▼ The interaction of intense laser light (I > 10¹⁸ W/cm2) with a thin target foil leads to the Target Normal Sheath Acceleration mechanism (TNSA). TNSA is responsible for the generation of high current, ultra-low emittance proton beams, which may allow for the development of a compact and cost effective proton therapy system for the treatment of cancer. Before this application can be realized, control is needed over the large divergence and the 100% kinetic energy spread that are characteristic of TNSA proton beams.The work presented here demonstrates control over the divergence and energy spread using strong magnetic fields generated by a pulse power solenoid. The solenoidal field results in a parallel proton beam with a kinetic energy spread of 10%. Assuming that next generation lasers will be able to operate at 10 Hz, the 10% spread in the kinetic energy along with the 23% capture efficiency of the solenoid yield enough protons per laser pulse to, for the first time, consider applications in Radiation Oncology.Current lasers can generate proton beams with kinetic energies up to 67.5 MeV, but for therapy applications, the proton kinetic energy must reach 250 MeV. Since the maximum kinetic energy Emax of the proton scales with laser light intensity as Emax proportional to I⁰.5, next generation lasers may very well accelerate 250 MeV protons. As the kinetic energy of the protons is increased, the magnetic field strength of the solenoid will need to increase. The scaling of the magnetic field B with the kinetic energy of the protons follows B proportional to E¹/2. Therefor, the field strength of the solenoid presented in this work will need to be increased by a factor of 2.4 in order to accommodate 250 MeV protons. This scaling factor seems reasonable, even with present technology.This work not only demonstrates control over beam divergence and energy spread, it also allows for us to now perform feasibility studies to further research what a laser- based proton therapy system might look like. A theoretical beam transport system is presented at the end of this dissertation. It shows us that pulse power magnetic optics generating reasonable field strengths can transport a large bandwidth, high kinetic energy proton beam around and into a patient. This gives us insight into the spectrum available per laser pulse at the exit-port of the gantry as well as what types of dose deposition routines and spectral shaping techniques will need to be developed to contour a given dose to a given tumor volume. ▼ Introdução: bronquiolite obliterante (BO) é uma forma rara de doença pulmonar crônica que ocorre após uma injúria grave ao trato respiratório inferior e resulta em estreitamento ou obliteração total das pequenas vias aéreas. Há várias etiologias possíveis, mas em crianças a doença é geralmente pós-infecciosa. O tratamento básico é de suporte, pois não há medidas terapêuticas comprovadamente efetivas. Há evidências, baseadas em estudos animais, de que o uso de corticóides na fase inicial da doença poderia modificar seu curso, revertendo a atividade inflamatória, especialmente a deposição de fibroblastos. A pulsoterapia é utilizada para reduzir os efeitos colaterais da administração sistêmica prolongada de corticóide oral, e é uma alternativa para os pacientes com quadros mais graves. Objetivos: avaliar parâmetros clínicos, laboratoriais, radiológicos e de função pulmonar nos pacientes pediátricos com bronquiolite obliterante submetidos a pulsoterapia com metilprednisolona, antes e após o procedimento, e demonstrar que ocorre melhora clínica, considerando-se o grau de hipoxemia, número de agudizações pulmonares, função pulmonar, número de internações, uso prolongado de corticóides orais e suas complicações. Avaliar também os efeitos colaterais e complicações do tratamento. Métodos: foram avaliadas 40 crianças na faixa etária de até 18 anos, de ambos os sexos, em seguimento na Divisão de Pneumologia Pediátrica do Instituto da Criança do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, com diagnóstico de bronquiolite obliterante por dados clínicos e tomográficos e/ou de biópsia pulmonar, e que foram submetidas à pulsoterapia com metilprednisolona endovenosa pelo período mínimo de seis meses. A análise estatística foi feita usando testes não paramétricos para comparação de medidas repetidas (Friedman, Wilcoxon), e para comparação múltiplas foi realizado o pós-teste de Dunn com nível de significância = 5%. Resultados: houve diminuição no número de agudizações pulmonares comparando o período de seis meses antes da pulsoterapia e 24 meses após o início da pulsoterapia (p=0,0042). Houve diminuição no número de internações hospitalares comparando o período de seis meses antes da pulsoterapia e 18 meses após (p<0,0001). Houve melhora na saturação de oxigênio comparando o período de seis meses antes e no primeiro (p=0,0002) e segundo ano após (p=0,0005). Houve aumento na estatura e peso das crianças comparando o início e o final da xii pulsoterapia: escore Z aumentou de -1,08 para -0,63 (p=0,015) e de -0,91 para -0,59 (p=0,039) respectivamente. Dos 23 pacientes em corticoterapia oral contínua, foi possível suspensão em 20 (87%). Dos 20 pacientes em oxigenoterapia domiciliar, foi possível a suspensão completa em seis pacientes. As piodermites, superficiais ou profundas, representam uma das dermatopatias caninas mais comuns no cotidiano da clínica dermatológica de cães. Dentre as classificadas como superficiais, destacam-se a foliculite bacteriana e a piodermite esfoliativa ou superficial disseminada, que representam a grande maioria dos casos. Em alguns cães, as piodermites superficiais são idiopáticas, apresentam evolução crônica e caráter recidivante. A despeito de criteriosa investigação acerca de sua possível etiologia, por vezes, não se consegue evidenciar a causa para a instalação e recorrência do quadro mórbido. Tais animais apresentam frequentes recidivas, em variáveis períodos de tempo, após o término da terapia antibiótica. Para estes caninos, na bibliografia médico-veterinária há recomendação de emprego das controvertidas imunoterapia ou de antibioticoterapia sob a forma de pulsos. Portanto, objetivou-se na casuística do Serviço de Dermatologia do Hospital Veterinário da USP, determinar as principais espécies bacterianas envolvidas, a susceptibilidade destas frente a diferentes antimicrobianos, bactericidas ou bacteriostáticos, assim como, comparar a eficácia e a segurança de três distintos protocolos terapêuticos no manejo a longo prazo, de piodermites superficiais recidivantes idiopáticas. Utilizaram-se 23 animais, de quaisquer dos sexos, de distintas raças e faixas etárias. A amostragem foi aleatoriamente disposta em três grupos de experimentação (GI, GII, G III), respectivamente submetidos à imunoterapia com bacterina comercial (Estafilin®) e às pulsoterapias de \"finais de semanaöu em \"semanas alternadas\", empregando-se cefalexina \"per os\". O principal patógeno, em 67,6 % da totalidade de isolamentos bacterianos, foi Staphylococcus intermedius. Evidenciou-se (32,4 %), ainda, as espécies S. hyicus, S. schleiferi subespécie coagulans, S. warneri e Micrococcus sp. Segundo os antibiogramas, os fármacos mais efetivos in vitro foram a amoxicilina associada ao clavulanato de potássio, a cefalexina e o ceftiofur. Verificou-se resposta plena em percentis de, respectivamente, 50,0 %, 33,3 % e 28,6 % nos Grupos II, III e I; destarte, ao se agrupar as respostas plena e moderada evidenciaram-se valores relativos de, respectivamente, 83,3 %, 50,0 % e 42,9 %. À luz da estatística (Teste do Qui Quadrado), não se observaram quaisquer diferenças estatisticamente significativas (p > 0,05) entre os percentuais de resposta aos três protocolos empregados. Todos eles mostraram-se seguros, sem acarretar efeitos adversos farmacodérmicos.

▼ Advanced hemodynamic monitoring with determination of cardiac output and other parameters relevant for the assessment of volume status and heart function is well established as an integral part of intensive-care monitoring and therapy. Since the introduction of invasive bedside cardiac output measurement using pulmonary thermodilution in the 1970s, hemodynamic monitoring methods have been continuously improved and now permit minimally or even non-invasive, technically simple, quick and continuous determination of cardiac output, and thus assessment during ongoing therapeutic measures. However, in contrast to thermodilution, using these methods cardiac output is calculated based on complex mathematical models and databases stored in the systems, and thus is estimated rather than actually being measured. The methods are therefore of limited use for hemodynamically unstable critically ill patients because the calculation algorithms cannot at present sufficiently account for changing vascular conditions during therapy with vasoactive substances. Determining cardiac output baed on thermodilution is therefore still to be regarded as the standard in intensive care. Since the 1990s, the less invasive transpulmonary thermodilution measurement method, which requires only a central venous catheter and a special arterial thermistor catheter, has established itself as an alternative to pulmonary artery thermodilution with pulmonary artery catheters. With the PiCCO system, in addition to measuring the cardiac output using the thermodilution curve, parameters are calculated that should allow conclusions to be drawn about intrathoracic blood volume (ITBV) and extravascular lung water (EVLW). Here too, the calculation is carried out on the basis of complex mathematical models. The validity of the calculated values may be limited by disrupted cardiac output measurement as well as by various other influences. Only with knowledge of the underlying mathematical formulas is it possible to adequately assess plausibility and deviations in the calculation of these values. There are special challenges in terms of hemodynamic management for intensive care patients with renal failure, since their volume status must be frequently regulated with renal replacement therapy and physiological compensation mechanisms are also impaired. The assessment of ITBV and EVLW is therefore of particular importance for these patients, especially because the method used to determine the volume reduction in renal replacement therapy for hemodynamically stable patients on dialysis can hardly be used for intensive care patients. The volumetric parameters calculated using the PiCCO system can here provide valuable information despite the above-mentioned limitations. During on-going renal replacement therapy, however, there may be disruptions in the thermodilution measurements, which in turn affect the determination of ITBV and EVLW. ▼ Background: The use of a goal-directed haemodynamic management guided by the oesophageal Doppler monitor (ODM, CardioQ-ODMTM) has been shown to reduce postoperative complications. The goal-directed management aims at increasing circulatory flow and is based on measurements of stroke volume. However, except for the ODM, further haemodynamic monitors are available to measure stroke volume while most of them rely on arterial pressure waveform assessment. In this regard, we aimed to compare the oesophageal Doppler monitor (ODM, CardioQ-ODMTM) with a calibrated pulse-contour analysis (PCA, PiCCO2TM) regarding stroke volume changes after volume administration within a goal-directed haemodynamic algorithm during non-cardiac surgery. Methods: The data were prospectively obtained in patients undergoing multivisceral cytoreductive surgery due to ovarian cancer. During surgery, fluid challenges were performed according to the goal-directed haemodynamic algorithm guided by the ODM. The ODM and PCA were compared with respect to precision and trending. Extended regression analysis was used to investigate clinical characteristics being associated with trending. Results: A total of 1524 paired measurements were analysed according to 762 fluid challenges performed in 41 patients. Precision of ODM and PCA was similar (5.7% vs. 6.0%, P=0.80). Polar Plot analysis showed a poor trending between ODM and PCA with an angular bias of -7.1°, radial limits of agreement of -58.1° to 43.8° and an angular concordance rate of 67.8%. Regression analysis revealed that dose of norepinephrine [scaled 0.1 µg kg-1 min-1] (adjusted Odds ratio (OR) 0.606 [95%-CI: 0.404-0.910]; P=0.016) and changes in mean arterial pressure to a fluid challenge [scaled 10%] (adjusted OR 0.733 [95%-CI: 0.635-0.845]; P<0.001) were related with trending between both monitors, whereas there was no association found with respect to type of intravenous solution. Conclusions: In spite of a similar precision, ODM and PCA were not interchangeable with respect to measure stroke volume changes within a goal- directed haemodynamic algorithm. Increasing norepinephrine levels and greater changes of mean arterial pressure to a fluid challenge were associated with a decrease of interchangeability between ODM and PCA. ▼ Long-term oxygen therapy (LTOT) is the only therapeutic modality proven to alter the late course of chronic obstructive pulmonary disease (COPD). In particular, patients with chronic hypoxaemia (resting PaO₂ level<55mmHg in room air) under steady disease state as well as those hypoxaemic patients (resting PaO₂:56-59mmHg) in the presence of cor pulmonale and/or polycythemia require LTOT administration. It is widely known that LTOT improves survival rates in these patients if oxygen is received for at least 15h/day, as it exerts beneficial effects on several pulmonary haemodynamic variables (pulmonary artery pressure, right and left ventricular function, haematocrit, mixed venous PaO₂). Two different oxygen administration systems have been developed for LTOT: concentrators and liquid oxygen, which have actually eliminated high-pressure compressed gas cylinders, present in the early years of LTOT implementation. Several studies have been conducted in different European countries, concerning patients under LTOT and its influence on compliance rate, exercise capacity and quality of life as primary outcome measures for survival. Greece is one of the countries where the issue of LTOT is poorly investigated. Our study is divided into two phases. The aim of the first phase was to investigate the compliance of patients under LTOT in the Prefecture of Ioannina (North-Western Greece) and to identify factors that might affect it. The study included 249 patients under LTOT during one year period. They were visited at home by the investigating staff (a chest specialist and a primary care nurse). Compliance to therapy was defined by the daily duration of the O₂ concentrator use (hours). Patients underwent pulse oximetry, basic spirometry, and trial inhalation of bronchodilators. The results were as follows: Only 67 patients (26.9%) complied with therapy, while the daily concentrator use lasted for 9.7 ± 6.09 hours. SaO₂, FEV₁ and FVC were negatively correlated to patients’ compliance. No significant differences concerning LTOT compliance were noted between men and women, urban and rural population, and between smokers, ex-smokers and non-smokers. A chest physician recommended LTOT for the majority of patients (86.3%), while COPD was the main reason for LTOT prescription (74.7%). Nevertheless, compliance to LTOT was not significantly higher, when prescribed by a respiratory physician. No specific medical instructions and information was recorded for the majority of the examined patients, under LTOT. In conclusion, LTOT overuse and the concomitant poor compliance to therapy is a strong message indicating further investigation to all links included (medical and nursing staff, concentrator dealers and, of course, patients). The relative results and conclusions would reinforce any attempt to settle a national LTOT policy. An organized home-care program for COPD patients is the cornerstone of a positive patients’ attitude regarding LTOT thus improving its efficacy.
