Background: Evidence based guidelines can improve medical healthcare but the development is a demanding and time consuming process. Therefore it is important to share the knowledge about the development of such guidelines. Objectives: To present the methodical development process of evidence based guidelines by the means of the publications on the European evidence based guidelines on the systemic therapy of plaque psoriasis, the method report of these guidelines and the evaluation of the German evidence based guidelines on psoriasis, and to critically appraise this process. .Methods: A search for national evidence based guidelines and an additional systematic search and evaluation of new literature was performed for the European guidelines. Further steps included a structured consensus process to develop the recommendations, as well as several internal and external reviews. For the implementation of the German guidelines 223 dermatologists of Berlin and surrounding Brandenburg were invited for an implementation workshop. Results: Three national guidelines and a total of 114 studies served as base for the efficacy chapters of the European guidelines. The consensus process includes a structured consensus meeting and a DELPHI procedure. The external review was performed by European national dermatological societies, the European Dermatology Forum, the European Academy of Dermatology and Venereology and the Union Europeenne des Medicins Specialistes. Concerning the implementation of the German guidelines 27% of the invited dermatologists took part in the two- hour implementation workshop. A total of 79% felt that guidelines would be helpful in improving physicians’ confidence and would ultimately lead to an increased use of systemic treatments. Conclusion: The development of evidence based guidelines can improve medical healthcare. Therefore the acceptance of the users is required. A good precondition for acceptance is a high methodical quality of the guidelines (e.g. evidence and consensus based), transparency in the development process (e.g. by a method report) and an implementation besides publication of the guidelines.
