▼ Since the seminal work of Brown and Birley, the potential link between life events and psychosis has been the focus of research and speculation. However, to date, there have been few studies of life events prior to the onset of psychosis; making it impossible to disentangle whether a higher prevalence is a cause or consequence of the disorder. Furthermore, studies have neglected important characteristics, such as severity and type, and rarely considered potential psychological mechanisms. The primary aims of this study were to extend the current literature by investigating the impact of life events and difficulties on the onset of psychosis, and investigate potential synergistic effects and mediating factors. Data on 253 first-presentation cases and 301 population-based controls were drawn from the Childhood Adversity and Psychosis study. Life events and difficulties experienced one year prior to onset (cases) or interview (controls) were assessed with the Life Events and Difficulties Schedule. Potential causal partners included negative schematic beliefs (assessed using the Brief Core Schema Scales) and potential mediators included symptoms of anxiety and depression (assessed using the Hamilton Anxiety and Depression Questionnaires). There was strong evidence that severe and intrusive experiences were particularly associated with psychosis, showing a three- to twelve-fold increase in odds. The impact of severe experiences was found to be cumulative. There was also tentative evidence that low social class and negative self-schemas combined synergistically with these experiences to increase the odds of psychosis. However, there was no evidence of mediation via affective symptoms. The one year period before the initial onset of psychosis is likely to be a time of serious psychosocial stress, potentially characterised by threatening and intrusive experiences. Research must continue to examine potentially modifiable mechanisms that may link such stressors and psychosis in order to improve our understanding and treatment of these disorders.

A relação maternal perinatal tem fundamental importância no desenvolvimento de circuitos neurais saudáveis que permanecerão como herança mental ao longo da vida. Logo, eventos adversos nesse período tem potencial para desencadear psicopatologias na idade adulta, aumentando a vulnerabilidade a transtornos psiquiátricos e abuso de substâncias. Neste trabalho foi realizada a Separação Maternal (SM) em ratos Wistar machos, no intuito de mimetizar um evento estressor sustentado na infância de humanos. Em seguida, os animais foram submetidos ao protocolo de Enriquecimento Ambiental, uma estratégia não farmacológica empregada num período de plasticidade cerebral, como estratégia potencial para reverter os efeitos prejudiciais da SM. Na idade adulta, procederam-se os testes comportamentais, para aferição de depressão, ansiedade e abuso de álcool, e bioquímicos, como a dosagem de corticosterona plasmática, indicativo da reatividade do eixo hipotálamo-hipófise-adrenal (HHA) ao estresse agudo, e dosagem de dopamina e seus metabólitos em estruturas envolvidas em processo de gratificação cerebral via mesocorticolímbica (núcleo accumbens e córtex pré-frontal). A análise estatística foi realizada por meio do teste t de Student, análise de variância de uma via, duas vias, três vias ou medidas repetidas. Observamos que um protocolo de SM mais extenso tem maior impacto no desenvolvimento de comportamento depressivo na idade adulta. Não observamos hiperresponsividade do eixo HHA em animais separados em resposta a estresse agudo. O EA aumentou a preferência pelo consumo de sacarose e SM e EA concomitantemente aumentaram os comportamentos ativos no Teste do Nado Forçado, sugerindo potencial antidepressivo do EA. O EA parece ter potencial ansiolítico, ao reduzir a ansiedade aprendida aversiva no Teste de Odor de Predador, sem efeito significativo da SM nesses comportamentos. A SM aumenta a preferência pelo consumo de etanol e o EA foi capaz de prevenir esse efeito. No córtex pré-frontal a SM aumentou a quantidade de dopamina e o EA aumentou o turnover dopaminérgico, sugerindo recuperação até certo ponto da atividade dopaminérgica cortical. No núcleo accumbens, SM e EA concomitantemente reduziram os níveis de DOPAC, sem alteração no turnover dopaminérgico. SM e EA parecem não interferir nas memórias de curta e longa duração. Dessa forma, observamos por meio de alterações comportamentais e bioquímicas que eventos adversos perinatais aumentam a vulnerabilidade ao desenvolvimento de abuso de álcool e outros transtornos psiquiátricos na idade adulta; e o EA, empregado na fase de plasticidade neural, tem potencial para exercer efeito compensatório sobre os déficits gerados.

A necessidade de correlacionar fatores psicológicos menores em pacientes portadores de DTM (Disfunção temporomandibular) foi realizada como meio de direcionar um diagnóstico fidedigno para um prognóstico seguro durante a realização do tratamento de pacientes portadores dessa disfunção. O objetivo deste estudo foi verificar a correlação entre distúrbios psiquiátricos menores e disfunção temporomandibular severa em pacientes que procuraram voluntariamente o Programa de atendimento a pacientes com Disfunção do Aparelho Estomatognático do curso de Odontologia da UFRN e clínicas de odontologia da Universidade Potiguar/UnP. Foram selecionados 137 pacientes de ambos os sexos, sendo 57 diagnosticados com DTM Severa e 80 sem DTM (grupo controle). Os pacientes foram submetidos ao questionário anamnésico (índice de Fonseca) com o intuito de quantificar o grau de DTM. Após a aplicação do questionário anamnésico, os pacientes responderam ao Questionário de Saúde Geral (QSG), o qual avalia fatores como estresse psíquico, desejo de morte, desconfiança no próprio desempenho, distúrbio do sono, distúrbios psicossomáticos e saúde geral. Os resultados foram submetidos a testes estatísticos (teste t-Student não pareado) para observar se há correlação entre os fatores analisados, a análise demonstrou diferenças significantes entre os fatores psicossomáticos, distúrbios do sono, estresse, desconfiança no desempenho e saúde geral em pacientes portadores de DTM (p=0,001). Conclui-se que a relação entre DTM Severa e as variáveis psicológicas (estresse psíquico, desejo de morte, distúrbios do sono, distúrbios psicossomáticos, saúde geral) é de fato estreita e estatisticamente significativa. Devendo o cirurgião-dentista fazer uma análise específica e individualizada em cada caso.

The need to correlate minor psychological factors in patients with temporomandibular disorder was performed as a means to target a reliable diagnosis for a prognosis insurance during the treatment of patients with the condition mentioned. The purpose of this study was to assess the correlation between minor psychiatric disorders and severe temporomandibular dysfunction in patients who voluntarily sought the assistance program for patients with dysfunction of the apparatus Stomatognathic the course of Dentistry of UFRN and clinical dentistry at the University of Potiguar / UnP. ▼ Schizophrenia (SCZ) is a severe mental disorder affecting about 1 percent of world population. This chronic neural developmental disorder persists throughout patients’ life and causes enormous emotional distress and economic burden for family members. Schizophrenia is a highly heritable disorder. Despite decades of research, the etiology and pathophysiology of schizophrenia is still unclear, and the treatment of this disorder is largely depending on the anti-psychosis drugs partially relieving the symptoms but with many side effects. It is important to have a better understanding of the molecular mechanisms of schizophrenia in order to find new treatment for the disease. However, so far there are very limited animal models that can be used to study the mechanisms of schizophrenia and related disorders. GABAergic neural transmission deficits have long been proposed to be an important pathophysiology of schizophrenia. Recent genetic and genomic studies have identified mutations in GABA-related genes in a substantial amount of schizophrenia patients. We have recently reported several mutations in nlgn2, a gene encoding cell adhesion molecule neuroligin 2 (NL-2), in a group of SCZD patients. NL-2 is critical for GABAergic synapse formation and function. Our previous in vitro experiments showed that one of the NL-2 mutations, R215H, is a loss-of-function mutation that failed to support GABAergic synapse formation and function. Here, we introduced the single point mutation R215H into mouse genome and generated the NL-2 R215H knock-in (KI) mouse line. We found that NL-2 R215H KI mice display anxiety like behaviors, impaired pre-pulse inhibition (PPI), working memory deficit, and hypersensitive stress response, partially recapitulating schizophrenia-like symptoms. Functionally, we discovered that NL-2 R215H KI mice have reduced GABAergic synapse development, decreased GABAergic neural transmission, and impaired hippocampal activation toward acute stress. Transcriptomic analyses through RNA-seq revealed a number of genes in the NL-2 R215H KI mice that are susceptible genes in patients with psychiatric disorders including schizophrenia patients. Our studies suggest that these newly generated NL-2 R215H KI mice may be a unique mouse model for the study of SCZD and related psychiatric disorders, in particular those disorders caused by GABAergic deficits. Future studies on this mouse model may provide new insights into the molecular and cellular mechanisms of psychiatric disorders and lead to potential new drug treatment. ▼ Early and accurate prediction of future cognitive impairment can promote increased use of interventions, which in turn can lead to delayed decline and therefore higher quality and potentially longer length of life. There are a number of known risk factors/biomarkers that predict future cognition; however, prior literature is mixed and incomplete. Further, previous studies suggest that combining multiple risk factors may lead to increased utility above and beyond that of any single predictor, and a limited number of risk score algorithms have been derived accordingly, though more research is needed. The present study examined the ability of regional brain volumes, the Montreal Cognitive Assessment (MoCA), and self-reported cognitive complaint to predict future cognitive status (cognitively impaired or unimpaired), and a risk score algorithm was derived based on these findings. Exploratory analyses were also conducted to examine the ability of the aforementioned factors, excluding the MoCA, to predict future performance in various neuropsychological domains. Total brain volume, left hippocampal volume index, MoCA total score, and cognitive complaint significantly (all p's < .05) predicted diagnostic group (cognitively impaired or unimpaired), with a correct classification rate of 73.8%, controlling for sex (Nagelkerke R2 = .33, p < .001; Hosmer-Lemeshow p = .21). A risk score algorithm was created using these variables, which yielded an area under the curve (AUC) of .81 (p < .001). Regarding the exploratory analyses, whole brain volume was retained in models for all cognitive domain composites except verbal memory; and cognitive complaint was retained in models predicting global, language, and processing speed and attention composites. While a number of regional brain volume variables were retained in exploratory analyses, the majority of results were opposite the direction expected. The present study yielded a theoretically and statistically sound approach in producing a simple risk score algorithm to predict future cognitive decline that requires minimal resources and can be implemented across clinical settings, pending cross validation. Its use could lead to earlier intervention and ultimately improved preparation for, as well as reduced risk, delayed onset, and slower rate of, cognitive decline, thereby resulting in higher quality and potentially even length of life. ▼ Background: Globally, studies have established that 40-50% of psychiatric patients with SMDs are readmitted within one year of discharge from the acute hospital admission. Lowand middle-income countries (LMICs) such as Malawi have also reported high rates of psychiatric readmissions. This poses challenges when providing psychiatric care to patients. Most of Malawi`s health institutions, including Saint John of God Psychiatric Hospital (SJOG), rely primarily on donor funding. In order to maximise the available donor funding, there is a need to reduce readmissions resulting from modifiable or controlled factors. There are no studies in Malawi which have investigated these risk factors. The study aimed to establish the frequency of readmissions and the associated factors among patients at SJOG Psychiatric Hospital in Mzuzu, Malawi. The specific areas examined were sociodemographic and clinical-related factors associated with readmission. Methods: This was a retrospective cohort case record review study. Two hundred and seventy five clinical files of patients admitted for the first time at SJOG Psychiatric Hospital Mzuzu, Malawi between 1 January, 2014 and 31 December, 2015 were extracted. Data on socio-demographics and clinical information were collected using an extraction sheet at 3, 6 and 12 months post-discharge from the acute (first) hospital admission. Logistic regression models were developed to investigate the associations between socio-demographics, clinicalrelated factors and readmissions. Ethical approval for this study was granted by the Faculty of Health Sciences Human Research Ethics Committee at the University of Cape Town. Approval to conduct this research in Malawi was obtained from the National Health Sciences Research Ethics Committee. Results: Readmission rates of 1.5%, 4.4%, and 11.3% were found within the 3, 6 and 12 months of discharge from the acute hospital admission respectively. None of the independent variables predicted readmission within the 3 month of discharge from the acute hospital admission. In the unadjusted logistic regression model, having children (OR=0.26, 95% C.I 0.07-0.96) protected against readmissions within the 6 month of follow-up period. In the unadjusted logistic regression model, having children (OR= 0.40, 95% C.I 0.18-0.88), staying outside the hospital catchment area (OR=0.44, 95% C.I 0.20-0.96), and having insight (OR=0.22, 95% C.I 0.10-0.49) into their illness were protective factors to readmission, while taking SGAs (OR=4.67, 95% C.I 1.33-16.39) predicted readmission within the 12 month follow-up period. After adjusting for age and gender in the multivariable analysis, staying outside catchment area (OR=0.33, 95% C.I 0.14-0.79) and having insight (OR=0.19, 95% C.I 0.08-0.46) to their illness were protective factors, while taking SGAs (OR=5.29, 95% C.I 1.43-19.51) remained a predictor of readmission within 12 months of discharge from the acute admission. ▼ Over the last decade, multiple autoantibodies targeting brain proteins and receptors have been identified in adults and children. Autoantibody exposure in the brain can lead to transient or permanent behavioural or cognitive abnormalities. Current treatment options for autoimmune brain-reactive autoantibody-associated diseases include global immunosuppressive therapies, but these can have severe side effects, and are not always efficacious. Understanding the nature and specificity of brain autoantibodies may reveal potential therapeutic strategies alleviating or preventing the neurological pathologies and behavioural abnormalities associated with antibody-mediated brain disorders. Recently, anti-dopamine-2 receptor (D2R) antibodies have been identified in a subgroup of children with autoimmune movement and psychiatric disorders. D2R is an important brain receptor involved a variety of functions including voluntary movement, learning, memory, attention, and hormonal regulation. Currently, knowledge of the function of D2R tertiary structures, including the extracellular N-terminus, is limited, and its role in autoantibody binding is unknown. Here we report a major biological role for D2R extracellular N-terminus as a regulator of receptor surface availability, and as a major epitope targeted in brain autoimmunity. Human embryonic kidney cells were transfected with D2R mutants modified in their extracellular domains, and the level of cell surface expression and epitope specificity of 35 anti-D2R antibody-positive patient sera were analysed using a quantitative flow cytometry assay. We found that N-glycosylation at amino acids N5 and/or N17 was critical for high surface expression via interaction with the last 15 residues of extracellular D2R N-terminus. No anti-D2R antibody-positive patient sera bound to the three extracellular loops, but all patient sera (35/35) targeted the extracellular N-terminus. Overall, patient antibody binding was dependent on two main regions encompassing amino acids 20 to 29, and 23 to 37. Residues 20 to 29 contributed to the majority of binding (77 %, 27/35), among which sera from 26 % (7/27) of patients bound to amino acids R20, P21, and F22, 37 % (10/27) patient sera binding was dependent on residues at positions 26 and 29, and 30% (8/27) sera required R20, P21, F22, N23, D26, and A29. Seven patient sera bound to the region 23 to 37 independently of D26 and A29, but most sera exhibited N-glycosylation-independent epitope recognition at N23. Interestingly, no evident segregation of binding pattern according to patient clinical phenotypes was observed. Finally, we describe the optimisation of a method for single-cell isolation and RT-PCR for the generation of monoclonal antibodies. D2R N-terminus is a central epitope in autoimmune movement and psychiatric disorders, and this knowledge could help the design of novel specific immune therapies tailored to improve patient outcome.

Estudo transversal de base populacional foi conduzido tendo como objetivos avaliar a prevalência dos transtornos mentais comuns (TMC) e verificar sua associação com determinadas enfermidades crônicas e com o número de doenças crônicas relatadas pelo indivíduo. Para a avaliação de TMC, o Self-Report Questionnaire (SRQ-20) foi aplicado em 1276 adultos com 40 anos ou mais, identificados por meio de amostragem aleatória em múltiplos estágios. O ponto de corte utilizado foi de 6 ou mais pontos para homens e 8 ou mais pontos para as mulheres. Variáveis sócio-demográficas, comportamentais e relacionadas à saúde foram obtidas por meio de um questionário estruturado. Os TMC apresentaram uma prevalência de 30,2%, estando associados à baixa escolaridade e classe social, e à faixa etária de 46 a 65 anos. Todas as doenças crônicas pesquisadas mostraram-se associadas aos TMC, mesmo após ajuste para as demais variáveis. Entretanto, o número de enfermidades apresentadas pelo indivíduo teve maior importância do que cada uma delas individualmente, com uma razão de prevalência (RP) de 4,67 com intervalo de confiança de 95% (IC95%) de 3,19 6,83 para cinco ou mais doenças relatadas. O presente estudo realça a importância de se atentar para os transtornos mentais em indivíduos com enfermidades crônicas, principalmente naqueles que se apresentam com um grande número de doenças.

A cross-sectional population-based study was conducted to identify the prevalence of common mental disorders (CMD) and verify its association with chronic illnesses and the declared number of chronic conditions. The Self-Report Questionnaire (SRQ-20) was applied in a multi-stage random sample of 1276 adults aged 40 and older. Socio-demographic, behavioral and health-related variables were also obtained with a structured questionnaire. The CMD prevalence was 30.2%. Lower educational level and social class and the group 46- 55 years old were associated with psychiatric morbidity. Each chronic illness was associated with CMD. However, a stronger association was found between CMD and the declared number of chronic conditions, with a prevalence ratio of 4.67 (IC95%: 3.19 6.83) for five or more declared conditions. The present work emphasizes the importance of CMD in chronically ill patients, particularly when a greater number of chronic conditions is declared.

Bakgrund: Enligt Hälso- och sjukvårdslagen ställs kravet att vården ska bygga på respekt för patientens integritet och självbestämmande. Trots detta visar studier att kravet inte uppnås, då patienter inte känner sig delaktiga i vården i den utsträckning de önskar. Studier visar på att det inom psykiatrin kan ta tid att införliva patientdelaktighet, då ett gammalt paternalistiskt synsätt på patienter lever kvar på sina håll. Inom ätstörningsvården beskrivs patientdelaktighet som en ledstjärna i behandlingen, men många människor med ätstörningar är svåra att engagera i behandling då de kan vara ambivalenta eller förnekar och inte ser allvaret i sjukdomen. I vårdsituationer uppstår ibland en konflikt mellan patientens rätt till delaktighet i sin egen vård och vad vårdpersonal tänker är det rätta och bästa för personen. Sjuksköterskan är den som står patienten närmast i omvårdnaden och har ett professionellt ansvar att göra patienten delaktig i sin behandling. Syfte: Syftet med föreliggande studie är att undersöka hur sjuksköterskor beskriver patientdelaktighet vid ätstörningsbehandling i psykiatrisk heldygnsvård. Metod: Studien är en kvalitativ studie med induktiv ansats där semistrukturerade intervjuer genomfördes med sex sjuksköterskor verksamma inom ätstörningsvården, från två olika heldygnsavdelningar i Stockholm. Intervjuerna spelades in och transkriberades för att sedan analyseras enligt kvalitativ innehållsanalys. Resultat: Studien resulterade; i sex huvudkategorier; Påverkan, Bemötande, Relation, Organisation och vårdmiljö, Sjuksköterskans situation och Patientens situation med vardera tre underkategorier. Diskussion: Resultatet diskuterade utifrån Kari Martinsens omvårdnadsteori samt annan relevant forskning.

Background: The Swedish Health Care Act states that all care in Sweden should be based in the respect for patient integrity and self-determination. Despite this, studies show that this requirement is not achieved, as patients do not feel involved in the care to the extent that they would wish. Studies have shown that in psychiatric care, it may take time to incorporate patient- participation, as healthcare professionals' view of patients in many places still tends to be characterized by paternalism. Patient participation is described to be a guiding principle in the treatment within eating disorder care, but as patients can often be either ambivalent to their illness, deny it or not take it seriously, their involvement in the treatment can be difficult to achieve. In situations of care giving, a conflict sometimes occurs between the patient’s right to participate in his or her own care, and the healthcare professional´s ideas of what is right and best for the person.
