▼ Women with a history of childhood
sexual abuse (CSA) are at a higher risk of
sexual dysfunction than non-abused women and standard treatments for
sexual dysfunction have been less effective in abused women. Although low desire is the
sexual difficulty most commonly reported by women in general, for abused women arousal problems are most prevalent. The variation in presentation of
sexual dysfunction and treatment response for abused women suggests that the
sexual dysfunction of abused women may develop via unique pathways from that of their non-abused peers. The aim of this study was to identify mechanisms underlying the relationship between a history of CSA and
sexual dysfunction. Prior work suggests that
sexual arousal may be associated with negative affect and trauma memories for abused women, therefore I hypothesized that greater negative and shameful and less positive appraisal of genital
sexual arousal would explain the relationship between a history of CSA and
sexual dysfunction. I also tested a number of other candidate mechanisms to explain this relationship. Participants were 107 women from the local community: 61 abused women and 46 non-abused women. Participants completed a single laboratory session in which they viewed an erotic film and completed questionnaires on appraisal of genital
sexual arousal and other candidate mechanisms. Results supported the notion that the
sexual difficulties of abused women develop through distinct pathways by indicating that appraisal of genital
sexual arousal and body image focused on
sexual attractiveness explain the relationship between a history of CSA and
sexual dysfunction. Greater negative appraisal of genital arousal and one’s own
sexual attractiveness during
sexual arousal could be highly distracting to abused women trying to engage in consensual
sexual activity and suggest an extension of Barlow’s cognitive affective model of
sexual function to abused women. Although Barlow’s model helps to explain the
sexual function of women in general, the current results suggest that the content that distracts women during
sexual activity may differ by abuse history. Clinically these findings call for greater attention to negative appraisals of genital arousal and the overall body when treating
sexual dysfunction in women with a history of CSA.

▼ Recently, there has been a push for a maternal-focused model of postpartum care; however, this model of care tends to focus on postpartum depression. Two issues that are related to postpartum depression but may go unaddressed by health-care professionals are persistent postpartum pain (PPP) and postpartum sexual function. Traditionally, these issues have been investigated using a biomedical approach. With a biomedical approach, PPP is attributed to tissue damage during childbirth, and sexual problems emerge due to an overemphasis placed on the (heteronormative) functionality of the genitals. However, the biomedical model provides insufficient explanations for PPP and postpartum sexual function. Therefore, the primary goal of the current studies was to apply a multidimensional framework to the study of PPP and postpartum sexual function using comprehensive conceptualizations of “pain” and “sexual function”. A secondary goal was to elucidate the relationships among postpartum depression, PPP, and sexual function by investigating the role of potential mediating variables.
Women within one year postpartum completed an online survey. In Study 1, pain characteristics of PPP (pain persisting for two months or more) were described. Women with PPP were compared to women whose pain resolved within two months postpartum on birth-related physical and psychosocial factors. The results of Study 1 indicated that PPP is pervasive in nature and more common after a Caesarean section. Somatization (functional impairment) was the only biopsychosocial factor to significantly predict PPP, and it mediated the relationship between depressive symptoms and PPP. Study 2 examined the role of birth-related physical, psychological, and relationship variables on several facets of postpartum sexual function. In this study, fatigue and relationship satisfaction predicted sexual function as well as independently mediated the relationship between depressive symptoms and postpartum sexual function.
The results of the current studies indicate that PPP and postpartum sexual function are best addressed using a multidimensional approach, which goes well beyond the biomedical model that is so commonly used in the current literature. Furthermore, the results of these studies have significant implications for health-care providers with respect to what information should be integrated into pre- and postpartum health-care.

▼ Perhaps one of the most embodied of human experiences, sexuality can be greatly affected by the way in which women perceive their body. Historically, scholars have dedicated their attention to negative facets of body image and how it is associated with poorer female sexual health, while mostly overlooking the positive aspects of this relationship. Similarly, although ageing is a key factor to consider when examining body image and sexual health, only a dearth of studies has attempted to describe the experiences of non-university aged women. To fill these gaps, this research program was designed to investigate the associations between positive and negative aspects of body image and explore how each contributes to sexual health in age-varied samples of adult women. Two survey studies were carried out. A total of 215 heterosexual women, aged 18 to 88, participated in the first study. Despite a high statistical overlap between body appreciation (i.e., positive body image) and body dissatisfaction (i.e., negative body image), the former was found to be a greater contributing factor to indicators of sexual health. Specifically, body appreciation was related to improved sexual function, lower sexuality-related distress, and higher sexual satisfaction, even when controlling for body mass index. Although many changes occur to the body as women get older, body appreciation was unrelated to age in this sample. Nevertheless, it was shown to moderate the negative association between age and sexual satisfaction, such that older women with high appreciation for their body reported being significantly more sexually satisfied than those with low body appreciation. While the first study explored the body image and sexual health experiences of adult women in general, the second article focused on the mechanisms through which one is related to the other in midlife and older women specifically. A total of 193 heterosexual women, aged 50 to 83, completed an online survey. Support was provided for the use of objectification theory (Frederickson & Roberts, 1997), a well-established theoretical framework in body image research, in explaining sexual health in midlife and older women. Body self-consciousness during sex partially explained the relationship between body shame, appearance anxiety, and sexual function, distress, and satisfaction. High body appreciation mitigated the detrimental effect of self-objectification constructs (i.e., body surveillance, appearance anxiety) and body self-consciousness during sex on midlife and older women's sexual health. Overall, based on the results of this dissertation, body appreciation appears to serve as a protective factor for improved sexual health. ▼ Homonegativity is a multidimensional construct that encompasses the negative affective, cognitive, and behavioural responses directed towards individuals presumed, correctly or incorrectly, to be gay or lesbian. Given the affective component of homonegativity is relatively understudied compared to the cognitive and behavioural components, two studies were designed to better understand its role in the expression of homonegativity, its association with the different functions of homonegativity, and possible physiological manifestation. The purpose of Study 1 (N = 737), which involved an online questionnaire, was four-fold: (1) to examine the prevalence of homonegativity across the cognitive, affective, and behavioural domains, (2) assess the relationships between the ego-defensive
function and measures of affective reactions, (3) assess the value of the affective component of homonegativity in predicting past anti-gay behaviours, and (4) to create a sampling pool for Study 2. The majority of participants scored below the scale midpoint of the ATG and MHS-G and they most often reported engaging in subtle behaviours directed toward gay men. The self-identified heterosexual men (n = 411) reported more negative affective reactions toward gay men than the self-identified heterosexual women (n = 325) and negative affective reactions were positively correlated with the ego-defensive
function. Further, stronger negative affective reactions were the best predictors of past homonegative behaviours, compared to gender and homonegative attitudes. The purpose of Study 2 (N = 40) was to examine the physiological manifestation of homonegativity using penile plethysmography and its association to affective responses and ego-defensiveness. Genital and subjective
sexual arousal to the gay male videos did not significantly differ by level of homonegativity, affective reactions, or scores on measures of defensiveness. In sum, the affective component of homonegativity is associated with the ego-defensive
function and has a significant impact on the enactment of homonegativity, but is unrelated to differences in
sexual arousal responses. The broader clinical implications of the affective component and a proposed re-conceptualization of the ego-defensive
function as it applies to homonegativity are discussed. Limitations of the study including the lower sample scores on the attitudinal measures of homonegativity, directions for future research, and possible interventions are also presented.

▼ Introdução: Durante a transição menopausal, as mulheres podem sofrer alterações na atividade sexual, devido à diminuição dos níveis plasmáticos de estrogênio e androgênio. Além disso, fatores como a idade, o tempo de relacionamento e o aumento de morbidades que causam diminuição do bemestar podem levar à redução da atividade sexual. Objetivo: Estimar a prevalência da disfunção sexual em mulheres no período da transição menopausal e pós-menopausa e identificar potenciais fatores de risco. Método: estudo transversal que incluiu 756 mulheres de 35 a 65 anos cadastradas na Saúde da Família de Pindamonhangaba. Para avaliar a função sexual, foi utilizado o Female Sexual Function Index (FSFI), questionário composto por 19 perguntas referentes às últimas quatro semanas, que abrange seis domínios: desejo, excitação, lubrificação, satisfação, orgasmo e dor. Foi realizado um modelo de regressão múltipla de Poisson, no programa Stata 11. Resultados: A prevalência de disfunção sexual foi de 57,7 por cento (IC95 por cento : 54,0 por cento 61,4 por cento). Quando consideradas apenas as mulheres sexualmente ativas, foi de 39,8 por cento (IC95 por cento : 35,4 por cento 44,2 por cento). A disfunção sexual associou-se positivamente a idade (p<0,001), ao estado civil (p<0,001), a religião (p=0,003), a depressão (p<0,001) e a diabetes (p=0,013). Dentre as mulheres sexualmente ativas, a disfunção sexual associou-se positivamente também com a idade (p<0,001), a depressão (p<0,001) além do uso de medicamento ansiolítico (p=0,011). Conclusão: Houve alta prevalência de disfunção sexual entre as mulheres participantes do PROSAPIN, e os fatores associados foram: idade, o estado civil, a religião, a depressão, diabetes e uso de medicamento ansiolítico
Introduction: During the menopausal transition women may experience changes in sexual activity due to decreased plasma levels of estrogen and androgen. Beyond that, factors such as age, length of relationship and increased morbidities that cause decreased well-being can lead to reduced sexual activity. Objective: To estimate the prevalence of sexual dysfunction in women during the menopausal transition and postmenopausal women and to identify potential risk factors. Method: A cross-sectional study that included 756 women from 35 to 65 years indexed in the program Saúde da Família in Pindamonhangaba. To evaluate sexual function, we used the Female Sexual Function Index (FSFI) questionnaire consisting of 19 questions concerning the last four weeks, covering six domains: desire, arousal, lubrication, satisfaction, orgasm and pain. We performed a multiple regression model the Poisson in the Stata 11. Results: The prevalence of sexual dysfunction was 57.7 per cent (95 per cent CI: 54.0 per cent - 61.4 per cent). sexual function and satisfaction. It was concluded that undergoing voluntary medical male circumcision have a high degree of satisfaction with the procedure and its consequences in terms of
sexual function and satisfaction. It was recommended that men are educated about these findings so as to clear myths and misconceptions around circumcision.

▼ Injury to the spinal cord is devastating, leading to permanent, debilitating, and life altering injuries that can be fatal in some instances. Research suggests there are approximately 250,000 people living in the United States with some form of spinal cord injury (SCI) with approximately 11,000 new cases occurring annually. Men are four times more likely than women to sustain a spinal cord injury and account for 82% of those injured. Spinal cord injuries have significant impact on the individual's physical, emotional, and
sexual wellbeing. Studies evaluating
sexual activity potentiality in males with SCI date back to 1948 where Munro and associates completed extensive research on the
sexual function of 84 paraplegic men injured in World War II. Since the earliest studies, regaining
sexual function has consistently been identified as the highest priority for individuals with SCI, ranking above improving bowel and bladder
function, extremity
function, independence, and performing activities of daily living. Approximately 42% of men living with SCI are dissatisfied with their
sexual lifestyle, and nearly 50% experience feelings of
sexual inadequacy. More importantly, studies show that the vast majority of persons with SCI never discussed their
sexual concerns with health providers, and 90% of these individuals had unrealistic expectations of
sexual function and satisfaction. However, there is a paucity of research found within the literature regarding
sexual function and satisfaction in individuals with SCI. The purpose of this phenomenological inquiry was to explore, describe and gain a deeper understanding of the lived experiences of adult males regarding
sexual dysfunction and dissatisfaction following spinal cord injury. Utilizing Heidegger's seven concepts of qualitative inquiry, this research will contribute to the current literature. Purposeful sampling was used to recruit participants. A total of eight participants were recruited for this study, six of which completed the study. Face-to-face, audio-taped interviews were conducted. Finally, Diekelmann and colleagues 7-step approach was utilized to analyze the data. Seven main themes and two subthemes were identified throughout the interviews to produce the overall essence.

Sexual side effects are associated with non-compliance or discontinuance of antidepressants (Werneke, Northey, & Bhugra, 2006). Despite this, there are few empirically supported treatments for antidepressant side effects. However, in laboratory studies, exercise immediately before
sexual stimuli improved
sexual arousal of women taking antidepressants (Lorenz & Meston, 2012). I evaluated if exercise improves
sexual functioning in women experiencing antidepressant-induced
sexual side effects. Fifty-two women reporting antidepressant
sexual side effects were followed for 3 weeks of
sexual activity only. They were randomized to complete either three weeks of exercise immediately before
sexual activity (3x/week) or 3 weeks of exercise separate from
sexual activity (3x/week). At the end of the first exercise arm, participants crossed to the other. I measured
sexual functioning,
sexual satisfaction, depression and physical health. Completers showed modest improvements in
sexual functioning and satisfaction. For women taking selective serotonin and norepinephrine reuptake inhibitors, exercising immediately before
sexual activity was superior to exercise in general. As well as known effects in improved physical and psychological health, exercise may help improve
sexual health and pleasure in women taking antidepressants. These findings have important implications for public health, as exercise is accessible, cheap, and does not add to burden of care.

sexual functioning and sexually related personal distress are weakly related in women, with only a minority of
sexual difficulties resulting in significant levels of distress. However, there has been little systematic research to date on which factors moderate the relationship between
sexual functioning and
sexual distress. Our aim was to assess the degree to which relational intimacy and attachment anxiety moderate the association between
sexual functioning and
sexual distress in college-age women. Two hundred women (mean age = 20.25) completed surveys assessing
sexual functioning, relational intimacy, attachment anxiety, and
sexual distress. Relational intimacy and attachment anxiety moderated the association between multiple aspects of
sexual functioning and
sexual distress. For lubrication and
sexual pain, functioning was more strongly associated with distress in low-intimacy vs. high-intimacy relationships, but only for women with high levels of attachment anxiety. Results regarding desire were mixed and neither intimacy nor attachment anxiety interacted with subjective arousal or orgasm in predicting distress. We conclude that both relational intimacy and attachment anxiety are important moderators of the association between
sexual functioning and subjective
sexual distress in women. Theoretical and practical implications are discussed.

Abstract
Background Restorative proctocolectomy is the preferred surgical alternative for reconstruction after proctocolectomy for ulcerative colitis. The majority of patients are satisfied with the functional outcome. However, a proportion of the patients suffer from complications and impaired pouch function. Furthermore, about 10% of the patients will have a definitive failure of the pouch. The aim of this thesis was to explore some of the long-term aspects of this surgical procedure.
Methods Paper I: 42 patients were assessed with a pouch functional score and manovolumetry. The outcome after median 16 years were compared to two years after surgery in a paired analysis. Paper II: Grade of inflammation, possible dysplasia and pouch related problems were assessed in 13 patients with pouch failure and the pouch still in place but deviated with an ileostomy. Paper III: 36 patients with pouch failure were compared to 72, age and gender matched patients with functioning pouches, regarding sexual function, body image and health related quality of life. The instrument used for sexual function was the female sexual function index (FSFI) and the international index of erectile function (IIEF). Body image was assessed with the body image scale (BIS), and health related quality of life with SF-36. Swedish version. 2.0. Paper IV: is a randomized, placebo-controlled, double blind study on the effects of probiotics (Lactobacillus plantarum 299, Bifidobacterium infantis Cure21) on 31 patients with poor pouch function. Assessments were made with a pouch functional index, the pouchitis activity index (PDAI), endoscopy, histology and faecal biomarkers.
Results The pouch functional score showed impairment at 16 year as well as the manovolumetric characteristics, except for resting anal pressure. Increased age and pouch volume were correlated to a worse functional score (Paper I). The majority of patients had no problems with the defunctioned pouch and dysplasia was not found (Paper II). Patients with pouch failure demonstrated lower scores in all domains in the FSFI and IIEF, as well as lower summary score in both instruments. However, the differences were not statistically significant. BIS summary score was significantly lower for both sexes in the patients with pouch failure. All domain SF-36 scores were lower for both sexes with pouch failure, though not statistically significant (Paper III). There was no significant difference between the probiotics and placebo groups regarding pouch functional score, PDAI or faecal biomarkers after treatment. Initial values of PDAI correlated significantly to all faecal biomarkers (Paoer IV).
Conclusions A decline in pouch function at long-term, concurrent with alterations in pouch physiology as assessed with manovolumetry was demonstrated. ▼ Spinal cord injury (SCI) causes severe neurological impairment with widespread motor, autonomic and sensory deficits, leading to a substantial quality of life impairment. The number of individuals with SCI increases by approximately 12,500 annually, and over 80% of individuals with SCI are males. SCI individuals rate
sexual function as a top priority quality of life issue, and men with SCI likely suffer from
sexual dysfunction, such as erectile and ejaculatory dysfunction, as well as infertility. Regardless of the high status of importance of
sexual function, limited numbers of experimental studies in SCI animal models have focused on
sexual function after SCI. Interestingly, human clinical research participants with SCI undergoing daily locomotor treadmill training have reported changes in
sexual function. To advance targeted recovery of
sexual function after SCI, a better understanding of the post-injury neural circuitry is necessary. The objective of this project was to determine the effect of activity-based training (ABT) on
sexual function in a rat model of SCI and examine the effects of a contusion SCI on
sexual function using improved quantifiable measures. Two well-established measures of
sexual function were used to determine the effects of ABT on
sexual function after SCI: the penile dorsiflexion reflex (PDFR) and bulbospongiosus electromyography. The second and third studies focused on determining the effect of a clinically relevant contusion injury of varying severities using kinematic analysis of the PDFR and real-time intracavernosal pressure recordings from the penis of rats undergoing mating behavior testing. ABT was shown to positively affect
sexual function after SCI, where task-specific stepping and/or hindlimb loading impacts the local spinal cord circuitry responsible for
sexual function. ABT may strengthen the residual fibers crossing the lesion epicenter, providing better coordination of supraspinal influences on the spinal cord
sexual circuitry.
Sexual function after SCI was determined to be correlated with the percent of white matter spared (a measure of injury severity), further supporting the disruption of spinobulbospinal coordination as the cause of
sexual dysfunction after SCI. Kinematic analysis of the penile dorsiflexion reflex and telemetric recording of intracavernosal pressure may provide improved quantifiable measures for examining
sexual function after SCI.

▼ The objective of this research was to analyze the function of the pelvic floor (PF) and sexual function according to age and physical activity in adult women. The sample was 152 women and divided according to age - between 18 and 30 years (Age Group 1, n = 51), 31 and 50 years (Age Group 2, n = 78) and a group of 51 to 75 years (Ages Group 3, n = 23) - and physical activity level - very active women (GMMA, n = 72), slightly active women (GMPA, n = 52) and sedentary women (GMS, n = 26) - as classification of the International Physical Activity Questionnaire (IPAQ). The instruments used were: Index Disorders Identification of the PF; Physical Examination of PF; short version of IPAQ, Female Sexual Function Index (FSFI), and a range of silhouettes. We adopted a significance level of 5%. From the evaluation of the PF, it was noted that only 4.0% of women do not experience contraction objective and subjective (Grade 0). With respect to disorders of the PF, 31.3% had urinary incontinence. With regard to age categories, there is an association with urinary leakage (X2 = 18.906, p <0.001), being group 3 to greater frequency of disorders. Nevertheless, no significant difference between age and functionality of the PF (p>0.005). Regarding the analysis performed in pairs of sexual function were decreased with the increase in the median age in the final score (X2 = 11.858, p <0.003). The women of group 1 had lower BMI (X2 = 14.411, p <0.001) and perceived themselves more capable physically (X2 = 10.410, p <0.005). The level of physical activity, the presence of disorders not associated with these groups (p>0.05). The perceived physical ability (RPC) was higher among women are very active (X2= 18.613, p <0.001). There are relationships between variables and the variables RCP: Role of AP (rho = 0.212, p <0.001), Excitement (rho = 0.168, p ≤ 0.05) and satisfaction (rho = 0.219, p <0.001) the function PF was related to obstetric history: number of pregnancies (Rho = -0.184, p = 0.023) and number of normal deliveries (Rho = -0.193, p = 0.0017). The final score of the FSFI questionnaire correlated negatively with this background (Rho = -0.240, p = 0.003, and Rho = -0.179, p = 0.03, respectively). It was noted that the areas, except, orgasm and pain, are positively related to frequency of sexual practice. By performing a linear regression whose dependent variable was the final score of the FSFI and the Independent Age, Number of birth, BMI, waist circumference, the IPAQ score and RPC, presented an R = 0.220 and R2 = 0.040. The same occurred with PF function that showed an R = 0.220 and R2 = 0.04 (4%). Given these results, note that the age and obstetric history characteristics are important factors in the PF, sexual function and anthropometric indexes.

A absorção é um traço de personalidade que diz respeito à tendência para momentaneamente focar a atenção em experiências sensoriais e/ou imaginárias de tal modo que se altera a perceção de si próprio, do tempo e do espaço. Maior absorção tem-se correlacionado com maior desejo sexual em mulheres e maior frequência de relações sexuais em homens. Porém, a absorção correlaciona-se com a dissociação, que poderá ser um fator de risco para o funcionamento sexual. O objetivo do presente estudo é examinar as associações da absorção-traço com dimensões do funcionamento sexual em ambos os sexos, em correlações de ordem zero e em correlações parciais controlando a dissociação. A amostra é constituída por 319 adultos (216 mulheres e 103 homens). A absorção foi medida pelo Tellegen Absortion Scale (TAS) e pela subescala de Auto-Esquecimento do Temperament and Caracter Inventory - Revised (TCI-R); as experiências dissociativas pelo Dissociative Experiencies Scale (DES); o funcionamento sexual feminino pelo Female Sexual Function Index (FSFI) e Female Sexual Distress Scale - Revised (FSDS-R); o funcionamento sexual masculino pelo International Index of Erectile Function (IIEF) e Premature Ejaculation Diagnostic Tool (PEDT); a frequência sexual é acedida mediante a questão da frequência (em dias) das atividades sexuais no mês precedente. Nos homens, não houve correlações significativas. Nas mulheres, ambas as medidas de absorção se correlacionaram com maior desejo em correlações parciais. O auto-esquecimento correlacionou-se com maior desejo na correlação simples. O desejo sexual feminino poderá beneficiar do aumento da absorção através do desenvolvimento de terapêuticas para o efeito.
Absorption is a personality trait, which refers to the tendency to momentarily focus the attention on sensory and/or imaginary experiences, in such a fashion that the perception of oneself, time and space is altered. Higher absorption has been correlated with higher sexual desire in women and higher frequency of sexual intercourse in men. However, absorption correlates with dissociation, which could be a risk factor for sexual function. The objective of the present study is to examine the associations of trait absorption with dimensions of sexual functioning in both sexes, in zero order correlations and in partial correlation controlling for dissociation. The sample consists of 319 adults (216 women and 103 men). we perform.OSA has an estimated prevalence of 2 % in women and 4 %
in male and is characterized by repetitive complete (apnoea) or
partial (hypopnoea) cessation of breathing. I wondered how the
patients managed sexuality and closeness when suffering from OSA
and in particular how the Continuous Positive Airways Pressure
(CPAP) treatment affected their sexuality and closeness. A review
of the literature showed there was little published information
concerning the impact of CPAP treatment on sexuality; why it was
difficult to advise the patients on this matter and how to predict
whether closeness to one's partner would be reduced or improved
when treated with CPAP. Furthermore; in order to guide and inform a
patient as to how CPAP treatment may affect sexuality you have to
know about sexual function and dysfunction as well as the
difference between women and men. The overall aim of this thesis
was to study the impact of nocturnal CPAP treatment on sexuality
and closeness in patients with obstructive sleep apnoea. Patients
with a diagnosis of OSA were consecutively recruited. After
recruitment; 308 men and 92 women were included and after one year
150 men and 44 women remained in the study. Data was collected by
self-reported questionnaires; the Epworth Sleepiness Scale and the
Life Satisfaction 11. The specific sexual questionnaires for women
were the Female Sexual Function Index and the Female Sexual
Distress Scale. Manifest Female Sexual Dysfunction is present when
the respondent has both sexual difficulty and sexual distress. The
specific sexual questionnaire for men was the Brief Sexual Function
Inventory. Male. Satisfaction with Sexual Life (Li-Sat 11) and both
general and functional aspects of sexuality (BSFI) were
significantly improved after 1 year of CPAP treatment. Female.
Manifest Female Sexual Dysfunction was significantly improved after
one year of CPAP treatment; but no significant changes in isolated
sexual difficulties or sexual distress were found. Daytime
sleepiness (ESS) decreased significantly in both female and male
patients after 1 year of CPAP treatment. Daytime sleepiness
decreased significantly in both male and female; indicating a
positive effect of CPAP treatment. Sexuality improved both in
general and specific aspects in male patients and female patients
had a decrease in sexual dysfunction. Many patients may be
reluctant to use a treatment involving the use of a mask; a tube
and machine every night for many years to come. However; one of the
main findings is that CPAP treatment per se does not negatively
affect family life or partner relationship. I consider this
important information when initiating CPAP
treatment.

Amostragem: Mulheres que tiveram seus filhos nas maternidades de Pelotas de setembro de 2002 a novembro de 2003. População: De 2741 parturientes no período perinatal, obteve-se uma amostra aleatória de 920 mulheres aos 180 dias após o nascimento. Métodos: Realizadas análise uni e multivariada e somente as variáveis com p<0,05 foram consideradas associadas ao desfecho de forma estatisticamente significativa. Principais medidas: Razão de prevalência com intervalo de confiança. Resultados: A prevalência de dificuldade no retorno à atividade sexual foi de 25,5% e 80% das mulheres retomaram a vida sexual nos primeiros dois meses após o parto. O principal motivo para não ter relação sexual foi o fato de a mulher não ter companheiro (63%); as demais causas foram a falta de vontade ou de tempo. As primigestas e as mulheres com problemas de saúde no pré ou no pós-natal, apresentaram uma prevalência mais elevada de dificuldades: dores pélvicas, insegurança ou medo e falta de desejo. Conclusão: Encontrou-se uma alta prevalência de dificuldades em retomar as atividades sexuais após o nascimento. Foi um estudo realizado numa cidade do sul do Brasil e, por isso, os achados estão contextualizados à cultura e aos hábitos destas mulheres. Aim: Evaluating the time and problems met at resuming sexual intercourse after childbirth concerning women in Pelotas. Outlinig: Prospective Cohort Study. Sampling: Women who had their children at maternity hospitals in Pelotas, from September of 2002 to November of 2003. Population: From 2,741 parturients at perinatal period one obtained a random sample of 920 women on their 180 days after childbirth. Methods: Uni and multivaried analyses were performed and only those variables with a p<0.05 were considered as associated to the outcome with a significant statistically way. Prime Measures: Prevalence reason with a confidence interval. Results: The difficulty prevalence at resuming sexual intercourse was 25.5%, and 80% of the women resumed sexual intercourse on the first six months after deliverance. The main cause for not having sexual intercourse was for the woman not having a mate (63%); other causes were the lack of desire or time. The primiparas and women with a health problem on the prenatal or postnatal presented a higher prevalence difficulty: pelvic pains, insecurity and lack of desire. Conclusion: One found a high prevalence difficulty upon resuming sexual intercourse after deliverance. ▼ This project analyzes the sexual dimorphism of the femur and tibia using geometric morphometrics. The study sample includes 250 individuals of known sex and age at death with complete, non-damaged, non-pathological skeletal remains from the William M. Bass Donated Skeletal Collection at the University of Tennessee, Knoxville. Ages range from 19-96 for males (mean=56.92 years) and 29-97 for females (mean=59.48 years). A combination of landmarks and semi-landmarks were collected on the proximal and distal epiphyses of each bone using a Microscribe, which helps capture the overall size and shape variation present in the sample. Only individuals from one population, White, where analyzed in order to eliminate population variation bias. Classification rates for males and females for the proximal femur were 80.8% and 78.4% respectively, for the distal femur 92.6% and 89.6% respectively, for the proximal tibia 80.8% and 83.2% respectively, and the distal tibia 81.6% and 80.8% respectively, all with a p<0.0001. These rates created a classification model for which epiphysis gave the most accurate assessment of sex: the distal femur, followed by the proximal tibia, then the distal tibia, and lastly the proximal femur. This study indicates the knee joint is the most dimorphic, followed by the ankle and then the hip. The results fall in line with another study indicating the knee is more sexually dimorphic in a modern White population (Spradley and Jantz 2011), though in contrast to their results this study found the distal femur was more dimorphic than the proximal tibia. This method indicates that in comparison to standard measurements, geometric morphometrics may provide a more reliable method for sex estimation when used, specifically on the knee. Certain landmarks were then selected based on the standard taphonomic process of coffin wear and postmortem damage (Pokines and Baker 2014) for exclusion to determine the usability of the method on fragmented or damaged skeletal remains. When combinations of landmarks were removed, the distal femur still possessed the highest classification rates with over 80% accuracy.

Adolescent and young adult cancer survivors constitute a group in society of which the numbers are increasing. To optimize rehabilitation efforts, we need to understand the possible physical and psychological consequences of cancer treatment for adolescent and young adult cancer survivors. The purpose of this study has been to identify the needs and topics that adolescents and young adults acknowledge as being important to them, and furthermore to explore possible physical and psychological complications post cancer treatment.
Methods
This thesis uses a combination of qualitative and quantitative methods. Eleven focus group interviews were conducted with adolescent and young adult cancer survivors aged 15-29. Subsequently, a study-specific web-based questionnaire was sent to a population-based cohort of adolescent and young adult cancer survivor and matched controls. 540 out of 721 (74%) eligible participants responded to the questionnaire. The topics covered in the questionnaire were psychosocial health, body image, sexuality, fertility, education, work and leisure.
Results
Adolescent and young adult cancer survivors reported in focus group interviews that they had unmet needs during and after cancer treatment, including age-appropriate information about sexuality. In the questionnaire study, they reported feeling less attractive than controls due to scars on their bodies, and they reported low satisfaction with their sexual function compared to controls. The participants did not find that questions addressing very personal and emotionally charged issues, like sexual activity, affected them negatively.
Conclusions
Adolescent and young adult cancer survivors perceived themselves as being unattractive due to scars on their bodies and were less satisfied with their sexual function than matched population-based controls. They also expressed the need for support to reintegrate into normal social life.

▼ Background:la stretta relazione anatomica tra il mesoretto e le strutture nervose che attraversano la pelvi comporta un rischio elevato di lesioni accidentali del plesso ipogastrico in corso di TME, di conseguenza vi è un¿altra frequenza di disfunzioni genito urinarie dopo chirurgia rettale.
La chirurgia robotica offre al chirurgo una visione tridimensionale, stabile e ad alta definizione del campo operatorio e consente un controllo molto più accurato degli strumenti che sono dotati di sette gradi di libertà, di un sistema di soppressione del tremore spontaneo e della possibilità di demoltiplicazione del movimento.
La finalità del nostro studio è stata quella di valutare l¿impatto della chirurgia robotica sulla preservazione della funzionalità genito urinaria nei pazienti sottoposti a resezione anteriore del retto.
Materiali e metodi: nel periodo compreso tra Gennaio 2007 e Dicembre 2010 nella Divisione di Chirurgia Addomino-Pelvica dell¿Istituto Europeo di Oncologia sono stati inseriti nello studio 74 pazienti consecutivi sottoposti a resezione completamente robotizzata per tumore del retto. Per analizzare la funzionalità sessuale maschile sono stati adottati i questionari IIEF (International Index of Erectile Function), per quella femminile i questionari dell¿ FSFI (Female Sexual Function Index)
Risultati: nel nostro studio non abbiamo riscontrato alcuna differenza, per entrambi I sessi, in termini di funzionalità urinaria e di QoL a un anno dall¿intervento. Per quanto riguarda la funzionalità sessuale abbiamo registrato una riduzione di tutte le variabili analizzate dai questionari a un mese dall¿intervento. A partire da sei mesi dopo la chirurgia i punteggi calcolati per ogni item sono progressivamente aumentati fino a tornare paragonabili a quelli pre-operatori.
Conclusioni: questo studio conferma che la TME nerve sparing eseguita con tecnica robotica è efficace e adeguata per il trattamento del cancro rettale e consente una migliore preservazione della funzionalità genitourinaria in comparazione con le tecniche chirurgiche classiche: open e laparoscopica.

sexual dysfunction within the population of English and Spanish speaking women visiting outpatient clinics and to compare the rates of patients discussing symptoms of painful sex with healthcare providers between language groups. Design: Cross-sectional survey, descriptive study Setting: The University of Kansas Obstetrics and Gynecology Clinic and affiliated Jay Doc Free Health Clinic Patients: Convenience sample of English and Spanish speaking women, ages 18-45 Methods: Subjects completed anonymous surveys in either English or Spanish. The surveys were administered through REDCap and included the validated questionnaires for the Visual Analog Scales (VAS) for pain, Female
Sexual Function Index (FSFI) and Patient Global Impression of Improvement (PGI-I). Data on demographics and discussion of pain with healthcare providers was also collected. The prevalence of
sexual dysfunction and dyspareunia was calculated for each cohort and the rate of discussion of dyspareunia was identified for each language. Prevalence, FSFI domain scores and discussion rates were compared between groups using t-test and chi-square analysis. Main Result: A total of 160 women were surveyed (107 English speaking and 53 Spanish speaking). The prevalence of dyspareunia was found to be 42.06% and 28.30% in the English and Spanish cohorts respectively (p=0.087) and the rates of discussion of dyspareunia was significantly higher in the English cohort compared to the Spanish (20.56% vs 5.66%, p=0.011).The prevalence of
sexual dysfunction, based on an FSFI score <26.55, was found to be 46.73% and 62.26% in English and Spanish cohorts respectively (p=0.048). When comparing the individual FSFI domains, scores for desires, arousal, lubrication, orgasm and pain were significantly higher in the English cohort (p<0.05). Conclusion: Our main outcome measures show a significantly higher prevalence of dyspareunia and rate of discussion in the English cohort compared to the Spanish cohort. The FSFI score and 5 out of 6 of individual domain scores were significantly higher in the English cohort. A significantly larger portion of the Spanish speakers scored below 26.55, signifying
sexual dysfunction. Our measurements for
sexual dysfunction and dyspareunia prevalence are higher than estimates found in literature. We encourage continuing research to obtain a larger sample size for expanded analysis. The differences in Spanish speakers low FSFI scores compared to their lower prevalence of subjective dyspareunia requires more investigation. Future studies should investigate specific barriers in communication between Spanish speakers and clinicians when discussing

sexual pain, inability to reach orgasm, and low satisfaction. Worse
sexual function is associated with disordered eating behavior; however, the mechanism through which this association exists remains unclear. Theory of
sexual function suggests that depression mediates the association of disordered eating behaviors and poor
sexual function in female service members and veterans, but this has yet to be tested empirically. The purpose of the current study was to examine whether depression mediated the association of disordered eating behaviors and
sexual dysfunction.
Participants (
N=511) were partnered females who completed measures of
sexual function, disordered eating behavior, depression severity, and demographic and military characteristics.
Nearly 60% reported probable
sexual dysfunction, 19.6% reported probable eating disorder, and 44.4% reported probable depressive disorder. Higher depression symptoms were associated with lower
sexual function (r=-.40, p<.001) and higher disordered eating behavior (r=.45, p<.001). Mediation analyses revealed that the association between disordered eating behavior and
sexual function was indirect, through depression severity (indirect effect: -1.12, 95% confidence interval [-1.42, -0.85]).
Depression is a potential mechanism through which disordered eating behaviors are related to poor
sexual function. Findings are congruent with
sexual function theory, and underscore the importance of addressing depression when treating
sexual function in those with disordered eating behavior.

▼ The estimated lifetime prevalence of
sexual assault (SA) ranges from 9% to 47% depending on the sample and definitions of assault used by researchers. Victims are likely to experience other mental health related disorders. Stigma and negative social reactions associated with the disclosure of SA are critical factors related to the development of pathology for survivors of SA and have been linked to the incidence of psychological disorders like Posttraumatic Stress Disorder (PTSD). Recent research has suggested that shame may play a significant role in the development and maintenance of PTSD by interfering with the cognitive processing of the trauma as well as influencing behavior, such as social withdrawal and other avoidant coping strategies that maintain and possibly cause PTSD symptoms. Emotion research in social psychology and sociobiology suggests that the expression of shame should garner support and sympathy from others, thereby serving an adaptive and useful purpose for the survivor. This information is not congruent with research suggesting the experience of shame has a maladaptive impact for survivors of SA, suggesting the experience of shame does not garner support from others, or that the context of
sexual assault may interfere with the proposed functions of expressions of shame. The purpose of this study was to experimentally assess social responses to expressions of shame within the context of
sexual assault to explain a potential contributing factor in the stigmatization and negative social support often reported by SA victims. A college student sample was asked to view a series of 90-second video clips. Participants then answered questions to assess their perceptions and attributions regarding the individuals in the videos. Results suggest perceptions of adjustment were impacted by facial expressions of shame. In addition, facial expressions of shame elicited sadness/pity. Context was also a significant factor as participants generally attributed less culpability to individuals who were assaulted.

▼ Myocardial infarction (MI) negatively impacts many aspects of a patient’s quality of life, one of which is
sexual function. Though
sexual function is a crucial component of quality of life for many individuals, it is critically understudied in the post-MI population. The lack of evidence has left health care providers and patients unsure on how
sexual function components change when facing an acute or chronic illness, and what options are available to assist with maintaining optimal
sexual function. Identification of modifiable predictors of both increased and decreased
function is paramount before effective interventions can be developed. Therefore, the purpose of this dissertation was to illuminate the changes in
sexual function over time and apply exploratory modeling to examine potential predictors of
function in patients post-MI. The specific aims of this dissertation were three-fold including:
1. Describe
sexual function and satisfaction in a population of adults within two weeks and at three months post-MI, and examine changes from two weeks post-MI to three months post-MI.
2. Determine the impact of depression, fear, anxiety, and use of coping strategies on predicting
sexual function, satisfaction, and activity at three months post-MI.
3. Describe the relationships between the biopsychosocial domains of
sexual function and satisfaction at two weeks post-MI.
A total of 99 patients in a Midwestern hospital were approached out of 395 screened. Of these, 63 provided informed consent (consent/decline rate 63.6%). Participants were mailed surveys at two weeks and three months following discharge from the hospital for an MI.
Sexual function data was captured with a multidimensional instrument, the Female/Male
Sexual Function Index. Additionally, anxiety, depression, coping strategies, and
sexual fear data were gathered using validated survey instruments. Cross-sectional and longitudinal description were performed, and linear and logistic regressions were utilized to examine associations with
function and activity at three months. Eighteen participants returned all survey data (response rate 33%). There were three major findings from this dissertation. The first finding was that physical components of
function (i.e. erection, orgasm), were poorly correlated with psychosocial components (i.e.
sexual satisfaction) at both two weeks and three months post discharge for MI. The second finding was that
sexual function scores improved among male participants within the first few months of discharge (18.8%), particularly among those who were sexually active at three months. The third finding was that
sexual function was negatively associated with
sexual fear, and positively associated with utilization of problem-solving and support-seeking coping strategies at three months. Furthermore, problem-solving and support-seeking strategies revealed a statistically significant association with

▼ Introdução - A disfunção sexual é uma queixa comum, porém, ainda pouco valorizada no âmbito da saúde pública, acometendo indistintamente homens e mulheres e com potenciais reflexos negativos na sua qualidade de vida e bem-estar. Objetivo - Avaliar a prevalência de disfunções sexuais e seus possíveis fatores associados entre mulheres de meia-idade residentes na Região Norte do Brasil. Métodos - Estudo transversal e prospectivo, envolvendo 1.415 mulheres entre 35 e 65 anos atendidas no Ambulatório de Ginecologia do Hospital das Clínicas do município de Rio Branco - Acre - Brasil. Para avaliar a sintomatologia menopausal e a sua função sexual, foram aplicados a Escala de Classificação da Menopausa e o Índice da Função Sexual Feminina. Na análise dos dados, usou-se o pacote estatístico Stata 10, aceitando-se um nível de significância de 5 por cento . Na análise de proporções, usou-se o teste não paramétrico do Qui-Quadrado de Pearson. No estudo dos fatores associados às disfunções sexuais, recorreu-se à análise multivariada através de regressão logística múltipla. Resultados - A média etária das mulheres estudadas foi de 47,7 (+8,5) anos. A sua maioria era de baixa escolaridade (6,4+4,6) anos completos de estudo. A menarca, em média, foi aos 13,4 (+1,6) anos. A maioria referiu gestações anteriores (4,6+2,8). Cerca de 35,9 por cento eram pós-menopáusicas, tendo a menopausa ocorrido ao redor dos 48,3 (+4,9) anos. A autopercepção de saúde foi considerada pelas entrevistadas ruim/muito ruim em 54,6 por cento . A irritabilidade foi a queixa mais frequente (78,3 por cento ), seguida pelos problemas osteoarticulares (74,8 por cento ) e ansiedade (72,7 por cento ). A prevalência de disfunção sexual foi de 62,3 por cento . Ajustados os possíveis fatores de confusão, mostraram-se associados a disfunção sexual: baixa escolaridade (OR:1,70; [IC 95 por cento :1,31-2,19]; p<0,001); sedentarismo (OR:1,73; [IC 95 por cento :1,23-2,42]; p=0,001); autopercepção de saúde ruim/muito ruim (OR:1,99; [IC 95 por cento :1,55-2,57]; p<0,001); estado de ânimo depressivo (OR:1,16; [IC 95 por cento :1,05-1,27]; p=0,002); problemas sexuais (OR:2,50; [IC 95 por cento :1,96-3,20]; p<0,001); ressecamento vaginal (OR:1,49; [IC 95 por cento :1,33-1,66]; p<0,001) e a fase de pós-menopausa (OR:1,82; [IC 95 por cento :1,39-2,38]; p<0,001). Conclusão - Entre a população de mulheres da Região Norte Brasileira estudada, a prevalência de disfunção sexual encontrada foi elevada e a análise dos possíveis fatores associados a sua ocorrência revelaram a influência das condições socioeconômicas, estilo de vida, além da sintomatologia e do estado menopausal, desvelando uma multidimensionalidade de aspectos biológicos e não biológicos envolvidos na sua gênese.

▼ A vida sexual no pós-parto é influenciada pelas mudanças anatômicas, hormonais, da estrutura familiar e do relacionamento com o parceiro. A redução do desejo e o medo do retorno da atividade sexual são comuns. A episiotomia parece ter impacto negativo e interferir na função sexual das mulheres neste período. Objetivo: Identificar a existência de evidências sobre o efeito da episiotomia na função sexual da mulher no pós-parto. Método: Revisão Sistemática realizada por meio da metodologia do Instituto Joanna Briggs (JBI). A pergunta desta revisão sistemática foi: a episiotomia interfere na função sexual da mulher nos 24 meses após o parto? A estratégia PICOs para a revisão foi a seguinte: P (participantes): mulheres até 24 meses após o parto vaginal de todas as paridades;I (intervenção): mulheres que tiveram parto normal com episiotomia; C (controles): mulheres que tiveram parto normal e não receberam episiotomia; O (resultados): função sexual avaliada por meio de da pontuação do Female Sexual Function Index (FSFI), tempo de retorno à atividade sexual e dispareunia;S (estudos): estudos observacionais, de coorte e transversais. Foram utilizados os descritores e as estratégias de busca específicas para cada uma das bases de dados a seguir: CINAHL, Embase, LILACS, Proquest, PubMed, ScienceDirect, Scopus e Web of Science. Os artigos selecionados segundo a estratégia PICOs foram submetidos aos critérios de análise crítica de qualidade e seus dados foram extraídos com o auxílio do JBI Meta Analysis of Statistics Assessment and Review Instrument (MAStARI), para estudos quantitativos. Cada artigo foi avaliado por dois revisores. Após a extração dos dados dos estudos os resultados foram apresentados em forma narrativa pois todas as metanalises realizadas apresentaram p<0,05 para o teste de heterogeneidade. Resultados: 784 publicações foram identificadas, 11 foram submetidas à avaliação critica, e seis estudos foram incluídos na revisão sistemática. Os dados dos estudosnão apresentaram diferença estatística significativa na função sexual com seis semanas, três e seis meses entre os grupos, o tempo de retorno à atividade sexual foi menor para mulheres que tiveram parto vaginal sem episiotomia em comparação com as que tiveram episiotomia em três estudos.Adispareunia foi analisada em apenas um estudo e apresentou maior proporção em mulheres com episiotomia, porém, sem diferença estatística. Conclusão: A realização da episiotomia pode retardar a retomada da atividade sexual no pós-parto. Os estudos com abordagem quantitativa não apresentaram evidências sobre o efeito da episiotomia na função sexual mensurada por meio do FSFI e na ocorrência de dispareunia. Sugere-se a realização de revisão de estudos qualitativos para se estudar a influência da episiotomia no pós-parto a partir das narrativas das mulheres.

▼ Abstract: Introduction: Severe maternal morbidity and maternal near miss currently are better health indicators than maternal mortality ratio. Together with the identification of women who died during pregnancy and/or childbirth, the new concept allows also to investigate the prevalence of potential life-threatening conditions. However, little is known about possible long-term consequences after those episodes over several aspects of the lives of survivors. It has already been described that uncomplicated pregnancy and childbirth might modify female
sexual response. Notwithstanding, only few studies have evaluated aspects of sexuality of women after episodes of severe maternal morbidity. Objectives: To perform a systematic review of aspects of sexuality, including
sexual function, in women who had had any kind of complication during pregnancy or childbirth. To evaluate aspects of female
sexual response in women with and without severe maternal morbidity. Methods: Investigation included a systematic review through the databases PubMed, EMBASE, and SciELO, assessing general and severe maternal morbidity associated with altered female
sexual response. The review followed the protocol method proposed for observational studies (PRISMA). The female
sexual response has been studied as one of the outcomes at a retrospective cohort study on maternal severe morbidity (COMMAG). The Female
Sexual Function Index questionnaire (FSFI) was applied at exposed women (severe morbidity) and unexposed (pregnancy without complications). Along with FSFI, the survey included also questions on general and reproductive health. Results: Major perineal injuries (3rd and 4th degree) were evaluated as general morbidity outcomes at 12 studies, and severe maternal morbidity was analyzed at 2 studies. Compared to control group, both women exposed to general and severe morbidity delayed resumption of
sexual activity after childbirth. The exposed group had also more frequently dyspareunia after childbirth. The mean total FSFI scores were similar at both groups. The heterogeneity of the studies allowed only a qualitative synthesis, and meta-analysis was not feasible. To assess female
sexual response at the cohort study, 638 women who delivered at UNICAMP's maternity unit were included. 315 of them were severe maternal morbidity cases, and 323 were women who had had uncomplicated pregnancy or childbirth. The mean total scores of FSFI were similar in both groups, though below cut-off values for suspected dysfunction. Women after severe maternal morbidity resumed
sexual activity after birth later, when compared to control group. However, there was no significant difference at three months. Multivariate analysis showed association of lower FSFI scores with maternal low maternal weight and no partner. Conclusions: Altered female
sexual response might be a long-term consequence after episodes of severe maternal morbidity. Since there is a growing population of women who survive these episodes, proper evaluation of

A literatura aponta para associações entre maior alexitimia, menor interoceção e problemas no funcionamento sexual. No entanto, a investigação é escassa e dada a diversidade de conceitos e medidas utilizadas, pouco conclusiva. Este estudo visa explorar as intercorrelações entre a alexitimia, interoceção, e algumas dimensões do funcionamento sexual como o desejo, excitação, lubrificação, função erétil, orgasmo, satisfação, dor, distress e frequência de atividade sexual. O estudo conta com uma amostra de 340 indivíduos (228 mulheres e 112 homens), e uma subamostra constituída pelos indivíduos com relações sexuais com o sexo oposto nas últimas 4 semanas perfazendo um total de 244 indivíduos (152 mulheres e 88 homens). Utilizou-se um Questionário Sociodemográfico, a Escala de Alexitimia de Toronto (TAS-20), a Escala de Avaliação Multidimensional da Consciência Interocetiva (MAIA), um Questionário de Frequências Sexuais, o Índice de Funcionamento Sexual Feminino (FSFI), a Escala de Distress Sexual Feminino - Revisto (FSDS-R), o Índice Internacional de Função Erétil (IIEF) e a Ferramenta de Diagnóstico de Ejaculação Precoce (PEDT). Foram encontradas correlações entre maior alexitimia e menor interoceção. Maior alexitimia e menor interoceção correlacionaram-se com mais dificuldades em todas as dimensões do funcionamento sexual feminino avaliadas, com exceção da alexitimia com o desejo. Nos homens, menor interoceção correlacionou-se com mais sintomas de ejaculação precoce e maior alexitimia correlacionou-se com mais sintomas de ejaculação precoce e mais dificuldades na ereção. O presente estudo corrobora a noção de que a consciência das emoções e das sensações corporais internas é um fator importante no funcionamento sexual. Limitações e implicações clínicas são discutidas.
The literature shows an association between greater alexithymia, lesser interoception and more impaired sexual function. However, the investigation is limited, and given the diversity of concepts and measures, inconclusive. This study aims to explore the intercorrelations between alexithymia, interoception and several dimensions of sexual function, namely desire, arousal, lubrication, erectile function, orgasm, satisfaction, pain, distress and frequency of sexual activity. ▼ This research explores factors that influence women’s attitudes and experiences regarding female genital cosmetic surgery, including female genital image and related areas of body image, in the Australian context. The research identifies two dimensions of female genital image and explores their relationships to related areas of body image and women’s attitudes and experiences regarding eight types of female genital cosmetic surgery The sample consisted of 592 sexually active Australian women (18 to 71 years). Participants completed an anonymous Internet-based survey and completed a series of questionnaires. This was a quantitative correlational study that used a cross-sectional survey design. Results indicate that, in general, sexually active Australian women have moderate levels of genital satisfaction, moderately high positive genital self-image, moderate levels of body satisfaction and most women did not feel self-conscious about their body in the sexual settings. Results further indicate that while many sexually active Australian women approve of other women undergoing genital cosmetic surgery, most would not consider undergoing genital cosmetic surgery themselves. The study found a very small percentage of women in the research sample had undergone some type of cosmetic surgery on their genitalia. Bivariate analysis found that women who desire at least one type of female genital cosmetic surgery tend to have genital dissatisfaction and negative genital self-image. Multiple regressions analysis found that body image during sexual activity statistically predicts genital satisfaction; that body image during sexual activity and the sexual functioning domain of desire statistically predict genital self-image; and body image during sexual activity, the sexual functioning domains of lubrication and desire, and genital self-image statistically predict women’s attitudes towards female genital cosmetic surgery.

▼ Sex, intimacy and partner communication after cancer
treatment are understudied aspects of survivorship for women, yet
integral to quality of life and relationships. Self-efficacy is an
important personal state variable that is potentially modifiable by
interventions to enhance women’s adjustment after cancer treatment,
and is therefore important to measure in interventions to improve
sexual wellbeing. There is, however, no publicly-available
validated standardized measure to evaluate women’s self-efficacy
related to sex and intimacy in the context of cancer treatment
impact for use in research and clinical practice. The purpose of
this study was to develop the Self-Efficacy to Communicate about
Sex and Intimacy (SECSI) scale and to examine its psychometric
performance in women treated for cancer. A mixed-methods approach
with two sequential phases was used to refine (Phase I; Aim 1) and
to test (Phase II; Aims 2 and 3) the SECSI scale. In Phase I, 20
female cancer survivors provided iterative qualitative feedback,
through cognitive interviews, on 25 draft candidate items included
in the initial draft instrument. Cognitive interview feedback was
coded and incorporated to further refine the draft items in the
SECSI scale, including revisions to clarify phrasing, and to remove
problematic items. The main result from Phase I was the generation
of a final draft version of a 10-item SECSI scale intended to
evaluate women’s confidence in their ability to communicate with
their partner about changes in sex and intimacy after cancer
treatment. In Phase II, quantitative methods were used to evaluate
the psychometric performance of the SECSI scale in a sample of 226
partnered women treated for cancer. Phase II participants completed
an online survey that included the SECSI scale and
sociodemographic, clinical, and cancer characteristics. A SECSI
scale retest survey was administered two weeks later. The SECSI
scale was reliable, with a Cronbach coefficient alpha of 0.94, and
high 2-week test-retest reliability, r = 0.82, p<.001. Construct
validity of the SECSI scale was supported by results consistent
with hypotheses for discriminant, convergent, and divergent
validity. Factor analysis results were consistent with either a two
or one factor (subscale) solution, with relatively best theoretical
support for two subscales related to communication about impact of
illness, and general intimacy. In conclusion, Phase I of this study
established content validity for the final 10-item draft version of
the Self-Efficacy to Communicate about Sex and Intimacy scale. In
Phase II, the reliability and validity of the newly-developed SECSI
scale was established in a sample of partnered women treated for
cancer. The SECSI scale should be integrated into future research
and clinical practice interventions targeted to improving women’s
sexual wellbeing and partner communication after cancer treatment.

clinically relevant cutpoints for SECSI scale scores that would

▼ The aim of this thesis is the endoscopic TEP hernia repair, which is an appealing technique for patients with inguinal hernias. TEP, offering an adequate intraoperative diagnosis and subsequent treatment of all hernia subtypes is in experienced hands associated with low recurrence rates and rates of (chronic) postoperative pain. The supposedly substantial learning curve remains its most important disadvantage, emphasizing the need to have TEP performed by experienced surgeons.
Limited and conflicting data are available on the extent of the learning curve. In Chapter 2 of this thesis, the relation between (surgeon) expertise and operative time, perioperative complications, conversion rate and recurrences was studied, in order to define the end of the learning curve of TEP. It was observed that during the first 50-100 TEP repairs, outcomes optimized in terms of intraoperative complications and recurrences, while a decline in the conversion rate, incidence of short-term postoperative complications and operative time was still observed after more than 400 individually performed TEP procedures. In addition, the overall perioperative complication rate and the incidence of chornic pain compared favorably to these outcomes reported by others. Since the setting of this thesis is a high volume TEP clinic, these outcomes also imply relevance of “concentration of care”.
During the learning curve of TEP, the risk of serious complications is higher and operative times are longer. Perioperative difficulties encountered during this period may be reason for the surgeon to abandon this operative technique. In Chapter 3, patient- and hernia related characteristics associated with perioperative complications and operative time were studied. It was observed that the surgeon gaining experience with TEP repair should select young and slender male patients with a unilateral inguinal hernia and no previous abdominal surgery, in order to best overcome the learning curve.
In Chapter 4 the outcomes of TEP hernia repair in a subset of female patients were studied. It showed that femoral hernias are very common in women, but that preoperative diagnosis of the correct hernia type is difficult. A TEP repair offers the opportunity to diagnose all hernia types intraoperatively and to treat them appropriately with excellent outcomes.
The second part of this thesis focuses on several longterm outcomes after TEP. Reported incidences of debilitating chronic pain vary between 2-5% after endoscopic hernia repair, hence significantly lower compared to pain rates of 11-21% after conventional open hernia repair. Chapter 5 describes pain after TEP being selflimiting within the first postoperative year, with only 1% of the patients reporting substantial pain. The incidence of impaired
sexual function due to pain also decreased from to 1% postoperatively.

▼ Sex determination of human skeletal remains is of fundamental importance in many bioarchaeological and medicolegal investigations. In the present study, a method based on discriminant
function analysis of metric data from the foramen magnum, occipital condyles, intercondylar dimensions and mandibular canines is proposed to determine the sex of sub-adult and adult skeletons. These elements are thought to be
sexual dimorphic in previous studies and are fully developed prior to puberty. Therefore they are selected for this method. The combination of measurements of these elements has not been tested before. The use of discriminant
function analyses based on multiple measurements was beforehand expected to reach higher accuracies than independent measurements.
The proposed method is tested on the nineteenth century Middenbeemster collection to determine its reproducibility, suitability and accuracy. The accuracy rates this study was aiming for was a minimum of 75-80%. The Middenbeemster skeletal collection was suitable for this study because skeletons could be linked to the archive of the cemetery. Therefore, of some skeletons age and sex were known, which was required for this study. The known age and sex skeletons were divided into a sub-adult and an adult sample.
The results for the adult sample showed statistical significant difference between the sexes in measurements of the foramen magnum width, right occipital condyle length, maximum bicondylar breadth and minimum interior distance between the condyles. Also, all permanent mandibular canine measurements were significantly
sexual dimorphic. The sub-adult sample showed no significant
sexual dimorphism in the cranial base elements or the deciduous mandibular canines. Of the permanent mandibular canines three out of four measurements turned out to be statistically significantly different.
Of the single-measurements methods, eight out of twelve measurements in of the permanent and deciduous canines reached the minimum success rate of 75- 80%, which makes them likely to be suitable for sex determination in adults as well as sub-adults. The measurements cranial base elements provided accuracy rates between 52.0% and 71.7% in the adult sample and 47.1% and 73.3% in the sub-adult sample. These were all lower than the minimum accuracy of 75-80% this study was aiming for and are therefore considered as too low to be acceptable as method for sex determination.
The discriminant
function analyses based on multiple measurements provided higher overall accuracies in both samples. Especially the combinations including permanent mandibular canine measurements reached high overall success rates, up to 100%. Combining all cranial base and permanent mandibular canine measurements reached an overall accuracy of 100% in the adult sample and 86.7% in the sub-adult sample, which are successful results when looking at the aim of the study.
