▼ Background: Despite advances in techniques and analgesics for pain management, pain remains a major health problem. Regular assessment and reassessment of pain using guidelines with measurable goals is essential for effective pain management in acute hospital settings. Unfortunately, no such guidelines exist in South Africa. To implement appropriate precepts for the South African context, current practice must be understood. Aim: To evaluate pain assessment and management of patients in two surgical wards at Groote Schuur Hospital. Setting: Groote Schuur Hospital is a government-funded tertiary academic institution within the Western Cape Province of South Africa. The patients at this hospital are generally from the low-income strata and live in resource-poor communities. Methods: A cross-sectional, retrospective medical record audit was conducted. The folders of all 215 patients admitted to a specific orthopaedic trauma and urogynaecological ward at Groote Schuur Hospital in June 2015 were targeted for review. Medical folders not available or with missing notes were excluded. Variables evaluated included the number of pain assessments recorded, pain assessor, assessment tool and management plan. Results: 168 folders were available for review. Nearly half of the patients had no documented pain assessment. When pain assessment was conducted, the verbal rating scale was the predominant tool used, and assessments were mostly conducted by the ward doctor. Pain interventions appeared to be primarily based on the professional knowledge and experience of the practitioner and were not evidence-based. Conclusion: Pain assessment and management was a problem in the two wards reviewed, which is similar to the findings of studies referenced in this text. Health professionals need to be empowered to manage pain adequately. An assessment tool, which integrates the biopsychosocial factors that influence the pain experience, should be routinely employed by a multidisciplinary team to facilitate goal-directed therapy. ENGLISH SUMMARY : It is known that the South African healthcare system is overwhelmed by a relentless burden of infectious and non-communicable diseases, social inequalities, and insufficient human resources to provide healthcare to the growing population. Therefore, patient flow management has become the cornerstone of patient processing and throughput in hospitals in order to alleviate the disproportionately high burden of disease. To compound the issues of patient flow inefficiencies and unexplored South African patient flow practices, healthcare services are continually attempting to improve their systems in order to adequately address the challenges they face. However, the reality of patient flow management in hospitals is heterogeneous and methods to improve patient flow do not result in similar outcomes, but enough evidence exists that patient flow-related methods and techniques can assist hospitals to improve patient flow. Therefore, it is believed that healthcare professionals’ perception of patient flow is mainly influenced by their background or work environment. The goal of this study is to explore the patient flow system of GSH. The researcher describes the current patient flow system of GSH by using information from hospital statistics, reports and project information. A questionnaire and interviews were used to collect empirical data of how nurses, doctors and managers perceive the patient flow system of GSH. The preliminary results of the research show operational employees find it extremely difficult to ensure efficient patient movement without proper patient flow guidelines. It is also revealed that access block is one of the biggest contributors of increased waiting times. Although GSH managers promote innovation and improvements to the patient flow system, operational staff argues that additional tasks hampers their ability to perform their normal duties optimally, which proves that managers and operational staff does not share the same vision. Another interesting result is that patient flow is not being measured at GSH, which makes it difficult to monitor improvement strategies and the impact new innovations have on the patient flow system. Following an in-depth multi-dimensional analysis and discussion of the preliminary research results, suitable recommendations are presented, which could be used to improve the patient flow management system of GSH.

▼ This dissertation investigates the evolution of environmental protection (EP) policy in Hubei Province from the early 1970s to the middle 1980s. It addresses the local impacts of economic and environmental policies during this remarkable period of transition in China, taking Hubei as a case study to capture the local evolution of policy implementation and establishment of EP by state agencies in China. Discussion illuminates the early origins of a shift in governance in Hubei from an agenda dominated by economic growth objectives toward more balanced regulation, revealing a series of features in China’s early policy failures in the regulation of industrial waste emissions, and later policy adaptations by sub-national government agencies. The study consists of an introduction and six substantive chapters. Chapter 2 outlines Hubei’s historical and environmental setting. Chapter 3 examines how agencies responsible for EP were established in Hubei from the early 1970s to the mid-1980s, identifying four groups of agencies. Chapter 4 reviews scholarly writing by Chinese economists on EP issues, illuminating the early formation and underlying assumptions of China’s national economic policies of EP during the 1970s and 1980s. Chapters 5 and 6 present case studies on Hubei’s industrial waste pollution, describing how communities were affected by industrial waste emissions that imposed enormous damage to water, air and soil. Chapter 7 examines how local EP agencies struggled with administrative red tape, under-funding, and under-staffing. Guided by their mandate to protect the environment and by growth-oriented economic principles, Hubei’s EP agencies also demonstrated local initiative and policy adaptation in implementing EP policies. In conclusion, I discuss how practices of pricing the “right to pollute” evolved in Hubei and how the practical recognition of industries’ right to pollute compromised the local implementation of national environmental policies.

▼ Turvallisuus on käsite, joka tällä hetkellä näyttää liittyvän itsestään selvänä, mutta hahmoltaan epämääräisenä niin kansalaisten hyvinvointiin kuin kaikkeen muuhunkin yhteiskunnalliseen toimintaan. Turvallisuus yhdistetään myös erilaiseen huono-osaisuuteen, jota nykyään kutsutaan usein syrjäytymiseksi. Konkreettisena osoituksen turvallisuuden immanenssista syrjäytyminenkin nimetään sisäasiainministeriön julkaisemassa viimeisimmässä sisäisen turvallisuuden ohjelmassa turvallisuuden uhaksi. Tutkimuksessani pyrin jäljittämään tähän johtanutta kehitystä ja tarkastelemaan sitä osana laajempaa yhteiskunnallisen toimintamallin muutosta. Yhteiskunnallinen toimintamalli kriisiytyi 1990-luvun vaihteessa. Työssäni tutkin suhtautumista huono-osaisuuteen kyseisen murroksen molemmin puolin. Tarkastelen kriittisesti erityisesti sitä, kuinka asennoituminen huono-osaisina tai syrjäytyneinä pidettyihin ihmisiin on muuttunut yleisen järjestyksen säilymisen ja kriminologian näkökulmasta. Vertailen ajankohtiin liittyvien dokumenttiaineistojen avulla huono-osaisuuden ja syrjäytymisen problematisointitapoja ja ratkaisumalleja suomalaisen hyvinvointivaltiollisen kehityksen voimakkaassa vaiheessa 1970-luvulla, erityisesti kymmenluvun alussa, ja toisaalta 2000-luvun ensimmäisellä vuosikymmenellä. Lähestymistapani on empiirinen ja teoreettinen. Olen lähtenyt siitä, että huono-osaisuus, köyhyys ja erityyppinen marginaalisuus ovat suhteellisen pysyviä ongelmoinnin kohteita jopa vuosisadasta toiseen. Kysymyksen hallinnalliset ratkaisut kertovat sen vuoksi ainakin yhtä paljon kulloisestakin yhteiskunnasta ja vallinneista ajattelutavoista kuin marginaalisuudesta itsestään. Teoreettisia välineitä tutkimuksen suorittamiselle antavat semiotiikka, diskurssianalyysi ja jälkifoucaultlainen hallinnan analytiikka. Rinnalla ja erikseen pohdin turvallisuuden sisältöä ja merkitystä tässä sommitelmassa. Vertailun perusteella ja hallinnan analytiikkaan kontekstoituina on pääteltävissä, että aikaisemman valtiokeskeisen sosiaalisen tukemisen ja puuttumisen sijaan huono-osaisuuden hallinnassa ja sen myötä yleisen järjestyksen ja turvallisuuden tavoittelussa on tapahtunut siirtymää yhteisöjen ja yhteisöllisyyteen vetoamisen käyttöön tässä pyrkimyksessä. Myös vastuullisen vapauden ja aktiivisen kansalaisuuden teroittaminen nähdään keinoiksi estää kielteinen kehitys. Oman vastuun korostumisen ohella jako mukanaoleviin ja ulkopuolelle joutuneisiin on kuitenkin jyrkentynyt. On merkkejä siitä, että ehdottoman turvallisuuden perfektionistinen tavoittelu on johtanut jopa neuroottisiin reagointitapoihin ja tämän ihmisluonteeseen kuuluvan piirteen soveltamiseen käyttäytymisen ohjailussa.

▼ Cette recherche se penche sur un événement souvent négligé dans la littérature et qui constitue pourtant la pierre angulaire de l'histoire actuelle de l'Afghanistan: il s'agit de la révolution afghane de 1978 qui a précédé la soi-disant invasion soviétique de ce pays. Elle met en lumière l'absence de dialogue théorique entre les auteurs qui l'ont abordée et relève comment cette absence limite la compréhension de la société afghane et, de ce fait, la compréhension de la nature du conflit ainsi que ses facteurs déterminants. Nous soutiendrons la thèse à l'effet que le fil conducteur manquant, qui relie les différentes analyses, est la corrélation entre la lutte de classes et les intérêts nationaux des puissances régionales et internationales, c'est-à-dire le système capitaliste ayant besoin de l'État comme instrument au service des intérêts privés d'une classe dominante. Afin de mieux saisir notre objet d'étude, nous en circonscrirons la période analysée, soit de 1973 à 1992, à partir de cette revue de la littérature mais avec la perspective d'une analyse critique et d'une confrontation des discours. Nous mettrons respectivement en lumière le conflit afghan en regard de ses dynamiques internes, du débat sur la nature, les causes et les conséquences de la révolution et d'une analyse de la contre-révolution et de la lutte de classes. Nous présenterons les trois différentes interprétations du conflit afghan avec trois théories des relations internationales. Nous analyserons la complexité de la société afghane à travers son passé et son agrégat sociopolitique, résultat d'une construction sociale séculaire et d'une grande diversité culturelle, linguistique et religieuse. Nous examinerons ensuite la Révolution afghane à travers un débat théorique sur ses causes, sa nature et ses conséquences, en démontrant l'importance de cet événement. En dernier lieu, nous établirons le lien à faire avec la lutte de classes qui s'est jouée à différents niveaux, par l'étude de la contre-révolution dans laquelle sont impliqués divers acteurs nationaux, régionaux et internationaux. Nous conclurons qu'il y a bel et bien eu une lutte de classes qui a mené à la révolution et à la contre-révolution. Nous soulignerons également qu'une analyse basée sur le simple déterminisme économique ne permet pas de comprendre le conflit afghan. ▼ Successful treatment of canine pyoderma has become increasingly difficult due to the development of antimicrobial resistance and recurrence of infection. The development and spread of antimicrobial resistance has major implications because treatment failures have been associated with increased mortality, morbidity and costs related to disease. All canine skin samples submitted to Vetdiagnostix Veterinary Pathology Services for microbiological culture and sensitivity between January 2007 and June 2010, from which Staphylococcus intermedius was isolated, were selected for this investigation. A total of 319 samples from dogs of various ages, sexes and breeds from various locations throughout South Africa were obtained. In addition to the antimicrobial susceptibility data, data relating to dog signalment (age, gender, breed), case history and any other aspects of the history provided e.g. diagnosis, sample type and geographical location of the patient were collected. The number of skin samples yielding Staphylococcus intermedius was high in dogs up to the age of 6 years and, then decreased with only a few cases in dogs aged 11 years or older. The distribution of samples collected in 2010 was unusual in that there were two peaks, one in dogs two years old or less and one in 6 to 9-year-old dogs. With the exception of skin samples taken in 2008, a high percentage of dogs were under the age of one year old. 2008 had a lower percentage of affected dogs less than one year of age compared to the other years. Staphylococcal pyoderma is more common in dogs below the age of five years. Dogs above this age are less likely to contract this condition. The genders tended to be equally distributed throughout the years, with almost equal proportions of affected males and females present. The Bull Terrier types and Shepherd types were grouped separately as they were over-represented and are known to be prone to pyoderma. Large short haired dogs were consistently worse affected throughout the years sampled, followed by the Bull Terrier types. In 2010, small short haired breeds were worse affected compared to the numbers between 2007 and 2009. The monthly distribution, with the exception of 2009, tended to be consistent throughout the years. The unusual temporal distribution in 2009 could be associated with a general distribution in the number of samples compared to the other years sampled. Samples tended to be mainly from practices located in the KwaZulu-Natal province of South Africa. This is most likely due to the fact that the laboratory is located in this province, with a courier network that arranges collection from these practices. Practices in other regions send their samples to the laboratory via private courier companies and the South African Post Office. Antimicrobial resistance of S. intermedius was greatest to ampicillin followed by tetracycline and then potentiated sulphonamides. The results also showed that, in general, antimicrobial resistance was low. Very few methicillin resistant isolates were detected. Esta dissertaÃÃo analisa as experiÃncias sindicais dos trabalhadores da construÃÃo civil de Fortaleza no perÃodo compreendido entre meados da dÃcada de 70 e 90. Analisando as peculiaridades do mundo do trabalho na construÃÃo civil, particularmente o cotidiano operÃrio nos canteiros de obras, o estudo procura destacar o desenvolvimento da oposiÃÃo sindical no ano de 1988 com uma proposta de reorganizaÃÃo das relaÃÃes sindicais que eram desenvolvidas pela direÃÃo do sindicato desde 1976, e assim refletir sobre o estabelecimento de uma convergÃncia de interesses em relaÃÃo aos operÃrios da categoria. Assim, analisamos como foram construÃdas as primeiras experiÃncias nessa categoria a partir das concepÃÃes polÃtico-sindicais da nova diretoria: o I congresso dos trabalhadores da construÃÃo civil, as primeiras greves gerais, a elaboraÃÃo do novo estatuto e suas diretrizes polÃtico-jurÃdicas, o projeto de educaÃÃo popular, a construÃÃo do jornal operÃrio, as atividades de lazer e o desenvolvimento de redes de solidariedade nos momentos de agudizaÃÃo nos embates com os empresÃrios do setor sÃo algumas dessas experiÃncias que, ao serem construÃdas pelos operÃrios, eles acabavam por se construir enquanto agentes protagonistas no seu mundo de trabalho.

This composition analyzes the syndical experiences of the workers of the civil construction of Fortaleza in the period between the 70âs and the 90âs. Analyzing the peculiarities of the world of the work in the civil construction, particularly the daily laborer in the seedbeds of workmanships, the study wants to detach the development of the syndical opposition in 1988 with a proposal of reorganization of the syndical relations that were developed by the direction of the union since 1976, and this way reflect on the establishment of a convergence of interests in relation to the laborers of the category. Thus, we analyze how the first experiences in this category from the politician-syndical conceptions of the new direction had been constructed: the I congress of the workers of the civil construction, the first general strikes, the elaboration of the new statute and its politician-legal lines of direction, the project of popular education, the construction of the laboring journal, the leisure activities and the development of nets of solidarity at the moments when the conflict with the entrepreneurs of the sector became hard are some of these experiences that, when being constructed by the laborers, finished for constructing themselves agents protagonists in their world of work.

▼ Au départ phénomènes indépendants, le narcotrafic et le paramilitarisme ont convergé à partir des années 1980 dans des structures politico-militaires qui ont, depuis lors, marqué profondément la réalité colombienne. Le narco-paramilitarisme a rallié les objectifs antisubversifs de la classe dominante et a brouillé le spectre social du conflit. De plus, il a permis l'apparition d'une classe émergente au service des groupes dominants qui, en s'enrichissant, a obtenu le droit d'entrer dans le cercle exclusif du pouvoir: peu après les élections législatives de 2002, les paramilitaires déclaraient avoir pris le contrôle de 35% du Congrès, alors qu'ils contrôlaient déjà une grande partie du territoire national. Ceci est une illustration de la reconfiguration politique qui s'est encore consolidée par la récente démobilisation de 41 000 paramilitaires (2002 -2006). L'objectif de la présente recherche est de comprendre le modèle violent de développement économique de la Colombie, de caractériser son économie actuelle et d'identifier les rapports sociaux de production prédominants. Afin de dépasser la superficialité et la fragmentation des études conjoncturelles et des récits de la violence, nous étudions comment se forment historiquement les conditions favorables à ce phénomène mafieux. Nous nous pencherons sur la logique sociale qui est à son origine, notamment le contexte de la guerre froide et certains aspects de l'héritage colonial. En nous basant sur l'approche du matérialisme historique et de la sociologie historique, particulièrement sur les concepts de rapports sociaux de production (RSP) et d'accumulation extra-économique, nous analysons l'essor du narco-paramilitarisme et la complexité du conflit dans le modèle de développement colombien. Cette recherche démontre que le mode de production existant en Colombie est imprégné des mécanismes d'accumulation extra-économiques par une articulation des modes de production qui n'est pas temporaire mais stable. ▼ Influential authors fashioned a pessimistic take on society in Dutch literature between 1990 and 2005, in which the ‘West’ and ‘Islam’ almost automatically collide. In their work and in their columns, interviews and other contributions to the public debate, they sketch a seemingly unbridgeable divide between Muslims and non-Muslims. Literary scholar Sjoerd-Jeroen Moenandar comes to these conclusions in his thesis, for which he will be awarded a PhD by the University of Groningen on 22 March 2012. The alleged collision between the ‘West’ and ‘Islam’ has been a hot topic in the Netherlands for some years now. PhD student Sjoerd-Jeroen Moenandar has outlined how Kader Abdolah, Robert Anker, Abdelkader Benali and Hafid Bouazza have conveyed this collision in their work. The authors were chosen for their popularity within the literary world and outside it. They have received prestigious awards, and their work is widely reviewed and sells well. In addition, the writers are interviewed regularly and engage in the wider public debate with columns, essays and opinion pieces. Pessimistic view Moenandar finds that all four authors sketch a pessimistic view of society. The divide between Muslims and non-Muslims seems to be unbridgeable in their work. A striking similarity between the works Moenandar studied is that many of their ‘Western’ and ‘Islamic’ characters are completely convinced of the differences between their cultures. They are incapable of setting themselves in someone else’s world, and incomprehension and even violent clashes are the result. Caricatures The authors all magnify the differences between the cultures in their own manner, Moenandarestablishes. Anker, Bouazza and Benali caricaturize the Muslim and Dutch stereotypes and in so doing call into question just what Muslims and Dutch really are. They challenge the reader by bringing up cliché after cliché about ‘Muslims’ and ‘Dutch’, only to ultimately pull out the rug from under them. The writers seem to suggest that if we had a more nuanced take on each other’s cultures, the divide between them would be bridgeable. However, they make very clear that this is a thought experiment, a fata morgana; in reality the divide between Muslims and non-Muslims will never be mended. Role in society The public hopes that writers can contribute to resolving problems in society, according to Moenandar. Of the authors he studied, Kader Abdolah best fits this expectation. Moenandar: ‘In contrast to the other authors, Abdolah does not play games with clichés and caricatures. He does not use them to ironize the supposed collision of civilizations or to ridicule it, but to portray the collision between the West and Islam as a manageable problem. As a result, Abdolah tells his Western readers little more than what they already supposed they knew about the “mystical world of the Orient”. Forschungsdesiderat war die Phase der Reurbanisierung Wiens 1970-1990, als deren wesentliches Merkmal die Alternativlokal-, Diskotheken- und Clubszene untersucht wurde. Drei wesentliche Forschungsstränge sind: + Wiener Stadtentwicklung: Gewachsene Eigendynamik der Musik- und Kunstszene bzw. von der Wiener Stadtregierung gesteuerte Maßnahmen (vermehrtes Kulturangebot im Fahrwasser des gesamten gesellschaftlichen Modernisierungsschubs unter Bruno Kreisky). – 1 CD in der Printversion vorhanden + Jugendkultur mit ihren Vorläufern in den 1950er/60ern (Halbstarke, Rock'n'Roll) und die konkrete Betrachtung der 1970er/80er, als die Alternativszene entstand. + „Phänomen Diskothek“ und die Wiener Musiklokalszene im Speziellen (Indikator der Reurbanisierung). Zur Illustration dienen vergleichende Karten mit Lokalkonzentrationen, eine chronologische Zeitleiste sowie ein Überblick über die Lokallandschaft des Untersuchungszeitraumes 1970-1990. Der Quellenkorpus setzt sich zusammen aus Archivalien des Falter Verlags, der Statistik Austria sowie diversen Bild- und Tonquellen (u.a. die Österreichischen Charts prägenden Austropop-Hits). Oral History ist die bei 10 Erinnerungsinterviews mit ZeitzeugInnen der Jahrgänge 1937-1959 primär verwendete Methode. Intention war es nicht, ein quantitativ aussagekräftiges, objektives Bild zu zeichnen, sondern schlaglichtartige Einblicke in diese Zeit zu gewähren und die Geschichte jenes Wandels zu erzählen, mittels dem Wien weltoffener und bunter wurde. Mitte der 1970er war Wien dem allgemeinen Tenor zufolge v.a. abends alles andere als lebendig. Lösungsansätze wie die Einrichtung von Fußgängerzonen, die Verdichtung öffentlicher Verkehrsmittel und die Eröffnung von neuen Alternativlokalen, Diskotheken und Clubs markierten den Beginn einer Entwicklung, die Wien wieder attraktiver, jugendlicher und moderner machte und leisteten einen Beitrag, dieses heute so selbstverständliche und geschätzte urbane Flair Wiens zum Leben zu erwecken.

This dissertation was written at the Department of Economic and Social History of the University of Vienna. Desideratum of research was the period characterised by reurbanisation of Vienna in 1970-1990. As their essential characteristic alternative settings, discotheques and the club scene were analysed. The three main research strands are: + Urban development of Vienna: The growing momentum of the music and art scene respectively the actively managed measures by the Vienna city government (increasing cultural offer in the wake of an overall move towards social modernisation under Bruno Kreisky). + Youth culture with its forerunners in the 1950s/60s (beatniks, Rock'n'Roll) and concrete examination of the 1970s/80s, when the alternative scene emerged. + “Phenomenon discotheque” and the Viennese pop music scene in particular (indicator of reurbanisation). ▼ Staphylococcus pseudintermedius is the most common microorganism isolated from canine pyoderma and opportunistic infections. Prevalence of methicillin-resistant S. pseudintermedius (MRSP) has increased and multi-drug resistance has become common. A total of 734 S. pseudintermedius isolates collected from dogs presented to the Veterinary Medical Teaching Hospital from 2007 to 2012 were studied. Isolates were analyzed for antimicrobial resistance and virulence gene carriage. With the emergence of methicillin resistance, veterinarians have begun to use antimicrobials such as amikacin, to treat life-threatening MRSP infections. The most widespread mechanism of amikacin resistance is drug inactivation by aminoglycoside modifying enzymes (AMEs). The most prevalent gene detected here was aph(3′)- IIIa found in 75% (24/32) of isolates followed by aac(6′)/aph(2′′) and ant(4′)-Ia in 12% (4/32) and 3% (1/32), respectively. There was a significant association between amikacin and methicillin resistance. Since AMEs can be transferred from one bacteria to another, amikacin resistance may represent a new nosocomial and zoonotic threat. Clindamycin is an alternative to β-lactam antimicrobial therapy for canine pyoderma. Inducible and constitutive resistance to clindamycin can occur. Approximately forty erm genes encoding methylases involved in clindamycin resistance have been reported, with ermB most commonly found among S. pseudintermedius. We found eight of 608 isolates tested, positive for inducible clindamycin resistance by D-test and PCR detection of ermB. A vaccine against staphylococcal pyoderma would reduce the reliance on antimicrobial drugs. Staphylococcal cell-wall associated proteins (CWAPs) involved in colonization of the host are attractive potential vaccine targets. Eighteen CWAPs encoded by sps genes have been described in S. pseudintermedius; however, four vary in occurrence. Isolates were analyzed by polymerase chain reaction (PCR) for the presence of mecA, SCCmec type I-VI, and spsF, spsO, spsP, and spsQ. There was a significant association between methicillin resistance and carriage of spsP and spsQ. spsP and spsQ may be viable vaccine targets. ▼ Successful treatment of canine pyoderma has become increasingly difficult due to the development of antimicrobial resistance and recurrence of infection. The development and spread of antimicrobial resistance has major implications because treatment failures have been associated with increased mortality, morbidity and costs related to disease. All canine skin samples submitted to Vetdiagnostix Veterinary Pathology Services for microbiological culture and sensitivity between January 2007 and June 2010, from which Staphylococcus intermedius was isolated, were selected for this investigation. A total of 319 samples from dogs of various ages, sexes and breeds from various locations throughout South Africa were obtained. In addition to the antimicrobial susceptibility data, data relating to dog signalment (age, gender, breed), case history and any other aspects of the history provided e.g. diagnosis, sample type and geographical location of the patient were collected. The number of skin samples yielding Staphylococcus intermedius was high in dogs up to the age of 6 years and, then decreased with only a few cases in dogs aged 11 years or older. The distribution of samples collected in 2010 was unusual in that there were two peaks, one in dogs two years old or less and one in 6 to 9-year-old dogs. With the exception of skin samples taken in 2008, a high percentage of dogs were under the age of one year old. 2008 had a lower percentage of affected dogs less than one year of age compared to the other years. Staphylococcal pyoderma is more common in dogs below the age of five years. Dogs above this age are less likely to contract this condition. The genders tended to be equally distributed throughout the years, with almost equal proportions of affected males and females present. The Bull Terrier types and Shepherd types were grouped separately as they were over-represented and are known to be prone to pyoderma. Large short haired dogs were consistently worse affected throughout the years sampled, followed by the Bull Terrier types. In 2010, small short haired breeds were worse affected compared to the numbers between 2007 and 2009. The monthly distribution, with the exception of 2009, tended to be consistent throughout the years. The unusual temporal distribution in 2009 could be associated with a general distribution in the number of samples compared to the other years sampled. Samples tended to be mainly from practices located in the KwaZulu-Natal province of South Africa. This is most likely due to the fact that the laboratory is located in this province, with a courier network that arranges collection from these practices. Practices in other regions send their samples to the laboratory via private courier companies and the South African Post Office. Antimicrobial resistance of S. intermedius was greatest to ampicillin followed by tetracycline and then potentiated sulphonamides. ▼ The Gram-positive bacterium Staphylococcus intermedius is regarded as the major cause of canine pyoderma, a common skin infection of dogs. However, despite its clinical importance, the population genetic structure of S. intermedius is poorly understood. The current study examined the population genetic structure of S. intermedius using a multilocus DNA sequencing approach. A collection of 99 isolates phenotypically identified as S. intermedius and originating from a broad array of animal hosts in several different countries was investigated. Phylogenetic analysis indicated that the isolates belonged to three distinct species including S. intermedius, staphylococcus pseudintermedius, and Staphylococcus delphini, together referred to as the S. intermedius group (SIG). Importantly, it was discovered that all canine isolates investigated belonged to the S. pseudintermedius phylotype and it was concluded that S. pseudintermedius, not S. intermedius, is the common cause of canine pyoderma. Further, it was revealed that S. delphini is more clinically important than was previously thought. The allelic variation of agrD, which encodes the autoinducing peptide (AIP) of the agr quorum sensing system in staphylococci, was determined for all isolates. Four AIP variants were identified, including three which were present in all three species, suggesting that a common quorum sensing capacity has been conserved despite species differentiation in very different niches. Considerable clonal diversity was revealed within the S. pseudintermedius species, including several methicillin-resistant clones which have evolved by recent acquisition of the mecA gene. Using the sequence diversity identified, a simple diagnostic test was developed based on a PCR-RFLP approach to discriminate S. pseudintermedius from S. intermedius and S. delphini. Having established that S. pseudintermedius is the common canine pyoderma pathogen, this study aimed to investugate key host-pathogen interactions involved in colonisation of its canine host. Bioinformatic analysis of the whole genome sequence of a clinical isolate of S. pseudintermedius (strain ED99) revealed 17 genes encoding predicted LPXTG-containing cell wall-anchored (CWA) surface proteins. A diverse collection of S. pseudintermedius isolates and closely related staphylococcal species was screened for the presence of the genes encodng the novel CWA proteins. The majority of genes were widely distributed among the isolates examined, with nine genes being exclusive to S. pseudintermedius and eight being also present in other members of the SIG. In Gram-positive bacteria, a family of CWA proteins called microbial surgace components recognising adhesive matrix molecules (MSCRAMMs)mediates bacterial adherence to extracellular matrix proteins of the host. ▼ Le terme de médiation est à la mode, mais souffre d’un manque de clarté et de vision uniforme ; le grand public est retient en général une image confuse. Nous commencerons donc par donner une image aussi nette que possible de cette notion. Puis, le lecteur sera immergé dans l’univers de la médecine, par une étude sociologique qui présentera la complexité des rapports entre les individus et les enjeux de pouvoir. En outre, nous partageons l’avis que la médecine n’est pas seulement une somme de connaissances (savoir-faire), mais nécessite également un savoir-être (empathie et communication). Avec le développement de la technologie et l’augmentation des progrès, l’équilibre entre ces deux éléments composant l’art de guérir s’est fragilisé, le savoir-faire marginalisant le savoir-être. Cette problématique est essentiellement présente au sein des établissements de soins, lieu de contraintes, de brassage de population et de soins de masse. La médiation serait-elle un moyen efficace pour rétablir cet équilibre ? Pour y répondre, nous analyserons tant dans la législation suisse que belge, les droits du patient d’une part et la médiation en santé d’autre part, pour finalement nous arrêter sur un instrument dédié au respect des droits du patient : le médiateur local en Belgique. Sur la base de ce modèle, nous suggérerons une nouveauté : l’ombudsman hospitalier. Dans notre pensée, il s’agit d’une personne de confiance et de contact au sein de chaque établissement hospitalier ou groupement d’hôpitaux, chargée d’effectuer une méta analyse des doléances, de gérer les rapports relationnels difficiles, d’assurer une écoute, prodiguer des conseils et diffuser les informations. Cette entité pourrait être conçue comme un moteur de changement ou une amorce d’autocritique tant chez les profanes (patient, visiteur, proche, etc.) que chez les professionnels (administratif, technique, médical, soignant, etc.) face aux difficultés inhérentes aux relations humaines. Telle est notre opinion, moyennant quelques conditions indispensables, partant de l’hypothèse que le dialogue est un élément incontournable de la qualité des soins et de la prise en charge des patients.
