연구 목적 : 본 연구는 remifentanil의 목표농도주입시 중심효과처 농도를 여러 가지 regimen으로 단계적으로 증가시켰을 때 remifentanil 유발 기침을 예방하는데 효과적인지 알아보고자 하였다. 연구 방법 : 이전 연구에서 140명의 환자들을 무작위로2군으로 나누었다. 두 군 모두 2단계의 중심 효과처 목표 농도 증가를 이용하여 remifentanil을 투여받았는데, 한 군은 1.0에서 4.0ng/ml로, 한 군은 2.0에서 4.0ng/ml로 증가시켰다. 이 연구 결과를 바탕으로 본 연구에서는 3단계의 목표농도주입을 시행하였고(1.0에서 2.0, 그리고 4.0ng/ml), 처음부터 4.0ng/ml로 즉시 목표농도주입을 시작한 군과 비교하기 위해 새로운 140명의 환자를 무작위로 두 군으로 나누어 연구를 진행하였다. 기침이 발생했을 경우를 기록하였고, 그 정도를 약함(1-2), 중간(3-4), 심함(5회 이상)으로 분류하여 기록하였다. 결과 : R1-4군의 1명(1.5%)의 환자가 1단계에서 기침을 하였으며, 5명(7.3%)의 환자가 2단계에서 기침을 하였다. R2-4군에서는 9명(13.2%)의 환자가 1단계에서 기침을 하였고, 2단계에서는 기침을 하는 환자가 없었다. 1명의 환자만이 R1-2-4군의3단계 목표농도주입시에 기침을 하였고, 이는 R4군에 비교하였을 때 기침의 발생율과 정도에서 통계적으로 유의하게 감소된 결과를 보였다. (P < 0.001) 결론 : Remifentanil의 목표농도주입시 단계적 증량은 remifentanil 유발 기침의 발생율을 줄여주고, 특히 3단계 증량 시에는 거의 발생하지 않았다.

▼ O propofol é um anestésico geral que apresenta características desejáveis como efeito ultra-curto, metabolismo rápido e baixa toxidez, podendo ser utilizado para indução ou manutenção da anestesia. O remifentanil é um opióide recentemente sintetizado, apresenta grande poder analgésico, curto tempo de ação e capacidade de potencializar os anestésicos gerais. São escassas as informações sobre o uso da associação remifentanil e propofol na Medicina Veterinária, especialmente quando se trata da espécie felina. O presente trabalho teve como objetivo avaliar a anestesia por infusão contínua de propofol associado ao remifentanil em gatos submetidos a cirurgias eletivas como a ovariosalpingohisterectomia e orquiectomia. Foram utilizados 30 gatos, distribuídos aleatoriamente em três grupos de dez animais cada, com igual número de fêmeas e machos. Após jejum hídrico de 6 horas e alimentar de 12 horas, todos os animais receberam acepromazina por via intramuscular, na dose de 0,1 mg/kg como medicação pré-anestésica (MPA). Decorridos 15 minutos procedeu-se a indução anestésica com propofol, em bolus de 6 mg/kg. A anestesia foi mantida por 60 minutos através de infusão contínua. O Grupo 1 recebeu propofol na velocidade de 0,6 mg/kg/minuto; o Grupo 2 recebeu propofol na velocidade de 0,4 mg/kg/minuto, associado ao remifentanil na velocidade de 0,1 g/kg/minuto e o Grupo 3, propofol na velocidade de 0,4 mg/kg/minuto, associado ao remifentanil na velocidade de 0,2 g/kg/minuto. Avaliaram-se a temperatura corporal, freqüência cardíaca, pressão arterial sistólica, freqüência respiratória, volume corrente, volume minuto, saturação da oxi-hemoglobina, valores hemogasométricos do sangue arterial, eletrocardiograma, analgesia, miorrelaxamento, reflexos palpebral, corneal, interdigital e laringo-traqueal, período de recuperação e tempo de preenchimento capilar. Os momentos estabelecidos para mensuração das variáveis foram: M0 Antes da administração da MPA; M1 15 minutos após a MPA; M2 Imediatamente após a indução anestésica e intubação orotraqueal; M3 15 minutos após M2, após incisão da parede abdominal nas fêmeas ou do escroto nos machos; M4 30 minutos após M2, no momento da ligadura do primeiro pedículo ovariano nas fêmeas ou do primeiro cordão espermático nos machos; M5 60 minutos após M2, após o término da cirurgia. A avaliação dos resultados permitiu concluir que: a infusão contínua de propofol associado ao remifentanil para a anestesia injetável no gato foi considerada satisfatória e demonstrou maiores vantagens quando comparada à anestesia onde o propofol foi utilizado como agente único por promover recuperação anestésica mais rápida; a infusão do remifentanil na velocidade de 0,1 g/kg/minuto foi superior à velocidade de 0,2 g/kg/minuto por promover analgesia equivalente e menores alterações nos parâmetros cardiorespiratórios; o incremento da analgesia proporcionado pela infusão do remifentanil nas velocidades de 0,1 g/kg/kg/minuto e de 0,2 g/kg/minuto permitiram redução de cerca de 33% na velocidade de infusão do propofol; a redução… Advisors/Committee Members: Luiz Gonzaga Pompermayer, Andrea Pacheco Batista Borges, José Dantas Ribeiro Filho, Eliane Gonçalves de Melo, Ricardo Junqueira Del Carlo.

O remifentanil é um opióide μ-agonista, que apresenta grande poder analgésico, curto tempo de ação e capacidade de potencializar os anestésicos gerais. O presente trabalho teve como objetivo estudar a infusão contínua de remifentanil em cadelas pré-medicadas com levomepromazina e anestesiadas pela associação T-Z. Foram utilizadas 20 cadelas, distribuídas aleatoriamente em dois grupos de dez animais cada. Após jejum hídrico de 8 horas e alimentar de 12 horas, todos os animais receberam levomepromazina por via intravenosa, na dose de 1,0mg/kg como medicação pré-anestésica (MPA). Decorridos 15 minutos procedeu-se a indução anestésica com a associação T-Z, na dose de 3mg/kg. O Grupo 1 recebeu infusão contínua de solução NaCl 0,9% durante 45 minutos, na velocidade correspondente aos animais do grupo 2 que receberam infusão de remifentanil (0,15ml/kg/min); o Grupo 2 recebeu infusão contínua de remifentanil, na velocidade de 0,3μg/kg/min, durante 45 minutos. Avaliaram-se a temperatura retal, freqüência cardíaca, eletrocardiograma, pressão arterial sistólica, freqüência respiratória, volume minuto, saturação da oxihemoglobina, valores hemogasométricos do sangue arterial, analgesia cirúrgica, com a realização de OSH, miorrelaxamento, reflexos palpebral, corneal, interdigital e laringotraqueal, salivação, período de latência, período hábil, dose total administrada e período de recuperação. Os momentos estabelecidos para mensuração das variáveis foram: Antes Antes da administração da MPA; M1 15 minutos após a MPA; M2 Imediatamente após a indução anestésica e intubação orotraqueal; M3 No momento da ligadura do primeiro pedículo ovariano (aproximadamente 15 minutos após início da infusão); M4 No momento da ligadura do segundo pedículo ovariano (aproximadamente 30 minutos após início da infusão); M5 15 minutos após M4, após o término da cirurgia. Houve queda significativa da temperatura retal em ambos os grupos. Um aumento significativo da freqüência cardíaca e da pressão arterial sistólica também foi observado nos dois grupos porém com maior intensidade no G1. A freqüência respiratória se comportou de maneira diferente nos dois grupos, havendo um aumento no G1 e no G2 a FR manteve-se dentro dos valores fisiológicos. O volume minuto apresentou depressão significativa em ambos os grupos, mas manteve-se dentro dos valores fisiológicos. Não foram observadas diferenças significativas do pH do sangue arterial na comparação entre os grupos. Porém houve diminuição inicial, para depois manter-se estável, sendo que os valores permaneceram na faixa de normalidade, não sendo observada acidemia em nenhum dos dois grupos. O miorrelaxamento e analgesia foram considerados mais intensos no G2. O G1 apresentou período hábil maior e recuperação anestésica mais rápida, em comparação ao G2. The objective of the present research was to study the comparative effects of dexmedetomidine and remifentanil on the renal function of morbidly obese patients submitted to bariatric surgery. Casuistic and Method: After approval by the local Ethics Council, in the period from August 2004 to August 2005, 61 patients were studied prospectively independent of race; ages ranged from 19 to 63 years (35.6 Â 9.9 years); 17 (27.8%) were males and 44 (72.2%) female. The individuals were divided randomly into two groups: remifentanil (group R = 0,1 Âg.kg-1.min-1) and dexmedetomidine (group D = 1 ug.kg-1 during 10 min and after 0,5 ug.kg-1.h-1). Isoflurane was used to maintenance BIS around 50 in both groups. The study monitored hemodynamic and ventilatory parameters and anesthesia depth. Renal function was evaluated at moments in each of three phases: before anesthesia (M0), after anesthetic induction (M1) and after surgical incision (M2). Plasma concentrations were determined for glucose, antidiuretic hormone, creatinine, urea, sodium, potassium and osmolarity and urinary concentrations for creatinine, urea, sodium, potassium and osmolarity. Results: The plasma concentration values of sodium, creatinine, urea and antidiuretic hormone at MO, M1 and M2 did not present differences between groups R and D. Significant differences were found between groups for potassium at M1 (p <0.05), osmolarity at M2 (p <0.05) and glucose at M1 and M2 (p <0.01 and p <0.001, respectively). The urinary volume was significantly different between groups only at moment M2 (p <0.001). Mean clearance values did not present significant difference between groups at M0. At the moment M1, the differences were significant between groups for potassium (p <0.001), creatinine (p <0.05) and urea (p <0.01). At M2 there was a difference between groups only for potassium and urea (p <0.05 in both). In group R there was a drop in mean clearance values from M0 to M1 for sodium, potassium, urea, creatinine and osmolarity; in group D the mean clearance values of sodium and osmolarity rose while values for potassium, creatinine and urea fell. There was a reduction in both groups for all variables between moments M1 and M2 and from M0 to M2. Conclusions: In both groups, the clearance values fell from moments M0 to M2. This result indicates that renal function of obese patients submitted to bariatric surgery presented a physiological response compatible with the effect of anesthetic-surgical stress. In group R, all clearances were reduced at M1, indicating a response compatible with anesthetic-surgical stress. In the dexmedetomidine group (D), creatinine and sodium clearances were elevated at M1. Urinary volume was greater at M2. These results are suggestive of better preservation of renal function.

▼ Les progrès médicaux de la prise en charge des nouveau-nés prématurés ont permis la diminution de la mortalité néonatale, mais la prévalence des handicaps neurologiques d'origine périnatale reste stable. Ainsi, la mise en place de stratégies de neuroprotection est un enjeu primordial de la médecine périnatale actuelle. Parmi ces stratégies, l'administration de sulfate de magnésium en cas de menace d'accouchement prématuré permet l'amélioration du devenir neurologique des enfants à l'âge de deux ans. La neuroprotection concerne aussi l'ensemble des interventions qui ont lieu après la naissance, pendant la période néonatale. Depuis les années 2000, les agents anesthésiques sont mis en cause dans la survenue d'anomalies du développement lorsqu'ils sont administrés chez le nouveau-né. La balance bénéfice/risque doit donc être évaluée pour chaque anesthésique prescrit en période néonatale afin de limiter une potentielle neurotoxicité. La première partie de cette thèse a pour objectif le suivi à l'âge scolaire d'une cohorte d'enfants prématurés exposés en anténatal au sulfate de magnésium. La deuxième partie est consacrée à l'évaluation in vivo de l'impact du rémifentanil, morphinique de synthèse utilisé comme agent anesthésique, sur le cerveau immature du souriceau en condition excitotoxique. 1 - Etude de l'influence de l'administration anténatale du sulfate de magnésium sur le développement psychomoteur à l'âge scolaire de prématurés de moins de 33 semaines d'aménorrhée : essai Premag. L'étude Premag, débutée en France en 1997, avait permis l'inclusion de 688 nouveau-nés prématurés exposés au sulfate de magnésium ou à un placebo, dont 606 ont été suivis à l'âge de deux ans. Un suivi à plus long terme était nécessaire pour affirmer l'innocuité de la molécule sur le devenir neurocognitif à l'âge scolaire. Entre 'décembre 2009 et avril 2012, les familles qui avaient participé à l'essai Premag ont été recontactées. Quatre-cent-trente-et-un enfants (moyenne d'âge 11 ans) parmi les 606 survivants ont été évalués à l'âge scolaire par un questionnaire parental. Le critère d'évaluation principal était un critère combiné déficit moteur et/ou décès. Il n'a pas été mis en évidence d'effet délétère de l'exposition anténatale au sulfate de magnésium. Par ailleurs, une diminution non significative des troubles du comportement, une meilleure scolarisation et une réduction du recours aux services d'éducation à domicile ont été observées en cas d'exposition au sulfate de magnésium. 2 - Effet neuroprotecteur du rémifentanil dans un modèle murin de lésion excitotoxique néonatale. Le rémifentanil, morphinique de synthèse, est utilisé en période périnatale, chez la mère en cas d'anesthésie générale ou chez le nouveau-né en cas de nécessité d'anesthésie/analgésie. ▼ The use of propofol and propofol in combination with remifentanil by nonanesthesiologists is a controversial topic. Much of the concern centers on adverse respiratory effects: loss of responsiveness, respiratory depression, and airway obstruction. The aim of this study was to investigate these adverse drug effects at propofol-remifentanil combinations commonly used in procedures requiring esophageal instrumentation and build response surface models of drug effects. A second aim was to investigate published dosing regimens through simulation with these models. A third aim was to develop an optimization algorithm to identify an ideal propofol-remifentanil dosing regimen for upper endoscopy procedures. Twenty-four volunteers received escalating target controlled remifentanil and propofol infusions. Responses to insertion of a bougie (40 cm), responsiveness, respiratory rate, and tidal volume were recorded at 384 targeted concentration pairs. Four published dosing regimens of propofol alone or in combination with opioids were simulated for a 10-min procedure. An optimization algorithm was developed to identify an optimal propofol-remifentanil dosing regimen from a set of possibilities. Models for loss of response to esophageal instrumentation, intolerable ventilatory depression, and respiratory compromise were built. Simulations of published dosing regimens showed that once drug administration ended, loss of responsiveness, and respiratory depression effects dissipated quickly. Respiratory compromise dissipated more quickly in propofol only techniques compared to propofol-opioid techniques. An optimal dosing recommendation was identified for a simulated 55 year-old, 75 kg, 175 cm male undergoing an anticipated 10-min upper endoscopy and consisted of a propofol bolus of 0.8 mg/kg and infusion rate of 40 mcg/kg/min and a remifentanil bolus of 0.2 mcg/kg and an infusion rate of 0.05 mcg/kg/min. High propofol-low remifentanil concentration pairs can block the response to esophageal instrumentation while avoiding intolerable ventilatory depression in spontaneously breathing volunteers. Propofol combined with remifentanil or fentanyl improved conditions for esophageal instrumentation and had a rapid return to responsiveness. Optimization techniques identified a remifentanil propofol dosing regimen that minimizes the duration of loss of responsiveness, respiratory depression, and airway obstruction and, according to expert opinion and models of drug effect, provides conditions that will permit upper endoscopy procedures. This dosing regimen merits clinical validation in patients undergoing brief endoscopic procedures.

Twenty female mongrel cats were used, all healthy, with a mean weight of 3.50.49 kg, admitted for an elective overiohysterectomy. Pre-anesthetic medication was made with acepromazine, 0.1mg.Kg-1 intravenously, and etomidate was used for anesthetic induction, also intravenously, on a dose of 3 mg.kg-1. Continuous act, animals were randomly divided in 2 groups of 10 animals each, maintaining anesthesia for 45 minutes using the protocols: for group 1 (IE), continuous infusion of etomidate at a rate of 0.4 mg.kg- 1.min-1; for group 2 (IER), continuous infusion of etomidate and remifentanil at rates of 0.3 mg.kg-1.min-1 and 0.4 μg.kg-1.min-1, respectively. The following parameters where analyzed: heart rate; oxyhemoglobin saturation; systolic arterial pressure; respiratory rate; body temperature; arterial blood gas; sodium, potassium, ionized calcium, chloride and glucose dosages; autonomic responses to nociceptive stimulus; electrocardiogram; palpebral, corneal, laryngeal and tracheal reflexes; time needed for recovery from the anesthesia; adverse effects record; final solution osmolarity; total number of erythrocytes; hematocrit; hemoglobin measure; total, direct and indirect bilirrubin; plasma free hemoglobin; haptoglobin and; erythrocyte fragility. Scheduled times for measurements were T0 (before), before pre-anesthetic administration (MPA); T1 (MPA), 20 minutes after MPA; T2 (5 min), 5 minutes after anesthetic induction and orotracheal intubation; T3 (15 min), T4 (30min), T5 (45min) and T6 (3h 45min) at 15, 30, 45 and 3 hours and 45 minutes after T2, respectively. Continuous infusion of etomidate and its association with remifentanil in cats promoted heart rate and blood pressure stability, hypoventilation and respiratory acidosis. Etomidate in continuous infusion could not suppress autonomic responses to nociceptive stimulus and so, inadequate for surgery procedures. Etomidate associated to remifentanil efficiently controlled autonomic responses to nociceptive stimulus before and after surgery. Both the infusions tested promoted transitory hyperglycemia during anesthesia and after it. Etomidate associated or not with remifentanil by continuous infusion promoted a longer recovery with intense side effects such as myoclonus and emesis. Etomidate and etomidate associated with remifentanil infusions, associated with acepromazine promoted important decrease in erythrocytes number, hematocrit and hemoglobin. ▼ Previous clinical studies showed increased postoperative pain intensity and cumulative morphine consumption following intraoperative high dose remifentanil. In this study we investigated in patients undergoing lower abdominal surgery i) whether postoperative pain and analgesic consumption is increased following intraoperative high versus low dose remifentanil, and ii) whether this is prevented by preoperative administration of the NMDA antagonist amantadine. 48 adult patients undergoing major abdominal surgery were randomly assigned to three anaesthetic regims: group RL recieved a continuous remifentanil infusion 0,1 µg/kg/min and sevoflurane titrated to autonomic responses; group RH recieved a continuous inspiratory concentration of sevoflurane (0,5 MAC) and a remifentanil infusion titrated to autonomic responses; group RHA recieved the same anaesthetic plan as RH and preoperatively 200mg amantadine iv. Patients who were blinded to group assignment controlled their own morphine administration via PCIA. Pain scores and morphine consumption were recorded to the sixth hour on PCIA by a also blinded study observer. Results: The mean remifentanil infusion rate was RH 0,20 ± 0,01 and RHA 0,22 ± 0,01 µg/kg/min , which were significantly greater than in group RL. Postoperative pain scores were not significantly different between the groups. All patients required morphine in the same amount: 30,7± 4,1 (RL), 28,9 ± 5,2 (RH) and 30,6 ± 2,0 mg (RHA). Relative large-dose intraoperative remifentanil did not increase postoperative pain and morphine consumption. Also, preoperative amandatin administration did not attenuate the postoperative response. BACKGROUND: Cough on anesthetic emergence should be prevented considering its dangerous complications. Target-controlled infusion (TCI) of remifentanil can reduce emergence cough effectively, and sex-related differences in effect-site concentrations (Ce) of remifentanil has been evaluated in young patients. In this study, we determined the Ce of remifentanil for preventing emergence cough following extubation in male and female elderly patients and evaluated the sex-related difference. METHODS: Twenty-three male and 22 female elderly patients aged between 60 and 75 years were enrolled. Anesthesia was maintained with sevoflurane and remifentanil TCI. The Ce of remifentanil for preventing emergence cough was determined for each sex using isotonic regression method with a boot¬strapping approach, following Dixon’s up-and-down method. RESULTS: The Ce of remifentanil for preventing emergence cough in 50% (EC50) and 95% (EC95) of the population were significantly lower in females than in males. Isotonic regression revealed the EC50 (83% CI) of remifentanil was 1.67 (1.55–1.83) ng/mL in females and 2.60 (2.29–2.91) ng/mL in males. The EC95 (95 % CI) of remifentanil was 2.30 (2.02–2.62) ng/mL in females and 3.41 (3.27–3.58) ng/mL in males. Dixon’s up-and-down method indicated that the mean EC50 in females was lower than in males (1.23 ± 0.21 ng/mL vs. 2.43 ± 0.21 ng/mL, P < 0.001). CONCLUSIONS: The remifentanil requirement for preventing emergence cough was lower in female than in male elderly patients, indicative of sex-related differences in Ce of remifentanil. Sex should be considered when using remifentanil TCI for preventing emergence cough in elderly patients.

전신마취 후 기관내튜브를 제거할 때 발생하는 기침은 다양한 합병증을 유발할 수 있기 때문에 예방하는 것이 좋다. 레미펜타닐을 목표농도조절주입법으로 투여하면 발관시 기침을 효과적으로 줄여줄 수 있다. 젊은 환자에게서 성별에 따른 레미펜타닐의 적정효과처 농도의 차이는 이미 밝혀져 있다. 본 연구에서는, 노인환자에서 발관시 기침을 예방할 수 있는 레미펜타닐의 적정효과처 농도를 측정하고 성별에 따른 차이를 평가하고자 하였다. 60~65세의 노인환자, 23명의 남자와 22명의 여자를 대상으로 하였다. 마취는 세보플루란으로 유지하였고 레미펜타닐을 목표농도조절주입법으로 투여하였다. 발관시 기침을 예방하는 레미펜타닐의 적정효과처 농도는 isotonic regression 및 bootstrapping approach, Dixon’s up-and-down method를 사용하여 결정되었다. 발관시 기침을 예방하는 레미펜타닐의 적정효과처 농도는 남성에 비해 여성에서 의미있게 낮았다. isotonic regression을 사용하였을 때, 50%의 환자에서 발관시 기침을 예방하는 레미펜타닐의 적정효과처농도(EC50)는 여성에서 1.67(1.55~1.83)ng/mL였고 남성에서 2.60(2.29~2.91)ng/mL였다. EC95는 여성에서 2.30(2.02~2.62)ng/mL였고 남성에서 3.41(3.27~3.58)ng/mL였다. Dixon’s up-and-down method로 구한 EC50 또한 여성에서 남성보다 낮았다.(1.23±0.21 ng/mL vs. 2.43±0.21 ng/mL, P < 0.001) 노인 환자에서 마취 후 기관내튜브 제거 중 기침 예방을 위해 필요한 레미펜타닐의 요구량은 남성에 비해 여성에서 낮았다. 즉 레미펜타닐의 적정효과처 농도에 있어서 성별에 따른 차이가 있음을 보여주었다. 따라서 노인 환자에서 발관시 기침 예방을 위해 레미펜타닐 목표농도조절주입법을 사용할 때 성별을 고려할 필요가 있겠다.

Os diversos protocolos anestésicos visam abreviar os efeitos hemodinâmicos indesejáveis, proporcionando um retorno mais seguro, com os mínimos efeitos colaterais para o paciente. Dessa forma, objetivou-se com este trabalho avaliar as possíveis alterações de parâmetros vitais relacionadas à eletrocardiografia, freqüência respiratória e temperatura corporal em cadelas pré-tratadas com atropina, anestesiadas com quetamina e xilazina, associadas ou não a diferentes doses de remifentanil sob infusão contínua. Para tanto, foram utilizadas 15 cadelas clinicamente sadias, distribuídas de acordo com os grupos experimentais: G1 = quetamina + xilazina ; G2 = quetamina + xilazina + infusão contínua com remifentanil (0,125μg/Kg/min); G3 = quetamina+ xilazina + infusão contínua com remifentanil (0,25μg/Kg/min). As mensurações das variáveis foram realizadas antes da pré-anestesia (M0), simultaneamente à incisão da pele (M1), durante a ligadura do pedículo ovariano direito (M2), durante a ligadura do coto uterino (M3) e na metade do término da síntese cutânea (M4). Os resultados apresentaram aumento nas médias e desvio padrão na freqüência cardíaca em todos os grupos no M1 (178,620,6), sendo posteriormente decrescente, porém, com tendência à taquicardia até M4 (157,318,3). Tal elevação em M1, corresponde ao momento de incisão cirúrgica, sugerindo o momento de maior estímulo nociceptivo, seguido da ligadura do pedículo direito (M2, 176,015,2). Não se observou apnéia em nenhum dos animais, apenas queda considerável da freqüência respiratória em seis animais (6/15), sendo um animal (1/5) no G1, três animais (3/5) no G2 e dois animais (2/5) no G3. Os valores da temperatura corporal,mensuradas via retal, não apresentaram diferenças biologicamente significativa entre os grupos, mantendo suas médias dentro dos parâmetros fisiológicos para espécie. De acordo com os resultados apresentados pode-se sugerir que o aumento da onda T pode estar relacionado a desequilíbrio eletrolítico, além de disrritmia cardíaca, sendo observado quadro de hipoxemia. Estudos posteriores com um maior número de animais poderão confirmar a eficiência do protocolo anestésico utilizando atropina, xilazina, quetamina associadas ao remifentanil sob infusão contínua para o efetivo controle da dor.

Dessa forma, objetivou-se com este trabalho avaliar as possíveis alterações de parâmetros vitais relacionadas à eletrocardiografia, freqüência respiratória e temperatura corporal em cadelas pré-tratadas com atropina, anestesiadas com quetamina e xilazina, associadas ou não a diferentes doses de remifentanil sob infusão contínua. Para tanto, foram utilizadas 15 cadelas clinicamente sadias, distribuídas de acordo com os grupos experimentais: G1 = quetamina + xilazina ; G2 = quetamina + xilazina + infusão contínua com remifentanil (0,125μg/Kg/min); G3 = quetamina+ xilazina + infusão contínua com remifentanil (0,25μg/Kg/min). Os resultados apresentaram aumento nas médias e desvio padrão na freqüência cardíaca em todos os grupos no M1 (178,620,6), sendo posteriormente decrescente, porém, com tendência à taquicardia até M4 (157,318,3). Tal elevação em M1, corresponde ao momento de incisão cirúrgica, sugerindo o momento de maior estímulo nociceptivo, seguido da ligadura do pedículo direito (M2, 176,015,2). Não se observou apnéia em nenhum dos animais, apenas queda considerável da freqüência respiratória em seis animais (6/15), sendo um animal (1/5) no G1, três animais (3/5) no G2 e dois animais (2/5) no G3. Os valores da temperatura corporal,mensuradas via retal, não apresentaram diferenças biologicamente significativa entre os grupos, mantendo suas médias dentro dos parâmetros fisiológicos para espécie. De acordo com os resultados apresentados pode-se sugerir que o aumento da onda T pode estar relacionado a desequilíbrio eletrolítico, além de disrritmia cardíaca, sendo observado quadro de hipoxemia. Estudos posteriores com um maior número de animais poderão confirmar a eficiência do protocolo anestésico utilizando atropina, xilazina, quetamina associadas ao remifentanil sob infusão contínua para o efetivo controle da dor.

A utilização de ventilação mecânica em terapia intensiva é uma prática comum. Além da sua ação terapêutica, é aplicada para corrigir os efeitos depressores consequentes da ação dos fármacos em pacientes sob sedoanalgesia. Por isso, vem sendo estudada para uma melhor compreensão da sua função, principalmente no que diz respeito às diferentes concentrações de oxigênio e as modalidades ventilação controlada a volume (VCV) e ventilação controlada a pressão (VCP) que podem desencadear diferentes respostas dos indivíduos. O objetivo deste trabalho é realizar um estudo comparativo da dinâmica cardiorrespiratória de suínos (Sus scrofa domestica) sedados com propofol-remifentanil sob dois aspectos: avaliar os animais submetidos a três diferentes frações inspiradas de oxigênio (FIO2) (0.8, 0.6, e 0.4) sob ventilação mecânica; avaliar e comparar os animais submetidos à VCV versus os submetidos à VCP. Utilizou-se 20 animais para realizar o comparativo entre as frações inspiradas e destes, seis para o comparativo entre as modalidades ventilatórias. Para sedação receberam em média 0,29 mg.kg-1.min-1 de propofol e 0,3 mcg.kg-1.min-1 de remifentanil, sendo que a velocidade de infusão de propofol variou buscando manter o animal em uma escala de consciência baseada no índice biespectral entre 60 e 70. Na comparação entre as FIs os animais ficaram sedados por duas horas e na comparação entre VCP e VCV, duas horas em cada modalidade. Foram avaliadas as frequências cardíaca e respiratória, eletrocardiografia, saturação de oxigênio em hemoglobina, pressão arterial, fração de dióxido de carbono no final da expiração, ventilometria e a hemogasometria. Com a obtenção de resultados percebeu-se uma vantagem na utilização da FIO2 0.4, pois apresenta menor porcentagem de shunt pulmonar, mas não ao ponto de descartar a utilização das outras FIs. No estudo comparativo VCV versus VCP não se encontrou diferenças. Durante a experimentação o protocolo empregado demonstrou ser adequado, pois causa mínimas alterações hemodinâmicas nos pacientes, proporcionando rápido e tranquilo despertar dos animais. Vale ressaltar que durante toda a experimentação os parâmetros se mantiveram constantemente dentro do aceitável para a espécie, o que permite indicar a técnica para futuros estudos. Com relação às diferentes FIs para a ventilação mecânica, utilizando-se oxigênio 40% teremos menos efeitos adversos.

Introdução: Os opioides são os analgésicos mais efetivos para tratamento da dor moderada a intensa. No entanto, evidências crescentes têm demonstrado que seu uso pode levar a mudanças na sensibilidade dolorosa. Nesse contexto, a hiperalgesia induzida pelo remifentanil (r-IH) envolve um desequilíbrio nos sistemas inibitórios e excitatórios. Postula-se que um dos mecanismos centrais seja a disfunção do sistema modulador descendente da dor. Então, neste estudo, testamos a hipótese de que a estimulação transcraniana de corrente contínua (ETCC), devido aos seus efeitos analgésicos, poderia prevenir a r-IH. Os desfechos primários incluíram a escala numérica de dor (END 0-10) durante o teste repetitivo ao frio (rCOLDT), e a alteração na END (0-10) durante o teste de modulação condicionada de dor (CPM-TASK). Os desfechos secundários foram os limiares de dor ao calor (HPT) e o tempo de reação durante o teste de dor à água gelada [zero graus oC, (IPT)]. Métodos: Ensaio clínico randomizado, fatorial, duplo cego, que incluiu 48 homens saudáveis, com idades entre 19 e 40 anos. Os sujeitos foram randomizados em quatro grupos (n=12): ativo (a) - ETCC / solução salina, Sham (s) - ETCC / solução salina, a-ETCC / remifentanil e s-ETCC / remifentanil. Foi aplicado o ETCC sobre o córtex motor primário, com uma sessão única de 20 min e 2 mA. Resultados: Durante o rCOLDT, houve um efeito significativo entre os grupos nos escores cumulativos da END (P = 0,01). O grupo s-ETCC / remifentanil apresentou maiores escores de dor durante rCOLDT, [media (SD) 5,49 (1,04)] e a-ETCC / remifentanil apresentaram escores relativamente menores [4,15 (1,62)]. Este achado mostra que o efeito da ETCC bloqueou a HI-R. Os grupos a-ETCC / solução salina e s-ETCC / salina apresentaram menor índice de dor durante rCOLDT, [3.11 (1.2)] e [3.15 (1.62)], respectivamente. A incidência de hiperalgesia definida como um aumento de 15% na END durante o rCOLDT foi de: 31% no grupo s-ETCC/remifentanil; 22% no grupo a-ETCC/remifentanil; 11% no grupo a-ETCC/salina; e 8.3% no grupo s-ETCC/salina. Os grupos com remifentanil apresentaram escore positivo na END (0-10) durante a tarefa CPM, ou seja, produziu um desengate do sistema modulador descendente de dor (DPMS). Além disso, s-ETCC / Remifentanil em comparação com a-ETCC/remifentanil apresentou menor HPT e maior tempo de reação durante o IPT. Conclusão: Esses achados sugerem que os efeitos da a-ETCC previne a disfunção da capacidade inibitória do sistema modulador descendente da dor induzido pelo remifentanil durante o rCOLDT.

O remifentanil é um opióide potente comparável ao fentanil, mas, possui rápida eliminação plasmática e por isso necessariamente deve ser adiministrado por infusão contínua. A administração transdérmica pode liberar fármacos de forma prolongada, contínua e controlada. Características que facilitariam a administração do remifentanil, contornariam sua limitação farmacocinética e ampliariam os casos em que este fármacos seria útil. Lipossomas deformáveis são formados por fosfolipídios e por tensoativo que normalmente é um surfactante de cadeia simples que aumenta a flexibilidade da bicamada lipídica da membrana e torna os lipossomas deformáveis. Os lipossomas deformáveis são capazes de disponibilizar, de forma expressiva, várias substâncias transdermicamente incluindo macromoléculas e podem ser um eficiente sistema de liberação controlada de remifentanil. O presente estudo desenvolve diferentes dispersões de lipossomas deformáveis de remifentanil e caracteriza essas dispersões quanto a tamanho de diâmetro dos lipossomas, eficiência de encapsulação dos lipossomas, elasticidade da membrana, permeação e retenção cutânea in vitro. As dispersões de lipossomas deformáveis preparadas foram compostas por remifentanil, fosfatidilcolina de soja, tensoativo tween 80 ou tween 20 e etanol a 10% ou a 20% e foram preparadas por dois métodos: sonicação e hidratação de filme e sonicação. O tamanho e a distribuição de tamanho foram obtidos por espalhamento dinâmico de luz, a eficiência de encapsulação por diálise, a elasticidade por filtração, seguido de determinação do tamanho. Células de difusão de Franz e pele de orelha de porco foram usadas para estudar a permeação e a retenção cutânea in vitro. Os lipossomas deformáveis preparados apresentaram pequeno diâmetro (40 a 71 nm) quando comparado a outros na literatura e seus tamanhos foram dependentes da formulação. Todos os lipossomas deformáveis preparados apresentaram boa elasticidade e permeação dependente da eficiência de encapsulação e do tamanho de diâmetro. A solução hidroetanólica com remifentanil e a mistura física dos excipientes com remifentanil não apresentaram permeação e retenção. A eficiência de encapsulação dos lipossomas deformáveis é coerente com outros resultados na literatura e foi dependente da formulação e do método de preparação. A avaliação da permeação de dispersões de lipossomas deformáveis com remifentanil não encapsulado foi feita para se obter informações sobre os mecanismos de ação pelos quais estes lipossomas agem. Verificou-se que o transporte de substâncias encapsuladas nos lipossomas foi o mecanismo de maior importância

▼ BACKGROUND: Propofol infusion syndrome (PIS) is seen in paediatric and adult patients in the Intensive Care Unit and is related to long term propofol sedation at high dose. Its main characteristics are lactic acidosis, rhabdomyolysis, renal and heart failure and the outcome is usually fatal. The syndrome description and the study of pathogenesis has been based mainly on a small number of case reports and histopathologic findings. The aim of the present experimental project was to study PIS on the basis of clinical, biochemical and histopathologic findings. MATERIALS AND METHODS: Thirty six healthy male New Zealand White rabbits, 3-8 months of age, weighing 3.5-4.0 kg were used. The study design inolved 4 phases of experimentation. Experiment 1. The creation of an animal model of propofol tolerance after propofol administration for the long term sedation of mechanically ventilated rabbits. One group of rabbits received propofol 2% by continuous intravenous infusion for a period of up to 48 hours (group P1, n=6). The initial infusion rate (IR) was 20 mg/kg/h and was adjusted, if needed, in order to maintain the desired sedation level. Propofol tolerance was studied in terms of IR adjustments, propofol concentration in the arterial plasma (method HPLC) and the clearance rate (CR), where CR=IR/propofol concentration. Experiment 2. Clinical, biochemical and histopathological findings following prolonged administration of propofol at high dose for the sedation of artificially ventilated rabbits. The effects of propofol 2% infusion for the sedation of artificially ventilated rabbits for a period up to 48 hours were studied in a group of animals (group P2, n=6) in terms of clinical, biochemical and histopathological findings using the model of propofol tolerance developed in Experiment 1. The initial IR (20 mg/kg/h) was adjusted, when needed, in order to maintain the desired sedation level. Another group of animals was sedated using the inhalation anesthetic sevoflurane for a period equal to the maximum surviving period of group P2 (group S, n=6). The initial sevoflurane concentration was 1.5% and was adjusted, if needed, to maintain the desired sedation level. A third group of animals received propofol’s lipid vehicle (Intralipid 10%) by continuous intravenous infusion under sevoflurane sedation for a period equal to the maximum surviving rate of group P2 (group IS, n=6). Intralipid’s IR was 131 adjusted according to the mean IR of propofol’s vehicle in group P2. During the experiment the vital signs were evaluated (every 1 hour), biochemical parameters indicative of vital organs’ function were measured (every 12 hours) and finally tissue samples were excised from the heart, the lungs, the liver, the gallbladder, the kidneys, the urinary bladder and the quadriceps femoral muscle for histologic evaluation. Orthognathic surgery means surgical correction of dentofacial and congenital deformities, which includes unsatisfactory facial aesthetics, malpositioned teeth, jaw malformations and masticatory dysfunction. Correction of these conditions requires a multidisciplinary approach with a combination of orthodontics and surgery. Successful outcome of orthognathic treatment requires pre-, intra- and postoperative considerations with a multimodal approach in order to minimize morbidity and enhance recovery after surgery. Developments within the orthognathic field should focus on multimodal approaches with combined effects of modern anaesthetic protocols, minimal invasive surgery and pharmacological modification of inflammatory responses. However, surgical morbidity after orthognathic surgery is still associated with undesirable sequelae such as damage to teeth, facial oedema, pain, neurosensory disturbances, prolonged recovery time and removal of titanium plates. Intraoperative anchorage of the occlusion is a major keystone in the implementation of the orthognathic planning during surgery. Bone anchor screws are therefore occasionally required in transalveolar positions as reinforced rigid emergency anchor for proper intermaxillary fixation in cases when orthodontic appliances loosen or when preoperative orthodontic treatment isn´t indicated. Furthermore, steroids are recommended to reduce swelling, pain, nausea and vomiting (PONV) and may promote nerve healing after surgery. The multimodal effects of steroids needs further investigation, thus the optimal dosages and the timing of administration is of great interest. Moreover, removal of inserted titanium fixation plates after surgery occur due to plate related complications. The reason for plate removal needs further investigation. Finally, different general anaesthetic protocols influence haemodynamics and subsequently postoperative pain, recovery and hospitalization. It is needed to understand the recovery process and to promote mobilization of the patient after surgery. Objectives. The aim of the first study was to evaluate two types of surgical techniques for insertion of bone anchor screws for intermaxillary fixation, regarding frequency of iatrogenic dental root injuries. The second trial investigated the efficacy of single versus repeated betamethasone doses on facial oedema, pain and neurosensory disturbances after bilateral sagittal split osteotomy (BSSO). The main objective of the third study was to investigate the incidence and reasons for removal of titanium fixation plates following orthognathic surgery, identify risk factors predisposing removal and to explore if the patients discomfort was reduced after removal. ▼ In the last decade, (non)prescription opioid abuse, opioid use disorder (OUD) diagnoses, and opioid-related overdoses have risen and represent a significant public health concern. One method of understanding OUD is as a disorder of choice that requires choosing opioid rewards at the expense of other nondrug rewards. The characterization of OUD as a disorder of choice is important as it implicates decision- making processes as therapeutic targets, such as the valuation of opioid rewards. However, reward-value measurement and interpretation are traditionally different in substance abuse research compared to related fields such as economics, animal behavior, and neuroeconomics and may be less effective for understanding how opioid rewards are valued. The present research therefore used choice procedures in line with behavioral/neuroeconomic studies to determine if drug-associated decision making could be predicted from economic choice theories. In Experiment 1, rats completed an isomorphic food-food probabilistic choice task with dynamic, unpredictable changes in reward probability that required constant updating of reward values. After initial training, the reward magnitude of one choice subsequently increased from one to two to three pellets. Additionally, rats were split between the Signaled and Unsignaled groups to understand how cues modulate reward value. After each choice, the Unsignaled group received distinct choice-dependent cues that were uninformative of the choice outcome. The Signaled group also received uninformative cues on one option, but the alternative choice produced reward-predictive cues that informed the trial outcome as a win or loss. Choice data were analyzed at a molar level using matching equations and molecular level using reinforcement learning (RL) models to determine how probability, reward magnitude, and reward-associated cues affected choice. Experiment 2 used an allomorphic drug versus food procedure where the food reward for one option was replaced by a self-administered remifentanil (REMI) infusion at doses of 1, 3 and 10 μg/kg. Finally, Experiment 3 assessed the potential for both REMI and food reward value to be commonly scaled within the brain by examining changes in nucleus accumbens (NAc) Oxygen (O2) dynamics. Results showed that increasing reward probability, magnitude, and the presence of reward-associated cues all independently increased the propensity of choosing the associated choice alternative, including REMI drug choices. Additionally, both molar matching and molecular RL models successfully parameterized rats’ decision dynamics. O2 dynamics were generally commensurate with the idea of a common value signal for REMI and food with changes in O2 signaling scaling with the reward magnitude of REMI rewards. Finally, RL model-derived reward prediction errors significantly correlated with peak O2 activity for reward delivery, suggesting a possible neurological mechanism of value updating. ▼ Introduction: Advantages of intravenous anesthesia with propofol compared to inhalational anesthesia have been dicussed controversally. Conflicting results were obtained and are partly due to the fact that different opioids were used when comparing TIVA with balanced anesthesia. The aim of our study was to compare propofol, sevoflurane and desflurane if remifentanil was used as opioid in all groups. Methods: 75 adult ASA I-III undergoing elective ophthalmic surgery were enrolled in this double blind study. After standarized premedication and induction of anesthesia patients were randomly assigned to receive TIVA (propofol/remifentail: P) or sevoflurane/remifentanil (S) or desflurane/remifentanil (D). Remifentanil was maintained at a rate of 0,3 g/kg/KG. Propofol, sevoflurane and desflurane were titrated to achieve intraoperative BIS guided anesthesia levels between 40 +/-10. To detect explicit and implicit intraoperative memory patients listened to an audio tape with the Robinson Crusoe tale. Administration of anesthetic drugs was stopped after surgical activity was finished. In the recovery period patients were reawakened only with standardized acoustic stimulation (audio tape: surgery is done, please open your eyes ). We recorded the time until first reaction, until extubation and readiness until discharge from postanesthesia care unit (PACU) according to a modified Aldrete score. Additionally congnitive tests (postbox test, attention test) were performed. All drugs given in the PACU were recorded. The first postoperative day patients were asked by a blinded investigator to evaluate the satisfaction with anesthesia using a visual analog scale (VAS) and if they would choose the same anesthesia again. Additionally explicit and implicit memory (awareness) was tested. Statistics were performed with SPPS 10.0 according to institutional statistical consultation: T-test for parametric, Mann-Whitney U-Test for non-parametric data, P values < 0,05 were considered statistically significant. Results: 13 patients had to be excluded from further analysis: 10 protocol violations, 2 data losses, 1 resuscitation after standard anesthesia induction (before study drugs were given). There were no differences in demographic data, duration of surgery, satisfaction, awareness and cognitive test results between the groups. First reaction and extubation were seen significantly earlier in the D (12.7+/-4.4min rsp. 13.5+/-5.7min) and S group (16.9+/-6.5min rsp. 17.1+/-7.7min) than in the P group (21.2+/-6.6min rsp. 21.6+/-6.5min), the difference was significant between D and S patients for the time until first reaction. Time from end of surgery until discharge from PACU was not different between the groups (D 34.3+/-11min, S 38.9+/-10min, P 39.7+/-9min). D patients needed significantly more additional medication in the PACU (D 56% vs P 35%, S 39%). Implications: Recovery times in the OR were significantly shorter in the D and S patients.
