▼ Background: Universal screening is a critical component within a multi-tiered system of supports because it is the first mechanism by which students are identified as at-risk for failure. Screeners must have adequate psychometric properties to accurately identify students at-risk, however many measures result in moderate accuracy, indicating the presence of false negatives and false positives. These errors may affect the allocation of resources in schools, sometimes causing resources to be utilized unnecessarily and other times withholding services when they are needed. Purpose: This study examined the validity and diagnostic accuracy of a reading CAT using single time points and slopes within year, and longitudinally across one and two years. Methods: Screening and state test data were analyzed for students in third through fifth grade (N = 1696). Validity statistics were calculated using single time points and slopes on the CAT. Diagnostic accuracy analyses included the use of ROC curves to identify the CAT cut score associated with maximized sensitivity and specificity, and the cut score at the 25th and 40th percentile. Results: Correlation results for single screener points and the state test ranged from .60 to .79 (p = .0001). Correlation between slopes and the state test were weak for third and fourth grade (.21 and .14, respectively, p = .0001) and not significant for fifth grade. When ROC curves were used to identify the cut score associated with maximized sensitivity and specificity, overall accuracy was high for single points and moderate for slopes. Accuracy remained high for the cut score at the 25th and 40th percentile, though sensitivity levels were lower for the 25th percentile. Slopes yielded inadequate levels of sensitivity. Conclusion: Results suggest CATs may address issues of inadequate levels of diagnostic accuracy found in the screening literature but additional research is needed in samples with different base rates. ▼ In Mediterranean countries, sheep and goat’s milk production has traditionally been destined for the manufacture of cheese, often as raw milk. Cheese quality is closely related to milk composition but also to hygienic aspects such as somatic cell count, bacteriology or presence of antibiotic residues, currently regulated by European legislation. The implications of the presence of antibiotic residues in milk as a result of veterinary treatments include negative effects on consumer’s health such as allergies or antibiotic resistance and problems on the manufacturing processes of fermented products. For the screening of milk samples for antimicrobial residues, there are various methods available, microbial inhibitor tests and assays based on specific receptors, both widely used, especially in farms, the dairy industry and control laboratories. Screening methods have been validated for the use in raw milk from cows, but information on the performance of these tests in sheep and goat’s milk is rather limited. The aim of this study was to evaluate the performance of some microbial and receptor-binding screening tests to detect antibiotics in sheep and goat’s milk according to Commission Decision 2002/657/EC to determine their suitability to monitor the presence of antibiotic residues in milk and establish the most convenient analytical strategy in Spain. The Detection capability (CCβ) of microbial screening tests, the BRT MRL, the Delvotest MCS SP-NT, the Delvotest MCS Accelerator and the Eclipse 100, was at or below the maximum residue limits (MRLs) for most beta-lactam antibiotics assessed and other non-beta-lactam drugs such as neomycin, tylosin, sulfadiazine and sulfadimethoxine. However, they were less sensitive in the detection of quinolones and tetracyclines at safety levels. When individual milk samples were analysed, microbiological tests showed a higher occurrence of non-compliant results in sheep milk than in goat’s milk, being related in all cases to an elevated somatic cell count (SCC). The microbiological system consisting of two complemetary microtiter plates containing Geobacillus stereathermophilus var. calidolactis and Bacillus subtilis, respectively, allows improving the detection level in sheep milk with respect to the use of a single commercial test using G. stearothermophilus, detecting some quinolone and macrolide substances more closely related to their respective MRLs. The rapid receptor-binding assays (the Betastar Combo, the Charm MRL BLTET, the SNAP Betalactam, the SNAP Tetracycline and the TwinsensorBT) were able to detect most beta-lactams and tetracyclines at or below MRLs (CCβ ≤ MRL). A higher specificity of the rapid receptor tests was obtained in all cases even when individual milk samples were analysed. Only the TwinsensorBT test presented non-compliant results when antibiotic-free milk samples from individual animals were analysed, especially in the last weeks of lactation. ▼ Introduction:-Reference intervals are ranges of upper and lower limits outside of which values would be considered abnormal. It depends on the type of instrument and reagents, the principle or method and the type of population being served. Therefore it is obvious that it should be established for every population in different regions even within a country. Objective:-The aim of this study is to establish population specific reference intervals of thyroid stimulating hormone, free thyroxine and free triidothyronine levels of cord blood. Materials and methods፡- A cross sectional study was conducted from July, 2013 to November, 2013 using cord blood sample from 123 new born infants of both sexes born at Tikur Anbessa Teaching Specialized Hospital and Gandhi Memorial Hospital. Thyroid stimulating hormone, free thyroxine and free triidothyronine were measured using Elecsys 2010 immunoassay analyzer at Ethiopian Health and Nutrition Research Institute. The non-parametric method of reference range determination was used. Results:- 123 Cord blood samples collected from umbilical cord of newborns were analyzed for thyroid stimulating hormone, free thyroxine and free triidothyronine values. The birth weights ranged between 2500g to 4700g with mean (SD) value of 3241.46(459.495)gram. Their gestational age ranged between 37 to 44 weeks with an average of 39.74 weeks. The 2.5th and 97.5th percentiles of values were found to be 3.48 mIU/L and 27.57 mIU/L for thyroid stimulating hormone, 0.89 ng/dL and 1.53 ng/dL for free thyroxine and 1.19 pg/mL and 2.51 pg/mL for free triidothyronine respectively. Conclusion:- In the present study the reference intervals of thyroid stimulating hormone ,free thyroxine and free triidothyronine were tried to be established and based on the results obtained , were 3.48 - 27.56 mIU/L for thyroid stimulating hormone, 0.89 - 1.53 ng/dL for free thyroxine and 1.19 - 2.51 pg/mL for free triidothyronine. It has been concluded that the result can provide us with an important clue to establish population specific reference intervals using large scale studies. ▼ As forensic scientists, we are required to accurately test for certain substances. This may necessitate the use of presumptive tests such as the One Step Multi-Drug Screen Test Card with the Integrated iCup®/iCup®A.D . There are many circumstances where these tests are applicable, such as job-related drug testing, custody and parole cases. An immunoassay, or presumptive test, is designed to give the analyst, even a non-scientific analyst, a general idea of what substance(s) are present in the individuals system, so that he or she is able to more accurately confirm what substances, if any, the individual may have used or consumed. The goal of the validation study of the One Step Multi-Drug Screen Test Card with the Integrated iCup®/iCup®A.D was to determine the sensitivity of various THC containing compounds (delta-9-THC, 11-nor-9-carboxy-delta9-THC, and 11-hydroxy-delta-9-THC) as well as different solutions containing Synthetic Cannabinoids at various concentrations and stored at different temperatures. Each of the drugs were tested below, at and above the cut-off of the drug stated by the manufacturer. The cut-off of 11-nor-9-carboxy-delta9-THC, given by the manufacturer, was 50 ng/mL. For every trial that was conducted, the drug could be detected in the iCup® at this limit of detection of 50 ng/mL, except when the drug had been stored in the freezer for approximately two months prior to use. Delta-9-THC was given a cut-off of 15,000 ng/mL, which is a high concentration, especially when these assays are used in custodial cases and job-related drug tests, where living individuals are providing a fresh specimen. The concentrations of delta-9-THC and 11-hydroxy-delta-9-THC were higher than the cut-off for a positive result of 11-nor-9-carboxy-delta9-THC, but it was tested below the 15,000 ng/mL cut-off for delta-9-THC, established by the manufacturer. After these adjustments were made, both delta-9-THC and 11-hydroxy-delta-9-THC could be detected in a range between 1,500 ng/mL and 5,000 ng/mL. While 1,500 ng/mL is still high for a living specimen, it is substantially lower than 15,000 ng/mL. Analyzing the higher concentration of the synthetic cannabinoid working stock solution of 10,000 ng/mL, positive results were detected at 3,500 ng/mL and 5,000 ng/mL. There were eight cannabinoids, metabolites, and synthetic cannabinoids found in the working stock solution: (1) THC, (2) 11-Hyroxy-delta-9-THC, (3) 11-nor-9-Carboxy-delta-9-THC, (4) AB-FUBINACA, (5) AB-FUBINACA Met. 3, (6) AB-FUBINACA Met. 2a, (7) AB-PINACA-blood, and (8) AB-PINACA Pentatonic Acid metabolite. As the concentrations decreased, a positive result was not produced. Ultimately, the final conclusions of all the testing was that the One Step Multi-Drug Screen Test Card with the Integrated iCup®/iCup®A.D is not as sensitive when it comes to the synthetic cannabinoids, the primary compound present in marijuana (delta-9-THC), and the active metabolite of marijuana (11-hydroxy-delta-9-THC). ▼ Background Clinical usable assessment methods to quantify mobility, balance, stability and postural control functions together in a full kinetic chain movement are sparse. Normative data of such dynamic functions in different group of athletes are still lacking and therefore the relationship between dynamic functional mobility and overuse injuries are also unknown. The prevalence of overuse injuries among adolescent athletes are continuously high and team gymnastic is one example of a sport with multiple and underestimated overuse injuries. Aim The first aim of study was to establish normative data of dynamic functional mobility in a group of elite team gymnasts. The second aim was to investigate ongoing overuse injuries and evaluate associations between ongoing overuse injuries and dynamic functional mobility in the group. Method A descriptive cross sectional study including 31 female elite team gymnasts 16-19 years old were conducted. “The hand reach star excursion balance test” (HSEBT) was used to measure dynamic functional mobility in 20 different movement patterns. The "Oslo Sports Trauma Research Center Overuse Injury Questionnaire"(OSTRC) was used to record and analyse severity of ongoing overuse injuries in the group. Pearson correlation analysis was used to determine the relationship between the scoring in the HSEBT- tests and scoring on the OSTRC. Results A majority of the participants presented with scores near the maximum values on the HSEBT. Further, 97% of the subjects had an ongoing overuse injury. Severe injuries, affecting participation in sport were present in 67%. Correlations was found between prevalence of severe injury in the lower back and lower scoring on the right leg in a rotational movement pattern (r= -0,42) whereas prevalence of severe injury in the hip was associated with extreme high mobility in a sidebending pattern in both legs (r= 0,50, 0,49). There was also a relationship between individuals with multiple severe overuse injuries and dynamic functional mobility in an extended movement pattern of the left leg (r= -0,36, -0,38). Conclusion Normative data of dynamic functional mobility according to the HSEBT can provide clinicians with reference points to compare elite female team gymnasts. There was an indication of association between limited mobility in some of the included tests and prevalence of overuse injuries in the lower back and hip. This study included a small sample and therefore conclusions have to be interpreted carefully.

Parallèlement, des cliniciens observent une perte de l'automaticité de la marche lors du vieillissement normal ou de pathologies neurodégénératives qui peut être directement imputée au déclin des fonctions exécutives (FE) et aggravé lors des situations de double-tâche (DT). Plusieurs auteurs ont montré que la présence prématurée d'une atteinte motrice pouvait prédire une évolution défavorable vers une démence de type Alzheimer. Dans ce contexte, il devient évident que l'évaluation de la marche doit faire l'objet d'une investigation spécifique lors d'un bilan cognitif. Les travaux de cette thèse s'articulent autour du concept de troubles cognitifs légers, des FE et de l'évaluation motrice lors de diverses situations de marche.L'objectif principal est de déterminer si l'évaluation de la marche spontanée et/ou en DT constitue un outil de détection précoce des troubles cognitifs. Un objectif secondaire est de préciser les FE qui affectent la performance motrice dans les situations de DT. L'originalité de ce travail de thèse réside dans le développement de nouveaux exercices de DT de navigation spatiale basés sur l'adaptation de tests neuropsychologiques (test de Corsi, de Stroop et Trail Making Test) à la marche. Finalement, nous avons aussi proposé un « stroop écologique » qui s'intéresse à la prise de décision de traverser de rue au feu piéton.Les résultats révèlent que les interférences provoquées par les situations de DT entrainent une modification spécifique du schéma de marche du sujet âgé ayant des troubles cognitifs même lorsqu'ils sont infracliniques. De plus, la résolution des tests de DT nécessite la participation commune de plusieurs FE.En conclusion, la batterie de test que nous proposons présente un intérêt potentiel dans la détection précoce des troubles cognitifs chez les sujets âgés, mais aussi dans la compréhension des mécanismes régulant les FE.

Traditional psychometric and/or neuropsychologic tests alone, are not powerful enough to detect cognitive disturbances in aging subjects and therefore new criteria and tests should be developed to get relevant screening tools. Since walking is not anymore considered as a pure automatic motor task but as a task depending both on cognitive and executive functions (EF), clinicians became interested in studying walking disturbances in the course of neurodegenerative pathology development. Walking tasks can be complex and could be assimilated as a double-task (DT) when individuals have to simultaneously proceed with cognitive and motor tasks. Several authors have suggested that disturbances in walking abilities could predict cognitive disorders (e.g. Alzheimer Disease, Mild Cognitive Impairment (MCI)). In diagnostic accuracy studies, sensitivity and specificity are the most common measures to assess the performance of diagnostic or screening tests. The estimation of these measures can be done using empirical or model-based methods. The primary objective of this thesis is to use both the empirical and the model-based (logistic regression) approach to assess the effects of covariates on the performance of the visual inspection with acetic acid (VIA) and lugol iodine (VILI) tests using the data from women screened for cervical cancer in Kinshasa, the Democratic Republic of Congo. The secondary objectives are: first, to adjust for the false negative and false positive error rates by the two tests through latent class models (LCM), and second, to evaluate the effects of covariates on the agreement between the measurements of the two tests taken by nurse and physician through Kappa statistic. No particular pattern could be observed in the trend of empirically estimated sensitivity and specificity of the VIA and VILI tests measured by the nurse and by the physician across age and parity categories. From the logistic regression models, both age, parity, and their respective quadratic terms have significant effects on the probability of VIA and VILI tests to detect cervical cancer. However, there is no significant effect of marital status, smoking, and hybrid capture2 (HPV DNA) on the probability of VIA and VILI tests measured by nurse to detect cervical cancer while HPV DNA does in the probability of VIA and VILI tests measured by physician to detect cervical cancer. The trend of the estimated sensitivity of VIA and VILI tests measured by the nurse is not different across age groups but the specificity does vary. The trend of both the sensitivity and specificity of VIA and VILI tests are significantly different across parity groups. The reverse is the case for the sensitivity and specificity of VIA and VILI tests measured by physician across age and parity groups. The false negative and false positive error rates in the sensitivity and specificity of VIA and VILI tests measured by nurse are higher compared to that of physician. With Kappa statistic results, there is almost perfect agreement between the ratings by the nurse and physician for the dichotomized VIA and VILI test outcomes. In conclusion, there is a significant effects of age, parity and the quadratic term of age on the performance of VIA and VILI tests outcomes measured by nurse. On the VIA and VILI test outcomes measured by physician, age, parity, HPV DNA and quadratic term of age have shown significant effects on the performance of VIA and VILI tests outcomes measured by physician alone.

Objective: The aim of this study was to understand women's experiences, beliefs, and attitudes about cervical cancer screening and the influence of living with Human Immunodeficiency Virus (HIV) on preventive Papanicolau (Pap) smears in South Africa. Design and Setting: Semi-structured interviews were conducted with participants in a private office at Thandanani Children’s Foundation (TCF) in Pietermaritzburg, KwaZulu Natal during July 2015. Data from transcripts was coded for themes using NVivo qualitative research software to identify meaningful patterns for analysis using both content and thematic analysis. Participants: 30 women participated in semi-structured interviews Results: The median age was 48, all participants had some high school education or less, and 77% of participants were unemployed. Seventeen participants were living with HIV, and 70% of participants had received at least one Pap smear at time of interview. Lack of knowledge about Pap smears and fear of the screening procedure were the most common barriers to obtaining Pap smears. Being HIV positive was associated with increased awareness about cervical cancer screening, and a positive facilitator for having had at least one Pap smear in their lifetime. Participants described the role of religion, patient autonomy, financial burdens, healthcare providers, and community awareness as other important factors influencing the decision to obtain a Pap smear. Conclusion: In South Africa, the barriers to cervical cancer screening continue to include lack of education and awareness about Pap smears, financial burdens to attending clinics, lack of connection of HIV negative women to the health care system, and reduced community awareness about cervical cancer and the role of Pap smears in prevention. However, there is improved awareness and utilization of cervical cancer screening among women living with HIV in Pietermaritzburg, Kwa-Zulu Natal. This study highlights that living with HIV in a peri-urban community in South Africa, in clinics with stable integration of HIV care into other health aspects, allows women living with HIV to extend their health care access to other areas of their life, hence improving cervical cancer screening.

▼ Cognitive impairment (CI) is a serious concern for an aging global population, and its impact is not only felt within the family unit but also society at large. The current thesis involved two studies – an online survey about the use and role of screening instruments in New Zealand, and an evaluation of the clinical utility of three screens used in a clinical geriatric setting. The survey found that the Mini Mental Status Examination (MMSE) was the most frequently used screen, followed by the Clock Drawing Test (CDT), Addenbrooke‘s Cognitive Examination-Revised (ACE-R), Verbal Fluency (FAS), Three Word Recall (3WR), and Trail-making Test (TMT). The opinions of the survey respondents confirmed international publications suggesting that the MMSE does not fulfil the requirements of current assessment and/or screening practices. The survey further suggested that the ACE-R showed promise as an appropriate alternative to the MMSE due to its continuity from the MMSE, and because it appears to meet clinician requirements for brief screening instruments. The second study evaluated the clinical utility of the ACE-R, MMSE and the Modified Mini Mental State (3MS) within a larger assessment approach. It found that all three screens successfully differentiated between milder forms of CI and dementia; however, predictive ability for milder CI could not be determined. The ACE-R outperformed both the 3MS and MMSE in terms of predictive ability for dementia, with the 3MS showing marginally higher predictive ability than the MMSE. The study suggested that the 3MS‘s incremental validity did not justify its inclusion in a routine assessment process. Optimal sensitivity and specificity ratios – providing the best balance between sensitivity and specificity – were obtained with different cut-off scores than those recommended by the screens‘ original publications. This may have been due to the screens‘ authors seeking ratios that favoured sensitivity at the cost of specificity. However, in the data set from a clinical geriatric setting, used for this current study, the focus was on limiting both false-negatives and false-positives. While the MMSE showed adequate sensitivity and specificity, its known cultural and socio-economic bias makes it inappropriate for widespread use. The predictive ability (and incremental validity) of the ACE-R, coupled with the fact that it is cost-effective, relatively brief and covers all the recommended cognitive domains suggest that it is a suitable substitute for the MMSE in clinical geriatric service settings. Further research is however required to assess any potential biases inherent in the ACE-R. It is recommended that all initial assessments with CI patients include a mood screen due to their high comorbidity and the increased scope of treatment options available when a mood component underlies cognitive complaints.

▼ Studies have consistently shown that teachers ratings of behavior were predictive of academic difficulties. While research has clearly indicated that behavior has a reciprocal relationship with academic achievement, there is a scarcity of research on the relationship between outcomes on high stakes tests and student behavior. Early identification of children at risk for academic difficulties is vital for successful intervention and remediation. Therefore, this researcher investigated use of a brief behavior screener as a predictor of students at risk for failing a high stakes test. Results from the Behavior Assessment System for ChildrenTeacher Rating ScaleChild Screener (BASC-TRS-C Screener) provided an assessment of behavior. Georgias Criterion Referenced Test Reading and Math scores provided achievement in reading and math. An analysis of data on 636 second through fifth grade participants revealed a significant inverse relationship between teacher ratings of student behavior and achievement. Thirteen of the fifteen models suggested that teachers ratings of behavior indicated with greater accuracy students at risk for academic difficulties than did the model without the behavior ratings. While two models were not significant, they clearly suggested an inverse relationship between behavior and achievement. Logistic Regression analyses suggested that the BASC-TRS-C Screener predicted with 90% accuracy the pass fail classification group associated with the score. The odds ratio suggested that with each point decrease on the BASC-TRS-C Screener score, (in which high scores equal greater behavior concerns) the chances of passing the Criterion Referenced Competency Test reading high stakes test increased by 5%. In the area of math, the odds of passing increased by 6% with each point decrease on the BASC-TRS-C Screener. Other findings suggested that minorities have a significantly greater risk (p < .05) of failing the Criterion Reference Competency Test in the areas of reading, math, or both reading and math than do their same age peers. Introduction: Overload injuries are common among elite cross-country skiers. Screening tests are used in sports to prevent injuries. The tests are used extensively, however, the method has low validity. The knowledge from the current study may be used when developing new screening methods. Aim: To explore elite cross-country skiers’ own experiences, desires and needs for screening tests. Method: A qualitative method with an inductive approach was used and individual semi-structed interviews were analyzed using qualitative content analysis.. Seven elite cross-country skiers at the National Sport University in Umeå were strategically selected. Consisting of six men and one woman, the median age of the skiers was 24 years old. All had performed a Functional Movement Screen (FMS) within the last three years. Results: The result consist of one main theme, namely “Strivning for avoidence of injury and increased performance”. Within this main theme there were four categories “Insufficient challenge to improve performance”, “Physical check leading to exercise adjustments“,”Athletic posture and good technique improves skiing” and “Adapting and planning their exercise”. Each of the categories have 3-4 sub-categories respectively. The result represent the skiers’ desire to remain un-injured while increasing their performance. They request more tailor made and sport specific screening tests developed for cross-country skiing. This based on careful observations and meassurements of the specific requirements of the sport. The insights from the screening test would ideally result in suggested training schedules and individualized exercises. Conclusion: Elite cross-country skiers piont out the need for objective observations and analysis methods of the physical challenges of cross-country skiing. This in order to identify performance-enhancing measures, while at the same time avoiding overload injuries. The knowledge from this study can provide future researchers the direction in which they should focus when developing new sport-specific tests.

För att undvika skador används ofta screeningtester av idrottare men det har i litteraturen dock svag validitet. ▼ Background: Cervical cancer is the second most common cancer among women worldwide and its incidence is higher for certain ethnic groups in Canada, compared to White Canadians. As more women immigrate to Canada, the potential for prevention increases. Failure to prevent cervical cancer is partly due to non-participation in regular screening. The objectives of the research were to (1) explore whether there are cervical cancer screening differences between non-immigrant and immigrant women; (2) explore cervical cancer screening decision-making among immigrant women; and (3) explore the influence of acculturation on cervical cancer screening. Methods: This research consisted of two studies. The first study consisted of quantitative analysis of the 2007-2008 Canadian Community Health Survey (CCHS). Univariate analyses, cross-tabulations, and logistic regression modeling were conducted. Analyses were restricted to women aged 18-69 years old living in Ontario with no history of hysterectomy. Sample weights were applied and bootstrapping was performed. Analyses were conducted on the full Ontario sample (unweighted n=13,549) and the immigrant sample (unweighted n=2,904), the latter of which was stratified into two groups based on self-reported cultural/racial background: immigrant women more likely to report a time-appropriate Pap test (low risk) and those less likely to report a time-appropriate Pap test (high risk). The second study consisted of interviews conducted with 22 older (aged 50-69 years) South Asian immigrant women from Waterloo and Toronto, Ontario. The descriptive qualitative study was informed by grounded theory methodology. Interviews were audio-recorded and transcribed. Results: The first study identified that almost 17% of women reported not having a time-appropriate Pap test. Immigrant women were more likely to report not having a time-appropriate Pap test (21.73%) compared to non-immigrants (14.22%). Among immigrant women, almost 17% of White, Black, and other women (low-risk group) did not report a recent Pap test, compared to 28.67% of Chinese, South Asian, and other Asian women (high-risk group). Among the full Ontario sample, not having a time-appropriate Pap test was associated with being 50-69 years old, single, having low education and income, not having a regular doctor, being of Asian (Chinese, South Asian, other Asian) cultural/racial background, perceiving having less than excellent health, and being a recent immigrant. Among women in the low-risk group, not having a recent Pap test was associated with older age, lower household income, not having a regular doctor, and a lower proportion of life spent in Canada. Among women in the high-risk group, not having a time-appropriate Pap test was associated with lower education and not having a regular doctor. ▼ Falls in people aged over 65 years account for the largest proportion of all injury-related deaths and hospitalisations within Australia. Falls contributed to 1,000 deaths and 50,000 hospitalisations in older people during 1998 (Commonwealth Department of Health and Aged Care 2001). It has been predicted that by 2016, 16% of the Australian population will be aged over 65 years (Australian Bureau of Statistics 1999) placing considerable pressure on the health care system. Furthermore, prospective studies have shown that 30-50% of people aged 65 years and over, will experience a fall (Tinetti et al. 1988b; Campbell et al. 1989; Lord et al. 1994b; Hill 1999; Brauer et al. 2000; Stalenhoef et al. 2002) and this figure increases exponentially with age (Lord et al. 1994b). Many physiological falls risk factors have been established including reduced leg strength, poor balance, impaired vision, slowed reaction time and proprioception deficits. However, little research has been conducted to determine whether performance on aerobic fitness tasks is also a physiological falls risk factor. Aerobic fitness has previously been related to an individual's ability to perform activities of daily living, which in turn has been linked to falls. It was therefore proposed that aerobic fitness might also be a risk factor for falls among community dwelling older people. This research aimed to provide clinical evidence to inform public health practice. This thesis comprised of four objectives: the first to find suitable measures of aerobic fitness for older people; the second investigated relationships between existing clinical tests and future falls; the third explored relationships between aerobic fitness tests and future falls; the final objective was to examine the independent relationships between falls and clinical and physiological characteristics. The participants were recruited through a random sample from the local electoral roll, with an average age of 73 ±6 years. Of the 87 participants who completed the prospective component of the study, 37% were male and 63% were female. Sixty-three participants (65%) reported no previous falls, 19 (20%) reported a single fall, and 16 (15%) reported two or more falls in the previous 12 months. The first objective required participants recruited from the community to take part in submaximal and maximal fitness tests in order to find suitable measures of aerobic fitness. A further objective was to determine whether older people were able to fulfil the 'standard' criteria for completion of a maximum oxygen consumption test. The measures used in this research included: maximum oxygen consumption, peak oxygen consumption, ventilatory threshold, oxygen uptake kinetics, oxygen deficit, efficiencies, oxygen consumption at zero, 30 and 50 watts, predicted max and Six-Minute Walk Test distance. ▼ Introduction: HIV testing serves as the entry point into care for HIV-infected individuals and an effective prevention intervention because behavior change and antiretroviral treatment can reduce transmission. Increasing testing among men who have sex with men (MSM) may reduce the burden of HIV infection in this high-risk population. Methods: Data from MSM attending publicly-funded HIV/STI testing programs in King County, Washington, from 2003-2011 were used to evaluate temporal trends in and correlates of HIV intertest intervals (ITI) and reasons for testing. A deterministic, continuous-time model of HIV transmission dynamics was used to estimate the impact of replacing clinic-based tests with home-use tests on population-level prevalence. Results: The median ITIs among MSM seeking HIV testing at the Public Health - Seattle & King County STD Clinic and Gay City Health Project (GCHP) were 215 (IQR: 124-409) and 257 (IQR: 148-503) days, respectively. Over time, ITIs declined at both clinics. In multivariable analyses, younger age, having only male partners, reporting ≥10 male partners in the last year, inhaled nitrite use, history of STI, having a regular healthcare provider, and "regular testing" were associated with shorter ITIs (p<0.05). When asked reasons for testing, 49% of MSM attending GCHP reported it was time for their regular test, 27% reported unprotected sex, 12% reported sex with an HIV-infected partner, 21% reported sex with someone new, 24% were starting a relationship, 21% sought STI/hepatitis screening, 2% suspected primary HIV infection, and 16% reported other reasons. Based on observed levels of clinic-based testing, our model predicted an equilibrium HIV prevalence of 18.6% for Seattle MSM. If all men replaced clinic-based tests with home-use tests, prevalence increased to 27.4% assuming no changes in testing frequency. Regardless of how much more often MSM tested at home, any replacement of clinic-based testing with home-use tests increased prevalence. Conclusions: Efforts are needed to increase frequent, regular HIV testing among MSM to increase opportunities for early identification of HIV infection and linkage to care. Home-use tests may reach HIV-infected persons who would not otherwise test. However, our model suggests that replacing clinic-based testing with home-use tests may increase HIV prevalence among Seattle MSM. Advisors/Committee Members: Farquhar, Carey (advisor).

▼ Binary measurement systems (BMS) are widely used in both manufacturing industry and medicine. In industry, a BMS is often used to measure various characteristics of parts and then classify them as pass or fail, according to some quality standards. Good measurement systems are essential both for problem solving (i.e., reducing the rate of defectives) and to protect customers from receiving defective products. As a result, it is desirable to assess the performance of the BMS as well as to separate the effects of the measurement system and the production process on the observed classifications. In medicine, BMSs are known as diagnostic or screening tests, and are used to detect a target condition in subjects, thus classifying them as positive or negative. Assessing the performance of a medical test is essential in quantifying the costs due to misclassification of patients, and in the future prevention of these errors. In both industry and medicine, the most commonly used characteristics to quantify the performance a BMS are the two misclassification rates, defined as the chance of passing a nonconforming (non-diseased) unit, called the consumer's risk (false positive), and the chance of failing a conforming (diseased) unit, called the producer's risk (false negative). In most assessment studies, it is also of interest to estimate the conforming (prevalence) rate, i.e. probability that a randomly selected unit is conforming (diseased). There are two main approaches for assessing the performance of a BMS. Both approaches involve measuring a number of units one or more times with the BMS. The first one, called the "gold standard" approach, requires the use of a gold-standard measurement system that can determine the state of units with no classification errors. When a gold standard does not exist, is too expensive or time-consuming, another option is to repeatedly measure units with the BMS, and then use a latent class approach to estimate the parameters of interest. In industry, for both approaches, the standard sampling plan involves randomly selecting parts from the population of manufactured parts. In this thesis, we focus on a specific context commonly found in the manufacturing industry. First, the BMS under study is nondestructive. Second, the BMS is used for 100% inspection or any kind of systematic inspection of the production yield. In this context, we are likely to have available a large number of previously passed and failed parts. Furthermore, the inspection system typically tracks the number of parts passed and failed; that is, we often have baseline data about the current pass rate, separate from the assessment study. Finally, we assume that during the time of the evaluation, the process is under statistical control and the BMS is stable. ▼ To meet the needs of complex and/or underserved patient populations, health care professionals must possess diverse backgrounds, qualities, and skill sets. Holistic review has been used to diversify student admissions through examination of non-cognitive attributes of health care applicants. The objective of this study was to develop a novel methodology, the computer-based assessment of non-cognitive attributes of health professionals (CANA HP), to effectively screen non-cognitive attributes of applicants. Three research questions were delineated; 1.) To determine the CANA-HP instrument reliability (internal consistency & interrater), 2.) To determine if the CANA-HP measured attributes of noncognitive variables, as demonstrated by low construct validity scores when correlating the CANA-HP to traditional assessments reported to measure cognition, and 3.) To determine if differential item functioning on the CANA-HP revealed differences between groups based a variety of variables. The study used a sample of convenience of students interviewed as part of the admission process into the occupational therapy program at Wayne State University (N=37). Participants who consented to the study, completed a demographic survey followed by the 12 question CANA-HP. Data were analyzed using SPSS v. 25.0 (IBM, 2018) or Iteman v. 4.3 (ASC, 2013). Descriptive statistics of the sample population and 12 CANA-HP stations were computed. Cronbach's coefficient alpha was conducted on all of the stations for reliability, while interclass correlation estimates were run for interrater reliability. Pearson's correlation coefficients were calculated between CANA-HP scores and GRE / GPA scores at the time of program admission. Item difficulty, item discrimination, and bias were analyzed using mean average (P), Rbpis, and Fisher’s exact tests respectively. The six open-ended scenarios had minimally adequate internal reliability (α = 0.71), adequate item discrimination (Rbpis = 0.15 – 0.56), and adequate difficulty (P = 3.51 – 3.70). The traditional multiple choice questions need further refinement as these six scenarios had low reliability and discrimination. Initial results support the hypothesis of no correlation between the CANA-HP and standardized cognitive assessments (GRE and GPA scores). The one exception was non-science GPA which was significantly correlated to the total open-ended scores (p = .002) and total overall score (p = .008) and should be further examined. The CANA-HP is not biased toward the variables of sex, ethnicity, Pell grant status, family college history, or income level. Homogenous raters may improve interrater reliability which ranged from 0.67 – 0.91. These results should be viewed with caution due to the small sample size conducted at only one university. Bolezni obtočil so glavni vzrok smrti v razvitih delih sveta. Obtočila so pod nadzorom avtonomnega živčevja, zato se preko monitoringa srca zagotovi pogled v avtonomno živčevje. Zgodnja preventivna intervencija s presejalnimi testi pomeni pravočasno zdravljenje in znižanje umrljivosti. Raziskovali smo, ali je mogoče z medicinsko opremo, ki je primarno namenjena neprekinjenemu monitoringu v bolnišničnem okolju, izvajati hitre dinamične provokacijske teste, ki bi nam služili kot presejalni testi. Hemodinamične spremenljivke smo zajemali s kontinuirano neinvazivno protitlačno Peňáz metodo merjenja krvnega tlaka. Okno v delovanje avtonomnega živčevja pa je predstavljala variabilnost frekvence srca (HRV). Na zdravih prostovoljcih smo izvajali dva tipa provokacijskih testov obtočil: Valsalva manever in pasivni dvig nog (PLR). Podatke smo analizirali z Matlab programskim orodjem. Na podlagi opazovanja vseh štirih faz Valsalva manevra smo ugotovili, da je oprema sposobna zabeležiti hiter dinamični odziv. PLR test pa je pokazal nekoliko manjšo občutljivost. Oprema bi ob zelo natančni standardizaciji lahko predstavljala orodje za presejalne teste.

Circulatory diseases are the main cause of death in the developed world. Circulatory system is under control of the autonomic nervous system, therefore through the cardiovascular monitoring we get an insight into the autonomic nervous system. An early intervention with preventive screenings ensures timely treatment and reduces mortality. We researched whether it is possible to perform fast dynamic provocation tests with the equipment that is primarly meant for uninterrupted monitoring in a hospital environment, and use these tests as screening assays. Hemodynamic variables were determined by the continuous noninvasive Peňáz backpressure method of measuring blood pressure. Heart rate variability (HRV) provided a window into the functioning of the autonomic nervous system. Using healthy volunteers, we carried out two types of challenge tests of the circulatory system: Valsalva maneuver and passive leg raising (PLR). Data were analyzed using the Matlab software tool. Our findings show that the equipment is able to record fast dynamic response, as we observed all four phases of Valsalva maneuver. PLR test demonstrated slightly lower sensitivity. With precise standardization the equipment could be used as a tool to perform screening assays.

La voie pulmonaire suscite un intérêt grandissant pour l’administration systémique des peptides et protéines thérapeutiques, aujourd’hui encore administrés essentiellement par voie parentérale. Un microsystème a été conçu pour permettre de faciliter et accélérer les études in vitro de criblage de différentes biomolécules actives et de sélectionner les formulations les plus adaptées à leur pénétration à travers l’épithélium pulmonaire, en vue de sélectionner les meilleurs candidats à une administration par voie pulmonaire. Organisé en deux configurations distinctes, ce microsystème permet dans un premier temps d’obtenir des barrières épithéliales pulmonaires polarisées et jointives (cellules Calu-3) en seulement 7 jours dans des micropuits de 1mm², sans avoir à renouveler le milieu nutritif ni avoir recours à un appareillage externe associé au microsystème. Grâce à la mise au point d’une technique simple de fabrication, des plateformes de culture contenant jusqu’à 12 micropuits en parallèle sont aujourd’hui fabriquées de manière standardisée. L’évaluation du passage de molécules est ensuite réalisée sous une deuxième configuration dédiée à la mesure de la perméabilité des barrières épithéliales cultivées en micropuits. La capacité de différents candidats (nanoparticules et biomolécules) à traverser l’épithélium pulmonaire a été étudiée. Le passage de nanoparticules de PLGA revêtues de chitosane ainsi que le passage de l’insuline ont été démontrés avec succès. Enfin, l’électrophorèse capillaire couplée à une détection par fluorescence induite par laser (EC-LIF), compatible avec les faibles volumes manipulés dans ce microsystème, a été exploitée pour la détection et la quantification de l’insuline après passage des barrières pulmonaires miniaturisées. A cette fin, l’insuline a soit été marquée par le FITC, soit complexée à un anticorps ou a un aptamère fluorescents. A l’heure actuelle, seule la méthode développée pour le marquage de l’insuline par le FITC est utilisable à des fins de quantification, mais le recours à un aptamère a montré des premiers résultats encourageants.

The pulmonary route is of increasing interest for the systemic administration of therapeutic proteins and peptides, still largely administered parenterally. A microdevice was designed to facilitate and accelerate the in vitro screening studies of various active biomolecules and to select the most suitable formulations for penetration through the lung epithelium, in order to select the best candidates for an administration via the lungs. Organized in two distinct configurations, this microdevice allows as a first step the culture of tight polarized bronchial epithelial barriers (Calu-3 cells) in 7 days in 1 mm² microwells, without the need for medium renewal or the use of an external apparatus. A simple manufacturing technique was developed and glass culture platforms containing 12 parallel microwells can be obtained in a standardized manner.
