▼ BACKGROUND: Delirium is an “acute change in cognition” prevalent in the elderly population (9). It is a clinical diagnosis which results from complex relationships between patient vulnerability and precipitating medical factors (2,9). Delirium has a complex relationship with dementia and often these two cognitive impairments occur together. While delirium occurrence is exceptionally high in dementia patients, it’s recognition by physicians is unfortunately low (10). Delirium prevalence in dementia patients can be as high as 80% and failure to recognize occurs in as many as 75% of cases (10). Recognition of delirium is important due to its association with poor outcomes and potential for prevention by avoiding precipitating causes. Common poor outcomes associated with delirium include, prolonged cognitive and physical impairment, longer hospital stays, institutionalization and death. With the increasing aging population and the rising cost of healthcare in the United States more attention has been focused on better detection of costly conditions including delirium. Efforts have been made to develop tools for universal screening to aid in the recognition of delirium; however, these are not widely used outside of research. One of the challenges in recognizing delirium is that providers lack knowledge of the patient’s baseline cognitive status, making it difficult to determine whether an acute change has occurred. Caregivers are an untapped resource, holding this key piece of information, in the early recognition of delirium. SPECIFIC AIMS: The goal of this study is to build an argument for the implementation of a training program which would teach caregivers to complete the Family Confusion Assessment method (FAM-CAM) interviews on their loved ones at home in an effort to aid in the early recognition of delirium. This will be accomplished through the followings specific aims. Aim 1: Complete a comprehensive literature review to assess the need for family based screening for delirium. An attempt will be made to identify a gap in the literature for efficient and effective delirium screening and utilization of family caregivers. Aim 2: Data collected in a completed prospective observational study “Family Member Identification of Delirium in the Emergency Department” will be analyzed to assess family caregivers’ role in recognition of delirium, and family caregivers’ receptiveness and interest in a training program. METHODS: The information in this thesis used to build the argument for FAM-CAM training was found through a comprehensive review of published literature on delirium. Resources were found mainly through Pub Med and the Hospital Elder Life Program’s Delirium Bibliography. The study; “Family Member Identification of Delirium in the Emergency Department” enrolled 63 patients for CAM only and 108 Dyads for CAM and FAM-CAM with a total N of 171. ▼ An O-ring seal is of paramount importance in hydraulic and pneumatic machinery used in oil and gas drilling processes. O-ring seals are generally employed to contain the environments of working fluids existing on either side of the seal from interacting with each other, to maintain a uniform pressure chamber and to eliminate the environmental contaminants that interact at the rubbing interface. Failure of dynamic O-rings in hydraulic and pneumatic machinery often results in considerable productivity loss to the chemical and petrochemical industries. Therefore, it is highly important to determine the life of an O-ring for these applications under various operating conditions and to predict and prevent the possible O-ring seal failure. The purpose of this research was to design and build a new O-ring seal test rig (OSTR) capable of testing various O-rings. The O-rings were evaluated by considering effects of peripheral sliding velocity and operating differential pressure. This thesis contains both theoretical and experimental phases associated with the performance of the Buna-N O-ring seal with variations in peripheral sliding velocity and operating differential pressure. Experimental measurements of temperature, flow rates, differential pressures, shaft rotating speed, power loss, friction torque for corresponding tests were performed. Variations of frictional torque, O-ring power loss and interfacial temperatures with changes in sliding velocity and operating pressure differential are also presented. In order to analyze the temperature at the contact interface, a two-dimensional axisymmetric computational fluid dynamics (CFD) model is preprocessed in GAMBIT and thermal heat transfer analysis is carried out using a commercial CFD package, FLUENT. Results reveal that the O-ring friction power nearly linearly increases with increase in sliding velocity. Further, it is found that the friction power significantly increases with relatively higher pressure differentials. Friction torque is found to drop with increase in shaft speed and rises at relatively higher pressure differentials. Moreover, the interfacial temperatures are found to increase with increase in sliding speed and operating pressure differentials.

▼ Pulmonary rehabilitation (PR) has a positive effect on COPD patients. The response to PR demonstrates positive impacts on daily life activities and exercise tolerance. However, the referral criteria to PR program for COPD patients does not mention anything about the time factor or the stage of the disease. The recommended stage of starting the program should be included in the acceptance guidelines. More studies are needed to examine which COPD stage will benefit more than the others. PURPOSE: The purpose of the study is to determine the most optimal time to start pulmonary rehabilitation for COPD patients. METHOD: A retrospective study was utilized, and the data were collected from an urban hospital for the last five years (2013 to 2017). Patients were divided into four groups using the GOLD classification guideline for COPD patients. Every group represents different FEV₁ range and has pre- and post-PR program variables. The measured variables were 6MWT and Chronic Respiratory Questionnaire(CRQ). The evaluation focused on the comparison between the groups, not within the groups. DATA ANALYSIS: The analysis was conducted by SPSS version 24.0. Descriptive statistics, dependent sample t-test, ANOVA, and Welch test were utilized. T-test was used to find the difference between pre- and post-variables and ANOVA test to compare the difference between the four groups. RESULTS: comparison between groups ANOVA results were Six-minute walk difference F (3.65) =1.3, p=.281. Dyspnea difference F (3.65) =.155, p=.926. Fatigue difference F (3,65) =.640, p=.592. Emotional difference F (3,65) =.221, p=.881. Mastery difference F (3,65) =.363, p=.780. CONCLUSION: There was a significant difference between pre- and post-pulmonary rehabilitation results and all the groups responded positively to the program. There were no significantly different responses to the program between the four groups. As a result, there is no specific preferred stage to start the program. ▼ The objective of this study was to characterize the impact of grilling temperature on ribeye, top loin and top sirloin steaks. Boneless ribeye rolls, top loin and top sirloin butt subprimals (n = 16 each, 48 total) were purchased from a local meat supplier. After aging 21 d post-processing, 2.54-cm-thick steaks were hand cut and randomly assigned a grilling temperature treatment (177?C, 205?C, or 232?C), vacuum-packaged, and frozen at -10?C until testing. Steaks were grilled to an internal temperature of 71?C on a commercial flat top grill set at 177? C, 205? C, or 232? C. Consumers (n = 80) were served nine samples and prompted to rate their liking of overall, tenderness, juiciness, appearance, and flavor on a 9-point hedonic scale. Steaks cooked for Warner-Bratzler shear force were held over night at 4? C before removing six cores 1.3 cm in diameter from each steak. Steaks were also cooked and served to a sensory panel trained on flavor and texture attributes. Samples from the steaks used for trained sensory panel analysis were quick-frozen in liquid nitrogen and stored at -80? C for GC/MS ? olfactory analysis. Results were analyzed as a 3x3 factorial completely randomized design. No differences (P > 0.05) in consumer overall, tenderness, juiciness, appearance, and flavor liking were detected between steak type or grill temperature. The center color of ribeye steaks was lighter (P < 0.05) than top loin and top sirloin steaks. The ribeye steaks also had a greater (P < 0.05) hue angle than top sirloin steaks. Strip loin steaks had 0.27 kg less shear force (P < 0.05) than ribeye and top sirloin steaks. Ribeye and strip loin steaks received greater (P < 0.05) muscle fiber tenderness and less (P < 0.05) connective tissue scores. Grill surface temperature had no effect (P > 0.05) on trained panel tenderness scores. Of the volatiles present during an aroma event (n = 67), pyrazine compounds were most influenced by grill surface temperature. The tenderness and juiciness of steaks grilled at differing temperatures were not perceived to be different by consumers; however, grilling temperature impacted the flavor of the final product by generating more pyrazine compounds. ▼ Continual intake of high-fat foods, coupled with limited physical activity, can lead to metabolic inflexibility. Eventually, this may lead to significant health issues such as obesity, insulin resistance, cardiovascular disease, and other chronic diseases. Metabolic flexibility of human skeletal muscles is influenced by changes to mitochondrial, nuclear, and cytosolic proteins, in part as a result of posttranslational modifications (PTMs). O-linked B-D-N-acetylglucosamine, known as O-GlcNAc, has recently been identified as an important posttranslational modification that responds to nutrient sensing and cellular stress. Unlike other PTMs, O-GlcNAc has only two cycling enzymes. Because of its novelty, little research has been performed on the role of O-GlcNAc in human skeletal muscle and metabolic flexibility. The purpose of the current study was to establish the effects of a 5-day high-fat diet on skeletal muscle O-GlcNAcylation. In the proposed study, 13 non-obese, sedentary, college-aged males consumed a controlled diet for two weeks followed by a high-fat diet composed of 55% fat, 30% carbohydrate, and 15% protein. Muscle biopsies were taken from the vastus lateralis both fasted and four hours after a high-fat meal, following both the control diet and the high-fat diet. Western blot analysis was used to assess global O-GlcNAc and protein concentrations of O-GlcNAc transferase (OGT) and O-GlcNAcase (OGA) in whole-homogenates and isolated mitochondria from skeletal muscle. Results were analyzed using independent, two-tailed t-tests and 2-way ANOVA analysis with repeated measures and Bonferroni corrections; a p-value was set to α less than or equal to 0.05. It was found that O-GlcNAc and OGT levels remained stable, although fasting levels of OGA significantly decreased after the 5-day high-fat diet. It is possible that healthy individuals are capable of maintaining normal levels of O-GlcNAc and its cycling enzymes, but there is still more to learn about O-GlcNAc and its role in metabolic flexibility. ▼ Capillary electrophoresis laser-induced fluorescence (CE-LIF) is widely used to detect both the presence and concentration of fluorescently labeled biomolecules. In CE-LIF, a plug of sample fluid is electrophoretically driven down a microchannel using a high voltage applied between the opposite ends of the microchannel. Molecules of different sizes and charge states travel at different velocities down the channel. Laser light with a wavelength in the excitation band of the fluorophores is focused near the end of the channel. As each species of molecule passes through the laser spot, the fluorophores emit a fluorescence signal which is measured with an optical detector. Commercial CE-LIF systems are available as a complete, expensive package. Custom CE-LIF systems are a collection of commercially available components that meet the specific needs of the end user. Using the custom system in Dr. Woolley's lab as the standard, we hypothesized that 3D printed parts in conjunction with low-cost components could be used to significantly reduce costs and simplify the system, which in turn would make such systems more widely available with a lower barrier to entry. Testing this hypothesis began with five semesters of small teams of senior undergraduate students trying to design and assemble a low-cost CE-LIF system as part of their mandatory one-semester senior project. I was one of the seniors who worked on the system. Although none of the senior project teams were successful, a partially functioning system was ultimately produced. I reference this system as the starting point system throughout this thesis, which is focused on identifying and solving the system's obstacles in order to reach a working state. I re-designed and re-built each sub-system of the starting point system as needed if within the available budget to create a system that was functional. Budgetary constraints were included in evaluating potential improvements. The end goal was to compare the improved system's performance with that of an expensive conventional system (hereinafter referred to as the standard system) available in Dr. Adam Woolley's laboratory on the Brigham Young University campus. The ultimate conclusion of my masters' thesis work is that a low-cost CE-LIF system based on 3D printed and low-cost components results in a system that does not offer repeatable performance. In the course of my work, many lessons were learned as to what would reduce overall system costs while maintaining a user-friendly experience. My analysis is given on a subsystem basis to explain what limited the ability of the system to run consistently or what caused it to fail altogether. Details and methodology of my contributions including circuits designed, code written, components used, and 3D models printed in order to test the hypothesis are documented.
