Abstract: Objective: To investigate the existence of adjuvant effect of botulinum toxin A (5u intraoperative injection) in monocular recess-resection procedure for correction of large angle horizontal strabismus in adults under local anesthesia and to describe types and frequencies of side effects. Subjects and Methods: A randomized clinical trial. Twenty-three patients with large angle horizontal deviations were randomized into two groups: surgery associated with intraoperative injection of botulinum toxin A (CG+BTA) or surgery only (CG). The recess-resection procedures were performed under local anesthesia on the non-fixating eye. The clinical characteristics, amounts of corrections at the last visit (6 to 12 months) and between the first and last visits, the percentages of changes in the pre-operative deviations at 1 day, 7 to 15 days, 1 month, 3 months and 6 to 12 months, the amount of correction between the first and last post operative visits, and the percentages of satisfactory results were compared between groups at the last visit. Results: Means and standard deviations of the preoperative angles of deviation for the groups CG+BTA and CG were 65.8 Dp ± 14.9 and 60.0 Dp ± 16.8 (p=0.26) and the means and standard deviations of the amounts of corrections at the last visit were respectively: 51,5 Dp ± 14,8 and 41,3 Dp ± 14,0 (p=0.18). The percentages of changes in the pre-operative deviations were significantly higher in the CG + TBA group at 30 days (p=0.05), and did not differ between groups at others post operative visits. Amounts of corrections were significantly lower at the last post operative visit in relation to the first for both groups (p=0.006) with no difference between them (p=0.59). There was no difference between groups in the percentage of satisfactory results at the last visit (p=0,63). Side effects of botulinum toxin A more common were: transient ptosis and transient vertical deviation both in 5 of 12 patients treated. Objetivo: Avaliar a eficiência da Triancinolona Acetonida como agente modulador da resposta inflamatória e cicatricial em coelhos submetidos a cirurgia de músculo extra-ocular. Método: Foi realizado estudo prospectivo, mascarado, em dois estádios. No primeiro estádio, dez coelhos foram submetidos a retrocesso do músculo reto superior em ambos os olhos. Aplicouse, em um deles, 0,15 cc de Triancinolona Acetonida (40mg/cc) nos tecidos circunjacentes ao local de reinserção muscular e, como controle, 0,15cc de solução de cloreto de sódio a 0,9% no local equivalente no olho contra-lateral. Quinze dias após, cinco coelhos foram submetidos a exenteração das órbitas e os restantes dos animais tiveram o mesmo procedimento realizado após trinta dias. O material do sítio de reinserção muscular foi avaliado por meio de análise histopatológica qualitativa e quantitativa (morfometria). No segundo estádio, com incrementação da agressão cirúrgica, dezesseis coelhos foram submetidos aos mesmos procedimentos com exenteração das órbitas após quinze dias, e posterior análise histopatológica dos tecidos. Resultados: Houve efeito inibitório sobre a intensidade da resposta inflamatória nos olhos tratados em comparação com os olhos controle. Conclusão: Nas condições de realização do presente estudo o uso per-operatório da triancinolona acetonida foi efetivo no controle da resposta inflamatória em olhos de coelhos submetidos a cirurgia de músculo extra-ocular.

Purpose: To evaluate the efficiency of triamcinolone acetonide (TRI) in limiting the postoperative inflammatory response and scarring following strabismus surgery. Methods: A prospective, two-stage, masked, controlled trial was conducted. In the first stage, the inflammatory response at the extraocular reattachment site was analyzed following superior rectus recession in ten rabbits. One eye had 0,15 cc of triamcinolone acetonide (40mg/cc) applied around the new insertion site and, similarly, 0,15 cc of isotonic saline solution (0,9%) was applied to the fellow eye following the same procedure, thus serving as a control. Fifteen days later, orbital exenteration was performed in five rabbits and the remaining five were exenterated thirty days later. The reattachment site tissues were submitted to qualitative and quantitative histological examinations. In the second stage 16 rabbits were submitted to amplified surgical trauma, after which the aforementioned steps were also carried out. Granuloma total area at the extraocular muscle reattachment sites of control and treated eyes were compared. Results: There was an inhibitory effect of TRI on the inflammatory response of treated eyes as compared to control eyes. Conclusions: TRI was effective in controlling the postoperative inflammatory response in rabbit eyes submitted to traumatic recession of the superior rectus muscle.

Dissociative Vertical Deviation (DVD) is a type of Strabismus. It may present as the only kinetic eye disturbance or accompany a congenital esotropia or an inferior oblique muscle overaction (IOOA). A variety of surgical procedures have been performed for the correction of the DVD. The Anterior Transposition of the inferior oblique muscle (ATIO) is an operation that is proposed during the recent years. OBJECTIVE of this study is to investigate the effectiveness of the anterior transposition of the inferior oblique muscle for the correction of dissociated vertical deviation in cases with or without concurrent inferior oblique muscle overaction. METHODS: Records were analyzed on 68 eyes belonging to 44 patients with DVD who received ATIO m one (20 cases) or both (24 cases) eyes and who had a follow up from 2 to 7 years. The pre and post surgery vertical deviation as well as the effect of the operation on the IOOA was evaluated. A comparison of the effectiveness of ATIO on the cases with or without IOOA was made. RESULTS: There was significant difference of the effectiveness of ATIO according to the grade of IOOA (p<0,001%) and the magnitude of the DVD (p<0,001%). There was no significant difference of the final results between the groups with and without IOOA (p=0,90%). A low grade of IOOA remained in 20 out of 68 eyes. CONCLUSIONS: The inferior oblique muscle recession with non graded anterior transposition is an effective procedure and can be used as a safe initial operation for the correction of DVD independently of the presence of inferior oblique overaction.

Η ασύνδετη κάθετη παρέκκλιση (Α.Κ.Π.) είναι μια μορφή στραβισμού. Μπορεί να αποτελεί την μόνη οφθαλμοκινητική διαταραχή, αλλά και να συνδυάζεται με βρεφική εσωτροπία ή με υπερλειτουργία των κάτω λοξών μυών. Έχουν προταθεί, κατά καιρούς, διάφορες χειρουργικές μέθοδοι για την αντιμετώπισή της, καμιά όμως από αυτές δεν έχει αποδειχθεί να είναι απόλυτα αποτελεσματική. Μεταξύ των χειρουργικών μεθόδων που έχουν επιχειρηθεί είναι και η πρόσθια μετατόπιση του κάτω λοξού μυός. Σκοπός της παρούσης μελέτης είναι να διερευνηθεί η αποτελεσματικότητα της χειρουργικής αυτής μεθόδου στην αντιμετώπιση της Α.Κ.Π. Υλικό – μέθοδος: Μελετήθηκαν 44 παίδια με Α.Κ.Π. (68 οφθαλμοί) που αντιμετωπίστηκαν χειρουργικά με τη μέθοδο της πρόσθιας μετατόπισης του κάτω λοξού μυός, ετερόπλευρα (20 ασθενείς) ή αμφοτερόπλευρα (24 ασθενείς). Στη μελέτη συμπεριλήφθηκαν τόσο αυτά που εμφάνιζαν την Α.Κ.Π. χωρίς άλλη οφθαλμοκινητική διαταραχή, όσο και αυτά στα οποία η Α.Κ.Π. συνόδευε μια βρεφική εσωτροπία με ή χωρίς συνύπαρξη υπερλειτουργίας των κάτω λοξών μυών. Η μετεγχειρητική παρακολούθηση διήρκεσε από 2 έως 7 έτη και πραγματοποιήθηκε σε τακτά χρονικά διαστήματα. Αποτελέσματα: Η διαφορά μεταξύ της προεγχειρητικής και της μετεγχειρητικής γωνίας ήταν στατιστικά λίαν σημαντική (t=15,77 p<0,001) όπως και η επίδραση της επέμβασης στην υπερλειτουργία του κάτω λοξού μυός (t=7,03 p<0,001).
