There is a need to understand the outcomes of trauma in South Africa, as it is a country rife with violent crime. Thus far there has been little research into the aftermath of trauma, yet many trauma centres are coming into existence. A better understanding of the aftermaths of trauma could be integrated into current trauma intervention models so that locally researched models could be utilised by these centres. A grounded qualitative study on the outcomes of violent trauma in the South African context was thus conducted. Existentialism was included as a philosophical underpinning to the formal literature. The formal literature itself deals with the evolution of the cognitive tradition before a detailed explication of the Shattered Assumptions Model is given. This model stresses the role of trauma in disrupting central schemas regarding the manner in which both the self and the world are viewed. The impact of trauma itself in terms of the individual’s thoughts, memories and emotions was also explored. Participants in the study comprised of individuals who had been traumatised and sought counselling at the Rand Afrikaans University Trauma Centre. The participants were interviewed using the semi-structured interview format. Six such interviews took place. Selection of themes to discuss with the respondents was based both on existential theory as well as theory from the technical literature on trauma. The interviews were coded and analysed such that themes could emerge from the data itself. This is consistent with the qualitative tradition of psychological research. It was found that the South African respondents differed markedly with regards to outcomes following traumatisation. Results were discussed in terms of emotional sequelae, cognitive attributions, behavioural modifications as well as pervasive symptoms. Significantly, it was found that most respondents were still emotionally disturbed by their experiences, reporting high levels of fear and anxiety. These results are interpreted within the context of the South African environment where the incidence of violent crime and its resulting trauma is prevalent. The researcher speculates whether the recovery from trauma in South Africa is retarded due to the presence of continued threat. However, the limited external validity of the current study precludes the drawing of any universal conclusions. Further research in the field of the experience and aftermath of trauma in South Africa is recommended, with particular emphasis on the role that continued threat may play.

▼ Trauma can impact all people of all identities, and mental health professionals are experiencing an influx of clients presenting for services with symptoms related to trauma (Webber, Kitzinger, Runte, Smith, & Mascari, 2016). According to the U.S. Department of Veterans Affairs (2016), an estimated 6 in 10 men and 5 in 10 women will experience a traumatic experience in their lifetime. Roughly 8 million adults meet diagnostic criteria for Posttraumatic Stress Disorder (PTSD) in a given year (U.S. Department of Veterans Affairs, 2016), and trauma is considered a public health crisis by the U.S. Department of Health and Human Services (2003). In response to this increasingly prevalent concern among the population, it is imperative that counselors are trained to effectively recognize and treat trauma symptomology. Porges’s Polyvagal Theory (PVT; 2018) offered a conceptualization of trauma responses that could prove useful for counselors. Porges outlined the physiological underpinnings of how one’s autonomic nervous system (ANS) responds to stress. PVT explores three types of stress responses: social engagement, sympathetic arousal (fight/flight responses), and dorsal shutdown (freeze responses). These responses are connected with various symptoms that correlate with trauma, and they offer one lens for conceptualizing how clients present to counseling for trauma. Within the literature, authors are conceptually connecting symptoms of psychopathology with ineffective autonomic regulation and vagal dysregulation (DePierro et al., 2013; Fiskum, 2019), as outlined by Porges (2011), but there is limited empirical literature on applying PVT to counseling. Thus, autonomic regulation is an important - but overlooked - factor at play within counseling and trauma treatment. Because autonomic regulation is tied to which trauma response a client experiences (and exhibits in session), it is important for counselors to have a way to screen for it. In the current study, visual markers indicative of autonomic regulation related to the social engagement, fight/flight, and freeze responses were organized into the Autonomic Response Screening Tool for Counselors (ARSTC). These items did not categorize into the specific trauma responses as outlined within PVT, but they did seem to be representative of two broader yet distinct nervous system processes: sympathetic and parasympathetic processes. These analyses suggested initial evidence for the validity the ARSTC, specifically related to changes in heartrate variability (HRV) between the ARSTC categories of social engagement and freeze, and fight/flight and freeze. Limitations of the study and implications of the results are outlined, and offer a launching point for both counselors and researchers to continue exploring the role of autonomic processes and PVT at play in counseling session.

▼ The Centers for Disease Control and Prevention (CDC) define “intimate partner violence” or IPV as “a serious, preventable public health problem that affects millions of Americans” (CDC, 2017). The term “intimate partner violence” is an umbrella term that describes physical violence, sexual violence, stalking, and psychological aggression as part of an ongoing pattern of power and control perpetrated by a current or former intimate partner – a multifaceted phenomenon that is prevalent among millions of people around the globe irrespective of age, race, ethnicity, religion, socioeconomic status, or sexual orientation (CDC, 2017; Murray & Graves, 2012). The negative, and oftentimes, long-term health consequences of IPV are well documented in the research literature, ranging from physical health consequences to long-term psychological impact (Bergman & Brismar, 1991; Campbell & Soeken, 1999; Coker et al., 2005, 2002, 2000; Lindhorst & Beadnell, 2011; Mertin & Mohr, 2001; Neustifter & Powell, 2015; Sutherland, Bybee, & Sullivan, 1998; Zlotnick et al., 2006). While the destructive ramifications of IPV represent a harsh reality for many survivors, many of these same survivors also endorse positive, growth-promoting experiences within the recovery process from past IPV. The Intimate Partner Violence Recovery Measure (IPVRM) was created to assess the unique recovery experiences of IPV survivors in the long-term. Data analysis results testing the psychometric soundness of the instrument yielded preliminary evidence for a valid, 15-item IPV recovery-specific instrument characterized by two primary subscale factors: Self-Love, Purpose and Transformation and Advocacy and Psychoeducation. Internal consistency of the IPVRM yielded an overall Cronbach’s alpha of 0.851. The Self-Love, Purpose and Transformation yielded a Cronbach’s alpha of 0.845 while the Advocacy and Psychoeducation subscale yielded a Cronbach’s alpha of 0.661. Convergent validity results computed between final IPVRM scores and the Posttraumatic Growth Inventory produced a Pearson’s Product Moment correlation of 0.653, which was significant at the 0.01 level (r = 0.653, p = 0.000). Divergent validity results computed between the final IPVRM, Kessler Psychological Distress Scale (K10; Kessler et al., 2002) and the PTSD-8 (Hansen et al., 2010) produced a Pearson’s r of -0.595 and -0.338, respectively, both of which were significant at the 0.01 level (r = -0.595, p = 0.000; r = -0.338, p = 0.000). From a research perspective, researchers now have an instrument to begin rigorous, quantitative investigations of the long-term IPV recovery process (e.g., cross-sectional designs, longitudinal studies). Future studies are needed to confirm the accuracy of initial findings and stability of the overall IPVRM model and identified subscales. Clinically, the IPVRM can be utilized to help inform treatment intervention strategies and outcome-based assessment in clinical settings from a strengths-perspective. A presente dissertação configura-se como uma interrogação à teoria psicanalítica acerca das ressonâncias do traumatismo na função psíquica da memória. Ambos são conceitos que remetem aos fundamentos da psicanálise, apontando para a constituição do psiquismo, bem como para seus limites. A dissertação procura ampliar o estudo da temática para além da obra de Freud e alcançar as contribuições de Sandor Ferenczi e seus desdobramentos na obra de Nicolas Abraham e Maria Torok. Em Freud, as relações de trauma e memória, principalmente a partir da conceituação de um além do princípio do prazer, apontam para o funcionamento, ou melhor, às falhas de funcionamento nos limites do psíquico - entre corpo e psique, entre percepção e representação - responsáveis pela instauração da memória e a diferenciação psíquica. O traumático foi associado à dinâmica da pulsão de morte e a da angústia automática, que faz continuamente uma demanda de trabalho psíquico, de ligação, anterior à instauração do princípio de prazer. Quando não há possibilidade de ligação e transcrição do acontecimento, seus efeitos apresentam-se de forma negativa como danos narcísicos. Ferenczi considera o papel do objeto como determinante em relação ao destino traumático de um acontecimento. Caso o objeto não possa adaptar-se às necessidades do sujeito e fornecer ou legitimar um sentido ao vivido, interrompe-se o processo de introjeção e inscrição psíquica. Frente ao desamparo psíquico decorrente da ausência de investimento do objeto, o psiquismo se defende por meio da clivagem das impressões traumáticas ou imerge em comoção, da qual não resta memória. Nicolas Abraham e Maria Torok acrescentam que um acontecimento que permaneceu clivado no psiquismo de uma geração - impossibilitado de circulação e figurabilidade - é transmitido enquanto lacuna de memória para a próxima geração. A imagem do trauma como avesso da memória é paradoxal, pois remete tanto às impressões que aguardam uma revelação por meio de uma ligação com uma imagem, no modelo dos sonhos traumáticos, como à pura negatividade relativa à falta de representação, da qual um sentido pode advir mediante somente uma construção que produza um sentimento de convicção. Tal imagem paradoxal pretende oferecer uma reserva psíquica/teórica ao analista enquanto uma figurabilidade possível das ressonâncias do traumático na memória.

▼ The purpose of this study was to investigate intergenerational relationships between trauma, dissociation, and emotion. Short and long term consequences of betrayal trauma on cognitive and emotion coping strategies in a sample of 67 mother-child dyads were explored. Group comparison, correlation, and regression strategies were used to examine relationships between the following variables: maternal and child trauma histories, maternal and child dissociation, maternal alexithymia, and child emotion coping strategies in response to distressful events. Experiences of high betrayal trauma were found to be related to higher levels of dissociation in both children and mothers. Furthermore, mothers who experienced high betrayal trauma in childhood and were subsequently interpersonally revictimized in adulthood were shown to have higher levels of dissociation than a group of mothers who had experienced high betrayal trauma in childhood but were not revictimized in adulthood. This may indicate that dissociation from a history of childhood betrayal trauma involves a persistent unawareness of future threats in the environment. Additional evidence consistent with this hypothesis was found. Maternal revictimization status was related to child interpersonal trauma history, suggesting that a dissociative unawareness for threats may extend to children. More generally, an association was found between maternal interpersonal trauma history and child interpersonal trauma history. Maternal dissociation was also predictive of maternal alexithymia. This relationship was examined because mothers high in alexithymia were hypothesized to display deficits in emotion socialization that could put their children at greater risk for dissociation. Evidence consistent with a relationship between maternal alexithymia and child dissociation was found. Furthermore, a significant association between maternal alexithymia level and child emotion coping strategy was revealed. Children with highly alexithymic mothers displayed higher levels of passive emotion coping strategies on a task assessing their reactions to a distressful parent-child event. This study provides evidence that the experience of parental trauma has intergenerational effects on children. It is an important first step towards longitudinal studies that can provide additional clarification of the nature of the relationships between these variables, as well as parent-child intervention studies that may help to prevent child trauma exposure and reduce symptomatology.

ENGLISH ABSTRACT: No previous studies have investigated the prevalence of traumatic events, Posttraumatic Stress Disorder (PTSD) and PTSD symptoms in HIV test seekers, and very few published studies in South Africa have used a structured clinical interview to measure these constructs. Despite there being a high prevalence of PTSD associated with persons living with HIV, little is known about the occurrence of the disorder amongst people seeking HIV testing. This study undertook a cross-sectional approach amongst individuals who enrolled for an HIV test at several testing centres in the Cape Town region. PTSD was assessed with the Structured Clinical Interview for the DSM (SCID) which was administered by trained researchers. In addition, a PTSD self-report scale was administered to the participants (PTSD Symptom Scale Self-Report version [PSS-SR]). Overall, the prevalence of traumatic events was 62.6%. Of the 493 participants, only 4.4% of the total sample fulfilled current for PTSD. The majority of the participants (60.5%) reported mild to moderate symptom severity. The first aim of this study was to determine prevalence rate of traumatic events and PTSD among people seeking HIV testing. The second aim was to determine PTSD symptoms in the same sample of people seeking an HIV test. The third aim was to determine the optimal cut-off scores on the PTSD Symptom Scale using the SCID as the gold standard, using a Receiver Operator Characteristic curve (ROC) analysis The PSS-SR demonstrated excellent sensitivity and specificity in identifying SCID defined PTSD (area under the receiver operator characteristic curve, 0.86). This finding suggests that the PSS-SR is robust measure for assessing PTSD caseness. The current study’s findings suggest that exposure to traumatic events does not necessarily lead to a diagnosis of PTSD but may make individuals susceptible to PTSD symptoms.

ENGLISH ABSTRACT: For many years, First Peter has had the reputation of being an “exegetical stepchild” in New Testament studies. It is typically associated with the terms “suffering”, “submission” and “silence”, as suggested in the main title of the dissertation. The argument of the study is that there is another prevalent dimension regarding 1 Peter, namely the presence of trauma. This dissertation examines the deeper embeddedness of these three associations with the letter by means of multidimensional exegesis as methodology and trauma theory, as developed by Shelly Rambo, as theoretical (hermeneutical) framework. Rambo’s point of departure is that trauma distorts memory and concept of time, it causes a loss for words and communication about trauma and that trauma distorts the relationship of an individual to their physical body, as well as social networks. The methodology and theoretical framework serve as conversation partners in the dissertation in an attempt to answer the main research question: How can 1 Peter be read from a 21st century perspective, to respect its nature and purpose as an ancient canonised text? The methodology and theoretical framework are appropriated in the first place to examine whether 1 Peter can be read as a text that reflects a situation of trauma. This is done by means of studying literary, socio-historical and rhetorical aspects of the text to see if and how trauma shattered the lives of 1st century CE Jesus followers. The core of this leans towards the rhetorical situation that the author sees or anticipates – an exigence that probably motivated him to write this letter. Secondly, with the help of multidimensional exegesis and trauma theory, this study examines alternative perspectives or coping strategies that the author probably suggests to the audience for them to cope with their trauma and to live life beyond it. Here, the suffering and death of Jesus Christ, God’s story with Israel as found in the Hebrew Scriptures and the emphasis on identity and ethos (especially in terms of temple metaphors, family and household language), stand central. This leads to possible coping strategies for present-day South African believers to cope with their trauma (with the focus on community), namely the sacraments of baptism and the Eucharist. This study does not suggest that 1 Peter gives easy answers to the realities of trauma. Journeying with trauma is complex in nature and the possibility to be traumatised over and over again is a reality. However, the study wishes to communicate that there is more to 1 Peter than has typically been assigned to the letter and that the author seems to focus on encouraging the audience (1st century CE and present-day) to perceive their trauma differently, and even offers the possibility of life after trauma.

The present study explores the experiences of abused women who kill their intimate male partners and are imprisoned as a result. It looks at the multiple traumas associated with the abuse, killing and imprisonment. Abuse of women violates their right of freedom and security, as well as the right to be free from torture and other cruel, inhuman and degrading treatment. The experiences are explored within a prison context in which these women are serving hefty sentences as a means of punishment. This is a means of prosecuting perpetrators by the criminal justice system, thus sending out a message that violence is unacceptable. The prison context is metaphorically and physically associated with phenomenon such as isolation, control, labelling, punishment, reform and rehabilitation, among many others. Social Constructionism as a postmodern epistemology becomes relevant in this study in that the concern is in explicating the process by which people come to describe, explain, or otherwise account for the world (including themselves) in which they live. Therefore, the abused women’s experiences are descriptions to be understood through the analysis of the intersubjective influence of language, family, and culture. The implication being that social construction reflects on that which is said about the world, which is the product of shared conventions of discourse that are guided by and limited by the systems of language that we use. Our understandings of reality are embedded in our patterns of action, and these understandings constrain future constructions. Language as an important tool in social constructionism is embedded in the ideas, concepts and memories arising from social discourse and is found in neither the speaker nor the hearer, but somewhere in between. Furthermore, the context of prison afforded me with the opportunity to experience a sense of communality with the women, which according to a social constructionist stance suggests that reality is co-created between people in their quest for meaning from the interpreted experiences. There is no absolute truth that represents its objectivity, implying that as the researcher, I am not entering the system searching for some single truth that is ultimate. This acknowledges that there are realities and reflexivity of events and situations that look for many alternatives deconstructed and constructed equally between the researcher and participants. In conducting this study, a qualitative method of research was used, which focuses on the description, exploration and elaboration of experiences and perspectives of the people being interviewed. The qualitative method is not concerned with numbers and statistical analysis in the way that the quantitative method is. The participants take active charge in describing and exploring experiences that bring about meaning to them and the study. The aim of this study is to explore and describe the police officials' experience of trauma and the trauma intervention programmes in the SAPS, in order to create guidelines and to make certain recommendations with regard to client-focussed programmes for police officials' attached to the high-risk units in the Eastern Cape. The aims of this study were best realised by the exploratory and descriptive goals of fundamental research as expounded by de Vos et al. (1998). The exploratory and descriptive goal ofthe problem is best achieved by combining the qualitative and quantitative approaches to research, as proposed by Cresswell (1994). For the purposes of this study, the researcher has first made use of a qualitative approach to research. The population with regards to this phase of the study consisted of the police officials' attached to the high risk units in Port Elizabeth. The researcher made use of the purposive sampling method in order to select the participants of the focus group. A semi-structured interview schedule, consisting of four pre-designed questions, was employed in order to obtain maximum information on the subject. The four questions are as follows: experience of trauma, impact on and reaction to trauma, coping with trauma and the experience of trauma intervention programmes. The themes arising from the findings are divided into eleven categories including the classification of their respective sub-categories. The categories are as follows: 1) the concept trauma, 2) traumatic incidents, 3) trauma reinforcers, 4) post traumatic events, 5) post traumatic stress disorder, 6) social support, 7) avoidance/denial, 8) problem-focussed coping, 9) consumer orientation, 1 0) consumer satisfaction and 11) a core marketing strategy. A quantitative approach was used during the second phase of the study, whereby the themes that resulted from the findings of the focus group interview, were distributed amongst the police officials' representing the high risk units in the Eastern Cape. The latter was done in order to explore the extent of the items arising from the focus group in the population. For the purposes of the second phase of the study, the researcher employed the simple random sampling method, whereby 10% of the population was selected. The researcher made use of the group administered method whereby respondents were involved in a group and were thereby provided with an opportunity to complete the questionnaire. The social workers responsible for the high risk units in the various areas in the Eastern Cape were trained as field workers in order to facilitate the completion of the questionnaire by means of the group administered method. Since 1994, there have been various changes in social work in South Africa, changes that reflect developments in international social work. Social workers are described as generalist practitioners, who must be able to address their clients’ problems on different levels of service delivery and drawing on an eclectic range of theories and intervention models. Trauma is a definite reality in South Africa and only one of many problems that social workers face. Eye Movement Integration Therapy (EMI) is a therapy that has its roots in neurolinguistic programming (NLP). Steve and Connirae Andreas researched the possible link between eye movements and therapeutic growth. Subsequently EMI was developed to facilitate the reduction of trauma symptoms. Danie Beaulieu studied under the Andreas’s and developed the technique further. According to contemporary research, the amygdala is responsible for storing trauma memories. These memories are fragmented, as they are stored in the sensory modalities and have no narrative. The precise mechanisms of EMI are still unknown. It appears, however, that EMI, with its 22 eye movements, assists with the integration of fragmented trauma memories. The effectiveness of EMI with the adult population has been studied, but not its usefulness with children. The goal of this study was therefore to explore the usefulness of EMI in overcoming childhood trauma. A sample of 12 children, aged 14-16 years, who had experienced trauma, underwent a single session of EMI with the researcher. A multi-method approach was utilised as both qualitative and quantitative methods were implemented. The quantitative component took the form of the Trauma Symptom Checklist for Children (TSCC) administered before and after the single EMI session. The qualitative component of this study had two parts, namely a semi-structured interview with the parents/caregivers of the children conducted after the EMI session, and a journal that the researcher kept throughout the data collection process. The study found that EMI effectively and significantly reduced the trauma symptoms of the respondents. The successful clinical application of the intervention with children also showed that EMI is a useful technique in the recovery from childhood trauma.

▼ Gene-by-environment studies have shown that an interaction between FKBP5 genotype and childhood trauma can help predict psychiatric disorders. Different methodology has been used in these studies thus leading to discrepant findings between studies. The aim of this study was therefore to perform a meta-analysis on those studies that have examined the interaction between FKBP5 genotype and exposure to childhood trauma in predicting psychiatric disorders. A systematic search of peer-reviewed articles in English was conducted using PubMed and Web of Science. Keywords consisted of “FKBP5” along with different keywords for childhood trauma. For every study included, a Cohen’s d effect size was calculated. All statistical analysis was performed in the software R. A total of 39 studies met the inclusion criteria (n > 30.000). There were fourteen studies on the interaction between FKBP5 genotype and exposure to childhood trauma predicting depression and anxiety, nine on post-traumatic stress disorder, two on aggression, two on substance use disorder, 10 on psychosis and dissociation and two on psychiatric symptoms and other psychiatric difficulties. Results provided evidence in support of an interaction between FKBP5 and exposure to childhood trauma in predicting psychiatric disorders. Utilization of these findings could aid in developing future clinical treatments. ▼ Background: Trauma is a global problem and the leading cause of death in low and middle-income countries (LMICs). The trauma registry is considered to be a vital component of a trauma system; there is good evidence that organised trauma care systems decrease deaths. No trauma registry exists in Zambia. Trauma scoring systems are routinely used in conjunction with trauma registries, as a measure of injury severity and as a predictor of mortality. The revised Kampala Trauma Score (KTS II) has been piloted as an appropriate alternative trauma scoring system in various LMICs; it has not been used in Zambia before this study. Method: A prospective, cross-sectional observational study was conducted from September to February 2012 of patients presenting with injuries to the University Teaching Hospital (UTH). The aim of the study was to develop a template for a trauma registry, define the epidemiology of trauma and define clinically measurable risk factors for mortality (using the KTS II) at UTH. Data was collected on injured patients 24 hours/day including: circumstances of trauma, transport method and time, injury type and location, vital signs on arrival and disposition. A KTS II score was calculated for each patient. Basic demographic data, time of injury to presentation and alcohol use were recorded. Length of stay, operations, use of blood products, radiological services and primary diagnosis were also noted. Data was analysed using descriptive statistics and the KTS II validated by calculating area under the receiver operating characteristic curves.Results: 3425 patients were captured in the study: 72% were male, 28% female. The top three causes of trauma were found to be falls, road traffic accidents and assault. The highest number of trauma victims were children (0-11years) [27%], followed by the 31-50 year old group (26%), followed by the 21-30 year old group (25%). Alcohol abuse was linked to assault and pedestrian traffic accidents. Less than 25% of patients arrived within an hour of injury; the most common form of transport to the hospital was private car (53%), followed by public transport (38%), followed by public ambulance (6%). 54% of road traffic victims were pedestrians, with 85% non-drivers. Nearly half of presenting trauma patients were admitted; the most common injury requiring admission was fracture, followed by lacerations and then burns. The mortality rate for the admitted data set was 4.1%. The highest mortality was sustained by the burns patients (43%); followed by road traffic victims (36.5%), then assault patients (24%). The KTS II was found to be a reliable predictor of mortality (P value <0.0001), but a poor predictor of length of hospital stay.Conclusions: The epidemiology of trauma for injured patients presenting to UTH was defined. A surprise finding was the high number of falls.
