研究目的:乳児の病院外心肺機能停止の発生率は高いが、有効な救急救命措置に関する知見は不足している。そこで、本研究では病院外心肺機能停止になった乳児の1ヶ月後生存に関連する要因を検討した。方法:わが国における2005年から2008年に発生した乳児の病院外心肺機能停止に関する全症例を全国ウツタイン様式から抽出した。無作為折半サンプリングと信号検出分析法を用いて、病院外心肺機能停止乳児の1ヶ月生存に関連する要因を特定した。結果:救急救命措置および他の要因の交互作用が特定され、対象者は5つの下位グループに分類された。病院外心肺機能停止乳児の1ヶ月生存に関連する要因は、心停止の目撃あり、バイスタンダーによる人工呼吸あり、および救急通報から病院到着が18分未満、もしくは、心停止の目撃あり、人工呼吸はないが救急通報から現場到着までが7分であった。これらの要因を持った集団内の乳児の生存割合はそれぞれ、44.83%(95%信頼区間:25.58-64.08)と19.18%(95%信頼区間:14.64-23.72%)であった。結論:バイスタンダーによる心停止の目撃および速やかな人工呼吸の実施により、病院外心肺機能停止になった乳児の1ヶ月生存率を改善できる可能性が示唆された。
