▼ Aim: To determine an association if any, between on and off-axis
ocular aberrations and a myopic change in refractive error using the data obtained longitudinally. Methods: On and off-axis aberrations of cyclopleged eyes (5 mm pupil diameter) of 646 children were measured during 2004-2005 (age, 12.7 ± 0.4 years) and 5 years later in 2009-2010 (age, 17.1 ± 0.5 years). Change in spherical equivalent (M) from baseline to follow-up visit was determined and eyes characterised as myopic change (≥ 0.50D), no change (from 0.49D to +0.49D) and hyperopic change (≥ +0.50D). Baseline data of eyes with myopic change and no change were compared to determine if any baseline characteristics predicted a future myopic change. Also, the change in on and off-axis aberrations with time were correlated with change in M, using general linear model. Significance level was set at p<0.05. Additionally, data from 675 adolescents (16.0 ± 0.7 yrs) were analysed to determine the contribution of corneal and internal aberrations to total aberrations. Results: At baseline 15% of eyes were myopic, 27% emmetropic and 58% hyperopic. 26%, 70% and 4% of all eyes showed myopic change, no change and hyperopic change respectively. None of the baseline on and off-axis aberrations and retinal image quality measures was associated with a myopic change in M. With time, eyes with myopic change showed a decrease in on-axis spherical aberration [C(4,0)] and a hyperopic shift in relative peripheral refractive error (RPRE), whereas eyes with no change had an increase in on-axis C(4,0) and no difference in RPRE. Eyes with Type III and Type IV skiagram at baseline showed the most (61%) and the least (17%) myopic change respectively. Across refractive error (RE) groups, corneal aberrations were not different and were compensated for with internal aberrations resulting in less net total aberrations. However, internal aberrations differed between RE groups with less negative internal C(4,0) observed in hyperopes (-0.038 ± 0.05 μm) compared to emmetropes (-0.081 ± 0.04 μm) and myopes (-0.090 ± 0.04 μm). Conclusion: None of the baseline on and off-axis aberrations predicted a future myopic change in refractive error. However, the eyes with Type III skiagram (asymmetry in astigmatism in nasal and temporal quadrants) showed the most myopic change in
refraction. It may be possible this asymmetry in astigmatism (characteristic feature of Type III skiagram) observed at temporal retina could play a role in myopic change and needs to be explored further. The hyperopic shift in RPRE with time for eyes with myopic change is possibly a consequence of

▼ O objetivo deste projeto baseia-se na avaliação do efeito da instilação de fenilefrina em
diversos parâmetros oculares para averiguar a existência ou não de alterações nos mesmos. Essa
avaliação foi realizada com o propósito de comparação entre dois grupos de diferentes faixas etárias
(um grupo de voluntários jovens e um grupo de voluntários présbitas), ao longo de um período de
180 minutos, após a instilação da primeira gota do colírio. Também foi realizada uma avaliação de
todos os parâmetros no olho contralateral, para averiguar eventuais efeitos de enervação pupilar
simpática adversos à instilação do fármaco.
Foram avaliados 20 voluntários, 10 no grupo de jovens e 10 no grupo de présbitas, que
realizaram medidas de baseline e aos 15, 30, 90 e 180 minutos posteriores ao da instilação da
primeira gota de fenilefrina. Os parâmetros avaliados foram os seguintes: refração objetiva,
aberrometria, tonometria, paquimetria e biomecânica corneal, diâmetro pupilar (para diferentes
condições de iluminação), a tomografia da córnea e do segmento anterior e inquéritos subjetivos de
forma a avaliar sintomatologia (descrição e duração) inerente à instilação de fenilefrina tópica.
Os resultados experimentais indicaram o pico de ação da fenilefrina aos 90 minutos, no
entanto, estes já se mostraram significativos a partir dos 20 a 30 minutos iniciais. Ambos os grupos
de estudo, jovens e présbitas, atingiram um máximo de dilatação pupilar de cerca de 8 mm, sendo
que, no grupo présbitas, verificou-se um diferencial de diâmetro pupilar entre a baseline e os 90
minutos, superior em 2 mm, comparativamente ao grupo jovens.
Apesar de se obter uma dilatação aos 30 minutos, aos 90 minutos ainda se consegue
ganhar mais 1 mm de diâmetro pupilar, o que poderá ser importante nalgumas aplicações clínicas,
tal como, na oftalmoscopia. Aos 15 minutos de estudo verificou-se, no Grupo Jovens, um menor
valor de diâmetro pupilar face à baseline, possivelmente devido a uma adaptação à menor
iluminação da sala, apenas conseguida nos 15 minutos posteriores. No Grupo Présbitas, não se
verificou a mesma tendência, o que sugere que, no Grupo Jovens, haja uma maior latência na
absorção do fármaco, devido a barreiras biológicas mais resistentes. Para além do efeito midriático
pretendido, diversos parâmetros óticos e estruturais do olho sofreram alterações após a instilação
de fenilefrina, embora geralmente muito ligeiros. Futuros estudos com amostras mais alargadas são
necessários para confirmar as hipóteses lançadas neste manuscrito.

▼ The increasing prevalence of myopia over the past few decades and its association with potential ocular complications make myopia an important research topic. The present work is concerned with the structural and functional characteristics of a group of myopic and emmetropic individuals. The technical experiments in this work investigated firstly the effect of instrument alignment on peripheral refraction measurements and revealed that the corneal vertex was an acceptable alignment position of the Shin-Nippon NVision-K 5001 autorefractor, allowing consistent alignment with other instruments used in this research. Secondly, spectacles could be used to provide comparable vision to contact lenses in the visual performance studies. In the main experimental parts of this work, visual performance and multiple aspects of ocular structure were assessed across a wide range of eccentricities along the horizontal and vertical meridians within the same eyes. The structural properties of the myopic eye were measured through central and peripheral autorefraction, and through cornea to retina dimensions using non-contact biometry. In addition, the central and peripheral resolution acuities of myopic and emmetropic eyes for high and low contrast levels were investigated. Our structural and functional measurements revealed relatively prolate myopic eyes with reduced high contrast resolution acuity, compared to emmetropic eyes. Moreover, multiple regression analyses were performed at the fovea and outermost retinal eccentricities common to all core experiments but revealed no strong relationship between the structure and function of the myopic eye. Finally, regarding asymmetry, the nasal and superior retinae were found to be longer and to perform better in comparison to the temporal and superior retinae respectively.

▼ INTRODUÇÃO: A falta da correção óptica recentemente foi reconhecida como importante causa de baixa visão e parece ser um problema fácil de ser resolvido com o uso de óculos. No entanto, a falta da correção óptica é um problema complexo do seu diagnóstico ao tratamento, envolvendo a procura e alcance da assistência médica, além da confecção e uso dos óculos. OBJETIVOS: Determinar a frequência da necessidade de correção óptica em escolares; determinar a influência do absenteísmo no estudo da frequência da necessidade de correção óptica e suas causas; determinar o impacto da reconvocação dos faltosos na cobertura do atendimento oftalmológico; verificar cobertura oftalmológica prévia; verificar uso da correção óptica prescrita e causas de abandono de óculos. MÉTODO: Foi realizado um estudo transversal analítico. Escolares de 7 a 10 anos do ensino público de Guarulhos foram triados e encaminhados para exame oftalmológico na Campanha Olho no Olho 2006 e 2007. RESULTADOS: Foram triados 40.197 escolares e encaminhados 11.741 escolares com suspeita de problemas visuais. O absenteísmo na primeira convocação foi 42.5% e na reconvocação foi 62.2%. A Campanha alcançou 73.6% dos escolares encaminhados. A reconvocação ampliou a cobertura do projeto em 16.1%. A falta de orientação (não ter recebido aviso ou guia de encaminhamento) foi a razão mais frequente (35.6%) alegada para o não comparecimento, seguida de trabalho (20.6%) e doença (12.4%). A necessidade de óculos não foi fator associado ao comparecimento. Para 60.2% dos escolares foi a primeira oportunidade de exame ocular, sendo que 27.3% procuraram mas não tiveram acesso a consulta médica. Dos 1.379 escolares que já haviam feito exame, 48.3% utilizaram o SUS e 1.029 receberam prescrição de óculos em exame prévio. Destes, 899 (87.4%) fizeram os óculos e apenas 590 (57.3%) usavam os óculos no dia do exame. Dos óculos doados na Campanha, 84.7% dos escolares utilizavam a correção óptica após um ano. A estimativa da frequência da necessidade de correção óptica, na população estudada, foi de 6.9%. CONCLUSÕES: O erro refracional não corrigido foi a maior causa de baixa acuidade visual entre os escolares. A Campanha foi a primeira oportunidade de exame ocular para a maioria dos escolares. Existe uma parcela de pais que não leva seus filhos para exame, mesmo com oferecimento de facilidades (acesso, transporte, alimentação, duas oportunidades diferentes de exame gratuito realizado em fins de semana e doação de óculos). O absenteísmo poderia ser evitado em 87,1% dos casos, com melhor estruturação da primeira convocação. A reconvocação não é recomendada quando os recursos financeiros são escassos

A ametropia é uma das causas de diminuição da acuidade visual, podendo comprometer grandemente a visão de um cão. Neste estudo pretende-se avaliar o erro refrativo em cães submetidos a cirurgia de cataratas por facoemulsificação com implante de lente intraocular, a eficácia do poder dióptrico usado das lentes intraoculares usadas atualmente, estabelecer uma correlação entre cães microftálmicos e os seus valores refrativos e em paralelo avaliar o estado refrativo de uma população de cães saudáveis que constituem o grupo controlo.
Uma população de 53 cães, perfazendo um total de 99 retinoscopias, foi dividida em 3 grupos: pseudofáquicos (n=43 olhos), afáquicos (n=4 olhos) e fáquicos (n=52) correspondendo ao grupo controlo. As retinoscopias foram feitas através da refração dos meridianos vertical e horizontal; os meridianos oblíquos apenas foram avaliados na presença de reflexos em tesoura. Não foram usados quaisquer fármacos cicloplégicos.
O estado refrativo médio foi de +3.74 +/-5.45D (-3.5; +21D) no grupo dos pseudofáquicos; +16.94 +/-1.39D (+15.5;+18.25D) nos afáquicos e -0.03 +/-1.64D (-5.0;+4D) nos fáquicos. No grupo dos pseudofáquicos 72.2% apresentaram hipermetropia e 13.8% miopia e emetropia. Foi diagnosticado astigmatismo em 3 olhos. Observou-se anisometropia em 10 cães (1;20.5D) e antimetropia em dois. A correlação entre o valor refratado e o comprimento axial sugere uma tendência para a emetropia em animais com diâmetro ocular superior (P= 0.0845; rho=-0.3). No grupo fáquico, 44% apresentaram-se emétropes, 30% hipermétropes e 26% míopes. A hipermetropia baixa foi predominante nos cães hipermétropes (66.7%). Não foram observados casos de astigmatismo ou anisometropia. Foi encontrada uma relação entre o estado refrativo e o tamanho da raça no grupo fáquico (P=0.002). A análise de correlação entre o valor refratado e a idade dos cães sugere a tendência para a miopia ou diminuição de valores de hipermetropia em cães mais velhos (P = 0.054; rho = -0.273).
As raças médias demonstraram estar mais próximas da emetropia quando comparadas com raças grandes e pequenas (P=0.002). No grupo operado, as raças grandes estavam mais adaptadas a IOLs de 41D, enquanto as raças médias e pequenas apresentaram grande variabilidade no estado refrativo. Os cães microftálmicos operados apresentaram alta hipermetropia. Mais estudos devem ser conduzidos para relacionar caraterísticas biométricas individuais com o poder dióptrico necessário, levando assim a uma oferta mais alargada de IOLs em cães.
ABSTRACT - Ametropia is a possible cause for diminished visual acuity in dogs and may lead to a severe compromise of their vision. ▼ Aberrations affect image quality of the eye away from the line of sight as well as along it. High amounts of lower order aberrations are found in the peripheral visual field and higher order aberrations change away from the centre of the visual field. Peripheral resolution is poorer than that in central vision, but peripheral vision is important for movement and detection tasks (for example driving) which are adversely affected by poor peripheral image quality. Any physiological process or intervention that affects axial image quality will affect peripheral image quality as well. The aim of this study was to investigate the effects of accommodation, myopia, age, and refractive interventions of orthokeratology, laser in situ keratomileusis and intraocular lens implantation on the peripheral aberrations of the eye. This is the first systematic investigation of peripheral aberrations in a variety of subject groups. Peripheral aberrations can be measured either by rotating a measuring instrument relative to the eye or rotating the eye relative to the instrument. I used the latter as it is much easier to do. To rule out effects of eye rotation on peripheral aberrations, I investigated the effects of eye rotation on axial and peripheral cycloplegic refraction using an open field autorefractor. For axial refraction, the subjects fixated at a target straight ahead, while their heads were rotated by ±30º with a compensatory eye rotation to view the target. For peripheral refraction, the subjects rotated their eyes to fixate on targets out to ±34° along the horizontal visual field, followed by measurements in which they rotated their heads such that the eyes stayed in the primary position relative to the head while fixating at the peripheral targets. Oblique viewing did not affect axial or peripheral refraction. Therefore it is not critical, within the range of viewing angles studied, if axial and peripheral refractions are measured with rotation of the eye relative to the instrument or rotation of the instrument relative to the eye. Peripheral aberrations were measured using a commercial Hartmann-Shack aberrometer. A number of hardware and software changes were made. The 1.4 mm range limiting aperture was replaced by a larger aperture (2.5 mm) to ensure all the light from peripheral parts of the pupil reached the instrument detector even when aberrations were high such as those occur in peripheral vision. The power of the super luminescent diode source was increased to improve detection of spots passing through the peripheral pupil. A beam splitter was placed between the subjects and the aberrometer, through which they viewed an array of targets on a wall or projected on a screen in a 6 row x 7 column matrix of points covering a visual field of 42 x 32. In peripheral vision, the pupil of the eye appears elliptical rather than circular; data were analysed off-line using custom software to determine peripheral aberrations. ▼ Background. The study examined the effects of optical blur on the visual performance and comfort of computer workers. Since most workers have significant degrees of optical blur, the study may elucidate improved methodology for workplace comfort and function.
Methods. Since young and old workers experience blur from different sources, the study incorporated two different designs. The study examined the correction of optical blur using a randomized, double-blind, placebo-controlled, crossover design. The primary outcomes of the study were visual comfort and productivity. In study one, subjects were required to be 19-35 yrs of age, have at least 0.50D uncorrected refractive error (URE) in at least one eye, use a computer for at least1-hr/day and have at least 20/40 corrected visual acuity (VA) in each eye. During two one-month study periods, subjects wore either lenses fitted for their best or habitual correction and completed 4 hours of testing. In study two, subjects were required to be 40 years of age or older with corrected VA of at least 20/40 and to require a near plus lens addition. Subjects completed a total of 4 hours of testing in ten 15-minute periods with their near plus prescription and a test pair of lenses (plano, +/-0.50 or +/-1.00D) with head free or fixed on a chin rest.
Results. Analysis for the first study confirmed a relationship between the visual comfort index (VCI) and the number of eyes meeting the criteria of 0.50D URE and the task order (p = 0.0003, 0.0025) and suggested a trend for a similar relationship for correct comfortable work (CCW, p=0.0149). For the second study, the VCI declined by 15.4% (p = 0.0001) while total correct work (TCW) and CCW increased (36.4, 25.9%, respectively; p=0.0001, 0.0094) over the 2.5 hr work period. Analysis supported a hypothesis of a significant interaction of lens and head for VCI and CCW (p=0.0049, 0.0088, respectively) as well as a main effect for head position for TCW (p = 0.0069).
Conclusion. Subjects were most comfortable and productive while working with low or absent degrees of optical blur in at least one eye.

▼ Stopa prematuriteta konstantno raste, a sve više prevremeno rođenih beba koje prežive neonatalni i perinatalni period zahteva adekvatno sistemsko i oftalmološko praćenje i tretman. Prematuritet sam po sebi, predstavlja rizik za razvoj vida. Taj rizik dodatno povećava prisustvo prematurne retinopatije i refraktivnih mana. Skrining i tretman promena na retini u sklopu prematuriteta, kao i optička korekcija refraktivnih mana obezbeđuju uslove za razvoj vida. Cilj ovog istraživanja je bio utvrditi promene biometrijskih karakteristika oka prematurusa, sa i bez prematurne retinopatije, tokom šestogodišnjeg praćenja, utvrditi refraktivni status ove dve grupe dece sa šest godina, te povezati promene biometrijskih karakteristika oka sa refrakcijom. U ispitivanje je uključeno 192 prevremeno rođena deteta (384 oka). Vršena su tri pregleda, u uzrastu od 3 meseca, 12 meseci i 6 godina starosti. Nakon prvog pregleda ispitivani uzorak je, u zavisnosti od nalaza na očnom dnu, podeljen na bebe sa prematurnom retinopatijom i bebe bez ovog oboljenja, a kako bi se uporedile proučavane karakteristike. U sva tri navedena uzrasta je pregledano očno dno i merene su tri glavne biometrijske karakteristike koje utiču na refraktivni status: aksijalna dužina oka, dubina prednje očne komore i debljina očnog sočiva. Sa šest godina je određena refrakcija, uključujući zakrivljenost rožnjače odnosno kornealni astigmatizam, vidna oštrina bez korekcije refraktivne mane, a zatim i nakon korekcije. Ostali neophodni podaci su dobijeni iz medicinske dokumentacije. Pokazano je da prematurna retinopatija utiče na refraktivni status i vidni ishod sa šest godina, pre svega uticajem na zakrivljenost rožnjače, aksijalnu dužinu oka i dubinu prednje očne komore, dok su promene debljine očnog sočiva bez posebnog značaja. Vidne oštrine oba oka u predškolskom uzrastu prevremeno rođene dece sa prematurnom retinopatijom su statistički značajno manje kada se uporede sa vidnim oštrinama prevremeno rođene dece bez retinopatije, ali razlika u ovim dvema vrednostima gubi statističku značajnost nakon optičke korekcije. Učestalost astigmatizma u prematurnoj populaciji sa 6 godina je visoka, a od onih koji imaju astigmatizam u tom uzrastu, više od dve trećine je imalo prematurnu retinopatiju. Ukupna učestalost miopije, hipermetropije i emetropije u prematurnoj populaciji sa 6 godina je 18,9%, 54,7% i 13,2%, a preostalih 13,2% su anizometropi. U više od trećine dece, sa prematurnom retinopatijom u najranijem uzrastu, sa šest godina postoji gubitak vidne sposobnosti, koji je najčešće blag, no može biti i potpun.

▼ INTRODUÇÃO: Aproximadamente 20% das crianças brasileiras em idade escolar apresentam algum problema oftalmológico e cerca de 95% dos distúrbios poderiam ser evitados ou minorados com promoção de saúde e assistência. A triagem visual é realizada por agentes comunitários de saúde, professores e alfabetizadores. Apesar do treinamento que os professores recebem, muitas crianças são dispensadas na triagem realizada nos serviços especializados e ainda existe um alto índice de absenteísmo, evidenciando-se a necessidade de melhorar a gestão dos recursos humanos e financeiros envolvidos no rastreamento visual e refrativo de escolares do ensino fundamental. OBJETIVOS: O estudo buscou comparar o rastreamento visual com corte de acuidade visual monocular, sem correção, <= 0,7, corte <= 0,6, medida de acuidade visual binocular e corte <= 0,7 e rastreamento refrativo com Spot Vision ScreeningTM PediaVision (SPOT). O segundo objetivo deste estudo foi avaliar a performance do SPOT como auto-refrator. MÉTODOS: Durante o Projeto Visão do Futuro, ocorrido em 2012, no HCFMUSP, avaliamos 1554 crianças, que preencheram os critérios de inclusão do estudo e destas, 148 foram submetidas ao photoscreening. RESULTADOS: A adoção de corte <= 0,6 no rastreamento visual monocular reduziria em 29% o número de crianças encaminhadas para avaliação oftalmológica e a triagem binocular com corte <= 0,7 em 41,1%, porém 61 casos de ambliopia deixariam de ser diagnosticados. O SPOT apresentou sensibilidade em detectar erro refracional com necessidade de prescrição de 73,3% e especificidade de 93,18% e, em média, a diferença da refração do Spot com a refração clínica subjetiva foi de + 0,63 DE com -0,33 DC no eixo de 4°, para o olho direito de cada paciente. CONCLUSÕES: As mudanças de corte para 0,6 e binocular 0,7 reduzem consideravelmente o número de avaliações, porém, a última deixa de diagnosticar parcela importante das crianças amblíopes. O SPOT apresentou bons índices de sensibilidade e especificidade no rastreamento refrativo em escolares e os valores de refração obtidos com este equipamento assemelham-se clinicamente aos valores de refração clínica subjetiva

▼ Purpose: To investigate the prevalence, distribution, demographic, anthropometric and biometry associations of refractive errors in a sample of adolescent pupils in Thessaloniki, Greece. Methods: 709 pupils answered in a detail questionnaire about near work activity, educational experiences, parental myopia and family demographics. Then, they had a physical (height, weight, periphery of the head) and ophthalmological examination (refraction and biometry) by the same ophthalmologist. Results: The prevalence of myopia, hypermetropia, astigmatism and anisometropia was 50,92%, 4,65%, 31,17% and 10,6% respectively. Myopia appears particularly strong statistical association with neat-work and school achievement, the myopia of mother and brothers (a weaker one with the myopia of father), with height, weight and BMI. Myopia also, has a particular strong statistic relation with the axial and vitreous cavity length, a statistical one with the anterior chamber depth and lens thickness, but only a weak one with the radius of corneal curvature. Conclusions: Myopia is the most often refractive error in adolescence. Its pathogenesis due to genetic factors and environmental ones, as well. The latter are responsible especially for the increase of the prevalence of myopia, with the intensive rhythms of school life, the general changes of the way of living with the abandonment of the country-life and the increase of the urban residence.
Σκοπός: Να προσδιοριστεί η σχέση των διαθλαστικών ανωμαλιών με επιδημιολογικούς παράγοντες και με τα σωματικά και βιομετρικά χαρακτηριστικά εφήβων μαθητών. Δείγμα και μεθοδολογία: 709 μαθητές λυκείου και των δύο φύλων, ηλικίας 15-19 χρ. από 16 σχολεία του πολεοδομικού συγκροτήματος της Θεσσαλονίκης, συμπλήρωσαν ένα ερωτηματολόγιο σχετικά με τις ατομικές τους συνήθειες, το ατομικό και οικογενειακό οφθαλμολογικό ιστορικό, καθώς και το κοινωνικοοικονομικό επίπεδο της οικογένειάς τους. Κατόπιν, μετρήθηκε το ύψος, το βάρος τους και υπολογίστηκε το ΒΜΙ, η διάθλασή τους και τα βιομετρικά χαρακτηριστικά τους. Αποτελέσματα: Η μυωπία παρουσιάζει συχνότητα 50,92%, ενώ η υπερμετρωπία 4,65%. Ο συνολικός αστιγματισμός φτάνει το 31,17% και η ανισομετρωπία στο 10,6%. Η μυωπία βρέθηκε να έχει στατιστικά σημαντική σχέση με: κοντινή εργασία, σχολική επίδοση, οικογενειακή μυωπία, ύψος, βάρος, BMI και την βιομετρία. Συμπεράσματα: Η μυωπία είναι η πιο συχνή διαθλαστική ανωμαλία στην εφηβική ηλικία και παρουσιάζει μεγάλη αύξηση τα τελευταία χρόνια, που πιθανόν οφείλεται στην αύξηση της κοντινής εργασίας (εντατικοποίηση του Λυκείου, φροντιστήρια για τις Πανελλήνιες, πολλά επιπλέον μαθήματα των παιδιών-εκτός του σχολείου, αυξημένη χρήση Η/Υ), στην αστικοποίηση πληθυσμού και στην βελτίωση επιπέδου διαβίωσης.
