocilizumab ne modifiait pas le taux de B10. Les CD24hiCD27+ et CD24hiCD38hi induisaient plus de lymphocytes T régulateurs chez les contrôles que les autres LB (CD24lo/-) alors que ça n'était plus le cas chez les patients atteints de PR, montrant que ces sous-types ont perdu cette fonction régulatrice dans la PR. ▼ Selkärankareuma on tulehduksellinen reumasairaus, joka ilmenee tyypillisesti nuorella aikuisiällä. Selkärankareuman oireisiin kuuluu levossa pahentuva tulehduksellinen selkäkipu, lanne- ja rintarangan jäykkyys sekä perifeeriset tulehdukset. Pohjoiseurooppalaisen väestön keskuudessa selkärankareumaa esiintyy 0,2-0,5 %:lla. Selkärankareumapotilaiden kuolleisuus on noin 50 % väestön keskiarvoa korkeampi. Selkärankareumaa hoidetaan ensisijaisesti fysioterapialla sekä tulehduskipulääkkeillä. Potilaille, jotka eivät saa näistä riittävää apua, voidaan käyttää myös TNF-salpaajia. Suomessa on markkinoilla viisi selkärankareuman hoitoon käytettävää TNF-salpaajaa: infliksimabi, etanersepti, adalimumabi, golimumabi ja sertolitsumabipegoli. Vuonna 2015 TNF-salpaajista lääkekorvausta saaneen potilaan keskimääräinen lääkekustannus Suomessa oli yli 12 000 euroa. TNF-salpaajien kustannusvaikuttavuutta selkärankareuman hoidossa muuhun hoitoon, kuten tulehduskipulääkkeisiin tai tavanomaisiin reumalääkkeisiin verrattuna on tutkittu laajasti, mutta kustannusvaikuttavuuksien eroista TNF-salpaajien välillä on vähemmän tietoa. Valtaosa tutkimuksista on satunnaistettuihin kontrolloituihin tutkimuksiin perustuvia mallinnuksia. Tässä työssä tarkasteltiin TNF-salpaajien kustannusvaikuttavuutta selkärankareuman hoidossa käsittelevää kirjallisuutta ja toteutettiin suomalaiseen biologisten lääkkeiden rekisteritietoon pohjautuva potilaskohtainen mallinnustutkimus. Mallinnuksella selvitettiin TNF-salpaajien (infliksimabi, etanersepti, adalimumabi ja golimumabi) kustannusvaikuttavuutta selkärankareuman hoidossa potilaan ensimmäisenä biologisena lääkehoitona verrattuna toisiin TNF-salpaajiin. Mallinnus toteutettiin elinikäisessä aikahorisontissa ja siinä huomioitiin suorat kustannukset vuoden 2015 tasolla. Mallinnuksen lopputuloksena havaittiin adalimumabin, infliksimabin sekä golimumabin dominoivan etanerseptia. Suurin vaikuttavuus saavutettiin golimumabilla, kun taas alhaisimmat kustannukset olivat infliksimabilla. Golimumabin inkrementaalinen kustannusvaikuttavuussuhde infliksimabiin verrattuna oli 63 840 €/QALY. Herkkyysanalyyseissä havaittiin mallin olevan erittäin herkkä TNF-salpaajien hinnoille. Niiden lisäksi herkkyyttä havaittiin myös diskonttokoron muutoksille sekä mallin aikahorisontille.

▼ RESUMO:As terapias biológicas revolucionaram o tratamento das doenças autoimunes nos últimos anos. Tipicamente têm como alvos mediadores importantes no mecanismo das doenças. Os antagonistas do fator de necrose tumoral-α (TNF-α) são um grupo de agentes biológicos muito prescrito, pois estão indicados no tratamento de doenças imuno-mediadas comuns, tais como artrite reumatoide, artrite idiopática juvenil, artrite psoriática, espondilite anquilosante, doença de Crohn e colite ulcerosa. Com o uso frequente de inibidores do TNF-α, tem-se tornado evidente que estes agentes têm um potencial imunogénico importante, que pode comprometer o prognóstico a longo prazo dos doentes cronicamente tratados. A produção de anticorpos anti-fármaco parece causar falência terapêutica secundária em muitos doentes. Um dos efeitos dos anticorpos anti-fármaco é o aumento da eliminação do fármaco. A eliminação do fármaco, por sua vez, varia entre indivíduos, refletindo diferentes perfis farmacocinéticos. A determinação dos níveis séricos mínimos do agente anti-TNF-α é assim muito informativa e pode auxiliar nas decisões terapêuticas. Contudo, os testes imunológicos para determinar as concentrações séricas do fármaco não estão facilmente disponíveis na prática clínica. De forma a investigar uma nova técnica potencialmente fidedigna e prática para a deteção e quantificação dos agentes biológicos anti-TNF-α, foi testada a técnica por HTRF (homogeneous time-resolved fluorescence resonance energy transfer) para a determinação de concentrações séricas de infliximab. Apesar de apresentar algumas limitações relacionadas com as condições de leitura da fluorescência, esta técnica provou obter resultados próximos das concentrações obtidas por ELISA (enzyme-linked immunosorbent assay) bridging. Adicionalmente, tem a vantagem de ser de execução muito mais fácil e rápida. Deste modo, a técnica por HTRF poderá ser otimizada e tornar-se uma valiosa ferramenta laboratorial para orientar as decisões terapêuticas em doentes autoimunes com falência da terapêutica anti-TNF-α. – – – ABSTRACT: Biologic therapies revolutionized the treatment of autoimmune diseases in the last years. Typically, they target important disease mediators. Tumor necrosis factor-alpha (TNF-α) antagonists constitute a very prescribed group of biologic agents as they are indicated for the treatment of common immune-mediated diseases, such as rheumatoid arthritis, juvenile idiopathic arthritis, psoriatic arthritis, ankylosing spondylitis, Crohn’s disease and ulcerative colitis. With the increasing use of TNF-α inhibitors it has been noticed that they have an important immunogenic potential that can compromise long-term outcomes in chronically treated patients. ▼ The subject of this thesis is the discovery of novel small-molecule inhibitors of Tumor Necrosis Factor (TNF). TNF has been related to autoimmune diseases such as psoriasis, Crohn’s disease, Alzheimer’s disease and plays a pivotal role in Rheumatoid Arthritis pathogenesis. Anti-TNF therapy is used for diseases such as ankylotic spondylitis, rheumatoid and psoriatic arthritis, psoriasis, inflammatory bowel disease etc. SPD304 is a small molecule that inhibits the activity of TNF by interacting with the protein trimer, promoting protein detrimerization and thus an inactivated dimer is formed in complex with SPD304. SPD304 is toxic and for this reason it can not be utilized as a drug for anti-TNF therapy but only as a basis for the discovery of new small-molecule inhibitors that prevent protein trimerization. Thus, the development of small-molecule TNF inhibitors remains a desirable goal.The subject of RAlead project was the discovery of small-molecule TNF inhibitors and for this purpose a series of SPD304 analogs was designed and synthesized aiming at the elimination of toxicity and the enhancement of activity. Additionally, commercially available compounds that have risen from docking studies as well as compounds designed on the basis of the results of the binding assays were studied.For the evaluation of the potential inhibitors the dissociation constant (Kd) was measured with fluorescence spectroscopy. The binding mechanism was studied with Isothermal Titration Calorimetry (ITC). The binding mode of SPD304 to TNF protein used for the binding assays, was studied with X-ray crystallography. For all the assays above, TNF was expressed as a GST-protein in E-coli cells and purified according to the protocol that was developed. A serious problem that has risen during this study was the low aqueous solubility of the studied compounds. Techniques on solubility enhancement were studied and a solubilization protocol for small molecules was developed. This protocol was adapted to the binding assay in order to be compatible with the assay and the protein. Except for in vitro biochemical assays such techniques can be a powerful tool in crystallization of protein complexes with small molecules.A series of 86 SPD304 analogues were studied together with 60 potential inhibitors derived from docking studies. The majority of these compounds was not soluble enough for the measurent of dissociation constant. With solubility enhancement most of them became sufficiently soluble and the binding studies took place. During solubility studies different cosolvents (DMSO, PEG3350) and other additives (β-cyclodextrin) were tested and it was shown that PEG3350 is more effective than DMSO for moderately insoluble compounds while β-cyclodextrin is efficient even for extremely insoluble compounds. ▼ Biological therapies broadened new therapeutic horizons in the treatment of the chronic inflammatory arthropathies. It has been shown that these drugs are effective both for the resistant forms of the rheumatoid arthritis and for the seronegative spondylarthropathies, achieving significant clinical improvement as well as inhibition of radiological lesions’ progression. The aim of the present doctoral thesis was to investigate the long-term efficacy, the safety (toxicity), the residence time (survival) and the reasons of discontinuation of the therapy with biological agents in chronic inflammatory arthropathies. For this purpose, medical records of patients diagnosed with rheumatoid arthritis (RA), psoriatic arthritis (PsA) and ankylosing spondylitis (AS), who were under treatment with the anti-TNFa biological agents infliximab, adalimumab and etanercept and were also under the supervision of the Rheumatology Clinic of the University Hospital of Ioannina during a period of 16 years, from October 1999 up to November 2015 were studied. The data related with the parameters of assessment for the effectiveness and safety were collected at determined time points: at baseline, every six months for the first two years of observation and then annually until the completion of an eight-year period for rheumatoid arthritis and a six-year period for spondylarthropathies (PsA and AS). More specifically, at the beginning of the study, the data that are related to the demographic characteristics of the patients (age, sex, etc.) and the nature and severity of the disease (disease duration, less or more destructive disease, presence or not of inflammatory spondylitis, presence or not of positive RF, any prior therapy with other biologic agent or DMARDs etc) were recorded. In each patient's assessment (every 6 months or a year), the dosage of medications, parameters of clinical and laboratory assessment (number of tender and swollen joints, ESR, CRP etc.) as well as specific indices of clinical response which are different for each disease (DAS 28 ESR, DAS 28 CRP, SDAI, ACR 20, 50 and 70 and HAQ for RA; DAS 28 ESR, DAS 28 CRP, SDAI, modified ACR 20, 50 and 70, PsARC and PASI for PsA; BASDAI, BASFI and ASAS for AS) were recorded. Τhe side effects (including allergic reactions) and other adverse events which were possibly related to the biological treatment, disturbances from laboratory tests, appearance of new diseases, surgical operations that were carried out during the biological treatment and any discontinuation or change of the biological agent to another biological agent, including the reasons of this discontinuation or change were also recorded. The primary objective of the present study, as far as the RA patients are concerned, was the calculation of the rate of patients who responded according to the activity index DAS 28, for the PsA patients according to the index PsARC and for the AS patients to the index BASDAI respectively. ▼ The main purpose of this study was to evaluate medication use patterns (i.e., dose escalation, medication adherence, persistence, and switching) of rheumatoid arthritis (RA) patients on etanercept (ETN), infliximab (IFX) or adalimumab (ADA) and the associated healthcare utilization costs using Texas Medicaid data. Study participants were Medicaid beneficiaries (18-63 years) with an RA diagnosis (ICD-9-CM code 714.0x) who had no claim for a biologic agent in the 6-month pre-index period (July 1, 2003 - Dec 31, 2010). The index date was the first date when the patient had the first fill for any of the study TNF inhibitors (ETN, ADA or IFX) within the study identification period (Jan 1, 2004 – Aug 31, 2010). Data were extracted from July 1, 2003 to August 31, 2011. Prescription and medical claims were analyzed over an 18-month study period (i.e., 6-month pre-index and 12-month post-index periods). The primary study outcomes were adherence, persistence, dose escalation, switching and cost (i.e., total healthcare, RA-related and TNF inhibitor therapy cost). The study covariates were demographic factors (age, gender, race/ethnicity), pre-index use of other RA-related medications (pain, glucocorticoids and disease modifying antirheumatic drugs), total number of non-study RA-related medications used at index, pre-index RA and non-RA related visits, pre-index healthcare utilization cost and Charlson Comorbidity Index score. Conditional regression analyses, which accounts for matched samples, were used to address the study objectives. After propensity score matching, 822 patients (n=274/group) comprised the final sample. The mean age (±SD) was 48.9(±9.8) years, and the majority of the subjects were between 45 and 63 years (69.2%), Hispanic (53.7%) and female (88.0%). Compared to patients on ETN, the odds of having a dose escalation were ≈ 5 [Odds Ratio= 4.605 [95% CI= 1.605-12.677], p=0.0031] and ≈ 8 [Odds Ratio=7.520, [95% CI= 2.461-22.983], p=0.0004] times higher for IFX and ADA patients, respectively, while controlling for other variables in the model. Compared to ETN, patients on IFX (p=0.0171) were more adherent while adherence was comparable with patients on ADA (p=0.1144). Compared to patients on ETN, the odds of being adherent (MPR ≥ 80%) to IFX was ≈ 2 times higher [Odds Ratio= 2.437, [95% CI=1.592-3.731], p < 0.0001] while controlling for other variables in the model. Persistence to index TNF inhibitor therapy and likelihood to switch or discontinue index TNF inhibitor therapy were comparable among the 3 study groups. In addition, the duration of medication use (i.e., persistence) prior to switching or discontinuation of index therapy was comparable among the 3 study groups. ▼ Trotz steigender Fallzahlen ist die Dünndarmtransplantation (Intestinal Transplantation - ITX) noch immer eine seltene Organtransplantation. Aufgrund der mit ihr verbundenen immunologischen und chirurgischen Herausforderungen wird sie nur in spezialisierten Zentren durchgeführt. Während sich das Kurzzeitüberleben von Patienten nach ITX in den letzten Jahrzehnten erheblich verbessert hat, ist das Langzeitüberleben aufgrund der hohen Immunogenität des Dünndarms, der möglicherweise auftretenden chronischen Rejektion und der Folgen einer kontinuierlich hohen Immunsuppression weiterhin erschwert. Um die Aussichten langfristiger Transplantatakzeptanz nach ITX zu verbessern, haben sich unsere Studien im Schwerpunkt mit Faktoren beschäftigt, die insbesondere das Langzeitüberleben beeinflussen können. Hierzu zählen humorale Immunreaktionen, TNF-alpha-induzierte entzündliche Transplantatschäden und die Identifikation geeigneter nicht-invasiver Rejektionsmarker. In zwei Studien über humorale Immunreaktionen konnten wir zeigen, dass donorspezifische HLA- Antikörper einen Einfluss auf die Rejektion des Transplantats haben und - sofern nicht sofort eine Behandlung erfolgt - im Blut überdauern, was weitere Schädigungen des Transplantats und sogar eine chronische Rejektion zur Folge haben kann. Darüber hinaus konnten wir beobachten, dass diese vaskuläre Form der Rejektion durch Non-HLA-Antikörper wie etwa AT1R-Antikörper und ETAR- Antikörper ausgelöst oder beschleunigt werden kann, sodass wir regelmäßige Screenings für HLA- und Non-HLA-Antikörper sowohl vor als auch nach der Transplantation eingeführt haben. Zusätzlich dazu haben wir ein konsequentes Therapiekonzept erstellt, welches unter anderem die Gabe verschiedener Wirkstoffe beinhaltet, um Antikörper zu filtern oder das Vorhandensein Antikörper produzierender Zellen zu verringern, wie etwa intravenöse Immunglobuline (IVIG), Plasmapherese (PP), Rituximab und Bortezomib. Um die Persistenz von Antikörpern im Blut des Patienten und damit langfristige Schädigungen des Transplantats zu verhindern, muss die Therapie umgehend nach dem ersten Nachweis von Antikörpern begonnen werden. Auf der Grundlage eines experimentellen Rattenmodells haben wir den TNF-alpha-Inhibitor Infliximab als Immunmodulator in unsere Induktionstherapie aufgenommen. Wir konnten zeigen, dass Infliximab den Ischämie-/Reperfusionsschaden im Transplantat reduziert und bei akuten Rejektionen, zusätzlich zur Standardimmunsuppression gegeben, eine effektive Therapie darstellt. Darüber hinaus konnten wir im Rahmen einer klinischen Studie zeigen, dass Infliximab auch die Entzündungsreaktionen bei der spät auftretenden Rejektion und der Transplantat-Enteropathie hinreichend reduziert. In umfangreicheren prospektiven Studien wird zu untersuchen sein, ob die Transplantat-Enteropathie bereits aufgrund der Immunaktivierung im Rahmen der Ischämie/Reperfusion hervorgerufen wird oder ob sie auf ähnlichen Mechanismen beruht, wie beispielsweise NOD-2-Mutationen bei Morbus Crohn. Basal-like breast cancers (BLBC) display aggressive clinical behavior attributed to the presence of cancer stem cells (CSCs). TRA-8, a monoclonal antibody that binds specifically to TRAIL death receptor 5, preferentially induces apoptosis in BLBC subtype cells; however it is unknown if TRA-8 is also cytotoxic to the CSC population. Moreover, some BLBC cell lines are resistant to TRA-8, and overcoming resistance is critical for clinical translation of this therapy. One potential approach to overcome TRA- 8 resistance is to target the Wnt/β-catenin pathway. LRP6 is a cell surface receptor that is an indispensable element of the Wnt/β-catenin signaling pathway that regulates BLBC tumor progression. Niclosamide is a potent Wnt/β-catenin inhibitor that degrades the LRP6 receptor. This study examined two questions: 1) whether TRA-8 or niclosamide as single agents are cytotoxic to CSCs and non-CSC populations of BLBCs; 2) and whether niclosamide enhances TRA-8 sensitivity of these cell population both in vitro and in vivo.CSCs from BLBC cell lines expressed cell surface DR5 and were sensitive to TRA-8 mediated cytotoxicity. TRA-8 inhibited secondary tumorsphere formation and produced a decrease in the percentage of CSCs. Ex vivo treatment of the BLBC CSCs produced significant inhibition of tumorigenicity in NOD/SCID mice. BLBC cells and CSCs showed a dose-dependent cytotoxic response to treatment with niclosamide, and reduced Wnt/β-catenin activity. Similar results were observed in primary BLBC cells isolated from patient pleural effusions obtained from patients with TNBC. The combination of niclosamide and TRA-8 further reduced Wnt/β-catenin activity and resulted in synergistic inhibition of secondary tumorsphere formation in the BLBC cell lines. In vivo studies showed that intraperitoneal administration of niclosamide in combination with TRA-8 significantly suppressed growth of established 2LMP orthotopic tumor xenografts.The results described in this study indicate that combining TRA-8 with niclosamide might be an effective way to overcome TRA-8 resistance in differentiated cancer cells and CSCs by inhibition of the Wnt/β-catenin pathway. This study provides valuable pre-clinical results that give insight into the potential use of these novel agents in future clinical trials in patients with BLBC.
