▼ Autism is a heterogeneous neurodevelopmental condition. There are a range of mixed findings across the literature which indicate that autism might actually consist of several homogenous subtypes. Longitudinal and intervention studies have shown that individuals on the spectrum can diverge into a range of trajectories and outcomes across the lifespan. The identification of subtypes could therefore lead to the development of tailored and targeted supports and help improve the outcomes of individuals on the spectrum. Ideally, gaining a better understanding of what works for whom, can help ensure that all individuals on the spectrum are given the resources they need to achieve their full potential. The aim of this thesis was to explore the clinical and neurobiological heterogeneity in autism. Further, it sought to determine which factors were related to good outcomes for individuals on the spectrum. Unsupervised clustering algorithms were used to investigate whether subtypes of autism exist by examining the behavioural phenotype, neurobiology and longitudinal outcomes of individuals on the spectrum. The results of this thesis showed that subtypes with distinct neurobiological and phenotypic profiles could be identified when examining the neuroanatomical presentation and longitudinal outcomes of individuals on the spectrum, but not when examining clinical presentation on its own. It also showed that psychiatric comorbidities and white matter connectivity in the brain may have an impact on the ability of children to make improvements over time. Overall, this thesis generated empirical evidence that the current diagnostic criteria of for autism describes a condition that consists of several homogenous subtypes. In addition, it was able identify groups of individuals that showed better outcomes compared to the rest of the autism population. These findings call into question the characterisation of autism as ‘one’ condition and instead provide insights into subtypes of autism with unique aetiologies and needs. The identification of individuals with unique clinical, neurobiological and outcome profiles opens up a new realm of targets for therapies and intervention. ▼ Subjective and objective data on correlates of sleep were collected from adults with Attention-Deficit/Hyperactivity Disorder (ADHD) to determine whether ADHD of the Predominantly Inattentive (ADHD-I) and Combined (ADHD-C) subtypes differ with respect to the nature, prevalence, and impact of sleep disturbances on the core symptoms of ADHD. Subjective data were collected from 45 ADHD-I (mean 40 ± 11 years, 24% female) and 81 ADHD-C (mean 37 ± 10 years, 27% female) adults. A high proportion of ADHD subjects had complaints of excessive daytime sleepiness, poor sleep quality and fatigue. Sleep quality and fatigue were worse for ADHD-I than for ADHD-C subjects. The interrelationships between daytime sleepiness, sleep quality, and fatigue also differed between ADHD subtypes, suggesting differences in the interplay of sleep and wakefulness in ADHD-I and ADHD-C. The idea that the interplay of sleep and wakefulness differs in ADHD subtypes was further supported by findings associated with objective measures of sleep in 27 ADHD-I (mean 39 ± 12 years, 33% females) and 41 ADHD-C (mean 37 ± 9 years, 20% females) adults. Specifically, dim light melatonin onset (DLMO) time was found to be associated with inattention in ADHD-I, and with hyperactivity/impulsivity in ADHD-C. Regarding polysomnography (PSG) measures of sleep, symptoms of inattention were found to be associated with respiratory-event related arousals (RERA) in ADHD-I, and symptoms of hyperactivity/impulsivity were found to be associated with correlates of REM sleep in ADHD-C. These findings suggest a differential role of circadian cycles and sleep in the neuromodulation of the core symptoms of ADHD in ADHD subtypes.Taken together, the findings suggest that ADHD subtype differences extend to the realm of sleep. Importantly, subtype differences lie, not in measures of sleep per se, but rather in the interrelationships between measures of sleep and the core symptoms of ADHD, suggesting ADHD-subtype differences in the interplay of sleep and wakefulness. ▼ The present study describes molecular aspects of inherited congenital cataract in captive-bred Vervet monkeys. Congenital cataracts are lens opacities that are present at birth or soon after birth and include hereditary cataracts or cataracts caused by infectious agents. The MRC Primate Unit is housing a colony of captive-bred Vervet monkeys in which 7.5% is suffering from congenital cataract. However, the parents of the affected individuals were asymptomatic. Six families within the colony have been identified to be affected by two types of morphologies (Y-sutural and total cataract). Based on the evidence provided above, it was speculated that the colony was affected with autosomal recessive cataract. The main aim of this study was to facilitate a strategy for managing breeding programs by minimizing cataract occurrences in captive-bred Vervet monkeys. Integrated combination of clinical, molecular and bioinformatic strategies were used to identify and assess reciprocal candidate susceptibility genes for cataracts. The genes that are known to be responsible for most human congenital cataract cases were prioritized. The genes include Heat shock transcription factor 4 (HSF4), Crystalline Alpha A (CRYAA), glucosaminyl (N-acetyl) transferase 2 (GCNT2) and Lens intrinsic membrane protein 2 (LIM2). Twenty two subjects were selected based on their morphology (5 carriers, 5 controls and 12 cataracts). 2ml of blood was collected for Deoxyribonucleic acid (DNA) extraction. Coding exons and flanking regions were screened by polymerase chain reaction (PCR) amplification and sequenced. The CLC DNA workbench was used for results analysis. The screening of four genes revealed 20 sequence variants which were not present in the control individuals. Sequencing of HSF4 revealed three mutations: R116R, L245>L and P421>L in exon 5, 10 and 14, respectively. The coding exons for CRYAA showed two sequence variants: S134W and K166N in exon 3. Twelve mutations were identified in exon one of all three GCNT2 transcripts (A, B and C). These mutations include: G212G, H256>H, M258>V, N275>N, V16>I, Y122>F, S15>S, S24>N, S38>S, I118>I, D194>D and Y373>Y which was found in exon three of all transcripts. There were no mutations in LIM2, however, three single nucleotide polymorphisms (SNPs) were identified in exon 2 (P66>P) and 3 (I118>T and A127>T). The above mutations were conserved when aligned with other species. The sequence variations vary among the families and those individuals with the same or different cataract phenotype. Based on these findings, it can be concluded that the four candidate genes harbour mutations that are responsible for both phenotypes. ▼ Schizophrenia is a severe neuropsychiatric disorder characterised by positive symptoms such as delusions and hallucinations, negative symptoms such as amotivation and asociality and cognitive symptoms such as working memory and processing speed deficits. It is also accompanied by consequential clinical features and paradigms such as suicidality and treatment resistance. Clozapine, an atypical antipsychotic medication, mitigates suicidality and is efficacious in treatment resistance. The unique actions of clozapine may be due to its ability, but not other anti-psychotic medications, to recruit the Epidermal Growth Factor (EGF)/EGF Receptor (EGFR) signalling system. In this thesis, we used post mortem brain samples from the dorsolateral prefrontal cortex (DLPFC) of 37 individuals with schizophrenia and 37 without the disorder, who were a matched cohort. We utilized quantitative PCR to examine the EGF receptor. Then, to establish the distribution and localisation of EGFR in schizophrenia, we supplemented our quantitative studies with protein assays for the EGFR protein and in situ hybridization followed by autoradiography for EGFR mRNA. In order to then examine the relationship of cognitive functioning in schizophrenia and the EGF/EGFR signalling molecules, we examined the cognitive profile of 449 persons with schizophrenia and 637 control participants from the Australian Schizophrenia Research Bank database. We also examined if the cognitive performance in schizophrenia cluster to inform disorder sub typing and subsequently explored the relationship of cognition in schizophrenia to EGF/EGFR genomic signals by way of a candidate gene approach. In our studies, we found that EGFR protein levels are significantly increased in the DLPFC of persons with schizophrenia, although, there were no mRNA expression differences. The older neopallidal layers of the cortex showed that participants who had died of non-suicide had lower EGFR mRNA expression. Similarly, those without a history of suicide attempts showed a decreased mRNA expression more widely in the neopallidal layers. We also showed layer specific changes in relation to a number of clinical features in schizophrenia. In comparison to all other previous studies, we showed a case control difference without the need for stratification that persons who carry the AA genotype of the EGF A61G single nucleotide polymorphism (SNP), are more likely to have schizophrenia. We also showed that there are three clusters of cognitive performance within schizophrenia and treatment resistance and 2 NRG1 SNPs associate with various cluster memberships significantly. Our results show evidence to support the role of EGF/EGFR signalling in mediating and moderating several clinical parameters in schizophrenia such as treatment resistance, suicidality and cognitive functioning. We also have identified a relationship between EGF/EGFR signalling system hypofunction and schizophrenia and propose that clozapine may augment this signalling thereby mitigating this hypofunctional state.…

▼ Anorexia Nervosa (AN) is the most severe eating disorder (ED), affects significantly more females than males, and typically begins during adolescence. The development and maintenance of AN is influenced by the complex interrelationship between a number of factors; some of which relate to eating, weight, and body shape (e.g., body dissatisfaction) and others that may underlie the presentation of these concepts (e.g., perfectionism). The aim of this thesis was to build upon the current theories of the development and maintenance of AN by evaluating the characteristics of adolescent females with AN beyond diagnostic criteria. Study 1 investigated whether a group of adolescent females diagnosed with AN or subthreshold AN could be grouped into clinically relevant typologies based upon a range of variables, which were not limited to the diagnostic criteria for AN. A clinical audit was conducted on 39 adolescent females aged between 13 and 18 years. Medical, psychological, behavioural, and family information were collected from case notes, as well as follow-up medical data recorded six months after the initial diagnosis. A two-step cluster analysis derived two clinically distinct typologies. Cluster 1 was characterised by severely underweight patients who reported no clinical elevations on any eating or mental health disturbances. Conversely, Cluster 2 was characterised by healthy weight patients who reported significantly higher levels of eating, psychological, and behavioural problems. Limited follow-up data highlighted that Cluster 1 patients tended to report better outcomes in terms of weight gain and other health outcomes compared to Cluster 2 patients. Study 2 assessed the early maladaptive schemas (EMSs) reported by adolescent females, aged between 13 and 18 years, who were diagnosed with AN or subthreshold AN compared with a community group. Thirty-six adolescent females with AN or subthreshold AN were recruited from outpatient hospital and private ED clinics in Melbourne, Australia. The community group comprised 111 female secondary school students. Participants were assessed on EMSs, adaptive and maladaptive personality and behavioural features, and general psychopathology. In general, the AN clinic group reported greater EMSs, general psychopathology, and behaviour problems than the community group. A two-step cluster analysis based upon responses to a general measure of psychopathology and problem behaviours used in both studies derived two AN clusters, which replicated the results of Study 1. Additionally, 32% of the community group had indicated a possible ED, according to results of a brief ED screen, thus, the community group was divided into an At-risk group and Non-ED group. Subsequent analyses showed that: Cluster 2 patients reported the highest level of EMSs and psychopathology; few differences were found between Cluster 1 and Non-ED participants; and At-risk participants reported greater EMSs and psychopathology than Non-ED participants. HLA-B27 SUBTYPES IN GREEK POPULATION WITH SERONEGATIVE SPONDYLOARTHRITIS. The HLA-B27 subtypes distribution differ in ethnic origins. In Mediterranean AS patients tend to carry HLA- B*27:05 as well as HLA- B*27:02 . Aim. To investigate the distribution of HLA- B27 subtypes in B27 positive Greek patients with seronegative spondyloarthtritis (SpA) and associate with the clinical manifestations.Subjects -Method. HLA-B27 subtypes was performed in a cohort of HLA-B27 positive Greek patients with seronegative spondyloarthtritis according to Amor criteria, using a commercial available kit, PCR-based method ( B27 SSP UniTray-Code no 470003D, Invitrogen ) without knowledge of the their clinical details. Results. 150 HLA-B27 positive Greek patients with seronegative spondyloarthtritis (129 males and 21 female, age ranged from 18-40 years old) of whom 103 (69%) with Ankylosing Spondylitis (AS), 17 (11%) with Psoriatic arthritis (PsA), 28 (19%) with undifferentiated spondyloarthropathy (uSpA) and 2 (1%) with inflammatory bowel disease-associated arthritis (IBD-aA). The subtypes detected were B*27:05 N= 99 (65,8%) [ in AS 66/103 (64%), in PsA 15/17 (88%) , in uSpA 16/28 (57%), in IBD-aA 2/2 (100%)] and HLA B*27:02 Ν= 51 (34,2%) [in AS 37/103 (36%), in PsA 2/17 (12%), in UnSpA 12/28 (43%)]. In 20/103 pts (19%) AS -uveitis HLA-B27 positive, the subtype B*27:02 was present in 11 (54%). No novel HLA- B27 subtypes were detected in this group. Conclusion. The subtypes detected in this Greek population with SpA were HLA –B*27:05 and HLA-B*27:02. The HLA- B*27:05 been the most frequent subtype observed. This finding support the theory of the North-South gradient of HLA-B*27:05 and the role of regional characteristics in the evolution of HLA-B27. A trend in AS-associated uveitis was observed for B27: 02 positive AS patients, similar findings have been reported by other groups in the Mediterranean region.

ΥΠΟΤΥΠΟΙ ΤΟΥ HLA-B27 ΣΕ ΕΛΛΗΝΕΣ ΑΣΘΕΝΕΙΣ ΜΕ ΟΡΟΑΡΝΗΤΙΚΗ ΣΠΟΝΔΥΛΟΑΡΘΡΙΤΙΔΑ. Ο ρόλος του HLA-B27 είναι ουσιώδης στην αιτιοπαθογένεια των οροαρνητικών σπονδυλοαρθριτίδων. Σήμερα ο έλεγχος HLA-B27 αποτελεί χρήσιμη εργαστηριακή εξέταση στη διάγνωση τυποποίηση των οροαρνητικών σπονδυλοαρθριτίδων. Στην μελέτη η παρουσία του HLA-B27 βρέθηκε σε ποσοστό 30% (261 δείγματα) σε σύνολο 866 δειγμάτων ( 88% σε άνδρες και 12% σε γυναίκες). Characterizing the different HIV-1 subtypes prevalent in a geographic region as well as understanding the origin and dissemination of these subtypes are essential to define prevention and intervention strategies targeting local HIV/AIDS epidemics. The present study analyzed 895 pol sequences, available at GenBank, aiming to describe the main HIV-1 subtypes prevalent in Brazil, the resistance profile of naïve lineages, the prevalence of signatures and epitopes in protease and reverse transcriptase (RT), and reconstruct the origin and the main dissemination patterns of HIV-1 subtypes B and C in Brazil. The subtype B (65.6%) was the most prevalent followed by subtypes C (14.1%) and F (6.1%). The recombinant forms represented 14.1% of the analyzed sequences. At protease, eight major mutations which reduce the susceptibility to protease inhibitors (PIs) were found: L24I, D30N, M46I/L, I54L/V, V82L and I84V. At RT, 16 mutations associated to nucleoside reverse transcriptase inhibitors (NRTIs) resistance, A62V, D67N/G, T69E/N/S/d, K70R, V75I/L/M, F77L, V118I, Q151M and M184V, and 18 mutations that confer resistance to the non-nucleoside reverse transcriptase inhibitors (NNRTIs), V90I, A98G, L100I, K101E/N/Q, K103N/R, V106I/L, V108I, E138A/G/K, V179D/T, Y181C and G190A, were identified. The transmission of lineages resistant to PIs, NRTIs and NNRTIs were 1.2%, 1.8% and 2.1%, respectively. Regarding pol genetic variability, we identified two possible signatures, N37K e R41N, at HIV-1 subtype C protease, two T CD4+ epitopes and 18 T CD8+ epitopes, of which five were at protease and 13 at RT. Concerning these last ones, KLVDFRELNK, GIPHPAGLK, TVLDVGDAY, NETPGIRYQY, IRYQYNVL and VIYQYMDDL are highly recommended to the production of vaccine prototypes to be applied in Brazil. The HIV- 1 subtype B introduction in Brazil occurred in the beginning of 1970s, through Rio de Janeiro and Rio Grande do Sul, from where the lineages exponentially spread to Northeastern and Central Brazil. The subtype C, in turn, entered in Paraná, during mid- 1970s, and, briefly after its introduction, started to spread exponentially in the South region. The application of high-resolution phylogenetics, coalescent analysis and phylogeography, allowed an in-depth characterization of the HIV-1 epidemic in Brazil, the reevaluation of hypothesis and the formulation of new insights about the origin and dissemination of subtypes B and C in the country.

▼ Considerable evidence suggests that the presence of a callous and unemotional interpersonal style identifies an important subgroup of antisocial and aggressive youth. Relative to other children with conduct problems, youth high on callous-unemotional (CU) traits are distinguished by the absence of empathy, lack of guilt, callous and uncaring behaviors, and poverty in emotional expression. Despite the recognized heterogeneity of high CU youth as well as importance of the presence of CU traits, no study to date has attempted to disaggregate these youth into meaningful subgroups. Therefore, the current study sought to address this void in the literature by investigating whether justice-involved male youths could be disaggregated into distinct CU trait variants, consistent with theoretical and empirical conceptualizations of primary and secondary variants of psychopathy. The study involved a multi-ethinic, community corrections sample comprised of 151 male juvenile offenders. The entire spectrum of criminal offenses and levels of supervision were represented in this sample. The initial set of model-based cluster analyses failed to yield conceptually coherent primary and secondary variants, despite the inclusion of additional theoretically relevant variables. The failure of the ICU dimensions to identify meaningful clusters among the current juvenile offender sample raised concerns about the psychometric properties of the ICU, along with its factor structure. To address these concerns, additional model-based cluster analyses with various permutations of revised, unidimensional ICU subscale(s) and theoretically relevant variables were conducted. Additionally, an alternative subtyping/classification approach using scales from the Personality Assessment Inventory-Adolescent Version (PAI-A; Morey, 2007b) was applied to the sample in an effort to identify meaningful subgroups. However, both sets of supplementary analyses still failed to yield meaningful, homogeneous psychopathic variants consistent with the theoretical and empirical literature. Contrary to expectations, the current study findings offer little support for the utility of callous-unemotional traits to disaggregate justice-involved youth into meaningful homogenous subgroups. Thus, the present study’s contribution to the growing subtyping literature appears to further complicate our understanding of juvenile psychopathic variants. In order to more concisely parse out the heterogeneity of juvenile psychopathic traits, future research of the distinct developmental pathways of callous-unemotional traits among juvenile samples is needed. ▼ Blastocystis hominis (B. hominis) is one of the most common intestinal protozoan parasites in humans. Pathogenicity of B. hominis remains controversial and it has been suggested that it may be associated with certain subtypes of organism. Based on the genetic diversity of SSU rRNA (small subunit ribosomal RNA gene, SSU rRNA), B. hominis can be classified into 17 subtypes. From the results of stool samples for parasite test from the regional hospital in southern Taiwan, it was demonstrated that B hominis had the highest prevalence rate (90.2%) of intestinal parasites. The infection rate of B. hominis in foreign population was significantly higher than that of nationals (14.7% vs 4.2%). The aims of this study were to set up nested polymerase chain reaction (nested PCR) for DNA amplification and subtyping. The results were shown 81 of 112 specimens positive for B. hominis and 18 of 200 specimens negative for microscopy were shown positive by nested PCR detection. The sensitivity and the specificity of the nested PCR molecular detection method were 72.3% and 91%; respectively. Our results revealed the positive detection rate increased 9% by nested PCR compared with by microscopy. We hoped that this technology can be applied in the clinical laboratory for B. hominis screening. The sequences of 99 PCR positive samples were analyzed. The most common subtypes were ST3 (44.4%, 44/99), ST1 (43.4%, 43/99), followed by ST2 (5.1 %, 5/99), ST6 (4.0%, 4/99) and ST7 (3.0%, 3/99). A phylogenetic tree was reconstructed using the neighbor-joining method based on the Kimura-2-Parameter model and it was found that five subtypes (ST1, ST2, ST3, ST6 and ST7) can be regarded as five independent monophyletic groups. By phylogenetic analysis, Bh97 and ST1 (AB107961, host: pig), Bh149 and ST3 (AB107965, host: cattle) were highly correlated. It can be inferred that human Blastocysts is transmitted by humans and the possibility of common infections between humans and animals. Although the current prevalence of B. hominis in Taiwanese is lower than that of foreign workers, there is still the possibility of mutual infection through fecal-oral route. In this study, we found the subtype distribution of B. hominis were very similar between Taiwan population and Indonesians. Therefore, the foreign workers should be conducted appropriate health education to reduce B. hominis transmission and dissemination. ▼ Children with autism spectrum disorder (ASD) present with a myriad of diagnostic characteristics and associated behaviors. Secondarily, this population is extremely heterogeneous. Efforts have been made by many disciplines to identify more homogenous subgroups in order to improve both research and clinical outcomes. In occupational therapy, the focus has been on establishing sensory-based subtypes. This dissertation is a compilation of three separate research papers related to sensory-based subtypes in children with ASD. The first paper is a systematic review on sensory subtyping systems published in the last 12 years. Findings indicate that the majority of subtyping schemes characterize group differences by patterns of sensory responsivity (i.e., hyperresponsivity, hyporesponsivity and sensory seeking). One subtyping scheme has emerged as the most well researched of these, and includes responses to specific sensory domains for four different subtypes. The subsequent two papers presents additional research examining this subtyping system. The second paper examined neurophysiological response to sensory stimuli between the four subtypes. Salivary cortisol, skin conductance level (SCL) and respiratory sinus arrhythmia (RSA) were used examine neuroendocrine function, parasympathetic and sympathetic nervous system responses. Results indicate that parasympathetic response (as indexed by RSA) may best distinguish subtypes with typical sensory processing versus those with atypical sensory processing. More discrete differences between each of the subtypes hallmarked by different sensory processing differences were less substantial. The third paper examined functional and adaptive behaviors, in addition to clinical behaviors (psychopathology) in relationship to subtype membership. Subtypes with greater sensory processing dysfunction were found to have poorer communication, socialization and performance of daily living skills. In addition, subtypes with atypical sensory processing characteristics had higher levels of internalizing and externalizing behaviors. Again, certain subtypes were not found to differ significantly from each other on these measures. Overall findings suggest that current sensory-based subtyping schemes may not fully explain sensory processing differences or the variety of behavioral traits observed in this population. In addition, neurological reactivity patterns may not completely align with these subtype divisions. Stronger statistical differences found between certain subtypes indicates particular sensory processing characteristics may be more impairing and have more clinical relevance than others. Individuals with higher functioning autism spectrum disorders (ASD) have been understudied relative to their lower functioning counterparts even though they comprise a large proportion of those with ASD. Social and communication symptoms are particularly valuable areas of study within this population as previous research has established impairments in these domains are important predictors of functioning across multiple domains including language development, social functioning, cognitive skills, and adaptive functioning. The goal of the current study was to examine the heterogeneity in social and communication impairments in a sample of individuals with higher functioning ASD using exploratory factor analysis (EFA) and latent class analysis (LCA). In addition, an examination of the relationships between age, verbal abilities, and ASD symptomatology was also undertaken. Participants included 367 individuals with higher functioning ASD (mean full scale IQ > 70) who were verbally fluent and ranged in age from 4-28 years old. Twenty item scores from the Communication and Reciprocal Social Interaction sections of the Autism Diagnostic Observation Schedule (ADOS) were used in the EFA and LCA. A two-factor model best fit the data and the factors were conceptualized to be Social Use of Language and Abstraction (SULA) and Qualitative Social Impairment (QSI). A three-class solution fit the data well and was conceptually meaningful. Classes differed on ASD symptomatology, age, overall and verbal IQ, and aspects of adaptive functioning. Multiple regression analyses indicated that both age and verbal abilities significantly predicted impairments in communication while only age significantly predicted social impairments. These findings indicate social and communication impairments are meaningful variables to differentiate subtypes amongst samples with higher functioning ASD. The current study also provided support for prior work documenting the relationship between age, verbal abilities, and ASD communication symptoms. However, findings regarding the relationship between age and changes in social impairments were mixed relative to previous work.

Uvod Seroepidemiološke studije pokazuju da je između 27% i 80% humane populacije inficirano sa dva poliomavirusa BK virusom (BKV) i JC virusom (JCV). Posle primarne asimptomatske infekcije, koja se obično dešava u toku ranog detinjstva, oba virusa uspostavljaju latenciju u ćelijama bubrega i gornjih partija urinarnog trakta. Sporadična asimptomatska reaktivacija oba virusa se dešava kod 0.5-20% imunokompetentnih seropozitivnih osoba. Međutim, kod imunosuprimiranih osoba njihova reaktivacija može dovesti do nastanka životno ugrožavajućih oboljenja. Reaktivacija JCV, pre svega kod pacijenata sa T ćelijskom imunodeficijencijom, dovodi do litičke infekcije oligodendrocita u centralnom nervnom sistemu (CNS) i nastanka progresivne multifokalne leukoencefalopatije (PML), dok reaktivacija BKV dovodi do nastanka hemoragijskog cistitisa i poliomavirus-udružene nefropatije. Pre epidemije AIDS-a, limfoproliferativna oboljenja su bila vodeći uzrok reaktivacije JCV i BKV, dok danas HIV infekcija predstavlja najčešći uzrok reaktivacije JCV i nastanka PML. Očigledno je da imunosupresija predstavlja fundamentalni predisponirajući faktor za nastanak PML, poliomavirus-udružene nefropatije i hemoragijskog cistitisa. Ipak, ova oboljenja ne nastaju kod svih imunosuprimiranih osoba, tako da se smatra da i drugi faktori, poput genetičke varijabilnosti BK i JC virusa, mogu doprineti njihovom nastanku. Ciljevi Ciljevi ovog istraživanja bili su: da se odredi prevalenca izlučivanja BKV i JCV u urinu HIV-inficiranih pacijenata i pacijenata kontrolne grupe i ispita moguća povezanost virurije u grupi HIV-inficiranih sa demografskim, virusološkim i citološkim parametrima kao i sa stadijumima bolesti i vrstom primenjene terapije; da se utvrdi distribucija genotipova i subtipova, struktura nekodirajućeg regulatornog regiona kao i prisustvo mutacija u VP1 regionu JCV i BKV u obe grupe ispitanika...

Preeclampsia (PE) and fetal growth restriction (FGR) are two of the most common pathologies of pregnancy, both thought to be primarily driven by placental dysfunction. Despite decades of research into the underlying etiologies of these disorders, as well as potential biomarkers and treatments, no single discovery has been found to be applicable to the entire clinical spectrum of PE or FGR patients, likely due to the existence of multiple disease subtypes. Therefore, the main goal of this thesis was to investigate if the application of unsupervised clustering techniques to placental gene expression data could elucidate transcriptional subtypes of PE and FGR with increased clinical and histopathological homogeneity. Clustering of three overlapping large microarray datasets revealed 3-5 molecular clusters, depending on the study. However, three subtypes of PE placentas were consistently identified within clusters 1-3, and, eventually, each co-clustered with a group of placentas from normotensive suspected FGR pregnancies. Within cluster 1, PE and suspected FGR samples demonstrated less severe clinical outcomes, molecular similarity to healthy term controls, and either no placental lesions or mild histopathology, suggesting a dominant non-placental source of the disease. Cluster 2 PE and FGR placentas revealed overwhelming evidence of “canonical” maternal vascular malperfusion features and increased placental expression of hypoxia and hormone activity genes. Cluster 3 PE and FGR samples displayed signs of an “immunological” pathology, with a transcriptional signature of immune response, apoptosis, and cytokine activity, and histological lesions affiliated with allograft rejection, such as massive perivillous fibrin deposition. In the largest microarray dataset (N=330), two additional clusters were discovered. Cluster 4 samples were preterm controls with histological chorioamnionitis, while cluster 5 was associated with confined placental mosaicism, but no clinical or histological cohesion. Furthermore, specific differences in the expression of three genes by qPCR were found to be sufficient for separating placentas into transcriptional clusters 1-4, which will allow future studies to focus on the identification of subtype-specific biomarkers and therapeutics for PE/FGR without having to first cluster microarray data. Matched maternal samples will also be necessary to comprehend the development of hypertension in some patients but not others with similar placental profiles.

▼ It is important to understand the neurobiological, cognitive, and behavioral factors that underlie ADHD and the ADHD subtypes. It has often been hypothesized that the ADHD-Combined Type (ADHD-CT) and ADHD-Predominantly Inattentive Type (ADHD-PI) subtypes may be the result of different underlying factors and may be associated with different types of attentional deficits. The current study compared 30 children with ADHD-CT and 35 children with ADHD-PI on several neuropsychological variables associated with various aspects of attentional processing. However, it is important to note that only 47 (22 with ADHD-CT and 25 with ADHD-PI) of the participants were assessed with some of the experimental measures. All participants were between the ages of 6 years, 0 months and 12 years, 11 months. Participants were evaluated using several neuropsychological measures associated with various aspects of attentional processing in order to test five hypotheses regarding possible subtype differences in specific types of attention. The attentional processes of interest included response activation, sustained attention, encoding/working memory, the focus/execute aspect of attention, and attentional stability. These constructs were taken from the theories of Tucker and Williamson (1984) and Mirsky and Colleagues (1999). The attentional measures used in this study were taken from the Test of Variables of Attention (TOVA), the Children’s Memory Scale (CMS), and the Wechsler Intelligence Test for Children-Third Edition (WISC-III). Analysis of possible subtype differences in the occurrence rate of reading disabilities was also conducted. Finally, post hoc analyses were conducted in order to test the hypothesis that specific attentional processes might impact other specific aspects of the neuropsychological functioning of children with ADHD. The attentional processes of interest were response activation and attentional stability. The dependent variables for the post hoc analyses included measures of language ability as well as behavioral ratings of attention and hyperactivity. No statistically significant group differences were found for any of the variables reflecting the five aspects of attentional processing. Nor were any significant subtype differences in the occurrence rate of reading disabilities or the post hoc analyses. The post hoc analyses did not yield statistically significant results either. Finally, weaknesses of the current study as well as future directions for ADHD research were discussed.
